November 20, 2025 To the National Highway Traffic Safety Administration (NHTSA) Office of Defects Investigation Subject: Safety Defect Complaint – Tesla Model S I am submitting this complaint regarding a serious and dangerous safety defect involving my Tesla Model S. Safety Defect Summary:    •   After the high-voltage battery failed, the vehicle would not enter Neutral.    •   I had to use Tow Mode in the middle of a street on a hill to avoid blocking traffic.    •   Once in Tow Mode, the car could not enter Park and the emergency/parking brake would not engage.    •   The car became completely free-rolling, posing danger to myself, other drivers, and pedestrians.    •   When Tesla returned the car to my home, their technicians also could not get the vehicle into Park or activate any emergency braking system.    •   The car is currently stuck in Tow Mode in my driveway with physical objects placed behind the wheels to prevent it from rolling. Why This Is a Safety Hazard: A vehicle that cannot shift into Park, cannot use its emergency brake, and remains free-moving after a failure represents a serious safety defect. This occurred without warning and left the vehicle unsafe to operate or even store. Request: I respectfully request that NHTSA investigate this as a potential safety defect affecting the Tesla Model S when the high-voltage battery fails. A car should never lose all ability to secure itself.

When parked and opening the drivers door or rear passenger doors, the front passenger door will unlatch. Something is setting off the relay to open the front door when other doors open. Does not happen 100% of the time, but 50%+

About 6 mo. ago, a sticky substance started leaking under the center touch screen. I went to the dealer, and was told that the screen was leaking due to high temperatures inside the car while parked. They refused to do anything about it. I went online to a Tesla forum, and learned that this is a common problem with the Model S touch screen. Ultimately, the screen will "bubble" and the go completely blank. Without this screen I cannot access navigation, climate, or entertainment features. I believe this to be a major safety issue.

The central display computer goes blank while driving or when the car heats up. Suddenly the car does not respond to any touch screen. AC cannot be managed, rear camera is not available and the car is undrivable. The screen comes back when the car cools down completely. This happens everytime the car heats up -whether it is parked outside for couple of hours or inside the garage on a hot day or when I am driving for over an hour. This incident has occurred multiple times and I have to take it in to be repaired

In November 2020, I had my Tesla MCU replaced under the NHTSA recall (Exhibit1). Attached is the invoice of the repair (Exhibit 2). On July 3, 2023, my MCU malfunctioned again. The dashboard is frozen showing the car is at 72 MPH in drive in a parking lot (exhibit 3). This is a safety issue since I cannot tell how fast I am driving when I am going with the flow of traffic. I nearly hit a person since I could not tell what gear I was in and ended up going in reverse rather than drive. I contacted Tesla requesting repair under the recall. Their response was that my car is old and they would not cover it even though it drives fine. I drove home in the slow lane since I could not tell my speed. They sent me an estimate of $1914.13 to replace the Nvidia Tegra MCU with an Intel Tegra MCU (exhibit 4). Since the car drives fine and the battery is good, I want to continue using my car but this is a safety issue. It seems that when Tesla pushes out updates on older cars, this crashes the MCU. It seems that they are trying to force owners of older cars to buy new ones. What are my options of solving this safety issue?

the uncured conductive adhessive between the display and the glass. The supplier did not use a proper UV light technique (they used expired bulbs" to cure the adhesive into a solid form and over time, the liquid is now coming out of the bottom of the screen and making a mess.

My Tesla 2013 model S has the exact same problems as ones in your recall notice NHTSA Campaign # 21V035 yet My VIN # does not show up in your list of recalled cars. I took my car into Tesla dealer on Dec 14 2022 and they fixed it and charged me $103.35 for the repair. I asked how long a warranty I would have and they replied "to the end of parking lot". I have had car in to Tesla twice since then as problem has gotten worse and each time they fiddled with it and it came back working for a short time each time I went in. On Feb 14, 2023 I went in again as I could drive the car but screen dark. Made an appointment to have it fixed and was told cost will be $2385.00 including a new radio as old one won't work with new screen. This is robbery as problem is the same as with the recalled vehicles. This is a safety issue and I thought manufactures were responsible for safety defects. In addition Tesla refuses to give me back the old parts unless I pay them $750.00. I asked what they did with old parts and they would not tell me. Again this is wrong.

I am complaining about Tesla’s dishonest and underhanded repair center and battery. In October 2021, I was told I was receiving a new battery. When I picked up my car, I noticed it was not getting the maximum charge. Through investigation, we found a refurbished battery was given. Less than 3 months later, the refurbished battery failed. Michael Hilgado (Tesla manager) explained it was a faulty battery and that I would receive a replacement at no charge and with a new warranty because the one they placed in October was faulty. The battery was replaced. Please find below the timeline of all additional repairs and breakdowns. The Tesla App locks you out and changes prices, times, etc. without owners having access to information for verification. When paying for services, there is no review or education regarding the vehicle, warranty, etc. December 2022, ten months after the refurbished battery was replaced, the battery failed again and Tesla wants to charge another $15,000 for repairs. 1. 2nd row seat behind driver malfunction for years. (Mean can not insert) 2. On Dec. 5th, 2022, Drive off the Tesla model S off highway to local road and car just die. Tow to Tesla service station in Bellevue, WA. I was told drive unit was water damaged a day later. This drive unit was put in on June, 2018 to replace original drive unit, which broken in June, 2018 at 76k miles. 4.5 years later, and under 45k miles driving splashed water from road cause damage of drive unit. Unbelievable. If car died on the 520 bridge before I got off it really can be a serious car crash. This is really a safety issues.

Car completely turned off. I took it to service and they said that my battery was dying and was out of warranty $13k to fix. Second time turned off in the freeway while my son was driving it. Now the same service will be $22k. I learned that I’m not an isolated case and Tesla is not explaining what is really wrong with it. Telsa MCU screen has large bubble. This is apparently a common problem. Tesla informed me there is no recall or warranty and I would have to purchase a new screen or an upgraded MCU system. The screen controls various functions of the car, including safety controls like dehumidifier.

The Model S door handles extend/retract. The handle mechanism involves a gear that extends the handle out and a spring of sorts that retracts the door handle. The door handle can be programmed to automatically extend when in proximity to the key fob or be programmed to extend only when the car is physically unlocked with the key fob. Over the course of ownership (9 years) I have needed at least 7-8 door handles replaced because of a failure of the door handle to extend. When the door handle does not extend, the door cannot be opened from the outside. It can be difficult to pry open the door handle if the door handle cannot extend. If the driver door handle does not function, and if the driver becomes unconscious or incapacitated, there is no easy way for an individual outside the car to open the driver door without breaking the window or climbing into the car via another door that can be opened with a functioning door handle. This represents a significant safety concern and could render the door unusable and impede the efforts of rescue personnel from intervening on behalf of the driver. These door handles seemingly have a shelf life before failure, and I have been told by Tesla that they consider door handles to be "consumables," which suggests that they, like belts, brake pads, tires, etc. simply need to be replaced after a certain time. Over 30 years of owning and driving automobiles, I have never had to replace defective door handles at all, and certainly not once every year or two. Not to mention the safety issue, but Tesla also charges for parts and labors every time a door handle needs to be replaced.

The car suddenly has a powertrain issue, but it is a widely known problem with some estimates of 66% of all early Teslas having a powertrain issue by 60k miles. Mine happened at 56k. There is clearly a defect with early Tesla powertrains, as they increased the warranty to 8 years or 50k miles. My car is now at 9 years, and because of the low mileage due to covid, they will not cover the powertrain issue of the car. Tesla, Please assist me in the following: 1. I have LIFE Time charging, explain the $305.00 fee for a brand new battery Tesla charged/bill me and installed. 2. Tesla has a suggested billing of $100.00 to put air into a (1) tire. Or $100.00 to bring all tires to federal standards. Basically, no tire repair just air into tires. Please explain the charge. 3. Tesla called me at 9:21 A.M. inform me that my 27 March appointment was canceled (I had no say so). Then, I was instructed to have Tesla Roadside Assistance to tow my vehicle to your kearny mesa service station for $386.00 (Aaron - 1-877-798-3752). 4. I was instructed that Tesla Roadside assistance memberships was not an option to me. 5. I have an appointment but was told parts for my vehicle might be available or might not be available for a 2021 Federal Recall, but today date is 22 March 2022. I've waited 1 year not months for Tesla to contact me when my vehicle parts were available in regards to the recall. It feels like a scam: $630.00 Tesla $386.00 Tesla Roadside Assistance. 6. Also, an Uber credit would be grant. Please explain why my vehicle that has an appointment for air in four tires, supercharged, recall and software update, is not a priority when I have an appointment. 7. In 2021, I was made to believe and instructed to purchase a 12 volt battery to repair Recall. 8. In 2022, Tesla again instructed me that my brand new battery to be replaced

The Tesla Center Console Display screen which controls the entire car functions has bubbles and LCD leaks developing on the screen making screen extremely reflective and dangerous to use. The lamination glue leaks often and makes hands extremely sticky to touch steering wheel or any other parts. The Glue is industrial quality and doesnt come off easily - This screen should be recalled/replaced by Tesla free of charge as its a Safety Hazard while driving and operations of cars critical functions like Lights, Door, Locks , Windows etc

The rear view camera did not deploy when I was backing out of a driveway and I was struck by another vehicle. The police determined it was a no-fault accident. My insurance company deemed me primarily responsible. I was unable to see any oncoming traffic because the camera was not working and there were obstacles to seeing oncoming traffic. The car was deemed totaled by the mechanic and my insurance company. The car battery died soon after the accident and the vehicle remains in my garage and is immovable. I sustained minor head, neck and back injuries.

March 2021: Experienced center touch screen display persistently shutting off. This is a safety hazard because the center screen is the only way control much of the car's safety components and functionality. Received safety recall notice, NHTSA Recall_No.: 21V-035. Attempted to make an appointments through mobile app and phone calls to service the recall multiple times but was told they do not have the part to repair the car and to continue driving the car. August 2021: Somehow managed to successfully schedule an appointment through mobile app. Took car to service center. Was told by service advisor that the purpose of the recall was to repair my issue with center display shutting off while car was in use. Car was repaired by replacing 8GB eMMC and released back to me on the same day. September 2021: Recall issue with center display shutting off was not resolved such that not only does the center display continue to shut off while car is in use, the instrument cluster screen directly in front of the steering wheel is now shutting off as well, another clear safety hazard. The cluster screen displays information such as speedometer, battery state of charge, gears (P,N,R,D). October 2021: Scheduled another service appointment through mobile app to repair issues of both screens shutting off. Reviewed repair estimates prior to appointment which showed zero cost to me. Took car to service center. Was told by service advisor that it they will charge me $200 to inspect the car, if issues are related to recall there will be waive the $200 fee and repair at no cost, however if issues are not related to recall, they will charge me $2000. I declined the offer and drove the car home while both screens are persistently shutting off. I believe the car is dangerous to drive in its current state and Tesla refuses to repair the car and resolve these safety hazards.

i was driving over 100mph, let off the accelerator and the car kept going that fast. Then all of warning lights came on, traction turned off, anti skid turned off. etc etc. I immediately applied brakes to NO affect. I called the Tesla Road Side Assistance. Lady was on the phone looking at my warnings. She could see the same warnings i was seeing. Applying brakes as hard as possible got the car to eventually slow down. It acted like the Cruise Control was stuck on 100mph. BUT, i never turned it on. EVER. This happened around first of May. Well two weeks later it happened again. This time Help Desk referred it to Jacksonville Service Center. The mechanic said he referred it to the "engineering" team at Tesla on 6/10/21. Have not heard back, I have been there several times, NO help at all. Refuse to discuss it.

I had a failure on my main computer. The MCU failed and the screen would not turn on. Tesla replaced a chip in my MCU but it did not fix all of my issues. I have to reboot my computer 95% of the time I am driving. Here are the issues: 1) 3g/GPS not activating. Leads to not being able to use GPS navigation. 2) Computer freezes. Usually while sleeping, computer does not wake up or the central screen does not respond leading to failures in many systems including: blinker, AC, radio, rear view for backing up, garage control, creep mode, etc. One time I left my computer in 'camp mode' so that it would keep cool on a hot day. While I was away from car, I checked on my mobile app to see the status of the car. The app could not connect to my car. When I came back, the computer had frozen up which meant that the AC was no longer working. This caused a unsafe level of temperature in my car of 100 degrees +. Luckily there was no dog or child in the car but I hate to think if there was. When I leave the car in 'dog mode' or 'camp mode', I expect it to do what it states it is going to do and keep my car cool. This could be very dangerous, especially on a hot day. In conclusion, even though Tesla did the fix for my MCU and my screen does turn on, it is not fixed 100% leading to constant reboots due to 3g failures and computer freezes. This happens almost every day.

Navigation and speedometer display went blank while driving , on 7/30/2021 at Palo Alto, CA. I pulled off the road, reset computer three times or so. Eventually, speedometer display came back, navigation was slow. I had to use Google map for a while. Service Center tested saying a chip update will be needed, and an upgrade, for $219.0. Set up service appointment for 8/10/2021at Tesla service center in Fresno, CA. I reside in Clovis , CA, zip 93619. Two days in service center, since 8/10-8/13/2021, I was told MCU board is in need of replacement, for a cost of $1500.0 extra. Tesla will not fix the condition on older model, 2013 S85!.?. OEM refusing to correct problem on older models. Driver confusion, and the car mechanism misbehaving, felt confusion while driving on three lane busy streets. Thank You, Bava

Hi, Tesla has a recall from the NHTSA regarding the MCU chip that powers the main center console screen. It can fail over time. My vehicle is under the recall, and they did the MCU chip recall but it did not help the screen. It is still failing. Without the screen working, the vehicle controls are not usable since 95% of the functions are through the screen. I contacted Tesla and they have ignored me and declined to fix the vehicle, even though the purpose of the recall was to fix the center screen.

The Tesla's main central LCD screen, in which many major car functions operate, leaks liquid (what Tesla refers to as glue or adhesive), which causes the level to decrease behind the screen and which, Tesla, says, will lead to the screen becoming in operable. Safety features like back up cameras, unlocking doors, window de-fog and de-frost, charging limits, and others, are operated through this screen. As you can see from the pics attached, the level has decreased and is leaking out near the passenger side front seat. Yes, NHTSA can come inspect if they'd like. I showed the dealer this problem, they said they were aware of it, that it happens to other Teslas, and will result in the screen becoming inoperable. There were no warning lamps or messages prior to or during this issue.

While driving , and all at a sudden i heard a boom knock coming from the back near the rear drive unit area, brought it to Tesla service center. They diagnosed it as a cracked subframe which let the drive unit attachment loose from one it's anchor point. The cost of repair is $3,447.95. Upon further review, i've seen numerous reports about the same incidents specially for Tesla Model S between 2013 and 2014. In numerous incidents, Tesla has replaced the subframe at will while out of warranty. Requested the same but it was denied. This happened all at a sudden and without any warning. Upon further review, I've noticed that Tesla have upgraded the subframe part number from REAR SUBFRAME ASSY(6007012-00-E) to REAR SUBFRAME ASSY(6007012-00-L). Reference is Tesla online part catalog search by model / year Possibly to address that issue.

The rear trunk will not open and pulling the emergency safety cord will not open the trunk. If you review the Tesla forums this is a known issue with the car. In my case this happened while the car was parked. However if I had the rear facing seats in the trunk and was involved in an accident I would not have been able to get my children out of the car due to the emergency cord not working when the trunk latch fails.

MY VEHICLE SHOULD BE COVERED UNDER THE 8GB EMMC RECALL REIMBURSEMENT. THE UNIT WAS REPLACED ON JUNE 4TH 2019 OUT OF MY OWN POCKET BECAUSE OF MALFUNCTIONING. TESLA HAD SET UP A SITE FOR CUSTOMER IN THAT SITUATION TO GET THEIR REIMBURSEMENT. HOWEVER, BECAUSE OF THEIR INSUFFICIENT INTENTIONAL OR UNINTENTIONAL RECORD KEEPING, THEY DENIED MY CLAIM. I TRIED TO CONTACT TESLA AGAIN TO TRY TO RECTIFY THE PROBLEM, BUT THEIR CS JUST REPLY, "IF YOUR CLAIM WAS DENIED, THE SYSTEM HAS IDENTIFIED THAT THE REPAIR IN QUESTION WAS NOT RELATED TO THE 8GB EMMC ACCUMULATED WEAR RECALL." I THEN REPLIED BY ASKING WHAT WAS THE REASON THAT UNIT HAD TO BE REPLACED AND THEY JUST IGNORE MY FURTHER QUESTIONING.

I WAS NOTIFIED BY TESLA THAT MY CAR WAS INCLUDED IN THE FEB 2 2021 RECALL REGARDING TOUCHSCREEN FAILURES. I HAD PREVIOUSLY REACHED OUT TO TESLA REGARDING SUCH FAILURES, BUT THEY SAID I HAD TO GIVE THEM SPECIFIC DATES & TIMES OF FAILURES FOR THEM TO TAKE ACTION, WHICH I DID NOT HAVE. WHEN NOTIFIED OF THE FEB 2021 RECALL. I CONTACTED THEM WITH 3 DATES/TIMES OF TOUCHSCREEN FAILURES. DURING THESE FAILURES, THE CAR COULD DRIVE, BUT I COULD NOT USE BACK-UP CAMERAS, OR USE MOST OF THE CAR'S FEATURES INCLUDED DEFOGGERS, ETC. AFTER CONTACTING TESLA ON FEB 2, 2021, THEY SCHEDULED AN APPOINTMENT FOR ME TO COME IN TO ADDRESS THE RECALL. I BROUGHT MY CAR INTO TESLA THIS WEEK, AND THEY KEPT IT FOR 2 DAYS. WHEN I CAME TO PICK UP THE CAR, THEY SAID THEY DID NOT DO ANY REPAIRS RELATED TO THE RECALL (EVEN THOUGH I HAD SHOWN THEM DATES AND TIMES OF REPEATED TOUCHSCREEN FAILURES). THEY SAID THAT THAT THEIR INVESTIGATION INTO THE MEMORY ISSUE INDICATED THE PROBLEM WAS NOT SEVERE ENOUGH TO ADDRESS (DESPITE TELLING ME PREVIOUSLY THIS WAS ALL COVERED UNDER THE NHTSA RECALL AND THAT IT WOULD BE FIXED). THE INVOICE THEY GAVE ME SAYS THE FOLLOWING: "PERFORMED MCU DIAGNOSIS, AND RESULTS WERE NOT WITHIN CONCERNING BOUNDS. PROACTIVELY INSPECTED VEHICLES MEMORY AND FOUND SPACE STILL AVAILABLE, CLEARED OUT NAVIGATION FILES, MAP TILES, AND MEDIA CACHE TO FREE UP ADDITIONAL SPACE. DUE NOTE TO ROUTINELY CLEAR TRIPS AS IT ALSO TAKES UP MEMORY SPACE. NO REPAIRS RECOMMENDED AT THIS TIME." THEY ALSO TRIED TO CHARGE ME FOR DIAGNOSING THE PROBLEM REMOTELY BUT I TOLD THEM THAT WAS INAPPROPRIATE AS THIS WAS COVERED UNDER THE RECALL AND THAT TESLA'S EMAIL SPECIFICALLY NOTED THAT CARS UNDER 8 YEARS OLD (WHICH MY CAR IS) WOULD BE ADDRESSED AT NO COST.

I HAVE HAD MULTIPLE ISSUES WITH MY 2013 MODEL S INCLUDING BUT NOT LIMITED TO TOTAL FAILURE OF THE MCU, TOTAL FAILURE OF THE BATTERY AND PARTIAL FAILURE OF THE TIRE PRESSURE MONITORING SYSTEM. WHILE DRIVING FROM BLAINE, MN TO ST. PAUL, MN WITH MY BLINKER ON THE MAJORITY OF THE TIME ON THE FREEWAY WITH OUT MY KNOWLEDGE BECAUSE THE WAS NO BLINKER SOUND. THAT WAS NOT ONLY FRIGHTENING BUT ALSO DANGEROUS. I HAVE HAD A MYRIAD OF ISSUES WITH THIS VEHICLE AND I AM REALLY FEARFUL OF A TOTAL FAILURE WHILE I AM DRIVING. ALL OF THE ISSUES NOTED ABOVE WERE DURING MOTION. ONE OTHER PROBLEM I HAVE NOTICED IS MY CAR TAKES AN EXCESSIVE AMOUNT OF TIME TO TURN ON.

THE TOUCHSCREEN SOMETIMES BECOMES UNRESPONSIVE WHILE TRYING TO START THE VEHICLE. THE ERROR MESSAGE "TOUCHSCREEN UNRESPONSIVE: PRESS SCROLL WHEELS ON STEERING WHEEL UNTIL IT RESTARTS" WILL APPEAR ON THE DISPLAY BEHIND THE STEERING WHEEL. THIS SEEMS LIKE A SAFETY ISSUE SINCE THE VEHICLE WILL DRIVE BUT WITHOUT THE TOUCHSCREEN THE BACKUP CAMERA AND HVAC CONTROLS ARE UNAVAILABLE, AND THE TURN SIGNALS DO NOT APPEAR TO NOT WORK, OR AT LEAST NO SOUND IS HEARD WHILE THE TURN SIGNALS ARE ACTIVE. THE TOUCHSCREEN REQUIRES SEVERAL MINUTES TO RESTART. THIS PROBLEM MATCHED THE DESCRIPTION OF THE RECENT NOTICE FROM TESLA ABOUT THE EMMC (EMBEDDED MULTI-MEDIA-CARD MEMORY) FAILURES. TESLA SERVICE WAS CONTACTED ABOUT THIS IN DECEMBER 2020 WITH A REQUEST TO DIAGNOSE IF THE EMMC NEEDS REPLACED. AT THE REQUEST OF TESLA SERVICE, SEVERAL SPECIFIC DATES/TIMES THIS OCCURRED WERE PROVIDED (SEE ATTACHED IMAGES FOR EXAMPLES). TESLA SERVICE STATED THAT A MORE "RECENT" OCCURRENCE WAS NEEDED IN ORDER TO EVEN PERFORM DIAGNOSTICS, BUT DID NOT DEFINE "RECENT".

THIS IS A HUGE PUBLIC SAFETY CONCERN THAT IMPACTS ME AND THE PUBLIC AS WELL. I LIVE IN A MOUNTAINSIDE AREA AND WE DEPEND ON THE BACK UP CAMERA TO AVOID ACCIDENTS AS OUR GARAGE BACKS UP INTO A VERY NARROW LANE. AND I CAN NOT SEE THE CHILDREN AND ANIMALS THAT ARE IN BACK OF MY CAR WHEN I EXIT MY GARAGE. NOR CAN I USE ANY OF THE GPS FEATURES FOR GUIDANCE AS WELL. THE TOUCH SCREEN HAS COMPLETE FAILURE IN THE MCU UNIT. I CONTACTED TELSA RE THIS ISSUE AND INFORMED THEM THAT THIS IS A LIABILITY TO ME AND THE BRAND BASED ON THE HUGE SAFETY ISSUES THIS PRESENTS. I TOLD THEM I WAS GLAD THAT THE CALL WAS BEING RECORDED AS I DID WANT THIS ISSUE TO BE UNREPORTED. AND I INFORMED TESLA THAT THIS PROBLEM IS EXIGENT AS WE LIVE IN SOUTHEN CALIFORNIA AND THE TEMPERATE IS NOW OVER 120 DEGREES. THIS COULD CAUSE HEAT STROKE AND ALSO KILL OUR ANIMALS AS WELL. TESLA REPLIED THAT THIS MY MY PROBLEM AND NOT THEIRS. SOME OWNERS BELIEVE THAT IT IS A PROBLEM WITH THE EMBEDDED MULTI-MEDIA-CARD MEMORY (EMMC) IN THE MCU AND THAT IT IS BEING OVERWRITTEN TO THE POINT OF FAILURE. II KNOW NOW THAT IS A RECOGNIZED DEFECT KNOWN AS THE 'EMMC FAILURE' PROBLEM. I UNDERSTAND THAT TESLA NOW HAS INTRODUCED A NEW MCU IN 2018 THAT DOESN'T HAVE THE SAME PROBLEM, BUT OWNERS OF OLDER VEHICLES ARE STILL EXPERIENCING THE PROBLEM IN HAVING TO REPLACE THE UNIT OUT OF WARRANTY, DESPITE SEEING TESLA'S MISTAKE AS THE SOURCE OF THE PROBLEM INSTEAD OF A NORMAL ISSUE THAT OCCURS OVER TIME. I NEED RESOURCES AND REMEDIES TO CORRECT THIS MANUFACTURE INDUCED MALAISE.

THIS WAS AFTER THE CAR WAS LEFT IN THE GARAGE AND NOT DRIVEN FOR ABOUT A WEEK. I HAD TESLA REPLACE THE MCU ON MY CAR WHICH COST ~$1700+. WHEN THE TESLA MODEL S MCU GOES OUT, THE CAR CAN STILL BE DRIVEN BUT THIS COMPUTER IS CENTRAL TO OPERATING MOST FUNCTIONS OF THE CAR. MANY VEHICLE FUNCTIONS NO LONGER WORK WITH A FAILED MCU INCLUDING THE AC/HEATER; BACKUP CAMERA DISPLAY; NAVIGATION INSTRUCTIONS, GPS, AND MAPS; INSIDE MANUAL DOOR LOCKS AND SUNROOF CONTROL; GARAGE DOOR OPENER; RADIO AND ALL MUSIC PLAY FUNCTIONALITY; ENERGY CONSUMPTION DISPLAYS; PHONE; VEHICLE CHARGE SETTINGS AND CHARGE STATUS DISPLAY; SELECTION OF AUTOMATIC PERSONAL POSITIONING FOR STEERING WHEEL, SEAT, AND MIRROR; MANY LIGHTING CONTROLS; OTHER VEHICLE SETTINGS; ON-SCREEN GAMES; AND MORE. THERE IS A NEWER VERSION OF MCU THAT I PLEADED WITH TESLA TO OBTAIN AND INSTALL IN MY CAR BUT THEY SAID THEY COULDN'T GET IT. THE MCU FAILURES ARE APPARENTLY RELATED TO OVERUSED COMPUTER MEMORY CHIPS AND THE MCU REPLACEMENT WAS ACTUALLY A REMANUFACTURED COMPUTER AND WASN'T NEW. THE REPLACEMENT DID HAVE A NEW TOUCHSCREEN AND UPGRADED LET MODEM BUT LATER HAD NEW 'LOW DISK SPACE' ERROR MESSAGES AND REBOOTED REPEATEDLY WITHIN WEEKS OF THE MCU REPLACEMENT.

TOUCHSCREEN THAT CONTROLS DOZENS OF VEHICLE OPERATING FEATURES INCLUDING AC, HEAT, GPS, LIGHTS, ACCESS TO MOBILE APPLICATION, SUNROOF, ETC. WENT BLANK AND WOULD NOT REBOOT. CAN CAN BE DRIVEN, BUT NOT SAFELY. THIS HAPPENED IN JULY 2020. THIS IS THE SECOND TIME, FIRST WAS OCT 2020 AND WAS REPLACED BY TESLA FOR NO CHARGE. THIS COMPLAINT IS FOR JULY 2020 WHEN IT HAPPENED AGAIN. ONLY 32,000 MILES ON VEHICLE. REQUIRED TO PAY $87 DIAGNOSTIC FEE AND $1,600 FOR A REPLACEMENT. KNOWN ISSUE TO TESLA DUE TO A MEMORY CARD FAILURE AS A RESULT OF WRITING LOG FILES TO IT BEYOND THE KNOWN LIFE OF THE CARD. SEEMS THEY REPLACED THE UNIT IN 2017 WITH ONE THAT THEY KNEW WOULD FAIL. THEY CALL ISSUE "END OF LIFE" BUT IT SEEMS LIKE IT WAS COMPLETELY PREVENTABLE. TOOK VEHICLE IN ON JULY 21, 2020. PART NOT AVAILABLE UNTIL AUG 10. NO LOANER OFFERED, JUST AN UBER VOUCHER WHICH EXPIRED ON JULY 26. HAVE SEEN MANY OTHER COMPLAINTS SIMILAR TO THIS BEING REPORTED WITH TESLAS OF MY MAKE AND YEAR.

MY CURRENT CONCERN IS THAT MY MEDIA CONTROL UNIT (MCU) TOUCH SCREEN IN MY MODEL S MANUFACTURE IN 2013 FAILURES OF THE TOUCH SCREEN WITH BUBBLES ON THE LCD TOUCH SCREEN AND SOMETIME JUST LINES ON THE LCD DISPLAY WHEN IT IS HOT UNDER THE SUN. THE CAR TAKES A LONG TIME TO START UP AFTER DRIVER GET INTO THE DRIVER SEAT- SOMETIMES IT CAN TAKE UP TO 3 MINUTES TO START UP THE CAR AND, LINES ON LCD DISPLAY, STICKY GLUE LEAKING DOWN UNDER THE DASH AREA FROM THE PASSENGER SIDE. THIS MCU AND LCD TOUCH SCREEN IS THE ONLY CONTROL THAT CAN BE USE AS AN INFORMATION SYSTEM FOR THIS VEHICLE. THIS IS A SAFETY ISSUES WHILE DRIVING AND CAN BE VERY DISTRACTING AND CAN CAUSE AN ACCIDENT WHILE DRIVING. THIS IS A MANUFACTURES DEFECT AND IT SHOULD BE RECALL FOR REPLACEMENT OR REPAIR. LAST AUGUST I HAD TO GET THE SCREEN REPLACED FOR 1100.00 NOW THIS REPAIR WILL COST 1600.00 AND IT COMES WITH ANOTHER SCREEN. THEY WILL NOT JUST FIX THE EMMC, P/N THAT IS ORDERED FOR THE REPAIR INCLUDES A NEW SCREEN THAT I DON'T NEED. I DECLINED TO PAY FOR THE REPAIR AT THIS TIME TO WAIT UNTIL MCU2 IS AVAILABLE, THEN A FEW DAYS LATER MY CAR WILL NO LONGER EVEN CHARGE. WELL IT CHARGES EXTREMELY SLOW TO A MAXIMUM OF 46 MILES. THEY SAY THE BATTERY AND DRIVE UNIT IS FINE SO GUESS WHAT, NO WARRANTY FOR ANYTHING ELSE FOR ALL THIS.

THE CENTER MEDIA TOUCHSCREEN (MCU) IN MY VEHICLE STOPPED FUNCTIONING AND SEVERAL FUNCTIONS CEASED WORKING. INITIALLY I RECEIVED A "TOUCHSCREEN UNRESPONSIVE" MESSAGE WHILE DRIVING SO WHEN I STOPPED AT A STOPLIGHT I ATTEMPTED TO REBOOT THE MCU USING THE STEERING WHEEL CONTROLS. THE SCREEN STARTED THE REBOOT PROCESS BUT EVERYTHING THAT WAS ON INCLUDING HVAC AND RADIO TURNED OFF FOR THE REMAINDER OF MY DRIVE AND WERE INOPERABLE UNTIL AFTER THE SCREEN WAS REPLACED. THE TOUCHSCREEN NEVER TURNED BACK ON. MANY FUNCTIONS DID NOT WORK WHEN THE MCU DIED WITH THE ODDEST ONE OF ALL BEING THAT I HAD NO TURN SIGNAL SOUNDS WHICH I THINK ARE CREATED BY THE CARS AUDIO SYSTEM AND THE MCU. WITH THE LOSS OF THE MCU, I LOST THE USE OF MY BACKUP CAMERA AND MY HVAC WHICH WAS STUCK IN AN OFF STATE SO I HAD TO ROLL DOWN MY WINDOWS IN ORDER TO HAVE ANY AIRFLOW IN THE CABIN. THANKFULLY I DID NOT NEED THE FORWARD OR REAR DEFROSTER BETWEEN WHEN MY SCREEN STOPPED WORKING AND IT WAS REPLACED AS BOTH ARE CONTROLLED VIA THE MCU AND WERE UNAVAILABLE. CHARGING AT HOME WAS EXTREMELY LIMITED AND MAXED OUT AT VERY LOW AMPERAGE AND I WAS UNABLE TO CHANGE THE CHARGING AMPERAGE DUE TO THE INACCESSIBLE TOUCHSCREEN. I DID NOT TRY A DC FAST CHARGER. MOBILE APP CONTROL WAS UNAVAILABLE SO I WAS NOT ABLE TO USE IT TO ADJUST MY HVAC OR CHARGING SETTINGS. THE CONTROLS FOR ADJUSTING THE ACTIVE AIR SUSPENSION WERE ALSO NOT AVAILABLE HOWEVER THE CAR APPEARED TO SET ITSELF TO A NORMAL RIDE HEIGHT. NO STEERING WHEEL CONTROLS (HVAC, RADIO, SUNROOF, VOICE COMMANDS) WORKED AND MY HEADLIGHTS WERE STUCK IN AUTO MODE THAT WAS THE SECOND MCU IN MY CAR, PREVIOUSLY BEING REPLACED ABOUT 14 MONTHS EARLIER DUE TO THE SCREEN ADHESIVE FAILING WHICH CAUSED AIR BUBBLES TO FORM WITHIN THE TOUCHSCREEN. THE CENTER TOUCHSCREEN ON MY CAR FAILED, AND WAS TOLD BY TESLA TECHNICIAN IT WAS DUE TO THE MEMORY BEING FULL. FROM FURTHER INVESTIGATION IT APPEARS THAT MEMORY BECAME FULL DUE TO SOFTWARE UPDATES PUSHED BY TESLA AND LOG FILES SAVED ON HARD DRIVE BY TESLA SOFTWARE. WHEN TOUCHSCREEN FAILED I LOST HVAC, STEREO, REAR VIEW CAMERA, NAVIGATION AND POTENTIALLY TURN SIGNALS (WHEN TURN SIGNAL STALK WAS HIT THE DASH DID NOT INDICATE TURN SIGNAL WAS ON, BUT WHEN HAZARD BUTTON WAS PRESSED THE DASH SHOWED THE TURN SIGNAL WAS ON). THE TOUCHSCREEN FAILED WHILE THE CAR WAS PARKED AND I WAS NOT IN IT. I GOT INTO CAR ONE MORNING AND THE TOUCHSCREEN NO LONGER WORKED. THIS SAME TOUCHSCREEN ALSO HAD ANOTHER ISSUE WHERE THE ADHESIVE ON THE SCREEN HAD MELTED AND LEAKED UNDER TOUCHSCREEN. THIS MELTED ADHESIVE CAUSED WHAT LOOKED LIKE A BUBBLE AROUND ~70% OF THE SCREEN. TESLA REPLACED THE TOUCHSCREEN WITH A REMANUFACTURED UNIT (THIS MUST OCCUR AT A HIGH ENOUGH RATE THAT REMAN UNITS ARE AVAILABLE) WHICH COST $1900 TO REPLACE. A NEW TOUCHSCREEN WOULD HAVE COST ME $2400. WHEN I PUSHED THE TESLA SERVICE FACILITY TO COVER THIS FAILURE UNDER WARRANTY THEY WOULD HEAR NOTHING OF IT, EVEN THOUGH BOTH FAILURES WERE KNOWN ISSUES. ALSO I CAN'T CHARGE TO 100% NOW. IT STOPS AROUND 97% AND SAY "CALCULATING REMAINING TIME" AND AFTER 30-40 MINUTES STOPS AND SAYS CHARGING COMPLETE. MY MAXIMUM RATED RANGE IS NOW 167 MILES DOWN FROM 208 MILES. THAT'S A 20%, 40 MILE LOSS OF RANGE. I HAD TESLA LOOK AT IT TWICE AND THEY SAY NOTHING IS WRONG. LAST APPT THEY SENT ME A TEXT ON FEB 10TH AND CANCELLED MY APPT SAYING THERE'S NO ISSUE. NOW IN THE WINTER I CAN'T MAKE IT HOME TO TOLEDO OH FROM COLUMBUS OH A 132 MILE TRIP. ON TUESDAY, DECEMBER 10, MY WIFE WAS SLOWLY APPROACHING OUR GARAGE DOOR WAITING FOR THE GARAGE DOOR TO OPEN WHEN THE CAR SUDDENLY LURCHED FORWARD SPEED SUCH THAT THE CAR WENT THROUGH THE GARAGE DOOR DESTROYING TWO GARAGE DOORS, THE LOAD BEARING COLUMN BETWEEN THE TWO DOORS. A LOLLY COLUMN INSIDE THE GARAGE WAS BROKEN IN TWO PIECES AND OUR OTHER CAR IN THE GARAGE WAS DAMAGED AS WERE OTHER ITEMS IN THE GARAGE. THE CAR EVENTUALLY STOPPED WHEN IT HIT THE CONCRETE WALL OF THE GARAGE BUT TWO MARKS ON THE GARAGE FLOOR INDICATE THE REAR DRIVE WHEELS CONTINUED TO SPIN EVEN AFTER HITTING THE CONCRETE WALL. 11 DAYS AGO, THE DOORS AND WINDOWS STOPPED WORKING ON MY TESLA MODEL S. IN ADDITION, AFTER CRAWLING IN THROUGH THE PASSENGER SIDE DOOR, I DISCOVERED THAT THE WINDOWS NO LONGER OPENED EITHER. TESLA PUSHED A SILENT SOFTWARE UPDATE TO THE CAR. THE UPDATE WAS UNSUCCESSFUL. THE MAIN COMPUTER FROZE LEADING TO THE MAIN DISPLAY SCREEN GOING BLACK. RESETTING THE COMPUTER VIA THE TWO SCROLL WHEEL RESET PROCEDURE WAS INEFFECTIVE. THE ONLY OTHER PERSON I KNOW WITH A 2013 MODEL S EXPERIENCED THE EXACT SAME ISSUE THE PREVIOUS DAY. SCENARIO - WHEN GETTING INTO THE CAR TO LEAVE IT WAS SLOW TO START, RECEIVED THE MESSAGE PLEASE WAIT WHILE SYSTEMS START UP. AFTER 20 SECONDS THE CAR WAS DRIVE-ABLE AND I PULLED OUT. THE STREAMING MUSIC SERVICE WAS NOT WORKING SO I SWITCHED TO A USB DEVICE PLUGGED INTO THE CAR. I ARRIVED AT MY DESTINATION AND PARKED. AFTER ABOUT AN HOUR I RETURNED TO THE CAR TO RETURN HOME. THERE WAS A SMALL DELAY IN THE SYSTEMS STARTING UP BUT I WAS ABLE TO DRIVE WITHIN 15 SECONDS. I TURNED ON THE AC AND BEGAN HEADING HOME. WHILE DRIVING THE MAIN SCREEN WENT BLANK - THIS MEANS THAT I HAVE NO ABILITY TO CONTROL MOST OF THE CARS FUNCTIONS. I STOPPED TO GET A COFFEE AND HOPED THAT LETTING IT "REST" WOULD ALLOW THE COMPUTER TO RESET AT THE NEXT STARTUP. WHEN I RETURNED TO THE CAR AFTER APPROXIMATELY 15 MINUTES I IMMEDIATELY NOTICED THAT THE CAR WAS COOL. THE AC NEVER TURNED OFF WHILE PARKED DUE TO THE COMPUTER BEING HUNG. I DROVE HOME WITH NO SYSTEM CONTROLS. I CONTACTED TESLA ROADSIDE SUPPORT AND SPENT HALF AN HOUR TRYING TO RESOLVE THE ISSUE. THEY TOLD ME THAT THERE WAS AN UPDATE PUSHED TO MY CAR THAT FAILED AND THAT I WOULD NEED TO GO TO THE SERVICE CENTER TO GET "A JUMP" THE BELIEVE BEING THAT THERE WAS PROBLEM WITH THE 12V BATTERY AND THE SERVICE CENTER WOULD BE ABLE TO QUICKLY RESOLVE THE ISSUE. AFTER WAITING AT THE SERVICE CENTER FOR 45 MINUTES I WAS GIVEN A LOANER WHILE THEY CONTINUED TO INVESTIGATE. 23 HOURS LATER I AM BEING TOLD THAT I NEED A NEW COMPUTER FOR $3,000. THEIR UPDATE WAS NOT BACKWARD COMPATIBLE AND BROKE MY COMPUTER

I WAS DRIVING MY TESLA MODEL S EARLY IN THE MORNING IN A QUIET NEIGHBORHOOD WHEN WAS SLOWLY APPROACHING TO FOUR-WAY STOP SIGN INTERSECTION BY GRADUALLY STEP ON THE BRAKE. AT THE VERY BEGINNING, I CAN FEEL THE CAR DECELERATED, SUDDENLY THERE WAS UNINTENDED ACCELERATION THAT I FELT. THERE WAS A JERKING FORWARD OF THE CAR, SO I WAS NOT ABLE TO MAKE A FULL STOP AT THE STOP SIGN. THEN THE SPEED DROPS DOWN SIGNIFICANTLY, ALMOST TO ZERO. I PRESSED THE FLASHING LIGHT BECAUSE THERE WAS A LINE OF CARS BEHIND ME AND I WAS IN THE MIDDLE OF THE ROAD. THE CAR WAS IN ALMOST ZERO SPEED, SO I TRIED TO MOVE THE CAR TO THE SIDE OF THE ROAD AND PRESSED THE ACCELERATION PEDAL A LITTLE. THE CAR DIDN'T ACCELERATE, INSTEAD, IT TURNED INTO A STOP SITUATION. I REALIZED I LOST DRIVING POWER OF THIS CAR. I PRESSED THE "P" BUTTON, THEN ALL FOUR MODE LETTERS "P", "N", "R", "D" TURNED RED, BLICKING. THE CAR STOPPED CLOSE TO THE CURB BUT PARTIALLY IN THE MIDDLE OF THE ROAD. I WAS ABLE TO GET OUT OF THE CAR AND CALLED TESLA ROADSIDE ASSISTANCE. CPSC: I OWN A TESLA MODEL S MADE IN NOVEMBER 2013. ALL 4 DOOR HANDLES ARE BROKEN OR MALFUNCTIONING. AFTER THE FIRST ONE WENT OUT, I TOOK IT TO TESLA AND THEY SAID IT WAS OUT OF WARRANTY AND WOULD COST $1,200.00 AND REQUIRE REPLACEMENT. AFTER TALKING TO SEVERAL OTHER TESLA OWNERS OWNERS AND DOING SOME RESEARCH, I HAVE DISCOVERED THAT IT'S MORE UNCOMMON THAT THEY DON'T BREAK THAN THAT THEY DO. THE ACTUAL COST TO REPAIR THEM WOULD BE MUCH LOWER, BUT THEY DON'T OFFER THAT. THE REAR PASSENGER DOOR ON MY CAR IN FACT SPONTANEOUSLY OPENS ITSELF. THIS USUALLY OCCURS WHEN PRESSING THE BUTTON THAT ENGAGES THE PARK BREAK. IT SEEMS CLEAR TO ME THAT THE PRODUCT IS DEFECTIVE. *BF UPDATED 07/19/2017*JS

THIS IS A COMPLAINT ABOUT THE ISSUE OF FIRES IN THE TESLA MODEL S. TESLA CLAIMS THAT THE FIRES IN 2013 WERE CAUSED BY IMPACT WITH ROAD DEBRIS AND THE DANGER HAS BEEN REDUCED BY FITTING A BATTERY SHIELD AND RAISING THE RIDE HEIGHT. THE RIDE HEIGHT WAS RETURNED TO LOW AFTER YOUR INVESTIGATIONS WERE COMPLETED. THE PHOTOGRAPHS FROM THE FIRES IN 2013 SHOW THAT THE FIRES WERE CAUSED BY COLLAPSED SUSPENSIONS AND NOT BY ANY IMPACT WITH ROAD DEBRIS. I BELIEVE THAT THE BATTERY SHIELD IS NO MORE THAN WINDOW DRESSING AND THAT THE LOW QUALITY VIDEOS SHOWING OBJECTS CRUSHED BY THE BATTERY SHIELD ARE FAKED. PANORAMIC ROOF HAS CRACK ON MY FIRST GENERATION 2013 TESLA MODEL S, WHICH TESLA CLAIMS TO BE DUE TO IMPACT WITH SOMETHING FROM OUTSIDE AND MY CAR IS SAFE IN GARAGE WITH NOTHING OVER IT WHEN THIS HAPPENED. I WONDERED HOW CAN THIS BE POSSIBLE ON A CAR WITH 5-STAR SAFETY RATING THAT ROOF CRACKS AND IT'S NOT COVERED PART OF THE VEHICLE WARRANTY PROGRAM.

THIS ACCIDENT REPORT IS A PREVIOUS ACCIDENT INVOLVING THE SAME 2013 TESLA MODEL S 60 AS REPORTED IN THE NHTSA REPORT FILED ON AUGUST 24, 2015. ON ON MAY 7, 2013 WHILE DRIVING MY 2013 MODEL S 60, I PULLED INTO A PARKING SPACE IN THOUSAND OAKS, CALIFORNIA MAKING A LEFT TURN INTO THE SPOT. AS I PULLED IN, I LET MY FOOT OFF OF THE ACCELERATOR,AND APPLIED THE BREAK. THE CAR LURCHED FORWARD, JUMPING A PARKING BLOCK, THEN OVER A CURB, AND FINALLY INTO A CEMENT LIGHT POST. THE AIRBAGS DEPLOYED AND I WAS STUNNED. I IMMEDIATELY CALLED TESLA WHO SENT A QUALIFIED TESLA TOW TRUCK. IT TOOK TWO HOURS TO GET THE CAR ONTO A FLAT-BED TRUCK BECAUSE THE CAR LOST ALL POWER AND SHUT DOWN. IT HAD TO BE DRAGGED OVER TWO PARKING BLOCKS, SUSTAINING EVEN MORE DAMAGE. TESLA SENT THE CAR DIRECTLY TO THE BODY SHOP. I INSISTED THAT THEY RUN A DIAGNOSTIC TEST BECAUSE I WAS (AND STILL AM) CERTAIN THAT THE CAR ACCELERATED ON ITS OWN. THE VAN NUYS SERVICE CENTER REP SAID THAT THE CAR HAD TO BE REPAIRED FIRST BEFORE ANY DIAGNOSTIC TEST COULD BE RUN. AFTER THREE WEEKS I RECEIVED A LETTER FROM TESLA. THE ENGINEERS REPORTED TO ME THAT THE ACCELERATOR WAS DEPRESSED TO 48%, SECONDS BEFORE THE ACCIDENT AND 98.4% AT THE TIME OF IMPACT. I STILL BELIEVE THAT THE CAR ACCELERATED BY ITSELF. IT IS MY SINCERE HOPE THAT BY REPORTING THIS ACCIDENT (AND THE SUBSEQUENT ACCIDENT ON JULY 16, 2015) FURTHER INVESTIGATIONS CAN BE CONDUCTED BEFORE PEOPLE ARE SERIOUSLY INJURED OR WORSE. MY INSURANCE COMPANY (FARMERS) DOES NOT INTEND TO INVESTIGATE. IT IS LESS EXPENSIVE FOR THEM TO PAY FOR A REPLACEMENT VALUE THAN TO DETERMINE THE TRUE CAUSE OF THIS INCIDENT. FORTUNATELY, YOU AND YOUR AGENCY ASPIRE TO A HIGHER PURPOSE.

TIMOTHY DEVINE WAS KILLED AFTER DRIVING HIS VEHICLE OFF THE ROAD AND DOWN THE CANYON. THE TESLA STARTED A BRUSHFIRE THAT WAS SO BAD THAT 180 FIREFIGHTERS WERE CALLED IN AND MALIBU CANYON ROAD WAS CLOSED FOR OVER 9 HOURS. TANG, A SPOKESMAN FROM THE HIGHWAY PATROL SAID THAT TIMOTHY DEVINE WAS SO BADLY BURNED HE WASN'T RECOGNIZABLE AS A PERSON AND THAT FIRE HAD MELTED THE CAR DOWN TO NOTHING. WAS THIS SUICIDE OR DRIVER ERROR? WAS THIS KNOWN PROBLEMS WITH STEERING, UNINTENDED ACCELERATION, BRAKE FAILURE, ETC. WAS THIS FATAL CRASH, FIRE AND EXPLOSIONS THE THE FAULT OF THE DRIVER AND OR THE FAULT OF THE VEHICLE?

THE REASON FOR WRITING: LETTER RECEIVED TODAY (LETTER DATED 6/27/14) FROM TESLA SAYS THEY HAD DEVELOPED A SOFT-WARE UPDATE TO PREVENT THIS (AS WELL AS REPLACING THE PART). MY INCIDENT HAPPENED AFTER MY SOFTWARE WAS UPDATED (I'M NOW 5.11, UPDATE WAS 5.8.4). CLAIMS OF SOFTWARE PROTECTION SHOULD BE VALIDATED. *JS
