On [XXX], I was driving my recently purchased 2018 Tesla Model S (VIN: [XXX], mileage approx. [30,536]). The vehicle was purchased directly through Tesla’s app in late August 2025 and delivered to me the same day of the incident. Approximately 4.5 hours after delivery, while driving with my son, white smoke began to come out of the cabin air vents. Within minutes, the smoke turned black and rapidly filled the interior of the car. I immediately pulled over and evacuated the vehicle with my son. Shortly after, the car caught fire and was completely destroyed. No modifications had been made to the vehicle, and it was sold to me as inspected and certified by Tesla. The sudden appearance of smoke from the vents and rapid escalation to a full vehicle fire posed a severe danger to me, my child, and others on the road. I am reporting this to NHTSA to document the incident and to request that the agency investigate whether Tesla Model S vehicles have underlying defects that may cause dangerous cabin fires shortly after use. Operating the car on summon and it crashed into a curb and trashcan, causing extensive damage. I currently have a claim in with my insurance.
•I’m not sure what component was responsble for either the braking issue or the fire. It is available for inspection. •When driving the brake pedal had a frightening delay that almost caused 2 accidents and unfortunately my car was charging on the 24th of October which could have something to do with arching that led to my garage catching fire rendering the car unable to move or turn on after the incident. I’ve insisted Tesla insurance to look into possible causes because the Fire dept. mandated the a LADWP (power company) to install new transformers to our power supply a couple of days after the fire, which they did. Tesla insurance seems uninterested in finding the cause and wants to compensate me for the vehicle but I believe it’s in their best interest to find a cause and document it. The car had no issues prior, in fact the service center sent a mobile tech to replace the 12volt battery on the car not even 10 months ago. After the fire the car died and has a bunch of codes that make it inoperable. • Service center rejected looking at it because of the risk. They said since it was possible salvage they could not look at it which is understandable but they worked on it before in decem •Insurance has been unwilling to look at any of this they just want pictures of the car to settle the issue. In my opinion they are ignoring a bigger problem. Tesla is my insurer. •The car had no issues prior to the fire and none during the near misses were the brake delayed.
I pulled into a driveway on a one lane road (due to construction) to allow a car traveling downhill to pass so they would not have to back up. I had come to a complete stop in the driveway with the car in drive. As I started to put the car in reverse it surged forward at a high rate of speed crashing into my neighbor's garage, tearing the door completely down pushing the door into the garage and my car entering the garage at least 15 feet hitting the car that was parked inside and pushing it with such force that the front end of the vehicle struck the garage cabinets tearing them down. My air bag did not deploy nor did the automatic braking system engage, it just sped up as it was doing the damage. The car has been deemed a total loss by my insurance company but i don't want this report to go unnoticed. To my knowledge the insurance company has not inspected the computer on the vehicle. I'm not sure how they will investigate this occurrence. I feel strongly that this was a malfunction with my car. The message that showed immediately was "driver assist unavailable", I have NEVER used driver assist.
Overnight, the vehicle's power completely died, and there was no mechanical way to open the doors from the outside. My child's car seat is stuck in the car. At my expense, I'm having to get it towed to Tesla so they can "figure out a way" to open the door and retrieve the car seat. If something like medicine were trapped in the car, this would be a life-or-death situation. I cannot believe the car has no mechanical way of opening the doors, but everyone at Tesla I've talked to and all the documentation I was sent show that one cannot open the doors from the outside without power or the windows being down.
I took my car to the dealership to have the MCU1 recall performed because my behind the steering wheel monitor and center touch screen monitor kept turning off and reseting while driving. When this happened I would lose the ability to see my speed or view my back up camera. The recall was performed and the problem was not fixed. The screen does not turn off as often, but it still regularly happens. I contacted Tesla to let them know the problem remains and they want me to pay $1750 to UPGRADE to the newer MCU2.
I was driving on the [XXX] to for a work meeting on Monday [XXX], 2024, at about [XXX] (central). My 2018 Tesla Model S shut down while driving on the freeway. The power steering went off and so did the hazard lights. I pulled over to the shoulder. Unfortunately, the shoulder is not too wide on [XXX]. I was driving at 70 mph or so when this happened. The only warning that popped on the Tesla Screen was “Vehicle Shutting Down Pull Over Safely”. The battery still had about 175 miles left to empty. I believe my software is hacked. Car was purchased 6 months prior to when the vehicle shows registered to me. The vehicle shows no location with Tesla and has no direct contact from the vehicle with Tesla. My display screen blacks out while I’m driving on the freeway or streets. Leaves me feeling unsafe in bad areas I am driving to pick up drop off kids from college, school, or film school projects . Some of the safety hazard warnings do not work at times. Does not give me correct supercharger locations which leaves me in scary situations. Traveling on [XXX] my car excelled to 91mph without warning and didn’t use any sort of automated system as I do not have that capability. Tried to step on brake but did not work and I changed lanes which suddenly caused the car to slowly bring the car to driving mode again. Very scary. The car speaker had a male voice and all the car occupants pulled out their phones to see if it was coming from their phones. But it definitely came from the vehicle speaker system. My iPhone was hacked with NSO Israel' Company Pegasus spyware and now my car is being hacked. I never connect my device to my car. Although my kids do periodically. I drove my car out of garage in reverse gear & put it to drive straight down my driveway ,instead Vehicle spun around going backward in front of the garage hitting a trash can, bushes ,flower pots &hit the patio wall ,then turned around in front of trees & stopped in position where it started thus completing full 360 degree circle.
My wife, [XXX] was driving the car on April 12, 2023 and following is her statement of the incident. April 12, 2023, I was pulling my car into a parking space of a supermarket. It was about to come to a stop and all of a sudden it accelerated, hit the car parked facing it and then dragged for about 20 feet before coming to a stop. it was an unintended acceleration, which I have been trying to contact Tesla for investigation. I requested pre-accident scan data using Tesla App. Did not get any response from Tesla for weeks. After making numerous calls and spending 40-50 hours on the phone trying to get a hold of someone regarding the data I requested, I drove down to West Covina (CA) Tesla service. The service manager told me that the report has been run and reviewed by Tesla and it is determined that the car did what it was supposed to be doing. I requested a copy of the report (scan) to understand how it was determined. He said it can not be released. I have to formally request using the link he provided. I did so and the report (vehicle data) that I received is a bunch of matrix which can not be understood. I am trying to contact Tesla and once again "No Response". It is very frustrating. I am afraid of driving my car so have not used since the day of incident. I want the bug to be fixed before using my car. Anybody whom I have spoken with knows somebody who has experienced similar situation. All of the Tesla owners are unhappy about Tesla's customer support. Steering wheel buttons INCLUDING HORN have stopped functioning. Not able to use the horn seems to be a serious safety issue to prevent accidents. Not repairable due to parts shortage. Epidemic failure according to Tesla Service.
Door handle not working and not popping out as expected and driver can’t go inside the car. A few complaints already from the internet and a lawsuit already filed by other owner and this seems to be recall already but nothing was mentioned yet Camera get blinded or get block easily with rain or sun. This is an issue if you’re using the autopilot.
On August 4th, 2022, I got in my car to continue north...and got a "can't drive not enough power" report on the dash. The car is still (as of today, 9/10/2022)in North Carolina ( at the Matthews-Charlotte Tesla service center, 9140 East Independence Blvd, Charlotte-Matthews, N.C.). Tow/hauler also took toll lanes to bring car to service. I received $20 in toll charges while I wasn't in the car for that day. This happened once before to this vehicle (in Charleston, S.C. serviced in Savannah, Ga 10/11/21 to 12/01/21), and twice before to a formerly owned Tesla. Battery died unexpectedly, suddenly (but while parked). I am still renting a gas car (being charged by Enterprise, weekly at over $500 a week) and I'm not happy with the car (engine light on twice RA # 766VYT) unit #7TBJHP ). Not comfortable driving with rental. Help, please?
The emergency release pull for the manual trunk open does not work. Truck is stuck closed and the release cable does nothing in case of emergency…
As I was driving alone on July 13th 2022 on the Capital Beltway 495 and was nearing exit 44 the rear passenger window exploded spontaneously. This occurred at 9:08am ET. I have footage of this (I was not able to upload the file as it is not in one of the acceptable formats (MP4)) which confirms there was no object that caused this breakage.

ON SEVERAL OCCASIONS BOTH OF MY DASHBOARD SCREENS (DRIVER CLUSTER AND CENTER CONSOLE) WENT BLANK WHILE DRIVING AT A VARIETY OF SPEEDS. I WAS UNABLE TO SEE MY SPEEDOMETER, BACK-UP CAMERA, CONTROL HVAC, OR ANY AUDIO. I ALSO WAS UNABLE TO USE COLLISION ASSISTANCE, BLIND SPOT DETECTION, AUTOPILOT, OR, PRESUMABLY, EMERGENCY BRAKING. TO REGAIN CONTROL I HAD TO REBOOT THE VEHICLE WHILE DRIVING, WHICH WAS HAZARDOUS TO SAY THE LEAST. THERE DOES NOT APPEAR TO BE ANY SPECIFIC TRIGGER TO THIS OUTAGE OTHER THAN THE VEHICLE WAS IN GEAR AND MOVING FORWARD. IT HAPPENED DURING THE DAY AND AT NIGHT. I REPORTED THIS TO TESLA USING THEIR "REPORT BUG" FEATURE IN THE VEHICLE, BUT I NEVER HEARD ANYTHING BACK. THIS HAS OCCURRED AT LEAST 6 OR 7 TIMES OVER THE LAST YEAR. THE MOST RECENT INCIDENT OCCURRED LAST WEEK, 15-16 MARCH.

MY SEPTEMBER 2018 BUILD TESLA MODEL S EXPERIENCES THE SAME SCREEN FAILURES (BOTH THE DASH + MEDIA CONTROL UNIT) AS THE RECALLED VERSIONS, BUT MINE FALLS OUTSIDE OF THE RECALL PERIOD. THE LATEST INCIDENT OCCURRED WHILE I WAS DRIVING ON A DESOLATE HIGHWAY TRAVELING AT 85 MPH. I LOSE MY SPEEDOMETER AS WELL AS FEATURES SUCH GPS WHICH OCCURRED ONCE IN AN UNFAMILIAR AREA.

THE LATEST MODEL S SOFTWARE UPDATE SHRANK THE SIZE OF THE FONT OF SEVERAL CRITICAL PIECES OF INFORMATION ON THE DIGITAL DASHBOARD. THESE INCLUDE THE GEAR SELECTION (PARK, DRIVE, ETC), THE REMAINING BATTERY CHARGE (FUEL TANK EQUIVALENT), EXTERIOR TEMPERATURE AND TIME. I AM NO LONGER ABLE TO RAPIDLY IDENTIFY THIS KEY INFORMATION WITHOUT EXTRA EFFORT OR PUTTING ON READING GLASSES. LAST WEEK, WHILE AT AN ENTRANCE TO THE PARKING GARAGE AT WORK, I ADVANCED BECAUSE I THOUGHT MY ACCESS BADGE HAD OPENED THE GATE. UNFORTUNATELY, IT (THE GATE) CAME DOWN BEFORE I HAD A CHANCE TO ENTER THE GARAGE. I BACKED UP, REAPPLIED MY KEY CARD, BUT FAILED TO IDENTIFY I WAS STILL IN REVERSE. WHEN I STARTED TO ATTEMPT TO GO FORWARD, I FAILED TO NOTICE DUE TO THE SMALL FONT THAT I HAD NOT SUCCESSFULLY RETURNED TO FORWARD DRIVE AND ALMOST BACKED UP INTO THE CAR BEHIND ME WAITING THEIR TURN TO ENTER THE GARAGE. I ALSO NOW HAVE DIFFICULTY IN PARKING LOTS CONFIRMING WHETHER I AM IN DRIVE, PARK OR REVERSE WHEN ATTEMPTING TO MANEUVER. THE TINY FONT IS A MAJOR SAFETY CONCERN!! I HAVE CHECKED ON THE TESLA WEBSITE OWNER FORUMS AND THIS PROBLEM IS THAT WIDESPREAD FOR 'MIDDLE AGED AND OLDER' OWNERS. THE STEEL BUMPER ATTACHMENT BAR IS UNDAMAGED WHICH SUGGESTS THAT THE DAMAGE WAS NOT THE RESULT OF A FRONTAL IMPACT. REGARDLESS OF THE CAUSE OF THE DAMAGE THE FACT IS THAT ONCE AGAIN THE CAST MONKEY-METAL STEERING KNUCKLE HAS FRACTURED IN HALF. THE CAR ALSO DOES APPEAR TO BE BOTTOMED OUT ON ITS AIR SUSPENSION. TESLAS DO NOT HAVE ANY LIMIT STOPS ON THE SUSPENSION. IT IS QUITE PROBABLE THAT THE MCU FAILURES THAT ARE UNDER NHTSA INVESTIGATION ARE CONTRIBUTING TO THE PROBLEM. HERE IS A THREAD IN WHICH ANOTHER OWNER REPORTS A FRACTURED STEERING KNUCKLE BUT THE ABSURDLY LOW SUSPENSION HEIGHT IS BLAMED ON A THIRD PARTY LOWERING KIT. I AM THE OWNER OF A 2018 TESLA MODEL P100D. ON DEC 17 AT APPROXIMATELY 11:40 AM I AND A BUSINESS ASSOCIATE WERE DRIVING TO MY HOME AT 804 DUKE STREET, ALEXANDRIA, VA. MY ASSOCIATE WAS DRIVING APPROXIMATELY 1-2 MILES/HOUR AND STARTING TO PARK. THE PARKING LOCATION IS IN FRONT OF A BRICK WALL. WHILE COMING TO OUR FINAL STOP THE TESLA BEGAN TO ACCELERATE ON ITS OWN AS THOUGH THE GAS WAS PUNCHED (IT WAS NOTE). THE CAR THRUST FORWARD AND SLAMMED INTO THE WALL, DESTROYING A PART OF THE WALL AND DAMAGING THE FRONT OF THE TESLA. THE AIR BAGS DID NOT ENGAGE AND WE WERE NOT INJURED. THE CAR SIMPLY ACCELERATED ON ITS OWN. I REPORTED THE INCIDENT TO TESLA BUT THEY HAVE NOT RESPONDED.

I QUICKLY CORRECTED TO THE RIGHT. IT SCARED ME. IN ADDITION I CAME HOME AND DROVE INTO MY GARAGE LIKE I ALWAYS DO. THE BUZZER WENT OFF AGAIN TELLING SOMETHING WAS IN THE WAY AND SHIFTED ITSELF INTO PARK. I GOT OUT OF CAR AND CHECKED TO SEE IF ANYTHING WAS IN THE WAY. THERE WAS NOT. I CALLED TESLA STATING THESE THINGS WERE SAFETY ISSUES. THEY WANTED ME TO DRIVE THE CAR IN TO THE SERVICE CENTER. I TOLD THEM MY 3 AND 6 YEAR OLD GRANDKIDS WHERE STAYING WITH ME AND I DID NOT FEEL SAFE DOING THAT. AFTER CALLING CONTINUOUSLY FOR 2 DAYS, A LOANER WAS BROUGHT TO MY HOUSE AND MY TESLA WAS DRIVEN (NOT TOWED) TO COSTA MESA SERVICE CENTER.

I SAW PICTURES OF SO MANY IRANIAN SEVERELY INJURED AND BURNED SOLDIERS BEING AIR LIFTED TO GERMANY FROM THE WAR ZONE, BECAUSE IRAN DIDN'T HAVE THE MEDICAL CAPABILITY TO HANDLE THIS SITUATION. NOTHING CAN BE DONE ANYWAYS. ONCE YOU ARE EXPOSED TO MUSTARD GAS, YOU'RE FINISHED. IT'S JUST THE MATTER OF QUICK DEATH, OR VERY PAINFUL PROLONGED DEATH.
