The contact owns a 2008 Lexus LS460. The contact stated while driving 70 MPH and depressing the brake pedal, the message "VSC Brake Malfunction. ABS Needs Services. Contact Dealer" was displayed. The contact was able to drive to the residence. The vehicle was taken to an independent mechanic, where it was diagnosed that the actuator needed to be replaced. The part was replaced; however, the failure recurred. An unknown dealer was contacted and informed the contact that the vehicle was not under warranty. The vehicle was not repaired. The manufacturer was contacted and informed the contact that the extended warranty coverage for the failure had expired. The failure mileage was 69,000.

WHILE IN MOTION, BRAKES STARTED PULSATING AND "CLICKING", THEN BRAKE PEDAL FEEL GOT VERY FIRM AND HAD DIFFICULTY STOPPING, ALMOST RESULTING IN ACCIDENT. TOYOTA HAS RELEASED WARRANTY ENHANCEMENT PROGRAM FOR THE BRAKE ACTUATOR ASSEMBLY, BUT MY VEHICLE IS JUST BEYOND THE 10YR/150K MILE PERIOD. I FEEL THIS IS A SERIOUS SAFETY MATTER AND NHTSA SHOULD HAVE WORKED WITH TOYOTA ON A SAFETY RECALL REMEDY. IF BRAKES FAIL, THE CONSUMERS WILL BE SERIOUSLY INJURED OR DIE. VEHICLE ONLY HAS 60K MILES AND THIS IS A KNOWN DEFECT!

I PURCHASED THIS CAR IN 2018 AND IT HAD LESS THAN 75,000 MILES ON IT. I'VE RECENTLY NOTICED THAT WHENEVER I PRESS THE BRAKE PEDAL, THERE IS A CLICKING/POPPING SOUND. I'M UNSURE WHAT IT IS. I KNOW THAT IT SHOULDN'T BE HAPPENING. I'VE READ ON THIS SITE THAT IT MAYBE THE BRAKE ACTUATOR. IS THERE A RECALL FOR THIS PROBLEM SINCE IT'S BEEN BROUGHT TO LEXUS/TOYOTA ATTENTION?
I WAS PARKED WITH THE ENGINE RUNNING FOR APPROXIMATELY 18 MINUTES WHEN SUDDENLY, I HEARD A BEEPING SOUND IN THE CAR IT WOULD NOT STOP!!! I TURNED MY ENGINE OFF IN HOPE TO RESET WHATEVER IT WAS BUT AS SOON AS ME FOOT TOUCHED THE BRAKE THE BEEPING RESUMED. IT WAS RAINING AND I HAD NOT CLUE OF WHAT IT WAS. AT THIS POINT, ALL OF THE BRAKE LIGHT ON MY DASH CAME ON . I WENT OUT OF THE CAR POPPED THE HOOD AND DIDN'T IMMEDIATELY RECOGNIZE ANYTHING. I THEN WENT BACK IN THE CAR AND WITH THE BEEPING STILL IN MY EAR, DECIDED TO HEAD HOME. THAT'S WHEN I NOTICED I COULD NOT STOP MY CAR !!! THE BRAKE WAS STIFF AND I LITERALLY HAD TO FORCE MY CAR TO STOP BY APPLYING AS MUCH PRESSURE AS PHYSICALLY POSSIBLE TO MY BRAKE PEDAL. I TOOK THE CAR TO MY MECHANIC AND AFTER DIAGNOSTICS HE STATED IT'S THE ABS PUMP AND BRAKE ACTUATOR.

I HAVE 2008 LEXUS LS 460 WHICH HAS MILEAGE OF 36,345. I HAD A SUDDEN FAILURE OF BRAKE SYSTEM AND WARNING LIGHT SAID "BRING THE CAR TO THE DEALER" THEY FOUND ABS ACTUATOR FAILURE. LEXUS ISSUED "WARRANTY ENHANCEMENT" ABOUT THIS PROBLEM IN 2017. BUT THEY WOULD NOT COVER MY CAR BECAUSE THE WARRANTY ENHANCEMENT EXPIRED 5 MONTHS AGO IN SPITE OF MY CAR'S LOW MILEAGE. I THINK NHTSA SHOULD RECOMMEND "RECALL" TO LEXUS BECAUSE SERIOUS SAFETY CONCERN OF THIS PROBLEM. APPARENTLY MANY OF THE OWNERS OF THIS VEHICLE HAVE HAD THIS PROBLEMS BUT HAD A LOT OF PROBLEMS WITH THE DEALERSHIP TO FIX IT. BRAKE ACTUATOR FAILURE IS A KNOWN ISSUE BY LEXUS AND LEXUS SAYS THEY WILL NOT REPLACE MINE. ITS A SAFETY ISSUE AND SHOULD BEEN A RECALL INSTEAD OF 'WARRANTY ENHANCEMENT' WITH LIMITATIONS TO GET LEXUS OUT OF FIXING SOMETHING THEY KNOW IS FAULTY AND A SAFETY ISSUE.

WHILE BRAKING GRADUALLY, WITH THE BRAKE PEDAL IN A STEADY POSITION, THE CAR LURCHES/PULSES, THE TIRES MAKES LOUD SKIDDING SOUND THEN THE CAR COMES TO A HARD STOP AS THOUGH IT HAS SLAMMED INTO SOMETHING. THIS HAS HAPPENED ON MULTIPLE OCCASIONS AT LOW SPEEDS, WHERE IT FEELS LIKE IM PRESSING THE BRAKE AND LETTING GO, AND PRESSING AGAIN, BUT IN REALITY, THE BRAKES ARE BEING PRESSED AT A CONSTANT PRESSURE. THIS IS DIFFERENT THAN THE ABS FEELING, AS THE BRAKE PEDAL DOES NOT TRAVEL BENEATH MY FOOT, AND THE RELEASE AND GRABBING OF THE BRAKES SEEM VERY SLOW. IT HAPPEN THE FIRST TIME WITH ABOUT 50,000 MILES, GOING ABOUT 25-30 MPH. THEN WITH 86,000, GOING ABOUT 5-7 MPH.

THE BRAKES HAVE A CHATTERING NOISE AND FEEL THROUGH THE PEDAL WHILE STOPPED, WHEN YOU RELEASE THEM. THE CAR CAN BE IN PARK, AND IT STILL DOES IT. IT'S A KNOWN PROBLEM, BUT TOYOTA IS DOING THEIR BEST TO AVOID REPAIRING IT UNDER THE NEWLY EXTENDED WARRANTY ON THE "BRAKE ACTUATOR". IT'S 100% REPEATABLE, AS MANY TIMES AS YOU LIKE, IF THE ENGINE IS RUNNING AND THE BRAKE PEDAL IS PUSHED AND RELEASED.

UPON ARRIVAL AT THE LEXUS DEALERSHIP, I WAS ADVISED BY THE SERVICE ADVISOR THAT THIS IS A COMMON PROBLEM ON THIS MODEL AND MORE LIKELY THEN NOT THE FAULTY BRAKE ACTUATOR WILL NEED TO BE REPLACED. THIS WAS CONFIRMED THE FOLLOWING BUSINESS DAY WHEN I RECEIVED A CALL FROM THE SERVICE ADVISOR CONFIRMING THAT THE BRAKE ACTUATOR HAS FAILED. UPON QUESTIONING THE ADVISOR OF THE CAUSE, HE ONCE AGAIN ACKNOWLEDGED THAT THIS IS A COMMON PROBLEM AND THAT LEXUS HAS ISSUED AN UPDATED PART AS A REPLACEMENT. IT CONCERNS ME THAT LEXUS HAS NOT PERFORMED A VOLUNTARY RECALL ON SAID BRAKE ACTUATOR SINCE THIS IS A CRITICAL SAFETY DEVICE AND THEY (LEXUS) ACKNOWLEDGE THAT THERE IS DEFECT TO THE ORIGINAL PART BY PROVIDING AN "IMPROVED" BRAKE ACTUATOR ASSEMBLY. FURTHERMORE, BY PERFORMING A SIMPLE INTERNET SEARCH I ENCOUNTERED NUMEROUS TOPICS ABOUT IDENTICAL ISSUES AS MINE ON THE MODEL/YEAR (2007-2010 LEXUS LS460). I WOULD LIKE TO DULY REQUEST THAT NTSB PERFORMS AN INQUIRY WITH LEXUS REGARDING BRAKE ACTUATOR ISSUES STATED ABOVE.

AFTER CHECKING TO SURE THE BRAKE STILL FUNCTION, I SLOWLY DRIVE HOME WITH THE EMERGENCY LIGHT ON. CAME HOME, I DID A LITTLE RESEARCH ON THE INTERNET, AND IT TURNED OUT THIS IS A KNOWN AND VERY COMMON PROBLEM FOR THIS CAR, AND LEXUS CONTINUE TO DENY THE PROBLEM, AND FAILED TO RECALL TO FIX THE PROBLEM. IT'S BRAKE ACTUATOR THAT FAILED AROUND 114,830 MILES. I REALLY DON'T KNOW WHAT TO DO NOW OTHER THAN TO TAKE THE CAR TO THE DEALER THIS SATURDAY.

I HAD REMARKED TO THE LOCAL DEALER THAT IT MADE A CLUNK NOISE WHEN STARTING THE VEHICLE AND DEPRESSING THE BRAKE PEDAL. THIS WAS AT 63K MI WHEN I PURCHASED IT. I WAS TOLD IT WAS NORMAL. APPROXIMATELY 1 YEAR AND 10K MI. AFTER I STARTED TO EXPERIENCE SQUEALING WHEN BRAKING. I WAS TOLD BY THE DEALER THAT NO ISSUES WERE FOUND AND THAT IT WAS MOST LIKELY CAUSED BY CHEAP AFTERMARKET BRAKES. I BROUGHT THE VEHICLE IN FOR THE BRAKES AGAIN AND WAS TOLD THAT THEY FOUND NOTHING WRONG, PROBABLY JUST CHEAP PADS. APPROXIMATELY 3K MI. LATER WHEN DRIVING AT APPROX. 35MPH. WARNING LIGHTS ILLUMINATED, AND STATED TO TAKE IMMEDIATELY TO THE DEALER. WHEN I TOOK IT I WAS INFORMED THERE WERE CODES REGARDING AN ELECTRICAL MALFUNCTION AND LEAK, BUT THEY FOUND NO EVIDENCE OF THE PROBLEM. THEY RESET THE CAR AND ATTEMPTED TO RE-ILLUMINATE THE WARNING LIGHTS, WHICH THEY WERE UNABLE TO DO. APPROX. 1K MI. LATER THE WARNING LIGHT RE-ILLUMINATED. WHEN I SPOKE TO THE DEALER THEY ASKED IF I HAD ANY DRIVEABILITY, OR BRAKING ISSUES, WHICH I DID NOT, THE CAR DROVE AND BRAKED NORMALLY. I WAS TOLD THEY COULD LOOK AT THE CODES AGAIN AND RESET THEM, BUT THEY WOULD MOST LIKELY COME BACK ON AGAIN IF IT WAS SOME OTHER TYPE OF MALFUNCTION. I WAS PLANNING ON PUTTING THE CAR AWAY FOR THE WINTER AND ADDRESSING THE ISSUE THEN, THE DEALER AGREED THAT IT WAS MOST LIKELY THE BEST TO BRING IT IN PRIOR TO TAKING IT OFF THE ROAD FOR WINTER. ONCE AGAIN APPROX. 1K MI. LATER A CONSTANT BEEPING STARTED WHILE DRIVING SHORTLY AFTER A HARD BRAKING INCIDENT. IT STOPPED, BUT STARTED AGAIN 2 WKS LATER WHILE DRIVING HOME AS I APPROACHED AN EXIT RAMP. I DEPRESSED THE BRAKE PEDAL TO FIND I HAD NO BRAKING ABILITY. PEDAL TO THE FLOOR. I ENTERED THE 1/16TH OF A MILE RAMP AT 65MPH. I SWIFTLY DOWNSHIFTED ENOUGH THAT I MADE THE TURN AT THE END T ON TO MY STREET WITH ONLY MINIMAL FISHTAIL AND FOUND THAT THE VSC WAS STILL FUNCTIONING.

HE MENTIONED THAT THERE IS A RECALL FOR A SENSOR ON THE 2008 LS 460 MODEL. AS OF THIS DATE, 4/16/15 (0406HRS) THE VEHICLE IS AT THE DEALERSHIP NOTE: I AM A LOYAL LEXUS CONSUMER SINCE 1998. WHILE DRIVING AT HIGHWAY SPEEDS (70 MPH) I ENCOUNTERED ILLUMINATION OF BRAKE/ABS WARNING LIGHTS ON THE DASH ADVISING ME THAT THE SAFETY BRAKE/ABS/TRACTION FEATURES ON MY CAR ARE NON OPERATIONAL. UPON PULLING OVER, I READ A MESSAGE DISPLAYED ON THE DASH ALERTING ME TO TAKE MY CAR TO THE NEAREST LEXUS DEALER. UPON ARRIVAL AT THE LEXUS DEALERSHIP, I WAS ADVISED BY THE SERVICE ADVISOR THAT THIS IS A COMMON PROBLEM ON THIS MODEL AND MORE LIKELY THEN NOT THE FAULTY BRAKE ACTUATOR WILL NEED TO BE REPLACED. THIS WAS CONFIRMED THE FOLLOWING BUSINESS DAY WHEN I RECEIVED A CALL FROM THE SERVICE ADVISOR CONFIRMING THAT THE BRAKE ACTUATOR HAS FAILED. UPON QUESTIONING THE ADVISOR OF THE CAUSE, HE ONCE AGAIN ACKNOWLEDGED THAT THIS IS A COMMON PROBLEM AND THAT LEXUS HAS ISSUED AN UPDATED PART AS A REPLACEMENT. IT CONCERNS ME THAT LEXUS HAS NOT PERFORMED A VOLUNTARY RECALL ON SAID BRAKE ACTUATOR SINCE THIS IS A CRITICAL SAFETY DEVICE AND THEY (LEXUS) ACKNOWLEDGE THAT THERE IS DEFECT TO THE ORIGINAL PART BY PROVIDING AN "IMPROVED" BRAKE ACTUATOR ASSEMBLY. FURTHERMORE, BY PERFORMING A SIMPLE INTERNET SEARCH I ENCOUNTERED NUMEROUS TOPICS ABOUT IDENTICAL ISSUES AS MINE ON THE MODEL/YEAR (2007-2010 LEXUS LS460). I WOULD LIKE TO DULY REQUEST THAT NTSB PERFORMS AN INQUIRY WITH LEXUS REGARDING BRAKE ACTUATOR ISSUES STATED ABOVE. *TR

I AM EXPERIENCING A CLICK OR SERIES OF CLICKS WHEN I DEPRESS THE BRAKE PEDAL. BASED ON ACCOUNTS AND RESEARCH OF OTHER OWNERS, THIS IS A COMMON PROBLEM WITH FAULTY BRAKE ACTUATORS. TL* THE CONTACT OWNS A 2008 LEXUS LS460. THE CONTACT HEARD AN ABNORMAL NOISE WHEN THE BRAKE PEDAL WAS DEPRESSED. THE DEALER STATED THAT THE BRAKE ACTUATOR NEEDED TO BE REPLACED. THE VEHICLE WAS REPAIRED, BUT THE FAILURE RECURRED. THE MANUFACTURER WAS NOT NOTIFIED. THE FAILURE MILEAGE WAS 75,000.

I DROVE MY CAR OVER TO PICK MY GRANDSON UP WE WENT TO THE PARK, WHEN I WAS TAKING HIM HOME THE VSC CODE CAME ON AND SAID TAKE CARE TO DEALER FOR SERVICE, ENGINE LIGHT CAME ON AS WELL. I CALL AND MADE AN APPOINTMENT IMMEDIATELY AND THEY TOLD ME TO LEAVE THE VEHICLE AND THEY WOULD RUN DIAGNOSTICS SEE WHAT WAS CAUSING IT. THE NEXT DAY THEY CLEARED THE CODE CALLED AND TOLD ME TO PICK UP. WHEN I WAS LEAVING THE SAME LIGHTS CAME ON AGAIN (ENGINE, VSC ETC). GOT OUT OF THE CAR AND TOLD SERVICE REP. THAT THE SAME THING WAS HAPPENING AGAIN. I LEFT THE CAR OVER THE WEEKEND AND THEY CALL AND SAID IT WAS A COMMUNICATION ERROR AND THE COMPUTER WASN'T COMMUNICATING WITH THE PARKING BRAKE. I TRULY BELIEVE THIS IS A SAFETY ISSUE AND SHOULD BE ADDRESSED BY THE MANUFACTURER. *TR
