The Tesla ran a red light on two separate occasions while in full self driving mode. The first time it went through without me being able to intervene. Given it had done it before, the second time I was able to stop it. Both times occurred at the intersection of [XXX] and [XXX] . It has not done it at other intersections. During driving, Tesla car spontaneously stated multiple active safety features and regenerative braking were no longer available and stopped working during driving with multiple warnings appearing. Driver safety at risk during this as well as others safety due to the lack of functioning. The car was taken to Tesla service center and was explained that front lower lateral suspension links were damaged by "impact" although the car was never in an impact or accident preventing proper alignment and functioning of the car. The part has been since repaired and currently undergoing further investigation for warranty dispute.

Curvature Assist feature agressively brakes while driving at highway speeds. This is supposed to slow down for curves, but frequently intervenes for straight, clearly marked lanes. Feature cannot be turned off. Drivers behind me have gotten agitated several times for the sudden, harsh braking, and I do not feel comfortable driving this car.

The full self driving is very dangerous. I had several events that put me in danger. Like, going to the wrong lane. Going to the wrong way. Changing lanes that almost hit a car good thing I took over right away. It made the other car upset and honk at me.

Software updates caused failure notifications (seat restraint system) and disabled cameras/emergency brakes, happened immediately right after software updates. All hardware performed and properly functioned before the software updates. 1. First failure noticed on the begin of March 2024, component affected : " Rear center safety restraint system fault". Second failure noticed on [XXX] component affected: " Autopilot cameras unavailable", "Automatic emergency braking is unavailable" 2. I feel extremely unsafe now when using the vehicle with failure notification of the car's components, especially while driving my children to school. I am unsure if the system failure that could causes accidents and put my kids at risk or danger. 3. First issue was notified Tesla representative immediately right after the software update (happened over night). They scheduled an appointment, but I saw a charge bill that I have to approve before taking the car in for an inspection. It happened when my car is just out of warranty. I called Tesla dealership in Buena Park, CA to discuss about my safety concerns and got told it might be just a software glitch and I should be ok to drive. (??). Second issue was also notified Tesla and got a schedule for an inspection/service on December 10th, 2024, and again with an approval of $650 that I might got charged for ahead of the service ???? (Why do I get charged for something that wasn't my false until Tesla pushed a bad software to make us pay for their bad scripts?) 4. There was no failure notification "at all" before the last 2 or 3 software updates. Tesla offered some kind of 1 month free FSD then software updates to remove the FSD then everything started to fall apart. I am very concerned with Tesla software updates now and failures it might cause that put our family at risk because of their bad scripts or batches. On several occasions, the last being July 19,2024, auto correct on steering took control of my vehicle when no correction was needed. I’ve been taken by surprise and struggled on all occasions to gain full control of the vehicle. In addition, my car is not able to receive the over-air update to address the hood latch recall which leaves me more vulnerable to a catastrophic event. I can not reach a person in Service as Tesla pushes every communication through the app. My appointment was pushed out over three weeks and I still have not heard from Tesla to confirm this appointment nor reach out with any concern whatsoever regarding this horrific safety concern.

Tesla Motors has two self-driving features, a mode they call enhanced autopilot and Full Self-Driving. Enhanced Autopilot is used primarily on freeways, and allows the car to switch lanes without driver's assistance, while maintaining the speed of traffic. Full Self Driving is a separate system in and of itself that relies on far more sensors, and is far more complex, as it needs to be in order to handle all the nuances of allowing a car to drive itself on and around city streets. During the month of April ,Tesla previewed the supposed improvements in its full self driving mode by releasing it to everyone for free for that month. I was exiting the [XXX] freeway in Los Angeles, utilizing the enhanced autopilot feature to navigate myself to my destination. At this exit, the car makes a right on [XXX], drives down about 500 feet and then makes another right so that you're briefly traveling underneath the freeway from you'd just exited. As the car made that second right, while on what should have full self-driving mode, the car failed to navigate the angle of that turn and ran right into a steep curb at a fairly fast speed, destroying the right drive train, the right tire, the entire front bumper, and the cars wheel chassis and suspension system all still have to be fixed. I believe the malfunction was caused by the car failing to properly switch between enhanced autopilot and full self-driving if they utilize GPS to make that determination, because it felt almost as if the car was trying to switch into the lane to the right (which didnt exist there was the curb there) and not like it was attempting to negotiate or preparing to make a wider turn maneuver. I don't believe it even decelerated as it hit the curb. I broke my toe but otherwise am all right. Tesla won't honor their warranty and is threatening to repo the car they won't fix. 1. My 90 year-old mother-in law got trapped inside the car in very bad weather because my phone key wouldn't open the door after I got out and walked her elderly friend to the door of her house about 20 feet away, and then walked back to the Tesla. I've been able to reproduce the problem repeatedly, of course without my mother-in-law in the car. 2. Several near-misses because, unlike most other cars, the Tesla's blind-spot warning system, and many other basic control functions such as adjusting the windshield wipers, opening the glove compartment, etc, require you to take your eyes off the road. For example changing lanes requires you to look away from the lane you're about to move into to check the blind spot display on the touch screen. 3. Several near misses because easing up on the accelerator slows down the car forcefully 98% of the time, but does nothing 2% of the time. Specifically, accidentally flicking down on the right-hand control stick, which turns on the windshield wipers in most cars, activates cruise control and gives minimal feedback to the driver that cruise control has indeed been activated. In this circumstance, easing up on the accelerator pedal doesn't slow down the car at all. This is very frightening and unexpected and could easily cause an accident.

Constant lane change while driving has gotten out of control. A couple of instance the vehicle has changed lanes without manual manipulation. This is getting out of hand and has certainly put risks on my family while operating vehicle.

Tesla autopilot drifted over two lanes and into the side of a semi-truck on the freeway. It was night time but with plenty of light, no rain, and clearly marked lines separating the lanes. Not only did autopilot fail, but the features to help avoid drifting out of a lane even when not using autopilot, also failed. Those features are supposed to auto-steer back into the lane and beep to alert the driver. None of this occurred. 1. Yes vehicle is available for inspection upon request 2. Could have easily led to extensive damage, injuries or fatalities. Everyone got lucky 3. Problem has not been reproduced by dealer, but I have video of the event 4. Vehicle has not been inspected by any 3rd parties

So thanks to YOUR interference my Autopilot on my Tesla is broken... You guys THINK you are making our cars safer by getting your controlling poorly educated stupid hands into Tesla systems that are DESIGNED TO BE MORE SAFE. On paper Tesla Autopilot is factually proven to be more safe than a human driver, these reports are publicly available perhaps your team was too lazy to read them. My car disengaged Autopilot randomly while driving with 2 cars behind me and freaked out and told me I was not paying enough attention to the road just because I reached to unlock my phone screen...not even pick it up off the wireless charging pad...just unlocked the screen. So thanks to YOUR agencies interference and forcing Tesla to levy this bogus "recall fix" on us I almost got rear-ended...in the car I work so hard to pay for, the car that I love because it keeps me and my family safe because guess what we are humans and not robots. Just as YOU do and everyone in your agency we ALL have to look at our phones briefly while driving because we live lives where we have to make sure those we love are ok or make sure our phone is charged for when we reach our destination. You are going to do exactly the opposite of your agencies supposed mission of keeping us safe. You will kill people with this stupid "recall". Get your crap together and tell Tesla to reverse Autopilot back to its normal safe operating parameters and STOP BANNING PEOPLE FROM AUTOPILOT otherwise there are going to be a lot more wrecks and deaths. I'm not saying you should be taking duck selfie pictures for social media with your phone while in Autopilot on a rural back road...that person deserves to be a victim of modern natural selection. But what your doing is over-controlling and you have no right to dabble in things you obviously don't understand. Your agency should be ashamed of itself, you are about as efficient as our government is at managing money...FIX IT BACK

Subject: Safety Concerns Regarding Recent Model 3 Autopilot Update To Whom It May Concern at the National Highway Traffic Safety Administration (NHTSA), I am writing to urgently express my concerns about the latest software update for the Tesla Model 3’s autopilot system, which, in my view, significantly compromises safety. As a Model 3 owner, I have generally found the vehicle and its features to be reliable and safe. However, the recent update has introduced an overly stringent hands-on-wheel detection mechanism that is not only inconvenient but also potentially hazardous. The new update requires frequent and often forceful interaction with the steering wheel to assure the system of driver presence. This change is drastically different from my previous experience, where I received only one hands-on-wheel violation in two years. The current sensitivity of the system disrupts the smooth operation of the autopilot, leading to frequent and abrupt disengagements. I have found myself struggling to maintain the system’s activation, inadvertently causing the vehicle to exit autopilot mode multiple times. This issue goes beyond mere inconvenience; it actively detracts from driving safety. The need for constant and sometimes aggressive adjustments to satisfy the system’s requirements is distracting and can lead to erratic vehicle behavior. The irony is stark: a system designed to enhance driving safety and ease is now a source of potential danger and stress. The unpredictability and over-sensitivity of the updated system could lead to dangerous situations, especially on highways or in heavy traffic, where sudden disengagement of the autopilot can be particularly risky. As a driver, I now find myself more focused on keeping the autopilot engaged than on the actual driving conditions and surroundings, which is surely contrary to the feature’s intended purpose. Please fix this issue. It is bad for everyone.

Autopilot now requires so much force to confirm you have your hands on the wheel it’s disengages causing rapid deceleration and steering into the other lane or shoulder. Also requires confirmation at times when the driver should be looking at mirrors when passing etc instead of watching the display.

Summary: FSD malfunctioned, failed to disengage, causing loss of steering control, thus causing car to steer into wrong lane and resulting in an accident. Tesla Service Center refused to inspect unless car is fixed. Car was sent to Crash Champions ( autobody shop) and was quoted $35k to fix. Car was deemed as totaled by Tesla insurance. No response from [email protected] I suspect that the latest FSD software update has caused my car to lose control. Time of incident: 19:28 pm Location : [XXX] , [XXX] Entering from [XXX] . Weather condition: Clear night, well lit. Low traffic. What happened:
 My car has been driving in FSD on Lawrence Expressway and it was doing everything correctly. Approaching that upcoming tight bend, the FSD slowed down the car, and I took over by steering the steering wheel to the right to deactivate FSD and taking the tight turn, right before the white rows of triangles on the street. At this moment, the steering wheel forcibly turned to the opposite direction (i.e. left). I tried to correct the steering wheel by turning right even more, but the steering wheel wasn’t responding to my input for a split second. At this moment, my car is now on the wrong lane of Wildwood Ave and my car finally responded to my steering input. I had to swerve in the opposite direction to avoid the incoming traffic and ended up brushing against an SUV, followed by an impact on the curb, before finally slowing down. Seeing that there was incoming traffic from the back, I drove down to a residential street to park my car for safety reason. The SUV driver called the police to handle the situation, we had our information and testimonies recorded down. I have already followed the safe practices of driving with FSD according to the manual, but in this situation, I am not certain which technology is responsible for causing my car to override my manual steering input. Vehicle started displaying warning messages as I pulled out of garage - "steering assist reduced" and "lane departure avoidance features unavailable." Afterwards, I noticed vibration and clicking sensation while turning steering wheel and driving at low speeds. I performed a software reset but messages were still there. Then I performed controls, safety, power off procedure. Messages disappeared, but steering still felt jerky. This happened when the vehicle had approximately 2,300 miles on it. A few days later, when exiting the garage, steering wheel was initially locked. It then "popped" when I applied gentle pressure to turn the wheel and the warning messages returned. Steering felt stuttery afterwards. I scheduled service appointment with Tesla. They replaced steering rack. I have had no problems since. Currently at 3,700 miles.

As part of transitioning from radar to a camera based system (this affects model years '22 onward), Tesla, has "temporarily" disabled key features such as lane departure, blind spot alerts and emergency braking. Though disabled, these features are still selectable (not greyed out) when the user chooses safety options. These features, which are available on most modern cars, have been disabled for many months with no committed date to restore functionality ... this poses a major safety risk. This should impact Tesla's NHTSA safety rating which will pressure the company to re-enable these basic safety features ASAP. Thank you.

Power steering stopped working when the car starts up, Cannot steer the wheel at all in PDNR. Basically cannot steer the car at all and it is not drivable.

I was driving and the power steering stopped functioning. The car had just over 1000 miles and both the lane departure avoidance and steering assistance cut out. Restarting the start would bring it back on temporarily but it would cut out again after a minute or two.

Adaptive cruise control / braking fail to function. Problem first reported to Tesla on 7/6/2023. Telsa installed software update to resolve remotely. Problem recurred 8/6/2023, towed to Tesla 8/7, Tesla replaced side camera assembly. Problem recurred 8/132023, software update installed & resolved. Problem recurred 8/19/2023. Vehicle to be towed to Tesla for further diagnosis / repair.

In past three months we have encountered multiple instances where Tesla was not able to perform as promised this morning I was driving tesla loaner Vehicle when I was heading to pick up my car and someone drifted into my lane without warning and Tesla did not apply emergency brakes or generated any warning signals. I have notified Tesla service about the incident and past incidents to tesla service representative. Tesla representative at Costa Mesa office opened the service request to get the software diagnosis completed so that they can get to the bottom of it, but [XXX] from Irvine office called me and he is in denial that it is a car issue and blame it on me that it is between the car and the car owners manual, I have requested an escalation from Tesla service Center. In the meantime, I am filing this complaint so that NHTSA can start their investigation before. It is too late and some life is lost because of irresponsible behavior by Tesla and send in selling the features which do not work I have Lexus RX 350, 2017 model and all these features work seamlessly in the car , I am shocked Tesla is falsifying and and selling inaccurate features to the consumers which cannot be discovered until it is too late. Tesla has serious safety issues automatic emergency, braking Blindspot warning, Lane, departure warning, and other basic safety features which are very common in the cars these days. I purchased Tesla for my teenage child. In past three months we have encountered multiple instances where Tesla was not able to perform as promised this morning I was driving tesla loaner Vehicle when I was heading to pick up my car and someone drifted into my lane without warning and Tesla did not apply emergency brakes or generated any warning signals. I took deliver of the Model 3 Performance in late October 2022 and have experienced phantom braking at least a dozen times over the last few months on the highway/interstate when using cruise control and Tesla’s standard autopilot, which keeps the vehicle centered in the lane. The phantom braking has occurred both in times with just the cruise control enabled and also when both cruise and autopilot have been enabled. The vehicle has slowed from 70 to 60 and even 50 miles per hour very quickly when there was no object nearby that would cause a collision. This is both frightening and upsetting, as such sudden braking could cause an accident, especially with any car following me from behind. I am fearful to use the cruise or autopilot as a result of this defect.
