I got into my car two days ago and the computer screen was black. I have tried everything to reset it, holding down both scroll wheels with the brake pressed for up to 2 minutes, unplugging the low voltage battery overnight and plugging it back in, and even doing the hard reset at the disconnect under the rear passenger seats and my computer will not turn on. I can't see any information about my speed or how much charge I have, I can't turn on climate controls and it's the middle of winter and I live in the mountains of Colorado. I can't even open my glove box without the screen. From everything I read, it sounds like an MCU failure, my vehicle has the newer MCU2. Tesla quoted about $800 for a tow and it sounds like the replacement costs could be $2500+

The contact owns a 2022 Tesla Model Y. The contact stated while his wife was driving 40-50 MPH in the evening with Self-Driving mode activated, the vehicle failed to decelerate while driving onto the exit lane and crashed into the rear of another vehicle. The contact was occupying the front passenger's seat during the incident. The front driver's side and passenger's side air bags deployed. There was no reported injury. The vehicle was towed to the residence. The vehicle was later towed to a Tesla autobody repair shop to be repaired. The vehicle was then towed to the autobody repair shop recommended by the insurance company. The vehicle was later totaled by the insurance company due to the cost of the air bags replacement. A police report was filed. The manufacturer was notified of the failure. The failure mileage was approximately 30,000.

Incident Summary: Car Computer (HW3) suffered internal hardware failure on 12/17/25 following software update 2025.44.3. While the media screen and rearview camera functioned, the Autopilot Board (APB) was "MIA." Tesla Randolph (Invoice #3000S0015357298) confirmed the internal failure and replaced the unit. Safety Risk: Failure disabled safety-critical ADAS: Automatic Emergency Braking (AEB) and Forward Collision Warning. Driving visualizations were severely compromised; the vehicle failed to detect pedestrians or lane markings while in motion, creating an immediate collision risk. The car’s active safety "intelligence" was essentially blind despite the camera feed being visible on the screen. Inspection & Contradictions: Tesla Service confirmed the "internal failure" (12/19-12/23). Note: The Service Manager verbally claimed "no codes," but the official invoice explicitly states technicians "found multiple alerts present." Part was retained by Tesla. Warning Symptoms: Failure followed a software reboot loop. Symptoms (MIA board/power cycle failure) match Recall 25V-002. Vehicle had only 36,289 miles. Reporting as potential HW3 defect expansion.

My 2022 Model Y, 213k miles, battery health 77%, no issues until Beginning of November 2025. After Tesla’s update, BMS_a079 popped up, capped charge at 22%. No dead cells, no imbalance—pure software bug remotely triggered by Tesla. Service refused written diagnosis, only said ‘nine thousand dollars for new battery. Risk of stranding.

The car stopped in a busy freeway saying electric issue and doesn’t give sufficient time to drive to shoulder. This is the 2nd time this happened within 2 weeks. First time, tesla said high voltage controller had an internal issue with voltage sensing. the hv controller was replaced along with pyrotechnic fuse. Tesla assured this shouldn’t repeat and it repeated again. This has kept us in a catastrophic situation. I request immediate attention to this error and matter.

I have repeatedly had electrical failure in the vehicle in the form of a "Reduced Power Alert" which renders the vehicle with significant limitation to drivability. This places the vehicle in "limp mode" and this frequently occurs on the highway. Once this Alert appears the vehicle has limited propulsion, limitations to steering and braking as well as other electrical systems. It has been to the dealership for repair on 4 occasions without remedy. It is leaving us on the highway in a dangerous situation repeatedly. It's not a one time event. There were 20 Alerts showing on the screen when I limped it into the dealership once again on [XXX] having nearly wrecked on [XXX] just before the [XXX] exit. The reason this is so dangerous is the failures occur at higher speeds such as highway driving. Leaving one with an immediate loss of forward motion as if you've hit the brakes but vehicles behind you have no warning. This is going to end up with someone getting rear ended or worse. Social media indicates it is a prevalent and not well understood recurring problem. Tesla has been quietly repurchasing vehicles from angry owners. Dreadful. To the National Highway Traffic Safety Administration, I am filing a formal complaint regarding a dangerous safety defect in the 2021 Tesla Model Y. On December 2, 2025 at approximately 4:25 PM, my vehicle suffered a sudden, complete electrical shutdown while I was driving, with no warning and no opportunity to reach safety. Critical details: The vehicle displayed an electrical system message, then instantly powered down. I lost all ability to steer or move the vehicle, which remained immobilized in active traffic. Hazard lights did not function, leaving the vehicle unmarked and invisible to oncoming vehicles. Electronic systems failed, requiring manual release of the driver door. The incident occurred at dusk and blocked the roadway, placing myself and others at significant risk. This failure presented an immediate threat to life and safety. I have since discovered that other Tesla owners have reported similar abrupt electrical or drive-unit shutdowns, suggesting this may represent a broader systemic defect. I am formally requesting that NHTSA investigate whether this issue meets the threshold for a safety recall and whether Tesla’s systems contain a defect capable of causing sudden, uncommanded shutdown while driving. Thank you for your prompt attention to this matter. I am prepared to provide all supporting documentation, including incident details, screenshots, timestamps, and service correspondence.

2022 Tesla Model Y Long Range AWD VIN: [XXX] Current mileage: 71,967 SAFETY DEFECT: Power Conversion System (PCS) internal hardware failure causes sudden, complete loss of Supercharging capability and extreme vampire drain. Tesla technician’s own words – invoice #[XXX] dated 11/04/2025: “The problem was caused by an internal hardware condition in the Power Conversion System.” This is a manufacturing defect – NOT wear/tear. Safety risk: • Stranding hazard – vehicle stalls at 99%/95% on V3 Superchargers. • One documented trip (Indiana → Las Vegas) required 27 Supercharger stops and 14+ hours of charging instead of the normal 10–12 stops. • Vampire drain: 95% → 70% in 4 days parked (all features off) – July 2025. • Battery degradation: 16.4% loss at 72k miles (83.5% health, 268 mi range) – far above fleet average. Tesla knew since 49k miles: • Nov 20, 2024 – full HV circuit integrity check – goodwill $0 • Feb 19, 2025 – Service Manager [XXX] ran diagnostics and wrote “everything checks out great” • 15 service visits, 3 charge-port doors replaced – same root symptom Tesla now demands $2,503.17 out-of-pocket and refuses to release the vehicle until paid – despite admitting the defect in writing. Cybertruck loaner (provided by Tesla) exhibits identical 99% stall 5 times – proving systemic issue. Attachments (will upload): 1. Invoice #[XXX] – “internal hardware condition” 2. Nov 2024 goodwill invoice 3. [XXX] “great” email 4. Vampire drain screenshots 5. Cybertruck loaner 99% stall photos 6. On July 22, 2025, at about 6:30 PM in a western suburb of Illinois, on a hot, humid, sunny evening after my car had sat in direct sun all day, my 2022 Tesla Model Y Long Range (Dual Motor AWD) suffered a total electrical failure. As I walked up, the car unlocked automatically with my phone key as usual. I placed my dog in the back seat, closed that door, and immediately tried to open the driver’s/front door—but it would not open. The phone key failed, and the backup key card at the B-pillar also failed to unlock the car. I then tried resetting and reconnecting my phone key through the Tesla app, but nothing worked. There were no 12-V or low-voltage warnings on the display or app. I could not find a phone number or live help in the Tesla app; my son and I searched YouTube for guidance but could not fix it. My dog was trapped inside in dangerous heat, so I called 911. Police first tried a slim-jim tool without success and then broke the driver’s window to reach in and open the front door, but the rear doors and windows remained inoperative. The dog was in the car for 30 to 45 minutes before the window was broken. Once inside, I found that the front seats would not move forward or backward, none of the windows would not roll down, and the rear doors would not open from inside. It took another 30 to clear enough glass to evacuate my 75-lb dog through the front seats; we both sustained cuts, and I sprained my ankle. The event was traumatic and posed a serious heat-injury risk (~85 °F ambient, interior >110 °F within minutes). Police documented the incident. Tesla Service Center (Naperville IL) later confirmed a 12-V failure as the cause, replaced the 12-V battery with an upgraded model under warranty, and repaired the driver’s window and door trim. Tesla Insurance was notified the next morning but closed the claim and covered repairs under warranty. No warnings appeared until the following morning, when a low-voltage alert appeared in my Tesla app when being serviced.

I saw that the US National Highway Traffic Safety Administration is investigating the Tesla Model Y (2021 and over) due to the electronic door handles becoming inoperative. I am happy and relieved to see this is being looked into since this recently happened to my vehicle a couple of months ago. The rear passenger side door unlocked initially for me to put by [XXX] old into the car seat but when I went to open the driver's side door, the handles would not open. I tried all other door handles and all were locked. Luckily, I had not shut the door next to my child's car seat yet, otherwise it would've locked my child inside the vehicle. I opened a ticket with Tesla and they sent a technician out. He said the low voltage battery needed to be replaced and when this happens all electric door handles will be unresponsive and locked. However, the vehicle never showed any warning or notification leading up to this. He said when the 12V battery fails, the only way to unlock the unresponsive doors is to use the manual release inside the car or jump the 12V battery under the frunk (assuming you have the right jumping cables outside of the car). The Tesla technician said it is a flaw in the Model Y design and he said I would've had to break a window to get my child out if they are not old enough to locate and release the manual door release inside the vehicle. This poses a huge safety risk to parents with young children. I really hope Tesla implements another emergency way to release the doors from outside the vehicle to avoid young children from being locked inside. on July 19, 2025 my 10-year old daughter entered the back seat and shut the door. When I (her mother) went to get into the drivers door the electricity in the car completely shut down locking my daughter in the vehicle. There were no visible icons or labels showing where the emergency/manual latch was. She was hystercial and heating up quickly. We called Tesla roadside assistance and the agent spent 10-15 minutes explaining how to get out of a Model 3 (we had a model Y). He then proceeded to tell us that he did not know how to advise her how to get out. He told us not to break the window and to search it on Google. when we asked for his name he would not provide it and he displayed hesitation and resistance to connect us with his superior. We explained that he was dealing with a potentially fatal situation and he displayed ZERO sense of urgency or compassion for the situation. We were able get her out of the vehicle after researching it online. But Tesla did not provide any guidance to help. Our car was at 80% charge and we did not get any low voltage notification or warning that the auxiliary battery was dead or low. My daughter was dehydrated and is forever traumatized by this situation and could have been seriously harmed. Tesla does not provide a standard vehicle delivery process when you purchase or lease a vehicle from them. They do not provide any tutorial or notification on what to do in the case you lose electrical power. The manual emergency latch is not labelled at all. You would never know it was there unless you search online. We sent an email to Tesla notifying them of the situation and they never responded. We even shared this with the service manager who replaced the battery and he said it is a common occurrence and brushed it off.

After installing the most recent Tesla software update, my vehicle immediately displayed multiple alerts: traction control disabled, stability control disabled, and reduced steering assist. Cameras were disabled, navigation was disabled and my vehicle location was wrong. The vehicle lost core safety systems that are critical to safe driving The car functioned normally before the update. Immediately afterward, these safety systems were disabled. Tesla Service has since diagnosed and confirmed, They state “an internal short within the drive assistance module (computer). The internal short is causing excessive current draw, resulting in loss of driver assistance features. The latest software has revealed the underlying root issue within the car computer.” This indicates that the vehicle was operating with a latent defect that Tesla’s diagnostic update exposed. The update did not cause wear-and-tear failure; it surfaced a hidden safety defect inside Tesla’s own hardware. Why This Is a Safety Concern Loss of traction control, stability control, and steering assist creates a significant risk of loss of vehicle control. If this had occurred while driving at speed, the sudden loss of these systems could have led to an accident. Attempting a reboot while driving (Tesla’s standard troubleshooting suggestion) would have created further distraction and risk. luckily when the car catastrophically failed and was unable to turn, I was in my driveway and not on a highway or any other roads.

On April 18th, 2025, I dropped off my 2022 Tesla Model Y at Tesla Collision Center (Paramus, NJ) after a minor accident. I later revoked consent for any repairs via email. Despite this, Tesla performed major body and paint work without my permission. No signed work order exists. The repairs were unauthorized and proceeded without my knowledge or approval until this day. These changes now risk altering my VIN history and vehicle safety records. I’m concerned this constitutes falsified documentation and could affect resale value, title accuracy, and liability in future accidents. I was never shown a repair summary or given the chance to approve or decline service. Even after I sent a formal cease-and-desist letter on June 6, 2025, Tesla towed my rental vehicle on June 4 (without warning), which I had been using through Tesla Insurance. My personal belongings were inside. The timing suggests retaliation for revoking consent and demanding documentation. There were no mechanical failures or warning lights before this, but my concern is that Tesla falsified records & carried out structural work under my VIN without my legal consent. That poses serious risk to my safety and consumer transparency. The work was done by Tesla’s own collision center. The vehicle is available for inspection. I request NHTSA to review Tesla’s repair authorization practices and investigate whether unauthorized repairs, falsified VIN records, or retaliation have occurred in my case and others. This incident reflects broader safety concerns around Tesla’s internal repair authorization system, which may allow collision centers to proceed without documented customer consent. If vehicle repairs are being logged under owner records without approval, this poses risk to accident liability, insurance disputes, and NHTSA transparency. I request a formal investigation into Tesla’s repair authorization practices and how they affect vehicle safety, ownership rights, and regulatory compliance. Thank you.

The low voltage battery went out after rear right door was opened to put children in car seats. Drove 45 minutes to a train museum with the kids. Everything was working fine on the way down. Once done with the museum and back at the car, opened right rear door to put my younger child in the car seat. My older child decided to enter the car through same door as she didn't want to wait for me to finish to take her to the other side. Once my younger child was buckled up, I closed the door to come over to the other side the buckle in my older child into car seat. The door would not open. The phone application would not get the car to unlock the doors. I could not open any of the doors. Children were trapped inside. My oldest child was not yet buckled and I was able to instruct her to crawl to the front and try opening both doors with the electronic button, which did not work. I then instructed her to use the mechanical door latch vs the electronic and she got the door open. Had it not been for the fact that she was still unbuckled she would not have been able to get to the front to open the doors. I would have needed to call 911 for help getting the kids out as the car was unresponsive.

Our 2022 Tesla Model Y Performance backed out of a driveway into the street and parked in the middle of the street with nobody inside the car, creating a traffic hazard. This happened after around an hour parked. The Smart Summon feature was enabled in the car. Possible cause was that Summon was accidentally activated by phone jostling in pants pocket. Car was looked at by Tesla and found to be in good working order and was considered safe to drive. No explanation of cause of the event was offered by Tesla. We turned off the smart summon feature in the car. Suggest adding an additional step in the Tesla app such as requiring a numerical code input to eliminate the possibility of accidentally summoning the car by random inputs to Tesla app when phone is in a pocket. We have time stamped surveillance video of the event. [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

The contact owns a 2022 Tesla Model Y. The contact stated that while driving at various speeds with the Full Self-Driving (FSD) activated, the destination was entered, and the system announced the direction steps when suddenly the Full Self-Driving (FSD) disengaged independently causing the driver to be required to take control of the vehicle. During the independent disengagement of the Full Self-Driving (FSD), the vehicle abruptly decelerated. The contact was concerned that the failure could result in a crash if a rear vehicle was tailgating. In addition, the contact stated that the Full Self-Driving (FSD) independently disengaging occurred during excessive sun exposure. On a separate occasion, the contact stated that the Full Self-Driving (FSD) was engaged and driving at various speeds, and the vehicle independently maneuvered into oncoming traffic on a 4-lane road causing the contact to have to take over control of the vehicle. A crash was avoided; however, the contact was concerned that if there had been a median barrier it could have resulted in a crash. In addition, the contact stated there was a second occasion where the vehicle was in Full Self-Driving (FSD) and veered into oncoming traffic, and manual control of the vehicle was required. The failure was recorded via the Tesla App and the data was entered via AI. A dealer was contacted, and a service appointment was scheduled for a diagnostic test. The Tesla Service Advisor informed the contact that moisture entered the camera and the windshield causing the failure and it was a known issue, but only an interim repair was currently available. The vehicle was not diagnosed or repaired. The manufacturer was notified of the failure. The failure mileage was approximately 30,000.

I took Tesla to arbitration over defects and deceptive practices and their legal representative committed fraud by playing my own dashcam video and muting it so you couldn’t hear the auditory warnings the car makes when it makes errors. In this case the car was driving itself into oncoming traffic and stopping on the highway for non existent stop signs. I have reported this to the Florida Bar and Florida State Attorney Office Ashley Moody who is now investigating.

What component or system failed or malfunctioned, and is it available for inspection upon request: My drivere side door fails to open every time. The cold weather feature in the tesla app claims to pop open the door so you can grab the door to fully open only to get stuck closed. This also happens when attempting to exit the tesla. The passenger side drivers door also does not open every time. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? Has been in for service many times. Concern does not replicate except at time of pickup of vehicle. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? Yes failure to open door. Also if you are in full self driving and come to a emergency stop: The vehicle is suppost to open the door and put the vehicle in park. The vehicle only puts it into park and have to press the button multiple times to open the door. It gets stuck.

Charge port didn't allow to latch the charge cable. So the car lost the ability to be charged. This failure first time happened in December 2023. A mobile service technician visited my home to look into it. He checked and was able to restore the working of latch again. He didn't replace any part. This problem again happened on a very hot day (above 43 degree C) on 4th July when me and my family were away from home and were routed to a fast charger which was more than 70 miles from my home. The battery at that time was showing only 35 miles range left. I was forced to drop my car in nearest service center which was closed due to 4th July. Me and my Family were stranded on the road in harsh temperature conditions. Some how I reached back home by alternate means. It took more than 5 days Service center touched the car and fixed the car. Per there report they replaced the charge port on my car. The problem happened again on July 29 but got resolved on its own. I visited Service center again on 15 August and was give a detailed form to fill up next time the problem happens. I was told that they will do some further diagnosis but they didn't even touched the car as the problem was not showing at that time. I have strong feeling that this problem can happen again and may leave me again stranded on road without any support. I have lost confidence to drive this car for longer distance where I may need to charge on the way. The issue happens without any warning and will show up right at the time when I have to charge it. I feel that this issue may leave me in a unsafe situation if I am at a remote place where the car looses the ability to be charged. I am not sure if this is the right forum to bring up this issue. Please feel free to direct me if this is not the right forum.

I just got T-Boned in my 2022 Model Y performance. I got out of the vehicle which wasn't in terrible condition, in fact the left side of the vehicle was still perfect and the doors worked fine. I had my dog with me and everyone was okay, but I opened my door and the back passenger door to grab a jacket and check on my dog. I shut the doors to make sure he didn't run out and was a bit out of it because of the accident impact so I took a moment to compose myself.. All of the car's lights flickered and then the electric cut off leaving the doors completely locked and inoperable even though they had just been used moments before and were not damaged in the accident. I had to climb in through a shattered glass window to get my dog as all the doors wouldn't open from the outside and I was unable to access the glove box, nor pry it open because the electricity was out and the impact caused that door to make the side panel to unlock the glove box manually accessible. If the car DID catch fire or an occupant were bleeding out: Anyone inside, unconscious, would be unable to use the manually release and would be burned alive while people pull on doors that won't open that were undamaged in the accident and working 60 seconds before. From what I have read the only solution would be to jump the car using the 12V battery in the Frunk of which the Tow company and Emergency Responders were not familiar with.

A Tesla Model Y has one screen which is all instruments, warning indicators and reverse camera. On March 3rd, 2024 I had just completed about a 40 mile drive. I parked the vehicle for approximately three minutes and upon returning to the vehicle the screen failed to turn on. The vehicle ran and drove perfectly normally, but I had zero instrumentation or warning indicators. I drove home about two miles this way without incident. Upon exiting the car and closing the door (which is the equivalent to turning off the key in a traditional vehicle), the screen turned back on as normal and has since not repeated this issue. I contacted Tesla service about it and even sent them a video showing the vehicle operating with the screen not functioning and I was told to just reboot the car and they brushed it off and would not see the vehicle over this incident.

Component or System Failure: The internal malfunction of the rear drive unit in my 2022 Tesla Model Y, which has been documented and addressed on two separate occasions by the Tesla service center, has raised significant safety concerns. This component is critical for the vehicle's operation, and its failure mode necessitates thorough inspection and a definitive resolution to prevent recurrence. Safety Risks: The sudden loss of power on a busy highway, without any warning, significantly compromised my safety and the safety of others around me. Being forced to restart the vehicle under such conditions was not only distressing but also exposed everyone involved to a heightened risk of accidents. This incident has profoundly impacted my confidence in the vehicle's reliability and safety. Problem Reproduction and Confirmation: The malfunction was initially confirmed and addressed by a Tesla dealership, which underscores the seriousness of the issue. However, the recurrence of the problem, even after professional intervention, and the subsequent attempt to rectify the issue without a clear diagnosis, adds to the uncertainty and concern regarding the vehicle's safety and the effectiveness of the repairs conducted. Inspections: The direct involvement of Tesla's service center in inspecting and attempting to repair the vehicle indicates that the manufacturer is aware of and is taking steps to address the issue. Nevertheless, the absence of a definitive resolution or clear understanding of the malfunction's cause continues to be a source of worry. Warning Signs Prior to Failure: The complete absence of any warning signs before such a critical failure is particularly alarming. This lack of forewarning deprived me of the opportunity to take preventive action, further exacerbating the fear and potential danger associated with the malfunction.

Mar 3, 2024 Crash

My vehicle encountered a severe loss of control, resulting in a collision with a concrete wall on I-90 west towards Seattle at 19,877 miles. This critical event, leading to non-deployment of airbags and subsequent hospitalization, occurred shortly after two prior services addressing a Rear Drive Motor unit malfunction, first at 14,768 miles and again at 14,820 miles. These incidents underscore a troubling pattern of safety concerns. Inspection Details: Front Suspension: Replacement of Stabilizer Bar Link (1188391-00-B). Steering Gear: Replacement of Steering Gear Assembly (1044831-00-H), essential for control. Sub-Frame & Steering Components: Reinstallation of the sub-frame and replacement of the tie rod end (1044841-00-E), indicating significant structural and steering issues. Safety Risks: The sequence of malfunctions, particularly the recurring issues with the Rear Drive Motor unit followed by a severe loss of control and airbag system failure, presents an extreme safety risk. The absence of warnings prior to these failures compounds the danger, suggesting systemic safety and reliability flaws. Conclusions & Reporting: The vehicle's history of critical malfunctions within a short mileage interval demands an in-depth examination by the manufacturer and safety regulatory authorities. The objective is to uncover root causes, evaluate if these issues are widespread among similar models, and implement corrective actions promptly. This report aims to highlight the critical nature of the encountered safety issues, advocating for immediate interventions to safeguard vehicle occupants and the public.

Tesla does not provide power to the Aux pin on the standard 7 pin connector for towing. This means that the vehicle will not keep a 12V battery on a trailer charged up at all times. In case of a breakaway situation, there will be no power to the brakes and thus the trailer will not stop safely. Tesla changed to a 16V battery and has refused to step down the voltage for towing purposes. This is extremely dangerous when they advertise the Model Y and X as towing up to 3500 and 5000 lbs respectively and do not advertise the fact (outside the manual, where it must be inferred) that there is no power on the Aux pin. The 7 pin connector is a standard and Tesla is not adhering to it.

Dec 11, 2023 Fire

The contact owned a 2022 Tesla Model Y. The contact stated while driving 60-70 MPH, she noticed a burning smell. There was no warning light illuminated. The contact then noticed sparks coming from the front passenger's side tire area. Another driver alerted the contact of an issue, and the contact immediately veered to the side of the road. The contact then noticed that the sparks coming from the front passenger's side tire area had become significant and had turned into flames. The front end of the vehicle caught on fire. The fire department extinguished the fire; however, a fire department report was not filed. There was no police report filed. The contact sustained minor bruises while exiting the vehicle. Additionally, the contact sustained an emotional trauma and intended to receive therapeutic assistance. The vehicle was towed to a tow lot. The insurance company then towed the vehicle to another tow lot, where it was totaled. The manufacturer was notified of the failure, but no assistance was provided. The failure mileage was 30,028.

I had just parked in a spot 30 feet in front of my clinic building. My foot was not on the brake nor accelerator — the car had stopped and was parked. Tidied my hair and was grabbing my purse placed on the passenger seat, the car seemed to “surge” and suddenly ACCELLERATED QUICKLY straight toward the sliding glass doors toward my medical clinic building! I screamed, dropped everything and stepped as hard as I could on the brake. It stopped suddenly about 5 ft, onto the curb, in front of the doors. No damage (except for the scare!), so no reports. The AAA tow truck driver said he had seen this before in electric cars in his recent 3 months ( 4 out of 5 was Tesla; the other a Volkswagen), and that the black box can’t detect the surge because it’s electrical? Didn’t report it because no damage, no proof. Not looking for compensation, am not hurt (yet), but please look into this. I could’ve mowed down precious babies and children, mothers, doctors in my hospital pediatric clinic. I have heard 2 more incidents from a coworker and a family member (model 3 and S), so although my experts not recent, I cannot with clear conscience not report this. Please, please research this, lives are in your hand. Even if it is just one.

Our High voltage battery died on the side of a highway, stranding us until a tow arrived at 4am. During this time, the onboard hazard lights were activated, and no other vehicle systems besides the standard interior features were active. The low voltage battery died within 35 minutes of the high voltage battery. After the low voltage battery died, we were left on the side of the highway at 4am with no active hazard lights, the emergency vehicle door openers were required to be used, however, the windows were rendered unusable, so the doors could not be effectively shut without risking damage to the windows. Having zero hazard or external lighting emitting from the vehicle after just 35 minutes at 4am on a dark stretch of highway put us in an extremely dangerous situation, and in our opinion, the hazards should have functioned for at least 2-3 hours. The vehicle has since been repurchased from the manufacturer as a lemon due to other unrelated issues.

Tesla Incident 9/8/2023 My Tesla Y started moving backwards suddenly while I was driving forward after I accidentally flicked the right drive lever, mistaking it rain wipers. I driving on Henry Hudson parkway in NYC. It was back to back traffic on HHP. The Tesla started racing backward despite my foot off the gas, and I barely had time to brake . Luckily, traffic was very slow due to rain and carbehind had stopped and it was not damaged at all. However, my Tesla trunk was damaged. I am very worried that the car could go from moving forward to backward with accidental flick on the drive lever

Yesterday I drove 1 shift on UberX in chill mode, very carefully and tried to save as much energy as possible. Here’s what happened. I left the house with 100% charged 274 miles range not 303 advertised I track everywhere I go with Everlance GPS. After a couple rides, I noticed the cars augmented reality for auto pilot wasn’t seeing the environment properly and like usual the safety features were disabled, but I am unable to record it responsibly while driving my passenger on UberX not Uber black. When I got to a red light with no passenger, I recorded on my iPhone the Tesla failing to see about 35 to 40 vehicles over the course of 45 seconds and also fails to see a black man walking slowly across the crosswalk just 2 feet in front of the car but it can see cars 100 feet away. Meaning there is an intentional Blindspot that would be in the collision warning zone that the car is purposely not seeing because the car is hacked. Directly after that I picked up a man from his house who also has a model Y and brought him to the airport. He’s an augmented reality engineer that looks familiar to me from Tesla. Turns out someone stole his brand new model Y from tampa Tesla, and brought it to a chop shop and went on a joyride, destroying the alignment, which he was given a large check for. He said Tesla was able to provide all the problems with his car with no issue. I told him all the problems with my car. Tesla has refused to even acknowledge after a dozen service appointment. Then I told him I have 100,000 GB of dash cam footage and secret recordings of the service manager telling me if I submit the footage to the FBI they would refuse service. Well, they already are refusing service. Also I drive a lot of federal agents and military personnel. When I got home, the car couldn’t account for 70 missing miles and it’s charging at one quarter speed. I reset the car three times. Now I’m trying to submit this on my computer, but the Internet cut out! BUSTED! Shut down Tesla!

A copy of this report is being submitted to the FBI, CIA, DOJ, my attorney and then I'm mailing it via USPS to my Attorney General [XXX] which responded to me about this car about a year ago saying she would investigate it. Yesterday my wife and I drove to Chevy and put a deposit on a new gasoline SUV. On the way back all the safety features disabled themselves and the car radio played "Break the Rules". Today I drove to Wawa to report my wife almost dying from being poisoned and on the way back the car in autopilot mode sped up towards an officer with his lights on and high visibility vest so I had to slam the brakes and move over at the last second and the Tesla didn't see the officer until he was 10 feet away and wrongly displayed the cops passenger door being open. I would NEVER drive towards an officer like that if I was in control of the car that's the faulty software. On my way back from Ford to the Mister Car Wash in Eustis to get the love bugs off the paint again I took the Tesla cam thumbdrive out, took out the micro sd from my dashcam, turned off my iPhone with the Tesla app on it and used autopilot to drive the scenic route to the carwash to avoid traffic. Then the car started playing the songs "Middle of the Ocean" which lyrics encourage you to drive your car into the ocean; then started playing "Swim Good" and then "Golddigger by Kanye West" the car disabled all its own safety features despite them still being turned on in the settings which would have allowed me to drive my Tesla off a CLIFF into LAKE EUSTIS just like that brain surgeon in California when his white Model Y drove him and his family off a cliff luckily they survived. I uploaded the video to my IG [XXX]which is currently banned for "using like increasing software" which is funny because I'm the only one that likes my posts but now I cant even like my own post. I have video evidence and screen recordings of everything I just said. As I type this the computer keeps stoppin INFORMATION Redacted PURSUANT TO THE FREEDOM OF INFORMATION ACT

Went on a 3,000 miles road trip and recorded countless bugs and safety issues. To keep this short and sweet because I’m pretty sure I’ve already broken the world record for DOT reports on a car without any intervention here’s some bullet points. 1. Car is falling extremely short of claimed mileage range. 2. Car refuses to charge or charges slow. 3. GPS is constantly wrong and sending me down dirt roads and trying to put me miles out of my way by giving wrong directions on purpose. 4. Car cannot switch lanes in Autopilot despite perfect visibility during night and day. 5. Car keeps slamming on the brakes with people behind me or in my blind spot for no apparent reason. 6. Car keeps telling me that charging stations are offline or unavailable and when I pull up it’s a bunch of lies and there are open spots and the car charges. 7. Whenever I pull up to a charger it ALWAYS happens to be the highest energy cost of the day. 8. Car sends me to wrong charging locations despite me selecting certain charging stations manually. 9. Car keeps making an awful scratching sound when music is paused or song changed. 10. Car keeps banning me from full self driving because the autopilot isn’t working properly even though I disabled FSD. 11. Car is constantly putting off a ton of heat to the point it raises my garage to over 100F. 12. Cars overheat protection mode doesn’t work properly and never has. 13. Car summon can’t make it 4 feet without side swiping the car next to it in the parking lot. 14. Car fails to see all emergency vehicles with or without lights on like a ghost car that doesn’t exist. 15. AC is inaccurate and is either not reaching temp and the AC is roaring so loud I can’t hear anything but the air moving or is so cold myself and passengers start shivering. 16. Charging takes twice as long as advertised and never charges advertised speed like 150-250kwh 17. Full self driving can’t drive me more than 2 minutes without trying to kill me so I disabled it.

Drove to the carwash today a couple miles away to see if its just emergency vehicles the car doesn't see but its not seeing any vehicles in the right lane or passenger side of the car until they are about 150ft ahead of me. To clarify the cars vision system is NOT seeing ANY emergency vehicles (driver side, passenger side, ahead, behind, moving away, moving towards) like they don't even exist (invisible) and ALSO not seeing ANY vehicles approaching from the passenger side blind spot. Switched my IG to private, was tired of all the hecklers and people reporting me and NONE of the government agencies doing ANY of their responsibilities except trying to screw me over on made up charges LOL. I'll DM it to you on the NHTSA IG page.

Tesla took my car in for service again, fixed nothing again. This time they messed up my dashcam even though it is clearly marked with bright red NOTICE stickers on both sides of the car that consent for audio and video recording is given by entering vehicle that is owned by me (not a lease). Car wouldn't charge yesterday, then I do the newest software update and when I get 3 miles down the street from my house I yield for an EMS EMT with siren and lights on in the middle of the day with perfect visibility and big surprise the car doesn't even register that I've not only left the lanes without lane keeping assistance but it also doesnt see the emergency vehicle so I have disabled FSD and am refusing to use AP because this is a death trap. When I complained to the FBI about it they tried to have the local PD basically SWAT my house at 4am and wake up my whole family. My wife and I are both FBI witnesses. I also received a letter from the IRS claiming they changed my taxes from 2020 (filed by my accountant) claiming that I owe $132,021 (On less than $800 in gains) by next month or they will charge me $20,000 a month interest. Today I received a notice from my insurance company of 1 year claiming they are canceling my coverage because I "drive UBER Black" which isn't true I never have because the Orlando PD wouldn't approve me to because the insurance agent lied about the coverage because Ana (Moore resources Insurance) and Mercury Insurance; was too drunk on New Years Day and has caused me to lose $30,000+ in lost revenue because she ruined my 4 month wait for UBER black approval which has completely destroyed my financial ability to provide for my family. I received a call from a DOT agent a couple weeks ago from Washington DC but the call was dropped so he called me back on a cell and we haven't been able to contact each other since. I am REALLY in fear for my life. I am an FBI whistleblower and POTUS is retaliating against me and anyone who helps us. SOS SEND HELP!

Made it less than a mile from my house this time when the Tesla couldn't see the FHP (Florida Highway Patrol) with lights on and perfect visibility. The car was updated yesterday but it has NEVER actually worked as advertised and the more I complain the more everywhere I go gets hacked. My wife Red Cross servers, hacked. My medical files, hacked. The pizza shop I was driving to, hacked, my grill, hacked, my alarm system, hacked. The lawn equipment store, hacked. My iPhone, hacked. My Tesla, Hacked. My insurance, hacked. My Uber, hacked. My Lyft, hacked. I am currently obtaining my Security+ cybersecurity certification so I can keep my family ALIVE. I am failing and being SMOTHERED! Tesla REFUSES to cooperate despite my countless attempts to be civil, involve the FBI, Local Sheriff, Local PD, IRS, CISA, my State Attorney [XXX] and Governor [XXX] and even the AIRFORCE with .mil emails are having trouble contacting me/my family. I applied to GS13 TS clearance under Spaceforce DoD was interviewed and told I was one of the final selections and ever since Biden took office I have been in a Hunger Games type survival nightmare where I am continually hacked, poisoned, banned and left for dead. May 28, 2023

Hello, me again. Been having ALOT of trouble with voice text when trying to respond to my wife at the stop light. I started telling her about how a white Cadillac escalade almost crashed into me and all the safety features 'randomly' disabled themselves while driving and I'm late because I tried to keep eyes on the car while the local PD dispatched and had to drive further than I intended but I eventually gave up after he got so far ahead after weaving thru traffic like he's playing grand theft auto. So the voice text has been doing this really 'cool' feature where I say some stuff that's incriminating about tesla or the FBI and then it just doesn't type it at all or says some stuff I never actually said that usually sounds incriminating like this one time I tried to voice text my wife that some guy tried to crash into me and then sped away at 100mph plus and later I caught up to him because he crashed at a toll in the rain; the car said I was racing a guy and I crashed! Then my wife got all freaked out and called me and I explained that I'm totally fine and wasn't racing him he raced PAST me. This car is scary as hell, it should have its own horror movie. Oh yea I just got a letter from the IRS for the third time saying they changed my 2020 tax returns and I owe $138,021 in back taxes by next month or I'll get $20,000 a month in late fees but I only made $729 that year and already paid my taxes thru my accountant. FBI tried to raid my house 2 nights ago AGAIN using localPD. All after Tesla refused my lemon law surrender and they claimed nothing is wrong with my car and tampered with my evidence camera despite them consenting to being recorded by entering my vehicle. Car is living in the garage now until DOT Agent finishes setting up our meeting because I really don't want to die that would suck. Here's a link to my shadow banned Instagram that has hundreds of videos of my car trying to kill me. https://www.instagram.com/reel/CsxNKLNRh-3/?igshid=MzRlODBiNWFlZA%3D%3D

Tesla refused my Lemon Law surrender and claims there's nothing wrong with the car once again, big surprise! They messed up my dashcam during fake service regardless of legally consenting to being video and audio recorded (VERY clear NOTICE stickers on the doors) when entering the vehicle which I own; it is NOT leased. Consent is important, without consent things become crimes. Like if a flight attendant says no to sex with her boss Elon you cant just rape her and then offer her a horse after and get away with it. Anyways the automatic indicators that Elon suggested everyone try on Twitter the other night doesn't actually work for automatically turning on or off with or without AP and FSD. I decided to turn off my FSD that I paid $12,000 because its a death trap. Here's a video since Tesla says I'm imagining it and I must be schizophrenic or something but this is exactly why I record everything and trust nobody. The link probably doesn't work because the government is trying to silence me in this huge coverup. https://www.instagram.com/reel/CsxNyMLsFkK/?igshid=MzRlODBiNWFlZA%3D%3D

Hello it's me for the 500th time. I was sitting at a light when a new song came on that's actually a movie clip from Pulp Fiction directed by [XXX] about hitmen that kill people that speak out, like whistleblowers; for their boss. So I'm sitting at the light and the cars behind me are all glitching between 3 lanes but nobody is moving and my volume is up and it starts 'yelling' "STORING DEAD [XXX] AINT MY [XXX] BUSINESS!". This was soon after almost being ran off the road by a white Cadillac escalade that I called in and reported to the local PD. I was rear ended by a white Cadillac escalade in 2019, I almost died and received death threats while leaving a competitors business. and I'm permanently partially handicapped at 31 years old. About 1 mile down the road my car started encountering 'ghost cars' that show up on the GUI of the screen but aren't actually there in real life lol. Just so I'm clear the car is basically threatening me after Tesla says there's nothing wrong with it so I disabled my $12,000 FSD and will be keeping this car in the garage from now on. Here's the link to my shadow banned Instagram that holds all the videos of this car trying to kill me because the government wants to cover it up. The FBI actually tried to SWAT me in the middle of the night for the second time in 3 years 2 nights ago but luckily the Sargent was cool and a desert storm veteran so he knows what's up. Truly terrifying! https://www.instagram.com/reel/CsxNKLNRh-3/?igshid=MzRlODBiNWFlZA%3D%3D INFORMATION Redacted PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6).

WHY I AM SUBMITTING THIS COMPLAINT: I am writing to you upon the advice of the CA Attorney General's office. Their guidance was in response to a question I submitted on their web about safe and legal towing with the Model Y factory-installed towing package. PROBLEM: Risk of silent brake controller failure. VEHICLES IMPACTED Model Y's with the Lithium Ion low voltage battery & towing trailers 1500 lbs or more. TECHNICAL ISSUES: The lithium Ion low-voltage battery operates at 15.5v, out of the normal 12v operating range. The wiring harness sends 15.5 v to the brake controller. Brake controllers connected to the Model Y towing harness do not throw an error. High voltage through a 12 v brake controller may result in unexpected brake controller failure per controller manufacturer's support teams. Tesla disconnected the 12v aux power to the 7-pin towing connector. This power is used for RF trailer brake controllers. RF brake controllers don't get the required power. RF Brake controllers will throw an error & the driver knows the brake system isn't connected correctly. WHY DO I KNOW THIS? I bought the Model Y to tow my 2,000 lb teardrop trailer w/ a Prodigy/Tekonsha RF Brake controller. When I attempted to connect & verify the rig was in spec, I discovered there was no power to the 7-pin plug. While troubleshooting, I found a Twitter post that described the problem, said Tesla disconnected the power & there would be a retrofit in mid-2022. See attached. ATTEMPTED RESOLUTION I've worked with Tesla Rocklin CA for a year. They submitted cases, escalated, and even unsuccessfully tried to connect a 24v controller. A fix isn't hard. I've heard corp. didn't expect people to tow. COMMUNICATION: To my knowledge, there's been no communication to affected owners or to brake controller manufacturers. NO FIX PLANNED Tesla Corp. doesn't intend to fix it or communicate to owners. See attached. ACTION NEEDED People towing may be at risk of silent brake controller failure.

The USB charging plug inside the middle console burned my charger causing smoke inside the cabin. Tesla has rescheduled the appt once already and doesn't seem to be a concern to them. This is a huge safety concern as it could have caused a fire if items were near this USB plug.

Leaving Mothers day dinner at Disney Orlando with all 3 generations of family in the car and I click 'Home' and the map sets a charging station in Miami 120miles away in the wrong direction, the FSD and AP wont work and the cars gps goes completely black. That doesn't even make sense, there was the Hooter's supercharger down the street and several others in Miami and probably a dozen more between Disney and Miami. If I wasn't a professional driver I fear this many bugs and susceptibilities could kill my entire lineage. Texting and driving is illegal and dangerous, that's got nothing on a malfunctioning Tesla screen while driving main street Disney. Look out [XXX] there's a new kid on the block, [XXX] doesn't need a tunnel to disappear your whole family. He can do it in broad daylight on mainstreet Disney in front of the police, God and the NHTSA and after you report it 30 times nobody will do anything about it. Almost everyday I have to restart the car while I'm driving because the car starts playing songs that are mean, discriminatory, or out right telling me to "Kill yourself on the beach" and "You can't sit here". While I'm driving I'm forced to restart the car to regain control of my instrument cluster which when resetting while driving you basically lose all features of the car minus steering gas and brake. The sentry mode isn't recording, the indicators aren't working, there is no speedometer or navigation, all safety features are disabled. It's the perfect time to commit a crime, no evidence except my iphone dashcam and anyone else around. I have to sometime do this with children in the car because the radio keeps cursing and I can't change the song. This car is a haunted death trap and I have entered pre-arbitration with Tesla but they have not responded. I have a lawfirm on standby that has offices in FL, CA and TX. They want my 10,000 gigabytes of EVIDENCE bad unlike NHTSA. That's because they are fascists and trying to cover this up. He also bought Twitter as part of this coverup. I talked about it in my podcast about 3 years ago that's still on YouTube. This is all premeditated torture and attempted murder. Since I received this awful vehicle it has never properly identified emergency vehicles. Sometimes it confuses neon/led signs with emergency lights and smashes on the brake for emergency vehicles that aren't there. Other times it slams on the brakes on the highway when behind an emergency vehicle and the reflective tape is lit up from my own headlights bouncing on the awfully hard and cheap suspension on this 'performance' vehicle. Lately I've captured video of the car completely failing to see a FIRE TRUCK right in front of it for at least 1 minute straight with the lights and sirens on. This is unacceptable. Tesla not only wants me and my family dead, they don't care about the lives of our brave firefighters, EMTs and police officers. Then don't care about YOU either. Shut it down this is beyond criminal! Shortly after this the car radio played "Kill yourself on a Beach" on my daily trip to McDonalds because it's all I can afford while slowly going bankrupt driving this Tesla on Uber because of all the 'defects' and false advertising if you can even call it anything other than intentional punishment for whistleblowing at this point. I started video taping after like the 20th time because my dashcam cant show you the screen also. I did this strictly to display that the Tesla failed to identify a State Trooper with his lights on for several hundred feet with 'Full Self Driving' enabled. Just to be clear the car is on the current software update and has ALWAYS been up to date. Tesla is once again lying about the cars capabilities and putting innocent Police officers and random civilians at risk of serious bodily injury or death. Once again I caught on video my car trying to suicide me (a Jeffrey Epstein (not a typo I'm in fear for my life) witness) by turning off all safety features and attempting to drive me directly into oncoming traffic. Tesla has REFUSED to repair or replace my car. They have done EVERYTHING they can to prevent me from getting service for months. Reschedule after reschedule, 'we cant reproduce the problem' and 'the car is fixed' after recording them doing NOTHING on hidden camera for 3 days besides unplugging my dashcam after I told them on my IG that I was recording them and they agreed because its clearly posted on the door used to enter and unplug the camera. This is racketeering, tampering with a witness and attempted murder not to mention conspiracy to defraud the US Government with false advertisements to convince blind and corrupt politicians to give Elon ANOTHER round of government subsidies even though co2 isn't the problem, METHANE is! As you can see in the video the cars path is stuck in the ":Kill yourself with oncoming traffic" mode. Its a feature that is activated on my Tesla way more than I have video evidence like this to prove it. Here's the video: https://www.instagram.com/reel/CrzUMD4xilo/?igshid=MzRlODBiNWFlZA%3D%3D

After the car couldn't drive me into oncoming traffic and put me out of my misery; I reset the car and then all the safety features were disabled again, the car couldn't see the solid white freshly painted lanes during the middle of the day with perfect visibility. As I approached the red light I demonstrated how the Tesla has no collision warning or assistance enabled although they are features advertised on the car that I paid for. Once again not a coincidence. Tesla has refused or is incapable of repairing this Frankenstein's monster covid Tesla it needs to be Totaled under lemon law. All the windows are from different countries FFS. Also the doors are sagging and scratching the base of the car but Tesla is also completely blind on that one too, go figure! They've also illegally tracked me home and to the grocery store but that's just what I can prove, I'm sure they've broken more privacy laws than just that. From a simple Google search you can discover how Tesla employees use the car footage to spy on you in your own home and pass around nude photos of you on internal company email and messenger apps. Less than 2 minutes into my drive the car decided to disable lane keep assistance and warnings for no reason. As you can see in the photo linked below the vehicles in the oncoming lane are lagging and look kind of like a ghost car while the rest of the screen is totally fine. Well all I did was switch from the middle lane to the left lane like I do everyday with my indicator on, on my way to McDonalds. Absolutely no reason why this happened like I mentioned I had my indicator on nobody was around me and visibility is perfect. Apr 15, 2023 Crash Fire

Same fatal error I've been reporting for over 6 months and about 10 "OVER THE AIR UPDATES" Yes we all really 'NEEDED' more ways to make fart and poop sounds and open our rainbow LED [XXX] by voice command BUT WHAT ID REALLY LIKE IS FOR LITERALLY ANYONE IN ALL OF AMERICA TO DO THEIR JOB! Maybe if someone would do an investigation into to how much FRAUD there is at literally every level of Tesla from the very top all the way to the pathetic worms at the bottom that somehow find the one vertebrae they have left to continue lying to protect the bank account of a lying megalomaniac. Maybe they would investigate how Tesla vehicles can be used as remote operated drones to kamakazi themselves and kill anyone in the vehicle you don't like or even worse they could hack it thru the radio and use it to crash into literally anyone on the road as described by [XXX] himself and the FBI when asked about drone technology. I mean if [XXX] is allowed to spy on all of planet earth without their consent, freeze their car radio, miss every auto payment for 8 months, keep your car in an overflow lot for 3 days on hidden camera and never work on it once 5 times, make your car stop on the highway 50 time, stop your car from charging at home with your own charger in your own house then why does anyone follow any rules? Why don't we all just go out in the street and rob and kill and rape anyone whos dumb enough to believe that a car can drive itself? Really tho why do I even bother doing any of this everyone on the internet just calls me a stupid, fat, [XXX], [XXX], [XXX] fan,[XXX]. So like why would you EVER buy a Tesla. It's made by SLAVES FOR F SAKE! I'm a slave for Uber, Tesla and Apple. Slavery is real. I lost $28,000 last year doing 1,000 Uber and Lyft rides in an electric car that's supposed to save money on gas but actually costs more than gas if you use a supercharger. Worst purchase in my entire life by far. https://www.instagram.com/reel/Cq-10appVGM/?igshid=YmMyMTA2M2Y%3D INFORMATION Redacted PURSUANT TO THE FREEDOM OF INFORMAT

In a 1 minute clip I captured how the Tesla constantly misses collision warnings and/or assistance while all safety features are turned on because the car misses the first collision warning and then goes off early on the second scenario with the same car and driver in front of my passenger and I. It's dangerously inconsistent and this is the third passenger on Lyft this week this has happened to and the passenger verified as a witness to the event in each example without any sort of coercion. Without the LiDAR the Tesla makes constant mis-judgements it never should have been removed. Last night I traveled down 429 West and had the same issue with the car reducing to 0mph like it has been for several months but this time I didn't let the car slow down all the way before canceling and leaving Tesla a nice explanation in my best Forrest Gump impression soundtrack was playing. I said "Life is like Tesla FSD, you never know if your cars gonna stop on the highway". What's the point in the voice recording if the error isn't corrected? To make me FEEL like something is being done? Everyone in person and online keeps telling me to just sell the car. Well I would if I could afford to but now I'm stuck between paying my overpriced car bill or driving the overpriced car that keeps failing at its job and almost causing tons of accidents. Thank God I have a dashcam otherwise I bet Tesla would say I'm drunk or something to cover their own butts! Hate to break it to ya but I don't drink that's all just bad programming and sensors. You can see once again the person behind us has to switch lanes last second to avoid rear ending us. I wonder all day every day for the last year or so if anyone actually reads these countless reports I've filed or if they're just another corrupt official on the Elon Musk payroll lol. https://drive.google.com/file/d/18br6n1pK0x3sViSs-q8_aQDvj26n3JHO/view?usp=share_link https://drive.google.com/file/d/1NVhSTKwR6BaQ7gQV8Q6CjzYHpau2zvBs/view?usp=share_link

Here's front and rear with audio of the Tesla failing to trigger a collision warning or assistance with passengers in the car during a Lyft ride. There should be a loud beep paired with some braking assistance but most of the time neither work or they work when they shouldn't. If my car actually got the miles range I paid for and actually charged as fast as they advertise I wouldn't be at home filling out these reports. The contact owns a 2022 Tesla Model Y. The contact stated while driving approximately 70 MPH with the Autonomous Self Driving feature activated, while attempting to make a lane change, the vehicle was independently self-correcting back into the original driving lane. Additionally, while driving with Autonomous Self Driving feature activated, the vehicle experienced phantom braking and the vehicle stopped in the middle of the highway. The contact stated that there were no obstacles in front of the vehicle to cause the vehicle to experience the phantom braking. The contact also stated that the "Emergency Lane Departure Avoidance" failed to function correctly and would overcompensate when the vehicle made slight lane adjustments. The contact stated no warning light was illuminated. The contact had taken the vehicle to a local dealer where it was diagnosed and determined that the Autonomous Self Driving and Emergency Lane Departure Avoidance features be deactivated. The vehicle was not repaired. The manufacturer had not been informed of the failure. The failure mileage was approximately 6,900.

I saw [XXX] on Twitter recommending a woman put her children or dogs inside his cars with overheat protection on however I have been recording and reporting this fatal error for several months and they have NOT corrected it. The car has overheat set at 100F I routinely screenshot the car reaching 130F (assuming the cars thermometer is even accurate) this is hot enough to kill basically ANY living thing. My baby, dead. Dog, dead. It was only 82F outside so I'm going to call BS on the 'tempered glass' that 'repels over 90% heat' the car is a fishbowl/greenhouse. So if my car doesn't kill me, my family and those around me 'Full Self Driving' it will kill your kids and pets while you run errands because you trusted a melomaniac. And if that doesn't kill you everyone who hates [XXX] and Tesla will routinely bully you on the road too because apparently there isn't a single person in ALL of the entire USA government who isn't a total puppy for [XXX]. Not to mention Tesla just awarded some hackers $350,000 and a new car for hacking a model 3's radio to take over the car when I've been reporting this exact exploit to NHTSA and FBI for months. Many times my car has played insulting music and not allowed me to change it, the car often driving with a mind of its own towards fatal crashes that would kill me if I didn't disengage every 2 minutes (that's being generous when not on a highway). "YOU [XXX] [XXX] I AINT [XXX] WITH YOU!" "IVE GOT A MILLION OTHER THINGS I'd RATHER [XXX] DO, THAN BE [XXX] WITH YOU; LITTLE [XXX]! I DON'T GIVE A [XXX]!" I have all this on video. Tesla harassed me when I reported it to them. The first photo is this month the second photo is in August last year. Bought a brand new Tesla model Y with 11 miles on it. 1 mile down the road it tells me to “pull over immediately” “reduced power” I was going 45 mph downhill and thankfully not a lot of cars around. I pulled over and it said reduced power, exit and enter vehicle to restore operation. I did and it worked fine. I drove home with no problems for 25 miles. An hour later I was headed somewhere else and stopped at a light, when it was green I accelerated then it did the same thing but I was on an entrance ramp to a bridge and 5 cars were behind me. The car then was in reduced power mode and told me to do the same thing again, exit then re-enter. It was busy traffic and took me 5 minutes to get out when there were no cars going past me. It reduced power and couldn’t excelerate and caused a dangerous situation. This happened 3 more times while I was on the phone with Tesla support before I had to wait 2.5 hours for a tow with 47 miles on it.

The contact's wife owns a 2022 Tesla Model Y. The contact received notification of NHTSA Campaign Numbers: 22V702000 (Visibility) and 23V085000 (Steering, Electrical System) however, the parts to do the recall repair were not yet available. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repairs. The contact stated while his wife was driving at various speeds with the Full Self-Driving Beta (FSD Beta) activated, the vehicle failed to recognize objects and other vehicles. The vehicle drove into the path of oncoming traffic. The contact's wife corrected the failure by maneuvering the steering wheel to realign the vehicle back into the lane. Additionally, the vehicle was shaking while making turns. The vehicle also experienced phantom braking while driving under an overpass. The failure nearly caused a collision. The dealer was made aware of the failure and confirmed that parts were not yet available. The dealer informed the contact that the vehicle performed as designed. The vehicle was not repaired. The manufacturer was made aware of the failure and confirmed that parts were not yet available. The failure mileage was approximately 1,000. Parts distribution disconnect.

While I was driving on the freeway, the screen/display froze. I was unable to see how fast I was going. I was unable to use my blinkers. Basically just driving with know knowledge of my speed or if people tell if I was changing lanes. Then the screen rebooted as I was driving and everything went back to normal. Just very concerning.

A backup camera is required on all new cars. Tesla has a backup camera system comprising 3 views. When reverse gear is engaged, camera views should appear every time on the instrument display. On the first drive of a given day, the camera views are not properly visible until the car has been driving for a few minutes. Specifically, “night mode” on the infotainment display (effectively the instrument cluster) becomes stuck on dark colors and unreadably low brightness. When reversing during daylight hours, the screen is not bright enough to view the backup cameras. Additionally, audible cues for turn signals are inaudible. After about 1/4 mile of driving, the display “catches up” to day mode and responds with appropriate brightness. I believe the dysfunction of the system is a safety hazard to vehicle occupants, other drivers, and pedestrians. I have previously contacted Tesla service about this issue. I was told that the issue is related to software; nothing can be done; it will be fixed at a later, indeterminate date via software updates. The issue persists consistently and has no fix aside from a workaround.

The vehicle started shaking and stuff wrong from the rear wheels and was told to pull over safely. The error codes Dl_a063 and Dl_a139 popped up and it seems there is a rear drive unit failure.

First my car was having issues charginging at the charge stations. Then as soon as that issue occurred, I had to replace the car battery. Then I noticed that the windows would randomly retract while I was driving my vehicle. My child had pointed out that his window was down, when he did not roll it down. Then I noticed some leakage from underneath my vehicle. My main concern of fluids leaking from my vehicle was my main concern. The Tesla dispatcher confirmed that concern by stating that fluids should not be leaking from my vehicle. When he started accusing me of having a car accident that caused me to be even more concerned. He told me that the Tesla service center on 1100 Colorado Ave, Santa Monica, Ca. would find out if I did have an accident by running a diagnostics. When I had my car towed to the Tesla service center, their main concern was a hole in my tire and their professional opinion was to just patch up the tire and not replace the damaged product. Crystal, a representative from the Service center said that she would address the main concern of the leakage which she never did. She told me that my car was ready for pickup. I was again confused because she never addressed my main concern. Then I asked what I was paying for and she simply said that it was on my estimate and that it’s a patch for my tire. She was very adamant about me picking up my car before the Service Center closed at 5:30 but told me at the last minute to do so. She did not explain the details of the leakage until I inquired about it. I did not receive the diagnostics that were promised to me by your Tesla dispatcher. Her explanation to my main concern was, “ No leak was detected under the car so just recommend monitoring and definitely taking a pic when it happens. If the leak is by my left front tire, it’s more likely to be my AC and can be normal.” That did not make me feel like Tesla’s Supplier Code of Conduct was being followed. Now I am being told that the only liquid coming fro

I am writing to file a safety concern with vehicles manufactured by Tesla Motor Company Inc (“TESLA”). The complaint relates to safety failures with the automatic retracting window system. This complaint is different from the NHTSA previous issued recall for pinch points (issued on or around September 19th, 2022). During the vehicle software update process, owners can enter the vehicle and close the door (there is no engineered prevention of opening the doors). However, upon attempting to exit the vehicle the window does not automatically retract. When the owner closes the door after exit the windows are prone to break or shatter (see attached photo) which creates a safety hazard. The specific hazard is with broken glass spread in the cabin of the vehicle. Tesla charges customers to replace the window in spite of the known defect (see attached paid invoice, which documents how the damage occurred). Tesla states "it is a known documented issue and is therefore the customers responsibility". In lieu of correcting the acknowledged known issue or providing a proper engineered control to avoid the safety hazard created by the defect; Tesla has apparently accepted the defect and passed the risk of the defect to customers who are not formally made aware of the problem and are expected to understand the problem based on documentation in third party forums. The issue poses a cutting hazard to the driver and occupants of the vehicle and also a foreseeable egress concern in an emergency.

I have Full Self Driving I paid $12,000 for and immediately wanted a return upon receiving the car it doesn't work as described. Tesla declined to refund me for the FSD software months ago. I have brought the car into service 3 times now and Tesla each time says there is nothing wrong with the car despite 5,000 gigabits of my own dashcam footage showing the car driving me into oncoming traffic, swerving towards concrete barriers, not seeing 5 people in a cross walk when turning at 25 mph, randomly SLAMMING on the breaks and much more. The touchscreen will freeze leaving the driver with no turn signals, no speedometer etc. I feel like the car is possessed trying to kill me, I am a full time Uber driver. Tesla service manager tells me with a straight face "just don't use it." "just don't make any left turns". The front facing camera keeps rattling so I pulled the Teslacam footage and the camera is so blurry you can barely see anything so I bring it in for service and they say nothing is wrong. This is beyond dangerous this is a dereliction of duty that will result in lost lives, NHTSA you MUST intervene I have already alerted my Governor and State Attorney, they have verified they have received my testimony. I make a choice every day to get in a car I know is trying to kill me or not be able to feed my family. Tesla service manager Prince HARASSED me by calling 15+ times while I was at dinner with my 9 month pregnant wife and threatening me by telling me he's tracking my car to Publix and to my home. Wouldn't take the car out of service mode until I drove 45 minutes back to return his phone; I should have thrown it off a bridge! because he left his phone in my car with a 1 hour timer counting down. This is a cover-up please launch a REAL investigation Tesla is stealing billions Elon makes Enron look like a hill next to a mountain this just ain't right! Last time I checked 3 service trips without a solution qualifies the car as a Lemon and needs to have recalls.

In addition to the car only achieving 65% of its actual miles range claimed the cars operating system will freeze while driving (once every 3 days on average), forcing the driver to reboot the car while driving (hold both scroll wheel buttons on steering wheel). This takes away the ability to read the speedometer, to use indicators and basically ALL of the functions of the car. The rear view camera display also fails about 10% of the time which I have recorded. The car will also SLAM on the brakes while on the highway, driving in the rain on autopilot to allow a merging DHL van into my lane instead of switching to the left lane as advised by DOT/DMV and I recorded this on dashcam yesterday coming back from NASA. It is a daily issue that passengers in my Uber cannot buckle their seatbelts because the buckle is too low into the seat for them to easily find, in some cases passengers get disgruntled and I am forced to park and wait until they can buckle themselves sometimes taking over 5 minutes. The driver assistance features do not always work and also alert me and my passengers when it is unnecessary making it seem more like a gamble than a safety feature; sometimes it works, sometimes it doesn't, sometimes when it shouldn't. The false claims and inconsistencies are unlike any other vehicle I've ever driven. Tesla is a CRIMINAL enterprise acting purely in self interest and preservation to maximize profits and MUST be held accountable and to the standards of all other vehicles in the USA driving on our roads. I am APPAULED at the way Tesla has silenced me (blocked emails and blocked my ability to schedule service) and accused me of trying to cheat on my wife with the non-existent female Tesla employees on my BBB complaint which the BBB closed out claiming they solved it when Tesla didn't address a single complaint, just attacked my character publicly also known as slander. My husband took ownership of a brand new Tesla Y on Saturday 18 June from the Tesla Loveland dealership in Colorado. In less than 2 months of receiving the car, the electrical system ‘totally failed and ‘malfunctioned’ 3 times, while driving on busy roads/highways which placed my family’s life and safety at high risk of injury and death from a potential head on collision and crash from vehicles driving behind. The 3rd and most recent ‘malfunction’ occurred on Saturday 6 August at 12.46pm when (with no warning from the car’s warning or emergency system) vibrated aggressively before completely stopping in the middle lane of a busy road. This potentially fatal ‘malfunction of the cars electrical system’ was on one of Boulder’s busiest roads. I had turned onto a busy road (so was traveling at a very low speed) when the car started vibrating very loudly and stopped. There was no safety warning by the car nor any time to drive the car to the side of the road. This ‘malfunction’ and total outage put my life and my children’s lives at risk as we were trapped in a 4-lane carriage way and risked our safety to avoid oncoming traffic to get to safety. My husband called the police who created a safety barrier around the car while a tow truck arrived to tow the car to Tesla in Loveland Colorado. The incident put my family’s life at risk including my two young children (aged 8 and 6) who were passengers in the back seat. The first ‘electrical malfunction’ occurred on Thursday July 14 at 9.30pm when the car’s electrical system totally malfunctioned and stopped on a busy road. The car was towed to Tesla in Loveland. Several days later when Tesla technicians advised the problems were fixed and arranged for a Tesla technician to drive to us, the car malfunctioned and stopped on a busy road while the Tesla driver was in the car. It’s was towed again back to the dealership. This car is a lemon, unroadworthy and poses a death risk.

Front trunk (hood) flew open while driving at approximately 40mph. The front trunk lid/hood opened completely and smashed into the windshield, completely blocking driver visibility and breaking the glass. It is unclear as to how the front trunk became unlocked, but in the event it was unlocked while the car was in park, the car should not have been able to shift into gear. Tesla Model Y's Owners Manual states: "If the front trunk is not closed, a message displays on the touchscreen that you cannot shift the vehicle out of Park until the front trunk is securely closed (see Park)." Additionally, the Owners Manual states: "Model Y will not shift out of Park if the front trunk is not securely shut" In the event the trunk was open prior to driving, the car should not have been able to shift out of Park. Or, if the trunk opened while driving, there was a latch or electrical problem. Tesla's representative reviewed the incident log from the vehicle and stated that an ignore button was pressed on screen, once the vehicle indicated that the front trunk was open. Quickly pressing a button while driving to a safe place to pull over is reasonable. But, regardless, there should be some sort of fail over that prevents the hood from flying open in the event the front trunk is accidentally opened, or in the event the trunk randomly opens on its own. Perhaps a secondary latch or disabling the vehicle from driving? The hood flying up, smashing the windshield and completely blocking the drivers' visibility could have caused a severe or potentially fatal collision. Additionally, the incident caused over $4,143.34 in vehicle damage on a car that has just 202 miles on it.

The car system been keep crash cause it screen frozen and while crash it will no speed display no ac no warning no back up camera and important after lifting gas pedal the car will not reduce speed as before and I almost crash the car. Been report the problem to Tesla since early July and the keep cancel my appointment said it need wait for updates but can’t provide the date. I requested get the loaner while they can’t solve the problem but they denied and cancel my service appointment . It dangerous to drive and very unpredictable when the system crash while driving. While driving expect the car will reduce Speed but it not and no speed will display will cause over speed limit. The the pre collision system will not work at all and the system crash every few minutes and screen will shut off it self. While system crash will need triple amount of force the press on brake pedal to stop the car when it on high speed it impossible to respond to stop at short distance.

I was driving on freeway 110S in heavy evening traffic. Suddenly Tesla touchscreen showed "Vehicle shutting down - pull over safely" and "Power reduced .. exit and enter may restore operation". Car had enough momentum and luckily I could change one lane and moved to shoulder. There was no crash and I called Tesla roadside assistance for towing car to Tesla service center. The car could not start and towed to nearest service station. This is a safety issue.

While driving the frunk/hood opened while vehicle was in motion. Everything says your not able to drive or opening while in drive. My wife had just used it shopping. Drove 300 yards with No warnings. Then stopped at a light then when she was accelerating it opened and broke windshield, hood and hinges, scratched left fender inside. She was able to pull over without cause anymore damage to her or anyone else. I then meet her and I was able to drive the car with the hood open and damaged. I then removed the hood completely in order to drive it home all while no safety warnings going off. We were on the highway the power went out. The car did not have emergency brake . We could have gotten hit. We finally was able to stop on the shoulder with the driver not letting go off his foot pedal to keep the car from moving. We called Tesla and he wasn’t able to restart the car and yet he said the car has lots of power. The back door doesn’t open in an emergency as well

Phantom braking using traffic-aware cruise control during 205 mile trip. Car brakes suddenly on two lane roads, especially for large oncoming semi trucks, but also randomly with no traffic. I have to keep foot on gas pedal to keep cars behind me from rear-ending my car. Also, one time the computer screen locked up and went black. I could still drive, but there was no indication of speed. The PC rebooted and went back to normal approximately five minutes later.

Phantom braking. When car is on autopilot the car suddenly brakes. It’s happening a lot when I’m driving back from work. I drive 72 miles one way to work on Mondays and some Fridays. On one day it did breaks 3 times with no object in sight. Glad I’m not rear ended

On January 19th, around 9:35-9:40am, while driving on Bollinger Canyon Road in San Ramon, the car jerked as if someone rear-ended it and then stopped abruptly in the middle of a busy road. The information screen displayed various error messages regarding battery and drive system. The stop was abrupt and we could have been seriously injured in busy traffic. This was a scary experience. The car had 260 miles range in the battery and no prior warnings/ messages indicating any problem. We called 911 and Tesla roadside assistance for help. San Ramon PD showed up and blocked the lane to avoid someone hitting our car. They had the car towed to the nearest Tesla Service Center in Dublin, CA. While waiting for the tow truck, we attempted to restart the car and computer system two times but the car did not go into "Drive" position to be moved. The service center is diagnosing the problem and have informed us that it could be a problem with the motor or drive terrain or inverter of the car. All these are major and crucial components on the car for safe and normal operation. Failure of any of these is catastrophic and can be fatal on freeway speeds or local roads if someone is following too closely and the car stops abruptly. We usually have kids in the car and this is a scary situation to imagine. This needs to be investigated in more detail by someone to find out how many card suffer this fate and how many accidents/ fatalities have been caused by something like this.
