The contact owned a 2017 Tesla Model S. The contact stated that while starting the vehicle, an Autopilot disabled warning message system was displayed a day prior. The contact reached out to the Tesla hotline for assistance regarding the failure. The contact was advised to continue driving the vehicle, and the system error message was expected to clear. The contact stated that after multiple drives, the warning message did not disappear. The contact stated that while coming to a stop, the brake system became inoperable. The contact stated that the vehicle then accelerated to approximately 35 MPH, causing the contact to rear-end another vehicle. Multiple warning messages were displayed, including: traction and stability control disabled, automatic emergency braking system fault, battery management system critical fault, and electrical system reduced, as reported through Tesla chat assistance. The contact also stated that all air bags deployed. No injuries were sustained, and no medical attention was provided. A police report was filed. The vehicle was towed and deemed a total loss. The dealer was contacted; however, the vehicle was not diagnosed or repaired. The manufacturer was not made aware of the failure. The approximate failure mileage was 98,787.

Software version 2025.32.3.1. Using Tesla’s auto-steer / enhanced autopilot (not fsd) causes many “phantom braking” events when going through several intersections on my commute. It’ll say a message “Curve Assist Active”, however I’ll be on a very straight and flat road with no curves. It’s 65 mph road and it will brake / slow down the car very hard and fast, if I don’t catch it in time it’ll get down to ~45 mph within a second. Even when traffic is going slow already, like 40 mph, it’ll still bring up the message and try to brake and slow down, though not as hard. Typically to about 25-30 mph. I usually can catch it and use the accelerator to prevent it, but sometimes it’ll happen so sudden and I worry about getting rear-ended by someone behind me. Very repeatable. Very startling and concerning when it happens when I don’t expect it to. I believe it’s a mapping issue as it has not been fixed in over a year and after several software updates.

The contact owns a 2017 Tesla Model S. The contact stated that while driving at an undisclosed speed, the Forward Collision Avoidance system became inoperable. There was no warning light illuminated. The contact stated that the Forward Collision Avoidance feature failed to activate, almost resulting in a crash with another vehicle. Additionally, the contact stated that the back over collision avoidance system was inoperable. The vehicle was taken to the dealer, where it was determined that the front radar and bracket had failed and needed to be replaced. The vehicle was not repaired. The contact expressed a concern that the failure posed a significant safety hazard, as the Forward Collision Avoidance was inoperable. The manufacturer was not made aware of the failure. The approximate failure mileage was 66,573.

Software 2025.14.1, Full Self Driving 12.6.4 (FSD). Vehicle when using FSD continuously slows to 10 MPH below the speed limit. You can manually press the accelerator to speed up but FSD will not brake when doing so and warns you of the same. After a minute of being back at the speed limit, FSD again slows down. Repeat this process constantly. Very dangerous.

Stoped at intersection. While trying to engage signal lights, hit the stalk below that engages full self driving. Car immediately jumped into intersection .in front of another car and almost collided. Since the last update, Cars with my configuration wave been acting irradely and it is not safe to use FSD.

I used Summon on [XXX] to bring my car to me because it was parked way at the end of the parking lot at Discount tire. There was only 1 car in the whole lot, which was parked next to my Tesla. While using the “come to Me” feature in Summon – My Tesla pulled forward and then turned and drove right in to the car parked next to mine. It did quite a bit of damage to my Tesla and the Mercedes parked next to me it ran into. When I called my local Tesla Service Center in Austin to ask what I am supposed to do, they told me they needed to run diagnostics to see if they can fix the “summon” issue. After running diagnostics they blamed the issue on the wrap I have on my car. They did not test the sensors obviously though, because they work fine through the wrap. Not to mention that Tesla offers wraps from the factory now. They also stated that Summon was aborted due to collision, so it did report their was a wreck. Repair Notes: The proximity sensors on the vehicle are being obstructed by the vehicle wrap, which results in interference or compromised sensor functionality. This interference can disrupt the accurate detection of surrounding objects, impacting the vehicle’s ability to engage safety features like autopark. Data analysis reveals that the autopark function was aborted during collision, due to the seat belt not being fastened at the time of the incident. Full Self driving did not show signs of slowing down at Everett traffic slow down from about 60 miles to a standstill. I realized the speed not slowing down (about 5 car distance from stopped distance) I tried to apply the brakes that were completely nonresponsive, instead the car sped up. I avoided backend collision with the first car be maneuvering to the shoulder, however the next car was parked halfway into the shoulder that didn't leave enough room to not hit it. The car hit two other cars before coming to a full stop. While checking for any injuries for myself and passenger, after a delay and hitting 3 cars the airbags uselessly deployed. Total failure of brakes, auto emergency braking and airbag deployment. I have requested Tesla to look into the logs but havent gotten any feedback.

I received a safety recall dated 2/8/2024, recall no 23V-838. Tesla has failed to remedy the defect & claims a remedy is not necessary. Specifically, I took my vehicle to Tesla on 3/18/2024 to update vehicle software to incorporate additional autosteer controls. See attachment: "Repair notes: "No update necessary", despite the vehicle running a software version less than 2023.44.30 as specified in the Recall Notice.

Autopilot drove me off the road during turning right on Hwy 183 on April 4, 2023. 9:04 pm. My tesla Model S was on Autopilot and Car went off the road during right turn and about to hit the red light pole.

Tesla has failed to provide the software updates to fix the recalls on my and ALL other MCU1 cars that have FSD paid for. This is because Tesla has not delivered sent FSD to these cars, and hopes owners will pay $2,000 to upgrade to MCU2. Tesla has LIED that all cars with FSD paid for will be upgraded to the required hardware necessary to use FSD. This has now became a safety issue as recalls are not being addressed!! Tesla States vehicles should be running software version 2023.44.30 or later... We are stuck at 2022.8.10!

5 times on a cross country trip, I experienced the phenomenon of "phantom braking" . As I was driving with Adaptive Cruise on, doing approximately the speed limit, the brakes were automatically applied and the car started to rapidly slow down. Luckily there was no one behind me so I was not rear ended ! I immediately took the car to the local Tesla service center thinking there was a problem with the car, but they told me this is a know issue with no solution at present. One tech told me he has a Tesla himself and has experienced the same issue ! He told me that he just punches the accelerator and keeps on driving !

The car continues to make unsafe lane changes during left or right turns and struggles to make a smooth turn and is hazard for vehicles behind the car when making such turns as it makes suddent stops.

During a road trip (May 2023) with cruise control activated, I had at least 5 phantom braking events. The most serious was in Las Vegas on the freeway with a very severe braking event. I now no longer use cruise control in crowded traffic so that I won't get rear ended when a phantom braking event happens.

An issue with "Pahtom Breaking." My Tesla Model S has suffered from phantom breaking since I purchased the vehicle. I have repeatedly mentioned it to Tesla repair representatives and am still waiting for a fix. The risks exceed those of operating a car—a danger to me and others on the road. Vehicles following me have no idea that my truck will randomly slam on the breaks. Even if following a "safe" distance, the Tesla stops so fast that driving at highway speeds, others cannot react in time to prevent an accident. This is an undone and unknown burden for other drivers as it is for me. Tesla must step up to the plate and correct this before people die.

When using the automated cruise control or full self driving, it will unexpectedly phantom brake. Seems to happen more when there is full sun. I have the latest updates. Did much better when cloudy, but it would still phantom break a few times during a 12 hour trip. But when in full sun, it did brake 10+ times.

This alert is about "phantom braking", which is well known to Tesla owners, and the subject of much online discussion among owners who seem to all experience this and are furious that it continues to exist. Here's what happens, very, very often yet: with adaptive cruise control engaged (with or without "autopilot", ie 'autosteer' also engaged), the brakes are applied suddenly without any need for braking (no objects in the way of travel, etc). We just completed a trip on I-94 from Billings/Red Lodge, Montana across North Dakota, where phantom braking was experienced probably 50 times, with no other vehicles ahead of us, and on perfectly dry pavement, etc. There had also been very dangerous icy conditions (lots of even "skating rink ice") on the stretch of I-94 in Montana, from Billings into North Dakota. Had the phantom braking occurred on one of the sporadic ice patches further east, it could have easily caused a serious accident, not just been a scary nuisance. In situations with heavy traffic, phantom braking could easily result in rear-end collisions by cars that may be following too close to react before we, the Tesla drivers, override the phantom braking by hitting the accelerator--even that simple maneuver cannot be executed before the car has suddenly decelerated 10-20 mph or more. Some of the phantom braking seems to have been somewhat corrected where we'd first encountered it, on I-394 west of dntn Minneapolis--it doesn't happen as often now as it did a few years ago. There we'd suspected it may have been triggered by the shadows cast on the pavement ahead of us by overhead bridges. But this rash of events across North Dakota this week brought our worst memories and fears back with a vengeance--and with zero theories that could explain it. On a recent online discussion about this issue, there were several pleas for Tesla to give us a "dumb cruise control" option till this gets fixed. Please help foster solutions to this difficult situation! Thank you.

Our Model S doesn't have full-autonomy software, and in our case the described "braking for no reason" happen in cruise-control mode, without the (semi) auto-pilot engaged. In one of our incidents the sudden emergency-like braking caused a rear-end collision with the car behind me, it was however light enough that no visible damage was caused to either vehicle. We had paid $5,000 for the (semi) auto-pilot feature when we purchased the vehicle in 2017. I reported these incidents to Tesla (I have the emails and their responses) and after several rejections from Tesla that the issues were not reproducible I asked them to take the vehicle back (trade-in) and remove it from the streets because it was unsafe to drive. I further argued that I can't sell this vehicle to someone with good conscience without letting them know that they should not use cruise control. Tesla never acknowledged an issue and also never took ownership of the problem, we are still driving this vehicle today. I'm available to provide further detail if needed.

On 10/25/2022 at approximately 2:54pm I was traveling north on I-5 through Salem Oregon in the middle lane. Autopilot was engaged as I approached slower traffic 1 block in front of me in the middle lane. A pickup was approx 1 block behind me in the left lane. I activated the left turn signal. The vehicle initiated the transition from middle to left lane. Halfway between both lanes the vehicle applied hard braking action while continuing forward halfway between lanes. I tried to take over with application of the accelerator and force on the steering wheel to return to the middle lane. The steering wheel fought against me and the vehicle made a series of s-turns while braking. I tapped on the brake pedal and the system deactivated. The vehicle decelerated from 65-70mph to what I would guess to be 20-25mph with maximum braking (objects in the cockpit flew forward). The pickup behind me was very close when I regained control. If any vehicles had been closer or had the roadway been slick, the incident could have caused a major accident. I filed a ticked with Tesla and they advised this was known as a "phantom braking" event. There is no known fix, timeline for a fix, or mechanism to advise when there is a resolution. Tesla closed the service request. I'm a commercial pilot (former Part 135 Captain) and former flight instructor. I've been driving for 38 years with zero accidents. I have not used the Tesla autopilot since this incident. It is not safe in the current configuration. Tesla tech characterized the vehicle response as "conservative". There is no reason for maximum braking when nothing is in front of the vehicle. I would like Tesla to provide me with updates on resolving whatever is causing this. I would like Tesla to provide notification when the software patch is applied. Until such point in time when I'm notified the trouble is resolved, I will not be using the autopilot. Tesla charged several thousand dollars for this system.

Random breaking was prevalent with driving computer 2.5, which recently upgraded to 3.0. The "phantom breaking" is more general and is very dangerous. There have been locations where the vehicle behind me almost wrecked due to my car brakes slamming on out of the blue. I looked up the issue online and found the problem has been happening for years. My wife will not ride in my car now due to the fear of an experience when this phantom breaking accused while she was with me. I am using adaptive cruise less often and not using autopilot. The downside is that the car's efficiency is reduced when not using these "smart" features.

The issue occurred twice over the period of the past two months, both times I was driving on highway with Tesla autopilot enabled ( It is Adaptive Cruise Control), for no apparent reason the car hit the brakes, all vehicles driving behind me had to hit brakes or change lanes to avoid Collision. Both times I created service request with tesla and got no clear answer to why its happening, first time I was told it is because the exit on map was about couple miles from car location so car had to hit brake hard and change lane to make it to exit but car never tried to change lane, it just did hard braking. 2nd time I was told it happened due to wheel alignment not being accurate. After wheel alignment was done I reached out to tesla again but so far they can not confirm or deny if tesla autopilot is safe for me to drive. I was told to use autopilot at my own discretion. Are car manufactures allowed to sell hardware or software without taking responsibility for its safety ?

Vehicle automatically and unexpectedly applies brakes when using Cruise Control when no other cars are in proximity or approaching. Sometimes braking is light but other times it is a jolt that is extremely starling. Often happens multiple times within minutes. Unpredictable when it will occur. I have taken the vehicle to Tesla 3 times and they say that they know about the problem but their Autopilot is still in Beta and are not actively working on a fix. On 4/28/2022, while on a trip, there were 5 incidents between 9:51am and 1:43pm.

Tesla cars receive OTA (over the air) updates, and these updates cannot declined. If an OTA update changes my car so that it no longer uses the radar with which it is equipped, I will have a much less safe vehicle, one that can't detect obstacles when there is poor visibility such as in the heavy fogs, rains, and snow conditions that we often encounter here on Washington's Olympic Peninsula. NHTSA should forbid the use of "Tesla Vision" without radar, and should require all Tesla vehicles to be retrofitted with functional, forward-looking radar.

On several occasions, I have been doing highway driving, and my 2017 Tesla Model S suddenly brakes without warning. It happens primarily on cruise control and around curves on the highway. On a trip to Atlanta earlier this year, it happened twice. I bought the car used in 2021, and it drives very well, but the unexpected braking at high speeds is scary.

I have experienced an increase in the number of phantom braking incidents when my autopilot is engaged. It's been on hilly roads but the car would just decelerate extremely fast when nothing was there. I also had two incidences on the highway where the screens froze and I couldn't control the instrument panels or see my speed etc.

It was brought into the dealer multiple times, who actually viewed the error, and when they were driving it, they were involved in an auto accident. Even with the warranty, they could never fix it, and it was amazingly dangerous almost killing me several times.

Those of us with the malfunctioning MCU1 were not included in the FSD program and were forced to bribe Tesla with $2500 to update our MCUs even when they were under warranty and should have been included under the FSD contract.

Additional report for same problem with DIFFERENT car. This report is not about a single incident, but rather a REPEATED problem over several years with the adaptive cruise control on 3 different Tesla cars I have owned. These cars are a 2017 Tesla S, and 2 different 2021 Tesla Y models. in each case, whenever these cars are driven on a highway with the adaptive cruise control engaged, several times during each hour of driving, the cars will rapidly brake/decelerate with no action taken to cause the situation by the driver, nor any external situations with the traffic that would justify them. With the Tesla regenerative braking, these deceleration events are so extreme that it would be easy to imagine an accident with a car following closely behind. Indeed, with these situations occurring with no warning and extreme braking they are quite frightening to the passengers traveling in the car. During the most recent occurrence, I noted the exact day and minute of it's occurrence, and filed a service request with the local Tesla office. Even with the exact time of this event, the service center was unable to find anything wrong with the car. The Tesla service center attributed this issue to problems with the Autopilot and Full Self Driving software, but none of my cars have had this optional software-only the software which came standard on each of these cars. While otherwise appreciating the good characteristics of these cars, this unexplainable deceleration problem is what caused me to sell the prior 2 models before purchasing this latest model Y, hoping that this problem would not recur. It is worth noting that I have owned several other make and model cars with comparable adaptive cruise control, yet this situation never occurred even once on any of these other makes of cars.

Driving with autopliot engaged. I was in the left lane of a two lane highway when another car entered the highway to my right and car suddenly slammed on the brakes. i disengaged the autopilot. It happen several more time, so it is now a function I no longer use unless the highway is empty. I believe is was a $1000 option.

I experienced the "Phantom Braking" phenomenon while driving on freeways. It has happened 3 or 4 times in the last year or so. I did not see any obvious reason for the car to have slowed down dramatically because there wasn't a car in front of me at such instances. it occurred while Autopilot was engaged. It has happened on different freeways (I-680, I-880), traversing in different directions and at different times of the day.

The contact owns a 2017 Tesla Model S. The contact stated that on three separate occasions while driving at highway speeds with cruise control engaged, the vehicle would suddenly brake on its own. The contact was uncertain if any warning lights would appear on the instrument panel prior to the failure. The service center nor the manufacturer had yet to be notified of the failure. The contact stated that he was able to document the exact time that the latest failure occurred on the vehicle. The vehicle had yet to be repaired. The failure mileage was approximately 48,000.

Ever since I purchased this car I have suffered from Phantom Braking when using Cruise Control. As a result, I am unable to use any of the advanced Autopilot features. I spent $10,000 for something that has never even begun to work. Phantom Braking occurs seemingly at random, with no vehicles in front, nothing on the road and no overpasses or shadows. I try every few months to see if anything has improved, most recently in January on 101 north of Petaluma. The car suddenly braked, freaking out my passenger. I only try it when no other vehicles are around, so that I am not rear-ended when it happens, after some very scary near-rear-ending events early on. I have reported this problem to Tesla on MANY occasions but after being told "it is a known problem" I have given up reporting it. Is it repeatable? Yes, just drive on cruise control. Safety risk is being rear-ended. Has the problem reproduced by service? No, has it been confirmed, Yes. Has it been inspected by Tesla? Yes Warnings prior to the failure? No

The brakes have engaged while driving at high speeds (perhaps on about a half dozen occasions) when using either the adaptive cruise control or auto pilot, at a time when there were no road hazards or other cars in the forward or side vicinities. The braking that occurred was severe - a hard braking if you will. On two occasions, this same kind of braking occurred at the same point on a certain freeway that I was traveling on. This braking also occurred once while driving on a city street at low speeds with auto pilot engaged. The dates in which these incidents occurred were all in 2021. Since Tesla has disabled the radar, and went to vision only, my car is a nightmare to drive on autopilot or FSD on two lane roads at night. Phantom braking, numerous times it has went onto the shoulder thinking an oncoming car a 1/4 away was in our lane, twice it even went into the oncoming traffic lane. Never did this when radar was enabled. My car has radar but they turned it off in software update

My Tesla Model S has emergency braked several times on a divided highway when I was driving at about 65 mph. This has happened multiple times over the last 4 years. Sometimes it happens when I drive under a bridge overpass. Sometimes it happens if the camera sees roadwork equipment on the side of the road - no where near I'm driving. When this happens I'm always afraid someone is going to rear end me. I no longer use any of the self-driving features on my Tesla and do not enable the emergency braking option. I don't trust it.

While on I65N, traveling at full speed on cruise control but not auto drive, the vehicle automatically applied the brakes. There was no apparent reason to brake. There were no cars in front or to either side and nothing on the road. We slowed quickly and abruptly, throwing everyone in the car forward. The slowing was so fast and so extreme that we were almost rear ended by the semi behind us. If he had not been able to move partially into the right lane and us into the left, he would have hit us at 70 mph. This is not the first “phantom braking incident ” that we have had but it definitely is the most dangerous one.

In the last couple of months the incidences of random braking have increased. Usually this has been on the freeway when using adaptive cruise control and often when there are no other vehicles around. However last week I was driving manually (no cruise control or “self driving”) on a two lane road. My foot was on the accelerator pedal, and had been since the last stop light about a half mile before, when the car suddenly and vigorously applied emergency braking. I immediately pushed the accelerator to the floor to avoid being rear ended, but the car continued braking almost to a standstill before I had control again. There were no vehicles in front of me, however there were vehicles in the oncoming lane. They were not near the center line. I turned off the automatic emergency braking at that time because I felt it was dangerous to leave it active. I would additionally state that Tesla has a habit of rearranging the touch screen so that you no longer know where to find certain controls and is dangerous because it’s then necessary to take your eyes off the road to figure out how it works now. Finally, I’ve been driving Tesla vehicles since 2016 and I love them. I paid for the “full self driving” option years ago and a “beta” version became available a few months ago. This is not ready for prime time. If I wanted thrills I would go to an amusement park. Even more basic things like auto lane change often don’t work because the cameras are constantly getting blinded when the sun is low in the sky. Here in the north that is a good part of the day during the winter. I don’t see how their system can ever be made to work with the existing hardware. I wish there was a way to go back to a firmware version that worked better instead of them always tinkering with it. I feel like a guinea pig

On autopilot, if I approach a 60-mile speed limit section with speed more than 60mph and on my right is there a big container truck, my car suddenly slows down drastically. This makes people behind me mad because they think I brake-checked them. If I pressed on the speed pedal, it will accelerate but it will brake again if I release the speed pedal too soon. It almost consistently behaves like that.

The contact owns a 2017 Tesla Model S 75. The contact stated that while driving at approximately 70 mph, the brakes were seized while on autopilot. In addition, there was a warning error the contact was unable to remember. The contact mentioned the emergency brake engaged independently and the contact was unable to control the vehicle which caused the vehicle to crash into the guardrail. A police report was not filed. The air bags failed to deploy. The vehicle was towed to a service center where a diagnostic was not attempted but the vehicle was repaired. The manufacturer was notified of the failure. The VIN was not included in NHTSA campaign number: 21v846000 ( forward collision avoidance). The approximate failure mileage was 54,504. Vehicle was not listed on the recall.

THESE SOUNDS MAY INCLUDE BLINKER NOISE, SPEED LIMIT CHIMES, LANE DEPARTURE WARNING, AUTOPILOT ENGAGE/DIS-ENGAGE CHIMES, GEAR WARNINGS, COLLISION SAFETY CHIMES, EARLY BRAKING WARNING, AND A LOT MORE SAFETY-RELATED SOUNDS. THE SUDDEN PHANTOM EMERGENCY BRAKING EPISODE I EXPERIENCED AT HIGHWAY SPEED FROM DALLAS TO HOUSTON WAS EXTREME. I SIMPLY DO NOT FEEL SAFE USING THE CRUISE CONTROL OR AUTOPILOT IN THIS VEHICLE. BROUGHT THE CAR IN FOR SERVICE AT THE MACADAM SERVICE CENTER IN PORTLAND, OREGON ON OCTOBER 29TH DUE TO MCU ISSUES; THIS WAS BEFORE ANY NOTICE HAD GONE OUT! I WAS NOT EVEN AWARE OF THE ISSUE AT THIS TIME. IT WAS REBOOTING, VERY SLUGGISH, GETS 'LOST' IE KEEPS OUTDATED INFORMATION ETC. THIS IS OBVIOUSLY THE 8GB MEMORY DEVICE ISSUE. AFTER A 40 YEAR CAREER AS AN ENGINEER AND VENTURE CAPITALIST IN THE IT INDUSTRY I KNOW WHAT MEMORY FAILURE LOOKS LIKE! THE SYSTEM REBOOTED A NUMBER OF TIMES, WAS EXTREMELY SLUGGISH AND HAD DATA UPDATE ISSUES. SPECIFICALLY: -DEPART LAKE OSWEGO AT 5:30AM ON 11/23. SOON THEREAFTER THE LEFT FRONT FENDER CAMERA INFORMS ME IT IS NOT WORKING. IT DOES SO REPEATEDLY EVERY MINUTE OR SO FOR A LONG TIME. THEN ALL CAMERAS INFORM ME THEY ARE NOT WORKING. I STOP, TWICE, CLEAN ALL THE CAMERAS AND IT DOES NOT RESOLVE ANYTHING. I GIVE UP ON THE CAMERAS WORKING AND STOP USING CRUISE CONTROL -AT 12:31PM IN THE SHASTA AREA STREAMING FAILS AND THEN THE WHOLE MCU SHUTS DOWN. WHILE DRIVING IN THE RAIN I AM FORCED TO REACT AND OPEN WINDOWS TO KEEP THE WINDOWS FROM FOGGING UP WHILE DRIVING AT 70MPH. OTHIS IS A SERIOUS SAFETY CONCERN. I HAVE UPDATED MY NHTSA COMPLAINT TO INCLUDE THESE SERIOUS SAFETY CONCERNS. -7:48PM 11/23, WHILE SUPERCHARGING IN EMERYVILLE THE MCU COMPLETELY FROZE. I WAS HAD TO FORCE A REBOOT. 2ND REBOOT IN ONE DAY -5:28PM 11/24 MCU CRASHES WHILE DRIVING AGAIN! HOW DO YOU NOT UNDERSTAND THE SAFETY IMPLICATIONS OF THIS?! THIS IS THE 3RD CRASH IN 2 DAYS!.
