2025 CHEVROLET EQUINOX EV Forward Collision Avoidance Problems

The automated emergency braking system triggered when nothing was in front of the car. It suddenly stopped the vehicle for no reason and it almost caused an accident.

Yesterday the car was moving forward at 2-3 mph, when the brakes suddenly engaged without warning, bringing the car to a sudden stop, violently enough that I thought I had been in a collision. However, there were no cars behind me, and the car in front was about 20 feet away, also moving forward slowly. No pedestrians were in view. A light mist was falling, but otherwise the weather and visibility were fine. If there had been a car close behind me, or if this had happened on a highway, a serious accident could have resulted. A quick internet search shows that this problem has been reported by other Equinox owners, and the only apparent fix is to disengage the automatic braking system. This is a safety issue (both the unexpected braking problem itself and the unsafe fix of disengaging a major anti collision feature), and warrants a recall.

On 10/26/25 at approximately 3:40pm while on a freeway (traveling approximately 65mph) with no traffic or cars in close proximity for the automatic brakes to be engaged, the 2025 ev equinox slammed its brakes on its own without driver braking. This caused whiplash to be experienced by two of the three passengers and it felt like we were rear ended but no cars were around. This is very dangerous. Had there been car behind me this would have caused serious accident.

I have driven the vehicle 13,000 miles without incident. However, twice this week, once moving forward and once backward, the emergency parking brake engaged without warning, bringing the vehicle to an abrupt stop. In both cases it was daylight, clear weather, with no visual indication of an obstacle in the vehicle's path. In response I have turned off its automatic braking/collision avoidance features and will see if that solves the problem. What is concerning, however, is that this did not occur for 13,000 miles, and then abruptly started.

On star Telematics is not working the issue is effecting GPS and most importantly the adaptive driving system including cruise control system. I have stopped using these services to due an intermittent failure while driving. The adaptive cruise will disengage itself. Also the collision avoidance system will initiate braking randomly. I was told that the replacement parts are not available and or “extremely long wait time”. I’m not comfortable driving this vehicle and have no options but to continue until the lease is up.

Car randomly brakes hard when traveling at highway speeds with adaptive cruise control active. Happens about once a day.

Automatic emergency braking engaged when there was no risk of a collision. I was in a roundabout (traffic circle) when the brakes suddenly engaged, nearly stopping my car. There was a car yielding to roundabout traffic ahead of me (waiting for me to clear before entering the roundabout). I was nearly rear ended by another car due to the sudden braking. I then disabled the automatic emergency braking feature to avoid similar “phantom” collision detections.

I was pulling into the parking lot of my job. The one pedal driving mode was on, and my vehicle was not slowing down like it generally does. So, I immediately stepped on the brake, and it still didn't slow down, it felt like a brick, I stepped on it harder. It still did not stop! I ran into the barn/building I work in. I was terrified!! What if there was a person there, or another car, or kids playing, etc. I immediately called the Chevy Dealership in Woodland at 8am, they had me tow the 2025 Equinox to them. I just talked to them at 4:30pm. They said there were no messages with regard to the braking system. They said they would install the update, and would continue to diagnose the problem. I do not feel safe in this vehicle, it poses a significant risk as a driver!

OnStar module has failed and malfunctioned. Vehicle is available for inspection. Vehicle now sates it needs an important safety update from GM, but vehicle will not update. Vehicle has not been to a dealership at this time, because I can not get a dealership to give me an estimate for repairs, not will they provide a loaner. But the error code has been verified in writing by OnStar- yet they refuse to just send a new module to preferred dealer so I can get it repaired. Absolutely no warnings on vehicle. Forward emergency braking system engaged when not needed and almost put myself and my grandkids through the windshield. (even though we were all properly restrained) I have since had to disengage this feature because it has now happened 2 times. There are thousands of persons with these same issues as can be found on the OnStar community or Chevrolet Forums.

This has happened about 4 times in 4 months. In each scenario I am driving very slowly (3 times forwards, one time in reverse) with no obvious obstacle or crash potential. The brakes will slam suddenly very aggressively, I assume it is the e-brakes as it feels like I hit a wall. It is a safety issue as if someone was behind me it could cause an accident. I called the dealer as well and they were not aware of any issues. I have not had the vehicle inspected

BRAKING FAILURE on 2025 Equinox EV LT model - my car has had two separate brake failures going down a steep dry rocky road in clear weather, once with one pedal driving on and once without, on July 21st, 2025 and August 11th, 2025 respectively. It doesn't happen every time going down this road and each time at no more than 5 - 10 miles per hour, with the brake pedal to the floor the car was not slowing down and the weight of the car on the steep track made it speed up considerably which made it dangerous due to the steepness of the hill and turns in the road. The first time I drove off the road onto flat grass and the car came to a stop, the last time it happened I pumped the brakes until the car stopped and the "ABS engaged" light came on the dashboard above the steering wheel. It happened twice on the road that time. There were no prior alerts or warning lights. The car has 5000 miles and is still under warranty, the Chevy dealership had the car for 3 days. The dealership was not able to reproduce or confirm the problem with the brakes. The dealership told me they did a series of resets. They have not given me the written service report yet. I have an appointment to take the car back to the dealer. The brake fluid level is good, there are no current alerts on the dashboard while driving. The car is available for inspection upon request. I am concerned that my wife and I may be injured or killed in a braking failure or that we may injure or kill others or damage property.

Every car I’ve ever owned the cruise control disengages once you touch the brakes and does not reengage without pushing the button to say resume. On several occasions, this car has reengaged sometimes many minutes after I’ve touched the brakes multiple times such as in a parking lot or parking garage going around the corner The car resumes cruise, and rapidly accelerates, which is very dangerous in a parking lot or parking garage

Yesterday while approaching a hairpin exit, all of a sudden lights went on for the Stabilitrack, Collision alert and a number of other lights. Messages popped up saying braking assist was not available, cruise control was not available. Basically, the braking assist was gone and I had to press REALLY hard on the brake pedal to get any slowdown. All regen was gone and it was like driving a car without power brakes. I also got a message saying it would not go faster than 62mph. There was no indication of any issues prior to the Brake assist failure. This put me at extreme risk because it was essentially a power braking failure and it was extremely difficult to slow down. I had to really push hard on the brake pedal. I was able to drive my car back to the dealership where it is sitting now, waiting to be looked at in the morning of 3/24/25. I have attached a PDF of the OnStar diagnostic report, cryptic as usual, but it is a record of the codes that were thrown. Also seems like some modules were not communicating properly. I have also attached some photos of the error messages that were showing this morning.

The car phantom breaks randomly when backing up, but seems to happen more often when it rains or snows.

This also results in other critical features being disabled, such as the auto-hold and forward collision systems. I believe it also impacts the cruise control as well. I am greatly displeased with this and now want to give the vehicle back. My lessor, GM Financial, has specific guidelines in place that make this difficult. What can Chevrolet do to address this? I've heard about the recall, and subsequent remedy, but I'm not satisfied with that, as the issue could occur again. I've lost faith in the vehicle and need to get rid of it. Five times since I bought the new car in November 2025, the car has suddenly slammed on the brakes. It has always been under 5 miles per hour, but it has always been total stoppage, like someone hit me from behind. I think I figured out that the emergency braking system gets nervous when objects are close, and when I touched the brake with my foot the emergency braking system would freak out and slam the brakes. I talked to a person who had an electric Volvo who said the slamming of the brakes freaked him out, too. I finally went into the Settings and turned off the emergency braking system (but left on the warning.) I've not had the experience, since.

Illinois I-94 Westbound near Torrence Ave. Exit 5:35/5:36AM 2/5/2025 cruise set at 70mph, passing 18 wheeler on my right and a car was passing me on my left. No car in front of or behind me in my lane of travel. Vehicle braked hard all by itself. I had to cancel the cruise control to stop the braking. This could have been bad had there actually been another vehicle directly behind me when this happened. It has happened before but I did not record the details. I will be making a written record of this failure and reporting it every time it happens.

I can’t remember if I was using adaptive cruise control but I believe I was. No other beeps or alerts were present (as with AEB events). There was NO other traffic present and it was middle of the afternoon with no weather except a bit cloudy. I have no idea what hazard the car could have been reacting to.

My car which is FWD, not AWD, is exhibiting the same behavior that the recall for the AWD Equinox has. With Adaptive Cruise engaged the car does not slow down or brake when approaching vehicles in front of it. I have to intervene and apply the brakes to prevent hitting the vehicle in front. Also, with ACC on the vehicle will accelerate on its own. I have to intervene to slow the vehicle down. For example, I set the ACC to 68 mph, all of a sudden it starts speeding up. I had to intervene when it hit 73 mph as it approached vehicles in front of me without slowing down.

The adaptive cruise control does not come to a stop when the cars ahead do. It works at high speeds as expected, but when traffic slows to a stop, it will not brake enough to prevent you from hitting the vehicle in front of you.

Does not adapt to front vehicles speed, just stays at the speed it is set at. I have to hit the brakes to disconnect ACC.

When I activate adaptive cruise control with cruise braking my car will follow the the car ahead of me as it speeds up or slows down. But if that car slows down to a stop light, my car will not slow down enough before the red collision warning activates and I need to apply the brakes immediately to avoid impact. Happens every time at any gap distance or set speed.

ACC works ok at normal highway speeds but when the vehicle in front slows to under about 25mph the ACC doesn't slow down anymore. Then the vehicle alerts with the emergency breaking light and you need to stomp on the breaks to avoid a collision. It has been like this since I bought the car and Chevrolet doesn't seem to know what the problem is

Brand new Chevrolet Equinox EV. When Adaptive Cruise Control is engaged, vehicle will accelerate or coast to mach speed of vehicle ahead, but will not engage brakes. ACC would collide with vehicle ahead unless driver acts. Problem is ongoing. Not one-time. Have scheduled dealer service.
