I am reporting two safety-related issues with my 2022 Honda Accord Hybrid. 1. Head Gasket Failure At approximately 93,500 miles, the head gasket failed without any prior warning signs. The vehicle had been regularly maintained at Honda dealerships, with one service done by a third-party mechanic around 87,000 miles. There were no engine warning lights or coolant leak warnings before the incident. The engine temperature warning suddenly appeared while driving, and I immediately stopped the car. A Honda dealership confirmed the head gasket failure and quoted over $4,000 for repairs. This kind of major engine failure is highly unusual for such a new vehicle and raises serious concerns about long-term engine durability or possible design flaws in the cooling system or head gasket components. 2. False Activation of Automatic Emergency Braking (AEB) The vehicle’s automatic emergency braking system frequently activates or issues a warning while driving, even when there are no obstacles or vehicles nearby. This has happened multiple times and creates a sudden sense of panic, increasing the risk of being rear-ended or losing control of the vehicle. Both issues suggest potential safety defects in this model. I respectfully request that the National Highway Traffic Safety Administration investigate whether similar complaints have been reported for the 2022 Honda Accord Hybrid, as these failures could pose significant safety risks.
Honda Accord Hybrid Touring Edition 2022 [XXX] [XXX] Complete Brake System Failure while pulling in to a Starbucks parking lot in Wayzata, MN. Braking system did not react or stop the car. Automatic braking system did not function either. As a result the car collided into a My Burger wall. No dashboard warning lights. After collision police and ambulance arrived on the scene. Driver was taken to Urgent Care after sustaining soft tissue injuries affecting clavicle, sternum, neck and back. The contact owned a 2022 Honda Accord. The contact stated that while driving 68-70 MPH, the vehicle stopped abruptly, causing another vehicle to crash into the rear passenger's side of the vehicle, and a second vehicle to crash into the rear of the vehicle, causing the trunk to be pushed inside the rear seats of the vehicle. The brake warning light was illuminated. The air bags of the contact's vehicle did not deploy. The air bags of the two other vehicles deployed. The contact stated that the rearview mirror separated and hit her head, causing a concussion. Additionally, the contact sustained whiplash, PSTD, and contusions on the back and on both ankles. The contact received medical assistance. The front passenger seat occupant of the second vehicle that rear-ended the vehicle sustained hip injuries, as the occupant was still recovering from a hip surgery that she had undergone prior to the collision. It was unknown whether the passenger received medical assistance. There was no property damage. A police report was filed. The vehicle was towed to Crash Champions, where it was totaled. The contact hired private investigators who retrieved the EDR data and informed the contact that the emergency braking system had engaged prior to the crash; however, the contact stated that there was no other vehicle or object around the vehicle at the time the emergency braking system had engaged. The vehicle was towed to the dealer, where the contact was informed that special equipment was not available to diagnose the vehicle based on the EDR data. The manufacturer was not notified of the failure. The failure mileage was approximately 37,600.

At approximately 9:20 a.m. on November 20, 2024 while driving at approximately 27 MPH the Collision Mitigation Braking System (CMBS) on my 2022 Honda Accord Hybrid activated, bringing my car to a near halt while driving on a clear road in perfect visibility. At the time of the incident there were three pedestrians and a dog present on the sidewalk approximately 5 feet to the right of the roadway. Traffic on this particular stretch of road was relatively light at the time of the incident. No collisions occurred, and no individuals were injured. However, on a normal day there will be numerous other vehicles driving in the same lane, which would likely result in a rear end collision should this problem recur. This incident occurred without any prior warning or other symptoms. The vehicle's CMBS has not yet been inspected by the manufacturer or any other parties.
Suddenly slows down without any reason while lane changing or at stop sign. It is a safety issues.
I have been driving for over 30 years and have never had a windshield. Have a crack in it and now with this car. In less than two years, I have about six cracks in my windshield, which i've had to repair. I have also had serious problem with the car.Automatically braking with adaptive cruise control where there's no one in front of me, but the car just breaks for no reason.
Vehicle safety braking automatically operates even when no other vehicles are nearby; no vehicles in front, on either side, and behind my vehicle. In other words, phantom braking occurs without warning. This has occurred numerous times in 2023 and 2024.
Car stops abruptly without warning or reason. This happens about once a week. Could cause an accident under certain circumstances.
There have been multiple instances of the car slowing or braking while passing other vehicles on the highway. In some cases the emergency BRAKE alert shows on the dashboard and the vehicle brakes on its own. In ALL cases there was no hazard or reason to slow or brake. This occurs often but intermittently. It has occurred in rain, night, day, sun, or other various atmospheric conditions and there seems to be no correlation other than the ACC is engaged at the time and we're passing other vehicles that are on the right side of the car. In several cases, there are other vehicles behind us while passing and the "brake check" occurrences have caused those behind us to have to brake hard or swerve to avoid us. It also creates a terrible habit for the driver of my car of using the accelerator when the BRAKE light illuminates in order to not get hit from behind. The dealer was notified and checked the car and claims there are no issues with ACC or the sensors. This indicates that the system gain tuning my be faulty or too sensitive and that the dealers are not able to make any changes to fix the issue.

My vehicle has fuel injector issues and Honda refuses to repair under manufacturers warranty although it's a safety issue and defects in their products . When my vehicle starts it misfires and causes the vehicle to go in limp mode disabling all safety features. No airbags or brakes etc . If these thousands of vehicles remain on the road it's not safe to drive. The vehicle stalls in the middle of roadways making it dangerous for everyone due to Honda bad quality control. While driving on a clear day with good weather, my car's Collision Mitigation Braking System (CMBS) slammed on the car's brakes with no one in front of me whatsoever. I was driving over a bridge at the time, and it was my assumption that shadows from the bridge structures above caused the system to go. This is the second time the CMBS went off for no reason. I am lucky no one was behind me as the car slams on the brakes when CMBS goes off.
My daughter and I just bought this 2022 Honda Accord on 04/29/2023. On 6/28/2023, she was headed home, driving North, and was coming to the busy intersection of a County Road and Highway. She went to brake, but the brakes did not respond. In order to avoid hitting another vehicle, she swerved and blew through the intersection. Thank goodness she did not hit anyone in the intersection AND no one hit her. It could have been so bad. She did come to rest in a farmer's field on the Northeast corner of the intersection. According to the shop that is currently processing the claim with State Farm and investigating the repairs and damage, many error lights are lit up on the dash as relates to the brakes, forward collision warning, and emergency braking. They indicated that they need to work with Honda to clear, verify, and repair everything related to those codes. The vehicle is scheduled to be towed to Honda of Greeley on Wednesday, 7/19/2023. In my research, I did see the NHTSA Acknowledgment of Recall Letter just signed on 06/22/2023 by Alex Ansley for the 2022 Honda Accord, indicating that a recall would be issued in regards to an issue with the brakes, that may be related to my daughter's accident. I also read further that the actual recall letters would be mailed out 07/24/2023. I am just not sure if this would also cover the 2022 Honda Accord Hybrid.

The Honda is equipped with a Collision Mitigation Braking System. Traveling at around 40 mph on a two lane road, one lane in each opposite direction, the orange brake warning light appeared on the dash. Simultaneously, the brakes were very forcefully applied as if there was going to be an unavoidable collision. There was nothing in front of the vehicle. I called the dealer where we purchased the Honda. The service rep said he had heard of other similar instances, asked what kind of road it was and said to see if it happened again on the same road. He knew of no known fix for the issue. My wife, who was in the back seat, and recently had 3 sections of her neck fused, is currently in a lot of pain. If the road had been a little slippery, all four in the car could have been seriously injured or worse. If there had been a vehicle close behind us, there is a good chance we would have been rear ended.

I contacted Honda and raised a complaint – 13553020 reading this issue and on their advice, I had my car checked by a different dealer Valley Honda at Naperville on 12/27/2022. They did the same and drove the car for another 30 miles and the code was gone. Again after two weeks, the same error messages are on the dashboard. I stopped using the car as it was a safety hazard to dive with the brake system malfunctioning and updated Honda the same. The car has been used for daily commutes for work and driving within 40 miles per hour depending on the allowed speed limits. Even though this qualifies as a lemon car but Honda is declining and asking me to take it to the dealer again (3rd time). The defects of the car substantially impacted my ability to use the vehicle. Several instances this rapid and unexpected brake application has required driver action to avoid rear-end collisions with the car behind who may be following too closely. Unable to completely turn off the CMBS system, must be done per drive. With that system turned off the braking application is expectedly absent during these events, but the car will still provide a rapid deceleration (as if a driver took their foot off the accelerator pedal) but without brake application which also has been problematic for cars following. Local dealer confirmed that the system is functioning without warnings or errors, no recalls are active for the car, and the alignment and function of all related sensors are per the manufacturer. Purchased car on 11-30-21. Three (3) times, most recently 3-5-22, while making a lane change from a single lane to multiple lanes on a right curve, the vehicle has suddenly decelerated and the braking system came on. Fortunately, no one was behind me on any of the three (3) occurrences. The vehicle did not come to a complete stop. Speed at time of each incident was around 45 mph and the vehicle slowed to under 30 mph abruptly.
