The bolt holding the shifter gear, fell off, operator was not able to shift down on the bike, making the operator to crash and harm the bike and the operator

The shifter gear bolt holding the piece to the bike , fell off the bike , allowing the operator to crash , because the operator was unable to shift down to operate the bike properly causing a crash. It harm the operator and the bike itself.
