Reversing onto roadway and shifted from reverse to drive via rotary dial and vehicle went into park and would not move causing me to block the roadway. In addition and even more concerning was that the vehicle could not be turned off and any and all actions to do so just left the car "on" and the hybrid system running. Eventually we pulled the battery cable on the main 12v in the trunk and the reset allowed us to move the vehicle again. I sat across a busy section for twenty minutes with no way to move, turn off, or overide the system with three kids with me in a busy traffic area. The vehicle has not reproduced the issue as of yet. The dash had multiple warnings for park system and service various items needed. this occurred 12/17/2025 with 70,000 miles.
While driving down the highway at 70 miles per hour ater driving 2.5 hours our 2021 Pacifica hybrid minivan flashed a warning on the vehicles display saying approximately "Warning Engine Shutting Down". The engine immediately shut down in the middle of the highway and we were forced to coast off the highway to the shoulder. The engine warning light came on. We turned the car off for a few minutes and proceeded to start the engine. The engine warning light was still on, but the other message was gone. We attempted to drive towards our destination and approximately 30 minutes later the same thing happened again. We tried one more time since we were several hours from home and within 30 minutes the vehicle had the same issue. At that point we decided to try to get home. We drove the vehicle 15-20 mins at a time and then exited the highway over and over giving the vehicle time turned off. After returning home with no additional events the engine warning light went off the next warning. This was on a Sunday so we were unable to get it to a dealer. The next day we dropped off the vehicle at Nappletom Chrysler dealership. The called us the next day and told us they could not cause the event to happen again. They could see a set of error codes showing the problem occurred, but told us there was nothing they could do and it was up to us to cause the problem and then return the vehicle. Now I have a minivan with 70k miles on it that my wife primarily uses my with to get my daughter from place to place and I have no idea when this vehicle will shut the engine down at any time potentially causing a dangerous accident. I believe Chrysler has the responsibility to determine what the issue is. Researching on the internet show this is a common problem for my vehicle and typically requires a new transmission or and engine wiring short.
The ESS battery fails constantly and manufacturer will not find problem and just replaces battery. When the ESS battery fails it drains the main battery which then leaves the driver and its occupants at risk of being stranded. The transmission is an electronic transmission and if the battery fails while driving that could compromise the occupants also. We have had to have the battery replace twice and a third time tomorrow 9/21 if the dealership will "fit" us in. The ESS system will tell you it is unavailable but only after the battery has failed.
While driving my vehicle soon after purchase I notice an engine light on along with a huge fan noise from under the hood. Addressed the issue with dealer they diagnosed the car and so called fixed the issue. about a month ago I’m driving the car on a residential street and noticed the car jerking loud fan noise and the whole dashboard light up saying service hybrid service engine and transmission.. Suddenly the engine stops in moving traffic. The vehicle windshield wipers cut on by themselves and the car stayed in park.. tow the vehicle to the dealer using only a dolly tow truck due to vehicle unable to be placed in neutral. Dealer calls to give update and noted no codes were stored and there is currently no issue with vehicle..He suggested to keep the vehicle longer to see if the problem returns. I’m afraid the electrical issue that is going on with this hybrid will cost me and my children life one day. this vehicle is not safe to drive and I am stuck in a purchase contract.
Since week 1 of owning my brand new 2021 Chrysler Pacifica Hybrid, I have had issues of loss of motive power. I have done all of the recall remedies yet my car with a 35day old 12v battery not only died on me, but drained every battery in my vehicle so that I couldnt open my doors. The vehicle was unable to be jumped so that it could be driven, so I ended up paying o or $1000 in towing to get the vehicle to my dealership to be diagnosed,AGAIN. That happened on July 12th, I finally had it towed on July 14th to my dealership & it has been there since. Although the PIM & BATTERY are under WARRANTY, Chrysler is refusing to reimburse me any of the tow fees & only 20 days of a rental I have now had for a month. The dealership installed the PIM which cleared a couple of codes, but not all. They installed ANOTHER new 12v battery, which was immediately drained. They have ordered another coolant something or other for the battery & here we are. I attempted in 2022 to retain a lemon law lawyer but he gave up very easily once Chrysler just stopped answering him.he said because I had my car updated under the class action lawsuit Moran is Chrysler FCA et Al, I was not able to continue under Lemon Law. I have not been able to find a lawyer, even one from the Class Action Lawsuit, to assist me in fighting Chrysler to do something to make me whole. My vehicle needs to be inspected in less than 40 days but at this point my 5 yr old vehicle can NOT pass the Pa inspection, so what am I supposed to do? I live in the mountains, have an 8 yrold grandchild that im raising & certainly cant afford to rent cars forever. I still haven't gotten my pension from the Coty of Philadelphia after 3 years of retiring after getting hurt on the job. I just read an article about the Office of Drfective products investigating Chrysler in 2022 for EXACTLY the problems I continue to have!!
When driving our van from Nashville to Memphis, we were 2 hours from home when we received an on screen alert stating “Stop Safely - vehicle will shut off soon”. This was in a construction area with little to no shoulders to pull the vehicle over as power loss began to occur. We contacted Chrysler for help but the nearest dealership told us the earliest they could see the vehicle was 3 weeks from Tuesday. I had 3 children in the car ages [XXX}. A web search for this shows how prevalent this issue is with several blogs and Reddit forums indicating a problem. The NHTSA even has an open investigation into this specific issue. Apparently the only fix is a full transmission replacement, but we have no assurance from Chrysler that will happen and now our van is left 3 hours away in Memphis as we had no choice but to leave there. On [XXX], I was driving uphill on the interstate at around 70 mph on gas power (hybrid battery was discharged) and warning came up "Stop Safely Vehicle Will Shut Down Soon" and lost power (Limp mode). I was able to move safely to the shoulder, but with 5 passengers (3 young kids) and a dog and a full load of camping gear it was a bit nervewracking. Could not get the car started again (stayed in limp mode) and used an OBDII scanner I had which popped up error code (P0A43 - "intermittent issue with the drive motor "A" position sensor"). The vehicle was towed to the dealer ( 4 hours from home) and after a few days they said the vehicle was fixed and I pushed for them to test drive it on the interstate. Was returned with this comment: " FOUND CODES FOR DRIVE MOTOR "A" POSITION SENSOR CIRCUIT INTERMITTENT AND HYBRID BATTERY PACK STATE OF CHARGE LOW. CHECKED TRANS PLUG AND WIRING VERIFIED OPERATING CORRECTLY. FOUND THAT RESOLVER NEEDS TO HAVE RELEARN PREFORMED. PREFORMED RELEARN ON RESOLVER AND CHARGED HYBRID BATTERY. TEST DROVE VEHICLE ON INTERSTATE AND VERIFIED THAT ISSUE DID NOT RETURN. VEHICLE OPERATING AS DESIGNED." I received a notice of an unlimited mile/year warranty on the transmission for this car, and from what I read on the internet, this is a recurrent issue with a known cause. We were able to get off the road safely in this case, but it is nerve-wracking to drive, especially with my family, in the current state. The side back doors do not work all of a sudden when you push the button on the vehicle frame inside of the van. This is on both sides of the van. They will only shut by pushing buttons on the outside door handles. Also, the windshield wipers will only work on high. There is no intermittent, low, or medium speed working.
1. This vehicle was recalled for a software update due to the rear camera failing to operate at times. The software was updated by the dealer. However, this did not completely fix the problem. While the camera fails less frequently, it still does not energize at times. I took the vehicle back to the dealer. I was then informed that there is water intrusion into the rear lift gate, which causes the electrical to short out. I was told that I would have to replace the wiring harness which energizes the camera as well as the rear vehicle lights. I have not noticed any problem with the lighting as of yet. However, I have been told that this is a problem with the Pacifica in general, as well as the Durango. I was told that replacing the harness would not guarantee that the problem would be solved as there would still be water intrusion, which could cause the problem again, even with replacing the harness. This is obviously a safety issue, as the camera fails to operate at times, and there is a potential of losing lighting on the back of the vehicle. This is also a known defect by Chrysler. 2. When using the Adaptive Cruise Control on streets and highways with multiple lanes traveling the same direction, the vehicle will brake suddenly and hard when the vehicle in front of me changes lanes. This occurs after the vehicle in front of me has completely changed lanes and is not impeding my forward progress whatsoever. My vehicle speeds up to the set speed and then violently brakes as if it is about to crash into the vehicle ahead of me, even thought the lane is completely clear. The vehicle has been inspected for this scenario. I was told that it didn't perform as described above when the technician drove it. This is a safety issue as the vehicle suddenly and violently brakes at highway speeds, with traffic behind me.
While driving 70MPH on the expressway the dash randomly sent a message saying “Pull over vehicle shutting off” and then it put itself into limp mode. Thankfully we were able to coast to the small shoulder and put on our hazards. This happened at 2am with 4 kids in the car. Nothing in the car would work, it wouldn’t turn on, shift, nothing. We had to sit on the side of the expressway for hours waiting for a tow. While waiting it had the check engine light on, limp mode light, and messages to service hybrid system & plug in battery for reconditioning. It is waiting to be serviced, but there is a very limited amount of techs that will service it due to being a hybrid.
The emergency brake came on while I was driving
On three separate days, 2021 Pacifica lost power at 70 mph. After second incident transmission was replaced. The third loss of power occurred after the new transmission was installed. All of these incidents occurred on freeways with heavy semi-truck traffic.
My car has the 03A recall, and had the software update in March 2024. In Summer 2024, I got the red turtle, "Stop, pull over. your car is going to stop" warning, my car stopped moving, etc. I was able to get it restarted and CEL came on and has been on ever since. I took it in for the open recalls (66B), and state inspection, as well as the liftgate recall that was performed by the dealership but the liftgate has never worked since then. They stated that all the recalls except 66B were completed and when I asked why I had gotten the STOP NOW message after the software "fix" was done, they had no answer. I provided them with research stating that Chrysler had issued an unlimited lifetime warranty for these transmissions, since that is where the short is located that is causing the cars to stop. They have insisted since then that the transmission isn't the problem; that my hybrid battery is the problem. I have had no issues that would indicate the battery is the problem, but plenty to indicate the transmission is the problem. My car's been in the shop since March 17, and it is April 27. The new battery was supposed to be in on April 10, but there is still no ETA. I have a STAR case and a case open with Customer Cares, who has been absolutely no help in getting this problem actually fixed. Despite my providing the dealership and Customer Cares the necessary information stating that the new transmission is the only repair that will stop the car from stopping randomly, endangering myself and my family and other people on the roads, they continue to encourage me to "trust the engineers" and let them replace the battery. They are REFUSING to fix the issue, and I will not drive that car until it's properly fixed. This transmission issue is creating death traps and there is a known fix that even Chrysler has acknowledged by issuing lifetime warranties on the transmissions affected by this recall, yet the dealership and Chrysler Cares refuses to perform the fix for it.
Driving on freeway at 70 mph clear conditions 65degree weather. Warning light said to stop safely engine would shut down soon. Pulled over safely, waited, restarted. Drove another 10 miles, same error message, car lost power.
We noticed our van having issues getting stuck in gear and then jerking to get out of the gear. After doing further research it seems to be a common issue in 2021 Pacifica vans. The next issue is electrical. Our van rear door opens if we are too close to it with the keys and now the sliding door will not open. They just fixed a recall regarding the entertainment system and back up camera. The entertainment system is still having issues. When we start the car up the system will reboot itself.
While driving home with my children in the vehicle. When coming to a turn, the vehicle completely turned off without warning while I was doing about 35 MPh.
Dear Sir, I am submitting this formal complaint regarding severe misconduct, fraud, and reckless endangerment committed by Hyundai Escondido, Hyundai Motor America, and Ally Financial in connection with the sale and financing of a dangerously defective 2021 Chrysler Pacifica Hybrid Touring AWD (VIN: [XXX] ). 1. Background of the Case On [XXX] I purchased the above-referenced vehicle from Hyundai Escondido under the representation that it was a Hyundai Certified Pre-Owned (CPO) vehicle that had passed rigorous manufacturer standards. The dealership provided me with a Carfax report showing no prior accidents or major repairs and falsely assured me that the car was in excellent condition. However, immediately after purchase, I discovered that the vehicle had severe hidden defects, including: •Extensive flood damage, which was concealed at the time of sale and significantly compromised the car’s electrical and safety systems. •Prior frame and structural damage from an accident that was never disclosed. •Critical electrical malfunctions causing the vehicle to swerve uncontrollably on its own, without any driver input. •Severe safety risks, which nearly resulted in an accident with another vehicle carrying a baby on board. •Substantial mechanical and structural issues requiring over $20,000 in repairs, making the car unsafe and unusable. Despite reporting these issues on the first day of ownership, Hyundai Escondido refused to acknowledge or rectify the matter. 2. Hyundai Escondido’s Role in Fraud and Misrepresentation •Knowingly sold a flood-damaged and structurally compromised vehicle without disclosure. •Misrepresented the vehicle as a Certified Pre owned. My Pacifica completely shut down. All the warning lights came on such as crash detected, ABS light, and the windshield wipers came on. The vehicle was towed to the dealership that I bought the vehicle from. They completed a hard reset of the vehicle, and I took it home. Two days later it did the same thing all the lights came on, but it allowed me to drive it to the dealership. Still waiting to see what they think the problem is this time.
I was backing out of the driveway. I had all the safety features on, and I was checking my surroundings. Typically, the blind spot and front sensors would alert me to dangers. However, on this and many other occasions the warnings didn't alert me to danger on my passenger side that I couldn't see. this resulted in damage to the front quarter panel of my van as well as damage to the bumper. The body shop said it would cost at least $5000 to repair the damage. I can't afford that; in turn I am driving around with the damage.
I received the "Red Wrench" warning light several times, but it would clear. One day while travelling in a 45mph zone the car suddenly stopped and I came to a skidding stop, luckily I was coming to my turn and was only going about 10mph. I did fear I was going to get rear ended, luckily it did not occur. I was stuck in the road and traffic built up. I couldn't push the car off the road because every time I put it in Neutral and opened the drivers door it would go into Park. My daughter lived nearby, so they came to assist with diverting traffic around me and to put safety cones out. I called the County Sheriff because I felt I was a safety hazard. When a Deputy arrived he and my Son In Law were able to push the car off the road while I steered. I was blocking traffic for about 30 minutes. I called a flat bed who towed the vehicle to the dealer. The issue was troubleshot to the Power Inverter Module (PIM) internally shorted causing the Main Battery Disconnect fuse to blow. The dealer replaced the PIM and the Main Battery Disconnect. This occurred on or about June 27, 2023. The dealer invoice is Aug 21, 2023. It took several weeks to get the part and repair. This issue has not re-occurred. At the time mileage was 29564, currently it is 48171.
On 12/20/2024 my wife was driving about 20mph on a road and with no warning lights or messages the vehicle completely shut down. Nothing would work on the vehicle. She could not even put my hazard lights on or put the vehicle in neutral to move out of the roadway. The vehicle blocked traffic for an hour. It was fortunate the car behind did not hit our vehicle and also that my children were not in the car with her. The police assisted in redirecting traffic around the vehicle but they also were unable to get anything working. A tow truck towed to the dealership to assess what's going on. There was absolutely no warnings or previous instances or issues which makes this extremely dangerous. At only 20mph the car still significantly jerked when it came to an abrupt halt.
My wife was driving driving the vehicle on October 21st, 2024. She was shifting from Park to Drive just after our children got out of the vehicle in the school parking lot when the whole vehicle froze up and the dashboard lite up with all error lights including the engine light. It was flashing "Service Hybrid Battery System", "Service Transmission", "Turn off parking brake" and the Vehicle began to power cycle on and off, then on with gas motor, then on with electric motor. The vehicle could not be powered off and it could not be shifted out of gear and the parking brake could not be disengaged. The tow truck had to drag our Pacifica onto the flatbed while it continued to turn on and off and cycle through the engine and hybrid system. This vehicle has 36,000 miles and just had the transmission replaced under the "Recall for hybrid transmission systems" This is the second time this has happened, it originally occurred at 7000 miles and after the dealer had it for a week we were told there was no problem. The dealer had to pull a bunch of fuses and the 12v battery to get the vehicle to stop turning off and on, the same thing happened this time when it was dragged off the flatbed at the dealer.
Two things: my auto start/stop often shuts off, and the display reads: "Auto stop/start is disabled/hood open," though it's not. I also often get a "Service Stop/Start system" notification and the car will require a restart before it will allow me to shift it back into gear and turn off parking brake.
I was driving in the highway with both of my kids in the car. We were driving from Ohio to Alabama. While on the highway, the vehicle shut down into a disabled mode and displayed the message “stop safely, vehicle will shut off soon”. It did this a total of 5 times on our trip down to Alabama.
The battery coolant is leaking from the rear of the vehicle
I purchased my vehicle from AutoNation Chrysler Dodge Jeep RAM Spring - 21027 I-45, Spring, TX 77388 on or about April 15, 2021. I began having trouble with the electric/gas drivetrain around January 30 of 2023. The vehicle lost all power, as well as throttle and proper braking response. I brought my vehicle back to Helfman Dodge Chrysler Jeep RAM FIAT at 7720 Katy Fwy A, Houston, TX 77024 for repairs on February 6, 2023. The repairs performed seemed to have worked until [XXX]. The most recent event on [XXX] my Pacifica Hybrid lost all power on the highway. While attempting to pull the vehicle off of the highway it abruptly entered its parking gear at 5mph, almost causing a rear end collision from the semi tractor trailer following behind us. I am having a problem with my under-warranty (32,000 miles) 2021 Chrysler Pacifica which the service department at my dealership appears unable to help. I was driving on an interstate in stop-and-go traffic for a couple of hours. It was a very hot day, temperatures well above 90 degrees. Six times over 15 minutes the following occurred: While coasting/braking, so at low RPM the engine turned itself off I braked to stop the vehicle The vehicle put itself into Park A message on the dash said to place my foot on the brake pedal and press the button to start the engine I did so, and the engine cranked for an unusually long time, becoming longer each of the 6 times, before eventually starting By the last time we were doubtful it would start at all After the 6th time I cut my long-distance trip short, turned around, and came home in normal (not stop-and-go) highway traffic conditions, with no more engine failure. The check engine light did not turn on, and no service codes were generated which could have been related to these power failures My local dealer's service department (Pohanka in Salisbury, MD) tested the alternator, starter, and battery and found nothing wrong. They did charge the battery up a bit, saying it took only 20 minutes. They have been unable to re-create the problem, which is not surprising, due to the unique conditions under which it happened. They say there is nothing they can do about my problem under warranty because there is no code and they can't reproduce the problem, so they want to give me back a vehicle that we know behaved unsafely six times under dangerous conditions. Engines failing without warning on interstates are likely to result in being rear-ended. I see online that the hybrid model of the same model year has a recall for exactly this same issue, so it is not a new issue for Chrysler, although maybe new-ish for my non-hybrid model of the same vehicle.
Traveling down [XXX] in Escondido at approximately 73mph. In the fast lane and moving left into the car pool lane the vehicle lost all motive power and the error "Service Charging System" came up on the dash then flashing back to "SHIFT TO P TO START". We were able to make it to a very small emergency lane on the left side of the interstate. Had to power vehicle off and restart twice before be regained motive power. There was a very good chance that we would have been stranded in the middle of the interstate with wife and 2 kids in the care on an 85 degree day. We were able to make it off freeway with "Check engine light showing" but no other errors on the dash. We were able to drive home 7 miles and will tow the Pacifica to the dealer tomorrow. The contact owns a 2021 Chrysler Pacifica. The contact stated that after parking the vehicle and shifting to park(P), the contact attempted to turn off the vehicle however, the engine remained running. The contact stated that the failure was intermittent however, on one occasion, the engine remained running for almost 2-hours after pressing the START/STOP button. The contact stated no warning light was illuminated. The contact had taken the vehicle to a local dealer, where it was diagnosed and determined that the wiring harness behind the dashboard needed to be replaced. The vehicle was not repaired. The manufacturer was informed of the failure. The failure mileage was approximately 76,000.

Vehicle has suddenly went into low power mode while driving three times. Was towed to shop each time to find no issues besides codes. Codes were cleared and vehicle was returned. Now vehicle is in the shop at chrysler due to a service charging system code for the hybrid system, causing the vehicle to not start or shift out of park gear. Vehicle has 58,000 miles and is not covered under the battery warranty with only 2 thousand miles remaining on the factory power train warranty.
For the 3rd time after driving approximately 200 miles, the vehicle has shut down on the highway. I receive a warning instructing me to pull over safely because the vehicle is shutting down. All three of these times happened on interstate highways. Two times this occured on the NJ Turnpike. My husband is disabled and the process of getting off the highway - involving exiting our vehicle and getting into a tow truck is frightening and daunting. By this third time I figured out that we can cool the vehicle by waiting. Which we did because this last breakdown happened again on the NJ Turnpike. I had 3 dogs with me in addition to my handicapped husband. I could not imagine getting out into the lanes of traffic with the three dogs and the disabled husband. I waited 45 minutes and the car started and we were able to proceed 10 miles. At which point we were several miles from the exit off of the turnpike. We were now within 7.5 miles of a dealership. We were instru cted by the vehicle again to pull over safely because the car was shutting down. We waited another 30-45 minutes. Managed to get off the highway. Now within 1.5 miles of the dealership we were told again to pull over. We waited again and made it to the dealership. The codes were the same codes we had on the April 7 breakdown -- the February breakdown also had our car not being able to start. I bought a used Chrysler 2021 Pacifica Hybrid that has broken down for the third time with the same codes P1A9A, P1A9AE, P1A21, and P1AA2, and the low-temperature active pump is suspected to be the issue with battery cooling and heating.
On two separate occasions (on April 30 and May 1, 2024), my Chrysler Pacifica's engine suddenly lost power while driving on highways. I was served with an error "Stop Safely. Vehicle Will Shut Off Soon." After a few minutes on the side of the highway, the vehicle was able to start again and drive. After the first incident, I drove to a Chrysler dealership, who plugged in a portable computer to check for error codes. Despite the severity of the issue, the vehicle's onboard computer recorded no errors. Since then, I found that the NHTSA is aware of the problem (NHTSA Action Number RQ24004) related to a wiring harness short circuit. Chrysler's supposed "recall" only entails a feature to supposedly allow time to pull over safely. I enjoyed no such time. I lost power immediately and had to immediately traverse multiple lanes in busy traffic to pull off the road.
I had purchased the vehicle three days beforehand. It was a lemon law buyback car. I had driven around 120 miles or so that day. The high that day was at least 90 degrees or so. It was past the halfway mark of driving that day, when I tested the Level 2 EVSE I had bought. It didn't work because the plug wasn't energized, but I did notice that the time to charge was reading 3 hours instead of the expected 2 hours. Once I got back to town, I plugged in to a Chargepoint charger, which was only charging at a Level 1 rate. I contacted the dealer to report the problem, and then went to my storage locker. On the way, the AC stopped working. When I tried to leave, the vehicle would not start. Suspecting an overheating problem, I left the hood open for a half hour or so, and after that, the vehicle started up. On my way home, the vehicle suffered a complete loss of motive power, displaying a message saying "Stop Safely Vehicle Will Shut Off Soon", leaving me stranded in a parking lot. This was followed by a message saying "Service Charging System". After another hour or so of sitting with the hood up, the vehicle worked again. This has happened multiple times since then. I have traced the problem to the battery pack reaching a temperature of 55 C, at which point the vehicle invariably shuts down. One time it did this, I was on stuck on the side of a road with no shoulder, after dark. The dealers I have taken it to could not reproduce the problem, as it takes hours of driving to trigger, and they are not willing to drive it enough to reproduce the problem. A case was raised with the manufacturer, but their protocols were insufficient to reproduce the problem. I have recently learned that the problem is apparently the same problem that the original owner had, and that the vehicle was bought back for originally. I have collected extensive CSV logs documenting the defect. The vehicle is obviously still unfixed, as they will not fix it until they duplicate the problem.
I was driving home from a trip of 208 miles. I drove to Bethlehem PA 208 miles, was there for 4 hours and was returning home driving another 208 miles. I did not plug in my plug in hybrid prior to leaving Bethlehem, PA. When I was less than 10 miles away from my home, lights came on and my dashboard said "Stop Safely Vehicle Will Shut Off Soon". I have a picture of the dash. In the upper left hand corner was a picture of the Battery on the right lower corner a picture of a plug. I had 288 miles range left on my gas. My husband is disabled and the police officer who pulled behind me had to close a lane of traffic on the Interstate Highway [XXX] ) to allow him to disembark the vehicle at 8:30 pm and to reenter the tow truck that towed us off the highway. Vehicle was having driving issues for months and dealership could not identify the issue. Van showed "check engine" light, stopped using electrical propulsion (while fully charged), and engine started making a lot of noise during driving. Vehicle was kept off the highway until it could be taken into the dealership for service. Dealership identified issue as battery pack coolant heater and replaced it.
When at a red light or while driving car throws its self into park and then powers off immediately and this isn’t safe with my children in the car while drive it’s also happened while driving during traffic car just throws its self into park and powers off
I was traveling at highway speed. The hybrid was utilizing mostly the battery. I needed to accelerate to the speed of those around me. The van switched from just battery to utilizing battery and gas power. About a minute after it switched, the engine completely shut off. A warning flashed up on the dash display to “Stop safely Vehicle will shut off soon”. The engine shut off. I could no longer use the gas, the only thing I could do was coast to a stop. This was very terrifying because I was traveling at such a high speed on the interstate. I immediately put on my hazards and coasted to a stop. This was harmful to others on the interstate because they had to slow down behind me or get around me. Eventually when I was slow enough, I managed to coast onto the shoulder and come to a complete stop. I was able to reproduce the problem upon restarting the vehicle. I have taken the van to a dealership but they have not been able to look into it yet. The engine light is now on in the vehicle. This happens consistently at high speeds
January 2023 the car was at the dealer for two months for the battery heater. The car was losing coolant as the battery heater was malfunctioning. The car was at the dealers for 2 months for the repairs to be completed. December of 2023 the car had “Stop Safely — Vehicle Will Shut Off Soon”. We pulled off the freeway in a confused state and the car shut off. We called Chrysler who said to have the vehicle towed or driven at our own risk to a dealer. But had no input if the vehicle was actually having a problem or just a sensor. The vehicle was towed and now has been at the dealer for 3 months with no remedy. They say a new transmission is on backorder but not approved by Chrysler.
As I was leaving my son's school parking lot - moments away from an on-ramp to a major freeway - an error message flashed on the dash saying "Service Charging System" followed by a message about the vehicle's speed being limited and a red turtle icon. The van instantly was driving strangely. Since I was in a parking lot, I turned everything off and waited for the van to shut down completely. I restarted the van and the warning messages were gone, but the check engine light remained. The van showed more than 15 miles of battery mileage remaining. The week prior, we had taken it into the dealership for the open recalls to be repaired, so we took it back for this issue. The dealership just messaged me saying the van is ready to be picked up and that the charging system was just low. But it still had nearly 1/2 the battery left when this happened! The dealership went on to say, "So here is the deal, we could not verify the concern at this time. There are no codes in the system, nothing to really test for. I can have someone drive it and see if anything happens, but without being able to duplicate your concern, we cannot diagnose it." My sister-in-law had the exact same issue on her own Chrysler pacific Hybrid just a few months ago, while she was on the freeway and her speed was instantly throttled. They replaced the entire electrical hybrid transmission. This is a known issue with this vehicle and it is going to put my life and my kid's life at risk since the dealership cannot reproduce the error. I even sent a picture of the error message, and they say there's nothing they can do.
Without any prior warning or predisposing symptoms, my vehicle lost all power when I was driving in the carpool lane at 70 miles per hour. I had to "coast" across 5 lanes of freeway and come to stop in the emergency lane. When I applied the brakes I had no power to brakes which made the car skid violently. I could have lost control because I was not expecting that secondary issue. The traction conrtol may have been triggered causing the car to overreact. The dashboard lights and "d" on my gear shifter were all flashing. I turned the car off and allowed it to sit for a few moments. I turned the car on again and nothing happened. I turned it off a second time and it turned on, went into drive and I was able to drive the remaining 5 miles home without incicent. I have already completed the recall notice for this very issue back in Jan 2023. However, even though the dealer states they completed my recall work the upbove described incident happened on 1/14/24 nearly a year after their recall completion.
Front park sensor will intermittently go off like it is detecting an object when vehicle is at stopped, but there's no object, went to Chrysler Dealership for warranty 7 Times, Last time dealer service confirm and recreate the problem but can not replace because NO FAIL CODE as me open case with Chrysler. When park sensor fault read when I was at stop I need to get out of the car to inspect to make sure no object is in the front, this is very UNSAFE sometime I have get out of the car in the middle on the road.
While driving early in morning on local highway (70mph speed limit), car flashed "Service Charging System" shut off the engine and power to the vehicle including acceleration, and check engline light illuminated. Thankfully early hours made it possible to drift to side of road to put car in park. Engine light illuminated dash and dash flashed "shift to P to start." Car would not shut off after pressing the start button nor depressing the break either (hazards were engaged at this point). Hazards had to be shut off in order to completely shut off the vehicle to restart it. Dangerous on major highway with no lights or hazards on side of road. 25 degrees outside. This is the Second time this has occurred to this vehicle. The last time occurred on 1/20/2023 the exact same way shutting off power and engine of the car on the same local highway with similar cold temperature (always issues with this car in the cold, hybrid battery system fails in cold below 40 or temperatures higher than 90). Last time a grounding issue was found in the transmission and was replaced. I will be contacting the dealership for repair today will update if it is still grounding issues or something new causing same problem. As i made in my last nhtsa report in january on this vehicle, the fact this car shuts down while in motion and you have no control over the car is dangerous and can cost people their lives. As a hybrid system, the van should automatically switch to gas only and bypass the hybrid charging system to continue functioning as any car would. By shutting down the engine and everything you are jeopardizing the lives of everyone on the roads. Both incidents of mine occurred on a highway at high speeds. I am just thankful I was not on a bridge at the time it failed as it would have caused an accident. I heard nothing about or from my last nhtsa report. Please for the safety of everyone look into these hybrids. Look into this car history and you'll see the constant repairs it has had.
My new 2021 Chrysler Pacifica Hybrid Minivan was repaired under the 03a recall on 3/2023. It had another loss of motive power, the same issue that was supposed to be fixed under the 03a recall. The car warned me that it would shutoff soon in the visual display and to move to a safe area. It did shut down immediately. I attempted to restart it several times. After many times it did start and the transmission light remained illuminated. I was driving on the highway at the time and blocking traffic. It's unsafe for the car to shut off randomly. This is unacceptable and unsafe. I paid for a brand new, safe, functioning car. This is a serious problem and is dangerous for my family. I feel unsafe driving a car that will shut off at random times. This has already been repaired once and remains a problem. I need a solution.
While driving 70 mph on the highway with my three children in the car, my engine suddenly and without warning, shut off. I had to coast over to the shoulder and wait for help. The dash displayed the warning "service charging system" and would not let me put the car in drive. I was VERY lucky that there weren't any cars around me when my acceleration suddenly stopped so I was able to get over on the highway. The car stalled in an unsafe place and left my children and me at risk of being hit from behind while waiting for help. I got the car to the dealer and they have informed me that this was caused by total transmission failure. This was a HIGHLY unsafe situation and it could have ended much worse for my family, and those in vehicles around us!
Chrysler recall 03A is installed on my pacifica hybrid. Since then, the past two times the van has been driven at highway speed, it has suddenly notified me that vehicle shutdown is imminenent and put the vehicle into limp mode. In both cases, turning the car off and on again allowed normal operation, with the check engine light locked in. The first time the code was P0A43, to be determined what the code is for this most recent event.
Was driving at 72-74mph on Dallas North Tollway on Oct 6th. Vehicle is a Chrysler Pacifica Hybrid 2021 model - Limited Plug in Hybrid trim. Battery charge was 2% (less than 1 mile range) Fuel was 95% showing 395 miles of range left After the battery depletes, the vehicle is supposed to switch over into ICE mode (Gas engine kicks on) Car was running only on Electric power and when the battery hit 0%, the car stalled on the highway and showed multiple warning lights Service Hybrid Electric System Charge system not available EBD2 Engine Malfuction Transmission Temperature Automatic Transmission Fluid overheating Check Engine light Car still had power (power steering and brakes working) but lost all propulsion suddenly Luckily i was able to safely pull over on the shoulder Turned car off and on and still had same warning lights and car's engine would not start Finally waited 15 minutes, disconnected battery (12v) and tried it and now engine started Came home and charged battery to 100% (showed 32 miles of range) but now car will never run on Electric motors - Only via Gas engine This is a serious safety issue - I had the same exact problem on my 2018 Pacifica Plugin Hybrid which was diagnosed as a HCM (Hybrid Control Module) issue. It was the subject of a recall. This is the 2021 model with less than 25,000 miles and its still a major safety concern. There have been several incidents that have to do with the electrical components site, such as the adaptive cruise control not working the lanes assist going off, stating that my hands are on the wheel. When my hands are on the wheel, there have been problems when I go to put it in drive. It'll switch back to park and throw on my parking brake. It has also the auto breaking lights have. Come on, my checken and light has come on. It taking in to the dealership and they found corrosion on some of the wires, replaced them. They also replaced the adaptive adaptive control module. It was supposed to magically fix all my electrical problems. Yet I'm still having electrical problems also my navigation on. A scream down by the cluster cluster. Stays on and doesn't go anywhere. I also have video and pictures and things like that of the situations that have gone on with my Van 4 months and nothing has been fixed. I've been told by the dealer and buy a representative from Chrysler. That's my Van is fine. There's nothing wrong with that. But clearly there isn't an electrical issue and. And I pretty God. Nothing happens to me or my family or someone else because of it. Also, the radio, when you shut your car off and open the doors to exit your vehicle, the radio is still left playing and should not also the auto break in just comes on. For no reason. It happened on the interstwhen we were merging in the traffic. And there's no reason it should be doing some of the things that it's doing.
This incidet happened on August 30, 10:30am PST. 2021 Chrysler Pacifica Plugin Hybrid vehicle while parked in the garage catches a fire due to Plugin Battery. The car was not in charging when incident happened. The car was last used previous dat afternoon around afternoon for a short trip of couple of miles only, and then parked inside the garage. At 9 am in the next day morning I was in the garage, and no smoke identified. At around 10:15am we noticed burning smell inside the house, and when we checked entire house inside and then garage, found garage was filled with smoke, and smoke is coming out of Chrysler pacifica vehicle. We immediately called 911, and fire department came immediately with in couple of minutes and started working on controlling the smoke and fire inside the car. Fire fighters noticed smoke is coming out of plugin battery, and tried to put out the smoke, they were continuously running water entire day, and could not stop the smoke/fire coming out of the battery. If they stop the water for few minutes, smoke starts again. They have been working on the car to put of smoke entire day, even called chrysler to understand on how to control the smoke out of the battery, no luck, At around 8pm, they have setup 2 hoses running water from the home to the car continuously, and check the car often to make sure it is under control. I will be checking all the night to make sure it won't caught the fire and cause additional damage to the house. My garage is filled with smoke, and my entire house is also had smoke. It is a nightmare situation for me and my family to experience this, I want to make sure every other family purchased 2021 chrysler pacifica plugin vehicle are safe.If we were not home or sleeping in the night, it could have been a much worse situation of losing the family and entire house due to the fire.
The most concerning and potentially dangerous issue I’m continuing to have with this vehicle is a random, complete shutdown of all engine/battery power. So far this has only happened while driving at a consistent high-speed on a freeway. A warning comes on the center display that says “Stop Safely. Vehicle Will Shut Off Soon.” There is still power steering and hazard lights, but ZERO motor power, neither gas nor electric. I am forced to glide to a stop. If I turn off the ignition, open the door to reset everything, close the door, let the vehicle sit for a minute, when I restart it (so far anyway) everything is back to normal - with the exception of a “Check Engine” light on the display. This first occurred in April of 2023. I subsequently learned there was a recall on the vehicle for this exact problem. The vehicle was taken to LAX Chrysler Dodge Jeep Ram where the recall was done, which required a software update. For the next two months there were no issues with this problem. It happened again on June 21st while driving from California to Minnesota. It shut down 5 times. After sitting overnight, it did not do it again and I was able to make it to MN. It was taken to Ryan Chrysler Jeep Dodge Ram in Buffalo, MN where the service technician said they were unable to duplicate the problem. Nothing they could do. Sent it home with me. The vehicle shut down again on August 12th. Then 10 more times while driving across Kansas on August 20th. You can imagine how harrowing it trying to safely pull to side of the road with dozens of other cars and trucks speeding past, and in one case serving to avoid colliding with me at the last moment. No longer willing to risk an accident, I was forced to ditch the vehicle at a dealership in Denver where it currently resides and awaiting a fix. So far no one at any Chrysler dealership has acknowledged the seriousness of this problem. I have documented most shutdowns on video, but am unable to send.
The contact owns a 2021 Chrysler Pacifica Hybrid. The contact stated that while driving approximately 65 MPH the vehicle suddenly shut off. The vehicle was taken to the local dealer who performed a software update under the NHTSA recall campaign 23V010000(Electrical System, Hybrid Propulsion System) but the failure continued to occur. The vehicle was taken back to the dealer who diagnosed that the transmission needed to be replaced. The vehicle was not yet repaired. The manufacturer was notified of the failure. The failure mileage was 38,000.

Vehicle thinks it is charging mode when it is not, sitting in a parking lot. Vehicle when attempting to start says 'Service Charging System' and won't go into drive or even neutral appropriately. Vehicle is stranded otherwise. Under 40,000 miles on vehicle. Online this appears to be a very common problem that Chrysler is yet to adequately address with a recall.
Im not sure if it’s electrical or what but we lock up car and everything is closed and in the morning we come out and the door is open and it’s the back sliding door not sure what to do about it or if it’s broken but having my van completely locked up and then coming out and the door being open is weird and it’s happened multiple times as well as both sliding doors slam shut and don’t stop closing once u click button and gets hands and fingers shut in door when is shouldn’t shut when u put on it or block it
Recalls 23V-010 and FCA campaign Z76 was completed on 7/11/23. On 7/13/23 check engine light and turtle warning light (limp mode) was illuminated and vehicle will either not move at all or move only 5mph. This happened without warning and could have left me in a dangerous location in lanes of traffic. Vehicle has only 25,000 miles and is under OEM warranty.
The 2021 Chrysler Pacifica Hybrid will lose all power while driving. I have had it back to the Dealer for this Problem. I got a Recall for this in April. I took it in to the Dealer to do. They told me that Chrysler does not have a fix for this yet. Within 400 miles after that it done it again while my wife was driving and caused a crash.
I was driving my child to daycare on my way to work one morning. I pulled into an intersection and my vehicle just stopped as if the engine had died with no warning signs, sounds or even symbols on the dash. I stepped on the gas and couldn’t get it to move all while I had cars coming at me from both sides. I tried turning it on and off nothing would get it to move, eventually the check engine light and another warning came up on the screen saying to service charging system. During all of this I was able to get my child to the side of the road out of harms way incase my vehicle were to be hit and luckily one other driver had noticed what happened and started waving at the oncoming vehicles to come to a stop. We had to have it towed to a dealership were it is currently being looked at and they are still unable to resolve the issue. I am now terrified to drive this car and am very nervous about intersections, this is a very serious problem and am just thankful that others around me were aware and able to help keep my child and myself safe.
The charging system is shutting down my vehicle while driving in high speed heavy traffic. I was traveling with my grandchildren and my dog and my vehicle stopped in 90+ degree weather in the middle of no where. Once it ran again I got it to a charging station so I could continue my travels. The check engine light went away and we were traveling home when the charging system shut us down again doing 65 mph on the interstate about 30 miles from any town with no cell service. My vehicle had to be towed from the 30 miles to the closest town and then towed again from the closest town to the closest Chrysler dealership. They can't even get to my vehicle for over 3 weeks and I am out thousands of dollars in rental car fees. I even took my car to the Chrysler dealer before my trip because the check engine light came on and they ran a diagnostics that said my front camera wasn't working. No a single thing about the problems I was about to encounter for about the 5th time!! This happened last year also and I was without a working vehicle for almost 2 1/2 months. This has been an ongoing problem that started about 5 months after I purchased my vehicle. I don't feel safe on the roads because the vehicle just STOPS!! This a such a lemon of a car and the Chrysler dealer knew about these problems before they sold me my vehicle.
I stopped my 2021 Chrysler Pacifica, placed the transmission knob into park, opened the driver's door, leaving it opened with the engine running, and walked to the rear of the vehicle. I opened the tail gate and removed some boxes, threw the boxes into a dumpster, which was approximately 20 feet away and forward of the vehicle. I then returned to get two additional boxes from the rear of my vehicle. I retrieved the boxes and as I walked from the rear of the vehicle, it started to back up. I dropped the boxes, jumped out of the vehicle's path and tried to get into the driver's seat to stop the vehicle, but was unsuccessful as the vehicle picked up speed and the driver's door caught me and I fell. My vehicle was stopped after hitting a parked car in our church lot. Just a scratch on my vehicle's rear bumper, but frontend damage to the compact car it hit. No one else was in my vehicle. I then drove to get urgent care from cuts and bruises. (FYI - My insurance covers 100% of my care). I have aa service appointment to have the vehicle checked out tomorrow morning at San Antonio CDJ.
Was driving when vehicle produced an error message that said it was powering down. Coasted on interstate to a stop. Was not able to restart vehicle. Appears to be a common issue with this model, extremely dangerous.
On 3/27/2023, while driving 75mph down the PA turnpike, our 2021 Chrysler Pacifica Hybrid lost power unexpectedly. Fortunately, my wife and kids were not in the car, as it was frightening to have the engine shutoff while driving in the left lane of a narrow highway that had a small shoulder. I was in the left lane of a 2 lane highway, I placed my turn signal on to try and coast into the right lane and then over to the right shoulder. I still had power steering, but the engine had entirely shut off. When I got onto the small shoulder, I placed my 4-way flashers on. I read the digital read-out on my gauge cluster, it said "Service Charging System" and the check engine light came on. I wasn't sure what this meant so I Googled it. It turned out that a lot of Pacifica Hybrid owners had been having this exact problem. I was able to start the van and slowly drive it the 60 miles back to my house. I called the Chrysler dealership to setup an appointment to have the van inspected. It took them a week to diagnose that it was the van's hybrid heater that was locking prematurely. The part order was a 3-4 week wait. After a month went by and no word from the dealer, I called and was told it would be another week before the part came in. April 19th was when the dealership was able to replace the hybrid heater on my Pacifica. I went out of town for work on April 21st, and returned today, the 24th. The Pacifica hadn't been used since leaving the dealership. The dealership technician also told me the problem I had was due to a software update. Today, the 24th, I took the van out and was driving to pick my kids up when the van digital display said "Stop Safely, Vehicle Will Shut Off Soon". The check engine light came back on along with many other display cautions. I pulled off the side of the busy highway, turned the car off and again, Google'd this new problem. After attempting to restart the van numerous times, I was eventually able to start it and again, limp it back home.
Description: My 3 year old daughter was getting out of the rear of the vehicle on the drivers side. The door is automatic and closes via button in the passenger compartment or the remote keyfob. She pushed the button to close the door and the door closed on her wrist and hand, the door did not automatically reverse and the door latched with her hand still inside. She was injured and needed to go to the hospital. This issue has also happened to several other people as well. There is a Facebook page dedicated to complaints on this Van. The door has closed on adults and children both. There were no warning lights, the vehicle has had a recall done on the auto open sensors under the doors. This issue occurred on April 11th 2023. The manufacturer was contacted and they have opened a case. The case number is 85938082
Currently, there is a recall of this vehicle that results in a loss of automotive power. This only occurs when driving in the highway going 70mph. This first occurred to us about a month ago. The recall was announced a little earlier to when we were experiencing the issue, but there was no fix available until about a couple weeks after we had this issue. The software fix was completed last Friday. The following day, during a 20 mile drive, the car again loss automotive power twice. The only difference after the recall is that the message on the screen changes as the car starts to shut down. As this only occurs during highway speeds, we were required to change multiple lanes across speeding traffic while the vehicle was slowing down, in order to get to the emergency lane. And this was occurring while the car was slowing down on its when because of the lost of power. After this incident, we drove slower at about 50+ mph, thinking we can avoid the sudden loss of power, but unfortunately it occurred again. Thankfully in both incidents, there were no crashes. Turning off the car completely for about 30 seconds would allow the vehicle to run normal again. The vehicle is back to the dealership and after a week, diagnostics haven't been done yet. Despite reporting the danger of this recall above, the dealership is offering to have us take it home over the weekend. This car is simply too dangerous to drive in highways until there is a real fix.
when slowing to make a turn the vehicle turned off and engaged the parking brake on its own. I had to disengage the parking brake before starting the vehicle again. I did not have a problem after that but I worry about it happening again. There was no warning to indicate anything was wrong beforehand and no indication of an issue after restarting the car.
I have tried to get a remedy for the major recall and Chrysler is being unresponsive. I can’t let my young family use the car until this is fixed and Chrysler won’t help so what do I do?
Traveling at 60mph up a large hill/mtn in passing lane and car went into limp mode or lost power from engine. Was unable to push gas for power and had to maneuver through 60mph traffic to shoulder by the time I was able to make it to the shoulder I was not able to get all the way off of road because of no more momentum. I was unable to turn off vehicle for quite some time until I used key fob and remote state option on key fob which then allowed me to turn off vehicle. Losing all power at 60 mph while passing was terrifying and could have caused serious bodily harm.
While driving the vehicle in traffic, it shuddered and turned off without warning. After several attempts to restart it, I had it towed to the Chrysler dealership where we purchased it less than two years ago so it could be repaired. While waiting for the tow truck, I looked online and learned there had been a recall.
Issue 1: Infotainment display failed or malfunctioned. Would not power on, turned off during travel. Pulled over to power the vehicle off and back on to try to remedy the situation but that did not work. Continued driving and after a few minutes the infotainment center powered back on. We were not put at risk during this experience, but it was inconvenient and not happened again. Issue 2: Remote started vehicle so it could warm up. After a few minutes I got into the vehicle and began to pull out of the driveway when the vehicle just powered down. I'd assumed it has something to do with remote starting the vehicle.... Until I saw Manufacturer Recall A03 NHTSA Recall 23V-010. Our family now feels very unsafe driving this vehicle knowing that this particular recall may have occurred while we were pulling out of our driveway. This incident occured approximately one week ago and has not happened since, so I did not take it in to the dealership to look into, but perhaps I should. This vehicle has not been inspected by the manufacturer, police, insurance representatives or others. There were no warning lamps, messages or other symptoms of the problem prior to the failure. Incident 3: Blind spot notification light on driver's sideview mirror has failed once resulting in swerving to avoid an accident.
The vehicle constantly stalls out when we driving on the freeway. The vehicle loss of the power on the freeway put me and my family's life in very dangerous situation when we drove the vehicle. Brough my vehicle to the dealership, and it verified the problem with engine light error code P0A43-00. The dealer stated that they are not able to fix it due to the issue was related to a active recall 03A. However, the remedy was not available.
I have a 2021 hybrid Pacifica purchased in May 2021. I have been experiencing loss of engine power for months. I brought it to service in January, they said they fixed a recall and changed the oil, said I was good to go. The problem started happening more frequently after that, and on 2/7 my vehicle lost power 7 times while driving roughly 20 miles. These instances occurred at highway speeds with immediate loss of drivability requiring me to coast to the shoulder. I brought it to service on 2/8. On my drive to the dealership the car shut off 5 times. After the tech assessed my vehicle, they replaced the electric coolant heater and said although there is no fix available for the recall yet, the battery is affected by the coolant heater and so it should be 100% good to go now. I made it safely home on 2/9. While I was driving to work the next day (2/10) the car shutoff 3 times on a busy interstate during rush hour before I was able to find a place to safely abandon it.
On December 7, 2022 I brought my Pacifica into the dealership because the day prior, my vehicle just shut off unexpectedly while driving. I was unable to get it to turn back on it just gave me a “service charging system” message on the dash. I had my vehicle towed home and it wasn’t until the following day 12/7/22 that I was able to finally start it back up. I drove straight to my local dealer and dropped it off. After a few weeks they told me they were going to replace the hybrid battery as that was the issue. On 1/20/23 I received a message from the dealer stating they had my vehicle back together and running and we’re going to fix a recall issue. On 1/26/23 I received another message stating my vehicle would no longer start and they didn’t know why. It has been 2 months now that they’ve had my Pacifica and as of Monday 2/6/23 they are unable to diagnose the issue
11/14/2022: WHILE DRIVING at 75mph, a battery icon appeared in the hybrid info system on left of dash. Immediately after the engine icon appeared, a message that said "shift to P to Start" appeared in the the dash info. The letter "D" along with a red icon of a charging cord also appeared to the bottom left of the electric range readout. ALL POWER TO THE VEHICLE CUT OUT WHILE MOVING AT 75mph simultaneous to these images/messages appearing. Following the above, an image of a Pacifica hybrid appeared in the middle of the dash info system. The Pacifica image included a red plugged in charging cord and the message "service charging system". Vehicle was pulled to the shoulder and put in park. Attempted to restart vehicle unsuccessfully for 15 minutes. After vehicle started, it was driven to the closest dealership that had a qualified PHEV tech, Frontier Dodge Chrysler Jeep in Lubbock TX. The error codes wouldn't repeat at the dealer. Dealer informed that, in speaking with Chrysler a part remedy had been determined but would require lead time to obtain part. On 12-08-22, the dealership informed us the repair had been made and vehicle was ready for pickup. The code found and repaired was POE15 and electric heater lock up. At pickup, the vehicle had not been fully charged. When driven out of service, the echarge was 24% and it wasn't depleting during operation. This was abnormal. Drove the vehicle from Lubbock to our residence in Taos, NM. Part of the drive includes a canyon, with two lanes, blind curves and zero shoulder. If the vehicle were to lose power in the canyon due to this issue, there is a HIGH PROBABILITY OF CRASH, INJURY, and/or DEATH. The vehicle repeated the issue on 12/12/22, 12/30/22 & at least two more times that are undocumented. Vehicle was towed from Taos to Larry H Miller CJDR in Albuquerque on 1/11/23. We were notified of recall while at dealership. My family's and my lives are at risk due to this recall issue.
The first incident, the vehicle lost power driving down the freeway at 75 miles per hour. That was only at 1,000 miles. Very scary!! Since then, I have had numerous electrical issues- too many to count. There has been front & side sensor collision warnings when nothing is even next to my car. The navigation has not worked the majority of time that I have owned the car. travel for work so I therefore have to look at my phone for directions- not ideal at all for safety. I had had a few instances of an extremely loud humming sound from the vehicle of unknown origin. I have had the control screen frozen & could not turn on heat in winter. I have most of these occurrences on video with my phone. I had sent a certified letter requesting arbitration. I have had no response from Chrysler. I had the car in 7 times to the dealership. They told me they cant fix it & dont waste my time returning until there is recall. I have these incidents in movie or video form. Your form will not allow me to upload these videos- only still pictures.
While driving down a 4 lane major highway where the posted speed limit is 70 mph, the vehicle suddenly flashed "Service Charging System" with the check engine icon came on and the propulsion on the vehicle ceased funtioning. The D for drive was flashing as well. As a hybrid vehicle, the car should transition to gas without incident, but it doesn't. Instead it cut power to the engine leaving me, the driver, scrambling to find a safe location to get off the road, which is not always feasible or safe, endangering everyone in the vehicle and on the road if others are not paying attention to the sudden loss of power and propulsion of the vehicle in front of them. Thankfully, I was able to safely get off the road as a result of some helpful semi's, but not everyone may be in such a situation. Should this happen to someone else who regularly drives across a bridge, for instance, could possibly cause an accident by being unable to pull off at a designated area. I was able to engage the gas engine by turning it off and back on after stopping, but it did not store the "service charging system" error. This was determined when dropping it off at the Chrysler dealership. There are a few stored error codes including a "low voltage differential signal" which also occurred last year when they said they replaced a part in relation to the heating/cooling battery fault on the high-voltage batteries (less than 6 months old at the time) and similar to the recall on the older models. Making this the second time in the shop for a battery issue. Should a connection between the battery faults and the Body Control Module or PCM (not sure what that one stands for), they were parts also replaced within the first month of the vehicle. The vehicle is currently at the dealership for review. There were no other warnings or messages prior to the failure. Pictures of the error as I saw them after safely stopping are provided. Never have I had a car just stop functioning and this is entremely unsafe.
Multiple occurrences have happened where I am driving the car on the freeway when the SERVICE CHARGING SYSTEM code comes on and the vehicle stalls. I am unable to accelerate and have to pull off onto the shoulder. This is very dangerous as the vehicles on the freeway are going at high speeds. This failure could easily cause an accident or worse. The car then flashes SHIFT TO P TO START and the check engine light comes on. The vehicle is currently at the Chrysler dealership to be diagnosed.
I have a 2021 Pacifica Hybrid in (Vin: 2C4RC1S70MR51943) that has encountered the dreaded "Service Charging System" message 5 times now in the past year, with the past 3 occurrences all happening in the last 3 weeks. Each time the car will not start, and the gear shifter will flash, the red charging light will illuminate on the dash, and the check engine light is on. On one occurrence, I turned the car off, then on again and was able to drive, however the check engine light was illuminated. I ran an OBD scan and the code was P1A21. On another instance, I was driving and lost all propulsion, the screen indicated "Service Charging System" message. Luckily I was on a local road and not a freeway and I was able to pull into the bike lane, still obstructing 1/2 of the right lane. After 5 minutes of trying to restart the car, I was able to move the car and I drove it to Earnhardt CJDR in Gilbert Arizona. When I got there, the check engine light had disappeared. I dropped off the car and to my disbelief the dealership told me there is nothing they can do if they can not replicate the issue. I am not stuck with a van my wife is afraid to drive due to the imminent threat of the car turning off while driving. I have 2 young children and I do not feel the van is safe in its current condition. Yet, the dealer is unable to resolve/replicate the issue. I have videos displaying the issue along with a screenshot of the OBD error code. It appears the dealership does not care about the safety of my wife and children by not being able to fix this issue. I have done a significant amount of research online and have found this to be a relatively common issue. In my opinion this vehicle is not safe to drive due to the risk of a complete shutdown and I commute on the freeway to and from work and the last thing I need is to have the van disabled on a busy interstate.
My 2021 Pacifica Hybrid with 19.2K miles threw a "Service Charging System" and Check Engine Light error while I was driving at 65mph on the highway on Christmas Eve in 8 degree weather. The minivan stopped accelerating and the transmission began flashing between P and D. We were forced to stop the vehicle on the highway shoulder, leaving us (2 parents, 2 kids 7 & 11) stranded on the highway. The vehicle then shut off and we were unable to turn on heating or accessories other than headlights and hazards. We were rescued by state troopers given that no tow trucks were available on Christmas Eve. The local dealer claims that the issue was related to the battery, but that doesn't make sense given the car was driving at highway speeds at the time and the alternator should keep the battery charged. I have seen a lot of owners with Service Charging System errors on this forum and believe Chrysler should have some knowledge as to a permanent solution. Without better understanding the root cause of the issue, it's hard for us to feel safe with a battery replacement, since that seems to be address symptom given that a car running highway speeds would keep the batteries charged through the belt and alternator system. My wife is fairly terrified of driving this car at highway speeds without some assurances it won't shut down and I get the sense that the dealer wants to wash its hands of this situation. I have a Chrysler warranty number for this repair, but have not received assistance yet. Would really appreciate some help!
There are series of incidents that happened with the vehicle on freeway and mostly all the time it was the same issue of engine failure . Me and my spouse - The engine failed in the middle of road [ freeway ] making me to pull on to the right lane showcasing the indicator "Engine failure " and Charger system malfunction. - Recovery : Stop the car , turn off all the engine and open the door and close, then turn on the engine - it works. but this is on freeway and i am worried of my family driving the car. - Once i took the vehicle to service center and they analyzed and said - nothing they could see which was wrong as the indicator goes away after some time surprisingly - Happened 4 times so far on free way , first time cop had to push the car out and later i realized the steps for recovery - There are warning lamps on the panel - " engine failure "
While traveling on the highway (>70mph), my Chrysler Pacifica Hybrid (~44k miles) suddenly lost power and the "service charging system" error appeared along with the instructions to "shift to park". The check engine light began to flash as well. Fortunately, I was able to cross three lanes of traffic in a van with an unresponsive accelerator to get to the shoulder. Once safely on the shoulder, I still could not shift to park and I could not turn the car off either. After sitting for 25 minutes or so, I was able to shift to park and turn the car off and then on again. I immediately took the car to the dealership (using surface roads). They kept the van for 10 days and diagnosed a software issue. I got the van back and for about 3 days it was fine. Then we were driving the same freeway (at speeds of 70mph or more) and the exact breakdown happened again. Chrysler has had the van for 4 weeks now and just informed me that the issues is metal shavings in the transmission; they will be replacing the transmission. This is related to the ODI investigation # PE 22-008
Second occurrence of 2021 Pacifica Hybrid stalling while driving. Same error messages with check engine light, Service Charging System, battery light. The recommended software update was done a month ago at a dealership after the first incident. The van worked fine until today when I had to unexpectedly maneuver to the roadside again as it stalled out at 35 mph. I waited several minutes and the van started again but the yellow engine light stayed on. I took it to the dealer where I bought it and it's there now. Yes, my safety was at risk again.
On Sunday, 12/4/2022, I went on a few errands not intending to drive very far on the highway since most of my stops were local. I entered the I-94 west bound from Moorland Rd in Brookfield WI and drove about 1 mile reaching about 65 mph during the busy traffic hours. The car suddenly stalled and I lost all ability to accelerate the vehicle. The brakes and steering wheel still worked fine. The dash was displaying a message: "Service charging system. Shift to P to start." Thankfully, I was in the right lane and the shoulder was clear. I was able to pull onto the shoulder and come to a controlled stop. I put the vehicle in Stop but when I pressed the off button, it would not switch to off and remained in the "run" mode. I tried several times for a few minutes without any success. I waited a minute and then tried again. This time, the vehicle was able to switch to "OFF" mode. I was able to start the car normally without any "System charging system" error coming on. I decided to return home immediately using local roads only. I no longer trust my vehicle on the highway - even if it is only for a short distance on the highway.
11/28/2022 - Infotainment system glitches with random disconnects from Apple iPhone connected via CarPlay followed by vehicle "shutting down" at highway speeds. Red battery light, Red charge cord light and Image "Service Charging System" After repeated attempt was able to reboot vehicle and drive to nearest dealer. Dealer pulled codes and advised to return to selling/servicing dealer. 12/2/2022 - Drove to selling/servicing dealer. They read codes and attempted to recreate the power system failure. Indicated they could not. Exited dealer's lot only to have same incident occur as on 11/28/22. Unable to reboot/restart vehicle. Had it towed back to selling/servicing dealer. Photos of repair ticket and technician notes available for subsequent submission but not readily available to include with this initial complaint.
On Wed, 11/23/2022, I was driving on I-94 Eastbound headed from Wisconsin to Illinois at about 70 mph. The car suddenly stalled and I lost all ability to accelerate the vehicle. The brakes and steering wheel still worked fine. The dash was displaying a message: "Service charging system. Shift to P to start." Thankfully, I was in the right lane and the shoulder was clear. I was able to pull onto the shoulder and come to a controlled stop. I put the vehicle in PARK and shut off the vehicle. I tried to restart it but was unable to. I then unbuckled my seat belt and opened & closed the driver side door (without getting out). After about a minute I was able to restart the car and continue on my way to Chicago. This happened 2 more times that trip. It also happened 1 time on the way back to Wisconsin for a total of 4 incidents in 1 day trip. This has significant safety implications, esp. when one cannot get to the shoulder (or there is no shoulder) to restart the vehicle. I felt very scared and no longer trust my vehicle on the highway. This has happened to me once before earlier this year. I took it to a dealer after that incident. They said the updated all the software but are not certain if this fixed the root cause. Apparently, it did not. There was no warning light to indicate a problem may be happening. However, now that this has happened, the "check engine" indicator is on and, apparently, needs to be reset. I have not yet taken the vehicle to a dealer for this recent set of 4 incidents.
The contact owns a 2021 Chrysler Pacifica Hybrid. The contact stated that the center console and the dashboard made a loud buzzing sound before the functions became inoperable. The contact stated that the display screen displayed service warnings and software updates erroneously before disappearing. The contact took the vehicle to the dealer who was unable to duplicate the failure. The vehicle had not been repaired. The failure persisted with the center console flickering before becoming inoperable. The failure recurred intermittently but more persistently. The contact returned the vehicle to the dealer who flashed the software and replaced the hybrid water pump; however, the failure recurred. The manufacturer was made aware of the failure and opened a case. The approximate failure mileage was 12,000.

Van stalled while driving at 45 mph, had to pull over to side of road and get a tow to dealer. No indication of a problem before the stall. Warning lamp showed engine light, charging system warning, shift to park to start but unable to restart van on side of road. High safety risk as this occurred while driving at night. Police eventually parked behind me for safety on the roadside. Van is currently at dealership after being towed there.
My 2021 Pacifica PHEV has 16,200 miles on it at time of incident on 10/31/2022. I had just pulled off the highway to get gas when the issue began. I received a “service electrical system” on the dash and a check engine light and I had to turn off and back on to get it to restart. The check engine light stayed on and upon continuing my drive home another warning light came on telling me that the emergency braking was no longer operating. The warning light stayed on for a few miles and then went off. I took snapshots I will include. I also noticed that the vehicle was recording erroneous data such as suddenly having an electrical charge of a large percent when I had zero electrical charge. I got the vehicle to the dealership for repair. On 11/1. No loaner or rental reimbursement was available from Chrysler so I was without a Vehicle during repair. I received the vehicle back on 11/3/22 and I was informed that the vehicle had error codes POE 15-50 Hybrid/EV battery pack coolant heater A control and P1649-00 (? Not sure I wrote the number correctly) HCP Int… code. The paper also showed POE 15-00 HVAC Blocker Power but he only mentioned the first 2. The service technician indicated that the vehicle had a Z76 recall (I was not aware of a recall) and that they had done a software update to fix it and the vehicle should note be fixed. Upon reading other online complaints I see multiple reports of the same thing happening repeatedly to other 2021 phev Pacifica’s with low mileage while driving at high speeds including within 10 miles of having the same repair done. I am now scared to drive this vehicle on the highway after reading of multiple other 2021 PHEVs no longer operating on multiple vehicles while at highway speeds, despite multiple attempts at repair. I am afraid for my safety and for anyone else in my vehicle and I I want Chrysler to pay off my note immediately and take back possession of the vehicle. I no longer feel safe driving it.
while driving the car down the high way at normal highway speeds (70mph) the car lost all propulsion. Shortly before the malfunction the electric motor started growling. A message on the screen appeared "service charging system" and " put car in Park". I was in the far left lane when this happened and traffic was low as it was early in the day (7 am ) I managed to move the car from the left lane through all the other lanes and stop on the side of the highway. I was not able to drive the car and called for tow. While waiting for the tow the car became responsive after about 15 min. I recalled the tow and drove to dealer. The dealer scanned for codes and found a code for "Motor A position error". This is the second time this has happened to this car - previous time the dealer "upgraded" firmware and said it should be fine. Hardware issues cannot be solved by software. On Saturday 9.3.22, while driving at approximately 40mph, my Chrysler Pacifica lost all power and slowed to a stop (I turned on my hazards immediately and pulled over to the shoulder). Based on the light traffic around me I was able to safety move accross two lanes even without the aid of power steering. Within 60 seconds of me coming to a stop, the Check engine light came on the cluster and an error message saying “charging system fault”. After calling for Chrysler roadside assistance, I attempted to restart the vehicle dozens of times unsuccessfully. At approx 60min after the vehicle lost power, it restarted. On Tuesday, 9.5.22 I took the vehicle to the dealer to diagnose & repair. As of today (9.12.22) they have been unable to recreate the event. Even though the error threw a CIL code, the dealer stated the error is now gone and no codes were stored. They have filed a “STAR” case with the manufacturer to determine next steps. I have refused to accept the vehicle back until the problem itself has been diagnosed and a repair is made.
I was on the highway, and my car suddenly lost power. I was unable to accelerate, and I had to pull over on the shoulder. I received an alert that said “service charging system error.” It was extremely dangerous for myself and my family.
Was driving the vehicle when it completely died. The “service hybrid electric vehicle system” warning came on the dash and could not get it to go into drive again. Completely dead in the water and could not move. I can’t imagine if I was going down the freeway and this happened. Very lucky to have it on a quiet road. This is a known issue and there are many cases of this happening to others if you check YouTube out or google it. It’s very scary and someone is gonna end up hurt or killed.
While driving on the freeway on the morning of June 18th, 2022 from Southern California to Las Vegas at 70-75mph, the car suddenly lost power. It displayed the error "Service Charging System" and "Shift P to to Start". This was incredibly dangerous as acceleration no longer worked and the car was slowly coasting to a stop. I was able to pull over to the side, turn off the car, and restart, but the check engine light persisted. After returning from Vegas, I brought the van to my local Chrysler dealership. They ran some flash updates and returned the car to me. The same issue then re-occurred on July 4th while traveling 65-70mph on the freeway. I brought it back to the dealer the next day, and it has been there since. After a lot of back and forth with Chrysler hybrid technicians, the dealership was authorized to replace the onboard charging module 1-2 weeks ago. They said the car was still throwing a charging error after that, and they are now looking into what else could be the issue, which they suspect might be the PIM (power inverter module). They are currently working with Chrysler hybrid techs again to diagnose as they are not able to order any parts for replacement without that approval. As of yesterday, the car has been at the dealership for 2 months now, well beyond lemon law timelines per California guidelines.

The vehicle had not been repaired. The failure reoccurred several weeks later. The contact returned the vehicle to the dealer and was informed that the manufacturer suggested that the transmission should be replaced. The vehicle was not repaired. The approximate failure mileage was 24,000.

I was driving down the road and the car just turned off. It will not start anymore.
I was driving 70 miles an hour, in the left lane ,in heavy traffic on I -75 when I suddenly lost all power. A warning flashed on the screen saying to service charging system. I barely avoided a collision trying to get off the highway. I could has easily collided with another car since I had no power and I had to cross lanes of traffic in order to get off the road.
Driving on the interstate, the vehicle simply shut down and gave the following error message: "Service Charging System." I was able to pull to the shoulder and get the vehicle started again. There had been no prior warning messages or symptoms of the problem. I took the vehicle in for service, and the dealership ran a diagnostic check and found no issues. A couple weeks later, the same thing happened. I was again able to get to the shoulder without incident. Took the vehicle to a different dealership where they identified the issue as a bad Power Inverter Module. The vehicle is still at the dealership (it has been a full week). While I was able to navigate the situation without injury. This could have been a very dangerous situation in the instance where a shoulder was not available or traffic was significantly heavier. It appears this is a fairly common problem for some of the new Chrysler Pacifica Hybrids, which brings to question why there has not been a recall issues.
Driving as usual and without warning, no check engine light, the van stalled. It lost all power in the middle of the road and would not start back up. Pushed out of the road & waited on a tow truck. 2021 with 14,000 miles. Dealership pulled the code poa43.
The vehicle is a PHEV. The system that malfunctioned is related to the vehicle charging system. The vehicle seems to think it is plugged into the wall when it is actually not plugged in. It is a safety issue because the vehicle will not move -- my family could have been stranded. Planning to tow the vehicle to a dealership to have diagnosed within one week or so. The vehicle has not been inspected by anyone yet. There were no warnings prior to the failure. Currently there is a check engine light on as well as an image of a plug/cable indicating that the vehicle thinks it is plugged into an electrical outlet (which it is not).
Driving around 40 MPH when car alert came on to Shift into P and lost all forward drive. Needed to pull onto a side street since car would not accelerate. Car said service charging system and would not recover. Could not turn car off but A/C system stopped working. The transmission light were flashing from P to R to D but would not engage. Had to be towed to dealer to await troubleshooting.
While driving in the freeway this morning the check engine light and service charging system came on. Shortly thereafter the car lost all power and I was able to coast to a stop on the shoulder of the freeway. Fortunately I was in the carpool lane and near the shoulder and would not have been able to get over otherwise putting myself and others in a very dangerous situation on the freeway. Once stopped, the steering wheel locked as well as the wheels and the car lost all function. Although there are no formal recalls, there are forums dedicated to this problem: https://www.pacificaforums.com/threads/service-charging-system-error.41145/ The dealer is currently advising but the threads in those forums suggests FCA does not have a fix in place. The car becomes dangerous to drive as this happens out of nowhere with limited to no warning.
While driving on hybrid system (battery at 0%) and braking toward a stop sign , the car went into neutral and would not power the wheels. A message appeared to service hybrid charging system. all gauges wen to zero (start position) . The car was pulled over and it would not go back to to any gear. I was only 2 miles from my house and was able to get tools to disconnect the 12V battery in hopes for system recovery. After disconnecting the 12V battery the system recovered and I was able to get back home to setup a service appointment. The entertainment system did not recover correctly and required a software reboot with all settings reset. The previous week the battery charge indicator was behaving strangely as it would start with 36 miles of range and after 2 miles of very slow driving it wold drop to 26 miles. After a few more miles it would drop to 21 miles and stay at that for a long time.
The vehicle loses power while driving. I have to pull over shut it off and turn it back on. Most of the time it happens when I'm driving 75 mph. The service charging indicator pops up while also telling me to shift the car to park. Dealership diagnosed it to being a transmission issue. Drive motor A is failing in the transmission and is causing it to over heat. Which causes it to shut down. A quick restart and the van is going again.
car stalled at high speed in HOV fast track in rush hour traffic - terrifying! was running on electric mode. Heart stopping emergency tow. First Chrysler dealer where car was purchased said they could not trouble shoot for 2 months. Found another dealer, had it towed there. Waiting to learn what the problem is. Only 14,000 miles and under warranty.
Occurred at 1000,1500,and 16000 miles. Car would turn on but steering column is locked and it does not get in to gear. D and R just put car in neutral. Dealer is replacing hybrid battery. Car eventually got in to gear after several hard resets.
While driving on highway at approx. 75 mph using adaptive cruise control, the red power cord shaped warning indication appeared on the instrument panel. At the time the warning appeared, the vehicle was in battery powered mode. At the same time the warning showed, the electric motor stopped providing power. The vehicle did not transition to hybrid mode (gas powered), and depressing the accelerator did nothing. The vehicle began to slow from highway speeds, while other cars were in the vicinity. While all this was happening, the "D" on the gear selector was flashing. There was also a message saying to put the vehicle in park to restart, however, i did not want to attempt that at highway speeds. Other systems seemed to operate as normal, brakes worked, the electrical system seemed to be functional other than the described fault. I pulled over to the shoulder and stopped the vehicle completely. I turned the vehicle off, then on again, and was able to put the vehicle in drive without any faults occuring. Normal operation resumed after that.
Vehicle has completely lost power multiple times while driving on the freeway at 65 approximately MPH and displays "Service Charging System" message on dash. Accelerator pedal was completely non-responsive. Have had to quickly try to get to side of freeway and stop. Each time the van had remaining fuel. Each time we have been able to get the van started again by putting the vehicle in Park and pressing the start button 3 times in a row without pressing the brake pedal. It is very concerning and has happened to us several times this month with about 14.000 miles on the vehicle.
I was driving uphill at slow speed; then a garbage truck in front of me needed to do a 3 point turn so I switched to reverse to back up. At that point the vehicle started flashing an alarm, I think said "service hybrid system" or something like that and instantly began rolling down the hill for about a block, while I was completely unable to engage the break or gas, to switch to drive, to control the wheel, even the emergency brake failed. I was rolling backwards for about a block completely unable to control any component of the car when the emergency break finally engaged. I was able to turn on my hazards at that point, but the vehicle would not start, nor would it turn off. I could not make it reverse or drive or even turn off as I sat stuck in the middle of an intersection. After several minutes like that I was eventually able to restart the car and drive slowly home. This is MURDER by Chrysler waiting to happen. I was very lucky no child or vehicle was behind me as I rolled downhill into an intersection. I was very lucky to not be hit by oncoming traffic as I sat stuck after rolling a block downhill backwards-- THESE ARE MINIVANS FILLED WITH CHILDREN!! MY 4 children! Chrysler in Berkeley service said they haven't heard of this before and will see me only next week, and not sooner! However, There are numerous similar complaints registered. I am afraid to drive it at all at this point. This is a very new car with less than 10K miles on it. RECALL IMMEDIATELY PLEASE BEFORE SOMEONE GETS KILLED.
Car stalled out while driving at 35 mph. Lost all propulsive power. Indicated “service hybrid system” and “shift to park” error on multi function display. Was able to safely coast to side shoulder and stop. Car would not move again after this reading the same errors even after shifting to part and attempting to restart car. If this had occurred on a multi lane highway it would have been extremely dangerous.
Driving on the freeway at speed (approximately 65 MPH), the vehicle suddenly lost power and displayed "Service Charging System" message on dashboard. Accelerator pedal was non-responsive. We had remaining battery charge and fuel in the vehicle (it is a PHEV). Pulled over to side of freeway as vehicle was decelerating and came to a stop in the safest place our momentum could carry us to. We were eventually able to get the vehicle restarted by putting the vehicle in Park and pressing the start button 3 times in a row without touching the brake. This issue has happened four (4) times in the last several months, and has occurred on the freeway each time, and each time with remaining battery charge and fuel in the vehicle. The vehicle has not yet been inspected by the manufacturer for this specific problem.

While driving, the vehicle flashes a warning that a charging fault has occurred and instructs the driver to stop and put the vehicle into park. Once stopped and in park, the vehicle will not shift or drive. This has happened at least 6 times and the dealer has not been able to fix it.
On freeway , running on gas - the vehicle gave an error “service charging system” and vehicle shutdown and de accelerated rapidly - forward momentum only We are on freeway and no shoulder and narrow escaped an collision trying to get a safe stop at speed of 20-25 miles and slowing down from initial of 70 mph This is serious safety concern and how was this released for sale is certified by NTSB is a surprise and shock Multiple owners have complained of this
When I first purchased my 2021 Pacifica Hybrid Pinnacle in January 2021, randomly when pressing the lift gate button in the top left of the trunk it would beep multiple times and the lift gate would not move. The key fob, front button, and rear button would not engage the lift gate motor. You had to manually shut the lift gate and it would latch. Then in November 2021 I had the Y50 2021 RU Power Lift gate recall done on my vehicle and since then three times while driving the lift gate has popped up on its own. When you pull over all buttons are unresponsive and when you manually close the lift gate the locking mechanism does not engage. It just pulls right back open by hand or while driving. To resolve you must try over and over shutting the door, pressing the buttons, turning the vehicle on and off multiple times, letting it sleep and turn it back on and eventually the locking mechanism will engage. Have brought it multiple times to the dealer and every time they cannot recreate. Poses a dangerous safety issue while driving for the lift gate to open as you don't realize its not latched.

the electric heater for the battery has been broken for 3 months now. Chrysler cannot seem to find a replacement part. what this does is cause the engine to come on and go off way more then expected. this also can cause weird transmission and acceleration issues when the car doesn't know which one to pick. this is also putting a lot more pressure on the transmission and causing issues with which device is sending the power. this has been going on all winter and Chrysler cannot find the part to replace.
Malfunction light indicator (MIL) light turned on which caused vehicle EV only mode to be disabled and now the vehicle only runs under engine power poorly with the battery is always fully charged (MPG is cut in half since battery is never used). Manufacture service center found error code P0E15 on 3/3/2022, which indicated the EV battery coolant heater failed and needs to be replaced. Part is on backorder and no ETA is available, they indicated they have a few in their service center alone that have the same issue.
The contact owns a 2021 Chrysler Pacifica Hybrid. The contact stated while her husband was inside the vehicle, he noticed the instrument panel was not functioning properly. The contact stated that the HVAC, radio, backup camera, and Bluetooth failed to function as needed. The contact also stated the backup camera would function intermittently. The contact stated that the airbag warning light was illuminated. The contact took the vehicle to the local dealer, where it was diagnosed with needing the radio control unit to be replaced. The vehicle was not repaired due to the parts being on backorder. The manufacturer had been informed of the failure. The failure mileage was 7,519.

While driving, the vehicle shut off. Steering and brakes were still working, it's already happened 2 times and I was able to coast off the highway. After turning off and on the with the starter button the car starts normally with only the engine light on. There are several other posts on forums of others owners having the same problem.
We were driving on the interstate when the vehicle all of a sudden lost power. We were luckily able to get it to the side of the road, but if there was more traffic, we would not be able to do so. The car was then towed to the dealership and they said they will be replacing the the power inverter. The car has been in the dealership since the October and it is February now.
While driving, the vehicle shut off. Steering and brakes were still working, so I was able to coast off the highway. The check engine light, a battery fault indicator, and a service charging system warning all displayed on the dash. This is the second time this has happened, and a Chrysler dealer service center installed software updates they claimed fixed the issue. It is now back at the dealer. The first time this occurred, it was on a busy road with no shoulder near a dangerous turn. I was stopped in the road for several minutes before I got it to restart and was able to limp to a safe parking spot. There are several other posts on web forums and YouTube of other owners having the same problem.
We currently have 13170 miles on our 2021 Pacifica. We have seen this charge system error now for the third time. Here is the list of errors below. First time I was driving 45 mph and the vehicle lost power and had a charge system error. I was able to safely stop on the side of the road. I was able to restart the vehicle after about 2 minutes with a check engine light. I was able to take it to the dealership and had all of the current software updates and removed the check engine light. Second time I was at work and I attempted to start the vehicle from the run button. The radio turned on but I had a charge system error and the vehicle would not turn off. I worked for about 5 to 10 minutes to turn the vehicle off. Luckily I was not driving at the time and was parked. I again had a check engine light. The dealership diagnosed that I have a failing PIM. The vehicle sat at the dealership for a month, the part was continually on back order every two weeks. The check engine light was removed and I took the vehicle home still waiting on the replacement PIM. Third time I was in the garage, PIM still not replaced, on back order and the vehicle would not start from the run button. I was able to turn on the vehicle after waiting about 2 to 3 minutes. After driving for about 25 minutes the issue came up again. I was able to wait and the vehicle restarted. A check engine light illuminated on the dash. On the way home after a stop, I had loss of power and had the charge system error going 25mph . I was able to stop the vehicle in a empty parking lot. I waited about 5 minutes and was able to restart the vehicle. I drove the vehicle straight home and is now sitting in the garage at home. As of today I am still waiting for this back ordered PIM since October. The vehicle now will sit in the garage, unusable, until the replacement part can be installed. This may be a reliability and safety issue in the future.
The van displayed various error messages in the dash screen, and issued a warning beeped, then told me to shift the vehicle to park. The van then locked itself in park and could not be moved. Rotary gear selector non-responsive, transmission selector gear lights went out. We had to have the vehicle towed out of the restaurant drive through lane and delivered to the dealership. Check engine light illuminated and there was a warning message on the dash screen that I was not able to capture. This model of van is a plug-in hybrid. With this failure the engine systems are NOT utilizing the hybrid traction battery portion of the system. I have driven many small trips up to about 65 miles, but no drive has yet used the battery capacity. So the vehicle is running 100% on the gas engine, and is not able to function in hybrid or electrical only drive mode. The dealership is unable to tell me if driving the vehicle in this state will cause further damage to the van, therefore I am stuck unable to drive the vehicle for fear of causing even more damage. Error code pulled from personal obd2 reader: P0E15 - Hybrid/EV Battery Pack Coolant Heater A Control Performance On 1/4/2022 the dealer service advisor called and confirmed that the issue is the expected part failure/replacement, and that there are no parts available in the US for this repair. The last instance that my dealer repaired took 4 weeks to secure the needed parts for repair. Still no confirmation the van would be safe to drive in the interim. Therefore I am without my new 2021 vehicle purchased 5/10/2021, while it only has 1608miles... for what seems like will be at least another month. The dealer SA also mentioned to me that Chrysler currently had another ~450 instances of this same failure happening right now, all which going to be significantly impacted by the lack of parts availability. This is unacceptable.
The infotainment system would crash and freeze, causing the backup camera not to be visible on the center screen when in reverse.
Driving twice we have gotten a message "Pullover vehicle will shutoff" on the display, and the motor and engine shut off shortly after. After restart, the console says "Service Hybrid System" the van then has a top speed of 40-45 MPH, not safe to drive on interstate even to get to the next exit. The first time the dealer had the van for 10 days said the problem was in the hybrid system related to the charging cord (provided with the vehicle) The second time we were 2 miles from home and able to get the vehicle home safely with the hazard light on. The dealer is trying to tow it for the second time, the vehicle does not have enough power to drive up the ramps onto the roll-back tow truck.
While driving vehicle went into full lock down. Vehicle remained running with all systems on except completely unable to engage drive, shut vehicle down or even power it off. Towed to dealer. Dealer stating codes stating hybrid error and transmission errors. Vehicle has 7021 miles and was purchased March 2021. My wife and 2 children were stranded on the road in a vehicle that would not move. Terrifying for me.
The car stopped 2 times on it's own, told me to shift to park immediately but was in essence shutting itself off. Once approaching a stop light going ~10 mph and the second time I was going 50 mph down the road. The first time 3 warnings flashed across my screen, Service Charging System Service Required, Front Parksense Unavailable Service Required, Active Parksense Unavailable Service Required, I shut the car completely off waited a couple minutes and turned it back on. The check engine light stayed on. During the 2nd event it was only Service Charging System, after i shut the car off and restarted the Uconnect started updating and I couldn't drive and the check engine light came on.
We have now had two incidences where the vehicle is being driven both on the highway and in the city. The vehicle malfunctions without any indication or warning. While driving the vehicle beeps and the check engine light comes on, you lose all control or ability to accelerate. You still have steering. The screen on the dash in front of the driver flashes and continues to flash and says there is an issue, while saying to move the drive to park. The drive options (park, drive, reverse, etc) flashes on and off but will not move anyways. You are essentially forced to cruise to a stop hopefully in a safe place or emergently braking. The only way to get the vehicle to reset is once completely stopped. To push the ignition off button. Which does not fully turn it off. You then have to open the driver door hopefully safely. And allow the vehicle to completely shut off. Then get back in and it will usually start at this point with the check engine light still on. We have video and pictures of this happening to us both times and am have seen others experience this extremely dangerous situation. I am going to report to the dealer and the customer service for Chrysler as well. Then the news.
I purchased a pre-owned 2021 Pacifica Hybrid on November 4th 2021. After driving it off the lot, the vehicle stalled (essentially loses power and gas pedal does not work) while driving at a high rate of speed on the highway. Symptom: While driving at a high rate of speed (65MPH, 60MPH, 50MPH, 70MPH), van loses power and gas pedal will not work. Van is on <1% battery and running off gasoline power. After coasting to a stop on the side of the road, a message pops up that says "Service Charging System" and the check engine light comes on. Then it says "Shift to P to Start". Check engine light is on. Frequency: Occurrences happened immediately after taking ownership of vehicle and were 11/4/21, 11/6/21, 11/7/21, 11/9/21. After that we stopped driving the vehicle and took it back to the dealer because of safety concerns. Workaround: Shifting to P to start (starting like you would normally start the car) does not start the vehicle. We have been able to figure out a workaround to get it to start however. If you push the start button without your foot on the brake so it is in the "accessory" power mode, then put your foot on the brake, then push the start button again, it starts. The check engine light stays on for a period of time (less than 24hrs) after each event, but then goes off. I see this has been reported by several people with at least 4 NHTSA IDs filed with the exact same issue. This is a serious safety concern and is not only reported with these IDs, but also is prevalent on other online forums such as pacificaforums.com and Facebook group "Chrysler Pacifica Plug-In Hybrid 2017 and beyond." While driving on the interstate at ~60MPH the vehicle suddenly shut off and flashed a "Service Charging System" Error. The gas pedal was unresponsive and we were forced to swerve through traffic as the vehicle was rapidly decelerating to make our way to the side of the interstate. This is a minivan, and as a minivan, we were using it for it's intended purpose of transporting our 4 small children to school and daycare. Because this event happened on the interstate, not only did we risk a high speed crash, but we had to move the kids from one vehicle to another as cars whizzed by at 60-70mph. We tried multiple attempts at restarting the vehicle and it would not restart. The vehicle is currently at the dealership and is being looked at under warranty.
I wanted to write to you and let you know about our recent and scary experience with our new 2021 Chrysler Pacifica Hybrid Limited. We purchased our van on Labor Day, September 6, 2021. Since then, we have had 2 out of town family funerals and a family vacation so we have right at 5k miles on the vehicle. On 11/5/2021, at approximately 1:15pm, I was driving our van on a 6-lane major thoroughfare with my two small children with me. We had 43% of our EV battery remaining and so we were driving on EV power (and had a half tank of fuel also). Suddenly, the engine power stopped, I had no acceleration, and a “Service Charging System” message popped up on the instrument info panel along with a check engine light, a red battery light near the compass, and a red plug symbol just below the fuel gauge. I was not able to get the van to do anything but roll with whatever forward momentum we had toward the side of the highway. In nothing short of a miracle, we avoided multiple collisions and safely made it to the shoulder. We had electric power to run the radio and climate fan only but the car engine was completely inoperable and could not start or shift into gear after multiple attempts to restart it. I contacted roadside assistance and waited for a tow while trying to keep my small children calm and safe sitting in a dangerous location on the side of a highway. After about 30 minutes, the car engine fan shut off and so I attempted to restart the car and it finally worked. I immediately headed toward the closest dealership by taking the side roads and hoping that we wouldn’t have another incident and be a sitting duck on the highway again. This vehicle malfunction could have, and realistically still could, cause a traumatic and horrific fatal motor vehicle collision. This is not just a routine and minor malfunction like a door sensor issue, or check engine light, etc. This issue can lead to a severe safety issue that will, and did, render the vehicle a target with no escape.
I was in the garage near the car when it started spontaneously. I had the key fob in my pants pocket. It turned off when I sat inside and pushed the ignition off button. Did not notice any warnings/lights at the time. There were no safety consequences. My Concerns: What else could happen spontaneously? Could it shift into gear? Carbon monoxide in the closed garage? Has not recurred. The only recent auto malfunction was the Information display (UConnect) had locked up a couple of weeks prior after a routine software download to the car. No problem with vehicle operation with that episode. Checked by the dealer and felt to be OK. No recurrence. I attempted to notify MOPAR. Their web page email didn’t work for a several tries. I was unable to get past their customer service robot who got stuck on “other” when I couldn’t pick one of their menu options. I wanted NHTSA to be aware in case of other similar reports.
After unplugging from the charging station, the vehicle would not start. I pressed the start button several times, to no avail. Had to turn off the vehicle, wait 5 minutes, then try again to get it to start. This is the second time this has happened - with two separate charging stations.
We purchased our brand new 2021 Chrysler Pacifica Hybrid Limited on April 19th, 2021, from Hoblit Chrysler Dodge Jeep Ram in Woodland CA. At the dealership, my wife and I were able to add user profiles to the UConnect system, thereby customizing our driving experience to fit our individual needs. However, I started to notice a number of issues with the vehicle over the following weeks. UConnect Related: 1) The ability to add and remove user profiles disappeared. 2) The UConnect system randomly resets about once every couple weeks. 3) The UConnect system will randomly blast the AM/FM radio or will randomly start playing Sirius XM at loud volumes. 4) The Navigation System will sometimes navigate to the wrong location, despite the address being inputted correctly (I used google maps on my phone to confirm the wrong address). General Vehicle Issues: 1) The vehicle will sometimes keep itself turned on (despite turning the vehicle off) as long as a phone is connected to the vehicle via bluetooth (the car doesn't drop the bluetooth connection after turning the vehicle off!). 2) Sometimes, the vehicle will power on, allow you to shift into other gears, but won't steer or drive (I have video of this). Lane Keeping Assistance: 1) The driver-side lane departure correction is noticeably less accurate than the passenger-side. I'm hoping you all can help get some attention on these issues. It's crazy that Chrysler released the 2021 Pacifica Hybrids with such a faulty UConnect system. The internet seems to suggest that Chrysler is and has been aware of all of these issues but has not yet done anything to correct them. Mechanically, this car is great, but technologically, it's so bad my wife has on more than one occasion voiced her regret regarding this purchase. Our experiences are not one-offs, these issues are systemic across the 2021 Pacifica Hybrids. I have videos for items 1 and 2 under the "General Vehicle Issues" section above.
Here is our list they are "working on"... 1. had the no start issue 3 times...the first 2 times I could ONLY get it to start using the U-Connect app that we got free for the first year or whatever. The 3rd time I eventually got it to start after 3 tries. I did actually record the 2nd attempt so I have video proof of the exact issue. 2. we also have the issue of the entire system not shutting down after turning the car off AND completely exiting the vehicle...and even locking it. I can sit outside the van and listen to the music continue playing until it eventually shuts itself off after about a minute. Yes I know there are settings for it to remain playing after turning off the car but it is supposed to turn off when you open the door. 3. we have unreliable mirror folding issues...sometimes they fold when you park and unfold as they should...and sometimes they just don't at all...which really sucks in the garage and 3 kids running around the van. 4. a couple times while driving it, the main 10" screen just went black and all music stopped and then it rebooted itself and showed the Chrysler symbol and then all came back online. Side note, my workplace has a 2021 regular gas Pacifica and this happened at about our 1 hour mark into a 3 hour trip also...probably a software issue. 5. we don't get even close to 30 miles range on the battery...more like 20 if we are lucky...not sure on this one. 6. the rear lift gate button only works some of the time. basically if we don't close it within about a minute of opening it, then we have to manually pull it down (we never did get to try using the button up front or the remote, but the actual button at the rear liftgate area just doesn't work reliably). 7. this one is a little strange...but the XM stations don't update the actual song name when sitting in that multi-page view. It will just show the first song that played. The only way to update the song name is to actually hit the media icon to make it go full screen.
My issue matches the issue reported in NHTSA ID Number: 11414581. My own words describing the incident: We have a new 2021 PHEV Limited (purchased May 3, 2021), which has with 977 miles (554.5 electric, 423.1 Hybrid). On May 24, 2021 my wife had driven about 2 miles to a store parking lot, turned off the car and parked, then came back home, all on electric, all working fine. The car had a full charge when she left so it had 31 miles remaining when she got home. She pulled off on the side of the road in front of our house and put it in park and shut it off. About 30 seconds later (not exactly sure on the timing ) she tried turning it on to drive up our driveway and it wouldn’t turn on. The dash just kept saying “push break and push button to start”. She would do that, and nothing would happen - the message would stay up, then we’d here a ding and the message would disappear for a fraction of a second and reappear. This happened many times over a period of about 7 or 8 minutes (I actually recorded it on my phone). At one point we tried putting it in drive and it rolled backwards down the road like it was in neutral, while the the D indicator light flashed (rather than be on solid). She finally got it to turn off, then it started normally. This is unsafe as it sort of appears like the engine is running first (you can’t hear anything because it’s in electric mode) so you put it into gear and then you roll backward if you’re on a slight incline. We have not taken it to the dealership yet because our car was actually delivered with a scratch on the door and is being painted right now. Once it is done we will call Chrysler care for help and take the car in for service.
Vehicle broke down twice related to the hybrid electrical system. First Incident: vehicle would not start after being parked and turned off; upon attempt to start, vehicle displayed a "Service Electronic Throttle Control" error message in dash. Second Incident: vehicle was in motion on highway at ~65 mph, and suddenly displayed "Service Hybrid Electric System" error message in dash. Vehicle proceeded to enter into turtle mode and began deceleration which is dangerous while on highway. Took vehicle in both times to different dealers. Both dealers reported seeing error codes related to the hybrid system. Repair related to the First Incident: First Dealer flashed the BPCM module. Car ran fine until Second Incident Repair related to the Second Incident: still ongoing. 2nd Dealer reported that the 62-way wiring harness needed to be replaced. Dealer ordered MOPART replacement part number 68396626AA (the replacement part) to make the repair. At first, 2nd Dealer identified DTC codes related to the Second Incident but could not identify the issue. So 2nd Dealer reset the DTC codes without making repairs or further investigation, and attempted to get consumer to pick up vehicle. Chrysler pushed for further investigation, and resulted in identification of the wiring harness issue.
WHEN ATTEMPTING TO START THE VAN IT WILL PERIODICALLY NOT ENGAGE. THE GEAR SELECTOR WILL BLINK WHEN YOU SELECT A GEAR, BUT IT WILL NOT RESPOND TO THROTTLE. THE VEHICLE WILL ROLL HOWEVER. THIS BECOMES A GREATER PROBLEM AS YOU NEED TO REMOVE YOUR FOOT FROM THE BRAKE TO CYCLE THE VAN BACK TO OFF. ALL OF THESE ISSUES ARE COMPOUNDED BY FACT IT IS A PHEV AND THERE IS NO ENGINE NOISE TO HELP YOU NOTICE IT HAS NOT STARTED. CHRYSLER HAS SAID THEY ARE AWARE OF THE ISSUE, BUT THEY HAVE GIVEN NO INFORMATION OTHER THAN A FIX IS SUPPOSED TO COME SOMETIME IN Q2 OF 2021.
