Vacuum pump busted as shown in photos. Once that busted it sent metal shrapnel into the engine which broke a piece of the cam shaft into bits. There were metal fragments that had to be meticulously cleaned out of my engine block. The loss of vacuum seal caused me to lose brakes while driving in stop and go traffic on the highway.
COMPONENT: The piston in cylinder #1 cracked and created conditions for the fuel injector to bore a hole through it. SAFETY: This led to sudden loss of engine power and violent shaking of the vehicle at highway speeds. -What component or system failed or malfunctioned, and is it available for inspection upon request? 2016 Chevrolet Colorado VIN: [XXX] Engine: 2.5L I4 (camshaft-driven brake system assist vacuum pump failure). GM Technical Service Bulletin 21-NA-268 (Nov 2021) confirms that if vacuum pump drive lugs are broken, replacement of both the vacuum pump and the exhaust camshaft is required which was the case. Parts kept by repair facility that performed service. Autonation Chevrolet South Clearwater FL 33764. -How was your safety or the safety of others put at risk? May, 2025 the vehicle experienced a sudden loss of braking (hard/stiff brake pedal). This is a safety-critical failure. -Has the problem been reproduced or confirmed by a dealer or independent service center?Yes by shop stated above. Diagnosis showed a seized engine brake system vacuum pump (driven by the exhaust camshaft) that had sheared drive lugs and produced metal debris which damaged the exhaust camshaft and associated components. The repair required replacement of the vacuum pump and the exhaust camshaft; my total repair cost was over $4,000. The camshaft was not readily available and I required a rental car for over 30 days while waiting for parts. -Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? System and parts inspected by shop above. GM Special Coverage N182202780 (Apr 2019) addresses vacuum-pump related decreased brake assist for certain 2014–2017 GM trucks/SUVs, but Colorado is not listed in that bulletin’s model list. -Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? Check Engine came on at time of brake loss (Brake Vacuum Pump Failure). Immediate system failure. I request NHTSA send me a copy of any investigation number or record created for this complaint, and I request guidance on whether I may be eligible for reimbursement under any GM program. FPCM code P069E. Engine stalls when slowing down. Local dealer says to check wires around water pump area if rubbing and unsecured.
07-01-23 Vehicle died out, wouldn't start, got jumped, still wouldn't start. No indicator lights or warnings. Towed vehicle to the dealership, where it was for 11 1/2 months with the determination it was the ECM and Harness. Which it took that long to get. The vehicle was given back 06-14-24, even though it was in running condition, it was discovered parts were missing. Dealership put on order, received by end of July and installed. November 13, 2024, vehicle died out while driving on the freeway. Dashboard lit up everything, had to get the vehicle towed back to dealership. I have the Onstar diagnostics indicating the same components as the original reason the vehicle was held for an approximate year. Dealership now has vehicle and we have to wait for them to do a diagnostic.
06/13/2023: Odometer: 86881 Transmission-2nd Shift: Flare, 3rd Shift: rough shift flare, 4th Shift: shutter. Excessive metal particles in the fluid. No transmission codes or engine light detected. Result was a new rebuilt transmission. 11/01/23: Odometer: 90239 Engine Code: P00b7 coolant flow restriction, stuck thermostat. Result was to replace thermostat and flush coolant system. 12/06/23: Odometer: 91701 No codes detected. Fluids noticed on pavement occasionally. Valve cover gasket, vacuum pump gasket were failing. Result was to replace all parts. 02/25/24: Odometer: 92885 Code-engine overheating, idle engine. Critical 260 upon cold start. Failing temperature sensor. Result was to replace front and rear temperature sensor. Driver seat has begun to unravel and separate at the stich line on the side closest to the door. No repairs have been made. Issue began years ago. Approximately 2018. The area was small and has slowly widened. Approximately 6 inches currently even with avoidance of stretching and being cautious of that area. I do have documentation of this events and repairs if they are needed.
I have asthma and it is controlled by medication when needed. Recently I have had a lot more incidents of breathing difficulty since I purchased my vehicle. It was during one of these episodes that my wife noticed an odor associated with antifreeze. However, there is no moisture in the vehicle and no visible leaks or puddles on the ground. I have detected the same odor when standing near the back of the truck near the tailpipe. Also, I have had to refill the radiator with exactly five gallons of Antifreeze in less than 3000 miles of driving. It was my wife who alerted me to the odor being inside the vehicle and only during times when the heater was running. Since that time I have driven the vehicle only when I can safely leave the heater off, because I cannot be sure it is safe. If I need the heater to defrost, I leave the vehicle. Also, I noticed that when the engine coolant is low, the engine never seems to overheat, and the heater no longer blows hot air. Is this an issue involving only my vehicles and what other health risks are there? The most I have had to add to restore heat is a gallon of coolant.

The contact pushed the vehicle to the shoulder of the roadway, where the vehicle failed to restart. The check warning light was illuminated. The vehicle was towed to the dealer, who diagnosed failures with the purge valve and mass air flow sensor. The vehicle was repaired, but the failure reoccurred. The dealer was made aware of the failure and informed the contact that there was a possible failure with the vent valve canister. After a visual inspection of the engine compartment, the contact discovered water within the vent valve canister. After investigating the failure, the contact related it to Emission Recall: N202313200. The manufacturer was notified of the failure but referred the contact to the NHTSA Hotline for assistance. The contact had taken the vehicle to an independent mechanic where it was diagnosed and determined that the canister purge valve needed to be replaced. The vehicle was repaired. The contact stated that the vehicle experienced the failure two more times and was taken to the same mechanic. The contact stated that he had purchased the purge valve from an auto parts store and later purchased the valve from the manufacturer. The contact stated that the vehicle was repaired; however, the contact believed that the valves sold were defective. The manufacturer had not been informed of the failure. The contact owns a 2016 Chevrolet Colorado. The contact stated that while driving at various speeds the vehicle would have a sudden loss of power steering. The failure occurred intermittently. The contact believed the check engine light, traction control, abs, and stability track warning lights and or messages become illuminated. The vehicle goes into limp mode causing the contact to have to turn the vehicle on and off several times. The contact stated that while their son was driving approximately 5 mph, on icy road conditions, the power steering suddenly failed to cause the driver to lose control of their vehicle. The vehicle ended up veering off the side of the road into a ditch about 6 feet deep. The air bags did not deploy. The driver had no injuries and no medical attention was needed. A police report was not filed. The vehicle had to be lifted out of the ditch and then towed. There was no damage. The vehicle had not been repaired. The manufacturer was made aware of the failure. The approximate failure mileage 140,000.

The contact owns a 2016 Chevrolet Colorado. The contact stated that on several occasions, the engine, traction, and stabilitrak warning lights illuminated then the vehicle went into Limp Mode. The vehicle was taken to AutoZone where a diagnostic test indicated that the powertrain system and cylinder #1 had failed. The contact stated while driving downhill, the RPM increased however, the vehicle failed to respond. Additionally, the contact stated that the ABS warning light illuminated and the vehicle went into Limp Mode. The vehicle was towed to the dealer where it was diagnosed that the fuel pipe, fuel rail and fuel injector needed to be replaced. The vehicle was repaired however, the failure recurred. The vehicle was then taken to a different dealer where it was diagnosed that cylinder #1 and the power steering failed and needed to be replaced. The vehicle was not repaired. The manufacturer was notified of the failure and the contact was informed that she would receive a loaner vehicle however, the manufacturer was contacted a second time where the contact was denied a loaner vehicle due to a warranty issue. The failure mileage was 138,000.

While making a left turn through an intersection, the vehicle's engine shut down without any warning. The electronics were still powered. My safety was put at risk, as other drivers were also going through the intersection. If this issue occurs again at another random time and I lose power while driving, this could easily result in an accident. I have not had it inspected by a mechanic, since it just happened, but have read online that others with Chevy Colorados have had this issue and could not find the cause.
One morning I started my truck to go to work and the engine started ticking when I arrived at work the engine was still ticking . Being a European Automotive Technician for 34 years I knew this was bad . I performed an extensive diagnosis of the engine and found metal particles in the oil and filter at that point I knew that something in the engine had failed . I removed the engine and removed the oil pan for further inspection and discovered that the # 2 cylinder piston was broken below the wrist pin.

my truck has a issue when you are going down a hill it shifts into a lower gear. This is an automatic transmission. This can cause my truck to slide and spin in bad weather. I have seen people comment about this issue on some Colorado forums. It has done this many times and at times I have pulled over and placed it in park

The contact stated that while driving at highway speeds, after switching from the automatic drive mode to manual drive mode, the driver was able to manually switch to 1st gear which caused the speed to abruptly decrease. 2016-2020 Chevy Colorado and GMC Canyon diesel models have faulty DPF and DEF systems that fail within <20-60K miles. They do not warranty it as powertrain, and it is a common problem. SAFETY ISSUE: While out of state, the vehicle regenerating exhaust cleaning cycle can not properly clean the system. It then goes into limp mode. 2 dealers have tried to fix it, with no avail. Limp mode limits speed from 65mph with no power on hills, to 55, then 4mph. DURING ANY TYPE OF MOISTURE IE (FOG, RAIN) VEHICLE GOES INTO REDUCED ENGINE POWER MODE CODES FROM THE ECM P2122 AND P2138 BOTH RELATING TO APP. FIRST TIME IT HAPPENED WAS IN RAIN STORM IN MOTION, APP PEDAL WAS REPLACED WITH NEW OEM PART BUT THE PROBLEM PERSISTS. I SUSPECT WATER INTRUSION FROM WIPER COWLING AFFECTING ELECTRIC HARNESS. THIS PROBLEM DOES NOT EXIST DURING DRY SEASON. AFTER MANY HOURS OF ONLINE RESEARCH I HAVE FOUND THAT THIS IS A WIDESPREAD PROBLEM IN CHEVROLET VEHICLES.

I WAS TURNING THROUGH A BUSY INTERSECTION AND AS I COMPLETED MY TURN AND CAME TO A STOP THE ENGINE SHUT DOWN. PRIOR TO MAKING THE TURN THE ENGINE FELT LIKE IT WAS GOING TO STALL OUT AND QUIT RUNNING. I HAVE EXPERIENCED THE SLUGGISH IDLE BEFORE BUT IT NEVER SHUT DOWN LIKE IT DID TODAY. A GOOGLE SEARCH REVEALED THAT CHEVROLET HAS A TECHNICAL SB, 16-NA-284 THAT SAYS A REPROGRAM OF THE COMPUTER IS REQUIRED TO CORRECT THE PROBLEM. IN MY OPINION THIS IS A SAFETY ISSUE. I WAS FORTUNATE TO HAVE COMPLETED MY TURN BEFORE IT QUIT RUNNING BUT SOMEONE ELSE MAY NOT EVEN BE AWARE OF THE SITUATION PUTTING THEMSELVES AT RISK IF IT WOULD HAPPEN TO QUIT DURING THE TURN WITH ONCOMING TRAFFIC.

TOOK TO DEALERSHIP WHERE I PURCHASED AND THEY TEST DROVE IT AND SAID THE HILL DESCENT FUNCTION WAS NOT WORKING PROPERLY BUT THERE WERE NO RECALLS OR SOFTWARE UPDATE ISSUE AT THIS TIME THAT WAS COVERED UNDER WARRANTY. ISSUED COMPLAINT TO CHEVROLET AND THEY SAID THERE WERE NOT ENOUGH COMPLAINTS TO ADDRESS THIS ISSUE AT THIS TIME. VERY FRUSTRATED.

I HAVE A 2016 CHEVY COLORADO 4 CYLINDER TRUCK AT THE DEALER AGAIN. HAD IT IN ABOUT 8-10 MONTHS AGO ON THE ENGINE REVIVING REAL HIGH WHEN COMING TO A STOP. THEY SAID IT WAS PROBABLY THE HILL ASSIST SYSTEM AND NOTHING THEY COULD DO. NOW IT'S CHRONIC. IT CAN REV TO 2200 RPMS AS I COME TO A STOP. I HAVE TO KICK IT IN NEUTRAL TO KEEP FROM HITTING A KID IN A CROSSWALK OR THE CAR IN FRONT OF THE. BOTH FEET ON THE BRAKES. AND, WHEN I PUT IT IN NEUTRAL, IT CAN REV UP TO 4000 RPM'S. IT'S BACK AT THE DEALER NOW AND THEY DON'T HAVE ANY SERVICE BULLETINS ON IT. I TEST DROVE A V-6 300 HP MODEL WHILE THERE AND IT DID THE SAME THING 2 TIMES AND THE SALESMAN DIDN'T NOTICE, BUT IT NEVER GOT OVER 1800 RPMS. SO,THEY HAVEN'T FIXED IT. BUT THEY'RE SINCERELY TRIED BUT I THINK IT'S THE SYSTEM NOT THE DEALER. THE HILL ASSIST SYSTEM DOESN'T PICK UP ON IF YOU'RE LEVEL OR GOING DOWN A HILL ANYMORE. JUST REVS UP, AND IT'S IN 1ST GEAR WHEN COMING TO A STOP.THE TORQUE IS TREMENDOUS ON THE BRAKES. IT'S AN ACCIDENT WAITING TO HAPPEN. AT APPROX. 3500 MILES STARTED EXPERIENCING SHIFTING ISSUES. WHEN SLOWING DOWN TRANSMISSION WANTED TO STAY IN 5TH OR 6TH GEAR. ATTEMPTS TO ACCELERATE CAUSED THE TRUCK TO BOG DOWN UNTIL IT SHIFTED TO A LOWER GEAR TO ACCELERATE AGAIN. WHEN IN CRUISE CONTROL PROBLEM WAS REALLY BAD. WOULD LOOSE AS MUCH AS 5-8 MPH BEFORE A LOWER GEAR WAS FOUND TO MAINTAIN SPEED IN HILLY CONDITIONS. I TOOK VEHICLE IN FOR ITS FIRST SERVICE AT APPROX. 6000 MILES AND HAD DEALER CHECK TRANSMISSION. THEY REMAPPED SHIFT POINTS TO SPEC SOFTWARE. THIS WAS A TEMPORARY FIX. ISSUE IS SLOWLY RETURNING. I HAVE WATCHED ON CONTROL PANEL AT TRANSMISSION OIL TEMP ( TRUCK HAS TRAILER TOWING PACKAGE ETC. THIS IS A CLUSTER FEATURE AVAILABLE.) SEEMS TO SHIFT PROPERLY UNTIL OIL REACHES 100 DEGREES PLUS THEN ISSUE ARRIVES. DEALER RELAYED THERE HAD ONLY BEEN A TECH SERVICE BULLETIN HAS BEEN TO RESET MAP/SHIFT POINTS. RECENTLY PULLED MY BOAT WITH THIS TRUCK AND TRANSMISSION SHIFTED FINE. REALLY WOULD LIKE THIS ISSUE FIXED BECAUSE OTHER THEN THIS PROBLEM REALLY LIKE THE TRUCK.

ON FEBRUARY 21, 2017 WHILE MAKING A RIGHT TURN, MY 2016 CHEVROLET COLORADO 4WD CREW CAB TURNED ITSELF OFF. ON FEBRUARY 22, 2017 RICHARD LUCAS CHEVROLET LOOKED AT THE VEHICLE FOR 5 HOURS AN DROVE THE VEHICLE AROUND SEVERAL TIMES AND WERE "UNABLE TO DUPLICATE CONCERN." I WAS TOLD TO CALL BACK IF THE ISSUE EVER POPPED UP AGAIN, BUT IT WAS BELIEVED TO BE UNLIKELY. MARCH 28, 2017 WHILE DRIVING HOME FROM WORK, MY COLORADO IGNITION MADE A CLICK NOISE, AND THE IGNITION KEY TURNED ITSELF OFF YET AGAIN. THIS TIME AROUND WAS MUCH DIFFERENT THAN THE FIRST OCCURRENCE AS I HAD NO BRAKES TO STOP THE VEHICLE. TO STOP THE VEHICLE I HAD TO SHIFT INTO PARK WHICH I WAS ABLE TO DO. VEHICLE WILL BE RETURNING TO THE RICHARD LUCAS AUTO CENTER TO BE REEVALUATED. ON THE FIRST OCCURRENCE THE VEHICLE WAS IN MOTION ON A COUNTY ROAD WHILE COMPLETING A TURN AND THE SECOND TIME IT HAPPENED WHILE GOING STRAIGHT ON A RESIDENTIAL STREET.

BEEN IN SHOP NUMEROUS TIMES FOR SAME THING AND ADVISED CHEVY BUT THEY CANT FIGURE IT OUT AND IT IS STILL DOING IT. ALSO LOOKED ONLINE FOR POSSIBLE ANSWERS AND THERE ARE A LOT OF THESE SAME PROBLEMS REPORTED WITH NO ANSWERS.

SINCE PURCHASE OF VEHICLE, IT HAD A BUCKING AT SPEED OF 25-40 MPH, THIS WAS VERY NOTICEABLE IN CITY DRIVING, THIS ISSUE WAS COUPLE WITH UNCONTROLLED SURGES IN THE ENGINE. THE VEHICLE WAS PURCHASE OCT 28 OR 30, 2016. I TURNED IN THE VEHICLE FOR SERVICE ON DEC 19, 2016, DEALER SERVICE REP CONFIRM THERE WAS AN ISSUE AND KEEP THE TRUCK FOR REPAIRS. VEHICLE WAS IN THE SERVICE DEPARTMENT FOR OVER 40 CONSECUTIVE DAYS TO FIX ISSUE, DEALERSHIP TECHS, AS WELL AS GM FIELD REPS HAVE LOOKED AT THE VEHICLE AND HAVE TRIED TO FIX IT TO NO AVAIL. ON FEB 8, THE GM REP SAID THERE IS NOTHING WRONG WITH THE VEHICLE AND THE ISSUE WAS A SOFTWARE PROBLEM WITH THIS SPECIFIC TYPE VEHICLE AND THAT THE SOFTWARE THIS VEHICLE HAS IS A NEW SOFTWARE. WHEN I ASKED IF THIS ISSUE IS BEING WORKED BY THE SOFTWARE ENGINEERS FOR SOLUTION THE REP SAID HE HAS NO IDEA AND THAT I SHOULD TAKE THE VEHICLE BACK. THIS ISSUE IS A SAFETY PROBLEM ESPECIALLY WHEN SUDDENLY SURGES WITHOUT CONTROL, PARTICULARLY WHILE DRIVING IN A CITY CENTER A DRIVER CAN CAUSE AN ACCIDENT OR POTENTIALLY HIT A PERSON WHILE LOOSING CONTROL. GM COSTUMER ASSISTANT WAS CONTACTED, BUT THERE WAS NOT HELP AND GM REFUSES TO LET ME KNOW WHEN THE FIXES FOR THIS ISSUE WILL BE OUT. THEREFORE I HAVE REFUSE TO TAKE POSSESSION OF VEHICLE UNTIL IF FIXED CORRECTLY!! THIS ISSUE WILL AFFECT ALL 2016 AND UP COLORADO WITH DURAMAX ENGINE AND NEW SOFTWARE VERSION INSTALLED ON THEM.

IF YOU TRY TO SMOOTHLY ACCELERATE IT WILL RAPIDLY DOWNSHIFT AND JUMP FORWARD ALMOST REAR ENDING ANY VEHICLE IN FRONT. VERY UNSAFE TO DRIVE. TRYING TO DRIVE WITH THE FLOW OF TRAFFIC AT 35-40 MPH THE TRUCK SHUTTERS AS IF THE SPARK PLUGS ARE WORN OUT. ALSO TRYING TO ACCELERATE INTO ON COMING TRAFFIC IT WILL SOMETIMES CHUG TO WHERE YOU WONDER IF IT'S GOING MAKE MAKE THE MERGE. VERY FRUSTRATING TO DRIVE FOR A BRAND NEW TRUCK, THE DEALER REPLACED THE TORQUE CONVERTER AT 500 MILES, THEN HAS PREFORMED SERVICE BULLETIN #PIP5342B THREE TIMES WITH ABSOLUTELY NO IMPROVEMENT. GM STATES IN THE BULLETIN "NOTE: PREFORMING THIS CALIBRATION UPDATE MAY NOT TOTALLY ADDRESS THE SHUDDER OR CHUGGING CONCERNS" HOW CAN NEW VEHICLES CONTINUE TO BE SOLD WITH THIS KNOWN PROBLEM? NOT ONLY IS IT FRUSTRATING TRYING TO DRIVE SMOOTHLY, BUT IT IS ALSO A SAFETY CONCERN. DUE TO THE POOR PERFORMANCE THE MILEAGE IS ABOUT 13 MPG IN CITY TRAFFIC, NOT EVEN CLOSE TO THE MILEAGE RATING OF 17 MPG.

WHEN TRYING TO PASS, SEEMS TO BE ZERO ACCELERATION FELT WHEN FLOORING IT THEN SUDDENLY KICKS IN (ROUGHLY)..LIKE OH HEY-I NEED TO GO.. I COULD GET IN A WRECK-ENDED BECAUSE OF THIS AND I BETTER NOT TRY TO PASS ANYONE.. I FEEL MY SAFETY COULD BE AT RISK WITH AS MUCH HESITATION THEN SUDDENLY LURCHES AS IT HAS. I SEE QUITE A FEW COMPLAINTS ALREADY CONCERNING THIS AND/OR DESCRIBING JUST ABOUT THE SAME ORDEAL. DAILY - BUT RANDOM - CITY AND HIGHWAY, TURNING, STRAIGHT ETC. BASICALLY ANY SITUATION BUT IDLE.

THE SITUATION WHEN PULLING INTO TRAFFIC IS DANGEROUS BECAUSE THE TRUCK DOES NOT ACCELERATE AS IT SHOULD. THIS TRUCK WAS PURCHASED NEW AND HAS 1,500 MILES ON IT. THIS CONDITION HAS BEEN PRESENT SINCE THE TRUCK WAS NEW AND THE SEVERITY VARIES WITH TRUCK SPEED AND ROAD INCLINATION. THE SITUATION HAS BEEN REPORTED TO THE DEALER AND TO CHEVROLET CUSTOMER SERVICE. NO CORRECTIVE ACTIONS HAVE YET TO BE MADE."

ALL I COULD DO WAS WATCH IN THE REAR VIEW MIRROR AS TRAFFIC CONVERGED ON ME. THIS "DEAD THROTTLE / WRONG GEAR" ISSUE HAS HAPPENED REPEATEDLY ON SURFACE STREETS BUT LUCKILY ON SURFACE STREETS I WAS NOT IN SUCH A CRITICAL SITUATION. THIS IS A PROBLEM THAT HAS BEEN REPORTED REPEATEDLY AND AT MANY PLACES ON THE INTERNET. WHEN WILL NHTSA FORCE GM TO ADDRESS THIS ISSUE? MY PREVIOUS VEHICLE (2014 SILVERADO) WAS ALSO LISTED ON THE BULLETIN, BUT THE PROBLEM WAS NEVER NOTICED ON THIS VEHICLE. SEEM TO RUN OK BUT CHECK ENGINE LIGHT IS TILL ON NOW I AFRAID THE WILL JUST RESET IT AND SAY IT IS OK. AND NEXT TIME I WON'T BE SO LUCKY. IT HAS 5500 MILES ON IT.
