I am submitting this safety complaint as the vehicle owner of a Tesla that was involved in a collision on November 15, 2025. Although I am the policyholder and registered owner, I was not the driver at the time of the incident. My husband, [XXX] , was operating the vehicle, and Tesla Autopilot/FSD was engaged when the loss of control occurred. At the time of the incident, it was raining heavily, though visibility and road conditions were still within what Autopilot typically handles during normal operation. While driving under these conditions with Autopilot activated, the vehicle suddenly exhibited unexpected and erratic steering behavior, causing it to lose stability and veer out of control. The deviation happened rapidly and without sufficient warning or opportunity for my husband to intervene manually. The resulting collision appeared to be the result of an Autopilot system malfunction, not driver error. Key concerns include: Autopilot’s failure to maintain proper lane position Sudden abnormal steering input originating from the system Possible sensor, perception, or weather-related detection failure Lack of adequate warning or disengagement alert prior to the loss of control Tesla Insurance later determined the driver to be “principally at fault,” but this conclusion did not consider the possibility of Autopilot system failure under heavy rain conditions and does not reflect the sequence of events as experienced by the driver. As the vehicle owner, I am requesting that NHTSA review this incident as a potential safety defect involving Tesla’s Autopilot/FSD system. I am concerned that this malfunction—particularly under adverse weather—may also affect other Tesla owners and could indicate a broader systemic safety issue. While exiting the Chick-fil-A drive-thru lane, I lightly pressed the accelerator to move into the next lane. Suddenly, the vehicle jerked forward very quickly, as if I had floored the accelerator. At the same time, the display showed "Emergency Braking in Progress." It felt as though the truck briefly braked and then rapidly released, causing unexpected acceleration. No collision occurred, but it was startling and very scary.

Component/System Failed: The primary component failing is the Front Overhead Console Assembly (P-R-N-D manual gear selector/shifter and hazard light switch). The assembly detaches, rattles, and fails to remain securely affixed to the headliner. The defect has been present since delivery (Aug 2024, 208 miles). Safety Risk: The overhead console contains the crucial backup gear selector and the hazard light button. Should the vehicle’s primary touchscreen fail (rendering the main gear selector unusable), reliance on this repeatedly detaching or faulty manual shifter compromises the driver's ability to safely operate the vehicle (Park, Reverse, Neutral, Drive) or activate emergency hazard lights. Problem Confirmation/Inspection: The failure is confirmed by the manufacturer (Tesla Service Centers). Tesla attempted to fix the overhead console defect at least four times. The defect was confirmed again on October 22, 2025 (11936 miles), where Tesla noted only a "slight adjustment... as a temporary fix" and stated a "Permanent fix will be performed on the upcoming visit". The vehicle and its components have been inspected repeatedly by Tesla technicians. Warning Lamps/Symptoms: The primary symptom is the physical rattling, looseness, and detachment of the overhead console/gear selector. The center console can come off at ant time that the windshield visor is detached from the console. This has happened to me several times while I was driving and it caused the cybertruck to shift out of gear and has changed the gear to neutral and park on different occasions. The first time this happened while I drove, I was driving down the street and the car came to a hault when the gear shift cover came loose. This could have caused a serious accident, but fortunately it didnt. This problem existed from the time of delivery & we had to make our 1st service apt on the day we took possession of the truck. This happened several times afterwards and tesla was unable to fix this in 7 attemp

We received a notice from Tesla about a recall involving the cant rail. We reached out to Tesla and they said we must go to a Tesla dealer and the mobile team will not be able to do this repair. We are in South Dakota and the nearest dealer is 6 hours away in Colorado. We asked Tesla to give us a name of a local shop who can do the repair and they said we must go to Tesla. They are unwilling to handle this recall.

The gear selector attached to visor often loose and detached during drive, this action put the car gear into neutral immediately and are very dangerous, tesla try to fix this issue 3 time unsuccessful.

I am reporting a critical safety issue with my Tesla Cybertruck (VIN: [XXX] ). On [XXX], at [XXX] MDT, while making a turn at the corner of [XXX] , the vehicle, operating in Full Self-Driving (FSD) mode, unexpectedly veered into the oncoming traffic lane. I am leasing a 2024 Tesla Cybertruck and have experienced multiple serious and unresolved safety issues. When driving at speeds of 50–60 mph or higher, the Cybertruck begins to shake aggressively, particularly in the front wheels. The vibration makes it difficult to maintain control. I took it to the Tesla dealership, and service staff confirmed there is a known malfunction with the Cybertruck tires. They informed me they are waiting on new tires to arrive, but offered no timeline and sent me home with the issue unresolved. No loaner vehicle was offered, no workaround was provided, and I am still driving a vehicle Tesla knows is defective. In addition, the rear windows malfunction frequently. When I close the rear doors, the windows often jam or freeze. This concerns me because the Cybertruck is built entirely with electronic door systems—there are no manual overrides. If power is lost during a crash, occupants may not be able to exit. This is even more alarming in light of recent reports of Cybertruck-related deaths. Multiple individuals have died in crashes where the battery was damaged or the vehicle caught fire. Power loss caused the doors and windows to become non-functional, and due to Tesla’s armored glass, bystanders couldn’t break the windows to rescue the victims. These people burned alive while trapped inside. This is not a theoretical issue—it has happened. Despite these risks, Tesla has not issued a recall or urgent fix. I’ve made multiple efforts to resolve these problems, but Tesla’s delays, denials, and lack of proactive safety measures are deeply negligent. This is a dangerous vehicle, and I am requesting that NHTSA immediately investigate both the front-end tire instability and the entrapment risk caused by electric-only doors and unbreakable windows. I no longer feel safe driving this Cybertruck and believe others are also at serious risk.

While vehicle was in full self driving supervised mode, it made an illegal u turn from a traffic light crossing into oncoming traffic. I have the video taken by the vehicle. I am unable to upload it.

I was driving [XXX] going through Saint Paul and the truck sped up and became uncontrollable. I started braking as fast as I could and caused it to rear end into another vehicle. I’ve tried to contact Tesla about this, and they have not responded to me. I was told the engineering team would get back to me about the possible cause of the accident, but never heard back and they could not provide me any of the vehicle data or video which they told me in the service center. They would be able to find that when they diagnosed it, they did fix the vehicle and I ended up selling it due to not being too excited about driving ever again. Can my VIN please be removed from the NHTSA list? Tesla has not issued a fix for this concern, and I’ve contacted them for the past 6 weeks. This optional safety recall is preventing the vehicle from being used on ride share apps.

My child received second degree burns from playing the sky force reloaded game on the front display screen. See attached picture taking screen temperature measurements where it exceeds 125 degrees. This is when the truck is not in direct sunlight with the air conditioning running, it appears that the CPU dissipates heat through the screen directly which is not conducive to games that require a kid to keep his fingers on the screen. This is a safety risk that should be re-engineered. We noticed that it does not occur with all games, only with those that require a lot of processing power.

NHTSA Safety Complaint – Recall 25V170 (Tesla Cybertruck 2024) Vehicle Details: •Make/Model/Year: Tesla Cybertruck 2024 •VIN: [XXX] •Recall Number: 25V170 •Recall Notification Date: March 18, 2025 Issue Summary: I am filing this complaint due to Tesla’s failure to provide a timely remedy for an open safety recall (NHTSA Recall Number 25V170) affecting my 2024 Cybertruck. I was notified of the recall on March 18, 2025, and I have since attempted to schedule service appointments to address the issue. Service Attempts: I scheduled appointments with Tesla on the following dates: •April 4, 2025 •April 29, 2025 •May 7, 2025 Each appointment was canceled by Tesla, citing that the recall remedy was not yet available. When I inquired further, Tesla customer service was unable to provide a timeline for when a remedy would become available. Impact: I am currently out of the country for work until mid-August and have been listing my Cybertruck on the Turo car-sharing platform to cover its monthly payments. Due to this recall and Tesla’s inability to resolve it, Turo has unlisted the vehicle, citing the recall as a safety hazard. As a result, the vehicle is now sitting idle, generating no income, while I continue to make monthly payments on it. This has caused financial hardship due to the loss of expected income. Conclusion: It has now been over seven weeks since the recall was issued, and no remedy has been offered. I believe this delay is unreasonable and request that NHTSA investigate Tesla’s handling of Recall 25V170 and require the manufacturer to provide a timely remedy. WHILE DRIVING MY CYBERTRUCK ON FREEWAY USING TESLA'S FSD (FULL SELF DRIVING). VEHICLE SUDDENLY ACCELERATED, GOT OUT OF CONTROL AND CRASHED IN TO THE RAILING. LUCKLY I WAS ABLE TO CONTROL AFTER THR CRASH AND DROVE IT SAFE LOCATION. TRUCK IS SEVERELY DAMAGED AND TESLA IS NOT RESPONDING TO MY COMPLAINT. WE NEARLY GOT KILLED. PLEASE HELP.

The newest Tesla vehicles coming out of the factory with an unannounced HW4 computer upgrade, dubbed AI4.1, are seeing them short-circuit and die down shortly after purchase. In some case, the new AI4 computer gives up the ghost after several hundred miles, rendering not only the FSD feature, but the cameras and GPS navigation hence range estimates unavailable, too. I believe it's a safety defect and the VIN was used to be able to submit the form, it seems to affect most new Tesla vehicles. Thank you

Large driver's touchscreen in the Cybertruck gets really, really hot. Brought it to Tesla for a service today (August 5th, 2024) and they said the screen is working as expected. They showed me another vehicle and it also had the 110 to 125 degree screen. This can burn skin if you're older and lacking nerves or a child (which Tesla encourages to use the screen with their video games). As this is a design defect and not a one-off part issue, I'd personally like to see this recalled. There are other people complaining about the same issue on this very new vehicle -- [XXX] Worth noting that this high temperature is produced in the lower center area of the screen. Tesla Service says it's due to the computer processor being located in that area with no way for the heat to dissipate (ie, a TV would have room behind it to dissipate the heat, but the Cybertruck has that area blocked).
