Dear NHTSA, There needs to be an immediate recall on this (VIN#: [XXX] ) 2021 Honda CR-V Hybrid Motor Nightmare. Despite proper maintenance and care, the vehicle has experienced persistent problems that stem from known defects. The engine has a fatal flaw with the head gasket. The high compression turbo engine stretches the head bolts, lifts the cylinder head just enough to blow the gasket causing coolant to leak into the cylinders and mix with the oil and destroys the bearings. Along with oil dilution where gas washes pass the piston rings. This is a widespread problem affecting this model as indicated by the numerous technical service bulletins. The issue appears to occur during the 20,000 to 30,000 mile mark where it begins to experienced recurring engine performance issues, misfires, loss of power, and excessive radiator fluid consumption resulting in premature engine wear and sludge buildup. Honda sites have pointed out: "This can happen when the car is frequently idled or driven for short commutes, which can prevent the oil from flowing properly" and my commute is a 3 minute drive and 5 minute to the market. [XXX] "it’s a well documented issue and Honda America is aware of it that they’ve released an article about it for mechanics" One customer wrote "I have a 2021 Honda CRV and the engine had a terrible knock" [XXX] Please do something immediately! Several Mechanics acknowledge these widespread issues, the Lane Manager that advised me to reach out to the manufacturer pointed out that this is a “common issue”, and Your Website highlights that you all have already received similar recall campaigns for the engine oil and gasoline dilution issue that occurs approximate at the 22,500 mileage. My dash board lit up and started dinging. Many different messages were flashing across the screen such as driver assist feature problem, TPMS problem, and others related to the electronic driver assist features. I noticed a difference in the way my car handled as I was pulling off the highway to a gas station to check my tire pressure. It seems drivable at the moment and I don't have another vehicle option. It's a 2021 CRV.

Phantom emergency braking. Vehicle slamming on brakes at intermediate times when there is no emergency.

The contact owns a 2021 Honda CR-V. The contact stated that while parking, and the brake pedal was depressed, the vehicle accelerated inadvertently up a slight incline and collided with a stone boulder/wall. Neither the contact nor the passenger sustained injuries. The contact stated that no collision warnings were activated to detect the collision. The air bags did not deploy. The vehicle was towed to a dealer, where the failure could not able to be replicated. The vehicle was then towed to an independent mechanic, where the vehicle has yet to be diagnosed. The manufacturer was not notified of the failure. The failure mileage was approximately 56,000.

About a year ago, I was driving at speeds of approximately 10-15 mph when my car braked and immediately brought me to a dead stop despite no obstacles being present in the roadway. It appears the automatic emergency braking system malfunctioned. Had there been a car behind me, it likely would have caused an accident. On August 15, 2025, I was driving on the freeway using the adaptive cruise control when traffic in front of me slowed, engaging the automatic emergency braking. Traffic in front of me cleared almost immediately but my car was still very rapidly decelerating. I pressed the gas pedal, but the vehicle continued to bring me to almost a complete, immediate stop on the freeway (I had been traveling at 70-80 mph prior to the stop). I feel incredibly fortunate that I did not end up in an accident due to the traffic behind me and that no one was injured or killed. I pulled off onto the shoulder when it became clear that I was not going to be able to quickly regain control of my vehicle. Driving a vehicle without being able to properly control it was an absolutely terrifying experience. I was bracing myself for impact from behind as I desperately tried to regain control and accelerate back up to the flow of traffic. I am certain that this malfunction will result in deaths if not addressed. On August 23rd, 2025, I was driving on a highway at approximately 60 mph when the car in front of me slowed in preparation for a right turn off the road. I had disabled the CMBS feature at the beginning of my drive, yet the car still braked without me hitting the brake pedal. It was not an immediate, abrupt brake as I had experienced in the other two previously described incidents, but it was still highly unnerving to feel another loss of control of the vehicle, especially since the system was supposedly disabled.

• During the short time I have owned this vehicle the automatic emergency braking system has engaged 3-4 times when there was no object in front of the vehicle. The vehicle can be made available upon request. • All incidents happened when driving on the freeway. Fortunately no one was following me which could have resulted in my being rear ended for a sudden stop. • I took the vehicle into the dealership service center today (8/8/25) where they told me it is normal and I should just be a good driver, keeping my distance from vehicle to the front of me. They stated that if they were to replace it the same thing would occur. The problem has not been reproduced by the service center. • When these issues have occurred on one occasion the braking alert sounded ( beeping noise) and then the car started braking. On the other occurrences I do not recall the alert. The car just started braking. Other than that there were no warnings it was going to happen.

The CMBS system engaged abruptly, causing the car to decelerate rapidly and SLAMED BREAKS automatically. There were no vehicles, pedestrians, or obstructions ahead. This unexpected braking could have easily resulted in a rear-end collision, endangering me and other drivers. I have kids in the back sit and the car behind us almost hit my car. I am afraid the result might have been much worse if there was TRUCK behind my car. I took the vehicle to the Honda dealership, but they were unable to replicate the issue and did not offer a fix. They also mentioned that unless the problem could be duplicated, any diagnostic or repair fees would not be covered, despite this being a known issue under investigation by the NHTSA. I am aware that the NHTSA has an open Engineering Analysis (EA24-002) into this very issue affecting millions of Honda vehicles. I’m deeply concerned that this problem continues to put drivers and passengers at risk and believe a formal recall is necessary. I request that this issue be escalated and that Honda be held accountable for a fix before someone gets seriously injured or killed. The contact stated that while her daughter was driving at an undisclosed speed, the Collision Mitigation Braking System (CMBS) independently activated, causing the vehicle to suddenly come to a stop, and the front driver’s and front passenger’s side seat belts to independently lock. The contact stated that the brake warning light was illuminated. An appointment was scheduled with the dealer to inspect the vehicle. The vehicle was not repaired. The contact related the failure to NHTSA Action Number: EA24002 (Forward Collision Avoidance). The manufacturer was notified of the failure. The failure mileage was approximately 30,000.

My 2021 Honda CR-V has experienced incidents where it suddenly and unexpectedly applied the brakes while I was driving, despite: - No vehicle, pedestrian, or object is present in front of me - No driver input on the brake pedal - Not using cruise control at the time My car occasionally jerks or shows warning lights before stopping, sometimes without any warning. I suspect an issue with the Collision Mitigation Braking System (CMBS) or related driver-assistance components. These braking incidents occurred under normal driving conditions and pose a high risk of being rear-ended. My car's issues remain unresolved, and I feel unsafe while driving my vehicle. Two dealerships conducted four diagnostic tests (on 18, 19, 21, and 24 May 2025) to evaluate the Honda Collision Mitigation Braking System (CMBS) and the associated driver-assistance components. But no issues were identified during these evaluations. - 23 May 2025, I discussed my car’s issues with a Honda Case Manager (in California). He did not accept any responsibility for the reported Honda braking issues. - 27 May 2025, I filed a formal complaint with Honda Customer Service. Honda has not taken any action to address my car’s issues.

I was driving down a main road and with no cars in front of me, my Automatic Emergency Breaking came on, flashed Brake and then brought my car to an immediate stop. I made an appointment at my dealership. The dealership said there were no recalls and said they would charge me $50 to try to recreate the issue but they didn't think they would be able to recreate it.

I had 2 unrelated issues in the past 2 days: 1) rear door actuator malfunction. The door won't lock or unlock as part of the central locking system 2) The "break" alert went off without any cars around. Happened once, breaks weren't activated by the car.

Driving on the highway going 55 mph when the car in front of me in the left lane more than 5 car lengths began to break, recognizing this I put my blinker on and tried to switch to the right lane when suddenly the automatic breaking kicked in and slowed the car down to a near stop. I was unable to deactivate with the break or the gas for a few seconds and by that time I was stuck between the lanes nearly getting t-boned in the right lane before the car swerved to avoid my vehicle. Had there been someone behind me in the left lane I would have surely been in a terrible and fatal accident. I was in complete control of the car and received no more than a one second warning (in the form of a short beep) that the automatic breaks would kick in suddenly. This put myself and my passenger in serious danger. This is not the first time this has happened and has happened as I have approached an exit on the highway as well. Again, fortunately no one was driving behind me that time as well. There is something seriously wrong and I no longer trust the safety of my vehicle. There is also no way for me to turn this feature off until there is a formal recall issued. The dealership says there is no active recall therefore I will need to pay $175 to get the issue checked out.

On several occasions since I’ve owned the vehicle, it has come to a complete stop while driving at highway speeds without any apparent reason, accompanied by a warning of a possible collision. On one such occasion, the vehicle stopped so abruptly that I initially thought I had been rear-ended, though thankfully no cars were near me at the time. If other vehicles had been nearby during any of these incidents, I believe it could have resulted in a serious accident. While I am grateful for the technology, as it has also helped me avoid an accident on two separate occasions (different from the above referenced instances), I believe the system needs improvement to prevent these unexpected stops from occurring.

1) On 3 occasions the Automatic Emergency Braking (AEB) has self activated and the car attempted to come to a complete stop. The 1st incident, 10/19/24, happened while driving 35-40 mph while changing lanes, a red BRAKE light and alarms sounded bringing the car to a complete stop. While there was traffic around me, no one was directly in front of me or close enough to have required this to activate. A vehicle traveling behind came inches from rear-ending me. The 2nd incident, 11/9/24 concerns me the most, happening 3 weeks after the first incident while on the highway traveling 65 mph. I was in the left lane of multi-lane traffic. While there were vehicles two lanes to my right, there was no one directly in front of me by 100 yards. Again, alarms, red BRAKE light, and the car proceeded to come to a complete stop. The car felt like it was floating and for a moment, I felt like my life was over as the car was out of control. The 3rd incident, 12/9/24 happened 9 days after it was in for service, while traveling 30 mph in town. 2) The 1st incident avoided an accident because of the alertness of the driver behind me. The second incident, at 65 mph, is of the most concern, as for a few moments, I felt like my life was over. This incident is also causing me to have PTSD symptoms while continuing to drive, it has required me to see an MD. 3) An appointment was available about 3 weeks after the second incident. They were not able to replicate it or find anything wrong with the vehicle. After speaking about my concerns after the 3rd incident to the dealership, they have recommended that I turn off the safety equipment. While I have spoken with Honda America, they do not want to get involved unless there is a recall by you, the NHTSA. Nothing has been reported to the police but I have spoken to my insurance agent regarding liability if I turn the safety equipment off. 4) BRAKE light and alarms occurred at the same time as the braking.

On at leas 8 occasions in the past 3 years the Automatic Braking System (Forward Collision Avoidance System) engaged with audible warning while on the interstate driving at highway speeds unexpectedly with no vehicles or other obstructions in my lane in front of me. The majority of occurrences were as I was attempting to pass in the left lane, but on at least 2 occasions there were no cars to the left or right of me. The sudden braking could have caused a serious collision. I have not brought the problem to the attention of a dealer or independent service center as I doubt they would be able to reproduce or confirm the issue, as I have experienced before on intermittent issues before. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear

While traveling on the highway at 65 mph, in the left lane of three lanes of traffic with no cars in front of, back of, or side of me, (AdaptiveCruiseControl and LaneKeepAssist were both on) when the vehicle did an “emergency” self-brake. A large red BRAKE sign appeared in the dash and the car felt “out of control.” I physically hit the brake, slowed down and exited the highway. I REMAIN FOR ME TO FEEL VERY INSECURE on highway travel. This was a second incident. This occurred early November. The first incident happened a few weeks before this one while traveling around 40 mph in multi-lane traffic. I signaled to move into the left lane, no cars were within two car lengths from me, when the vehicle did a hard self-brake; the ACC and LKAS were NOT engaged at this time. Again, the car seemed “out-of-control” for a short time. This incident occurred mid October.

I was driving on a straight street doing about 35 MPH and my car all of the sudden braked. Luckily there was no one behind me, had there been, I would have definitely caused an accident. I contacted a Honda dealership and they said there are no recalls on my vehicle, so could only make an appointment, try to recreate the issue and run a diagnostic, but they said they never heard of the issue. I had no issues or warning lights prior too. My car was inspected in June and I just recently had the oil changed and brakes checked. I take the highway to work and I'm very afraid if this happens at highway speed what would happen.

I have experienced at least 4 occurrences in the last year of the automatic emergency braking system activating when there were no obstructions while crossing a bridge. I have also experienced at least 2 occurrences of the automatic emergency braking system activating while going under an overpass. I live in interior Alaska and in the most recent occurrence I was going over an icy bridge in the dark in below zero temperatures when the car automatically applied the brakes for no reason. Brakes need to be applied carefully on icy roads - had someone been behind me or had I lost control of the car because of the unexpected braking, this could have resulted in a serious accident.

I was driving on the street with no one in front of me going about 45 miles per hour when the car did "Phantom Braking" and the car tried to stop on its own. It beeped 3 times and BRAKE! flashed on my dashboard as if I was going to hit someone and the car overtook my brakes and tried to stop. There was no one in front of me. The very next day, it did it again. This is extremely dangerous and can cause an accident if someone is driving behind me and the car suddenly decides to stop on its own.

The car's forward collision warning system is defective. The car will randomly bring up "BRAKE" on the dashboard with nothing even close to the car. This has started happening many times over the past few weeks, with even at least one instance of the car braking on its own. I brought this up to the dealership during my last oil change recently, and they inspected and looked into it. They said there was nothing wrong with the sensors/system and they documented it (specifically saying no problem found vehicle is operating normally at this time). However, a day or two after the dealership, the problem returned and it continues to persist. In addition to this, the blind spot warning system also randomly goes off when nothing is nearby, but not as often as the braking system, which is more dangerous in my opinion.

The forward collision warning and braking has activated a couple of times without any car front of me for a wide distance or when everything is all clear. It also happened a couple of times when turning a curve or exiiting, also without any car in front of me. The only thing I could notice was the dashboard flashing but I could not read it because I had to focus on the road.

I was driving on the middle lane of a major highway, going around 70 mph, when a plastic grocery bag floated in front of the car, at a height of about two feet off of the ground. My vehicle suddenly slammed on the brakes. It slammed hard and I could not make it stop. I tried to push the brake but it was already down. I was nearly rear-ended and could have easily caused a multiple car accident. I may have been using the cruise-control at the time, but I'm not sure. I do not recall if the vehicle flashed an orange "BRAKE" warning. I did not take the vehicle to a dealer to try to recreate the problem.

My vehicle lost brakes as I was driving. The dealer stated it was because of wiring problem but I read where crv were having a problem with the accident mitigation system. They replaced my wiring harnes to my rear view mirror. The dealer couldn't explain why I lost power to my brakes.

I was driving on a 2 lane road. There were no other vehicles on the road. The word brake flashed on the dashboard and the car suddenly braked hard. Items on my back seat fell to the floor. The car then resumed normal driving. There was no wind or items blowing across the path of the road. This has not happened before. If there had been a car behind me it might have caused an accident. The vehicle has not been inspected by any entity.

I have had issues with the automatic braking when the car "thinks" there is something in front of me and slams on the brakes. 4 out of 5 times there was nothing there. 1 of those times was during a rain storm, so that time I chalked it up to the radar being wet. I have also had several instances when the car flashes "BRAKE" when there is no reason for it. Similarly, when in reverse, the car again "thinks" there is something behind me and beeps when there is nothing within 50 feet of my car. I keep my cameras and radar clean at all times. Before each trip, I wipe them clean.

A few times (4-5 times) the car activated the automatic emergency braking feature while I was driving. There were no obstacles, vehicles, or people in my path. The system simply activated. The cars behind me nearly crashed into me.

No vehicle was around me that time and I check my car surrounding and it was clear. Incident happen multiple times which I did adjust my sensor to low distance but still happens.

Since new and on numerous occasions, the vehicle will slow down drastically for no reason. No vehicle in front or object close to either side, it just brakes automatically. It only happens when the vehicle’s cruise control is on. On March 7th 2024 I was on the highway with the cruise control set at 75MPH. The vehicle for no reason slammed on the brakes and tried to come to a complete stop. No skidding or loss of control due to the ABS, but if had not stepped on the accelerator, it would have. I went from 75MPH to about 20MPH in seconds. Thankfully, the driver behind me reacted and went around me on my right side. I there was another vehicle to my right, I am certain, I would have been re-ended. I took the vehicle to the HONDA dealership and was told they were not aware of the problem, but they could attempt to fix it. I have lost confidence in driving the vehicle with the cruise control engaged.

Took my Honda CRV 2021 EX in because all of my warning lights (Critical Failure) were going off as I was driving. All safety features were failing including AWD control, braking, sensors, cruise control, etc. They replaced the battery and tested the alternator. They reset the system and said it was fixed. I picked mine up today, drove it for 10 mins on the highway and then hit the "MAIN" button on the stearing wheel to turn on the cruise control. As soon as I did that, ALL of the warning lights came on at the same time and the systems started to fail again. This is a hazard and could cause an accident at any moment. In reviewing the Honda forums, this is a massive problem with hundreds of Honda CRV EX 2020-2022 models. I'm 7000 miles over my warrant and Honda won't look at it. However, it is directly their problem and my car is only 2 years old.

Since purchasing the car new, there have been four instances in which the collision sensor has falsely detected object of vehicles that are no where in sight. The "BRAKE" warning flashes and then the car brakes hard, sometimes to a complete stop when no object/car are beside/in front of me. These have happened going at speed between 25-50mph. This is very dangerous because anyone behind could collide with me from behind and put my family in danger. These event happened at 10,000 miles. 20,000 miles, and up to 60,000 miles.

I would think this issue would be one of a very serious safety concern, but I suppose the dealer feels not. They did however tell us to turn that system off. Seems counter intuitive to have a safety system yet told to turn it off and wait about 2 weeks to be seen in the shop.

On two separate occasions on local roads while driving under 15 miles an hour I was drawing near an intersection and the car in front of me was actively turning onto a side street and I had ample distance from that car and my car suddenly decided I was about to get in an accident and slammed on the breaks. Its a good thing the cars behind me were responsible and were able to break in time or there would have been an accident both times because no one would have expected me to suddenly break in either situation. There were other people in the car each time and we were all surprised.

A car was way in front of me and had turned right into a driveway, before I was even a close and it stopped in the driveway entrance out of the road. I had my car turning away from the driveway knowing the car was moving slow in front of me. When I reached to pass this my ABS kicked on and brakes locked by itself and scared the pants off me!! My hands came off the steering wheel, my neck jerked forward and my glasses flew off my face at 50 MPH!!! I wasn’t even close to hitting this person. A few days later driving down the road a goose was on the same side right side on side of road as I’m rounding the bend my brakes lock up again!!! Again. The bird wasn’t even in the road!!!!! I had to pull over to read how to turn off this auto braking. I had to go to emergency room days later because my neck and shoulder pain was worsening. I went to ER last night. 3/23/23. Thank god just muscle damage. I was scared it was something worse! It’s just awful. Sometime you can see it say brake and nothing is in front of you. So I make sure I turn it off now. First incident was I think 3/12, when my car was doing 50 mph, second with the goose was 35 mph on 3/18.

My cars automatic braking system engages when nothing is around. This has happened multiple times. One of these time I recall traveling with my child at a sped of 50-+mph. It was fearful for the both of us. This alone is dangerous, it could become deadly given different weather conditions causing loss of control of the car. I believe this issue will eventually cause someone injury and/or death. During this time my child was recovering from traumatic brain trauma. The dealership is waiting on a part the they feel should repair the issue. The emergency brake light flashed, beeped and slammed on breaks. I believe this is more than a sensor issue I believe it’s an electrical issue. The lights sometimes brighten instead of deeming in on coming traffic, there have been times that the usb port don’t work an then times that they do with the same charger, the windows an sunroof open on it’s on, the volume will max out an not turn down or off, the pixels on the screen become broken up(I have pictures) also when the car is idling the rear exhaust smalls like fumes from spray paint or something similar. I bought the car in May 2021 w/5miles - March 2023 w/about 7,500miles, I’ve had to call roadside assistance 3/4 times. I am a single mom with a young son who suffers from 4 rare diseases. I am unable to run out an get a new car. If these issues fix I’ll be on my own in the next 13 months with unnecessary expenses.

2021 Honda CR-V with about 17,000 miles. On at least 3 occasions, the vehicle activated the emergency brake warning and applies brakes, on 2 of the occasions, driving in a straight road with no vehicles or persons in sight. On another occasion, the brakes were also applied in the highway reducing the speed dramatically and almost causing an accident behind me, there were no cars nearby in front but on the lanes adjacent to our driving lane.

On two occasions now, the Automatic Emergency Braking has engaged without there being an actual danger, meaning nothing in front. The first time I was driving at about 55mph on the highway and it engaged with a dash alert to brake and then proceeded to decrease my speed. Luckily, no cars were behind me or else I would have been rear ended. The second time, I was going 5mph at a grocery store parking lot and it did the dash brake alert and stopped. Again, luckily no cars were behind me. Honda will be inspecting the car this coming week but did not seem worried about my initial report. It wasn't until my second incident of 5mph that they suggested taking a look at it.

My car has done this phantom braking multiple times before, however this time it managed to pull my car into a curb and pop my brand new back tire and ruin my front tire. This is a huge safety issue because if there was anyone in front of or behind the car could’ve caused a major collision. I have done some research and noticed this has been an issue on previous years of my vehicle. I feel unsafe driving my car and my dealership has basically told me there is nothing than can do and that is not acceptable with me. The fact that my life was put in danger due to a malfunction with the forward collision warning is absolutely ridiculous. I will probably never buy another Honda if something is not done to correct the issues.

Twice on 10/10/2022, the automatic brake warning system engaged while the car was in cruise control at approximately 55 mph. There were no other vehicles or safety hazards in the lane of travel that should have prompted a brake system activation. The incidents happened on separate trips, with the car having been parked and turned off in between.

The vehicle braked very hard and had there been a vehicle behind me I suspect there would have been an accident. This is not the first time this has occurred - it has happened several times before - but it was the most severe braking that has occurred. There was no damage, so the police were not involved. I have not yet reported this to the manufacturer.

We had the adaptive cruise control on at these times. The highway was clear of obstructions and was in perfect shape. The car was clean on the outside. It was a bright sunny afternoon and the road was dry. We recall no warning as this happened. We had just purchased the vehicle used with 11000 miles on it.

I have had 4 separate instances where the automatic braking system abruptly applied the brakes. Two of them happened at a place on the road where the pavement color changed. One was a pedestrian crosswalk painted on an asphalt road surface. Another was where a new concrete roadway changed to new asphalt. Those 2 situations led me to believe the system may see road color change as if there was an obstacle in front of the car. However, 2 additional events happened with no obvious reasons as the road surface did not change. This is a 2021 Honda CRV. I have not taken it to a Honda dealer nor have I discussed it with Honda. I was fortunate that nothing was close behind and the road surface was not slippery. There is no telling what might have happened if the road was snow packed or icy.

I was driving at approximately 35 MPH on a straight, flat road. There were no cars in front of me, behind me, or oncoming. Without warning or any action on my part, the vehicle wheels locked up, the vehicle jerked to an abrupt stop, and the display went black. I noticed what appeared to be a red block of text on the display, but I was unable to read it before the vehicle started moving again. Luckily, I had both hands on the steering wheel, and I was alone in the vehicle. I was shaken up at the abruptness of the stop. If I had my dogs in the car, they would have been launched forward. If I or anyone else in the vehicle was not belted in and facing forward, there for sure would have been injuries. If there were someone behind me, there would have been impact. I am grateful none of these things happened, but I remain fearful of driving this vehicle if it were to happen again. I brought the vehicle immediately to my dealer for inspection. (Balise Honda, West Warwick, RI). The service manager stated this was a know issue with CR-V's, a result of the radar on the front of the vehicle that controls the ABS malfunctioning for some reason. He was unaware of a solution. His only recommendation was to turn off the ABS and LKAS, the very safety features that I purchased this car for, and paid extra for. I reported this to Honda on July 5, case number 13119359. They were to get back to me with an update by July 12, but I have yet to hear from them. The case manager only said there were no know recalls or bulletins on this issue. I had requested they open a bulletin as this is certainly a safety issue that needs to be addressed. For now, I am driving my vehicle without the safety features this vehicle advertised, and have limited my travel with this vehicle as well. NOT what I expect from a new 2021 vehicle.

I also started to brake, but allowed the gap between us to close somewhat due to a vehicle following closely behind me. The Honda’s emergency braking system suddenly took over with MAXIMUM braking, causing items in my vehicle to fly around AND the vehicle behind me to nearly rear end me. I had NO control over the braking. My wife screamed at me, thinking that I had slammed my foot on the brakes, but it was beyond my control. I’ve asked my Honda dealer if the system can be adjusted but I was given no help. I understand the emergency braking system’s intended purpose, but the manner that this system has engaged (3 times) is unsafe. It’s not like I wasn’t paying attention, I was, and I was already braking. It’s actually scary the way it locked up. Yes, I know that the brakes didn’t actually lock up, they are antilock brakes, but it was like I was standing on the brake pedal. Driving on empty road about 40 mph. Brakes engaged momentarily and red brake light came up on dash. Happened in daylight with excellent visibility. No cars in front or other lane and nothing on roadway. Car has 1040 miles on it. Will report to dealer but expect little resolution.

I'm trying to report an incident that involved my Honda CRV running into a totally stopped Tesla in the HOV lane on I-75 in Atlanta Georgia on Sunday, May 8, 2022, at around 9 AM. We came around a large curve and had no time to avoid running into the TOTALLY STOPPED Tesla in the HOV lane. I have seen reports of "phantom braking" in Teslas. The driver of the Tesla stated to us that he was using cruise control and when the Tesla sensed a car pulled off into the breakdown lane, it stopped completely and there was nothing he could do. Our car sustained serious damage. Fortunately, we were not injured, but I believe NHTSA should be investigating Tesla for these incidents. If this is not the correct form to report this, please advise. There was no problem associated with my CRV in this incident.

I was driving on a divided 4 lane road that offers a wide right side shoulder at around 43 MPH. I was traveling in the right lane. The lane markings are fresh and visible. It was late afternoon and sun was shining. The road surface was dry. There were no vehicles immediately in front of or along the side of mine. I had not activated the lane keeping assist or automatic cruise control features. Only the hazard mitigation/automatic braking and road departure mitigation features were operational. The car suddenly attempted to swerve left, away from the center of the lane being utilized. The automatic braking system also activated. I was able to simply "drive through" the malfunction without incident. This was the first instance I have noted of this kind. Due to the fact that it was late afternoon, there were shadows from overhead power line being cast onto the lane being driven. I have owned previous cars that sometimes attempted to track said shadow when the lane keeping assist is activated. That was not the scenario yesterday as LKAS was not selected. It was as though the sensors detected a pending pedestrian strike or other collision.

On more than one occasion while driving the touch screen went black couldn’t access my navigation nothing couldn’t get the screen to turn on it wouldn’t do anything. Then it will tell me connect device when my phone is plugged in I’ve changed the cords replaced them with generic apple cords nothing bought apple cord and still nothing, apple car play doesn’t connect says you need to unplug device for car play to work nothing. The sensors are not working like they should there will be nothing around the car will alarm brake then it will automatically brake. I should not have to at to replace the touch screen in my vehicle it’s under warranty well under the miles as well. I work at a car dealership and the touch screen issue is a big problem I get several complaints about the touch screens not working.

My 2021 Honda CRV is activating the brakes at inappropriate times. I've had 5 total instances that could have resulted in a rear-end collision both on the street driving 5-35 mph and freeway at 30-50 mph. The system on the dash board flashed "brake." However, there is more than a car space between the car in front of the car. The braking system activates and I am unable to counteract the brakes locking up on the car. I am very concerned that if I were to continue to drive my car , it may result in an accident with a car crashing into the back of my car. At this time I am waiting for the vehicle to be inspected.

The contact owns a 2021 Honda CR-V. The contact stated that while driving at various speeds, the vehicle would independently downshift without warning. The contact also stated that the failure would occur while the cruise control was activated. In addition, the contact also stated that the lane-keep assist would fail to differentiate between road salt and line marking on the roadway, resulting in inadvertent sudden downshifting. The contact's husband had taken the vehicle to a dealer where their concerns were ignored and no diagnostic test was performed. The manufacturer was not notified of the failure. The vehicle had yet to be repaired. The failure mileage was approximately 100. In addition to the above, the lane keep assist cannot distinguish between lines & cracks in the road, which is a big deal in Missouri, and an issue I fight with daily on roads I have no choice to travel on. And it's not downshifting that happens with the lane keep assist-it tries to correct, but the corrections if I don't fight them will either run me into the opposing lane or off the road.

I have a 2021 Honda Crv Awd. On 11/14/2021 the Forward Collision Avoidance System engaged . I was stopped at a red light and when the light turned proceeded in 30 feet the system engaged. There was no car or pedestrian in front of me. The car beeped and brakes engaged. Luckily the car behind me was able to stop. Like another car with the same problem brought it to the dealer and was told they couldn’t find anything was wrong. They did lower the sensitivity of the sensors. The also said that a leaf could set it off. My wife is not comfortable riding in the car now. She fears that it could happen on the HiWays.

Purchased car on July 7, 2021, that same day the car did not start when stepped on brake and pushed start button. Happened again on July 8, 2021. Called Dealer, Took it in on July 9, 2021. They could not duplicate problem. Assured me it was not a problem. Drove to CT on July 10, 2021, Did not start several times during the trip. On July 11, on return trip from CT a warning light came on dashboard. IN black letters in a red rectangular box - BRAKE. I called dealer on Monday July 12, 2021 and reported the warning. He said it meant there were cars in the way. The BRAKE IN A BOX occurred 2 more times (8/6 and 8/17). I called dealer who said it was not a problem. I subsequently searched the warning on line and it says "BRING YOUR CAR TO A DEALER. TO CONTINUE DRIVING COULD BE DANGEROUS!" I left the car with the dealer for 9 days. Sept 3 through September 11, 2021. They said they could not find anything wrong. I continue to have problems with starting the car, often getting the message that says TO START ENGINE: BRAKE+PUSH. Which is exactly what I do to start the car. When I picked up the car on September 11 (after 9 days with the dealer) the service manager told me he would contact their regional manager to report the problem to the regional manager. That's the last I heard from him except when I called and he said he had not heard from regional manager yet. I called again and left message but have not heard back. I have also submitted a complaint with HONDA and I have written 3 emails to the VP of Customer Relations. the VP has not responded to me at all. The complaint person contacted the dealer and dealer told him they could not duplicate the problem. After many weeks, complaint person contacted me to let me know this. He has no other recommendations. At this time i am afraid to drive the car and just drive it locally. I have avoided the highway, and any long trips because of my fear that there is something seriously wrong with the car and brakes.

I bought a 2021 Honda CRV on May 01, 2021. On July 22 and again on July 25 this car just suddenly put on the breaks for no reason. On the second occasion my sister was in the car with me and was slung forward. She has had three heart attacks and has a pace maker and was so upset I feared her heart might fail her. I have had one heart attack and my heart was pounding too. Fortunately there was no car close behind me on either occasion or I would have been rear ended. Now I feel I must drive slowly because of this but that increases the likelihood of someone driving too close behind me and hitting me if the car stops suddenly. I took the car to the dealership but they said there was nothing wrong with it.
