This represents a loss of manual override — I was completely locked out of steering and braking. [XXX]- [XXX]am. Trying to park in YMCA located in camp hill PA. Took soft right turn to park. Car accelerated instead of recognizing break, which I applied when I see it accelerating. Then I left break so it can take emergency break that didn’t happened it keep going and cross over the parking curb about six Torah inch high and went in a ditch. I did not hurt but car front damaged. Tesla was informed and they have the log of the time but told they don’t see any issue. My insurance is paying all the expenses. This was the third incident where car did not tool emergency break itself or recognized the break. I did reported last incident to Tesla. It only happened three times in last six years. * The AEB engaged when it shouldn't have. Yes. * Yes * No. * Yes, Tesla remotely told me that the error codes in my service menu had nothing to do with the AEB engaging when it wasn't supposed to. The problem has been reported to Tesla. * None. On [XXX] a car stopped suddenly in front of me and my AEB engaged when it shouldn't have. My car is a Model 3 Performance and it can stop much faster than most cars. The car behind me came inches from hitting me and my dog and myself were thrown violently forward. This is a *MAJOR* safety hazard and could have caused an accident and has caused accidents in other cases. [XXX] This thread has videos of the incident. There was around 20 feet between my car and the car in front of me when the AEB stopped my car. My foot was already on the brake and I was in the process of stopping at a reasonable rate when the AEB suddenly engaged. I could feel the ABS engaging. This happened another time about a month ago. A white box van two cars in front of me slowed down and turned right and the AEB engaged even though there was no need for it. Tesla has the ability to turn off the AEB but you have to do it each time you drive. Please require Tesla to make the AEB on/off choice sticky so you don't have to turn it off each time you drive. Also please make Tesla fix their software or disable AEB on their cars. Recent accident where AEB caused an accident. For the last few weeks while driving, my car will automatically put itself into FSD with out me authorization. Mean I will we driving and the car will just put itself into FSD. This has happened about 15-20 times over the last 2-3 weeks. Tesla wants to clean the front camera? Which makes no sense and charge me for it. If the front camera was obstructed, like in the case of fog. The car would alert me that it could not due to being obstructed, which isnt the case.

Phantom braking (as it's referred to by experts) happened to me twice on the highway using Traffic-Aware-Cruise-Control. Very scary. I was on cruise control going 60 mph when suddenly my car braked suddenly and inappropriately, losing at least 20 mph in a split second before I was able to slam on the accelerator and prevent an accident (rear-end collision). This happened TWICE in the course of 5 minutes on the same highway! There was no obstruction in the road, no cars moving into my lane, nothing but normal road conditions, when it occurred. From my research, this is a huge problem with Tesla vehicles that thousands of individuals are loudly complaining about online and now in court cases in the country. I see there are thousands of reports to your organization about this, but it says you have only complaints from 2021 and later years. However, I have a 2019 and it's happening to me. And it's happening to every single model year out there, just from researching and talking to Tesla owners. This is a major safety hazard, plain and simple. I was inches of being in a rear-end collision, and drivers behind me are extremely upset and vengeful on the road after an incident like this (understandably!). Because of this I fear for my safety and well-being on the road using Tesla's cruise control features. Very disappointing. Above for the ADAS option, I wasn't able to select 2 options, but "automatic emergency braking" is also applicable here as that is what happened. Somehow Teslas are falsely activating emergency braking during cruise control!

When cruise control or self-driving systems are engaged, there have been numerous times when the automatic emergency braking system mistakenly slowed the car suddenly. At 70+ mph, this is incredibly dangerous. The AEBS apparently sees shadows as dangers. Fortunately, there has never been an instance when another vehicle has been close behind.

Two major safety issues need to be reported: 1) The left front lower control arm falling off in the middle of driving, caused car wheel stuck on a traffic street (luckily it was off the freeway 5 minutes ago). The long bolts connected the lower control arms were loss due to the defective design (the bolts are screwed upward vertically, falling off is definitely just a matter of time). I found there was a class action against Tesla due to this problem (it is referred to Model X and S) 2) The other major safety issue I encountered as I turned on the “advanced autopilot “ feature, the car could break suddenly without warning on the freeway. There were no cars or obstacles in the front. I researched online and found many Tesla owners have experienced this so called “phantom breaking” problem as I had (I had experienced this unsafe auto breaking randomly since I have driven this car)

Driving on Hwy 8 (Houston Beltway), going west, dog came out from left side, Tesla auto pilot was engaged and did not detect collision with the dog. Car did not stop, did not alarm, dog was hit, my front bumper was damaged, and car kept on going.

I’m at the Tesla service center and Tesla is refusing to perform recall work without payment/diagnostic time. Even though Tesla’s website and notices states that they will perform recall work free of charge they are in fact refusing to perform recall work free of charge. They are mandating that I be charged diagnostic fees for them to determine whether they will perform the recall work as of 11/13 at 7:15am per Marc the service advisor at Tesla Service Center Kearny Mesa.

Tesla Model 3 with FSD (Full Self Driving) package. Engaged FSD to drive home today but vehicle failed to recognize ALL traffic control signals (stop signs, red & green stoplights. The first time this occurred, I had to brake emergently to avoid collision with cross traffic that had a ‘green’ light. Subsequently reactivated FSD to check the issue and vehicle attempted to run several more red lights and stop signs.It did, however recognize and stop for stopped traffic ahead and performed fairly well with autosteer. This is the first time we have had this potentially-fatal issue. We have, for several months had an issue where some or all of the Tesla cameras fail intermittently. This usually happens at the beginning of a drive and the vehicle alerts us ‘Automatic braking temporarily unavailable.’ Sometimes it recovers on a subsequent drive the same day and rarely recovers on same drive. When cameras are out, ALL the ADAS features are inoperative as they require camera input. Also, the standard feature of ‘speed control’ does not function because Tesla has only adaptive cruise control. Today’s drive was the first time cameras seemed functional on drive home & vehicle allowed me to engage FSD when there was apparently a major safety issue.

The brakes of my car did not work on July 2023. I stepped on the brakes and the car did not stop. It accelerated. I almost crashed into another car as the car kept on going. The car beeped when getting close to another car & slowed. I had to maneuver the car to the side of the road to avoid a collision. It did not stop until the length of 2 cars. My husband checked my tesla car and saw there was an alert that the brakes did not work, however, he was unable to take a picture of the message and it did not re-appear. When I scheduled a Tesla service, at first it was a mobile service & they said it had to be checked at a service center. At the service center, I was told that nothing was wrong with my cars brakes and I was charged a $240 diagnostic fee. They state that they "test drove [the] vehicle and was unable to replicate customers concern. Using customers provided times stamp of 7/13/23 between 5:45 PM and 6 PM verified no brake related alerts where present. Technician inspected from and rear brakes, front and rear brake pad thickness was at 8MM. Customers stated about a recall related to loos brake caliper bolts, as of now there is no documentation available to service of any steps to take. For piece of mind technician torqued front caliper bolts to knuckled to 94NM and rear caliper bolts to knuckle to 83NM. Advise customer to monitor condition as it was a one time occurrence." - Component that failed were the brakes & the automatic emergency braking as it did not turn on quickly. - My safety was put at risk as I almost got into a car accident. - The Tesla Service Center at Buena Park "drove the vehicle and was unable to replicate" my concerns. - The vehicle was inspected by Tesla, but not by the police, insurance, representative or others. - There was a warning message that showed up when my husband went back to see if there were any messages and it stated that the brakes did not work. Of note, I do not recall seeing any messages before or during the incident.

Tesla has combined cruise control with their Auto Pilot feature. Now there is no human override to what speed limit to use, Tesla determines the speed to used based on certain criteria. This puts the driver at risk who may want to lower or increase the speed limit based on certain conditions Tesla may not be able to detect. For me it was a 9 PM run to the store thru an area that had temporary road construction and known to be a speed trap zone.

The following recalls have been pending on my vehicle: 23V-838; TC2023-657, 24V-051 Tesla is unable to push over-the-air (OTA) updates to my vehicle to remedy these recalls due to an issue with the onboard computer being unable to receive updates. This issue arose when Tesla pushed a software/firmware update to my vehicle which effectively "bricked" the Advanced Drive System portion of the computer. In other words, I am unable to safely receive an update to resolve the issues (solved by the recall) because Tesla's failed software update damaged the onboard computer.

With the latest update of V12.3 FSD, the full self driving cannot predict the obstacles clearly, resulting in am incorrect left turn manuever, running over a central divider curb, and damaging tires. Not sure, what could have happened, if a human was standing there instead!

Thursday, 2/29/23 I was driving through a busy intersection. The car in front of me was slowing and I too slowed by gently easing the brake. This slow down was not unusual for steadily moving traffic. As my foot was resting on the brake, I felt the car pull the brake down below my foot hard, forcing my vehicle to abruptly stop and due to the force, my body was pushed into the steering wheel. No injury or accidents resulted however, if a car had been driving behind me, I would have been rear-ended. If my toddler had been with me, his car seat would have protected him but this would have been the greatest force ever placed on him. The car did not provide warning that the emergency brake would be activated. I am not aware of a publicly-notified defect at this time. I have scheduled the most soon appointment available with a Tesla service center in order to audit my software system with hope this glitch will be resolved.

I was driving on the road at night a few days ago and there was a car that was pulled over on the side of the road with hazard lights flashing. The Tesla slowed down and applied the brakes very hard. The Tesla also applied the brakes for no reason. It was on adaptive cruise control, and all of a sudden it would hit the brakes really hard and I would have to take it off the adaptive cruise control because I did not want to get into an accident. The system automatically applied the brakes in both cases which I would call it, “The “phantom braking”. The car had done this before but not to this extent on adaptive cruise control. This only happened when it was on cruise control. I have been very hesitant on putting it on adaptive cruise control because it acts up at times.

FORCED DISCONNET: Autopilot and Full Self Driving suddenly decelerated to 15 mph with no sign or road work signs. The curve onto Old Madison Pike means following traffic can not see the car suddenly creeping in the turn lane. The problem is reproducible. The problem has been reported to Tesla using their voice reporting system. There were no warnings. It has been going on since August 2022.

Since nhtsa recommended changes, the assist driver require more input to confirm I am paying attention and is distracting me from paying attention to surroundings. For example proir to the vehicle being "safer" I was able to focus my attention on immediate risk but possible risk that were greater than 12 seconds ahead of me. The vehicle was safer prior to the recall.

It is ridiculous that Tesla has ruined one of the best aspects of my car due to (ostensibly?) NHTSA pressure. Please allow their previous driver attentiveness system with the steering wheel to be reinstated. It is so miserable to have my car nanny camera flip out on me every time I look anywhere other than straight ahead. At the very least, every other manufacturer should be forced to have just as annoying a system if my Tesla had to be ruined. Its wrong to punish every Tesla owner because some people misused autopilot.

The vehicle suddenly decelerated by 22mph from 75 to 58 mph in approximately 2 to 4 seconds while cresting a hill on an interstate and navigating on autopilot. There were no vehicles or objects ahead of the vehicle. There was one vehicle behind at an indeterminate distance at the start of the phantom breaking event. The driver had their hand on the wheel before the initiation of the phantom breaking and the autopilot system alarmed and prompted the driver to take control at the onset of the phantom breaking.

The vehicle will randomly start emergency braking for no reason once so ever when using teslas full self driving. This is extremely dangerous as it can happen at any moment while the autopilot feature is in use. In my most recent instance autopilot was set to 70 mph, and with no vehicles are traffic control devices present what is known as “phantom braking” started to occur. Which requires me to quickly reassume control of the vehicle and bring it back up to speed.

The car is plagued by "phantom braking." It occurs when I am on the freeway - the car suddenly brakes and/or decelerates. (Sometimes it is a sudden braking and sometimes the cruise control just turns off.) This is very unsafe - I am fortunate that it has occurred when there is no car close behind me. I feel it happen and put my foot on the accelerator as soon as I can. It's happened as many as 3-4 times during a long (@500 miles) drive, and other times only once. It occurs in various kinds of weather and I have never been able to figure out why it happens. It has happened when there are no cars in front of or next to my car. I believe this problem has been reported many times before. I do not know what system or part is responsible for this. I have had the car since 2019 and this has been happening as long as I can remember. I have never received any kind of a warning or signal that the car would brake. NOTE: B/c the problem is ongoing, I did not put in an approximate date below.

Cruise control is on, autopilot may or may not be on (I have had it happen in both modes). It's a frightening and very dangerous problem. Speeds decelerate from 75 or so to about 35 or 40. It's sudden and scary and potentially life threatening. I have immediately hit the gas in the multiple times this has happened.

Many times this happens near state route 70 traveling north, but I have had this phenomenon happening driving south or headed north on 275. Cruise control is on, autopilot may or may not be on (I have had it happen in both modes). It's a frightening and very dangerous problem. Speeds decelerate from 75 or so to about 35 or 40. It's sudden and scary and potentially life threatening. I have immediately hit the gas in the multiple times this has happened.

I own this Tesla model 3 for approximately 4 years. I was driving this car on US interstate highway 10 east, on July 8, 2023. Cruise control and Navigation on Auto pilot was on. A very serious Phantom breaking took place at around 10:52 am, about couple of miles before exit number 400 on I-10. It was clear a day, there was no car in front of me or on my left lane. I was on right lane. auto pilot suddenly reduced my speed from 72 miles per hours to 39 miles per hour in about 2 to 3 seconds. Due to this, a 16-wheel truck behind my car has to swerve to avoid hitting my car from behind and running over me. My wife was in passenger seat. If the 16 wheeler driver would not have taken evasive action, we will have at least two potential deaths.

My vehicle, a 2019 Tesla Model 3, which is equipped with Tesla's Full Self-Driving Beta software, often fails to detect pedestrians and other objects in its path and react appropriately to them. It is highly concerning that this software is being trialled on public roads and pedestrians put at risk. I recently updated my 2019 Tesla Model 3's software to version 2023.7.15. Ever since I did so I have been getting random, unexplained, and unnecessary decelerations in both adaptive cruise control mode and while driving using the vehicle's autopilot (not FSD). While this is annoying at least it is also unsafe as I could easily be rear-ended for no reason when this happens. It has happened every time I drive the vehicle and usually more than once per drive.

There were no vehicles in front of me and no obstructions in the road when the car did heavy braking (aka phantom braking) for no known reason. If a vehicle had been traveling behind me when this happened, it would be highly likely that a collision would occur because of the random nature of the extreme braking. I did not use the adaptive cruise feature the rest of the trip in fear that a collision would occur. No warning messages were/are present. The Model 3 I drive is equipped with sensors on the front bumper that (I believe) Tesla has deactivated to use vision-only instead. I drove for years using the non-vision (sensor) method without experiencing phantom braking, so this experience is new to me.

At 2401-2499 AL-36, Hartselle, AL 35640, 34.447019, -86.811335, traveling West, the car suddenly slows to 25 mph from the normal 55-60 mph. This sudden slow down startles the driver, passengers, and following traffic which rapidly approaches the rear. The problem is reproducible. Yes, the vehicle has been reported to Tesla using their voice reporting system. There are no warnings prior to the failure. It has happened ever since the road work replacing a bridge completed two years ago. BTW, Google Map Pro still shows the construction barriers at that location.

Unintended acceleration during public street and highway while approaching a stop to make a right turn on November of 2022 in Orange County California onto a highway I let go of the pedal ready to press the brake and while turning it made a unnecessary acceleration to speed up making steering difficult, another incident where I believe in February 2023 as I was driving in the right lane a truck made a sudden stop and the accident avoidance never deployed nor warn me and the acceleration maneuver my steering pedal as I felt it degrease on the brake and increase on the acceleration pedal this was very traumatic and scary knowing I might have bodily injuries or even death also while setting the speed limit I notice while using the regenerative braking the brake pedal would vibrate very aggressively also while using the back up cam and side views it would glitch a lot after a bug update I feel this is very concerning for me and my kids and a lot of reviews and even attorneys or aware of the situation most of all it's a safety issue for California

On 10/17/2021 my son was killed along with his passenger in this Tesla. The car was thought to be going over 100 miles an hour on a city road Coconut Point Road in Estero Florida. The car was thought to have hit a tree with the rear tire and was sent flying through the air and hit another tree and split in half. My son knew this road well as it was close to his home and drove it on his way home from work. I feel strongly that this was an acceleration issue!! He has driven this car for 2 years, so knew the car well.

I live in a very rural area with very little traffic on two-lane country roads. When my cruise control is active, my Tesla sometimes brakes for no reason. A couple of days ago it violently braked from 70 mph to 50 mph for absolutely no reason. On that particular trip of 30 miles or so it "phantom braked" four times. I have had this car for almost 4 years and this problem appears to be getting worse.

While driving in Cruise Control at the Speed Limit at 65 mph with both hands on the wheel and my foot near the brake (on the 395 Freeway in CA on 5/5/23) and no vehicles in front of me, the car suddenly braked hard (the brake pedal fell to the floor under my foot without my foot making any contact with the pedal) and stopped, thus causing the driver/passenger and all contents to shift forward violently. Luckily, there was no one behind the car, no loss of control of the car, no collisions, and no injuries of the driver/passenger as we were both wearing our seat belts. There was a large 16-wheel truck approaching from about 100 feet away on the other side of the road that passed by without any adverse events. It was the second time that this phantom braking had happened on that same road this year. That other time was in Feb 2023.

The car will not allow me to use the cruise control, or put the car into auto-steer. Tesla says it do to a coax cable that is damaged or gone bad. Additionally the day a camera repeater also needs to be replace. The car has only 62,000 miles and is 4 year old. Which is out of Warranty.

In autopilot or cruise control, this vehicle will brake unexpectedly with no vehicle or obstacle in front of it. This has happened primarily on highways, and it has happened between 10 and 15 times, such that now the driver cannot trust either autopilot or cruise control because if someone is following close behind, it could cause a wreck. We reported it to the dealer in Wexford PA prior to a service appointment, and they have not been forthcoming with a solution. We do not believe they inspected the system. There was no warning that this failure was about to occur. Below I report the date of the last 3 occurrences.

The car on multiple occasions while driving on the highway with either the cruise control or autopilot engaged, drastically brakes and slows down for no reason. These slowdowns are drastic and have occurred when there are no other cars or conditions to cause the response. The braking is so dramatic that it is unlikely that I could have intervened in time to prevent a trailing vehicle from potentially rear ending my car. The speed change was so drastic that a trailing driver also may not have been able to react in time. This is a serious issue that must be corrected without delay. On Feb. 18, 2023 when the Tesla model 3 was being driven on I-66E in a regular lane (55mph speed limit) parallel to an I-66 express lane (posted limit of 70 mph) going in the same direction, the car display showed the higher posted speed limit of the express lane. As a result, in assisted cruise control mode, the car would speed up more than 15 mph faster than the permitted speed for the non-express lane. Using assisted cruise control in such a circumstance is a serious hazard, but it has been observed many times on different highways since the vehicle was purchased in 2019. The problem puts the occupants of the car and other vehicles on the road at risk. The problem was initially experienced on I-395 and reported to Tesla in 2019 soon after purchase, but no action or follow-up was made. Recently, the problem has been consistently detected multiple times in similar circumstances.

I think they should buy the vehicles back if you cannot use something as basic as cruise control. This problem is wide spread and extremely dangerous if not deadly under certain conditions. This morning, I was driving using FSD beta and the car swerved and veered almost directly into the curb, head-on. I was able to capture the data using the car's built-in dashcam and saved it, though I can't upload it to this site. I was able to slam on the brakes at the last minute and luckily no one else was around as it could have easily injured someone else and/or damaged my vehicle.

The car has repeatedly slammed on the brakes while using Tesla's Full Self-Driving functionality and made the car unusable at times. I've gotten into many near accidents, with the most recent happening on 1/8/23 where the vehicle slammed on the brakes on the freeway in front of a big rig, almost causing it to rear end me while me, my wife and two children were in the car. At the same time, the vehicle has also almost hit pedestrians while using the FSD beta, which I managed to avoid on 1/8/23 because I was paying attention. Had I not, I'm sure we would've struck a pedestrian walking across the street.

At threshold the light had turned Orange and my son proceeded with same pace as usual, without accelerating. A cross car with an Autistic Kid, most likely as my son is a Special Ed Masters and trained to spot one, rammed into the Tesla 3 which was fully wrecked. The Auto Collision Emergency Detection and Braking should have worked as it was Stop and Go Traffic at Peak Evening Hours between 3:50 and 4:30 pm. I have seen Toyota Prius Auto Collison Braking Work. Someone needs to stop Elon Musk and Tesla charging premium on false promises and taking toll on lives of innocent people trusting their marketing false propoganda and constant spinning on Auto Pilot and FUD with new additional confusion creating packages and options and moving features too and fro. This complicates the software and causes more bugs.

The Auto Collision Emergency Detection and Braking should have worked as it was Stop and Go Traffic at Peak Evening Hours between 3:50 and 4:30 pm. I have seen Toyota Prius Auto Collison Braking Work. Someone needs to stop Elon Musk and Tesla charging premium on false promises and taking toll on lives of innocent people trusting their marketing false propoganda and constant spinning on Auto Pilot and FUD with new additional confusion creating packages and options and moving features too and fro. This complicates the software and causes more bugs.

12/24 between 7:54am and 8:55am driving on the 10 west from Arizona to California while on auto drive within one hour experienced 4 times the car engaged the brake while traveling 80 mph on the highway. All 4 times there was nothing in front or side of the vehicle. The vehicle did not alarm a sensor that something was in the way. Fortunately we reacted quickly and continued with the accelerator so we didn’t stop abruptly on the Highway. 2 out of the 4 times it was a very hard brake the other two times it was hard but not nearly as forceful. Again nothing in the road, sun behind us, no indicators of obstructed cameras. We haven’t been on a long trip with the car in a long time, however my son drove from Orange County , CA to Los Angeles one evening about 6 weeks ago and he said the same thing occurred 3 times while on the trip and on the highway. I did Google the situation and it sounds like I am not the only one who has experienced the phantom braking situation. We have disengaged the auto drive and auto pilot the remainder of the trip.

Phantom braking is occurring regularly on a road trip from CA to LA. It has happened on clear roads with sunny conditions and no obstructions as well as in shadowy or dark conditions. It only occurs with cruise control on.

Two lane highway I was in the left lane with the cruise control on not Full Self Driving. In the right lane was a semi which I came up along side off on the left , I was driving 70mph. (Speed limit) The car suddenly hard braked on it’s own. The car behind me had to brake to avoid an accident and went into the dirt. After that on this same drive, there were at least 6 more instances of phantom braking when traveling over a “wash bridge “ going under a highway sign, driving along side another semi and once without anyone near and an open road. This also happened 3 months ago and when I reported it to Tesla they said when I received the FULL self driving down load which I got the next day, the problem should be corrected. I was traveling approximately 70 mph in passing lane when I engaged (standard) autopilot, as I approached to pass a vehicle with another vehicle following me in the same lane as I was traveling. My vehicle (phantom) braked hard to which the wheels began to squeal slightly and the vehicle behind me had to brake hard to avoid hitting my vehicle. I almost immediately applied pressure to the accelerator pedal to avoid a complete stopping of the vehicle and to which the vehicle responded to accelerate. And I disengaged the autopilot immediately by applying an upward motion on the gear stock.

I experienced more than 10 incidents of “phantom braking” while on Interstate highways (I90 and I29) on a round trip from Solon Iowa to western South Dakota. ( late Sep 2022). There were no apparent obstructions, nearby vehicles, or other dangers. In several incidents I was traveling at the speed limit (80 mph) and the car braked so suddenly and hard it jolted wife and terrified her. If a car or truck was behind me I would have almost certainly had a serious accident. This behavior seems to have gotten much worse in the past few months, although I have seen it occasionally over the 3 years (48000 miles) I have owned the car. I have noticed it happens most often near the crest of hills.

Yesterday I had the most frightening driving experience of my entire life. It was raining heavily and I was going about 35 mph on the local road in my model 3 standard, 3 years old. Suddenly I lost all traction and was slipping side to side +/-90 degrees from the direction of traffic. I immediately took my foot off the accelerator and tried to slow down but didn’t brake. I swung 180 degrees each way about 5 times. Luckily no one hit me but I was taking up all lanes as I careened all over the road. I can’t believe how easily the model 3 loses traction in the rain. This is unacceptable.

While driving on the freeway tonight at about 11:45 pm with no one nearby and no shadows or debris in the road, the car on autopilot slammed on the brakes, taking the car from 68MPH to below 45MPH in less than a second, which could have resulted in a rear end collision had someone been behind the car. The car has done this many times in the past but this time was unusual in that three beeps occurred right as the car slammed on the brakes. My friend was driving the vehicle (a 2019 Tesla Model 3) while it was engaged in Full Self-Driving Beta 10.12.2 mode. While FSD Beta was engaged, the driver approached an intersection, the FSD stopped correctly to yield to oncoming traffic on a one-way street. The vehicle was travelling across the intersection according to its navigation. There was also a cyclist ahead of the Tesla at the intersection. As the cyclist entered the intersection, the Tesla followed and attempted to travel around the cyclist to complete the manoeuvre and continue travelling straight ahead. However, the Tesla then deviated from its course and instead travelled left at the intersection, travelling the wrong way on one-way street, causing the driver to perform an emergency stop and reverse out of the dangerous situation. The Tesla appeared to panic at the presence of the cyclist when attempting to travel around it, and then headed directly into oncoming traffic while travelling the wrong way up the one-way street. My friend was driving the vehicle (Tesla Model 3, 2019) while Full Self-Driving Beta 10.12.2 mode was engaged. The FSD Tesla makes a left turn at an intersection on a protected left turn, but slows down considerably just before the traffic lights - which did not apply to the Tesla but rather the traffic from the opposite side of the intersection - after making the turn, stopping in the middle of the lane. The driver disengages and accelerates to complete the manoeuvre, to avoid a rear collision. The Tesla may have mistaken the red traffic light showing for other traffic as one it should pay attention to, almost causing a collision and displaying the inadequacies of its vision and cameras. My friend was driving the vehicle (a 2019 Tesla Model 3) while it was engaged in Full Self-Driving Beta 10.12.2 mode. The Tesla fails to slow down when approaching a curve in the road resulting in taking the corner far too quickly, despite the fact that there is a sign by the road warning drivers to slow down to 15mph. Tesla also failed to recognise the change in speed limit due to a blind turn in the road approaching, the car continued to go at a rapid speed (30mph) and even turned left when the road curved toward the right, leading to the driver taking control to steer the car back on path. During the turn, the Tesla was engaged in FSD Beta 10.12.2 and crossed the dividing line into the opposite lane. If another vehicle was approaching this incident would have resulted in a collision as the Tesla strayed into the path of oncoming traffic. My friend was driving my car (a Tesla Model 3) at the time of this incident. At the time of the incident the Tesla’s Full Self-Driving Beta system (version 10.12.2) was active. The Tesla fails to slow down when approaching a series of sharp bends in the road resulting in taking the first corner far too quickly. The posted speed limit according to the sign was 15 mph, however, the Tesla was travelling at approximately 30mph, double the posted speed limit. The Tesla therefore drives into the turn too widely and tries to steer into the oncoming traffic lane, forcing the driver to intervene quickly to correct the manoeuvre and reposition the car back into the lane. My car brakes for no apparent reason consistently in the same location every time. What has been described as “phantom braking”. When I hit two spots on I5 northbound near the berkley exit. The set max speed of my cruise control will suddenly drop and the vehicle will brake. The speed then adjusts up to the prior setting and the vehicle will return to normal. This in total lasts about 5 seconds but results in dangerous braking.

While in Tesla’s auto pilot mode, the vehicle breaks for no particular reason. This is particularly very dangerous while going 70 mph and someone is following in close proximity. I have gotten side eyes and hand gestures from fellow motorists, I presume it is due to them thinking I break checked but I am very defensive driver and I would not put anyone at risks for any reason. I got rear ended back in 2019 in slow speed but do attribute this to Tesla sudden breaking issue. I have since stop auto auto pilot in regular basis. I do use auto pilot once here and there often in light traffic.

A Tesla model 3 was eastbound on a two-lane roadway at approximately 7:20am local time. A westbound school bus slowed, and activated its amber warning lights, followed by the red stop lights and driver side stop sign. The Tesla, proceeded through the traffic control devices of the school bus while the bus was accepting child passengers. It was evident that the "driver" of the model 3 had full self-driving mode engaged, and was not fully paying attention to the road as he was clearly interacting with his smart phone, held at steering wheel height. The Tesla FSD system clearly failed to appropriately stop and wait for the school bus traffic controls. Had the students needed to cross the highway (instead of only boarding from the shoulder) a serious vehicle/pedestrian incident could have resulted. NOTE: Model 3 was NOT my vehicle - This form is flawed that it makes the assumption that the reporting party is the owner of the vehicle.

The traffic-aware cruise control frequently brakes hard at high speeds (70-80mph). It will brake for no reason, bringing the car's speed from 80mph to 30mph in a few seconds. This happens about 3 times an hour. If there was a car close behind me whenever it happens, this could have caused significant harm. I feel this is an extreme safety risk. My vehicle has not been inspected, but this is a common complaint among Tesla owners in recent months.

While streaming Bluetooth audio, the forward collision warning fails to activate sometimes until after a few seconds. What happens is when it is supposed to sound, all audio in the car stops and there is a clicking for a few seconds. Then after a few seconds, the alert goes off. The alert is very dangerous because it severely reduces the time for a reaction. Secondly the backup camera has ghost images and stutters. When placing the vehicle in reverse, a ghost image appears for a second or two then goes into the live view. The live view is very laggy and non responsive. The image freezes and does not update. This causes issues when backing as any sudden movements may not be recorded and will cause an injury because the driver will not be able to see what is going on.

I have a Tesla model 3 SR and it only has the standard autopilot option, not the full self driving option. When on the freeway in a single lane and autopilot is engaged, if the single lane splits into two lanes, the car will swerve violently trying to determine which lane to take. This is a danger for the other vehicles behind me, as they are confused as to which lane the vehicle will take. Sometimes it will take a left or right hand turn lane and then start beeping when it determines that it made a mistake. A huge safety risk, as somebody would be thinking that I would move left or right, but instead the car moves into the lane going straight ahead at the last minute.

The scenario mentioned has happened numerous times. When using Autopilot on a two lane highway, the vehicle's AEB kicked in and slammed on the brakes multiple times. My theory is that since ditching radar, the vehicle thinks that the oncoming traffic is coming into its current lane even though there is a set of double yellow lines dividing it. This phantom braking issue is especially prevalent when there are curves in the road, as momentarily it may appear that oncoming traffic is "head on". This resulted in me almost getting rear ended by the vehicles behind me and causing a multi-vehicle accident.

Even after the firmware update which was supposed to have FSD beta come to a full stop, the car still does not come to a full stop. The car slows to a speed of around 0.5 mph at a stop line with a stop sign and continues on if it is clear, but only comes to a full stop if it believes it needs to yield to other vehicles. Watching videos of other vehicles demonstrating the capabilities of FSD beta with the latest firmware version show the same behavior.

Several times while driving highway Route 2 in Massachusetts, westbound and eastbound, the adaptive cruise control system slammed on the brakes for absolutely no reason. There was nothing in front of the car for 100 yards, sometimes longer. It happened at least 10 times within an hour. This caused a dangerous situation for myself and the cars behind me who were not expecting my vehicle to brake suddenly and for no reason. There have been many other cases of this submitted to the NHTSA, it is not an isolated problem. There were no warnings displayed during the phantom hard braking incidents.

There is not an option for a regular cruise control, rather, Tesla activates an autopilot feature when the cruise is activated - the adaptive breaking is consistently applying the breaks when there is no reason. Unfortunately on vision the car brakes aggressively (10+ mph decrease) approximately one time per five miles on the local undivided highways of Central Oregon. This is not safe for me or for other drivers. I have contacted Tesla repeatedly to opt out of the FSD beta program and reenabled radar, but they have not responded. Customer service and service centers are not empowered to fix the situation. This situation puts me and other drivers at risk and could be easily remedied by Tesla by adding a process for FSD beta program removal. Clearly their current approach is not working.

Every time I am on a two lane road, and sometimes even on divided highways, I experience numerous instances where the car seems to think there is an imminent collision, where the brakes are immediately applied excessively and for seemingly no reason. This issue is present with either the FSD beta engaged or simply having cruise control engaged at all. The issue is far worse at night, and in a 1 hour drive it is likely to happen several times. In addition to the above, the car also forces the auto high beams back onto auto when re-engaging the system after I've had to take control to stop it from stopping or swerving unnecessarily. Since the auto high beams are really bad, the car often will flash oncoming drivers on its own, and again for seemingly no reason. I usually remember to disable it every time, but have also forgotten and flashed oncoming drivers due to this as well. In addition to the above, the FSD system will make swerves that make no sense at times as well. For instance, it turned into what it thought to be the turning lane a block early, but maintained speed for the actual intended turn. When it saw the lines ending, it abruptly made a 90 degree turn a block early as it didn't know what to do. While I feel competent enough to be MORE alert than an average driver should expect to be when driving, I feel it is a matter of time, if it hasn't already happened, where Teslas cause a rear end accident, injury, or even death, from their seemingly random and sudden movements by a driver that doesn't understand the limits of the cars. For the approximate date field below......it happens every time I drive on a two lane road, and even more so at night. It's not a one time instance. It's every single drive. I'll put the date the last time I drove longer than a couple miles from home, but want to emphasize that everything described above can be easily reproduced on every single drive.

The screen on my Tesla fails routinely (everyday) while driving and then reboots after about 3 minutes. This happens abruptly with no warning. Tesla has told me (verbally) that it was reproduced in their service location in Palo Alto. During this time, I have no idea of my speed or any other vehicle status, such as lane keeping, collision warning, navigation, heating, etc. I have tried to remedy this issue with Tesla service repeatedly over two months, and the interactions have been unsatisfactory. Tesla does not seem to appreciate the potential seriousness of this issue and the risk to ourselves and others. Tesla Service at Sunnyvale, CA, encouraged me to continue driving, even though I pointed out the risk of "driving blind". My family is very wary of this vehicle now.

June 2021 I had an accident while Tesla Auto Pilot was engaged. I was paying complete attention to the road, there was vehicle merging to the highway from right. Neither Tesla slows down neither it have any warning and vehicle collided. Tesla Auto Pilot is misleading, and have serious safety concerns. June 2021 I had an accident while Tesla Auto Pilot was engaged. I was paying complete attention to the road, there was vehicle merging to the highway from right. Neither Tesla slows down neither it have any warning and vehicle collided. Tesla Auto Pilot is misleading, and have serious safety concerns. After getting a shingles vaccination and eating lunch, a vaccine side-effect suddenly caused me to become extremely sleepy while driving. But AutoPilot drove me safely to my destination parking lot where I took a two hour nap. AutoPilot saved my life and car from an accident.

Ever since I have owned the car, there have been intermittent bouts with what most Tesla owners refer to as phantom braking while using the Traffic aware cruise control (TACC) or autopilot (AP). In all instances, I'm driving along the I-15 highway in Southern California at or near the speed limit with the flow of traffic, with no visible obstacles to safe driving in my lane or lanes adjacent to me, the car suddenly slams on the brakes out of nowhere and it is absolutely terrifying and unsafe. Today, had someone been tailgating me at 80mph and this occurred, I am certain I would have been rear-ended. The deceleration was so sudden and aggressive that I felt the blood rush to the front of my face and I became dizzy momentarily. I can't remain silent on this any longer as I hoped Tesla would have fixed this through software updates. I no longer have trust in the system. I've reported phantom braking to Tesla numerous times through their in-car bug report feature and there still is no fix. No other automakers are experiencing this with their ADAS systems and yet Tesla is apparently leading us into Full Self Driving. Please look into and investigate this for the safety of all drivers out there today. Thank you.

It was about 9PM, quite dark at 70 or 80MPH on I80 in a construction area. The headlights stayed on and the car continued driving but the dash went black as it seemed to fail, and then seemed to reboot with the "T" coming back up before the full dash came up again.

We were driving our Model 3 on autopilot on the LIE from Southampton to New York. We have the “Full Self Driving” software from Tesla. We drove past a parked SUV about halfway through the ride, but the Tesla did not notice. It drove straight through the side of the SUV, ripping off the side mirror and skullcap from our car. We checked and the SUV driver was totally fine, scared. We called Tesla immediately and told them that in our opinion, our car had gone crazy and did not see the but my wife and I were other car stopped in the shoulder as we drove by at 60 mph. We have the invoices from Tesla service documenting this issue and their repair of it, but we were then and remain now really scared when a car has its hazards on in the shoulder.

Jun 29, 2021 Crash

On June 6th, 2021, we were driving home in our 2019 Tesla Model 3. We were at the intersection of 7th Street and 36th Avenue in Moline, Illinois. We were stopped on 36th Avenue heading east trying to turn north onto 7th street. We were stopped behind another vehicle when our Tesla started creeping slowly forward toward the vehicle in front of us. My wife, who was driving, stepped on the brake. But instead of stopping, the Tesla suddenly accelerated, careening off the car in front of us and then heading into the intersection at full speed. My wife was able to avoid hitting two vehicles on 7th Street in the intersection, but then hit a steel stoplight pole across the intersection. The right front passenger tire was torn off the Tesla and the airbags deployed. No one was hurt. But we damaged two other vehicles and totaled our Tesla. My wife insists that she did not hit the accelerator pedal. I believe I remember when she attempted to stop the car, she might have accidentally bumped the control behind the steering wheel that activates the adaptive cruise control. I do not know. But the adaptive cruise control should not activate under 35 MPH. The Moline Police department investigated the accident and issued report number 2021005635. Front facing camera, used by autopilot, obscured by contaminant on the inside of the windshield. Autopilot engaged without warning, even though the front facing camera was significantly impaired due to the contaminant. Contaminant appear to be glue residue based upon tackiness. The front facing camera is surrounded by an anti reflection enclosure. Upon inspection, the anti reflection enclosure was significantly degraded and broke into many pieces while being serviced. As this enclosure is designed to absorb light, it’s likely getting extremely hot during the summer months in a car parked facing the sun.

WHEN I'M DRIVING ON AN INTERSTATE HIGHWAY WITH HOT LANE TO THE LEFT OF MY DIRECTION OF TRAVEL, THE CAR ERRONEOUSLY SENSES THE SPEED LIMIT AS 65 MPH (THE SPEED LIMIT FOR THE ADJACENT HOT LANE) RATHER THAN THE ACTUAL SPEED LIMIT OF 55 MPH IN MY LANE. THE RESULT IS THAT WHEN I AM USING THE ADAPTIVE CRUISE CONTROL IN THE SITUATION DESCRIBED, THE CAR SUDDENLY AND WITHOUT WARNING ACCELERATES BY AT LEAST 10 MPH OVER THE POSTED SPEED LIMIT. THIS HAS HAPPENED REPEATEDLY AND MAKES USE OF THE ADAPTIVE CRUISE UNSAFE.

MY SON WAS DRIVING ON THE GOLDEN GATE BRIDGE WHEN THE AUTO PILOT APPLIED THE BRAKES. HE SAID A TRAFFIC CONE APPEARED ON THE SCREEN. THERE WAS NO TRAFFIC CONE OR ANY OTHER OBJECT IN THE ROAD. THIS ISSUE CAUSED AN ACCIDENT BEHIND HIM BECAUSE OF THE SUDDEN BRAKE APPLICATION. THIS ALSO HAPPENED TO ME WHILE DRIVING ON THE FREEWAY. I HAD THE CAR ON CRUISE CONTROL. ALL OF A SUDDEN THE BRAKES WERE AUTOMATICALLY APPLIED WITHOUT ANYTHING IN THE ROAD.

WHITE BLOCK APPEARED ON SCREEN, WOULD NOT CLEAR. BLOCKED VISIBILITY OF ANY DISPLAYS UNDER THE BLOCK, INCLUDING GEAR SELECTOR, SPEED, ENERGY STATUS, TURN SIGNALS, MOST OF ANY OTHER DISPLAY, SUCH AS MAPS, CAMERA DISPLAY. BLOCK REMAINED WHEN WE LEFT THE CAR, AND RETURNED. ONLY COULD CLEAR W/ DISPLAY RESET, WHICH I UNCOVERED ONCE I RETURNED HOME AND LOOKED ONLINE. WHITE BLOCK DID NOT APPEAR TO IMPACT DRIVING, BUT OBSCURED BASICALLY EVERY DRIVING FEEDBACK YOU'D EXPECT FROM A DASHBOARD.

TL* THE CONTACT OWNS A 2019 TESLA MODEL 3. THE CONTACT STATED THAT HE PURCHASED THE SPECIAL FEATURE ADAPTIVE AUTO PILOT BACK IN 06/2020. THE CONTACT STATED THAT THE AUTOMATIC EMERGENCY BRAKING ACTIVATED INDEPENDENTLY WHILE DRIVING 65 MPH, ALMOST BRINGING THE VEHICLE TO AN ABRUPT STOP THREE TIMES ON THE SAME DAY. THE BRAKE PEDAL WAS DEPRESSED TO DEACTIVATE THE FEATURE, ALLOWING FOR THE VEHICLE TO ACCELERATE. THE VEHICLE WAS TAKEN TO TESLA (840 PROVIDENCE HWY, DEDHAM, MA 02026 (781) 471-3001) TO BE DIAGNOSED. THE MECHANIC WAS UNABLE TO DUPLICATE THE FAILURE. THE MANUFACTURER WAS CONTACTED AND INFORMED OF THE FAILURE. THE MANUFACTURER SUGGESTED THAT THE FEATURE BE TURNED OFF. THE VEHICLE WAS NOT REPAIRED. THE FAILURE MILEAGE WAS 14,000.
