I received a safety alert on my car screen: Front passenger safety restraint system issue. Service is required. The front passenger safety restraint system fault typically involves a malfunctioning occupant weight sensor in the passenger seat, which controls whether the airbag activates or deactivates EVGOER. I just traded in my vehicle for a Tesla from 3rd party dealership on [XXX] A light came on once I reached my home. I called right away before the dealership closed but no one answered. I received and indicator noting that my front passenger safty system has a fault. They indicated that they will have to replace the seat sensor and the airbag. I believe this to be an innate fault of the cars construction. It is dangerous and should be fixed by Tesla and not charged to the customer.

I was involved in a rear-end collision where I was completely stopped at a green light due to traffic congestion and the person driving behind me hit my car from behind that caused me to hit the car in front of me. The impact was hard that the Tesla model 3 immediately turned on the hazard lights and called 911. The screen of my Tesla model 3 stated "airbag deployed;" however, no airbags deployed. The Tesla immediately became inoperable and did not let me drive the car to the side of the road out of traffic. Also, the Tesla did not let me change to towing mode (since the screen eventually went black); which made it more difficult to tow the vehicle out of the way of traffic.

I started getting this alert on my screen a few weeks ago: RCM_a056 "Front passenger safety restraint system fault" Service Required. Tesla says the warranty only covered this issue up to 60,000 miles and I'm at 61,000. They are quoting like $2,000 to fix. They say they have to change something in the seat (sensor) and change the air bag since new sensor is not compatible with old airbag. Looked up issue and it seems to be common happening at before and after warranty for a lot of people. I see different quotes from $500 to $2000 (mine was the highest). This should be a safety recall. I do not suspect too many people getting this fixed on their own their due to the expense.

Seat occupancy system failure received. June 27, 2022 Mobile service inspected for wiring damage, as common cause. recommended appt at service center. Sep 2, 2022, could not fix issue, had to replace entire passenger seat. July 23, 2024 took car to service center for seat occupancy system failure received. Tesla stated I needed to pay for lumbar seat occupancy system sensor as it was failing. This was the same part (seat that was replaced in Sep of 2022). This part was 22 months old. Parts and labor for this part was $955. In addition, they stated this new part is not compatible with the existing seat occupancy system sensor in the seat portion of the seat and also not compatible with the passenger air bag. These parts including labor were an additional $1,500. These parts were not defective. Tesla has developed a new part of the safety system which is not backward compatible with other parts of the safety airbag system. This is improper practice and the cost for upgrading their defective manufacturing practices of safety systems should not be passed on to the customer.

Started receiving Error RCM_a056 Front Passenger Safety Restraint System Fault. The vehicle shows that the passenger airbags are DISABLED after the alert is issued. I am working under the assumption that all passenger airbags (front, side, overhead) are currently disabled at all times. This puts my family at risk if there were to be a collision. Tesla seems to know exactly what the problem is because the estimate came back shortly with a list of fixes required at a cost of $1,768.35 to 1) Correction: install an Occupant Classification System (OCS) filter Module, 2) Correction: Retrofit SBR in OCS Equipped Front Passenger Seat - Remove and Replace, and 3) Correction: Replace Front Passenger Airbag and Update Vehicle Configuration - Remove and Replace. Quick internet search shows this is a SYSTEMIC problem with a bad part (Tesla forums and search your own databases for the past year or so... these are the people who know to report this to you, there are probably a lot of others). I'm 6 months out of warranty for a sensor that should not have wear and tear. Furthermore a retrofit demonstrates that they are replacing a KNOWN problem part. Additionally, there is no rational reason to replace a perfectly good airbag. Replacing the airbag because it is incompatible with a retrofit sensor should be free. Retrofitting a sensor that has a known problem should not be a cost to the consumer for a part that is not subject to wear and tear. Adding a filter to a sensor line shows that the sensor is not an appropriate part or the filter should have been added to start with in original manufacturing. Note Tesla has a procedure already out to address the problem SB-23-20-002 which shows they already know it is a problem that needs to be addressed when the sensor failure shows up. Recommend and request an investigation into whether there is a lower cost fix for what seems like a known problem or require a recall to repair to a safe state.

This has been an ongoing issue in these models evidenced by SB-23-20-002 being revised 6 times. Tesla has resisted fixing this safety issue because the air bag also needs to be replaced to maintain compatibility with the redesigned Occupant Classification System (OCS) weight sensor evidenced in Tesla SB-23-20-001. Tesla has redesigned the known bad sensor part no. part 1121234-70-B with an updated part no. 099592-00-F which is only compatible with an updated airbag. The OCS weight sensor was defective from the point of manufacture. The RCM_a056 error only appeared after applying the latest firmware update that addresses NHTSA recall 24V-376. The firmware update made the firmware more sensitive to analog sensor readings which revealed the originally defective OCS weight sensor triggering the error message. Nearly half of the recent NHTSA complaints for this model involve this exact issue. This OCS hardware sensor was always defective and poses an ongoing safety concern. The defective system never should have been approved as it did not function as intended originally. The NHTSA must issue a new recall that covers the defective hardware instead of just the software as in NHTSA recall 24V-376. Without corrected hardware the OCS system will not work as intended which is allowing the airbags to prevent injury and death in the event of collision. The car issued me a "front left safety restraint system fault" saying service is required. The code it flashed was RCM_a021. I am able to get the code to go away by adjusting the seat, however, if i shift my weight or i hit a bump in the road, the warning will appear once again. This leads me to believe that there is a component in the seat that is defective. I took vehicle to the Tesla service center where they found a "broken wire in the pretensioner connector" and they replaced the wiring harness. Upon doing investigation it appears that there are multiple components that are used to determine the firing intensity of the airbags should they go off. If either are damaged the airbags will not fire correctly. Some of these components can be easily damaged due to the seats constant back/forth motion if the easy entry mode is enabled on the car. As the car was outside the warranty period, Tesla refused to cover the costs for repair even though there was a TSB (technical service bulletin) already out on this same issue (reference SB-21-17-005). Total cost for repair including parts, labor and tax was $631.10.

While driving normally, an error warning was received that stated "Front Left Safety Restraint System Error" and the icon for an airbag fault would come in simultaneously. This error would disappear and reappear, sometimes up to 10 times per minute, creating a driver distraction due to the loud error chime. The error code identified for the fault when checked on the Tesla service screen corresponded to pretensioner wire harness. The pretensioner harness connects the seat sensors for driver weight and positioning to the car's airbag control system, which then classifies passengers and ensures that the airbags fire correctly in the event of a collision. For example, if a smaller female driver was operating the car but was incorrectly classified as male, the airbag could fire with excessive force and cause injury. The set of symptoms occurs commonly in Tesla Model 3s and are caused by the wear over time of a poorly designed wiring harness, which chafes as the seat is moved forward and backward until it breaks. This is a known problem by Tesla and they have a specific service bulletin designed to address it, which I have attached below. When taken in to address the issue, Tesla performed the procedure in the attached bulletin and identified a damaged harness, which was subsequently replaced. I retained the damaged harness for inspection. The invoice for this service is attached. No other defects were identified that could have caused the issue. This is a safety related issue occurring due to a manufacturing related defect and should be a mandatory recall with complimentary repair.

While driving normally, an error warning was received that stated "Front Left Safety Restraint System Error" and the icon for an airbag fault would come in simultaneously. This error would disappear and reappear, sometimes up to 10 times per minute, creating a driver distraction due to the loud error chime. The error code identified for the fault when checked on the Tesla service screen corresponded to pretensioner wire harness. The pretensioner harness connects the seat sensors for driver weight and positioning to the car's airbag control system, which then classifies passengers and ensures that the airbags fire correctly in the event of a collision. For example, if a smaller female driver was operating the car but was incorrectly classified as male, the airbag could fire with excessive force and cause injury. The set of symptoms occurs commonly in Tesla Model 3s and are caused by the wear over time of a poorly designed wiring harness, which chafes as the seat is moved forward and backward until it breaks. This is a known problem by Tesla and they have a specific service bulletin designed to address it, which I have attached below. When taken in to address the issue, Tesla performed the procedure in the attached bulletin and identified a damaged harness, which was subsequently replaced. I retained the damaged harness for inspection. The invoice for this service is attached. No other defects were identified that could have caused the issue. This is a safety related issue occurring due to a manufacturing related defect and should be a mandatory recall with complimentary repair.

I purchased the car new in 2018. About two years into owning there would be a random notification about an "rcm_a036 : passenger safety restraint system fault". I reported and was told that the seat occupancy sensor was responding to noise from my cell phone. I stopped putting the cell phone on the seat, it kept happening with increasing frequency in 2021 and 2022 and I reported it two more times and was told to keep charging cables away from the seat, and then in 2021, they added a noise filter. I was told repeatedly that the fault was caused by noise and was not a real issue. The alert occurred with increasing occurrence and today I took it in and Tesla charged $1533.16 to replace the airbag and the occupancy sensor. I thought it was unsafe for Tesla to tell me the issue is a noise-related issue when it was related to the airbag and to tell me that the issue was not actually a real fault when both the OCS sensor and the airbag needed to be replaced. I think it's absurd to have a car 6 years old with 66,530 miles have to have an airbag replaced. I thought Tesla's comments about noise were disingenuous attempts to put off the repair until after the car was out of warranty.

We did get a warning on or around Feb 5th, 2024 and that when we proceeded to contact Tesla service and make an appointment. The reason for this complain is that we ended up paying for airbag and seat sensor repairs which should have been covered by Tesla since these parts are obviously defective. Tesla seems to be systematically charging a standard $1.5K repair to replace a $100 sensor harness, but they're charging so much because they claim an airbag replacement is required (for an extra $1.2K) as part of the new sensor/wire harness installation. From a quick google search, it seems this is a pretty widespread issue that MANY Model 3/Y owners are encountering as their car ages-- with many noting that they were able to self-service the issue with buying the required harness and replacing it themselves. I wanted to bring awareness that to this issue and Tesla's poor and systematic way of overcharging customers to fix a widespread defect in their car seat design.

When driver seat adjusted to end positions (e.g all the way up/down/forward/backward), it causes an error message stating “Front left safety restraint system fault” & and airbag failure icon would appear. This usually cause by the wire harness being pinched or stretched. The error message may come and go based on the position of the seat, and occurs more frequently as time goes by. I am concern that the airbag or other safety restraint system may fail to engage when the message pops up. Tesla community has reported similar failure.

This week I experienced an error RCM_a021, front left safety restraint system fault, which I've researched has affected many others and appears to clearly be a safety flaw. The service team mentioned this isn't covered. My airbag light doesn't go off and is constantly on, my seats no longer save the seat setting (which occurred a couple of months ago but I didn't think about it).This is a safety hazard should I get into an accident. There were no previous warning issues prior to this warning error which occurred this week. According to Tesla, if the airbag light remains lit, schedule a service appointment immediately and do not drive. The service team mentioned the seat harness is faulty remotely and according to what they can see. I have not had them look visually yet though. They do not cover the repair for this as it's out of warranty. There's even a bulletin posted on this issue/repair by NHTSA therefore is known problem. [XXX] I'm unable to upload any files. Then was told that it would cost $1100 more than estimate to replace an airbag that is in working condition. Intermittently over the past two months, the vehicle has been showing error code RCM_a056 with the accompanying message: Front passenger safety restraint system fault Service is required I reached out to Tesla and the service estimate was nearly $1500 to replace the Airbag "to maintain compatibility with Front Passenger Seat" for $942 and $414 to replace the Occupant Classification System (OCS) of Front Passenger Seat plus taxes. Basically, they want to replace the airbag and the seat sensor system. Many people on the forums have reported the same issue, which is sometimes fixed by replacing both components, just the sensor, or reseating the sensor cable. This is clearly a safety issue caused by a defect and these critical components should not fail this catastrophically in just 5+ years.

The front passenger seat driver occupancy system started sporadically malfunctioning and showing the "air bags disabled" warning even while the seat was occupied. This would put the passenger at risk in the event of a crash. The manufacturer's technician arrived to inspect the vehicle and upon hearing the description of the problem said it was likely that a cable in the seat's wire harness probably wore out from friction and that "it was a common issue in these vehicles". Front passenger safety restraint alert continuously stayed on with and without passenger seated. Took vehicle to solve problem immediately on 9/4/2023. After it was diagnosed, the occupancy sensor was faulting and the concerned technician had to retrofit the sensor system to address the problem. When the alert is present, the front airbag deployment will not be optimized for the passenger weight. Airbag replacement is necessary to be compatible with the seat sensor update and will ensure better serviceability for the future if it becomes necessary. Let me know if you have any further questions about the repair. Estimate of repairs, $1760.00. I have another tesla model 3, 2017 model year with the same fault. First of all, I drive the vehicle as a commuter car, and 99% of the time I drive alone. The vehicle is reporting a right passenger restraint error message on the screen. Tesla "fixed the sensor" in 2021 under warranty. Now the same issue is present and Tesla says they have to replace the senor again with a newer one which is now no longer compliant with the OCS system or the air bag. They are charging customers $1600 to replace the sensor, OCS and airbag, all of which are safety specific requirements and is a product failure on their part. The error comes and goes at random, and at any speed. As low as 0-5 mph and as much as at 75mph

My car left the road traveling at 60mph, and after leaving the road it was quickly approaching a metal pole. At this time it seemed that the breaks were not able to stop the vehicle. After totaling the car, and what I thought would be the loss of my life, my airbags did not deploy. To further explain the extent of the accident: the front bumper is destroyed, the hood is destroyed, the windshield is destroyed and caved in onto me, the roof was destroyed, the back glass was also destroyed, along with much more extensive damage to the build of the car. The pole involved in the accident came out of the ground and through the car. It is unfathomable how I am still alive with manageable injuries. Yet- no airbags. This could have ended much differently.

I was experiencing intermittent Bluetooth connection issues along with vehicle not powering off and the gear shifting to park when moving the vehicle at (very) low speed without the seatbelt buckled. I noticed the Bluetooth connection problem after a bad software installation in Jan 2023, however the other two issues have been noticed since at least 2020. When I took the car for a second time, Tesla diagnosed the problem as a faulty seat occupancy sensor (SOS). Tesla told me that the driver side seat SOS is “sticky” that the car didn’t consistently know there was a driver in the car when I sit on the seats (which explained why the car would put itself in park when driven at low speed (<3mph) without seatbelt on, and why the engine kept running even after I stepped out of the car when the car wasn’t locked ( like, when the car is parked in the house’s garage). Further research on the web (https://www.carproblemzoo.com/tesla/model3/2018/seats-problems.php ) shows that other teslas had faulty seat occupancy sensors at both driver side and passenger side. Faulty sensors may lead to improper, or failed, airbag deployment. This is a safety issue that Tesla had known and should have performed a recall and fixed, or the very least sent a billeting to its service centers so that they would check the seat sensor when someone’s complaining about any of the three symptoms I had. When I took the car to Tesla in Feb 2023 their (mis)diagnosis was that I had too many wires plugged into USB ports and that’s why the Bluetooth didn’t connect. I didn’t complain about the vehicle going to park by itself - as I thought it was a “safety feature”. I didn’t care to report the motor running after exiting, as I thought it was the battery mgmt system running to cool the battery!

The first instance of issue took place on 03/05/2020 and a second instance of the same issue has occurred again on 09/29/2022. It is available for inspection upon request. In 2022 the vehicle reported “RCM_a056 Front passenger safety restraint system fault service required”. Tesla’s proposed correction is to replace seat (part 1ST ROW SEAT ASSEMBLY – RIGHT HAND – PUR BLACK (7654324-91-C)). The safety risk may be serious and alert RCM_a056 is being reported by other owners on the web so the problem is not isolated. My understanding is the Occupant Classification System (OCS) Filter Module is part of the airbag system and weighs occupants in the front seat. The OCS Filter is used to determine if the front seat is occupied and possibly classify occupants by weight to determine if and/or by how much airbags should be deployed. The safety of a person in the front passenger seat maybe at risk because of this faulty OCS which could result in airbags failing or deploying dangerously based on erroneous OCS inputs. The problem was identified by two faults reported by the vehicle, the first OCS failure was confirmed by a Tesla Mobile Technician and the component was replaced (part SERVICE KIT – OCS REWORK (1479982-00-C)). The Tech said the seat heater was causing interference with the OCS’s unshielded wires. The second fault the vehicle reported was also confirmed by a Mobile Service Technician who said the replacement OCS filter module “can’t be calibrated”. Please note that this is the second failure of the same component. Tesla has the OCS Module replaced in March of 2021. No other parties have inspected any of the OCS Filter Modules. I will make an effort to retain the seat being replaced. Only two fault with “RCM_a056” code and “Front passenger safety restraint system fault” text were reported by the vehicle. Considering this is a manufacturer failure and not a wear and tear issue, a reasonable person would not lay cause on the owner but rather a quality issue, or the lack of it. I did a little research and found out that this is not an isolated case but rather an ongoing issue with other owners as well. A few instances would indicate an isolated issue but multiple occasions would suggest a systemic failure. Please consider reviewing the matter and resolve appropriately. If it is not Tesla’s responsibility so be it, just have it reviewed because so many other lives may be in danger without accountability. Thank you.

The steering system failed. I'm not sure about the availability for inspection. My safety was put at risk when my car collided with another car. I had just pulled out of my driveway and turned to go down the street a short distance from a stop sign where I would turn left. From a video taken by one of the car's cameras, the car went to the left side of the street and started toward the intersection where the car hit another vehicle. I have driven to the intersection numerous times. There is no way that I would have pulled toward the left side of the street. I believe that something in the steering system malfunctioned. This put me and other drivers on the road at risk. I don't know if the the problem has reproduced. There was a policeman at the site, but he would not inspect the vehicle for a malfunction. My insurance company must have inspected the car as they called today to inform me that the car is totaled. The failure happened so quickly, that I had no warning.

Here is what happened: Driving autopilot at highway speed at ~65 mph Stray wheel/tire came and hit the front center hood/bumper of the car headon The hood flipped open tearing the hinges/struts on both sides of hood; hood lay flat on the front windshield With hood laying flat covering the whole windshield, front visibility became ZERO, also covering front windshield cameras Airbags popped on driver side (steering and below steering), and DEFLATED right away Autopilot did NOT disengage even after airbags deflated; car continued to drive ~65 mph on autopilot Not been able to see anything in the front, with both airbags deflated, and car still on autopilot at ~65mph, I found myself in a very life-threatening situation Eventually pulled the car to the shoulder on the left after changing one lane (still not been able to see anything through the windshield at ~65mph). Requesting: 1. Review and identify what Tesla auto-pilot could have done different and make improvements (since car had data on both sides of the car and front and back, as a computer, it should have at the very least slowed down or braked; or better yet avoid the on-coming wheel all together in that split second) 2. Why didn't the autopilot dis-engage and continued driving? 3. Front hood needs to have two front latches as in other cars - i.e., one main, one backup for situations like these (e.g., like Mercedes); single latch is not enough; both hinges holding the hood are very weak and broke within a second with just the wind. Hinges/struts need to be stronger. Just a wheel and here we are with a totally broken hood? 4.
