The contact owns a 2002 Honda Civic. The contact received notification of NHTSA Campaign Number: 19V501000(Air Bags) and would like to be removed from the recall distribution list. The local dealer and manufacturer were contacted.
The death investigator's decedent owned a 2002 Honda Civic. The contact stated that after the decedent stopped at a stop sign, they attempted to make a left turn and struck another vehicle on the passenger side. The decedent's air bag was deployed which caused trauma to the decedent's neck. The decedent's vehicle caught fire shortly after impact. The other driver attempted to remove the decedent from the vehicle as authorities were called to the scene. The decedent received medical treatment at the scene and was later transported to the hospital. The decedent was pronounced deceased approximately 30 minutes after arriving at the emergency room. The other driver did not sustain any injuries. A police officer who arrived at the scene managed to extinguish the fire with a fire extinguisher. The fire department later arrived at the scene to ensure that the fire was extinguished. A police report was filed. The whereabouts of the vehicle after the accident were unknown. The dealer nor the manufacturer had yet to be notified of the failure. Upon investigation, it was discovered that the vehicle was included in NHTSA Campaign Number: 15V370000(Air Bags) and NHTSA Campaign Number: 15V320000(Air Bags) which the contact linked to the decedent's death. The failure mileage was unknown.

SRS light is on. Upon resetting, light comes back on in a couple days. Mechanic says the airbags will not fire with the SRS light on.
SINCE THE PURCHASE OF THE VEHICLE THE SRS WARNING LIGHT HAS CONTINUOUSLY STAYED ILLUMINATED ON THE DASH BOARD. MY FIRST THOUGHT OF COURSE WAS SOMETHING WAS EITHER WRONG WITH THE AIR BAG SYSTEM OR THE VEHICLE WAS INVOLVED IN A COLLISION THAT LED TO THE AIR BAGS (WHICH ONES I HAVE NO CLUE) BEING DEPLOYED AND SO WHEN I SIGNED THE BILL OF SALE WITH THE DEALER I INQUIRED ABOUT THIS ISSUE AND WAS GIVEN A PAPER THAT STATES THE DEALER DID NOT KNOW AS WELL AS TO WHAYLT THE AIRBAG SYSTEM STATUS WAS BUT WAS TO BE EXCLUDED FROM LIABILITY TO THIS ISSUE BY MAKING UP THIS RIDICULOUS DOCUMENT. AFTER SEEING THE CAR FAX THE VEHICLE WAS THEN KNOWN TO HAVE BEEN IN A HEAD ON COLLISION IN 2018 AND SO THE DEALER LIED A BOUT NOT KNOWING ANYTHING ABOUT THIS ISSUE. THIS DEALER HAS BEEN TOTALLY NON COMPLIANT WITH THE PROCESS OF GETTING THIS ISSUE ADDRESSED LET ALONE FIXED AND I HAVE TRIED ON SEVERAL ATTEMPTS TO TALK TO OWNER WITHOUT ANY CALLBACK OR ANSWERS. I KNLY GET HOUNDED ABOUT WHAT THEY BELIEVE TO BE THE BALANCE DUE ON THE VEHICLE THAT'S IT . JUST TODAY I RAN THE VIN INN THE DATABASE AND SURE ENOUGH THERE WAS AN OPEN RECALL ON THE AIR BAG SYSTWMM IN ACCORDANCE WITH THIS VIN AND THAT'S WHEN I DECIDED TO FILE THIS COMPLAINT .
RECURRENT AND UNSOLVED ISSUES WITH THE SRS AIRBAG WARNING LIGHT. IT WAS "REPAIRED" IN 2016, AGAIN IN FEBRUARY 2021, AND HAS RECURRED AGAIN IN MARCH 2021. I HAVE A HONDA CIVIC SI, WHICH WAS NOT INCLUDED IN THE TAKATA RECALL PROGRAM. THE SI IS AN UNCOMMON MODEL OF THE CIVIC, AND I BELIEVE THIS IS THE REASON IT WAS NOT INCLUDED IN THE RECALL. I HAVE CONTINUED TO HAVE ISSUES OVER THE YEARS WITH THE SRS WARNING LIGHT COMING ON, AND NOT BEING ABLE TO HAVE IT EFFECTIVELY REPAIRED. ACCORDING TO ONLINE FORUMS FOR OLDER CIVICS, THE SRS WARNING LIGHT APPEARS TO BE A COMMON ISSUE. THOSE OWNERS RECOMMEND DIY REPAIRS AND WORK-AROUNDS. GIVEN THE LIFE-SAVING IMPORTANCE OF AIRBAGS, HONDA SHOULD BE REPAIRING/REPLACING THEM AS NEEDED.

THE CONTACT STATED THE NEXT DAY HE SUSTAINED CHEST PAINS BUT HAD NOT SEEK MEDICAL ATTENTION. A POLICE REPORT WAS FILED. THE VEHICLE WAS NOT DRIVABLE AND WAS TOWED TO HIS RESIDENCE. I HAVE HAD MY SRS LIGHT ON SINCE I PURCHASED THIS CAR IN JULY OF 2017. MY SEAT BELTS DO NOT LOCK AT THE APPROPRIATE TIMES. I WAS ALSO INVOLVED IN A WRECK DRIVING IN RAIN ON THE INTERSTATE AND HYDROPLANED. I HIT A MEDIAN GOING ABOUT 50 MPH. NO AIR BAGS EJECTED FROM THE IMPACT. THE RESULTS OF THE IMPACT WERE A DENTED FENDER, REPLACEMENT OF THE DRIVER SIDE CONTROL ARM, NEW BALL JOINTS, AS WELL AS A DESPERATELY NEEDED TIRE ALIGNMENT. I FEEL IF THIS IMPACT WAS HARD ENOUGH TO BEND THE CONTROL ARM THEN MY AIR BAGS SHOULD HAVE EJECTED, BUT I'M NO MECHANIC. THANK YOU.

THE METAL CAP FROM THE DRIVER AIRBAG INFLATOR WAS BLOWN THROUGH THE DRIVER AIRBAG FABRIC AND INTO THE DRIVER'S NECK. THE DRIVER'S DEATH CERTIFICATE DESCRIBES THE CAUSE OF DEATH "COMPLICATIONS OF BLUNT AND SHARP FORCE INJURIES OF THE NECK". THE HOT INFLATOR MELTED A LOCATION ON THE DRIVER AIRBAG FABRIC. THE VEHICLE IS IN A STORAGE FACILITY IN PHOENIX, ARIZONA.

TAKATA AIRBAG ISSUE: I WAS IN A FRONT END COLLISION AT A SPEED OF ABOUT 20MPH ON A CITY STREET WITH NO ONE IN THE PASSENGER SEAT. THE VEHICLE SUSTAINED SIGNIFICANT FRONT END DAMAGE, RESULTING IN LOSS OF THE FRONT BUMPER, DAMAGE TO THE HOOD AND TO THE FRONT ENGINE COMPONENTS, INCLUDING LEAKING OF WINDOW WASHER AND TRANSMISSION FLUIDS. THE DRIVER'S AIR BAG DID NOT DEPLOY, BUT IN MY ESTIMATION, IS SHOULD HAVE DONE SO. I WAS NOT INJURED SINCE I WAS WEARING A SEAT BELT. THE AIR BAG IS A REPLACEMENT AIR BAG THAT WAS REPLACED BY A HONDA DEALER IN VIRGINIA, AT THE VERY BEGINNING OF THE TAKATA AIR BAG RECALL. I AM SIMPLY REPORTING THIS AS A POTENTIAL SAFETY ISSUE, SINCE I BELIEVE THAT THE REPLACEMENT AIR BAG SHOULD HAVE DEPLOYED AS NATURALLY AS A FACTORY INSTALLED ONE.

TAKATA RECALL: I ROLLED MY CAR ON MY WAY TO WORK IN AUGUST OF 2015. I HAD GOTTEN A LETTER 2 WEEKS PRIOR OF AN AIRBAG RECALL. I CALLED THE LOCAL DEALERSHIPS BUT DUE TO HIGH VOLUME I WAS PUT ON A WAITING LIST. MY AIRBAGS DIDN'T DEPLOY AND MY DRIVER'S SEATBELT DIDN'T LOCK. I SUFFERED A FRACTURE TO THE BASE OF MY SKULL AND A TRAUMATIC BRAIN INJURY. I COULDN'T WORK AT ALL FOR 9 MONTHS AND COULD ONLY WORK 10-15 HOURS A WEEK FOR SEVERAL MONTHS AFTER THAT. A WEEK AFTER THE CAR ACCIDENT I WAS FIRED FROM MY JOB WITHOUT SEVERANCE PAY AND I CALLED HONDA HEADQUARTERS AND THEY TOLD ME TO KEEP CHECKING FOR CLASS ACTION LAWSUITS. I WAS DRIVING ON A BACKROAD GOING 40 MPH AND I HIT SOME LOSE GRAVEL. I OVER REACTED AND TURNED THE WHEEL CAUSING ME TO MAKE A U TURN. I HIT MY BREAKS HARD TO STOP AND MAKE SURE MY SEATBELT WOULD LOCK. THE MOMENTUM OF STOPPING CAUSED MY HONDA TO ROLL. SINCE THE AIRBAGS DIDN'T DEPLOY MY FACE REPEATEDLY HIT THE STEERING WHEEL AND BECAUSE MY SEATBELT DIDN'T LOCK MY HEAD SMASHED THE ROOF OF THE HONDA REPEATEDLY.

TAKATA RECALL----CIVIC HAD UNCEASING LIGHTED SRS LIGHTS ON DASHBOARD DURING MONTH OF JUNE 2014. HONDA MECHANICS INFORMED ME THAT THE CONDITION MEANT THE AIR BAGS WOULD NOT INFLATE IF A NEED OCCURRED. REPEATED VISITS TO HONDA DURING WHICH THEY KEPT CAR FOR 2 DAYS AND THAN 3 DAYS. THEY MADE ME PAY BILLS OF $292.56 AND $705.19 FOR A NEW SRS COMPUTER SYSTEM SO AIR BAGS WOULD WORK

THE DRIVER AIRBAG WAS SAID TO NOT BE PART OF THE RECALL AND THE ORIGINAL WAS REINSTALLED INTO THE VEHICLE. ON 3/10/2016, AN SUV DROVE OUT FROM A SIDE STREET WITH STOP SIGN AND OUR VEHICLES COLLIDED. THE SUV WAS STRUCK ON ITS LEFT SIDE TOWARD THE FRONT END AND MY VEHICLE WAS STRUCK ON THE FRONT END TOWARD THE RIGHT SIDE. THE CONSUMER WAS HOPING, THE RECALL ADDRESSED THE SOCKET, BUT HE STATED THE MAIN PROBLEM WAS AT ANOTHER CONNECTION. ALSO, THE AIR BAG LIGHT ILLUMINATED AT THE BEGINNING OF THE YEAR. IN THAT CASE, THE AIRBAGS WERE IN DANGER OF NOT DEPLOYING AS WELL IF SOMEONE WAS OUT OF POSITION IN THE SEAT. IT SEEMS TO ME THAT THERE IS AN ISSUE WITH CIVIC'S SRS SYSTEM AND THE SEAT BELT BUCKLE SWITCH IS AN ONGOING AND RELATED ISSUE AS WELL. SEAT BELTS ARE COVERED UNDER A LIFETIME WARRANTY BUT FOR SOME REASON THE BUCKLE SWITCH IS NOT. IN ANY CASE, I'M WONDERING WHY THERE HASN'T BEEN A RECALL ON THE SEAT BELT BUCKLE SWITCH WHEN IT IS A PREVALENT PROBLEM WITH THIS VEHICLE. WHEN I PICKED UP MY 2002 HONDA CRV, THE SERVICE ADVISOR SAID IF THE SRS LIGHT COMES BACK ON, LET THEM KNOW. THE FOLLOWING DAY, IT AS BACK ON AGAIN. HOW DID THEY KNOW IT WOULD BE BACK. WHEN I CALLED ABOUT THE SRS LIGHT, I WAS TOLD THAT THE CONTROL UNIT HAD PROBABLY FAILED AND WOULD NEED TO BE REPLACED, BUT THAT THEY COULD NOT DO THAT UNTIL THE BATTERY HAD BEEN CHANGED. THEY ALL COULD TRIGGER THE THE SENSOR INOPERABLE....MAKING THE AIRBAGS NON-FUNCTIONAL. HOW CAN A CAR BE DESIGNED WITH SUCH A DEFECT. ANY PASSENGER WITH A CELL PHONE IN THEIR POCKET CAN SET THIS OFF. BOTH OCCUPANTS WERE TAKEN TO THE HOSPITAL FOR EVALUATION. HOW DOES THE ENTIRE FRONT END OF A CAR CRUSH UPON IMPACT AND THE TWO FRONT AIR BAGS DON'T DEPLOY! WHILE DRIVING AT 30-35 MPH CONSUMER WAS TRYING TO SLOWDOWN AND WITHOUT ANY NOTICE TWO FRONT AIR BAGS DEPLOYED. CONSUMER THEN RAN INTO A TREE. CONTACTED DEALER, AND DEALER WAS NOT WILLING TO DO ANYTHING. THERE WAS SOME INJURIES TO THE CONSUMER'S HANDS AND BACK. *AK
MY 2002 HONDA CIVIC EX HIT ANOTHER VEHICLE SQUARELY IN THE REAR END WHILE TRAVELING APPROXIMATELY 15 M.P.H.. NEITHER OF THE FRONT AIRBAGS DEPLOYED! I HAD OWNED THIS VEHICLE FOR APPROXIMATLEY 7 WEEKS. THE COLLISION REPAIR CENTER COULD FIND NOTHING WRONG WITH MY AIR BAGS. THE TOW TRUCK OPERATOR AND THE COLLISION REPAIR CENTER TOLD ME THAT THERE WAS SOME SORT OF ALERT OUT FOR 2001 & 2002 HONDA CIVICS WHERE THE AIR BAGS DIDN'T DEPLOY AFTER A FRONT END COLLISION. I HAVE NOT BEEN ABLE TO FIND A WEB SITE TO CONTACT HONDA WITH THIS PROBLEM. MY AUTO IS BEING REPAIRED BY G & M BODY SHOP, 2207 EAST 6TH, STILLWATER, OK 74074. THEIR PONE NUMBER IS 405-377-0430. ESTIMATED COMPLETION DATE OF REPAIRS IS FEBRUARY 10TH.*AK
I EXITED THE FREEWAY BEHIND ANOTHER CAR. WE WENT OVER A SMALL HILL. THE CAR IN FRONT OF ME MOVED SUDDENLY, SWERVING TO THE RIGHT LANE. AT THAT TIME, IT REVEALED TO ME THAT THE TRAFFIC WAS STOPPED. I ALSO BRAKED AND SWERVED RIGHT BUT WAS NOT ABLE TO STOP IN TIME DUE TO THE SUDDEN EMERGENCY. I HIT THE CAR IN FRONT OF ME GOING ABOUT 40MPH. I DO NOT THINK THAT MY SEATBELT LOCKED. MY AIRBAG DID NOT DEPLOY BUT MY PASSENGER SIDE AIRBAG DID. I HIT THE STEERING WHEEL VERY HARD! I HAD TO BE TRANSPORTED TO THE HOSPITAL. I HAVE SEVERE CHEST CONTUSIONS AND NECK AND BACK SPRAIN/STRAIN. MY INSURANCE CO BELIEVES MY CAR MAY BE A TOTAL LOSS BUT MY AIRBAG DID NOT GO OFF??? AND BECAUSE IT DID NOT GO OFF, I WAS INJURED. HONDA WAS CONTACTED THE NEXT DAY. NINE DAYS LATER, THEY SENT AN ADJUSTER OUT TO INSPECT MY CAR (BUT NOT AN ENGINEER). I HAVE BEEN TOLD THERE IS NO REASON TO SEND AN ENGINEER OUT THE ADJUSTER SEEMED TO BE MORE CONCERNED WITH INVESTIGATING ME THAN WHY THE AIRBAG DID NOT DEPLOY. I WOULD LIKE TO KNOW WHY IT IS THAT I DO NOT HAVE A FRONT END TO MY CAR ON THE DRIVERS SIDE AND MY AIRBAG DID NOT GO OFF, BUT THERE IS NO DAMAGE TO THE FRONT PASSENGER SIDE AND THAT AIRBAG WENT OFF!!! THE ADJUSTER THAT HONDA HIRED TO INVESTIGATE THE CLAIM IS DALE S THUMANN, CPCU FOR CSB GROUP. HE SAID THAT IF I DO NOT GIVE HIM MY HEIGHT, WEIGHT, AND SOCIAL SECURITY NUMBER THAT HONDA WILL JUST DENY MY CLAIM FOR LACK OF COOPERATION. I AM NOT WHAT SHOULD BE INVESTIGATED THE CAR IS! HE WAS NOT WILLING TO EVEN TAKE THE STEERING WHEEL COVER OFF TO SEE IF THERE WAS EVEN AN AIRBAG IN THE DRIVER SIDE! HE SAID HE WAS SURE THAT WAS AN AIRBAG IN THERE ... "THEY MIGHT MISS ONE EVERY ONCE IN A WHILE. THEY MAKE MISTAKES. " (BUT HE STILL DID NOT FEEL IT WAS NECESSARY TO HAVE HIM OR ANYONE ELSE CHECK TO SEE IF ONE WAS THERE OR NOT).
