The car veered suddenly to the left (out of the left lane of a highway) while supervised in auto steer. The driver could not move the steering wheel to avoid a collision with the guardrail as the steering wheel had locked up. The collision warning didn’t sound and emergency brakes did not activate upon impact. The airbags were deployed and the car was totaled.

The vehicle was involved in an accident due to a failure of the autopilot system. There was a recall letter sent out of 23V-838 a month prior, but changes either did not persist or was the primary cause of the accident.

EMMC failed prior to recall and was replaced by 3rd party vendor. Recall reimbursement was denied. Emailed Tesla multiple times without response
The MCU that controls the climate and Advanced Driver Assistance Systems stops working while driving. I sent a service request to TESLA and they replied that my older MCU1 is failing because of the new software updates. It isn't the eMMC card hardware failure, but the symptoms and safety concerns are the same. None of the controls work, as the main screen goes blank and resets while driving. TESLA told me the fix for this problem is to upgrade to MCU2, which they fail to provide for free as a safety recall. There was already a recall for the MCU memory chip failing and causing the same safety concern. The car disables the Advanced Driver Assistance Systems when this problem occurs. It happens about once or twice a month for the last year.

When backing out of my garage, and reverse my car often times stops violently on its own
My 2015 Tesla Model S was driving with autopilot on 3-17-2023 at 5:45 pm PST near the intersection of Lafayette St and Memorex Dr. in San Jose California when I noticed it panicked and slammed on the brakes while traffic was flowing at about 40 mph. The day was clear no rain, fog, or obstructions. There was a Jeep approaching faster from my rear left in the next lane but it wasn't in front of me. No other cars were in front of me either. There was traffic directly behind me and the car suddenly braked nearly causing an accident from the rear. The car showed an icon of a person in red (An indication of what it was braking for). However, there was no person in the area. I immediately hit the acceleration pedal to avoid the accident from the rear and to keep the car moving. The car never showed a "person" indicator before in the 4 years I have owned it. I know the newer models show it but not this year. I immediately requested service through Tesla as the car has phantom braked for ghost cars in the past but never a ghost person and it almost caused a serious accident. I was told by Tesla that there is nothing they can do and that I needed to press the "Push to talk" button in the car and say "Report a bug". I told them in a moment like that I am more focused on preventing an accident and that is not a good solution. I did provide the time, day and conditions to them.

When driving with autopilot engaged, the vehicle sometimes gives alerts even when there is no vehicle or obstacle in its path. Immediately once the driver takes control, the autopilot does not disengage but continue as usual. The autopilot suddenly brakes or slows down even when there is no obstacle on a freeway.

Instrument cluster not powering on or crashes/glitches while operating the vehicle. This makes the vehicle unsafe to drive while auto pilot is engaged because the auto pilot system stops operating which could lead to a serious accident. The vehicle speedometer and all other instrument cluster warning systems are unavailable because the screen is off/not working. Tesla has performed a remote diagnostic and claims no issues are identified. They then proceed to upsell me the new infotainment system that will rectify this problem for $3,000 dollars.
Two issues to flag: 1) Door handles are defective. I have now had to replace all 4 door handles at $500+ each. There is clearly a design issue that should have been addressed and every part failing in the same way is unacceptable. 2) Phantom braking when 'Autopilot' engaged... rarely the car will brake hard at highway speeds for no apparent reason. I have noted this seems to occur on specific parts of road such as driving under an overpass with an uphill slope... this has been dangerous and required quick reactions to prevent the car from stopping on the highway.
Sudden braking for no reason. On Autopilot on freeway. Sudden braking from 70 MPH to 15 MPH. Happens about 1 in 3 road trips. No car in front.
When using auto pilot on the freeway my vehicle sometimes comes to a halting stop for no reason putting myself and passengers in great danger.
On two or three occasions now my Model S will slow down for no apparent reason while autopilot is engaged. It's happened at speeds between 35 - 45 mph and highway speeds. Problem seems to have started after OTA updates. Car never had this problem before prior to the most recent OTA updates.
Wheel speed sensor kept getting disconnected. When it does, systems like ABS, traction control, and other anti-collisions systems are disabled immediately. This resulted in lost of vehicle control on wet pavement. The service center replaced the sensor twice but the problem continues to pop up randomly. They won’t look at the issue unless I pay for troubleshooting at $150/hr with no limit of how long or if they will find something.

Dan's Paint & Autobody and USAA Insurance Company adjuster My accident happened in the southbound passing lane at mile marker 178 on I-10, south of chandler on the Gila River Community in Maricopa/Pinal County It was dark outside with heavy traffic in both southbound lanes. My software version was 2021.40.6 I had the distance between cars at the closest setting , and had the adaptive speed control set at ~75. I was attentive with both hands on the wheel, eyes forward watching traffic with my wife whom was also watching the traffic. Without warning a Ford F250 stopped hard in front of me and my adaptive speed control did not react. The few beats I gave the adaptive speed control to respond first cost me precious braking time. I did hit my abs brakes and could feel the vibration as they attempted to slow down, but it was too late. I submarined under the bumper of the truck at a very high speed. There was no warning lamps, messages at the time of the accident. There was one early symptom that I recall. There was a Tesla software update a few days before the collision. On 11/27/22 before leaving for my trip, the dash screen to the far left did not display the map which it usually does when you set the GPS to map the path. I rebooted with the steering wheel toggles, and the display returned to normal. There were no other software malfunctions that I recall prior to the adaptive speed control failure.

No early alert before slamming on the brakes, only tapping a rear bumper on a 1999 Toyota Tundra. No damage to the Toyota but to my surprise, the insurance company said the Tesla's totaled. The airbags for the driver were deployed only, with no other passengers in the car. Quite a loud noise. I have an EDR report, not sure how to interpret it. Did not hear any warning lamps. Fortunately, both the other driver and I suffered no injuries. I would like to find out exactly what happened.

EERILY, BEC SOMETHING VERY LARGE WAS HIT WITH NO SOUND OF SQUEALING BRAKES OR CRIES, I WENT OUT FRONT TO LOOK LEFT WITHOUT SEEING ANYTHING. THEN I LOOKED RIGHT AND SAW A MAN CROUCHED TO THE GROUND BESIDE A CAR. I IOOKED FURTHER RT & SAW THE BACK OF A MAN THROWN FACE-FIRST INTO MY TREES. I DIDN'T IMME REGISTER WHAT I WAS LOOKING AT UNTIL I RECOGNIZED THE BODY AS MY HUSBAND, SCREAMED, & RAN TO HIM. HE WAS SNORING LOUDLY. I WAS AFRAID TO TOUCH HIM, SO I HOVERED OVER HIM CALLING HIS NAME. HE DID NOT RESPOND. I TURNED TO THE STREET, SCREAMING AT PASSERS-BY TO CALL 911. THE CROUCHING MAN SAID HE HAD, BUT I WAS FEARFUL OF HIM & THOUGHT, "HOW DO I KNOW YOU HAVE? WHY ARE YOU SO FAR AWAY? DIDN'T YOU EVEN COME OVER TO CHECK ON HIM?" I KEPT SCREAMING AS DRIVERS STOPPED THEIR CARS AND PEOPLE GATHERED. OUR 19-YR-OLD DAUGHTER CAME RUNNING OUT OF THE HOUSE, FOLLOWED BY OUR 17 AND 15-YR-OLD SONS. AN AMBULANCE CAME AND TOOK MY HUSBAND, REFUSING ME BECAUSE OF COVID. THEN I SPOKE WITH 1 OF 2 POLICE WHILE THE OTHER SPOKE TO THE DRIVER OF THE TESLA. HE SAID THE DRIVER ADMITTED TO BEING ON HIS PHONE AS THE CAR DROVE ITSELF, NEVER SEEING MY HUSBAND AND DRIVING 45 MPH. THE CAR THINKS THE TRUNK IS OPEN BUT IT IS NOT. AS A RESULT, THE CAR DOESN'T STOP BEEPING AS A WARNING TO A TRUNK THAT IS FULLY CLOSED. IN ADDITION, I CAN'T OPEN THE TRUNK AT ALL. WHEN DRIVING THE CAR I CAN NO LONGER USE AUTOPILOT AND OTHER SAFETY FEATURES OF THE CAR. I ALSO CAN'T LOCK THE CARE. ALL AND ALL, THE CAR IN UNUSABLE, BECAUSE OF AN ISSUE WITH THE TRUNK, THIS IS DEFINITELY A FAULTY DESIGN.

WAS A CONTINUATION OF THE FIRMWARE ISSUE WE HAD IDENTIFIED..."- "THIS WAS CAUSED BY A KNOWN FIRMWARE ISSUE",- "WE DO HAVE A COUPLE OF KNOWN FIRMWARE ISSUES THAT CAUSING OCCASIONAL RESETS OR SLOW PERFORMANCE",- "SINCE THERE ARE NO REPAIRS TO BE MADE AT THIS TIME .. I WILL GO AHEAD AND CLOSE YOUR SERVICE REQUEST" [AND EFFECTIVELY REFUSING TO REPAIR MY CAR UNDER THE WARRANTY ADJUSTMENT PROGRAM] THIS ISSUE IS NOT AN ACADEMIC OR FINANCIAL ONE. TESLA HAS FULL ACCESS TO LOGS. DIAGNOSTICS BY TESLA REPRESENTATIVE TELLS ME THAT MY MEMORY CHIP IS WITHIN PARAMETERS AND THUS NOT ELIGIBLE TO BE REPLACED. WHEN I TOOK IT IN I WAS TOLD MY CAR DID NOT HAVE BAD MEMORY MODULES. THEY SAID THAT IT WAS "NORMAL" FOR THE EMMC TO REBOOT FROM TIME TO TIME, AND AS A CURTSEY THEY WOULD NOT CHARGE ME A DIAGNOSTIC FEE FOR LOOKING AT IT. WE EXCHANGED SEVERAL EMAILS ABOUT HOW THIS COULD BE CONSIDERED "NORMAL" ESPECIALLY SINCE IT WAS HAPPENING MORE OFTEN, BUT THE OFFERED NO RESOLUTION TO THE ISSUE.
