The contact owns a 2011 Dodge Charger. The contact stated that the front headlamps failed to provide adequate illumination while driving. The vehicle was taken to an independent mechanic, who replaced the front headlamp bulbs and ballasts; however, the failure recurred. The vehicle was not repaired. The manufacturer was not made aware of the failure. The failure mileage was approximately 70,000.

Both low headlights went out while driving at night. I have changed the bulbs out multiple times and the continue to blow out.

The vehicle was taken to an independent mechanic where the headlight was replaced however, the failure recurred. The vehicle was taken to the same independent mechanic but the vehicle had not been diagnosed. The dealer or the manufacturer was not notified of the failure. The low beam left front headlight keeps going out every few months. It burns out and when I replace the bulb the harness looks hot and frayed. I keep loosing visibility while driving at night. Dodge stated I need a new headlamp harness. There are no warning lights present.
The headlights, one or both, arbitrarily go out while driving at night.
Front headlights go out randomly as the vehicle warms up. Appears to be related to the 2011/2012 headlamp recall 12V-042 but my vehicle is not coming up on the notification list. Loss of headlights decreases ability to see and increases the risk of an accident. No dash messages, no computer messages. Lamps work when initially turned on so vehicle hasn't failed any safety inspections. All obvious checks provide no possible reason for this issue.
My vehicle has the symptoms listed in NHSTA 12V-042. The dealership would not complete the recall due to my VIN number not being included or my manufacture date not matching the dates. They directed me to email the NHSTA to get this resolved

WE HAVE TO RUN THE FOG LIGHTS AT NIGHT JUST TO KEEP FROM WRECKING. THEY ASK ON ONE OF THE QUESTION, WHEN DID THIS HAPPEN. ITS BEEN HAPPENING SINCE WE BOUGHT THE CAR. THE BLIND SPOT WARNING IS WORKING EVERY ONCE IN A WHILE ALSO.

PASSENGER SIDE HEADLIGHT HAS GONE OUT 4 TIMES IN THE PAST 4 MONTHS. WHEN CHANGED/FIXED, EACH HEADLIGHT BULB IS BURNT AND 2 OUT OF THE 4 HAVE BUSTED INSIDE THE HEADLIGHT ASSEMBLY. IT IS THE CORRECT BULB/VOLTAGE AND CERTIFIED OREM REPLACEMENT HEADLIGHT BULBS FOR THE LOW BEAM. EACH BULB LAST ONLY 4-6 WEEKS. NO SIGNS OF MOISTURE IN THE HEADLAMP ASSEMBLY OR FAULTY INSTALLATION.
THE DRIVER'S SIDE LOW BEAM WILL COME ON THEN GO OFF. AFTER AWHILE IT COMES BACK ON FOR A FEW SECONDS THEN GOES OUT AGAIN. THE VEHICLE CAN BE SITTING RUNNING OR GOING DOWN THE ROAD AND WILL DO THE SAME THING.

DEALER ACTS AS IF THEY DON'T KNOW WHAT I'M TALKING ABOUT NEVER HEARD OF SUCH A PROBLEM AND CAN'T DUPLICATE PROBLEM SO CAN'T GET FIXED. THIS GOES FOR ME AND THOUSANDS OF OTHERS WITH SAME ISSUE WE NOW CAN'T DRIVE VEHICLE AT NIGHT OR TAKE A RISK THAT COULD EITHER GET US A TICKET FOR NO HEADLIGHT OR COULD BE IN A DRIVING SITUATION BEING PITCH BLACK OUT NO CITY LIGHTS TO HELP SEE MAKING A TURN AND LIGHTS GO OUT. I HAVE TRIED GETTING DODGE TO LOOK AT IT REPEATEDLY, IT IS CLEARLY A WIRING ELECTRICAL FAULT, THEY KEEP TELLING ME NOTHING IS UNDER REAL, AND I KEEP PUTTING MONEY AFTER MONEY INTO THE CAR, AND I THINK ITS GOING TO GET ME HURT ONE DAY.

I WAS TOLD THE FIX WILL BE $440.+ TAX AS THE PROBLEM IS A VOLTAGE DROP IN THE HARNESS THAT WAS INSTALLED AND NOT THE ORIGINAL MELTED WIRING THAT CAUSED THE PO8 RECALL. I THOUGHT FAULTY WIRING CAUSING LOSS OF HEADLIGHTS WAS A SAFETY ISSUE.

I TOOK IT TO A DODGE DEALERSHIP AND THEY COULD NOT FIND A PROBLEM. THIS PROBLEM STILL OCCURS. NEITHER FIRESTONE OR DODGE CHARGED ME ANYTHING FOR THE CHECK. THEY BOTH SAID "NO PROBLEM FOUND". NOW DODGE SENT ME A RECALL NOTICE FOR AN ALTERNATOR PROBLEM. I WONDER IF THIS IS RELATED.

IN ADDITION, THE FUSES WERE INSPECTED BUT THE FAILURE COULD NOT BE DETERMINED DIRECTLY. PEP BOYS LOCATED NHTSA CAMPAIGN NUMBER: 14V101000 (EXTERIOR LIGHTING) AND REFERRED THE CONTACT TO THE DEALER. AN UNKNOWN DODGE DEALER IN LUMBERTON, NEW JERSEY WAS MADE AWARE OF THE FAILURE AND STATED THAT THERE WAS NO RECALL ON THE CONTACT'S VIN. THE MANUFACTURER WAS MADE AWARE OF THE ISSUE AND AGREED WITH THE DEALER. THE FAILURE MILEAGE WAS APPROXIMATELY 80,000.

UPON INVESTIGATION, THE ALTERNATOR FAILED, SHORTING OUT THE ELECTRICAL SYSTEM AND KILLING/DESTROYING THE BATTERY. THE ALTERNATOR SHOULDN'T DO THIS. I FOUND THAT DODGE HAD A RECALL ( P-60) ON THEIR V-6 MODELS FOR THIS VERY SAME ISSUE. MY CAR IS A V-8. I WAS LUCKY I WASN'T DRIVING ON A MAJOR HIGHWAY OR AT NIGHT AS THE CAR HAD NO WAY IOF INDICATING DISTRESS TO OTHER DRIVERS AND THE HEADLIGHTS DIDN'T WORK. THERE IS A CLEAR DEFECT IN THE ALTERNATORS OF THESE CARS, ITS ONLY A MATTER OF TIME BEFORE SOMEONE IS INJURED OR KILLED.

ALL LIGHTS ON THE DASH LIGHT UP/FLASH THEN AFTER A FEW SECONDS EVERYTHING GO BACK TO NORMAL. HAVE HAPPENED SEVERAL TIMES WHILE DRIVING. AND ON TWO OCCASIONS THE POWER STEERING FUSES HAD TO BE REPLACED BECAUSE THEY KEEP GETTING BLOWN OUT. HAVE HAD ALTERNATOR EXCHANGED AT LAST RECALL. IT SOUNDS A LOT LIKE THE ALTERNATOR FAILURE RECALL INVOLVING THE 6 CYLINDER MODELS WITH THE 160 AMP ALTERNATOR. (I BELIEVE THIS IS THE SAME ALTERNATOR THAT WAS USED IN THE R/T 5.7L HEMI MODEL) THE POWER CAME BACK ON AND I DROVE THE LAST 4-5 MILES HOME WITHOUT FURTHER INCIDENT. I AM APPREHENSIVE ABOUT DRIVING ON THE FREEWAY NOW. LOOSING THE ABILITY TO STEER AND BRAKE AT FREEWAY SPEED COULD END IN AN ACCIDENT. A CO-WORKER INFORMED ME THAT MY DRIVER'S SIDE HEADLIGHT WAS OUT ON THE 30TH OF AUGUST. WHEN I CHANGED OUT THE BULBS FOR THE HEADLAMPS, I NOTICED THAT THE WIRING HARNESS SHOWED SIGNS OF OVERHEATING. THE DRIVER'S SIDE HEADLAMP WORKED FOR A SHORT PERIOD OF TIME AND THEN WENT OUT. UPON TAKING THE CAR TO THE DEALERSHIP, THEY INDICATED THEY DIDN'T THINK REPAIR WOULD BE COVERED UNDER THE WARRANTY. I SUBSEQUENTLY WENT ONLINE AND FOUND TWO RECALLS FOR 2011/2012 DODGER CHARGER POLICE VEHICLES THAT MATCH MY PROBLEM EXACTLY. THE RECALLS ARE #94303 AND #94301. WHEN I CALLED DODGE ABOUT THIS, THEY TOOK MY VIN AND TOLD ME MY CAR WASN'T COVERED UNDER THE RECALL AND WOULDN'T BE COVERED UNLESS ENOUGH PEOPLE COMPLAINED ABOUT IT. REMOVED THE LAMP AND FOUND THE CONNECTOR SHELL/HOUSING CHARRED AND BRITTLE. CONTACTS ON ONE SIDE OF THE CONNECTOR AND BOTH THE ORIGINAL AND REPLACEMENT LAMPS WERE DISCOLORED BY INTENSE HEAT. EVIDENTLY, THE CONNECTOR CONTACT WAS NOT PROVIDING A SOLID CONNECTION WITH THE LAMP TERMINALS, AND THE HIGH RESISTANCE CONNECTION RESULTED IN OVERHEATING AND MELTING OF THE CONNECTOR, A DEALER SERVICE ADVISOR INFORMED ME THAT THIS IS A KNOWN, ONGOING PROBLEM FOR WHICH THE SOLUTION IS TO REPLACE THE ENTIRE WIRING HARNESS AT THE OWNER'S EXPENSE. RESEARCH INDICATED THAT THE PROBLEM HAS BEEN ACKNOWLEDGED BY CHRYSLER, BUT ONLY POLICE VEHICLES HAVE BEEN RECALLED. TO AVOID SPENDING HUNDREDS OF DOLLARS TO HAVE THE ENTIRE HARNESS REPLACED, THE ORIGINAL CONNECTOR HAS BEEN REMOVED AND A REPLACEMENT CONNECTOR INSTALLED. REPLACED THE HARNESS AND ON THE SAME DAY OF PAYMENT AND RETRIEVING THE CAR, THE PASSENGER SIDE FOG LIGHT WAS OUT. OBVIOUSLY BY THIS POINT I WAS EXTREMELY DISAPPOINTED WITH THIS PROBLEM AS THIS WAS THE 4TH VISIT. TOOK IT BACK A 5TH TIME FOR THE FOG LIGHT. THEY REPLACED THE BULB AND AFTER 2 DAYS THE OTHER FOG LIGHT SHORTED OUT. ABSOLUTELY FED UP WITH THE PROBLEM I DECIDED TO TAKE THE CAR TO A DIFFERENT DODGE DEALERSHIP. UPON DIAGNOSTIC I'M TOLD THAT MY POWER DISTRIBUTION CENTER IS MELTED AND FUSES ARE CRACKED HENCE THE PROBLEM WITH THE A/C AND FOG LIGHTS SHORTING OUT. I'VE RESEARCHED THE PROBLEMS I'M HAVING WITH THE CAR ONLY TO FIND OUT THAT IT IS A COMMON THEME AMONGST THE SAME YEAR MODEL OF MY CAR AND WAS EVEN TOLD BY THE DEALERSHIP THAT THE PROBLEMS WERE ONLY RECALLED IN POLICE VEHICLES UNDER CODE M01. I WAS ALSO TOLD THAT IF THE PROBLEM ISN'T FIXED IT COULD LEAD TO THE VEHICLE BEING COMPLETELY UNABLE TO START. AFTER DEALING WITH HEADQUARTERS AND 2 DEALERSHIPS I'M BEING TOLD THAT I'M OUT OF WARRANTY AT 55,000 MILES AND THE REPAIR IS $984. THE DEALER CAME BACK WITH A GOODWILL OFFER OF $500 THAT IS ONLY VALID FOR 30 DAYS. TO ME THIS IS AN ADMISSION OF GUILT YET THEY STILL DO NOT WANT TO REPAIR AND I'M LEFT DRIVING IN AN UNSAFE VEHICLE. SEVERAL ONCOMING VEHICLES WERE FLASHING THEIR HEADLIGHTS TOWARDS OUR WAY IN ORDER TO WARN US OF AN ISSUE WITH OUR HEADLIGHTS, WHICH WE COULD NOT SEE FROM INSIDE THE VEHICLE AT THE TIME. AND IT WASN'T DARK ENOUGH TO SEE THIS ISSUE OCCURRING. WE PULLED THE VEHICLE OVER TO INSPECT, ONLY TO FIND OUR DRIVER SIDE LOW BEAM HEADLIGHT FLICKERING AND STROBING. I WAS ABLE TO PURCHASE YET ANOTHER HEADLIGHT LOW BEAM FROM THE LOCAL WALMART STORE, THAT WAS SPEC'D AS AN OEM REPLACEMENT FROM SYLVANIA. THIS WAS THE 2ND TIME THE LIGHT HAS BEEN REPLACED, BUT FIRST FOR ME TO DO SO. I TRIED TO REPLACE THE BULB, ONLY TO DISCOVER THAT THE LOW BEAM CONNECTOR WAS MELTED ON THAT SIDE OF THE CAR, WHICH WAS ENCASED BY IT'S OWN PROTECTIVE HOUSING. UPON CLEANING UP THE CONNECTION THE BEST I COULD, WE GOT THE LIGHT TO WORK. THE NEXT FEW MILES WERE OK, BUT THEN NOTICED THE SMELL AGAIN. THE OCCURRENCE IS NOW HAPPENING EVERY DAY, AND WE TRY TO CATCH IT BEFORE THE CONNECTOR MELTS. THE CAR MUST STAY AT HOME UNTIL THE DEALER CAN LOOK AT THIS AND SOLVE THE ISSUE. TO NOW END, THIS VEHICLE HAS HAD OVER 26 ISSUES IN A SPAN OF 16 MONTHS NOW.
