My 2015 Tesla Model S has a serious safety issue where the vehicle suddenly reverts to Park while driving and will not stay in Drive. I had the car serviced at Tesla’s Austin Ridgepoint location, and they documented that the issue was “resolved” and “validated,” but the problem continues exactly the same. Tesla replaced the rear motor speed sensor, performed tests, and charged me $522 for the repair. However, the vehicle still cannot stay in Drive and remains unsafe to operate. After I reported that the repair did not fix the issue, Tesla deleted the entire service appointment and technician notes from my account, removing documentation of the failed repair. This is a critical safety defect involving unexpected shifting behavior and high-voltage component concerns. Tesla has not resolved the issue and has removed evidence of the repair attempt from my service history. I am requesting NHTSA assistance so Tesla properly addresses this safety hazard.

An investigation has been conducted into a persistent and widespread defect affecting the 2015 Tesla Model S and other early models equipped with the first-generation Media Control Unit (MCU1). This defect manifests as the physical degradation and spontaneous leakage of a sticky adhesive substance from the edges of the vehicle's central infotainment screen and instrument cluster. The leaking substance is not merely a cosmetic issue but presents a credible safety risk to vehicle occupants and the vehicle itself. The substance is a source of Volatile Organic Compounds (VOCs), a class of chemicals with a low boiling point that are known to off-gas into the ambient air, a problem exacerbated by high temperatures. Exposure to these compounds can lead to both acute and long-term health consequences, including respiratory irritation, headaches, and in some cases, serious chronic illnesses. Furthermore, the presence of flammable solvents and components within the adhesive, combined with the heat generated by the electronic components of the MCU1, creates a potential fire risk. This report is submitted to the NHTSA to provide a detailed, technical analysis of the adhesive degradation and leakage in the 2015 Tesla Model S. The purpose is to formally document this specific defect as a safety issue that warrants regulatory action, separate from but related to the manufacturer's previous recall for the MCU1's eMMC memory wear. It is essential to demonstrate that the adhesive failure is not a cosmetic inconvenience but a distinct material failure with serious implications for occupant health and vehicle safety. This report aims to provide the necessary evidence and analysis to support a formal investigation and a comprehensive recall that fully addresses this physical defect.

Hi, my trunk won't open when I press the button. This is the second occurrence that jas happend. When inside the trunk compartment the emergency cable to open the trunk is also not responsive and can not open the trunk. I think this is a big safety concern as the safety cord is there for a reason and in both cases id did not worked.

The car veered suddenly to the left (out of the left lane of a highway) while supervised in auto steer. The driver could not move the steering wheel to avoid a collision with the guardrail as the steering wheel had locked up. The collision warning didn’t sound and emergency brakes did not activate upon impact. The airbags were deployed and the car was totaled.

Car isn't cooling while supercharging. Problem originated while at Tesla Collision in Fort Lauderdale. Mike DiPietro, the general mgr stated they don't have a supercharger at Tesla Collision Fort Lauderdale to test it and they're not willing to drive it to the supercharger to test it because it's a service issue. When I dropped the car off it cooled while supercharging and now that it's not cooling while supercharging it presents a fire and safety hazard because it will only cool the moment the car is unplugged from the supercharger and then cools full blast for 45 minutes after the car is unplugged. I am well aware of the proper functioning of the cooling of 2015 Tesla Model S P90DL's because I have two of them and have since 2017. The fact that the cooling system kicks ONLY AFTER I UNPLUG from the supercharger clearly indicates the car sees a need to cool but because of a software issue. MIKE DIPIETRO, THE GENERAL MGR OF TESLA COLLISION FORT LAUDERDALE IS NOW THREATENING TO SUE ME BECAUSE I'M REPORTING A FIRE SAFETY/ACCELERATED BATTERY DEGRADATION ISSUE AND I SIMPLY WANT TO PICK THE CAR UP IN THE CONDITION WHICH i DELIVERED IT WHERE THE SUPERCHARGING WAS WORKING PROPERLY COOLING WHILE SUPERCHARGING. THREATENING LEGAL ACTION BECAUSE I'M REPORTING AN ISSUE THEY MAY NEED TO COORDINATE WITH TESLA SERVICE TO FIX AMOUNTS TO BULLYING. If I pick the car up it implies my consent the car is fixed and is NOT a fire safety hazard. They are unwilling to fix the supercharging fire the car is presenting because Tesla Service is a separate entity than Telsa Collision. This is NOT a valid excuse and reason not to fix this fire and safety issue. Please coordinate so that I may forward the chronological email chain which clearly shows the fire and safety issue can be clearly traced back to the [XXX] service. Issue is with the rear hatch (trunk) latching mechanism. The rear trunk latch has partially disengaged but will not completely release and open the trunk. This is a safety concern because the emergency release inside of the trunk (pull cable) does not fully release the trunk either. If a child or person were to be in the trunk they could not escape either from inside the trunk (emergency latch) or externally (unable to close or open physically or with electronic release).

This is an updated video related to an issue I previously reported. I’m becoming increasingly concerned by the idea that Tesla’s recommended remediation is to replace the car’s MCU. However, the MCU in my car was upgraded to a newer version in July 2021. I'm writing following several months of an "Unresponsive Screen" issue in my car. I initially thought this was a symptom of cold weather but learned it was related to "corrupt files" after a visit to the local Tesla service center. A full detailed description of the issue video of the issue can be found in the description portion of this link: [XXX] . I was told the issue was resolved following a three-day visit to the service center from [XXX]. Unfortunately, I've continued to encounter the problem. There are obviously many concerns regarding safety and the inability to utilize all of the car's functions (e.g. headlights, child safety features, indoor lighting, etc.). Perhaps the greatest concern is the fact that I have to do a soft reset in order to restore functionality and the soft function can be performed while the vehicle is operating. While driving on the freeway, I thought I heard a rattling from the sunroof, and so I cracked the sunroof to 12% open, and then closed it again. The moment the sunroof closed, smoke with an electrical burn smell began billowing in through the air vents in the front of the car. We immediately opened the car windows to allow fresh air and exited the highway. Smoke continued to pour into the cabin while I pulled the car to the side of the road in a residential neighborhood. The car was parked within ~2 minutes of the smoke initiating. During this time, there were no warnings or hazards that I noticed on the dashboard. My family grabbed their immediate belongings and got out of the car, while I turned on the hazard lights, and also then exited the vehicle, popping the trunk of the car and the frunk to allow smoke to dissipate. The smoke didn’t seem to decrease after pulling to the side, nor opening and venting the car. After being at the side of the road for probably 1-2 minutes, a visible fire sprung up at the intersection of the hood/front windshield at which point I called 911. The fire quickly spread to consume the hood and cabin of the vehicle. Multiple explosions from the vehicle were seen and heard, sounding like gunshots at times, drawing many concerned neighbors from their homes. By the time the first responders arrived, the front of the car and cabin were largely consumed down to the metal framework, and the trunk had fallen shut and was also on-fire.

1)MCU1 flash memory failure in production from 2012 to 2018, part is available, but would require downing the car to remove/replace. 2)The symptoms of this failure include the loss of indicated gear state and reduced driver visibility from the loss of rearview camera, defrost/defog and exterior signal lighting which increases the risk of an accident. Driver display is black or booting up slowly during this time and the car can be put into gear and driven prior to the display showing DNR gear state making it a safety risk to anyone walking near the vehicle if it was accidently put into gear. 3)Issue was confirmed by Tesla and was told an update to MCU2 hardware at $2250 was needed at the customer (my) expense. The vehicle was under a full Tesla warranty, so I refused and asked them to fix this issue. Tesla Service cleared/freed up some memory but said the issue would return unless I paid to update to MCU2 hardware. The issue has returned as they stated. 4)Yes, by Tesla Service and Tesla Engineering. Both confirmed that this was a known issue with the MCU1 hardware in production from 2012 to 2018 and recommended that I (the customer) update the vehicle to MCU2 hardware for $2250. 5)There are no warning lamps or messages to inform the operator of an issue. Only the black driver display, but that doesn’t prevent the operator from putting the vehicle in gear. The symptoms start with slow driver display boot times and progressively get worse over time losing the rearview camera, defrost/defog and exterior lighting controls. The display lag becomes enough to be able to accidently put the car in drive or reverse without displaying indicated gear state on the dash making it a safety risk to anyone walking near the car.

The driver side screen and the central touch screen developed bubbles with multiple air fluid lines and bubbles. With the sun shining in from the side or rear of the vehicle, it made the screens nearly impossible to read. This created very unsafe driving conditions, as the driver was not able to see critical information from speed, dash board warnings, rear camera view and vital air conditioning functions such as defrost/defog. This was noticed within 1-2 days of having the recall repair for the VCM to upgrade 8GB eMMC. The Tesla Repair Center said the two events were not related. Ultimately, I was left to replace both screen through and infotainment system upgrade costing $2250.00.

Front passenger door randomly opens on it's own. Usually vehicle is parked but has happened at low speeds (under 30 mph). Advised it's the door handle which has been "fixed" twice before while under warranty. Tesla will no longer fix without charging (warranty expired). If it's the door handle or the door this needs to be a recall issue for Tesla to fix. Please recall! Data below is for most recent occurrence. This has happened at least 3 times this calendar year (2023).

Tesla is seemingly not producing replacement key fobs for early models. I have been attempting to order one for over two years and to my knowledge new stock has never once become available over that time. I have also checked with multiple dealerships in person. On older models, the key fob is required to operate the vehicle. I only have one working key fob. I am anxiously awaiting the day when it is lost or malfunctions and I am unable to rely on our only vehicle... seemingly indefinitely. Tesla cannot consider their mobile app as an alternative to physical key fob. I live in rural Vermont. It would be incredibly wreckless to rely on remote start/unlock. Internet connectivity is required on both the car and the mobile device on older models (newer models use Bluetooth locally).

I drove through 3 inches of water at about 20mph, Nothing else in the car got wet, it cause the car to go into shutdown mode with a high voltage error. We had hell getting it to neutral as the computer kept shutting down. I am still under warranty but they claim the seal fail because of high water yet nothing else got wet and is not under warranty, only the drive unit and the motors are. Well with out the battery connection to the motors the car does not run. I have tried to deal with them and they refuse to do anything about it. I think the seal was installed incorrectly at the factory to allow this to happen, I do have parts to view.

My instrument cluster is delaminating in my tesla model S. When I took it to the service center for the recall service the service center stated that the main screen was also delaminating. Service tech told me that the instrument cluster (and main screen) would eventually fail because of the delamination. I have had the car for less than 8 years. both the instrument cluster and the center screen have a lot to do with safe operation of the vehicle so I believe they should be replaced for free under recall or at least have a much cheaper upgrade option. It is extremely rare for any car to have a common problem of the intstument cluster delaminating and this should be a recall since it is an ongoing, common issue. This is a problem Telsa knows about and hasn't done anything to remedy for a reasonable price. They want $1,750 for the upgrade of the instrument cluster and the main center screen. I bought the car when they were talking about self driving features that are not even possible for this car so I was mislead on that too. I had the eMMC replaced under recall in December of 2021. The board failed again in December of 2022 resulting in all controls of the car being inaccessible, the center screen was black. I had no control of lights or HVAC and all the traction control, ABS, and emergency avoidance features were not working. Tesla told me there was no fix except to upgrade to the MCU2 computer which cost me $1,872. This was related to the original recall that didn't really fix the problem. The MCU just went dead, no waring.

The screen behind the steering wheel on the TESLA forms bubbles on the screen which is a common problem with these vehicles. The screen behind has controls of the vehicle and there is a fluid that leaks behind and is causing this issue. Tesla doesn’t want to accept it and repair it but so many customers with models prior to 2016 have the same problem.

With my 2015 Tesla Model S. The touchscreen component that controls rear backup camera, sound volume levels, air conditioning, mirror fold capabilities, basically all the car component settings that can be configured freezes mid drive or on car startup and the touch capabilities stop being responsive. This can also be a danger because it prevents turning sound down on radio/music in the event say EMS or ambulances are near. I have read limited other reports of this online for their MCU1 component. NHTSA forced them to do a recall on the daughterboard emmc memory issue and this feels similar. Tesla says they checked all my hardware components via remote diagnostics and the hardware checks out. The only "fix" right now is hard rebooting my car mid drive which can be scary as it causes center screen to blacken out and a 4-5 minute reboot cycle mid drive required. I plan to capture a video of this seeming software bug as well for my report to Tesla and could provide to NHTSA as well.

Like everyone else who owns an early Model S, we have had numerous issues with our door handles. A couple were replaced under warranty, but now the car is out of warranty. The design is OBVIOUSLY defective. Our best friends have the same car and they have had to replace all four of them. Our car just started opening two of the doors randomly. Not just unlocking, its actually opening the doors all the way. This is seriously concerning to me. Tesla wants $700 to fix the doors. It would seem to me that Tesla needs to recall these handles or at least fix them for free as issues arise for people. Its NOT okay to have doors that randomly pop open with children in the back seat. So dumb! Please help!

Instrument cluster is coming apart at edges and progressing into the actual display area. This will cause speedometer and other information to be distorted or unreadable. It appears as though the glue used to assemble the display is deteriorating and causing the issue. The dealer has explained that it is a common issue with these vehicles. The suggested repair would cost approximately $1500. This seems unreasonable for an obvious manufacturing defect affecting many vehicles. I have included pictures of my instrument panel as well as others with the same issue.

The Navigation system stops and doesn't start. Checking on Google, I see that this has happened to many Tesla Model S owners who have an older model. This is because of a chip store card that fails after 4-6 years. On a long drive, this doesn't take us to the next super charger. This can cause the car to stop suddenly when it runs out of charge, and create risk for drivers and for other vehicles on the road too. The navigation is important on Teslas to know that one can safely complete her journey without running out of charge.

No early alert before slamming on the brakes, only tapping a rear bumper on a 1999 Toyota Tundra. No damage to the Toyota but to my surprise, the insurance company said the Tesla's totaled. The airbags for the driver were deployed only, with no other passengers in the car. Quite a loud noise. I have an EDR report, not sure how to interpret it. Did not hear any warning lamps. Fortunately, both the other driver and I suffered no injuries. I would like to find out exactly what happened.

A recall for my main media control unit was announced in March 2021. I have STILL not received the replacement unit from Tesla. My main screen goes black while driving. I lose ability to control HVAC, navigation, music, suspension, security etc. I also am unable sometimes to drive the vehicle from standing as the car will "reboot" unexpectedly.

Trunk states it's open, but it's closed. I can't reset it. When I crawled into the area to use the safety pull cord to open it, nothing happened! Well, the pull cord broke. What if I'd been kidnapped and put in there?? I cannot open the hatch and it looks like an expensive repair from what I've read online. Surely this is a safety issue and should be covered under a recall.

Sometime about a month ago, when driving along the Capitol Expressway going home, the huge Center Console screen suddenly went blank for a few seconds. With the screen off, most vehicle controls as not accessible. Since i was near a Supercharging Station, i parked there and performed reset while proceeding to charge. After a few minutes, screen went back on.

Hello, My driver's side door handle broke back in November/December of 2020. As a result, it prevented me from entering the car on the driver's side. I had it repaired in December 2020. Now, my passenger side door handle has broken as well. My concern here is that if there is an accident where the car is on fire, etc., and the passenger(s) inside my car are unconscious, that someone from outside the car would have a difficult time opening the door to pull them out. they wold need to break the glass and pull them out through the window. Although this is an option/workaround, I believe it puts all passengers in the car at risk of sever injury or death if this type of situation is experiences and time of essence it critical. I'm submitting this information b/c I believe that with the multiple door handle failures that this creates a significant risk to the safety of passengers in my Tesla. As a result, I think a more robust longer term solution is needed for the Tesla Model S. Thanks, Trevor

I received an “important safety recall” letter from Tesla, dated July 14, 2021. This is to replace the embedded MultiMediaCard (eMMC) in the media control unit. The instructions were to set up an appointment with Tesla to replace the VCM daughter board with on containing an enhanced eMMC. Yesterday, upon receiving the letter, I created an appointment for this Friday per Tesla’s instructions. I received confirmation of the appointment. Today I received a message from Tesla: “For this recall, unless you are currently experiencing issues with this touchscreen that we are able to verify, we will not be replacing it at this time. Please let us know if you have any other concerns, otherwise we will close this work order.” I wrote back asking for them not to cancel my appointment. I have had the main screen “blank out” with no controls. This also turns off my air conditioner, rear camera, and other controls. This screen has gone out three times over the past year. I’m concerned that they are not going to fulfill their obligation for me and others who have these cars and are experiencing the same problem. I have asked Tesla to remedy the situation and have not heard back.

While backing out of the driveway I heard a loud crack and grinding noise when turning the wheel to pull out into the street. Upon stopping a checking where the noise was coming I found that the left lower fore link completely sheared/ cracked off and the wheel was up against the back of the wheel well. ***The area where the wheel was backed up against (Completely tore the liner) has the cooling tubing (now leaking) & a high voltage wireline. This could've resulted in serious issues if we were traveling at higher speeds. Tesla has failed to reach out and call back regarding the issue and the vehicle is rendered completely useless/ undrivable. I've owned many vehicles and this is most certainly NOT NORMAL WARE AND TARE! This is a big safety issue and needs to be addressed ASAP. Thank god my family wasn't on the highway or any road above 5MPH when this happened. The damage would've been catastrophic.

Tesla Model S door handles are futuristic in design yet they have a major propensity to break on all versions earlier than the v3.0 door handles. My 2015 Tesla Model S have v2.0 and my driver side door handle now fails to extend out to let me into a car. I imagine this is a safety concern if I was in an accident and unconscious and a battery fire were to erupt someone outside my vehicle would have no way to open the door to pull me out. In May 2019 Tesla pushed an OTA update on the car (including mine) with caused the car to reduce charging speed a lot. I normally go at the Green Acres Mall supercharger, in Queens NY near JFK airport and my car used to charge at 74kW (about 40 min of time from 50 miles range to 200 miles range). After the update the time more than doubled (about 1 hour and 30 min of time to add the same miles range). The update was in response to a Model S bursting into flames in Hong Kong, although there have been multiple seemingly spontaneous fires over the past several years. Tesla allegedly knew the cars' batteries were defective and decided that it would rather push an update that reduced range, performance and charging speed than replace batteries under warranty. My problem with this is that I need to know if my car is safe to operate. If there were a battery chemistry which was not safe and Tesla rather than change it decided to reduce the charge rate, is this solution acceptable for NHTSA? Also this solution is impacting significantly my schedule. When I bought the car, I considered using it for certain road trips assuming the downtime was reasonable, but after this update it basically made my car usable only for local trips. If you have to charge the car a couple of times over a 500 miles trip, one thing is to add a couple of 30 mins pit stops which is fine considering that most people need to use the restrooms, get something to eat or drink, but another thing is now having to wait a total of 3 hours.

I HAD MY MCU REPLACED IN APRIL, 2020 BY TESLA IN PEABODY, MASSACHUESTTS AT MY OWN EXPENSE, TO ADDRESS FAILING EMMC MEMORY AS COVERED BY RECALL 21V-035. UNDER THE TERMS OF THE RECALL, TESLA HAS PROMISED TO REIMBURSE OWNERS WHO HAD WORK DONE TO ADDRESS THIS ISSUE AND PAID OUT-OF-POCKET. I WAS ONE OF THESE OWNERS. TESLA IS STEADFASTLY REFUSING TO REIMBURSE MY CLAIM, DESPITE BEING THE ONLY OPTION OFFERED TO ADDRESS THE ISSUE WHICH SHOWED SYMPTOMS EXCLUSIVELY ASSOCIATED WITH FAILURE OF THE EMMC MEMORY. I WOULD LIKE NHTSA'S HELP WITH GETTING THE REFUND THAT TESLA OWES ME. THE INVOICE FOR THE WORK IS ATTACHED.

I INSTALLED THE RECENT SOFTWARE UPDATE THIS MORNING. PART OF THE SOFTWARE UPDATE IS A "DRIVER VISUALIZATION IMPROVEMENT" WHICH CHANGES THE LAYOUT OF INDICATORS IN THE SCREEN OF THE DASHBOARD. WHILE PREVIOUSLY THE CRUISE CONTROL SPEED AND THE AUTOSTEERING INDICATORS WERE ON EITHER SIDE OF THE SPEEDOMETER AND SET AT THE OUTER EDGE OF THE DASHBOARD'S "CENTER DISPLAY ZONE", THEY ARE NOW BOTH CLUSTERED TO THE LEFT SIDE OF THE SPEEDOMETER. WHEN DRIVING, THIS PUTS THE CRUISE CONTROL SPEED INDICATOR TOO CLOSE TO THE SPEEDOMETER AND I CANNOT DIFFERENTIATE THEM IN MY PERIPHERAL VISION. I NOW HAVE TO TAKE MY EYES OFF THE ROAD FOR UPWARDS OF A FULL SECOND TO TO PARSE THE INFORMATION GIVEN BY THE TWO INDICATORS. THIS, AS I AM SURE YOU ARE AWARE, IS UNSAFE. WHILE DRIVING IN HEAVY TRAFFIC THE CAR'S EMMC WENT DEAD AND TOOK OVER 10 MINUTES TO REBOOT. ALL SAFETY DRIVING FEATURES WERE NOT FUNCTIONING DURING THIS TIME. THIS IS THE FORTH TIME THIS HAS HAPPENED IN THE PAST SIX MONTHS. TESLA SENT AN EMAIL THAT THEY HAD A MEMORY MODULE PROBLEM WITH THE CAR'S EMMC THAT CAUSED THE SAME PROBLEM I WAS EXPERIENCING, SO I MADE A SERVICE APPOINTMENT. WHEN I TOOK IT IN I WAS TOLD MY CAR DID NOT HAVE BAD MEMORY MODULES. THEY SAID THAT IT WAS "NORMAL" FOR THE EMMC TO REBOOT FROM TIME TO TIME, AND AS A CURTSEY THEY WOULD NOT CHARGE ME A DIAGNOSTIC FEE FOR LOOKING AT IT. WE EXCHANGED SEVERAL EMAILS ABOUT HOW THIS COULD BE CONSIDERED "NORMAL" ESPECIALLY SINCE IT WAS HAPPENING MORE OFTEN, BUT THE OFFERED NO RESOLUTION TO THE ISSUE. THERE REALLY APPEARS TO BE SOMETHING WRONG WITH THESE EMMC UNITS AND TESLA DOESN'T KNOW HOW OR DOESN'T WANT TO FIX IT. WHEN I TURNED ON MY CAR IN THE GARAGE THE TOUCH SCREEN WOULDN'T LIGHT UP AND THE CAR WAS DEAD. I CALLED ROADSIDE ASSISTANCE AND THE MAN HAD ME GO THROUGH A SERIES OF RESETS WHICH FINALLY GOT THE CAR GOING ENOUGH SO I COULD DRIVE IT VERY CAREFULLY TO THE TESLA SERVICE CENTER WHICH IS 15 MILES AWAY. THE CAR COULD BE OPERATED BUT THERE WAS NO SCREEN AND THE DIRECTIONAL SIGNALS WOULDN'T WORK. THEY KEPT THE CAR FIVE DAYS TRYING TO RESTORE THE TOUCHSCREEN AND WERE FINALLY SUCCESSFUL. THEY SAID THEY DID NOT REPLACE PARTS AND THE COST WAS $186.11 THE SCREEN HAS GONE DARK A FEW TIMES SINCE BUT I HAVE BEEN ABLE TO GET IT BACK WITH REBOOTING AND WAITING. THIS HAS BEEN ONGOING SINCE AT LEAST MARCH 2018 WITH TESLA CONSTANTLY SAYING IT'S A SOFTWARE/FIRMWARE ISSUE AND THAT I NEED TO UPDATE WITH THE UPDATE NEVER SOLVING THE PROBLEM. AS LATE AS FEBRUARY 2020 TESLA DID A TOTAL "FACTORY RESET. WHICH WILL ERASE THE ENTIRE MCU AND ALL IT'S CONTENTS." THIS DID NOT DO ANYTHING TO SOLVE ANY OF THE PROBLEMS BUT IT DID ALLOW THEM TO INSTALL UPDATES THAT SEVERELY LIMITED MY CHARGING SPEEDS CITING CONCERNS ABOUT THE BATTERY, BUT DID CONTINUED ON TO SAY THAT NOTHING IS WRONG WITH MY BATTERY. IF THERE'S NOTHING WRONG THEN WHY DID THEY NEED TO LIMIT THE CHARGING SPEEDS 50%? AS FAR AS THE MCU THIS PROBLEMS HAS HAPPENED IN ALL SITUATIONS WHETHER DRIVING ON THE HIGHWAY, CITY STREETS, AT A STOP LIGHT OR ANY WHERE. TODAY, 12/30 TESLA TOLD ME THAT MY EMMC HAS FAILED AND THAT IS WHY MY MCU WAS GLITCHING BUT SINCE I'M PAST 100K MILES THEY WILL NOT COVER IT UNDER THEIR EXTEND WARRANTY EXPLICITLY COVERING THIS FAILURE. THEY ALSO CLAIM THIS GLITCH TODAY AT 114K MILES IS NOT RELATED TO ANY OTHER GLITCHING I'VE MADE CLAIMS ABOUT IN THE PAST BEFORE I HIT 100K MILES SO THEY WON'T COVER UNDER THAT EITHER BUT THEY CITE PROPRIETARY INFORMATION AND CONFIDENTIALITY WHEN I ASK FOR THE REPORTS/LOGS OF THESE FAILURES AND THE DIAGNOSTICS. I TOOK THE CAR SEVERAL TIMES TO THE TESLA SERVICE SHOP AND THEY USED TO JUST TELL ME TO REBOOT THE COMPUTER AND THEY MANAGED TO RESET THE COMPUTER A COUPLE OF TIMES WHILE IN WARRANTY. FINALLY AT ONE POINT AFTER THE WARRANTY EXPIRED, THE COMPUTER COMPLETELY DIED EARLIER THIS YEAR AND TESLA SERVICE ADVISED THAT I NEED TO GET THE COMPUTER MCU REPLACED AT MY OWN EXPENSE OF ABOUT $1200. I DID THAT. NOW AFTER ABOUT 6 MONTHS OF THE NEW REPLACED MCU, THE PROBLEM HAS STARTED HAPPENING AGAIN, JUST LIKE IT HAPPENED ABOUT 1.5 YEARS AGO. COMPUTER SLOWING DOWN AND NOW TWO TIMES, FREEZING UP. I AM FRUSTRATED AT HOW EASILY THIS COMPUTER KEEPS GETTING INTO PROBLEMS AND CAUSES POTENTIALLY SAFETY ISSUES WHEN THE COMPUTER MAY GO DARK WHILE DRIVING OR TRYING TO BACK OUT OF THE GARAGE OR PARKING LOT WITHOUT VISIBILITY FROM THE REAR VIEW CAMERA WHEN IT HAPPENS. THE SOFTWARE THEY SENT APPEARED TO CRASH THE MCU THE FIRST TIME. IT WOULD WORK FOR A WEEK AT A TIME, THEN SEVERAL DAYS NOW IT'S PERMANENTLY OFF. NO HVAC CONTROL, TURN SIGNALS, AND CHARGING IS LIMITED TO 8AMPS ON MY 30 AMP LEVEL 2 CHARGER AND SUPERCHARGING IS DISABLED. CAR IS ABLE TO DRIVE, BUT NOT FAR DUE TO THE PAINFULLY SLOW CHARGE. APP WILL NOT SYNC WITH CAR AND I CAN'T SEE MY SCHEDULED SERVICE APPT. WE PURCHASED THE CAR USED (3RD PARTY) AND WE DID NOT HAVE ISSUES FOR ALMOST 2 MONTHS UNTIL WE CODED THE NEW KEY, AND DID A SOFTWARE UPDATE OR TWO. THIS SCREEN CONTROLS ALMOST EVERYTHING IN THE CAR, INCLUDING BACKUP CAMERA, PARK DISTANCE CONTROLS, AUTO-PILOT, DOOR LOCKS, SUSPENSION, ADJUSTING BRAKING AND COLLISION WARNINGS, OPENING AND CLOSING SUNROOF, CHARGING (IT'S AN ELECTRIC CAR - NONFUNCTIONAL IF WE CAN'T ADJUST CHARGING), CLIMATE CONTROL, REMOTE COMMUNICATION, SOFTWARE UPDATES, ENTERTAINMENT SYSTEM, ETC. WE CONTACTED TESLA, AND THE SERVICE MANAGER TOLD US THESE SCREENS REGULARLY FAIL AT AROUND 4 YEARS (OUR VEHICLE IS 4-1/2 YEARS OLD). IN ADDITION, FAILURE IS APPARENTLY ACCELERATED BY THE FAILURE OF THE 12V BATTERY. AFTER WAITING MANY WEEKS FOR A "REPLACEMENT PART," WE WERE CONTACTED AND TOLD THEY HAD ACCIDENTALLY ORDERED A COMPLETE REPLACEMENT, WHICH WOULD COST $2,700 (WE HAD BEEN QUOTED $1,300). THE CAR HAS BEEN UNDRIVEABLE FOR MONTHS. IN ADDITION, IF THIS IS A PART THAT REGULARLY FAILS, IT SEEMS LIKE TESLA SHOULD COVER THE REPLACEMENT. ON THE SAME DAY (6/26/2020) AND ABOUT AN HOUR AFTER INITIAL RMAIL, I RECEIVED ANOTHER EMAIL THAT MY APPOINTMENT HAD BEEN CANCELED "PER YOUR REQUEST", BUT I HAD NOT REQUESTED A CANCELLATION. I VISITED TESLA'S SERVICE CENTER IN SAN DIEGO - MIRAMAR BETWEEN 7/7-7/10 TO INQUIRE ABOUT SERVICE CANCELLATION AND REQUEST A SERVICE FOR MY CAR, BUT I WAS TOLD THAT THEY HAD CANCELLED MY APPOINTMENT DUE TO PARTS UNAVAILABILITY AND I WOULD BE CONTACTED LATER ONCE PARTS BECOME AVAILABLE TO SCHEDULE ANOTHER APPOINTMENT; HOWEVER, THE SERVICE CENTER REFUSED TO LOOK AT/EVALUATE MY CAR WHATSOEVER "DUE TO COVID" AND TOLD ME THAT BASED ON OTHER SITUATIONS, THEY'RE VERY SURE THE MCU NEEDS TO BE COMPLETELY REPLACED (~$1500 OUT OF POCKET COST) BUT THEY DO NOT HAVE A REPLACEMENT YET; I EXPLAINED MY BLINKERS AND MANY OTHER SAFETY FEATURES (REARVIEW CAMERA, CLIMATE CONTROL, "AUTO-PILOT", ETC.) ALSO NOT WORKING AND THEY MENTIONED THOSE SHOULD TYPICALLY CONTINUE TO WORK AFTER A MCU MALFUNCTION BUT THEY WILL NOT TAKE A LOOK AT THE CAR UNTIL THEY RECEIVE REPLACEMENT MCU. ON JULY 23, 2020, AT AROUND 1:00 PM, I WAS HIGHWAY DRIVING AT 65 MPH ON EAST BOUND I-70 NEAR BLUE SPRINGS, MO. WHEN MY TOUCHSCREEN FAILED AND WENT BLANK. I LOST ACCESS TO ALL MY TOUCH SCREEN FEATURES AND IT RENDERED MY TESLA APP ON MY MOBILE PHONE IN OPERABLE. I WAS ABLE TO DRIVE IT TO THE TESLA SERVICE CENTER IN KANSAS CITY, MO. ON SAME DAY. ON JULY 23, 2020, AROUND 2;05 PM, I WAS ADVISED BY TESLA THAT MY TOUCHSCREEN FAILED AND WILL HAVE TO BE REPLACED. MY TESLA WAS NOT DRIVABLE AS IT COULD NOT BE CHARGED AND THE AIR CONDITIONER COULD NOT BE TURNED OFF. I LEFT MY CAR WITH THE TESLA SERVICE CENTER MENTIONED ABOVE AND I AUTHORIZED REPLACEMENT OF THE TESLA TOUCH SCREEN. I WAS INFORMED BY TESLA THAT MY CAR WAS OUT OF WARRANTY AND THE COST OF THE TOUCH SCREEN REPLACEMENT WOULD BE AT MY EXPENSE. I AUTHORIZED THE REPAIR. THE MILEAGE ON MY CAR IS ABOUT 25,000 MILES. TESLA PRESENTLY HAS MY CAR IN THEIR SERVICE SHOP AS IT IS NOT SAFELY DRIVABLE. THE REPLACEMENT SCREEN HAS BEEN ORDERED AND SHOULD BE INSTALLED AND MY TESLA READY FOR PICK-UP ON OR ABOUT AUGUST 4, 2020.

A WEEK AGO THE TOUCHSCREEN FAILED TO COME ON UPON ENTERING THE CAR WHICH CAUSED A LACK OF MANY NAVIGATION AND COMFORT FEATURES. UPON CALLING FOR SERVICE WAS TOLD THE COMPUTER NEEDED TO BE REPLACED. THEY APPEARED TO KNOW EXACTLY WHAT WAS WRONG AND OFFERED NO EXPLANATION. MY CAR IS DRIVABLE BUT LACKING CERTAIN SAFETY FEATURES SUCH AS TURN SIGNALS. THERE WAS NO PRIOR WARNING OF PENDING FAILURE OF THE SYSTEM. I HAVE TALKED WITH FELLOW TESLA OWNERS AND THEY ALL HAVE HAD THE SAME FAILURE AT THE SAME TIME MINE OCCURRED. FEEL THIS IS A TESLA PROBLEM THAT SHOULDN'T BE INFLICTED ON MANY OWNERS WITH NO EXPLANATION OFFERED. THE PRICE TO REPLACE WITH "NEW AND IMPROVED" COMPUTER IS $2,500 OR I CAN REPLACE WITH THE SAME MODEL CURRENTLY IN THE VEHICLE FOR A MERE $995. MY A/C WAS ON ALL THE TIME AND I COULD NOT OPEN CHARGE PORT TO CHARGE. I WAS SCARED I WOULD NOT GET IT INTO SERVICE SO I DROVE IT IN THERE ON 7/18 AND EXPLAINED ALL TO THE SALES REP. HE TOOK CAR AND I GOT A RIDE HOME. TODAY, 7/20 SERVICE CALLED ME AND SAID MCU1 WAS DEAD AND I COULD EITHER PAY 1275 TO REPLACE OR UPGRADE TO MCU2 FOR $2500. I TOLD THEM THAT IT WAS DANGEROUS AS SIGNALS WERE NOT EVEN WORKING AND THIS SHOULD BE COVERED AS IT WAS DANGEROUS. THEY SAID NO AND I TOLD THEM JUST TO DO THE MCU1 REPLACEMENT. NOW I HAVE TO PAY FOR THAT AND THEY SAID IT WILL BE OVER A WEEK TO GET IT IN. I DON'T KNOW HOW THE WHOLE COMPUTER COULD FAIL THIS QUICKLY AND I AM DEBATING GETTING NEW MCU2 THOUGH THAT IS TOO MUCH MONEY TO PAY. UNABLE TO ACCESS OR USE ANY BUILT IN SYSTEMS. UNABLE TO TURN ON OR OFF SAFETY FEATURES/ AIRBAGS. UNABLE TO MANUALLY CONTROL SUSPENSION HEIGHT TO PREVENT DAMAGE TO UNDERSIDE OF CAR ON ROADS WITH POT HOLES. UNABLE TO ACTIVATE SECURITY FEATURES LIKE PIN NUMBER TO DRIVE. UNABLE TO SYNC OR UNSYNC CLIMATE ZONES IN THE CAR. ESSENTIALLY ANY FUNCTION NORMALLY ACCESSIBLE THROUGH THE TOUCH SCREEN IS UNUSABLE. THE PROBLEM HAS DEVELOPED OVER TIME WITH INITIAL PROBLEMS WITH TOUCH SCREEN THAT WOULD BEGIN TO WORK AFTER A SHORT WHILE TURNING INTO A POINT NOW WHERE THE SCREEN NEVER WORKS.

WITH TESLAS, THE MEDIA CONTROL UNIT (MCU) IS THE POWERHOUSE OF THE VEHICLE, CONTAINING ALL OF THE SAFETY, A/C, AND DRIVING CONTROLS. WITH THIS BEING OUT OF COMMISION, THERE IS NO TELLING WHAT FEATURES ARE USABLE OR NOT. THIS LEADS ME TO WORRY ABOUT WHETHER THE AIRBAGS WILL DEPLOY, WHETHER THE CAR WILL STOP RANDOMLY ON THE FREEWAY, OR THE CAR NOT BEING REFUELED. IN FACT, SINCE THIS MCU HAS GONE OUT, THE CAR HAS NOT BEEN ABLE TO BE REFUELLED DUE TO THE MCU NOT ALLOWING CHAGE TO REENTER THE CAR. THIS HAS LEAD THE CAR TO BE UNUSABLE AND UNRESPONSIVE. THIS FIRST OCCURRED FOR ME WHILE DIVING ON A CITY STREET, WITH NOTHING UNUSUAL BEING NOTED AT THE TIME. THE CAR WAS RESPONSIVE, DRIVING NORMALLY, AND COMPLETELY USABLE AND OUT OF NO WHERE THE VEHICLES MCU WENT BLACK AND THE DRIVING FEATURES WERE HANDICAPED. THE CAR WAS STILL ABLE TO DRIVE, HOWEVER, A/C COMPONENTS AND UNKNOWN ELECTRICAL FEATURES (SUCH AS AIRBAGS, SEAT BELTS, ABS) MAY HAVE BEEN COMPROMISED.

THE MEDIA CONTROL UNIT IN THE CAR IS EQUIPPED WITH A LOGGING CHIP THAT WILL CAUSE THE UNIT TO FAIL AFTER A SPECIFIC AMOUNT OF REWRITES. THIS IS TYPICAL FOR CARS AT OR NEAR 50,000 MILES. EVEN AFTER BEING REPLACED THE UNIT IS NOT WITHOUT FLAWS. THE SAME ISSUE IS LIKELY TO HAPPEN AGAIN IN THE SAME AMOUNT OF TIME AFTERWARDS. WITH THE MCU NOT OPERATIONAL THE CAR TAKES AN EXTENDED AMOUNT OF TIME TO BE ABLE TO DRIVE. ADDITIONALLY YOU LOSE ACCESS TO CLIMATE CONTROL, MUSIC OR MEDIA OF ANY KIND, NO ACCESS TO NAVIGATION, AND MANY OTHER SAFETY AND DRIVING OPTIONS THAT THE CAR IS SUPPOSED TO BE EQUIPPED WITH. THE UNIT IS ALSO SUSCEPTIBLE TO HEAT. MANY OF THESE UNITS HAVE BUBBLES ON THE SCREEN DUE TO USING SUBPAR GLUE THAT ADHERES THE LCD SCREEN TO ITS INTERNAL COMPONENTS. WHEN THE GLUE FAILS THE LIQUID INSIDE DRAINS OUT AND INTO THE INTERNAL AREA OF THE MCU. THE SAME IS TO BE SAID FOR THE INSTRUMENT CLUSTER SCREEN. BOTH SCREENS UTILIZE THE SAME POOR PERFORMING ADHESIVE. THE COMPANY SHOULD RECALL THESE UNITS AND REPLACE THEM WITH THE APPROPRIATE HARDWARE TO REPLACE THEM. I'M NOT SURE IF THE LIQUID IN THE SCREENS IS TOXIC OR NOT, BUT ON MY LAST VISIT TO THE SERVICE CENTER THEY TRIED TO CONVINCE ME TO CHANGE THE SCREEN OUT BECAUSE "YOU DON'T WANT TO BE BREATHING ANY OF THOSE FUMES". THIS RAISED AWARENESS THAT I COULD POTENTIALLY BE BREATHING IN TOXIC CHEMICALS WHILE SIMPLY DRIVING MY CAR.

THE SYSTEM NEVER SHUT OFF THE HEADLIGHTS OR PREVENTING DRIVABILITY WHEN RESETTING IN THE PAST. MENU FUNCTIONS HAVE STARTED TO SHOW UP CRYPTIC AND UNUSEABLE FORCING MORE COMMON RESTARTS WHILE DRIVING. MULTIPLE TRIPS TO TESLA SERVICE CENTERS WILL NOT RESOLVE THE ISSUE. THE STAFF IGNORE IT AND SAY ITS NORMAL. THEY KEEP SAYING THE CACHE IS FULL AND ONLY THEY CAN FULLY CLEAR THE CACHE. IN ALL CASES THE VECHICLE WAS IN MOTION AND EACH COULD HAVE RESULTED IN AN ACCIDENT.

ON APRIL 16, 2019, I ATTEMPTED TO DRIVE MY VEHICLE AND PULL OUT IN REVERSE FROM MY GARAGE, HOWEVER, THE TOUCH SCREEN DISPLAY UNIT (MCU) WOULD NOT TURN ON RESULTING IN AN INABILITY TO VIEW THE REAR VIEW CAMERA. IT IS VERY RISKY TO DRIVE THIS VEHICLE WITHOUT THE MCU. IT IS ALSO WHAT DISPLAYS IF THERE ARE FAILURES IN OTHER PARTS OF THE VEHICLE. TESLA SERVICE TOWED IT TO THEIR FACILITY AND THEY ULTIMATELY CONCLUDED THAT THE UNIT WOULD STILL POWER ON BUT IT ULTIMATELY WAS STUCK IN A BOOT LOOP. EVEN THOUGH I HAD THE VEHICLE FOR LESS THAN 4 YEARS, IT WAS OUT OF WARRANTY BECAUSE OF MILEAGE OVERAGE, RATHER THAN TIME. TESLA REFUSED TO COVER IT UNDER WARRANTY AND FORCED ME TO PAY FOR IT OUT OF POCKET FOR OVER $2500. THIS FAILURE IS WIDELY KNOWN IN THE TESLA COMMUNITY AND BY TESLA ITSELF AS SERVICE EMPLOYEES ACKNOWLEDGED THE DEFECT. IT IS A RESULT OF THE EMMC NAND FLASH MEMORY FAILURE THAT DEGRADED QUICKER THAN TESLA PLANNED AFTER THE INTRODUCTION OF AUTO-PILOT. THIS HAS MADE DRIVING THE CAR NEARLY IMPOSSIBLE. BECAUSE OF THE LOW VISIBILITY OF THE REARVIEW WINDOW, I HEAVILY RELY UPON THE REARVIEW CAMERA. SINCE CENTER CONSOLE DISPLAY WAS OFF AND HENCE NO REARVIEW CAMERA, I ENDED UP DAMAGING MY BUMPER INTO A LOW-PROFILE OBJECT THAT OTHERWISE WOULD HAVE BEEN VISIBLE HAD THE CAMERA BEEN ENABLED. I ONLY DROVE MY VEHICLE ONCE AND DAMAGED IT ON THE FIRST DRIVE WITH THE CENTER CONSOLE OFF. MCU FAILURE WITH LESS THAN 3,400 MILES ON CAR. TESLA CLAIMED IT WAS OUT OF WARRANTY. THE CAR CAN NOT CHARGE OR DO ANYTHING WITHOUT MCU. NO WINDOWS, NO CLIMATE, NO CHARGING, ETC. I HAVE TEXT MESSAGES WITH TESLA GOING BACK TO JUNE, 2019 AND AGAIN IN OCTOBER, 2019 - THE DATE OF MY MOST RECENT SERVICE. IN EARLY JULY 2019 I NOTICED THAT MY 2015 TESLA MODEL S 85D WAS NOT CHARGING AS IT SHOULD BE. MY 85 KILOWATT BATTERY WAS CHARGING AT A GREATLY DIMINISHED RATE. I TRIED CONTACTING TESLA FOR SERVICE BUT WAS UNABLE TO GET THROUGH TO THEM. IT TOOK OVER 3 VERY FRUSTRATING WEEKS FOR ME TO FINALLY GET THROUGH TO SCHEDULE A SERVICE APPOINTMENT. MY SERVICE TECHNICIAN REPLACED SOME PARTS THAT HE BELIEVED WOULD REMEDY THE PROBLEM HOWEVER WHEN HE TOOK THE CAR TO THE TESLA SUPERCHARGE STATION THE PROBLEM STILL EXISTED. HE REACHED OUT TO THE ENGINEERING DEPARTMENT AT TESLA AND WAS INFORMED THAT MY CARS CHARGING CAPACITY HAD BEEN LIMITED AND RESTRICTED BY TESLA. I FILED A COMPLAINT WITH TESLA AND REQUESTED THAT MY BATTERY BE REPLACED UNDER THE FACTORY WARRANTY. IT TOOK 3 MORE WEEKS FOR ME TO GET A RESPONSE FROM TESLA WHICH RESULTED IN A DENIAL OF MY WARRANTY CLAIM. THEY TOLD ME THAT THERE WAS NOTHING WRONG WITH MY BATTERY THAT WOULD WARRANT REPLACEMENT. I THEN INSISTED THAT IF THERE WAS NOTHING WRONG WITH MY BATTERY THEN THEY SHOULD UN-RESTRICT MY ABILITY TO CHARGE AND RESTORE ME TO FULL CAPACITY. THEY SAID THAT THEY COULD NOT DO THAT AND IF I DID NOT LIKE THAT ANSWER THEN I COULD SUE THEM. I WAS ABSOLUTELY MORTIFIED. I HAVE SINCE LEARNED THAT TESLA HAS DISCOVERED THAT THERE IS A SERIOUS PROBLEM THAT EXISTS IN THE 85 KILOWATT BATTERIES AS IS THE ONE IN MY VEHICLE. YET IN THE PAST THEY HAVE REFUSED TO WORK ON SALVAGED CARS UNTIL THEY HAVE BEEN RECERTIFED BY TESLA. THIS COULD POTENTIALLY LEAD TO HUNDREDS OF THOUSANDS OF CAR ON THE ROAD THAT NEED ACCESS TO PARTS, SERVICE AND REPAIRS. THAT DUE TO TESLA'S POLICIES COULD POTENTIALLY BECOME A LIABILITY ON THE ROAD. THE SECOND AREA OF CONCERN IS THAT TESLA ALSO CAN SEND UPDATES THAT ACTIVELY DEACTIVATE FEATURES OF THE CAR. SUCH AS AUTOPILOT AND ACCESS TO THE SUPERCHARGING NETWORK. EVEN IF SAID FEATURES WHERE PURCHASED AND ARE THUS LINKED TO THE CAR FOR IT'S FORESEEABLE LIFETIME. THIS IS A TESTAMENT TO THE TECHNOLOGY AND CAPABILITIES OF OVER THE AIR UPDATES. HOWEVER, BY DISABLING SOFTWARE UPDATES. IT COULD LEAD TO SALVAGED CARS HAVING GROSSLY OUTDATED SOFTWARE. POTENTIALLY LEADING TO HUNDREDS OF THOUSANDS OF INDIVIDUALS OPERATING CARS WITH POTENTIALLY BUGGY OUTDATED SOFTWARE, THAT WOULD HAVE OTHERWISE BEEN PATCHED AND LEAD TO A ACCIDENT DUE TO A SHUTDOWN EVENT, ECT. AS I PARKED MY CAR IN MY GARAGE ON 1/19/2019, I HAD PLACED THE CAR IN PARK AND WAS PREPARING TO EXIT THE VEHICLE WHEN IT SUDDENLY ACCELERATED AND HIT THE FRONT OF MY GARAGE WALL. THE CAR WAS IN PARK AND NOT MOVING. I HAD MY HAND ON THE DOOR HANDLE TO EXIT. I IMMEDIATELY SLAMMED THE BREAKS BUT THE CAR SEEMED TO KEEP GOING INTO MY WALL AND THEN IT JOLTED AND STOPPED. REPORTEDLY A 2015 TESLA MODEL S ON "AUTOPILOT" CRASHED INTO A LAGUNA BEACH POLICE PATROL VEHICLE ON TUESDAY ABOUT 11AM 5-29-18 @ 20652 LAGUNA CANYON ROAD, LAGUNA BEACH, CALIFORNIA. THE DRIVER OF THE TESLA WAS INJURED. THE DRIVER IS PROBABLY AT FAULT FOR INATTENTIVE DRIVING, HOWEVER THE HYPE FROM TESLA, ELON MUSK AND THE CULT FOLLOWING HAVE CONTRIBUTED TO TESLA DRIVERS DRIVING RECKLESSLY BY OVERHYPING THE SAFETY AND AUTONOMY OF TESLA VEHICLES. IT SEEMS THE VEHICLE LEFT THE DRIVING LANE AND ENTERED THE PARKING LANE AT THE SIDE OF THE ROAD. REPORTEDLY ELON MUSK AND OTHER MANAGEMENT REJECTED SENSORS AND OTHER TECHNOLOGY THAT COULD HAVE IMPROVED SAFETY AND AUTONOMY. TESLA ALSO HAS A HISTORY OF SCAPEGOATING THEIR OWN CUSTOMERS AND OTHER TESLA DRIVERS; TO DRAW ATTENTION AWAY FROM THE FAULTS OF TESLA. THE "SWISS CHEESE MODEL" INDICATES THAT MOST CRASHES INVOLVE SEVERAL CAUSAL/CONTRIBUTING FACTORS. THERE ALSO NEEDS TO BE MORE EMPHASIS ON THE PSYCHOLOGY OF "HUMAN FACTORS". WHILE PULLING INTO A PERPENDICULAR PARKING SPOT IN A PARKING GARAGE, AT ~4-5 FEET FROM A WALL, THE CAR EXPERIENCED SUDDEN ACCELERATION, SLAMMING INTO THE WALL, RESULTING IN MODERATE BODY DAMAGE TO THE FRONT OF THE CAR AND DEPLOYING THE DRIVER-SIDE AIRBAGS. THERE WERE NO OTHER PASSENGERS IN THE CAR, AND THERE WAS NO SIGNIFICANT INJURY OR DAMAGE TO THE GARAGE OR OTHER CARS. THOUGH I RECOGNIZE THAT THE VAST MAJORITY OF ACCIDENTS OF THIS TYPE ARE DUE TO DRIVER ERROR, THERE ARE SEVERAL FACTORS THAT LEAVE ME WITH CONCERN FOR A POSSIBLE ELECTRONIC SYSTEMS MALFUNCTION. SPECIFICALLY, ~3 DAYS PRIOR TO THE ACCIDENT, THE CAR BEGAN DISPLAYING A CONSTANT ERROR MESSAGE: 'DRIVER ASSISTANCE FEATURES UNAVAILABLE". I NOTIFIED TESLA SERVICE BY PHONE AND EMAIL, AND ON THEIR INSTRUCTIONS, REBOOTED THE COMPUTER SYSTEM, AND FULLY POWERED DOWN AND RESTARTED THE SYSTEM. HOWEVER, THE ERROR MESSAGE PERSISTED. THE ACCIDENT OCCURRED BEFORE I WAS ABLE TO SCHEDULE A SERVICE CALL. I HAVE REQUESTED DETAILS FROM THE DATA LOGS FOR THE ACCIDENT FROM TESLA, BUT THEY HAVE PROVIDED ONLY VERY LIMITED INFORMATION, STATING THAT LEGAL ACTION ON MY PART WOULD BE REQUIRED TO OBTAIN MORE DETAIL. THEY HAVE TOLD ME THAT THE CAR SPEED WAS 3.6 MPH AT WHICH TIME THE DATA INDICATES THAT THE ACCELERATOR WAS PRESSED TO AS FAR AS 100%, RESULTING IN ACCELERATION OF THE CAR TO 7 MPH AT THE TIME OF IMPACT, WITH THIS ACCELERATION OCCURRING OVER A 2 SECOND TIME PERIOD. THIS INFORMATION SEEMS INCOMPATIBLE WITH THE OTHERWISE RAPID ACCELERATION TYPICALLY EXHIBITED BY THIS CAR WITH A FULLY DEPRESSED ACCELERATOR, AND THE SHORT DISTANCE TRAVELED (ONLY SEVERAL FEET).

WHEN CAR DETECTS FOB PRESENCE, OR FOB BUTTON IS PRESSED, DOOR HANDLE IS SUPPOSED TO EXTEND TO PERMIT OPENING OF DOOR. CONTACTED SERVICE AND WAS REPAIRED UNDER WARRANTY. BROKEN PART IDENTIFIED AS PADDLE (PIVOT) GEAR, PN 1042845-00-A. THIS MALFUNCTION IS VERY COMMON IN THE MODEL S. THE HANDLE MECHANISM SHOULD BE REDESIGNED AND REPLACED VIA RECALL.

IT WAS EXPLAINED AS HAVING TO DO WITH THE ANGLE OF THE CAR ON A HILL, THE LOCATION OF THE SUN . . . THE CAR SEEING UNDER THE TRUCK . . . WELL, I HAVE RECENTLY HAD TWO EXPERIENCES, BOTH CALLED INTO TESLA, WHERE THE SENSORS DID NOT DETECT ENORMOUS TRUCKS THAT WERE STATIONARY, PERPENDICULAR TO THE LANE OF TRAVEL, AND ENTIRELY BLOCKING THE ROAD. IN THE FIRST INSTANCE AUTOPILOT WOULD HAVE DRIVEN ME DIRECTLY INTO A GARBAGE TRUCK. A GARBAGE TRUCK! AUTOPILOT SIMPLY DID NOT REALIZE IT SHOULD REACT TO A GARBAGE TRUCK BLOCKING THE ROAD. IN THE SECOND INSTANCE A BOX TRUCK MISJUDGED ITS TURNING RADIUS AND STOPPED PERPENDICULAR TO MY LANE. AGAIN, AUTOPILOT DID NOT RECOGNIZE THAT IT SHOULD REACT TO THIS ENORMOUS OBSTACLE, AND DROVE STRAIGHT INTO THE CAB OF THE TRUCK (ALMOST, I REACTED AT THE LAST MOMENT AND MANAGED TO AVOID THE CAB, DRIVING INTO THE CARGO AREA INSTEAD). YES, AS THE DRIVER OF THE VEHICLE WE MAINTAIN RESPONSIBILITY FOR WHAT OUR VEHICLES ARE DOING. THAT SAID, WE PLACE SOME DEGREE OF CONFIDENCE THAT THE SOFTWARE THAT POWERS OUR VEHICLES IS GENERALLY IN GOOD ORDER AND ABLE TO PERFORM IN A BASIC SENSE . . . DRIVING WEST ON HAMPDEN AVE TOWARDS INTERSECTION AT TAMARAC ST. GOING THE SPEED LIMIT, & RELEASED THE ACCELERATOR SO THAT "ENGINE BRAKING" WOULD SLOW THE CAR. BUT I WAS SLOWING DOWN FASTER THAN NECESSARY WITH ~100 YARDS REMAINING TO REACH THE INTERSECTION. I LIGHTLY PRESSED THE ACCELERATOR TO STOP ENGINE BRAKING BRIEFLY, THEN RELEASED THE ACCELERATOR TO REINSTATE "ENGINE BRAKING". INSTEAD OF SLOWING, MY CAR BEGAN TO WILDLY ACCELERATE. ALARMED AT THE INCREASING SPEED OF THE CAR, AS I WAS RAPIDLY APPROACHING THE INTERSECTION WHERE THE SIGNAL WAS RED, (ANOTHER CAR WAS ALREADY STOPPED AT THE SIGNAL). QUICKLY APPLIED THE BRAKES, BUT CAR WAS TRAVELING TOO FAST TO AVOID IMPACT WITH THE OTHER CAR. OTHER CAR WAS SIGNIFICANTLY DAMAGED, THOUGH IT SEEMED UNLIKELY (BASED ON COMMENTS FROM THE PARAMEDICS & POLICE) THAT THE OTHER DRIVER & PASSENGER WERE INJURED, BUT WERE TRANSPORTED TO AN ER TO BE CHECKED. I WAS SO STUNNED AT WHAT HAD HAPPENED THAT I INITIALLY THOUGHT I MAY HAVE PRESSED ON THE ACCELERATOR RATHER THAN THE BRAKE ( I'VE NEVER DONE THAT IN MY 55 YEARS OF DRIVING). BUT 2 DAYS LATER, AFTER MY MIND HAD CLEARED, I COULD RECALL THE DETAILS OF THE SITUATION, AND I'M CONVINCED THAT THE ACTUAL EVENTS OCCURRED IN THE ORDER DESCRIBED ABOVE. ANOTHER PIECE OF INFORMATION: 2 TIMES WITHIN THE PRIOR WEEK, WHEN GETTING INTO MY CAR (AWAY FROM MY HOME), THE CAR INDICATED "PREPARING TO DRIVE" OR SOMETHING TO THAT EFFECT (NOT A MESSAGE I NORMALLY SEE), THE MESSAGE PERSISTED, SO BOTH TIMES I HAD TO REBOOT THE COMPUTER SYSTEM, TO MAKE THE CAR DRIVE-ABLE. THE WHEEL IS DISPLACED TOWARDS THE REAR OF THE WHEEL WELL WHICH IS MOST LIKELY THE CAUSE OF THE FIRE. THE FIRE HAS BEEN HOT ENOUGH TO MELT METAL SO IT IS ALMOST CERTAIN THAT THE LITHIUM BATTERIES WERE THE SOURCE OF THE FIRE. PLEASE INVESTIGATE THE PROBLEMS WITH TESLA SUSPENSIONS AND ALSO THE ISSUE OF BATTERY FIRES. LITHIUM BATTERY FIRES BURN BURN EXTREMELY HOT, EMIT TOXIC FUMES, AND ARE DIFFICULT TO EXTINGUISH. THE ONLINE OWNER'S MANUAL STATES THERE SHOULD BE 144MM OR 5.65" OF GROUND CLEARANCE ON MY 90D. I HAVE LESS THAN 5". I HAVE BROUGHT THIS TO TESLA'S ATTENTION AND THEY STATE THAT THERE IS A RANGE THAT THEY CAN HAVE. I HAVE BELOW THE MANUAL CLEARANCE. THIS LOWER CLEARANCE HAS MADE ME BOTTOM OUT AND TO TAKE SPEED BUMP/DITCHES SLOWER MAKING CARS ALMOST HIT ME FROM BEHIND. WHILE DRIVING THE CAR AND ACCELERATING TO HIGHWAY SPEED, WITH AUTOPILOT/TACC ENABLED IT WILL BRAKE HARD FROM HIGHWAY SPEEDS TO AND INCORRECT "KNOWN" 45 MPH (+5 MPH AUTO PILOT THRESHOLD). I USE "KNOWN" AS IT APPEARS TO BE GPS DATA THAT PRESENTS THE MPH. THE ACTUAL SPEED LIMIT IS 65 MPH. DRIVERS WILL NOTICE THE CARS DASH BOARD WILL DISPLAY THE INCORRECT MPH AS WELL. RESULT IS THAT WHEN GETTING ON TO HIGHWAY, IT SLOWS YOU BACK DOWN TO 45-50 MPH. IF ACCELERATING ON AN ON RAMP, WITH CRUISE CONTROL ACTIVE IT WILL SLOW THE VEHICLE. MERGING INTO TRAFFIC ALREADY AT HIGHWAY SPEEDS AND HAVING THE MODEL S CRUISE CONTROL/ AUTOPILOT SLOWING DOWN WILL CAUSE REAR END ACCIDENTS INTO THE MODEL S. THIS SPECIFIC SCENARIO WAS ON A HAS HAPPENED MULTIPLE TIMES AND I HAVE MADE TESLA AWARE OF THE DEFECT/BUG. THIS ALSO HAPPENS WHEN PASSING UNDER OVERPASSES WITH A MPH THAT IS LESS THAN THE HIGHWAY SPEED LIMIT. IT HAS BEEN 3 MONTHS AND IT HAS NOT BEEN CORRECTED. THIS INCIDENT HAPPENED IN A CLOVERLEAF INTERCHANGE. I HAVE UNCOVERED OTHER DEFECTS OF THIS NATURE AND WILL REPORT THEM IN SEPARATE INCIDENTS.
