To Whom It May Concern, I am submitting this complaint to report a serious vehicle safety issue involving a Honda vehicle that was under a lease agreement. From the start of the lease, the vehicle suffered from repeated malfunctions of its safety and proximity sensors. These issues were reported, and the vehicle was taken multiple times to an authorized Honda dealership. Despite these efforts, the problem was never resolved. I later discovered that the vehicle had an active safety recall that was never disclosed to me, even though the vehicle had been serviced at the dealership on several occasions. This failure to inform me of a recall represents a significant safety concern. Subsequently, the vehicle was involved in an accident that resulted in a total loss. I firmly believe that the unresolved sensor failures and the lack of recall disclosure played a role in this incident. I am submitting this report so that NHTSA may investigate this matter and determine whether there is a broader safety issue affecting other vehicles. Thank you for your attention. While driving, my 2023 Honda Accord Hybrid suddenly experienced a multi-system failure affecting: Road Departure Mitigation System Tire Pressure Monitoring System (TPMS) Vehicle Stability Assist (VSA) Hill Start Assist Electric Power Steering System Brake System (“Brake performance may be reduced”) All warnings appeared within seconds. The vehicle is available for inspection, and the incident is documented with multiple photographs. In addition to the system failures, the vehicle unexpectedly began to lose power while I was driving, causing it to slow down rapidly in active traffic. This created an extremely hazardous situation, including: Risk of being rear-ended since my vehicle slowed without warning Reduced steering assist Reduced braking performance Loss of stability control Inability to accelerate out of the way This combination could easily have caused a serious collision. I later learned that there is an active recall related to the systems that failed on my vehicle. However: I never received any communication from Honda, no letter, no email, no notice of any kind. I am extremely unhappy and frustrated about this. Had Honda informed me of the recall: I would have scheduled service immediately I would NOT have driven the car under normal conditions I would NOT have been exposed to this dangerous situation on the road The absence of recall notification directly contributed to my being put in a potentially life-threatening situation. A recall notification exists for a reason which is to prevent exactly the type of failure and danger that I experienced. Honda’s failure to notify me removed my ability to take preventative action. The issue has not yet been diagnosed or confirmed by a Honda dealer. I will be taking the vehicle in for inspection as soon as possible. No inspections have been conducted yet, but I have full photographic documentation from the moment the incident occurred. No prior warning lights, symptoms, or error messages occurred.
Summary of Incident: For approximately two years, my vehicle has experienced random electrical issues. All dashboard warning lights will flash at random times. When I take the vehicle to the dealership (Bell Honda), the problem disappears before it can be diagnosed. On September 27, 2025, and again on September 28, 2025, the vehicle completely stopped and shut off while I was driving. It would not restart. The hazard lights were stuck on and could not be turned off. This left me stranded and in a dangerous situation. After approximately 2-3 minutes, the vehicle was able to restart, but numerous warning lights remained illuminated on the dashboard. I had the vehicle tested with a diagnostic tool at an AutoZone, but no apparent issues were found in the system. Additional Information: This is an ongoing and unresolved safety issue that Honda has failed to address. This electrical problem, which causes the vehicle to shut down unexpectedly, is a significant safety hazard. This issue is in addition to another ongoing problem with the remote start system, which Honda has refused to diagnose and fix. Consequences and Concerns: The unexpected shutdown of the vehicle while in operation is extremely dangerous and could lead to a serious accident. The inability to use hazard lights properly further increases the risk to myself and other drivers. The intermittent nature of the problem makes it difficult to diagnose, but the consistent occurrence poses a serious risk that requires immediate investigation by Honda and the NHTSA.
The contact owns a 2023 Honda Accord. The contact stated that the vehicle failed to lock while the key fob was not inside the vehicle. Additionally, the contact stated that messages pertaining to the braking system failure, the EPS failure, the Collision Mitigation Braking System failure, and other unknown messages were displayed, with several unknown warning lights illuminated. The vehicle was taken to the dealer to be diagnosed. The vehicle was not yet diagnosed or repaired. The manufacturer was not notified of the failure. The failure mileage was approximately 37,200.

I will be driving down the road and my vehicle will pop up with the brake message and slam by brakes. It does it randomly. It has put my safety in risk multiple times. It has done this on the interstate and thankfully there was no one behind me. I was going 80mph and it dropped me to 30mph in less then 5 seconds. The issue has not been reproduced since it does it randomly. The vehicle has been taken to the dealership many many times. There were no warning symptoms or anything. It just does it very randomly. I had an investigation open with Honda and all they said to do was take it back to the dealership. This car is an issue to public safety. I could very well be in front of someone one day who is not paying attention and there could be a very bad issue. There has been so many times these issues have occurred.
Driving the vehicle at highway speeds the large orange “Brake” warning illuminates and flashes, and the brake activates. This has happened approximately 5-6 times since I have owned the car. This started within 2 months of purchasing the vehicle. The dealer has tried replacing the front camera and I have changed the battery. None of this has fixed the issue. According to my Innova 5610 code reader, there are 32 pending codes for the Electric Brake Booster, including bring temperature too high, brake booster driver, circuit, overheat, brake pedal stroke sensor component, internal failure, brake pedal stroke sensor one circuit short to ground, brake pedal stroke sensor one circuit short to battery or open, brake pedal stroke sensor two circuit, short to ground, brake pedal stroke sensor two circuit short to battery or open, sensor voltage, a short circuit to ground, driver seatbelt sensor, vehicle, communication bus a performance, vehicle communication bus C performance, lost communication with the ECM/PCM A, F – can malfunction PCM – TCM, can communication failure with driving support system control unit, lost communication with vehicle, dynamics, control, module, lost communication with park brake control module, can reception error SRS, can malfunction ECM/PCM gauge control, module, invalid data received – signal has too many transitions/events, invalid data received from transmission, control, module – event information, invalid data received from vehicle dynamic control module event information, electric parking brake system malfunction control module – internal electronic failure, battery voltage circuit voltage below threshold, battery circuit voltage above threshold, and control module input power a circuit open. After I changed the battery and took the vehicle to the dealer for recalibration they mentioned the pending items, but did not do anything about them or record them on their paperwork.
The idle stop system every time I turn it off it will automatically come back on and this feature makes me feel as if when it keeps doing that after I’m constantly turning it off from it popping back on that sometimes it don’t want to catch speed. I for see this being a long term issue if all Hondas are like this .
When driving home the car emergency braked with no cars, debris, or anything else in the road. Car breaked to a full stop from 40mph causing serious whiplash and almost causing an accident.
Cruise control will apply brakes when lane ahead is clear. This happens multiple times per trip, every trip. Automatic emergency braking way too aggressive. Has nearly caused me to be rear ended twice. I try to remember to disable it every drive. I wish I could permanently disable it.

Driving at highway speed (70 MPH) the power assist steering and all of the Honda Sensing suit subsystem totally quit working. The dash had multiple warming lights and dialog box's describing system failures, I continued driving till I could get off of the highway and turned the vehicle off, after about 10 minutes I restarted the vehicle and all systems and steering reset. Drove the vehicle to Lompoc Honda for maintenance diagnostics they found multiple codes but no real cause, the dealership cleared the code per Honda's direction and test drove the vehicle several times. I was told to return if the issue persists.
March - mpg display rapid fluctuations causing car to use more gas than should. It resolved itself a few weeks after taking it to dealership and them saying it not a problem but I don’t think it should’ve been like that. August 29 - acc stop sensing car in-front me immediately after turning on indicator and sped up resulting in and accident, luckily no one was injured. September 7 - while reversing into a frequently used parking spot the car thought ‘the wall was moving’ it has never done that before. Automatic collision braking activates when vehicle or object is nowhere near the car and almost caused accidents multiple times. I thought this was natural at first but then I heard of others having the same problem.

The car's dashboard will flash break and will use its emergency braking system to stop when there isn't anything in the way. Thus, the car stopping short and potentially causing an accident while there isn't a reason to break.

All the lights on my dash came on and the car would not move no faster than 10mph. Took it to the dealership and the mechanic stated that the brake light switch was not properly installed at the factory

I have reported that software cannot be updated on my car. Since my first report my rear seatbelts no longer need to be fastened with children in the car or the seat belt notification of not being fastened does not work. My air conditioner will stop if my infotainment system needs to be adjusted through my phone, and today in the rain my brakes locked up twice. The latter occurrence was coming down 1000 foot downhill in the rain with a car in front and an 18 wheeler behind me

I have since not been contacted and am having to visit back there almost weekly to get a solution to this. These cars are new and almost entirely automatic and this is becoming an area of concern for my safety as well as my families safety. Over time I have also noticed the automatic break system signaling me to break when there is no obstacle ahead or even in the vicinity. Not to mention that the adaptive cruise control causes a hard break while next to cars in the nearby lanes even when they are not merging or coming within my lines.
