On occasion (several times in a month) I will drive the car listening to the radio. I then park for a short time and turn the car on and drive away when I notice that the radio isn;t on as it was when I parked. I then have to pull over, park the car, shut off the engine then restart. Now the redio comes on. I am concerned that other safety items are also non functioning when this happens. The dealer has been no help.

The hybrid battery and hybrid system malfunctioned. The vehicle emitted a strong rotten-egg odor (consistent with hydrogen sulfide gas) inside the cabin from [XXX] to [XXX]. The hybrid system warning light first appeared on [XXX]. On [XXX], the air conditioner did not work while driving, and on other occasions it failed to turn on until the engine was restarted. On August 5, 2025, the vehicle was serviced for a related recall, but the warning light returned shortly afterward. From [XXX] to [XXX], my children (ages [XXX] and [XXX] ), my elderly mother, and I were exposed to fumes inside the cabin. We experienced headaches, nausea, and dizziness. On one occasion, my mother nearly fainted after dinner while visiting, which coincided with her exposure to the odor. The dealership did not warn me that the hybrid warning light could be associated with the odor or that an odor would be hazardous, so I continued driving the vehicle during this period. On September 3, 2025, the dealership confirmed the hybrid system malfunction, deemed the car unsafe to drive, and ordered a replacement battery. The vehicle was out of service until September 15, 2025, when the entire hybrid battery was replaced. The dealership inspected the vehicle multiple times, and Mazda corporate (Customer Experience Center case manager) was notified. The component was replaced under warranty, but the event created a significant safety risk due to exposure to toxic fumes and the potential for hybrid system failure while driving. My wife called me while I was at work today to let me know that our soon to be [XXX] old toddler locked himself in the car with her keys inside while the car was running with the air conditioning on. She repeatedly tried to use the My Mazda app to unlock the car but it kept giving an error message. She called 911 and luckily he finally unlocked it on his own just prior to the police department's arrival. Upon further research this appears to be a known issue that has been brought up to Mazda from other owners and it has been dismissed by Mazda as a safety feature designed to prevent theft. Apparently this is by design, the app will not let you unloco the car if the vehicle is running with the keys in it. Personally, the theft of the vehicle is nowhere near as important as the health and safety of my child. We were extremely fortunate that the air conditioning was on as the temperatures in our town are hitting triple digits right now. What if it had not been on? This could have had a much worse outcome. Additionally, how in the world does this deter theft? So what Mazda is basically saying is that somebody would get my phone, access my password/fingerprint protected app, unlock my car from my phone, then get in and drive off. The likely hood of this happening is way lower than a curious toddler or rambunctious dog accidentally getting locked inside the running vehicle. Additionally, if my wife locked her keys in the car, and isn't home to get the spare, or im at work and can't bring her the spare right away, she's [XXX]. Issue Summary: Since December 27, 2024, my vehicle has experienced a hybrid malfunction and leakage issue three times. Each time this occurs, the vehicle becomes completely inoperable and cannot move, requiring a tow truck to take it to the dealership. The seatbelt reminder system is broken and someone almost took my door off this morning because I had to open my door to get it to stop. Every day when I drop my kids off at school I have to make sure the vehicle is in park so the seatbelt reminder doesn't go off. If they open it a second before I put the vehicle in park it will keep beeping and beeping until I turn the car off, open my door, and turn the car on. I had to do that today and at a stop sign. It's busy by the school so everyone is in a hurry. So it was beeping non stop because my son unbuckled early and the car will keep beeping even if noone is in the seat. I stopped at the stop sign, turned off the car (and it still kept beeping), opened my door to get it to stop, It was at this point the person behind me thought my car was disabled and went around me, almost taking out my door and possibly my arm at the moment. I also have to do this when someone in the back seat moves seats (and we have to put a seatbelt on an empty seat) or even puts some pressure on a seat. If they sit inbetween seats we then have to put a seatbelt on an empty seat. Dealer says this is the way its designed.

On four different occasions, the most recent being last night (1/9/2025), the vehicle has suddenly lost drive power while driving. The ability to accelerate is suddenly gone. When you press the gas pedal the engine revs, as if it is in neutral, but it does not accelerate. When this happens, the vehicle is still in drive as indicated by light on the shifter showing drive and the fact the vehicle, just moments before, is driving. During all four instances, I had to bring the vehicle to a complete, put the car in park and then back into drive. On three of these occasions there was no pull-out lane and I had to bring the vehicle to a complete stop in a moving lane of traffic. The incident last night occurred mere seconds after making a left-hand turn with traffic coming up behind me at a higher speed then I was travelling. My safety, the safety all passengers in my car and the safety of other drivers are all at risk when this occurs. When this happens there are no warning lamps or messages that appear. After the third time it happened (on 12/6/2024), we took the vehicle to the Mazda dealership we leased it from and it was inspected and we were told they found nothing wrong with it. They noted they did some updates. They kept the vehicle from Monday 12/9 - Thursday 12/12. Less than a month after getting the vehicle back from the dealership this issue occurred a 4th time (on 1/9/2025 at about 8pm). We will be calling the dealership this morning and setting up an appointment for them to again inspect the vehicle for this safety concern. Our vehicle is available for inspection upon request.

Oct 13, 2024 Crash

On [XXX], I was heading home from Brooklyn. It was a rainy day. The vehicle was functioning fine when all of a sudden the sign on the dashboard said "Iactive sense deactivated" which is something that always happens when it's raining. It had happened the Thursday prior to the accident when heading to the city and many other times. I had just crossed the George Washington Bridge, on [XXX] , going the speed limit or less, when the the iactive sense sign came up. About 1000 feet after I saw the sign and repeating it to my daughter the vehicle tried to get into the right lane, I was in the left lane of three lanes. I knew it was the iactive sense because i had felt it before many times. I braked and controlled the vehicle back into the left lane. said, "I can see the lane" The vehicle then sped on, moved right, then sped all the way right into the guardrail, all the while I tried to brake and turn left to avoid hitting the guardrail but the wheel was locked and the brake did not work. My right arm got hit by the air bag deploying. After hitting the guardrail we span all the way back into the left lane with the vehicle facing the opposite direction facing on coming traffic. I then hit the hazard lights a few times out of panic, then I had my daughter crawl to me (she was sitting behind me) in the front so we can get out because I thought there was smoke. Turns out it was the airbags' powder, according to the fire department. Another lady who was from the NYPD stopped to see what happened and I explained to her what happened and then the NJ police showed up and she left. When the police got there, I told him what happened. He repeated, "you lost control" at which I said "no the vehicle took control" From my observation the vehicle was like was angry and confused about where to stay and hitting something was the only way it would stop. Sep 11, 2024 Crash

I was driving in EV mode and was behind another vehicle waiting to turn out of a plaza. The exit is on a slope which is important to mention. The car in front of me began to turn and as I was releasing the brake to move up the hill, the car engine kicked on, the RPM’s went up extremely high as if the accelerator was being pressed all the way to the floor, the car jolted forward on its own and hit the car in front of me. I kept trying to brake, but the car continued to accelerate on its own into the car in front of me a second time. I understand that when on a slope driving in EV mode, the cars engine will kick in to allow the car enough momentum to make it up the hill. However, in this case, the engine kicked in, but the momentum was extreme to the point that when I released the brake to roll forward the car shot forward so fast there was no avoiding a collision. This is extremely faulty and unsafe! My child and I suffered injuries as a result of striking the other car twice. We’re lucky we were not hit by multiple other cars as we were propelled into the intersection. I’m terrified to drive this car with my children. I contacted the dealership that instructed me to have the collision repairs completed first and then they would check out the car out.

Apr 18, 2024 Crash

Driving up snowy pass on I-90 approaching the top of Snoqualmie Pass. Vehicle stalled and warning lights flashed critical warning and loss of power. The vehicle stopped in the middle of the highway and would not start or go into drive. After a few minutes, because of speeding and adverse conditions, another driver collided with my vehicle from behind. Another following vehicle lost control and ran off the road. Nobody was injured. Mazda has not provided any information to what happened with the vehicle. We added about 12k miles in the 5 months of driving it, so we had lots of testing with it, and never ran into this serious of an issue with this or any other vehicle.

The car has an "Engine Malfunction" emergency light that goes on. I brought the car into the dealer right away - the car had $1k miles and is brand new, so it's quite scary to get this message. They cleared the error and told me not to worry about it, but were unable to provide a solution. It recurred immediately (within 1-2 days). I reached out to the service department and they told me to ignore it (that there is no fix in progress). My car is unable to change the Mi Drive mode (so it can only stay in Hybrid mode). This is unsafe and goes against the intended use of the car. For example, on a very cold day like today, I wish to warm the car in sport mode (aka not hybrid or electric) and I was unable to change the mode.

Vehicle was parked in garage overnight in our house. At approximately 630AM, carbon monoxide detector alarmed indicating high CO levels inside the house. Source likely appeared to the vehicle- distinct odor of burned electrical wiring/insulation; vehicle felt warm to the touch. Vehicle was dead and would not start. Mazda app indicated critical warnings for ACTIVESENSE, hydrid battery, hydrid system malfunction, high voltage battery depletion, and charging system malfunction starting abruptly at 0210 hours and terminating at 0340 hours. Vehicle had simply been parked in garage and was not plugged in or charging. Last use of vehicle~930PM the night prior. There have been no prior warnings or indications of problems , vehicle has~1k mileage. Vehicle was purchased 7/5/23 and event occurred 7/30/23. Vehicle was towed and is in dealership currently. Mazada USA has been notified by us. Main concerns: 1. Spontaneous carbon monoxide emission of a brand new hybrid EV vehicle model that was parked in a residential home; not plugged in or charging. House full of adults and children overnight. 2. Possible electrical issue with EV/hybrid system leading to spontaneous incomplete combustion of some electrical/battery component.
