When my car hit 40000 miles or so, I started getting all sorts of warning lights showing up when I turned my car on. Took my car in to Honda today, and they said upon checking it that the multifunction camera which is located behind the rear view mirror has stopped working. The car has 44,500 miles on it. It has been in no accidents. There seems to be no reason for such a part to just stop working (needing a $2500 repair to fix).
I am submitting this complaint to report a serious and recurring safety issue with my Honda Odyssey involving the front monocular camera system. After stopping for fuel and restarting the vehicle, multiple warning messages and fault codes appeared on the dashboard simultaneously, including: -Collision Mitigation System Problem -Adaptive Cruise Control Problem -Lane Keep Assist Problem -Auto High Beam Problem These systems are all disabled due to a failure of the front monocular camera. This failure occurred without warning and immediately compromised several critical driver-assistance and safety features. Based on my research and discussions with other Honda Odyssey owners, this is a known and widespread issue. Honda is aware that the front monocular camera is prone to failure, and numerous owners have reported identical symptoms and error codes. Despite this, no recall has been issued. The required repair involves replacement and recalibration of the front camera and carries an estimated cost of approximately $1,500, which places a significant financial burden on owners for what appears to be a known defect. More importantly, the sudden loss of collision mitigation, lane assist, and adaptive cruise control presents a clear safety concern, especially when drivers rely on these systems during normal operation. Given the safety implications and the number of reported cases, I believe this issue warrants further investigation and a formal recall by Honda. Owners should not be responsible for the cost of repairing a known defect that disables critical safety systems. Thank you for your attention to this matter.
Our front camera on windshield failed at 96k miles, causing all lights to come on the dash. This resulted in all safety features failed and even basic cruise control doesnt work. Lots of Honda owners have same issues and the result is that it is a failed front camera which could cost around 2k to replace. There is no other way around it. While driving with my family, my vehicle experienced a sudden and total failure of the front-facing multi-purpose camera mounted behind the windshield. This component is the primary sensor for the Honda Sensing suite. The failure caused multiple critical safety systems to become inoperative simultaneously, including the Collision Mitigation Braking System (CMBS), Lane Keeping Assist (LKAS), and Adaptive Cruise Control (ACC). These systems are essential for crash avoidance and driver assistance.
Honda sensing stopped working and threw a bunch of errors about ACC, LKAS, Collision mitigation, auto high beam error, forward collision, lane keep. Apparently the camera had an internal fault without any prior warning. Also vehicle’s timing belt tensioner already deemed leaking at just 75,000 miles, well below the 100k replacement point, despite regular checks and maintenance at Honda dealers. Front suspension leaking, honda replaced only 1, and declined to replace both, as recommended by dealer mechanic. If i did not have honda care, i would be left with a very large repair bill for critical components of the drivability and safety of the vehicle. This is not acceptable.

While driving down the road way the collision mitigation warning failure flashed and then pulled the steering wheel pulled me off the roadway and nearly caused me to crash.
Driving on the interstate with cruise control. All of a sudden the car braked (no one in front within eye shot) and cruise control stopped. I had to tap the brake manually then accelerator to get the car back under my control. Ever since then I get ACC, LKAS and other warnings when the car starts up. It seems like the front camera catastrophically failed while driving and almost caused the car to come to a complete stop on the interstate. It was very scary.
Suddenly, all the Lane Keeping Assistance, Auto Beam Headlights, Forward Collision Warning, Adaptive Cruise Control came on. When I took it in for diagnosis, they claim that the Camera Assembly Monocular no longer works. From searching forums, it appears this is a known issue. This is a safety issue and should be recalled.

It isnt an obstruction issues as I make it a point to regularly clean the sensor area on the grill and on the window where the camera is. A strange thing I notice is it seems 5o work better on days that are foggy or raining where youd think there would be obstruction. Its just been a whole distraction while driving. I would be content at this point just to have those options not function ever but keep the orange blinking annoying lights off that decide to work randomly with no reason or rhyme.

The monocular camera went out unexpectedly for my van back in March 2025. I have multiple failure warnings that all of the van's safety features were disabled. The van had around 80k miles at the time and Honda dealer quoted me well over $2000 dollars to rectify the problem. Looked through forums and online groups and realized it was a very common issues with late model Hondas. Seems like the internal electronic is cooked by heat over time and caused the camera module to go out prematurely. I ended up buying a module from Honda and had an independent shop to calibrate it for the van which still cost me well over $1000 out of pocket.

We began getting several warnings related to Adaptive Cruise Control, Collision Detection, Lane Departure not working. This was due to the multipurpose camera internal failure. The camera had to be replaced by a authorized Honda Dealer at a cost of $2,200. While the car was drivable no safety features related to the multipurpose camera were available. There were no indications of issues prior to the camera going bad. Due to the cost of replacement I would think that a large portion of drivers do not get the camera replaced and continue to drive the vehicle without use of the safety features provided.

All of the alarm lights are going on at once, immediately when the car is turned on. This includes the Collision Mitigation System Problem, Adaptive Cruise Control System, Lane Keeping Assist System (LKAS), Break System. I am told by my local garage that it is a Odyssey radar system failure, that probably originates in the camera or computer, but that they cannot fix it or get parts for it. It is unclear if these systems work or not.

After we took the car in all of a sudden randomly these warning lights will come on and my D for drive keeps flashing. LKAS, ACC, Collision Avoidance, Auto High Beam Problem And brake system warning. Sometimes some of them sometimes all of them. I read it’s a common problem but there hasn’t been a recall on it.
My engine lights started flashing. I was stranded with my children. Had to coast into a nearby gas station. I’m told it’s a failed monocular camera causing all of it. When I google that it looks like it has happened to SO MANY people! A recall needs to happen.
My 2019 Honda Odyssey EX-L has had sporadic errors happen: Adaptive Cruise Control Problem, Lane Keeping Assist Problem, Collision Mitigation System Problem, Road Departure Mitigation System Problem, Blind Spot Info System Problem. These errors have shown up before sporadically and the dealer, DCH Honda of Oxnard was notified April 27, 2023. They could not replicate and did not respond in writing that something was wrong. The errors went away on their own. 3 weeks ago, the errors showed up again, this time also including a transmission system problem warning. The car was taken to Honda in Valencia and they ran a diagnostic and recommended the transmission be replaced or they would give us $2500 for the car. We opted to have a well reputed private transmission mechanic replace the transmission. After doing so the errors are still there. The transmission system problem error even showed up sporadically the very same day we picked the car up. There is no rhyme or reason to these errors other than perhaps a faulty windshield camera, which takes 5 minutes to replace but Honda wants to charge $2k. We also experienced the delayed shifting due to the poor 9 speed transmission and vcm design. It was quite scary to have a car full of children making a left turn on a busy road and not have the car kick into gear but rather slip. Installing a vcm tuner to bypass the computer shifting helped as well as not driving the car in Econ. Honda knows they made a bad car and while Honda Pilots and other models have had recalls related to the transmission, I am atill waiting for NHTSA to take action and include my lemon Odyssey as well. Replacing the transmission cost me over $7k and it may not have been necessary in the end if in fact these errors were related to the camera.

Multipurpose camera failure, all advanced safety systems were affected (Adaptive Cruise, Lane Keep Assist, Blind Spot Warning, Rear Cross Traffic Warning, Auto-dim headlights, etc) Also, caused engine light, brake system and transmission system errors to appear but they were cleared by replacing the camera with no apparent damage to the actual systems. Replaced at a dealership for a total cost for parts and labor $2200+.
Front windshield camera failed and available for inspection if needed. All safety features tied to this camera are now disabled. Even basic cruise control is disabled. Problem has been check by a dealer with an estimated cost of $2600 to repair a simple camera module. Not inspected by police but was inspected by the manufactures dealer. I can get it inspected by the police if it helps. There are several warning lights that come up after starting the car. Even dinging when backing out of the driveway causing distractions. This is a known issue for this year Honda Odyssey van. There are lots of posts about it and how to try and fix it without the dealer charging you 10% of the cost of a 2025 model. For something designed for families this is not what I expected. This vehicles safety is impacted by the removal of these features.
Vehicle has code U3000-49 in the MCU module. All vehicle safety systems have shut down and are no longer working until vehicle is repaired (lane assist, collision mitigation, adaptive cruise control, road departure, etc.) Multiplex Camera Unit is on national back order with no ETA available. Multipurpose camera is defective. The camera is available for inspection upon request. A number of safety features do not work when this camera is not operational, including forward collision warning and lane departure warning. Honda dealership performed a diagnostic and confirmed it needs to be replaced. No other parties have inspected the camera. There were no warning messages prior to the camera failing, only after it had failed. This appears to be a common issue affecting many owners of this vehicle.

Was Driving on my way to work and I heard a sharp sound while going up a hill. I then got warnings that said transmission issue, pull over immediately when safe and the car shifted itself into neutral. I was in the middle of traffic and lost all acceleration controls, while being forced out of the drive gear. I lost all speed and had to move through traffic and get to a side street. The same thing happened to me about a year and a half ago. It was covered by a warranty but I still was experiencing losses of car power or stalling.

After two months of sitting outside in the Arizona Sun, the transmission was replaced on my 2019 Honda Odyssey. A day after picking it up all of the sensors came on. I have been informed that it’s an internal failure with the ADAS camera that’s housed in the windshield. After a brief web search, this seems to be a common issue related to electrical issues with the car. Since the electrical was connected and disconnected during the transmission replacement begs to reason that there was something faulty about this camera that didn’t make its way through the reset.

The front sensor has malfunctioned and all ADAS features are now disabled. There is no damage to the vehicle and was mentioned that this is a common issue yet no recall has been instituted yet.
While driving on the interstate (60-70 mph) in very light traffic (2 cars in view), there was a beep from the consol and the following errors flashed: Adaptive Cruise Control, Forward Collision Warning, Lane Departure Warning, Lane Keeping Assistance Warning. At the same time as the beep the car felt like the breaks were being slammed on for 2 second. There was no car within 30 feet of us. and we went from ~70 mph to 40. This startled us. Ever sense then the errors would randomly come on and the vehicle will break suddenly, leading to very unsafe moments.

8/10 times when turning on and driving the vehicle, an error pops up on the dash indicating the LKAS and ACC no longer work. Dealer found the code was U3000-49, they could not get it to clear. Sometimes the car will start without the problem, but then at some point through the drive it will occur and LKAS and cruise control will no longer be avaliable

Brought to the dealership, where diagnosis is DTC U3000-49 Multipurpose Camera Unit/Millimeter Wave Radar internal electronic failure. Technician advised that this failure often happens after a few years for no apparent reason. Repair is upwards of $4000. Without repair, none of these safety functions operate.
The lkas and several other lights are on in the vehicle and no one has been able to provide any answers as to why. Nor can the problem be fixed. There is also a problem with the transmission. Upon acceleration there is a hesitation that the car will not engage

I was driving the van and all this warnings came on my dashboard. ACC Adaptive cruis control problem, Lane keeping assist problem, collision mittigation problem. Road departure mittigation problem. All they come one after eachother on my dashboard. I did some research and a lot of vans 2019 have the sa e issues. My friend van same problem she payed 1900 to replace the camera. This is a safety issue and it cost a lot to replace. It cannot be deactivated. I hope it will be a recall issued for this model. I have 3 minivans honda. I never had an issue with thjs brand . Without any notable prior messages or symptoms prior to the failure, on [XXX] after dropping off the kids at school and then turning on my 2019 Honda Odyssey EX-L (28,170 miles), the "Brake System" light was on and the following 5 dashboard warning lights/messages turned on: 1) Adaptive Cruise Control Problem, 2) Lane Keeping Assist Problem, 3) Collision Mitigation System Problem, 4) Road Departure Mitigation System Problem, 5) Auto High-Beam Problem. The Advanced Driver Assistance Systems no longer works, and it becomes unknown to the driver whether the "Brake System" light is being triggered only by the Collision Mitigation System Problem, or an additional problemch as the anti-lock brake system not working. While no accidents occurred, the warning lights add unneeded driver anxiety, which can contribute to a potential safety impact. The problem has been confirmed by the San Leandro, CA Honda Dealer to be error code U3000-49. Dealer diagnosed problem as "Multipurpose camera / millimeter wave radar internal circuit failure" and recommended to replace the multipurpose camera and perform aiming. Dealer advised that safety features will not work. The diagnostic fee was $179.10, and the repair fee was quoted as $2456.55 The vehicle or component has not been inspected by the manufacturer, police, insurance representatives or others. Took the van to the dealership to fix a recall for the fuel pump. A few days later, error messages popped up saying there are problems with the adaptive cruise control, collision mitigation system, lane keep assist system, and brake system, among others. I test drove it from Arcadia to Pasadena on both the freeway and the sidestreets without any problems. We took it back to the dealership who assessed it and told us that the multipurpose camera was bad. It would cost us $2100 to get it fixed. We refused. No one was hurt, however, the dealership told me that there is a safety risk to my family if I did not repair it. Attached is the invoice for the last visit during which the dealership gave us the estimate for replacing the multipurpose camera.

All the symbols related ADAS suddenly popped in my car with no rain or snow or any accidents. When I checked with Honda dealership, they said it caused due to software issue and nothing related hardware and it needed 2500$ for replacing the entire system. If it is software causing this issue why would customer had to pay for it, and it could be a recall. I noticed similar issue for all 2019 and above models.
Upon starting the car, multiple alerts came on the dashboard reporting faults with multiple systems. There was no damage to the car, it had been parked safely and appropriately prior to starting. Dealership inspected the issue and reported the camera/sensor system in the windshield behind the rear view mirror is faulty. This faulty camera now causes multiple systems off line as the sensor is no longer reliable. This is a safety issue as this sensor assists with split second support to the driver to prevent collisions, leaving your lane or speed adjustment with cruise control.
LKAS, ACC, Collision Avoidance, Auto High Beam Problem - all these dash lights come on due to unknown reasons. Dealership blames the windshield camera but even when replaced same issue happens. This is a major safety concern and is a frequent issue amongst 2018- 2025 Honda odyssey owners. It’s possible that the issue is electrical CanBus issue. Please help

Out of nowhere I received 5 warnings regarding Adaptive Cruise Control, Collusion Mitigation, Lane Keep Assistant, Automatic High Beam... etc Failure. Upon restarting the car, the warnings would clear itself on occasion but would reappear on following start up.

The warning for these ADAS keeps coming on without any reason
While driving at 25-30 mph, all of a sudden the 2 warning signs came up: “transmission system problem” AND “blind spot info system problem.” The “D” in the dashboard started blinking. When I put the vehicle in park at a stop light, the “P” also blinked. After a couple of minutes of this I pulled over and turned the car off. A few minutes later I restarted the engine and these warning lights have not come back in the last 5 drives. I read online that this may be a software glitch that needs updating (which I think is a manufacturer defect). I do feel like the car was a bit jumpy while these warnings were going off and I was TERRIFIED that it would put itself into park while I was driving and cause an accident.

The adaptive cruise control stopped working while it was on and we were driving. Also the lane keep assist went out at the same time. It appears to be an extremely common issue in this cars with the front sensing camera which is the issue we have.

I took my van to the dealer and they told me that the sensor inside of the front mirror is faulty, causing the following lights to continue to cycle if the car is on: Adaptive cruise control problem (ACC); Auto high-beam problem (A); Road departure mitigation system problem; collision mitigation system problem; lane keeping assist problem (LKAS); brake system lights. I am unable to engage or disengage any of these systems, including when I am driving. This seems to be a major security hazard and I am aware of others that have experienced this with this exact year, make and model (across different packages) of Honda minivan. All of a sudden every ADAS system went down. When starting the vehicle I get numerous errors displayed - all having to do with the ADAS. 5 emergency lights came on at the same time, we went to the Honda dealer and the problem is the front camera, they told us there, for the diagnosis it was $250 dollars and if we want to fix it it would be $1900, I think that Honda should cover that because it is a car with only 30,000 miles, it is a very expensive repair for a car that is not that old.

While driving the car brakes hard when it thinks that it is going to hit another car even though there is not another car around. We have almost been in multiple accidents because of it. My car experience a sudden and catastrophic failure of the multipurpose camera, part number HP36160-THR-A81. Many of the safety features of my car are dependent on this part, which given the age and low mileage of my vehicle has obviously failed prematurely. The safety features impacted on my vehicle by this part failure include ACC, LKAS, Collision Mitigation and Road Departure Mitigation Systems. I receive continuous warning lights and alarms while driving the vehicle related to these systems. These warning lights and alarms cannot be stopped. Without these systems working properly the vehicle is not safe. I was told that 8tbwpupd cost roughly $1600 to replace the part necessary for these systems to work.

C/S MULTIPLE SENSORS ARE BLINKING, SUCH AS LANE ASSIST, COLLISION CONTROL, auto headlight sensor, ACC, ETC. THOSE MAY BE DUE TO THE WINDSHIELD CAMERA. RETRIEVED MULTIPLE DTCS. CLEARED ALL DTCS AND RECHECKED. DTC U3000-49 MULTIPURPOSE CAMERA UNIT/MILLIMETER WAVE RADAR INTERNAL ELECTRONIC FAILURE CAME BACK ON. PERFORMED TROUBLE SHOOTING PROCEDURE FOR DTC U3000-49 AND FOUND A FAULTY MULTIPURPOSE CAMERA. RECOMMEND TO REPLACE MULTIPURPOSE CAMERA FIRST AND RECHECK.
The front facing camera located at the top of the windshield goes bad and affects multiple systems. The manufacturer is aware of this issue. While the part only costs $150 - the dealerships want to charge upwards of $2000 to replace the camera - they say that owners are not allowed to replace it themselves. You cannot disable the advanced features. This issue is extremely common across all Honda models that use this camera.

A close friend and master mechanic believes that it is an electrical/ software and not a component issue since the components seem to be functioning at times. There are tons of issues with the car having bad cables/terminals for the rear camera, A/V systems, and SXM antenna, but Honda refuses to admit that the front camera issue is related.

Car Randomly started giving a failed warning sign that the LKAS system has an error. Took it to dealer and they say that the radar and the camera need to be replaced for 4000$. This is so completely random. It just broke on its own. Rear lift gate ceased operating, unlocking or opening. Later received errors on dash for blind spot malfunction and transmission error. Dealer says it is an issue with water leaking into the rear chassis getting into the exposed electronics.
Over the last three weeks, there were 3 incidents where the emergency braking system spiked the brakes when there was no reason. There was one incident where the lane departure spiked the brakes for no reason. Over the first 5 years owning the vehicle, this never happened. The first incident was going around a curve on a 2-lane road I travel at least twice a day. It sometimes beeps for cars in the other lane, but has never spiked the brakes until now. The car in the other lane was not over the line. There was a car behind us and they almost hit us due to the rapid braking. Incident 2 was on a straght road when there was a right lane closure with orange cones gradually blocking the lane. I started moving to the left lane well in advance and was most of the way in the lane when the brakes spiked. There was no oncoming traffic, but a car behind me had to brake hard to avoid hitting me. Incident 3 was similar to Incident 2, except there was a box in the road. With a straight road and no oncoming traffic, I was fully in the left lane, avoiding the box, when the brakes spiked. The 4th incident involved spiking the brakes with a lane departure warning. I was going around a curve that I travel at least twice a day. The road was clear and no traffic coming the other way. Going around the left curve in full control, the right lane departure warning sounded and spiked the brakes. The brakes had never applied before in a lane departure scenario. I took the van to the dealer and they made it sound normal, like four improper emergency braking incidents in 3 weeks was a regular occurrence. They tried to duplicate the issue and checked the computer for codes, but found nothing. This does not seem normal to me and is a safety issue that could cause an accident if the car spikes the brakes for no reason like this and someone is following closely, not expecting me to stop.
Warning light turn on without any cause. No accident involve. Just happen overnight.
The front camera is malfunctioning creating the following warnings and preventing safety systems from working properly: 1. Lane Keeping Assist Problem 2. Collision Mitigation System Problem 3. Road Departure Mitigation System Problem 4. Auto High-Beam Problem 5. Adaptive Cruise Control Problem In addition, the "Brake Systems" light is on. This issue not only prevents safety features from functioning, but may mask other issues since the "Brake systems" light remains on at all times. The vehicle has recently undergone the recall repair work for the MOST FAKRA Service Cables. No other mechanical issues were detected by the Honda dealership or an independent Honda certified mechanic.
In September 2021, I started occasionally receiving the error message "Collision Mitigation System Problem" on my dashboard. It would appear and quickly disappear. At that time, my car shut itself off when I came to a full stop at traffic lights and stop signs 3 different times. The car would boot back up, but it happened to me in the middle of intersections. I took the vehicle into the dealership and they told me it was the battery. I had the battery replaced, but with time, the "Collision Mitigation System" error reappeared along with other messages, "Engine Management System Problem" and "Road Departure Mitigation Problem." During this time, my car would brake on its own occasionally, happening 6 times in total, when there was no threat of a head-on collision. Three of these incidents happened in the middle of intersections with cars all around me and in one instance, my car come to a screeching halt on its own making me terrified that the car behind me was going to crash into me. The last straw was as I was driving to my child's school and when I came to a curve in the road, my steering locked up completely, not allowing me to turn with the curve. I was able to slam on my brakes and keep myself from crashing, but it was very dangerous. At the same time, the same error messages were flooding my dashboard. That is when I drove my just 3 year old car back into the Honda Dealership and told them they must fix this before I put my child back into that car, and my car still sits there. The dealership informed me that my Collision Assistance Camera that is integral to the Honda Sensing safety mechanisms is "faulty" and will cost me $1,500-$1,700 to repair. I see online that this is being investigated in 2017-2019 Honda CRVs and 2018-2019 Honda Accords sedans. I implore you to also investigate this same system error in 2019 Honda Odysseys. The Odyssey is a family car, but yet I am now scared to drive my family in this vehicle for fear of it causing a collision.
