TL* THE CONTACT OWNS A 2009 COSCO HIGH BACK CHILD RESTRAINT, MODEL NUMBER 22233-TSS, MANUFACTURED JULY 18, 2008. THE CONTACT STATED THAT SHE WAS INVOLVED IN A HEAD-ON CRASH. THE CHILD SEAT FAILED AND THE REAR PORTION OF THE CHILD SEAT FRACTURED, ALLOWING THE STRAPS THAT WERE SECURING THE CHILD IN PLACE TO RELEASE. THE CHILD WAS THROWN FROM THE SEAT AND STRUCK THE REAR OF THE FRONT PASSENGER SEAT. THE EMS HAD TO PRY THE CHILD SEAT AND THE CHILD FROM THE VEHICLE AND THE CHILD SUSTAINED FACIAL FRACTURES AS A RESULT OF THE CHILD SEAT FAILURE. A POLICE REPORT WAS FILED. THE CONTACT WAS CONCERNED THAT THE DESIGN OF THE CHILD RESTRAINT WAS NOT SUFFICIENT FOR A HEAD ON CRASH.

TL* THE CONTACT OWNS A 2009 COSCO HIGH BACK CHILD RESTRAINT, MODEL NUMBER 22233-TSS, MANUFACTURED JULY 18, 2008. THE CONTACT STATED THAT SHE WAS INVOLVED IN A HEAD-ON CRASH. THE CONTACT WAS DRIVING 2 MPH WHEN THE HARNESS THAT HOOKED THROUGH THE REAR OF THE SEAT SLIPPED OFF. WHEN THE FAILURE OCCURRED, THE CHILD WAS LEFT UNPROTECTED FROM THE LEFT SIDE. THE CONTACT HAD NOT NOTICED ANY PROBLEMS WITH THE SEAT PRIOR TO THE FAILURE. THE MANUFACTURER WAS NOT NOTIFIED OF THE FAILURE.

SHE STATED THAT THE HARNESS STRAP WOULD NOT SECURE THE CHILD INTO THE BOOSTER SEAT OR ALLOW ENOUGH ROOM FOR THE CHILD TO MANEUVER OUT OF THE SEAT WITHOUT ADULT ASSISTANCE. THE MANUFACTURER WAS NOT CONTACTED.
