accident involving my Tesla while the Full Self-Driving (FSD) feature was engaged. Vehicle Information: [2022, Model - Y, ] Incident Summary: While driving with FSD engaged, the vehicle hydroplaned and the steering wheel started spinning uncontrollably and despite attempts, couldn't regain control of the car, and it spun while crossing lanes. A truck hit the front of the car and ended up on shoulder.
Tesla's FSD functions slowly has been reduced in the last years. Firts Tesla disabled the double pull function, resulted taken away the standard cruise control function. Lately, the fully paid ($12000) FSD is not recognizing streetsigns anymore, which worked perfectly before. This function is crucial to manage speeds, lanes, pedestrians and school zones or just attend in public roads! Tesla service center refuses to fix this issue or offer any additional solutions! This is very concerning and those vehicles with full FSD are not safe to operate on public roads anymore! Thank you
Any of categories are matching with my issue. 3 1/2 years old, 64,000 miles Tesla model y, ECU, which controls Full self driving , and drive assist system, is fault right after warranty expires. The replacement cost is $2,800. Does it make sense that ECU fault on the owner? It is only 3 1/2 years old car. It is definitely defective part, but Tesla service is not replace for free. I am concerning if ECU fails while I am driving.
I took Tesla to arbitration over defects and deceptive practices and their legal representative committed fraud by playing my own dashcam video and muting it so you couldn’t hear the auditory warnings the car makes when it makes errors. In this case the car was driving itself into oncoming traffic and stopping on the highway for non existent stop signs. I have reported this to the Florida Bar and Florida State Attorney Office Ashley Moody who is now investigating.
The issue I encountered was with Tesla's Smart Summon Feature. While using the feature, the car crossed the parking line and crashed directly into a car parked beside it. Happening so quickly I could not react in time. All of the cameras and sensors were in working order and it seems to have been a software related issue. The safety of anyone around the vehicle could have been at risk thankfully no one was hurt. The vehicle has since been inspected by insurance and damages to both cars were repaired. There were no warnings of any kind prior to the collision. I have a video from the on-board cameras of the entire incident.
I am having difficulty with the mobile version to report it blanks out when I attempt to upload or attach documents but I have videos and documentation.

Tesla’s “Autosteer” function inexplicably and suddenly shifted lanes and sideswiped the other car that was passing me on the right. This defect of the “autosteer” function took control of the steering of the car, even with my two hands on the wheel. I do not know why the Tesla suddenly shifted in the intersection or, more importantly, why it did not detect the other car. I did see the other car approaching before the Tesla swerved into it. I wanted to remain in the left lane because it was the correct and safest thing to do. There were no obstacles ahead of me, and the left lane continued normally.

Hello, My name is [XXX] and I had the 2022 Tesla Model Y performance. I had an accident on [XXX]. Car just took control itself, we hit the tree. It was in a residential area, it happened so fast that I could not react to prevent it. After we hit the tree, the car caught fire, It was a nightmare for me, I had my best friend with me at the time of the accident in the front passenger seat. Both of us were unconscious, by the time I got awake, the car was in flames, my friend was still unconscious but then he got awake after I tried with the local neighbor to pull him out from the car. Unfortunately my friend passed away after an accident in 5 hours. I had autosteer assistance on my car as a subscription. Plus my car had a problem with the side cameras, from time to time it was giving me errors - Door Pillar error message. Cops were trying to get information from the car, but according to Tesla representative my car was out of network from June 30, 2022. And I never got any notifications that my car is out of network. I have a case against me VS State of New Jersey, the prosecutor's office charged me with vehicle manslaughter, at the time of accident i gave a statement to the police officer, in December 12, 2023 it was Nationwide recall on Tesla cars, and this recall it was exactly for the same problem what I described to the police officer at the time of accident, so my problem is, I can't retrieve any information from Tesla regarding what happened at the time of accident, DUE TO TESLA had my car out of network for 25 days prior. I can't prove my innocence because of that I'm facing serious charges right now. My complain is about my car being out of TESLA NETWORK FOR 25 DAYS, WITHOUT PROPER NOTIFICATION ON THE APP OR EMAIL, WHILE MY CAR WAS OPERATED EVERYDAY ON THE DAILY BASIS. The front triple camera glass was found to be dirty on the interior part of the glass. I had alerts of camera blocked/blinded and also I had several instances of emergency corrective maneuvers including braking, lane assists and forward collision warnings. Tesla cleaned the interior of part of the glass.
Two incidents: 1) Non-accident: My wife was slowing to make a right turn at a stop light and the steering locked up for about 3 seconds, where she wasn't able to regain manual steering control. We did not have full self-driving mode at the time. Luckily, she was only going about 10 mph, and was able to brake before running into oncoming traffic. 2) Accident: Our son was driving on a country road, along a section with a slight curve. We had a free trial for full self-driving mode during the previous month, but it ended two weeks before the accident, so he could not have had it in self-driving mode. It appears that he didn't make the curve, but as evidenced by a single long tire skid mark, continued almost perfectly straight, across the oncoming lane, and ran into a redwood tree. The car rolled and ended up upside down, against the bank back on the right side of the road, with the lithium batteries on fire. Our son and his passenger sustained severe injuries and neither has a memory of what caused the accident. Naturally we suspect the possibility that the steering locked up again. But it also seems likely that the AEB and ABS were not functioning correctly, as if they were, there should have been several short skid marks for the tires on both sides of the car, rather than only a single long skid mark. Additionally, there's no shoulder on that road, and there is a clump of bushes that come very close to the white line, so it's possible that a steering auto-correct initiated the incident. We have not yet been able to recover EDR or log data from the vehicle.
I've had Enhanced autopilot since June 24 2022. Since that time I've accumulated approximately 15,000 miles, with approximately 80% being auto pilot miles. On May 14th my car was upgraded to "Full Self Driving (Supervised)" without my consent. I was given 30 days of FSD trial, with no way to opt out. I was presented with a software upgrade that looked like a normal upgrade, however after the upgrade I was told I had FSD. I immediately disabled FSD, only enabling autopilot (Highway only), and attempted a drive from Fairfield, CA to Concord CA along [XXX]. During the drive, the car was not able to maintain lane control, and would suddenly apply hard braking randomly. On two occasions the car accelerated towards stopped traffic, and I had to disengage autopilot. I ended up manually driving the car as it was unsafe to use to assistive systems. I contacted Tesla, who said they where unable to remove the FSD trial, and I was stuck with the upgrade, even though it made the car unsafe to drive. Unwanted Lane Assistance in Tesla Model Y made the car swerve to side. When I tried correcting it, the steering got hard to control and whole car swirled 360 degrees and hit shoulder rail. Brakes didn’t slow the car.
I am using Full Self Driving (FSD). When i look at my screen to control the cars map it gives me a strike. I have 5 strikes it removes FSD but also removed auto pilot that has lane centering and ALSO removes the camera so if I were to have a stroke, Tonic-clonic Seizure, or heart attack or for any other issue the vehicle would not pull over like it would in FSD/Auto Pilot. THIS MAKES IT VERY UNSAFE and I could argue it violates the ADA act! For privacy I have had one of the previous issues and bought the Tesla with FSD just in case it ever happens again!!! Now i don't have that safety measure for a week and I DRIVE A LOT on two lane 75 mph highways. The basic lane assist will even shut off if i hug the lane !! How Dangerous is that???? Now i have absolutely no safety features!

The contact owns a 2022 Tesla Model Y. The contact stated that while driving at approximately 70 MPH with the Adaptive Cruise Control activated, the vehicle decelerated while driving at night in the rain. The contact was concerned about being rear-ended. There were no warning lights illuminated. The contact used the App to communicate with an unknown dealer, who responded and informed the contact that there were no hard wire issues. To contact took control of the vehicle and used the accelerator pedal to drive as needed. The failure might have been caused by false positive conditions in the surrounding area. The mechanic informed the contact to reboot the vehicle. The vehicle was not diagnosed or repaired. The contact stated that after the computer was rebooted, the Lane Departure Unavailable message was displayed. The manufacturer was not contacted. The failure mileage was approximately 23,000.
Mileage: 23,000
The incident happened on 12/22/2024 8:30 am to 8:40am. Weather: heavy foggy. On freeway, no raining. Road was dry. Autopilot was off. I was trying to change lane to 2nd lane (I was in 3rd lane) and I saw a white pickup truck suddenly changed lane to the 2nd lane from 1st lane, so I turned my steering to left quickly (speed was 55-60 miles) and back to 3rd lane, and then suddenly the steering wheel was out of control, the steering wheel was turning right - left many times quicky and tilting the car. It just happened so quickly (not even a second) I wasn't able control the steering. After 3-5 seconds, it stopped. the steering was back to normal. I kept the car in 3rd lane. I almost got killed. I am glad I am still alive because it's Christmas holiday, not many cars on freeway in the early morning. Many cars behind me stayed away from me when they saw my car was out of control. I reported the incident to the Tesla manufacturer, they said they checked computer log remotely and nothing wrong, just something related to wheel slips and then cancelled my service request. I didn't know if there was any warning lamps or messages pop up because I was so scared at the moment and forgot to check.
Ever since a recent update to my car, it has become far more difficult to keep the ADAS features active. It is incredibly intrusive, even just glancing at the screen to see what message it's telling me leads to the car threatening to cut off all driver assistance features. This is so dangerous. The car is a better driver on the highway than I am. It is better at holding speed, it never veers out of its lane, and it never misses someone attempting to impinge on my lane. I think something needs to be done to make this system more lenient. It used to be better but something changed in late December. People in other cars seem to be able to stare at their telephones with impunity while their car continues on without any issues. All I want is to be able to glance away from the road to see what the screen is trying to tell me.
Due to NHTSA Recall Number 23V-838, the car has become more dangerous than before. Often making me apply more force on the wheel when constantly alerting me which has inadvertently made me swerve into oncoming lanes. Not only has this mandatory recall made the autopilot experience much worse, but it seems highly politicized by the previous leader of nhsta and should be reinvestigated. This is such a failure of over regulation over biased leadership and should not be tolerated.
To Whom It May Concern at the National Highway Traffic Safety Administration (NHTSA), I am writing to urgently express my concerns about the latest software update for the Tesla Model Y’s autopilot system, which, in my view, significantly compromises safety. As a Model Y owner for over two years, I have generally found the vehicle and its features to be reliable and safe. However, the recent update has introduced an overly stringent hands-on-wheel detection mechanism that is not only inconvenient but also potentially hazardous. The new update requires frequent and often forceful interaction with the steering wheel to assure the system of driver presence. This change is drastically different from my previous experience, where I received only one hands-on-wheel violation in two years. The current sensitivity of the system disrupts the smooth operation of the autopilot, leading to frequent and abrupt disengagements. I have found myself struggling to maintain the system’s activation, inadvertently causing the vehicle to exit autopilot mode multiple times. This issue goes beyond mere inconvenience; it actively detracts from driving safety. The need for constant and sometimes aggressive adjustments to satisfy the system’s requirements is distracting and can lead to erratic vehicle behavior. The irony is stark: a system designed to enhance driving safety and ease is now a source of potential danger and stress. The unpredictability and over-sensitivity of the updated system could lead to dangerous situations, especially on highways or in heavy traffic, where sudden disengagement of the autopilot can be particularly risky. As a driver, I now find myself more focused on keeping the autopilot engaged than on the actual driving conditions and surroundings, which is surely contrary to the feature’s intended purpose. I urge the NHTSA to investigate this situation and cancel the recall.

I was stopped at a light, and I as I continued my trip after the green. I had put the car in auto pilot. As I approached the curve there deer to my left that got my attention. The vehicle didn't follow the road, nor did it announce with any warnings, or lane assists that the vehicle was departing the lane. Impact was made into a broken guard rail. Guard Rail impaled the vehcile from the left side into the cabin impacting my left leg causing serious injuries.
I was driving south bound on hwy90 at about 78mph using navigate on autopilot. Without warning the car swerved into a left turn lane and then slammed on the brakes when the turn lane came to an end. This caused a number of personal belongings to be damaged. Thus happened as I was on my way home from a service center visit that stated the autopilot systems were all functioning correctly. Tesla was notified immediately a d has not responded to my complaints.
I was driving at 64 mph utilizing the Adaptive Cruise Control and Autosteer (Beta). I was in the first of a two lane divided highway with no vehicles within 1000 feet in front of me and several cars passing me on my left. Fortunately, I had both hands on the wheel at the 10 and 2 positions and have good arm strength. Suddenly, abruptly and violently the car took over the steering and braking swerving to the left about a foot or so then applying the brakes hard while simultaneously turning the front wheels about 65 degrees to the right. I believe another vehicle without the Tesla's low center of gravity would surly have rolled over. I was now heading across the breakdown lane towards the guardrail. With great effort I took over control of the steering just 1-2 feet from the guardrail pulling hard left and pressing down on the accelerator (to release the brakes) causing a second roll over feeling. I then turned right to straighten the car out and continued down the highway in the same lane without contacting any objects. I felt the violent nature of the swerving in my lower back and left arm. The car did not want to release the steering to me. I have no evidence other than the experience of my wife and me. I was coming off a white concrete bridge deck with a slight left curve without any lane markings. At the moment of the incident the bridge deck was covered with a large area of black tar. I believe these two factors caused the car to go into some kind of avoidance maneuver. I also believe a lesser capable driver may have been killed.
I was driving my tesla model Y on the highway and suddenly the steering wheel turn 90 degrees and crash my driver side with the back of the car in the lane to the right. The night before the accident there was a system update for the car. no warning were made by the car
When I drive a Tesla Model Y under Self-Driving mode at night inside YellowStone NP, there is another car on the opposite side with a high beam turned on, and just near that car there is a bison on the road, the Self-Driving computer fails to detect the bison in the beginning, and too late to do emergency break, the car hit the bison after passing that car. The bison died but driver is safe, the front right head light part and right repeater camera was missing. It's very hard to see the bison in the dark when the other car turns on the high beam, for both humans and cameras, the self-driving without radar or LiDAR is not able to detect the obstacle at the blind spot. I want to report this accident as a reference, the car's self-driving software helped me avoid serious injury. The Tesla FSD(Full Self-Driving or Autopilot) is great but I still hope there's more sensor such as radar or LIDAR can be added to the car for better night vision self-driving. The car deemed a total loss and there is a video from the front dash cam record how this accident happen.
On 9/4/23, I encountered a serious malfunction with my Model Y. The issue in question involved the vehicle crashing into a wall on the top level of a parking structure while using the “Smart Summon” feature in Tesla’s mobile app. The “Smart Summon” feature allows you to use the Tesla app to retrieve the vehicle and have it move to your location (within 12 meters) while you are standing outside of the vehicle. This malfunction rendered the vehicle unsafe to drive and significantly affected its performance. I immediately contacted Tesla's customer support and was advised to take my vehicle to the nearest Tesla Service Center or certified repair service center. The Tesla Service Center located in West Covina, CA, performed a diagnostics inspection and technicians said that due to the “Smart Summon” feature being a beta version, they do not take responsibility for the damage caused. I explained that the “Smart Summon” feature was first released in 2016 and that it’s original feature design is for the vehicle to stop completely if it detects any objects in its path of travel. I also raised the question on what if it was our children or any other pedestrian that could have been injured. I would like to emphasize that my Model Y is only 1.4 year old, purchased in April 2022 and it has been meticulously maintained according to Tesla's recommended guidelines. It is perplexing and disheartening to learn that a significant malfunction, seemingly unrelated to any neglect or misuse on my part, would not be covered by Tesla's warranty. I understand that warranties have limitations, but I firmly believe that the malfunction I experienced falls well within the scope of what should be covered. This situation has placed an unexpected financial burden on me, as the estimated repair costs are substantial at almost $8,000. It also raises questions about the reliability and durability of Tesla vehicles. I kindly request your time and assistance in the matter.
There was an update last night on my Tesla model y. I've noticed two episodes of phantom breaking after this update this morning when I was driving to work. There was a truck in the next Lane but the car hit the brakes all of a sudden on the highway when the autopilot was on active. It was at a curve on the highway. I'm assuming the car thought that the truck was moving into my lane leading to sudden activation of brakes. This could have been dangerous. Fortunately there was nobody behind me and we were safe.
A copy of this report is being submitted to the FBI, CIA, DOJ, my attorney and then I'm mailing it via USPS to my Attorney General [XXX] which responded to me about this car about a year ago saying she would investigate it. Yesterday my wife and I drove to Chevy and put a deposit on a new gasoline SUV. On the way back all the safety features disabled themselves and the car radio played "Break the Rules". Today I drove to Wawa to report my wife almost dying from being poisoned and on the way back the car in autopilot mode sped up towards an officer with his lights on and high visibility vest so I had to slam the brakes and move over at the last second and the Tesla didn't see the officer until he was 10 feet away and wrongly displayed the cops passenger door being open. I would NEVER drive towards an officer like that if I was in control of the car that's the faulty software. On my way back from Ford to the Mister Car Wash in Eustis to get the love bugs off the paint again I took the Tesla cam thumbdrive out, took out the micro sd from my dashcam, turned off my iPhone with the Tesla app on it and used autopilot to drive the scenic route to the carwash to avoid traffic. Then the car started playing the songs "Middle of the Ocean" which lyrics encourage you to drive your car into the ocean; then started playing "Swim Good" and then "Golddigger by Kanye West" the car disabled all its own safety features despite them still being turned on in the settings which would have allowed me to drive my Tesla off a CLIFF into LAKE EUSTIS just like that brain surgeon in California when his white Model Y drove him and his family off a cliff luckily they survived. I uploaded the video to my IG [XXX]which is currently banned for "using like increasing software" which is funny because I'm the only one that likes my posts but now I cant even like my own post. I have video evidence and screen recordings of everything I just said. As I type this the computer keeps stoppin INFORMATION Redacted PURSUANT TO THE FREEDOM OF INFORMATION ACT

Driving in Autopilot, the vehicle steered itself to the left into a barrier with no warning.
Tesla took my car in for service again, fixed nothing again. This time they messed up my dashcam even though it is clearly marked with bright red NOTICE stickers on both sides of the car that consent for audio and video recording is given by entering vehicle that is owned by me (not a lease). Car wouldn't charge yesterday, then I do the newest software update and when I get 3 miles down the street from my house I yield for an EMS EMT with siren and lights on in the middle of the day with perfect visibility and big surprise the car doesn't even register that I've not only left the lanes without lane keeping assistance but it also doesnt see the emergency vehicle so I have disabled FSD and am refusing to use AP because this is a death trap. When I complained to the FBI about it they tried to have the local PD basically SWAT my house at 4am and wake up my whole family. My wife and I are both FBI witnesses. I also received a letter from the IRS claiming they changed my taxes from 2020 (filed by my accountant) claiming that I owe $132,021 (On less than $800 in gains) by next month or they will charge me $20,000 a month interest. Today I received a notice from my insurance company of 1 year claiming they are canceling my coverage because I "drive UBER Black" which isn't true I never have because the Orlando PD wouldn't approve me to because the insurance agent lied about the coverage because Ana (Moore resources Insurance) and Mercury Insurance; was too drunk on New Years Day and has caused me to lose $30,000+ in lost revenue because she ruined my 4 month wait for UBER black approval which has completely destroyed my financial ability to provide for my family. I received a call from a DOT agent a couple weeks ago from Washington DC but the call was dropped so he called me back on a cell and we haven't been able to contact each other since. I am REALLY in fear for my life. I am an FBI whistleblower and POTUS is retaliating against me and anyone who helps us. SOS SEND HELP!
Made it less than a mile from my house this time when the Tesla couldn't see the FHP (Florida Highway Patrol) with lights on and perfect visibility. The car was updated yesterday but it has NEVER actually worked as advertised and the more I complain the more everywhere I go gets hacked. My wife Red Cross servers, hacked. My medical files, hacked. The pizza shop I was driving to, hacked, my grill, hacked, my alarm system, hacked. The lawn equipment store, hacked. My iPhone, hacked. My Tesla, Hacked. My insurance, hacked. My Uber, hacked. My Lyft, hacked. I am currently obtaining my Security+ cybersecurity certification so I can keep my family ALIVE. I am failing and being SMOTHERED! Tesla REFUSES to cooperate despite my countless attempts to be civil, involve the FBI, Local Sheriff, Local PD, IRS, CISA, my State Attorney [XXX] and Governor [XXX] and even the AIRFORCE with .mil emails are having trouble contacting me/my family. I am a high profile witness that DEMANDS WITNESS PROTECTION. I am a witness against [XXX] Twitter, Instagram, Facebook, Local PD and Sheriff offices, the DOJ, the FBI, the USDA, UBER, Lyft and of course Tesla and MANY more. They want me and my family DEAD! They will stop at NOTHING! IMPEACH IMPEACH IMPEACH! HUGE COVERUP! WITNESS INTIMIDATION! VIOLATION OF HEAD OF STATE! RACKETEERING! I AM NOT SUICIDAL! I POSE NO HARM TO ANYONE! I applied to GS13 TS clearance under Spaceforce DoD was interviewed and told I was one of the final selections and ever since Biden took office I have been in a Hunger Games type survival nightmare where I am continually hacked, poisoned, banned and left for dead. Hello it's me for the 500th time. I was sitting at a light when a new song came on that's actually a movie clip from Pulp Fiction directed by [XXX] about hitmen that kill people that speak out, like whistleblowers; for their boss. So I'm sitting at the light and the cars behind me are all glitching between 3 lanes but nobody is moving and my volume is up and it starts 'yelling' "STORING DEAD [XXX] AINT MY [XXX] BUSINESS!". This was soon after almost being ran off the road by a white Cadillac escalade that I called in and reported to the local PD. I was rear ended by a white Cadillac escalade in 2019, I almost died and received death threats while leaving a competitors business. and I'm permanently partially handicapped at 31 years old. About 1 mile down the road my car started encountering 'ghost cars' that show up on the GUI of the screen but aren't actually there in real life lol. Just so I'm clear the car is basically threatening me after Tesla says there's nothing wrong with it so I disabled my $12,000 FSD and will be keeping this car in the garage from now on. Here's the link to my shadow banned Instagram that holds all the videos of this car trying to kill me because the government wants to cover it up. The FBI actually tried to SWAT me in the middle of the night for the second time in 3 years 2 nights ago but luckily the Sargent was cool and a desert storm veteran so he knows what's up. Truly terrifying! https://www.instagram.com/reel/CsxNKLNRh-3/?igshid=MzRlODBiNWFlZA%3D%3D INFORMATION Redacted PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6).
1. Personally, I believe ALL NEW CARS should be required to put BLIND SPOT MONITORING NOTIFICATION LIGHTS in the side mirrors ! It is such a GREAT SAFETY ADDITION. Tesla does not have it. 2. I have had the car since 4/16/22. I have replaced the windshield x 2 already and this weekend a rock hit the windshield which now needs to be replaced again. 4 windshields in 13 months? Is there a proble m with the windshields?
Once again I caught on video my car trying to suicide me (a Jeffrey Epstein (not a typo I'm in fear for my life) witness) by turning off all safety features and attempting to drive me directly into oncoming traffic. Tesla has REFUSED to repair or replace my car. They have done EVERYTHING they can to prevent me from getting service for months. Reschedule after reschedule, 'we cant reproduce the problem' and 'the car is fixed' after recording them doing NOTHING on hidden camera for 3 days besides unplugging my dashcam after I told them on my IG that I was recording them and they agreed because its clearly posted on the door used to enter and unplug the camera. This is racketeering, tampering with a witness and attempted murder not to mention conspiracy to defraud the US Government with false advertisements to convince blind and corrupt politicians to give Elon ANOTHER round of government subsidies even though co2 isn't the problem, METHANE is! As you can see in the video the cars path is stuck in the ":Kill yourself with oncoming traffic" mode. Its a feature that is activated on my Tesla way more than I have video evidence like this to prove it. Here's the video: https://www.instagram.com/reel/CrzUMD4xilo/?igshid=MzRlODBiNWFlZA%3D%3D
For the last 8 months I've been reporting this FATAL error where my Tesla in FSD with location set will slow down all the way to 0mph in a 70mph zone on the expressway for a stop sign and round about under the highway that is not visible by ANY of the cameras the car is supposedly using to 'read the environment' well that's a load of crap because its stopping for non existent stop signs and driving 57mph in a 70mph zone and misses several speed signs in the video. Biggest mistake Tesla ever made was not hiring me. Tesla has amateur's and liars that will do anything for an extra buck even if it means killing you and your family with a car that was put together by robots and slaves with your tax dollars. Zero ethics. When Elon says "May the 4th be with you" he really means "I hope you get killed by the dumptruck behind you while I slam the brakes on the highway". Modern day techno-hitler. Here's yet another video after 8 months: https://www.instagram.com/reel/Cr08uwDvNqD/?igshid=MzRlODBiNWFlZA%3D%3D
Less than 2 minutes into my drive the car decided to disable lane keep assistance and warnings for no reason. As you can see in the photo linked below the vehicles in the oncoming lane are lagging and look kind of like a ghost car while the rest of the screen is totally fine. Well all I did was switch from the middle lane to the left lane like I do everyday with my indicator on, on my way to McDonalds. Absolutely no reason why this happened like I mentioned I had my indicator on nobody was around me and visibility is perfect. The contact owns a 2022 Tesla Model Y. The contact stated while driving approximately 70 MPH with the Autonomous Self Driving feature activated, while attempting to make a lane change, the vehicle was independently self-correcting back into the original driving lane. Additionally, while driving with Autonomous Self Driving feature activated, the vehicle experienced phantom braking and the vehicle stopped in the middle of the highway. The contact stated that there were no obstacles in front of the vehicle to cause the vehicle to experience the phantom braking. The contact also stated that the "Emergency Lane Departure Avoidance" failed to function correctly and would overcompensate when the vehicle made slight lane adjustments. The contact stated no warning light was illuminated. The contact had taken the vehicle to a local dealer where it was diagnosed and determined that the Autonomous Self Driving and Emergency Lane Departure Avoidance features be deactivated. The vehicle was not repaired. The manufacturer had not been informed of the failure. The failure mileage was approximately 6,900.

My Tesla model Y reports that cameras are blocked or blinded nearly every trip. This happens day or night, and seems to be related to condensation in front of the camera. There is no other condensation in the vehicle; and there is nothing I can do to remedy this. I have brought it to the service center multiple times. On the last visit, the manager informed me that this is a known/common issue and that there is no current repair available. When this occurs, it either disables autopilot which can be alarming/distracting, or stops it from being enabled. Assumedly multiple safety features are disabled while the vehicle is in this state. I can provide additional documentation if required. The image supplied is from the last trip to the repair center.

This isn't another case of the Auto Pilot being engaged and the driver not paying attention. I was driving, in the rain, on a normal road surface, with both hands on the wheel. The car attempted to steer away and out of my lane when there wasn't an obstacle or any other hazard near me. My hand went from the 10 and 2 o'clock positions to the 11 and 3 o'clock position before it straightened back out. Because of this abrupt movement that the car made, the rear end of the car lost traction and spun around on me before the car came to a stop. Fortunately, no one else was hit during this accident. By looking at the Dashcam video, the car does show brakes were applied before spinning out of control but this wasn't by my input. I feel that the car made a descision for me that I had no control of. The car was not on Auto Pilot but yet still steered for me. Accident report indicates no issues with road surface, no speeding, and no citations.
I have experience multiple instances of "phantom braking" where the car suddenly brakes when there is nothing in front of the car. The most concerning incidents are when traveling on interstate highways while Autopilot is engaged. The car will brake suddenly, rapidly, and unexpectedly. I experienced this problem that first day I picked up the vehicle and continue to experience the problem on extended interstate drives of several hours. The car is available for inspection upon request. The biggest safety concern is that a closely following car could rear end the rapidly braking Model Y. Another concern is that the car could loose control on ice or snow while rapidly braking. Despite three requests, Tesla has declined to inspect the vehicle. During these incidences, none of the cameras have indicated that they were obscured nor were any other warnings indicated that I am aware of.
"Full Self Driving" without explanation stopped working while I was using it. I tried to park the car, walk away, lock it; I even rebooted the car by holding both scroll wheels on the steering wheel as I've been directed in the past. Nothing worked. I noticed during my shift that "Auto Pilot" still worked on the highway so I was using it for a couple miles successfully. Then when the time for the exit came the car took the exit normally but then when it switched to "FSD" the car stopped steering and drove me directly towards a drainage ditch next to the off ramp and all steering assist and lane keeping was turned off all at the same time (while traveling at 50mph on I4). I was really scared luckily I never took my hands off the wheel but anyone with lesser reaction time would have crashed and possibly died. I got this on dashcam, pulled over to calm down (McDonalds near MCO), then drove back to prove it happened with the iPhone as Tesla has directed me to do on many occasions to "prove it". So I got it all on video you can see on this link here since NHTSA only allows low quality short videos less than 10mb like its the 1990's. The more I report Tesla and tell my riders about the child slaves in the Congo mining the cobalt in the batteries the more frequently the car tries to kill me. Driving both east and west on 429 near the Wekiva river results in the car coming to a complete 0mph stop in a 70mph two lane highway with barriers on both sides and no signs or lights. Then the car starts changing lanes on the highway going about 15mph while cars are whipping past me at 70+mph with the horn on. The last 8 or 10 updates have not solved this issue I've been very alarmed and done everything in my power to avoid using the software and have demanded a refund many times, this is simply a deadly scam. The FSD contract is a license to kill and silence your attempts to alert the public like a good Samaritan; it must be shut down until proven safe. Each time they say they fixed it, each time I go out and risk my life and they lie, and lie and lie and now the stories of people dying from this exact scenario are starting to pile up (pun intended). The car is STILL confusing its sea level or altitude positioning on the GPS for the round about with stop signs below the sunpass highway. Fact of the matter is I have over 15,000 GB's of footage and there are hundreds if not thousands of scenarios where the car needs to be corrected. I am a professional driver and have software development experience but we cannot allow a private corporation to use human American citizens as lab rats and throw them aside when their software that is not safe yet kills the very people trying to improve the system. Not to mention innocent by standards who have never seen the inside of a Tesla nevermind an arbitration contract. This is all completely unethical. Windows havent been fixed after recall despite recalibrating windows, 2 software updates and restarting the car all as recommended. Cheap paint scratches, chips. Plastic snaps, upholstery loosens. I've reported to Tesla multiple times that on State Road 46 Tesla Auto Pilot and FSD BOTH come to complete stops on the highway while the speed limit is clearly posted 70mph the car is showing 45mph, AP is driving at 50mph then comes to a complete stop 0mph on the highway while traffic is doing 70+ passing me because the car thinks there is a stop sign that doesn't exist. When I spoke to Tesla about it the male tech (British accent) from Tampa service center said that's highly concerning and he would take the car home overnight to make sure they didn't overlook anything but they did nothing other than take away my FSD. Now I'm out $12,000 and Tesla keeps calling me a cheater/liar and I have many examples of the car malfunctioning and we are not talking a little glitch this is fully stopped on a highway the car is trying to drive around a roundabout that isn't there because the car thinks its 30ft below the highway on the street below. I've told Tesla multiple times while I was under consideration as AI engineer that they need to include the GPS altitude otherwise this exact example will happen; but what do I know, I'm just a guy with software development experience and a professional driver who just happens to be heavily invested in gps crypto. The highway has been opened for months, I've reported it to Tesla for months. Here's yet another example from today about 3 hours ago. This is like the 7th or 8th time I've reported fatal issues with the AP/FSD program Tesla can no longer gaslight me with excuses. This car has had more attempts on my life than all the countries of the world tried on Hitler combined and I'm guilty of nothing so I don't understand why Tesla has taken my money and treated me as a delusional idiot since day one. Here's a link to my Dropbox so you can watch the unedited authentic video I have dashcam footage and teslacam to match: https://www.dropbox.com/s/r33s5he9rvwtrwl/Video%20Jan%2009%202023%2C%2011%2014%2023%20AM.mov?dl=0
New update came out and they took some of my advice; however, the problem got worse. I had an appointment at a new dealership 1 hour away and they ask me to see if I can replicate the issue and send them a video of it (yikes this is getting very dangerous) so on my way to the service appointment I do as I'm asked but the car keeps malfunctioning saying "Cheater detected" and I'm driving with my hands on the wheel with my dashcam on and iPhone with camera open ready to just grab it for the picture. I send it to them and try to use FSD again. It malfunctions again, says I'm a cheater twice in five minutes. At this time I'm unaware of what a "cheater" even is other than the traditional term. I get to the dealership and the car pops up with a window says "Full Self Driving has been removed due to constant ignoring of the blah blah blah". I tell the Tesla employee what happened he immediately says there' nothing he can do he cant fix it and I can't get my money back. I got all of this on dashcam and audio fyi. I come back the next day, the second I get out of the car and my foot hits tesla pavement a Tesla employee is harassing me to move my car except its the Uber Tesla paid for me to come pick up my car, side note the guy next to him has a huge handgun on his hip, what a safe feeling welcome... The guy inside tells me what cheaters are, I look it up on YouTube. Basically its a wrist weight people attach to the steering wheel to trick the car. I don' own anything like that the car was confusing it with my apple watch which I told Tesla and was promptly updated to the car in 24 hours. So now I'm driving a car I paid $12,000 for FSD that has driven me into coming traffic many times and calls me a cheater until it steals my $12,000 like some sort of horror arcade game. Now the car only has Auto Pilot which also calls me a cheater so its all un-useable. I have heard every excuse in the book from Tesla. Now they want to start charging me $110 to have the AutoPilot looked at.
All safety features - backup, turn signal and blind spot camera detections stopped working on November 22, 2022. Tesla gave me appointment to bring in the car on 11/28/22 then emailed me informing appointment pushed back to 12/8/22 and today emailed informing appointment pushed back again to 12/12/22. The car is not drivable without these safety features as it poses the risk of accident that could harm driver and other on the road. Tesla is not responding at all for providing a replacement vehicle, while this car is still under warranty.
I was driving with my wife and two kids at around 8:30pm (it was dark outside already) on November 29, 2022. The street was pretty empty and it had 2 lanes in each direction. I was driving the left lane with the speed at about 40 mph and had the autopilot (the one that keeps lane, not FSD) turned on. When the car was approaching the intersection with the green light on, it suddenly decided to turn left (even though there was a dedicated left turning pocket on the left hand side). It looks like it was confused and after trying to change the direction it pointed the car right to the road sign. Hopefully I had my hands on a wheel and was watching the situation, so I immediately turned the wheel right to get back to our lane. If I wasted a fraction of second we could end up smashing into the road sign at about 40mph.
Hi, My main concerned about having a Tesla is not having warning beeping sound when reversing. I almost ran over someone because Tesla rear-view camera doesn't provide virtual alert/audio when getting too close to an object or if a pedestrian decides to walk behind your vehicle while you are about to reverse or pull in reverse into a parking space. I feel this should be a mandatory monitoring system for all Tesla when reversing, just like BMW or ACURA, you do get a beeping warning sound when there's something there or when vehicle is getting too close to any objects. Hopeful that this issue can be corrected asap. I appreciate your help [XXX] INFORMATION Redacted PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6).
Vehicle is approximately 9 months old with 13,488 miles. Starting today, the cruise control system which includes the adaptive cruise control and lane keeping assist features has failed. It will not activate, and attempts to activate it result in an error stating "cruise control unavailable." As a result, the car will not brake as it approaches other vehicles; will not maintain its speed; and will not maintain its lane. I made an appointment with the manufacturer's service center to have it inspected and repaired, but the manufacturer cancelled the appointment the same day it was made stating, in writing, that it is a "known issue," "replacing any component will not rectify the issue," and instead it "should be rectified in a future firmware update" without any time frame as to when. I asked is there is a service bulletin or recall notice and was advised that "specifics" cannot be provided "unless otherwise authorized."
After my 3rd trip to tesla service (required 1 month wait each time I've had this problem since the first week) I video and audio recorded the tesla service team dropping my car off in an overflow lot for 2 and a half days before unplugging my dashcam on the 3rd day. 8 hours later I'm at the dealership and they tell me and I quote "left turns isn't a feature of FSD, just try not to use it anymore. By the way, they unplugged your camera. We found nothing wrong with your car after reviewing one whole day of footage, your cars out front." Then I get to the car and plug my dashcam back in immediately and the master key for all the Teslas is in my car (last time the same manager left their phone in my car and harassed me for hours while I was driving). Then I return the key to the manager on video, leave the parking lot, set my home destination, turn on FSD (Full Self Driving) and less than one mile away at the first turn the FSD makes it drives into oncoming traffic. This is probably the tenth time it has happened and I drive my family and passengers in this car so having to constantly bring it in and be told it has no problems after 10 total days in the shop has opportunity cost me between $2,500-$5,000 in lost revenue plus the intentional emotional infliction by gaslighting me the customer for basically not just letting my car kill me and other innocent people who didn't opt into Beta. Elon wants a war with Apple about censorship whenever I post videos of my tesla driving into oncoming traffic on Twitter because Tesla wont fix it after months (or return my $12,000) it gets banned. Same company different boss, now they censor evidence of Teslas malfunctioning and say you're imagining it LOL. You want a war, fine. I have 5,000 gigabytes of dashcam footage with over 1,000 witnesses (passengers). Now they tell me they cant service my car without evidence being submitted but the app is blocking me from submitting photos and they deleted my job applications on Tesla OS.

I've brought the car into the dealership 3 times and they swear they have gone over the whole car and there are no issues with the car but at the first stop light next to the dealership the car starts driving into oncoming traffic which I recorded on dashcam. The car keeps phantom braking (braking for no reason) swerving towards concrete barriers and brick walls, driving into oncoming traffic on roads its navigated before in the same and different conditions. Just yesterday the indicator noises stopped working so I was forced to reset the car while driving my whole family, this is very dangerous and far too common. I have been harassed by Tesla employees who refuse to find any solution. I have been requesting a refund for FSD since the week I bought the car. This is a huge scam and cover up and when you post videos of your car trying to kill you on twitter or IG (post Elon) they shadow ban you. It is against their agenda. Don't believe the lies this is like ENRON x 1,000. FTX is NOTHING compared to the theft and deciet happening at Tesla.
On 11/17/2022 about 6pm when I drive on highway I -5 from San Diego to irvine, I am driving in the middle of the lane, the car keeps on going to the left and almost hitting the wall when I was on the left lane. I moved to the third lane and the car tried to do the same thing. At one point I lost the control of the car and it suddenly changed to the lane on my left. Fortunately there is no car on my left at that time. I used basic cruise control at that time, not Auto pilot .I have lane departure avoidance on at that time, not sure what's going on. The girl I carpool with said the lanes displayed in the screen is very clear. After I got off the highway, we checked the front camera and it is clean. My husband turned off lane departure avoidance function and restarted the PC of Tesla. We drove in the highway again and Tesla behavior is normal.
I have Full Self Driving I paid $12,000 for and immediately wanted a return upon receiving the car it doesn't work as described. Tesla declined to refund me for the FSD software months ago. I have brought the car into service 3 times now and Tesla each time says there is nothing wrong with the car despite 5,000 gigabits of my own dashcam footage showing the car driving me into oncoming traffic, swerving towards concrete barriers, not seeing 5 people in a cross walk when turning at 25 mph, randomly SLAMMING on the breaks and much more. The touchscreen will freeze leaving the driver with no turn signals, no speedometer etc. I feel like the car is possessed trying to kill me, I am a full time Uber driver. Tesla service manager tells me with a straight face "just don't use it." "just don't make any left turns". The front facing camera keeps rattling so I pulled the Teslacam footage and the camera is so blurry you can barely see anything so I bring it in for service and they say nothing is wrong. This is beyond dangerous this is a dereliction of duty that will result in lost lives, NHTSA you MUST intervene I have already alerted my Governor and State Attorney, they have verified they have received my testimony. I make a choice every day to get in a car I know is trying to kill me or not be able to feed my family. Tesla service manager Prince HARASSED me by calling 15+ times while I was at dinner with my 9 month pregnant wife and threatening me by telling me he's tracking my car to Publix and to my home. Wouldn't take the car out of service mode until I drove 45 minutes back to return his phone; I should have thrown it off a bridge! because he left his phone in my car with a 1 hour timer counting down. This is a cover-up please launch a REAL investigation Tesla is stealing billions Elon makes Enron look like a hill next to a mountain this just ain't right! Last time I checked 3 service trips without a solution qualifies the car as a Lemon and needs to have recalls.
Travelling 101 North from Los Angeles to Solvang. I was driving with tesla auto piloit set at 65mph. Middle of the highway, no vehicles around, no bridges or any shadows from anything. Tesla model y braked from 65mph to 20mph suddenly without warning. Serious safety issue, had a vehicle been following me i would have been rear ended.
Last evening , when doing a lane change on Highway 680 , I lost control of the car and the car began to swerve pretty badly. Thankfully I hit the brakes on time and everything was fine again. A blue car was trying to get in my lane and I quickly changed lanes to be safe and the severe swerving began. It was like I was in a roller coaster. Thankfully it was a 4-5 lane freeway and there were no cars around me. It felt like the autopilot tool control and the swerving began. How do I lodge a complaint so that Tesla can fix it? Can I turn something off to avoid this in future because I am scared to drive the car again? I have a 2022 Tesla Model Y. Also is it possible to see some sort of video from 680 between 4-5 pm on Tuesday Sept 20, 2022? I was returning to San Jose from Fremont, CA. Thank you, Jyoti
Phantom braking while using either cruise control or the autopilot feature in my brand new Tesla Y performance. Over a several hour drive from Utah to Wyoming and back , on two lane highway going one direction and two lanes the other direction. Speeds around 65-85 mph. Sudden scary braking that I had to override with gas pedal. Occurred about 6-10 times. About 3-5 times during 8 hour ride from UT to WY and 3-5 times from WY to UT. I would quit using cruise or autopilot for awhile as it was very disconcerting. One time I felt dizzy because of sudden deceleration.
Tesla cameras are equipped with internal cameras intended to monitor driver attentiveness while using “Autopilot” features. The internal monitoring can be bypassed by simply covering the camera. This reduces the attentiveness checks and can be a danger as drivers will use autopilot functionality without proper attention on the road.
The "advanced autopilot" feature is extremely dangerous in certain conditions especially construction zones, if a bug is on the windshield, while it is raining or night time without street lamps. I purchased this car as a full time rideshare driver for nearly $100,000 after driving the car for less than 1 month the car has terrified myself, my passengers and my pregnant wife and luckily hasn't killed any innocent bystanders, yet. While in autopilot the car will frequently 'phantom brake' when there is literally no vehicles in front of it, quickly reducing speed from 70mph down to 40mph. This most recent time 7/28/2022 I had a garbage truck like vehicle behind me but thank God I have excellent reaction time and canceled the autopilot and smashed the accelerator just in time before causing a pileup on the expressway. The car constantly misreads speed limit and street signs and will confuse the SR46 little sign for a speed limit sign and slam on the brakes from 65mph down to 46mph. Just yesterday the car decided to go in the middle of the two lane highway and slam on the brakes in the middle of the highway, twice, which terrified my wife and I caught on my 3rd party dash cam. I have tried to reach out to 3 Tesla emails and even SpaceX but I receive no response, big surprise. Additionally I'm smarter than the average bear and use a 3rd party GPS miles tracking app on my iPhone called Everlance which when I've cross referenced with my Tesla app about driving range its not even remotely close to their 303 mile range. I am proving a 33% discrepancy between their claim of mileage and what the car is actually achieving on the road. For example I charge at home to the recommended 90% charge of 269 miles, drive 99.1 miles over about 4 hours and get back home with 80 miles range. 269 - 99.1=169.9 miles but with only 80 miles on the battery that's actually less than half what it should be. I paid $6,000 for FSD I never received and have requested a refund without a response.
Several times, after over the air software updates, several components of the electrical systems would stop working, including GPS & cameras, resulting in no cruise control, no back up camera, no navigation, no blind spot warning/heads up collision warning/lane departure warning. Essentially, all safety features, cruise control and navigation fail when GPS and cameras fail. In all cases, the reset options provided such as powering off the carf, powering off the screen etc did not help and instead the 12 V battery had to be disconnected, sometimes more than once, before car would reset to a safe state. this requires removal of a panel, removal of cabin filter, removal of cable to battery. This is a time consuming process and could theoretically void warranties. We are waiting for the next software update to see if replacement of a control board by Tesla this week fixed the issue.
The car is automatically braking when it sees an approaching vehicle in the oncoming lane (2 lane highway). It puts me at risk for being hit from behind due to unexpected braking. The car is also occasionally going into the left turn lane instead of staying straight when on auto drive mode and then quickly veering out of the lane.
Autopilot feature hard braked twice suddenly while driving on highway . I was next to another car .I was shocked Auto lane departure with over taking slow traffic . While overtaking the car gives left signal option to lane change and then while crossing the vehicle in the right it gives another signal to turn right or tries to turn while the vehicle is still there in the blind spot . This happen nee few times which is scary .
