Incident Description (FSD Failure) "While operating the vehicle using FSD (Supervised) on December 30, 2025, at approximately [Time], the vehicle failed to detect and completely ran through a red light/stop light at the intersection of [Street Name] and [Cross Street]. I had to take over the vehicle to avoid a potential collision. The system did not provide a warning or attempt to brake for the traffic signal." 2. Control Failure (Sliding/Steering) "Under low-speed conditions (approximately 13 mph), the vehicle suffered an uncontrolled loss of traction and directional stability. The Traction Control System (TCS) failed to intervene or stabilize the car. I was forced to perform a manual steering intervention to correct the slide and maintain my lane. This occurred at [Location/Address]." 3. Hardware/Exit Concern (Door) "The vehicle has a mechanical defect in the [Front/Rear] door. It requires excessive physical force (hard slamming) to latch properly. I am concerned that in an emergency, the door may fail to open or close correctly, posing a safety risk to passengers." 4. Full Self Driving caused my Tesla to enter an intersection while the light was red. The FSD stopped at the red light on Monday Dec. 22. 2025 at 9:37 am. After a few moments is began to drive from the correct stop and enter the intersection while the traffic light was red. I had to manually take control to prevent it from continuing thru.

Subject: Complaint – Tesla North Hollywood Service Visit on Aug 30 Date: August 30, 2025 Time: Around 12:25 PM Location: Tesla North Hollywood Service Center Advisor: Anthony I visited the Tesla service center to report issues with my car: Possible seat belt problem in the back seat (my two kids sit there) Front and rear windows sometimes stop working I spoke with Anthony explained the issues and showed him the video i recorded it clearly show seat belt and windows malfunction . tesla Security cameras will confirm this. He smiled and asked if we wore seat belts. I said yes. He asked me to go to the car, but he didn’t inspect anything. I asked him again to check, and he said there’s nothing he can do unless the problem is happening right now. Then he walked away. This was very disappointing, especially because this is a safety concern involving children. No inspection was done, and my concerns were dismissed.

On April 18th, 2025, I dropped off my 2022 Tesla Model Y at Tesla Collision Center (Paramus, NJ) after a minor accident. I later revoked consent for any repairs via email. Despite this, Tesla performed major body and paint work without my permission. No signed work order exists. The repairs were unauthorized and proceeded without my knowledge or approval until this day. These changes now risk altering my VIN history and vehicle safety records. I’m concerned this constitutes falsified documentation and could affect resale value, title accuracy, and liability in future accidents. I was never shown a repair summary or given the chance to approve or decline service. Even after I sent a formal cease-and-desist letter on June 6, 2025, Tesla towed my rental vehicle on June 4 (without warning), which I had been using through Tesla Insurance. My personal belongings were inside. The timing suggests retaliation for revoking consent and demanding documentation. There were no mechanical failures or warning lights before this, but my concern is that Tesla falsified records & carried out structural work under my VIN without my legal consent. That poses serious risk to my safety and consumer transparency. The work was done by Tesla’s own collision center. The vehicle is available for inspection. I request NHTSA to review Tesla’s repair authorization practices and investigate whether unauthorized repairs, falsified VIN records, or retaliation have occurred in my case and others. This incident reflects broader safety concerns around Tesla’s internal repair authorization system, which may allow collision centers to proceed without documented customer consent. If vehicle repairs are being logged under owner records without approval, this poses risk to accident liability, insurance disputes, and NHTSA transparency. I was driving on the freeway in the fast lane using FSD (full self driving). The car attempted to merge into another Tesla in the lane to my right. I immediately took over control and pressed on the brake to prevent a high speed accident. At that speed the car almost totally lost control. My friend and I could easily have died. We both felt the car just tried to kill us.

Tail gate hinges will easily bend when the tailgate is closed manually during failure to power close. Tesla refuses the repair and states that vehicle owner will need to take vehicle to collision center to replace hinge even though no collision is involved. Resolution is to replace hinges and align door which tesla refuses to do under warranty.

Any of categories are matching with my issue. 3 1/2 years old, 64,000 miles Tesla model y, ECU, which controls Full self driving , and drive assist system, is fault right after warranty expires. The replacement cost is $2,800. Does it make sense that ECU fault on the owner? It is only 3 1/2 years old car. It is definitely defective part, but Tesla service is not replace for free. I am concerning if ECU fails while I am driving.

Collision while using Summon feature on a Tesla. I was parked, rear end in, in a parking stall at work. A minivan was parked, front end in, in the stall adjacent to me. The two cars were facing opposite directions with the drivers side of each car next to each other. As I approached my car, from a distance of about 100ft, I used the Tesla app to summon my car to me. I pressed and held down the "Come to Me" button and my car lights went on and the car began moving forward then made a sudden, unexpected sharp left turn before pulling out of the stall. The turn was too sharp and a collision was imminent. I let the button go on the app, but it was too late and my drivers side back door panel scraped the rear drivers side bumper of the minivan next to me. I got into the car, manually backed up and surveyed the damage. I thought it was minor, but I reported it to the minivan owner. Due to an estimate of damage in the several thousand dollar range, I filed a claim with my insurance company. They repaired the damage to the minivan. I deferred any repair to my own car because I thought it was minimal. No injuries occurred as the result of this collision. I did not mention the use of the Summon feature when I reported this collision to my insurance company. I simply mentioned that I hit the car next to me pulling out of a parking stall and that the fault was mine.

I experienced a safety issue with my Tesla vehicle where the automatic window and locking system malfunctioned. While the car was stationary, my baby was looking out of the window, and suddenly, the car locked itself and the window began closing automatically. Despite the presence of my baby near the window, the obstacle detection system did not respond, and the window continued to close. It seems the detection system is designed to sense firm or hard objects but fails to recognize softer or smaller obstructions, such as a child’s head or hand. This poses a significant safety risk, especially for young children. I urge NHTSA to investigate the following: 1.The sensitivity and reliability of Tesla’s obstacle detection system for power windows. 2.Whether this issue meets the required safety standards for automatic windows. 3.Potential updates or recalls needed to address this safety concern. This is a critical issue that requires immediate attention to ensure the safety of passengers, especially children, in Tesla vehicles.

What component or system failed or malfunctioned, and is it available for inspection upon request: My drivere side door fails to open every time. The cold weather feature in the tesla app claims to pop open the door so you can grab the door to fully open only to get stuck closed. This also happens when attempting to exit the tesla. The passenger side drivers door also does not open every time. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? Has been in for service many times. Concern does not replicate except at time of pickup of vehicle. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? Yes failure to open door. Also if you are in full self driving and come to a emergency stop: The vehicle is suppost to open the door and put the vehicle in park. The vehicle only puts it into park and have to press the button multiple times to open the door. It gets stuck.

On Friday 10/11 at 10:45 a.m I was in traffic on Day Street in Moreno Valley, California. This was the third time I experienced a sudden unintended acceleration with my Tesla. I would have hit the car in front of me, but I was able to swerve into an island on my left to avoid the car. The car went over the curve and stopped. I am unsure what happened but after my prior experiences, I knew how to react. Had I not reacted swiftly, I would have hit the car in front of me. There were no warning messages. I have an appointment with Tesla on Wed 10/26. When this happened the first time, they claimed there was nothing wrong with the car, but I think they are not looking hard enough. I am aware of a voltage problem that other Tesla owners have experienced that can lead to sudden unintended acceleration.

I have only driven my car for 10k miles in past 2 years. I have been on 3 long trips only. Every trip has become scary because one or the other tire turns flat right on busy highways with my little kids in it. I have had one replacement and two repairs on the only long trips I made. No other car I have have had any flat tires till date on the same routes in past 10 years. Tesla vehicles are heavy and they make tires prone to this posing a safety hazard. Tesla should either publicly announce this or give some solution since if known no one would want to take this vehicle with this kind of risk. I have asked tesla technicians and it seems it does not matter what company's tire it is.

On [XXX] we charged enough more than 80% limit to travel towards Knoxville . The vehicle indicated we have enough charge to reach Knoxville supercharger. Kindly note our vehicle has set like that it can’t go more than 80mph. While we’re closer to severville, the vehicle still indicated that I need to drive to Knoxville but indicated battery percentage will be -2% means we will not be able to drive safely and reach Knoxville supercharge as family and might be stranded in middle of night somewhere without any help and safety compromised. This is series issue in vehicle diagnostic and projection while driving it and trusting to take us to right place with proper guidance to supercharger availability. It created a great panic to our family in middle of night. This has to be investigated for misguiding the customers about mileage model-y can go and it’s capability to project supercharger availability to safely drive with family. We panicked and found out supercharger [XXX] in [XXX] in Sevierville that model-y will NOT route or guide us to get charged. This caused reliability and approvals to let such vehicle to be sold which can put everyone at risk. What if we didn’t find our self the charger and vehicle suddenly stopped in middle of traffic that could have caused accident or rear-ending. It’s serious safety issue that vehicle is not evaluating and projecting right information to travel on Road. I have taken pictures while this happened and can attach them. Also we all can witness evidence on this whole safety problem we experienced. Reporting this to ensure no other citizen should be put thru such safety issue on a vehicle that runs in American soil. Here is what happened. I was traveling on a narrow winding two-lane road at night when, desiring to see the information on the current drive, I reached down to the lower left corner of the touchscreen to change the small sub-panel located there. It was on the music panel so I swiped to the left to change to the trip information panel. As soon as I swiped to the left my lights went out and I'm driving in total darkness! In complete fear of running off the road I managed to stop and switch on the 4-way flashers. Someone that had been following me went on around about the time I got the flashes on. Fortunately, I had stopped still on the road and pretty much on my side. (It could have been much worse!) Quite scared and flustered I managed to get the lights switched back on. I don't remember, but I must have used the normal lighting control buttons on the main touchscreen panel. For a while I had no idea how it happened but eventually figured it out. Whenever you change your lights with the turn-signal stalk (go to dim, to bright or flash them) a small sub-panel pops up in the lower left corner of the touchscreen with all the light control buttons. It stays for about 4 seconds and goes away automatically. So, if you reach down to start or pause the music a few seconds after dimming you lights, your headlights go OFF - the OFF button falls where the Play/Pause button was located a few seconds before. But its much worse than that. You can also simply swipe to the left (what I had done) and the row of lighting buttons act like a slider! The lights can go from AUTO to OFF without warning, no confirmation needed! Answers to the five questions follow. - This problem certainly appears to be a design flaw in the touchscreen user interface so can be reproduced and inspected. - We could have crashed or been hit by other vehicles resulting in injury or death. - No - No - No warning whatsoever. Please don't write this off as "operator error". That would be irresponsible.

Tesla will not fix: Tesla Pushed a Spring Software update which introduces risk to pedestrians and risk of accident including rear-ending. In the past an image of the car is on the left side of the screen and a large map is on the right side of the screen. With this update, the map is removed from the right side of the screen when not in gear. If the car is in gear it remains on the screen. To setup ADAS destination it now works best if you have the car in gear. If the car is in park you have only full screen car image and extra steps to get the large map. If you voice command destination you may never see the map until driving on road. You should be driving at that point not interacting with the screen. Accident and pedestrians at risk while using screen. For instance if you get in the car and issue a voice command to destination with only one location the tiny map image in the far upper right corner of the screen displays a route so small and completely without detail it could be wrong but you just assume it is correct because it will take extra user interface interaction to make it large enough to tell what it is actually going to do. You put the car in reverse and back into the street, put the car in drive and now you get the full size map, route, destination for the first time and route is wrong. When in park, interacting with the map you don't really know what will cause the map to go away and be replaced by the car image. Can happen if you change destination entry. The map is removed from the screen every time you put the car in park. Now a big problem. If you are driving and decide maybe I should go to a different destination first and you pull over and put the car in park the map is removed from the screen again and the destination remains unchanged. You either fight the propensity to replace the map with the car image or don't put the car in park. To prevent the car screen from replacing the map unpredictably you have to keep the car in gear.

The (Part:REAR AERO SHIELD(1498771-00-E) SCREW - HEX -M6X16(1484170-00-A) and also the (Part:DIFFUSER REAR FASCIA(1494007-00-B) SCREW - HEX - M6X16(1484170-00-A) RVT PUSH - PULL 8X18X20.0). On June,05,2024 there was a storm and there was flood areas. I drove the vehicle while passing these parts came out because water underneath removed them. The manufacturer did not tight the screws correctly and water caused detachment underneath behind the electric battery where the Rear electric motor is located. I reported this and schedule a service appointment but Tesla the manufacturer said they will not cover this under warranty. This is not fair. This should be secure and safe and not detached on its own when water hits it. This should be cover under the manufacturer warranty.

On two occasions, the Tesla hijacked 911 calls during emergencies. One time I was inside someone's house talking to 911. Suddenly I was cut off. The 911 call was hijacked by the Tesla and the person in the car did not understand how to respond to the emergency. On the second occasion, I was outside the vehicle, but the windows were open. I called 911 and was talking to them, but then the Tesla hijacked the call. I tried to continue the conversation through the open windows of the car, but this route of communication suddenly stopped. I changed to speakerphone and was able to complete the call through my phone. Tesla states that all car systems that use Bluetooth (Tesla, Apple Carplay) have this feature and that all is operating normally. (I guess if all employees steal from their employer, all is normal when someone steals from their employer.) Tesla says that before making a call, someone would have to disconnect Bluetooth or unpair the phone from the car. But that is not the solution. Tesla should use, for example, the in vehicle camera to determine if someone is in the vehicle before hijacking the phone call through Bluetooth and there should be an active prompt for Tesla to take over the call. Since Bluetooth is needed in order to unlock the vehicle, it is not a good solution to force users to turn Bluetooth on and off on the phone. Tesla indicated that they would log the complaint, but kept saying that since every other car did this they would probably not take corrective action.

I just got T-Boned in my 2022 Model Y performance. I got out of the vehicle which wasn't in terrible condition, in fact the left side of the vehicle was still perfect and the doors worked fine. I had my dog with me and everyone was okay, but I opened my door and the back passenger door to grab a jacket and check on my dog. I shut the doors to make sure he didn't run out and was a bit out of it because of the accident impact so I took a moment to compose myself.. All of the car's lights flickered and then the electric cut off leaving the doors completely locked and inoperable even though they had just been used moments before and were not damaged in the accident. I had to climb in through a shattered glass window to get my dog as all the doors wouldn't open from the outside and I was unable to access the glove box, nor pry it open because the electricity was out and the impact caused that door to make the side panel to unlock the glove box manually accessible. If the car DID catch fire or an occupant were bleeding out: Anyone inside, unconscious, would be unable to use the manually release and would be burned alive while people pull on doors that won't open that were undamaged in the accident and working 60 seconds before. From what I have read the only solution would be to jump the car using the 12V battery in the Frunk of which the Tow company and Emergency Responders were not familiar with.

I was sideswiped by another vehicle which just joined our direction of traffic coming out of a cutout. As soon as I saw the vehicle moving fast from inside the cutout to join our direction and into my lane, I immediately decided to move to the lane next to me. I was at the leftmost lane at the time. Halfway into the lane I just felt the violent impact of the other vehicle hitting us. My wife and I were shaken so bad, we were shocked and terrified at what just happened and what else might happen to us. I felt like we were travelling on the front wheels with our rear end up in the air. Next thing we knew, the car stopped in front of a tree. Our iphones and the our car's phone called 911. We were pulled out of the car and taken immediately into an ambulance and Orange Global Hospital, where we stayed for about 2 days. X rays, CT Scan and MRI were taken. NOTE: A ROD STICKING OUT FROM THE FLOOR HIT MY WIFE during the accident. This rod was connected to the wheel and was not supposed to be stick out inside the car during an accident! I BELIEVE THAT A DEFAULT IN CAR DESIGN CAUSED THIS ROD TO HIT MY WIFE , WHICH CAUSED SO MUCH PAIN, DOCTORS COMPARED HER BRUISED RIBS TO BROKEN RIBS!!

In the Tesla Full Self-Drive adaptive cruise control system the set speed arbitrarily and abruptly drops from the set speed to 40 mph on an interstate highway. In other words, if set to 70 it will drop instantly to 40 mph for no reason.

The front license plate holder, which is attached to the vehicle with two strips of double-sided adhesive tape, became detached while driving on the highway. This occurred while driving on the highway without any prior collision or impact that could have caused such damage. The detachment resulted in damage to the vehicle’s paint on the bumper, dislodgement of the ultrasonic parking sensor, and the license plate holder itself flying off the vehicle. I have ordered a replacement plate at my expense. Safety Concerns: There was no prior warning or indication of instability of the license plate holder. Tesla’s provided literature does not include instructions for regular checks or maintenance of the plate frame, nor are there warnings against the potential risks of car washes on the adhesive’s integrity. Previous Incidents: Online forums and discussions among Tesla owners reveal multiple instances of similar occurrences, indicating a pattern that suggests a potential defect in the design or materials used for the license plate holder’s attachment. Response from Manufacturer: Upon contacting Tesla regarding the incident, I was informed that they consider this situation akin to a collision and hold me solely responsible for all repairs. Despite the availability of video footage and telemetry data from my vehicle that could confirm the absence of a collision, Tesla has declined to utilize these resources to investigate the incident further. Requested Action: I urge the NHTSA to consider the potential safety hazards posed by the detachment of the front license plate holder on Tesla vehicles. An investigation into the frequency and circumstances of these incidents could be warranted to ensure the safety of Tesla vehicle owners and the driving public.

I was driving the car in a straight lane. I missed a speed bump but the car was still driving straight. After 100-150 feet driving ahead, the car suddenly steered the wheel completely towards the right side resulting in an accident where the car crashed into a wall.

Since purchasing the car, I've had issues with the driver's side floor mat sliding and/or flapping. By sliding I mean it slides to the right and then interferes with the braking. By flapping, I mean the left separate portion/flap of the drivers side mat just randomly bends over - again causing interference with braking. I've tried to correct this issue multiple times by repositioning the floor mats. There is no way to secure the mats. I also added this concern to a pre-existing service request as it is quite annoying; however, I was told to just purchase new mats - even though my vehicle is under warranty.

The incident happened on 12/22/2024 8:30 am to 8:40am. Weather: heavy foggy. On freeway, no raining. Road was dry. Autopilot was off. I was trying to change lane to 2nd lane (I was in 3rd lane) and I saw a white pickup truck suddenly changed lane to the 2nd lane from 1st lane, so I turned my steering to left quickly (speed was 55-60 miles) and back to 3rd lane, and then suddenly the steering wheel was out of control, the steering wheel was turning right - left many times quicky and tilting the car. It just happened so quickly (not even a second) I wasn't able control the steering. After 3-5 seconds, it stopped. the steering was back to normal. I kept the car in 3rd lane. I almost got killed. I am glad I am still alive because it's Christmas holiday, not many cars on freeway in the early morning. Many cars behind me stayed away from me when they saw my car was out of control. I reported the incident to the Tesla manufacturer, they said they checked computer log remotely and nothing wrong, just something related to wheel slips and then cancelled my service request. I didn't know if there was any warning lamps or messages pop up because I was so scared at the moment and forgot to check.

Sudden Unintended Acceleration. TESLA Service refuses to check the vehicle, it happened 4 times so far. On first occasion they sent it to their engineering team, and result came back after five months and they claimed nothing is wrong. They refused to provide a copy of engineering report. IF nothing is wrong, why did they refuse to provide car owner a copy. IT happened three times after that and they asking for exact day and time, which was provided for second time and they mentioned that there is nothing they can do. I had the car for about 14 months and had less than 6000 miles because I am afraid of driving it and cause into accident.

ALMOST POOPED MY PANTS - backed up into a parking spot to get food for family gathering. Took two family members with me to pick up the said food. Put collected food in trunk and we all got back in. Placed MYLR in “D” (drive) and sweet baby jesus it was going backwards! My family member in back yelled and I said that wasn’t me it’s in “D” look! And we all looked at each other puzzled. Then I tapped the “go” pedal one more time and sure enough backwards again. I put it in park and I was like “you all saw that right!?” We all agreed. Put it in “D” again and worked fine. Two family members said I had to watch some end of the world movie and they no longer want a Tesla after seeing that.

I was stopped at a light, and I as I continued my trip after the green. I had put the car in auto pilot. As I approached the curve there deer to my left that got my attention. The vehicle didn't follow the road, nor did it announce with any warnings, or lane assists that the vehicle was departing the lane. Impact was made into a broken guard rail. Guard Rail impaled the vehcile from the left side into the cabin impacting my left leg causing serious injuries.

Ever since I bought my car, the mat under the brakes and accelerator jams under them, I can get jammed and prevents me from operating them. I had a Lexus and that car was recalled because of car mat jamming problems.

On 9/4/23, I encountered a serious malfunction with my Model Y. The issue in question involved the vehicle crashing into a wall on the top level of a parking structure while using the “Smart Summon” feature in Tesla’s mobile app. The “Smart Summon” feature allows you to use the Tesla app to retrieve the vehicle and have it move to your location (within 12 meters) while you are standing outside of the vehicle. This malfunction rendered the vehicle unsafe to drive and significantly affected its performance. I immediately contacted Tesla's customer support and was advised to take my vehicle to the nearest Tesla Service Center or certified repair service center. The Tesla Service Center located in West Covina, CA, performed a diagnostics inspection and technicians said that due to the “Smart Summon” feature being a beta version, they do not take responsibility for the damage caused. I explained that the “Smart Summon” feature was first released in 2016 and that it’s original feature design is for the vehicle to stop completely if it detects any objects in its path of travel. I also raised the question on what if it was our children or any other pedestrian that could have been injured. I would like to emphasize that my Model Y is only 1.4 year old, purchased in April 2022 and it has been meticulously maintained according to Tesla's recommended guidelines. It is perplexing and disheartening to learn that a significant malfunction, seemingly unrelated to any neglect or misuse on my part, would not be covered by Tesla's warranty. I understand that warranties have limitations, but I firmly believe that the malfunction I experienced falls well within the scope of what should be covered. This situation has placed an unexpected financial burden on me, as the estimated repair costs are substantial at almost $8,000. It also raises questions about the reliability and durability of Tesla vehicles. I kindly request your time and assistance in the matter.

Tesla Incident 9/8/2023 My Tesla Y started moving backwards suddenly while I was driving forward after I accidentally flicked the right drive lever, mistaking it rain wipers. I driving on Henry Hudson parkway in NYC. It was back to back traffic on HHP. The Tesla started racing backward despite my foot off the gas, and I barely had time to brake . Luckily, traffic was very slow due to rain and carbehind had stopped and it was not damaged at all. However, my Tesla trunk was damaged. 2022 TESLA MODEL Y- Consumer sent in a copy of settle request sent originally to Tesla Corp. and a copy of proposed complaint document. Short summary of settlement doc is as followed: "To Whom it May Concern, Please regard this as my last and final attempt to resolve our disputes. I had previously directed Counsel to transmit a letter to you on January 6, 2023. That letter indicated my Revocation of Acceptance pursuant to the Uniform Commercial Code §2608 and notice of defect under the Lemon Law. You have neither responded or taken any action to investigate or make me whole, despite the vehicle having less than 3,000 miles on it, and thus being well within the scope of your warranty. As you are aware, the vehicle you sold me contains material manufacturing design and marketing defects, which I summarize here and in the attached proposed complaint, as follows: -The Vehicle has been contaminated with dangerous VOCs. Independent environmental testing has detected the presence of methl ethly ketone (72 ppbv), ethyl acetate (120 ppbv) and Methyl methacrylate (140 ppbv). This renders the vehicle unsafe to drive. These chemicals are industrial adhesives and solvents used in the manufacture of Tesla's products, and their use has been experimentally tested with fast charging lithium ion cells, specifically, the product I purchased. -The Vehicle does not meet purported manufacturer charging specifications as sold. The vehicle I purchased takes 48 hours to charge - far longer than the normal time represented to me both in terms of materials you posted online in addition to your sales staff. Moreover, the driving range is far shorter than what you purported it to be when the vehicle is sold. -The Software is defective. It freezes. The driver seat's manual door opening latch does not open the door. That means in case of battery failure, fire or accident ( when the car 's battery is not working) driver cannot get out of the car from driver's seat. And from the design there is no manual door opening function in the back seats.

Yesterday I drove 1 shift on UberX in chill mode, very carefully and tried to save as much energy as possible. Here’s what happened. I left the house with 100% charged 274 miles range not 303 advertised I track everywhere I go with Everlance GPS. After a couple rides, I noticed the cars augmented reality for auto pilot wasn’t seeing the environment properly and like usual the safety features were disabled, but I am unable to record it responsibly while driving my passenger on UberX not Uber black. When I got to a red light with no passenger, I recorded on my iPhone the Tesla failing to see about 35 to 40 vehicles over the course of 45 seconds and also fails to see a black man walking slowly across the crosswalk just 2 feet in front of the car but it can see cars 100 feet away. Meaning there is an intentional Blindspot that would be in the collision warning zone that the car is purposely not seeing because the car is hacked. Directly after that I picked up a man from his house who also has a model Y and brought him to the airport. He’s an augmented reality engineer that looks familiar to me from Tesla. Turns out someone stole his brand new model Y from tampa Tesla, and brought it to a chop shop and went on a joyride, destroying the alignment, which he was given a large check for. He said Tesla was able to provide all the problems with his car with no issue. I told him all the problems with my car. Tesla has refused to even acknowledge after a dozen service appointment. Then I told him I have 100,000 GB of dash cam footage and secret recordings of the service manager telling me if I submit the footage to the FBI they would refuse service. Well, they already are refusing service. Also I drive a lot of federal agents and military personnel. When I got home, the car couldn’t account for 70 missing miles and it’s charging at one quarter speed. I reset the car three times. Now I’m trying to submit this on my computer, but the Internet cut out! BUSTED! Shut down Tesla!

Went on a 3,000 miles road trip and recorded countless bugs and safety issues. To keep this short and sweet because I’m pretty sure I’ve already broken the world record for DOT reports on a car without any intervention here’s some bullet points. 1. Car is falling extremely short of claimed mileage range. 2. Car refuses to charge or charges slow. 3. GPS is constantly wrong and sending me down dirt roads and trying to put me miles out of my way by giving wrong directions on purpose. 4. Car cannot switch lanes in Autopilot despite perfect visibility during night and day. 5. Car keeps slamming on the brakes with people behind me or in my blind spot for no apparent reason. 6. Car keeps telling me that charging stations are offline or unavailable and when I pull up it’s a bunch of lies and there are open spots and the car charges. 7. Whenever I pull up to a charger it ALWAYS happens to be the highest energy cost of the day. 8. Car sends me to wrong charging locations despite me selecting certain charging stations manually. 9. Car keeps making an awful scratching sound when music is paused or song changed. 10. Car keeps banning me from full self driving because the autopilot isn’t working properly even though I disabled FSD. 11. Car is constantly putting off a ton of heat to the point it raises my garage to over 100F. 12. Cars overheat protection mode doesn’t work properly and never has. 13. Car summon can’t make it 4 feet without side swiping the car next to it in the parking lot. 14. Car fails to see all emergency vehicles with or without lights on like a ghost car that doesn’t exist. 15. AC is inaccurate and is either not reaching temp and the AC is roaring so loud I can’t hear anything but the air moving or is so cold myself and passengers start shivering. 16. Charging takes twice as long as advertised and never charges advertised speed like 150-250kwh 17. Full self driving can’t drive me more than 2 minutes without trying to kill me so I disabled it.

Drove to the carwash today a couple miles away to see if its just emergency vehicles the car doesn't see but its not seeing any vehicles in the right lane or passenger side of the car until they are about 150ft ahead of me. To clarify the cars vision system is NOT seeing ANY emergency vehicles (driver side, passenger side, ahead, behind, moving away, moving towards) like they don't even exist (invisible) and ALSO not seeing ANY vehicles approaching from the passenger side blind spot. Switched my IG to private, was tired of all the hecklers and people reporting me and NONE of the government agencies doing ANY of their responsibilities except trying to screw me over on made up charges LOL. Hello, me again. Been having ALOT of trouble with voice text when trying to respond to my wife at the stop light. I started telling her about how a white Cadillac escalade almost crashed into me and all the safety features 'randomly' disabled themselves while driving and I'm late because I tried to keep eyes on the car while the local PD dispatched and had to drive further than I intended but I eventually gave up after he got so far ahead after weaving thru traffic like he's playing grand theft auto. So the voice text has been doing this really 'cool' feature where I say some stuff that's incriminating about tesla or the FBI and then it just doesn't type it at all or says some stuff I never actually said that usually sounds incriminating like this one time I tried to voice text my wife that some guy tried to crash into me and then sped away at 100mph plus and later I caught up to him because he crashed at a toll in the rain; the car said I was racing a guy and I crashed! Then my wife got all freaked out and called me and I explained that I'm totally fine and wasn't racing him he raced PAST me. This car is scary as hell, it should have its own horror movie. Oh yea I just got a letter from the IRS for the third time saying they changed my 2020 tax returns and I owe $138,021 in back taxes by next month or I'll get $20,000 a month in late fees but I only made $729 that year and already paid my taxes thru my accountant. FBI tried to raid my house 2 nights ago AGAIN using localPD. All after Tesla refused my lemon law surrender and they claimed nothing is wrong with my car and tampered with my evidence camera despite them consenting to being recorded by entering my vehicle. Car is living in the garage now until DOT Agent finishes setting up our meeting because I really don't want to die that would suck. Tesla refused my Lemon Law surrender and claims there's nothing wrong with the car once again, big surprise! They messed up my dashcam during fake service regardless of legally consenting to being video and audio recorded (VERY clear NOTICE stickers on the doors) when entering the vehicle which I own; it is NOT leased. We have 2 drivers in the family. Using the Tesla app, one individual accidentally opened the trunk of the car while the other individual was driving that car several miles away. The car was in "drive", not in "park". Luckily the car was going at a slow speed so there was no accident. But the open trunk blocks the view out the rear view mirror. And the vehicle and its contents are not secure when it is driving in this mode (it is open to the road in the rear of the car as it is a hatchback-style car). This has happened twice in the last 3 months, the most recent on May 11, 2023. The confusion that led to both accidental openings of the trunk is that the family has 2 Teslas, so the individual operating the app meant to open the trunk on the other car that he was about to drive. It is an easy mistake to make when a family has multiple Teslas set up in their app. Another disturbing part of the event was that in both cases, the driver had no way to close the trunk from within the car unless the car was stopped and then placed in park. The Tesla trunk can only be opened and closed by the driver inside the car when it is in park. But the Tesla app can open and close the trunk when it is in park or drive. The car should only allow the trunk to open when it is in park, even if the app requests it to open. Upon reporting this to Tesla on May 25, the service team ran diagnostics on the car and said that there were no issues with the car. And then the issue was passed on to engineering to look at. Tesla did not report to me whether this was an intended use of the Tesla app or if there was a problem with my specific car.

WHY I AM SUBMITTING THIS COMPLAINT: I am writing to you upon the advice of the CA Attorney General's office. Their guidance was in response to a question I submitted on their web about safe and legal towing with the Model Y factory-installed towing package. PROBLEM: Risk of silent brake controller failure. VEHICLES IMPACTED Model Y's with the Lithium Ion low voltage battery & towing trailers 1500 lbs or more. TECHNICAL ISSUES: The lithium Ion low-voltage battery operates at 15.5v, out of the normal 12v operating range. The wiring harness sends 15.5 v to the brake controller. Brake controllers connected to the Model Y towing harness do not throw an error. High voltage through a 12 v brake controller may result in unexpected brake controller failure per controller manufacturer's support teams. Tesla disconnected the 12v aux power to the 7-pin towing connector. This power is used for RF trailer brake controllers. RF brake controllers don't get the required power. RF Brake controllers will throw an error & the driver knows the brake system isn't connected correctly. WHY DO I KNOW THIS? I bought the Model Y to tow my 2,000 lb teardrop trailer w/ a Prodigy/Tekonsha RF Brake controller. When I attempted to connect & verify the rig was in spec, I discovered there was no power to the 7-pin plug. While troubleshooting, I found a Twitter post that described the problem, said Tesla disconnected the power & there would be a retrofit in mid-2022. See attached. ATTEMPTED RESOLUTION I've worked with Tesla Rocklin CA for a year. They submitted cases, escalated, and even unsuccessfully tried to connect a 24v controller. A fix isn't hard. I've heard corp. didn't expect people to tow. COMMUNICATION: To my knowledge, there's been no communication to affected owners or to brake controller manufacturers. NO FIX PLANNED Tesla Corp. doesn't intend to fix it or communicate to owners. See attached. ACTION NEEDED People towing may be at risk of silent brake controller failure.

Leaving Mothers day dinner at Disney Orlando with all 3 generations of family in the car and I click 'Home' and the map sets a charging station in Miami 120miles away in the wrong direction, the FSD and AP wont work and the cars gps goes completely black. That doesn't even make sense, there was the Hooter's supercharger down the street and several others in Miami and probably a dozen more between Disney and Miami. If I wasn't a professional driver I fear this many bugs and susceptibilities could kill my entire lineage. Summon feature has never worked properly and has always tried to crash itself into cars next to it or drive the wrong way down the parking lot with no one inside. I have several videos of the car trying to crash itself after driving less than 4ft out of a parking spot at Walmart. Couldn't even make it 100ft to the shopping cart deposit. You cant summon and record at the same time with your phone so when it failed I switched to record. Before that car was driving itself up the sidewalk towards the entrance of the building where people were entering and exiting, thank goodness for the concrete pilons. This car is worse than a piece of garbage its an intentional death trap and liability on wheels. I feel like they're trying to murder me for reporting them. I am in fear for my life. I am still censored online and just an hour ago I had a US Government vehicle just about rear end me at 30mph+ over the speed limit. I have it on dashcam. Then they just followed me thru traffic for the next ten minutes. This is rackateering and fascism. Everyone said Hitler was a great guy and ignored all the people he was killing for personal gain. How is this any different than Tesla and the US Government in bed together passing each other tax incentives and sales contracts like its sugar free candy while silencing and trying to kill and smear whistleblowers like me. I am beyond repulsed that the NHTSA has allowed this vehicle to still be on the road driving Uber and Lyft customers around. Almost everyday I have to restart the car while I'm driving because the car starts playing songs that are mean, discriminatory, or out right telling me to "Kill yourself on the beach" and "You can't sit here". While I'm driving I'm forced to restart the car to regain control of my instrument cluster which when resetting while driving you basically lose all features of the car minus steering gas and brake. The sentry mode isn't recording, the indicators aren't working, there is no speedometer or navigation, all safety features are disabled. It's the perfect time to commit a crime, no evidence except my iphone dashcam and anyone else around. I have to sometime do this with children in the car because the radio keeps cursing and I can't change the song. This car is a haunted death trap and I have entered pre-arbitration with Tesla but they have not responded. I have a lawfirm on standby that has offices in FL, CA and TX. Since I received this awful vehicle it has never properly identified emergency vehicles. Sometimes it confuses neon/led signs with emergency lights and smashes on the brake for emergency vehicles that aren't there. Other times it slams on the brakes on the highway when behind an emergency vehicle and the reflective tape is lit up from my own headlights bouncing on the awfully hard and cheap suspension on this 'performance' vehicle. Lately I've captured video of the car completely failing to see a FIRE TRUCK right in front of it for at least 1 minute straight with the lights and sirens on. This is unacceptable. Tesla not only wants me and my family dead, they don't care about the lives of our brave firefighters, EMTs and police officers. Then don't care about YOU either. The car radio keeps freezing on songs insulting me or wont play music at all. After resetting the car while driving for the 937,845,609,238,457,609,823,756,098,273,546,908,723,549,068th time the car started playing "Car Radio" a song about a guys car radio being stolen and the silence making him want to pull the steering wheel and kill himself in a car accident. Shortly after this the car radio played "Kill yourself on a Beach" on my daily trip to McDonalds because it's all I can afford while slowly going bankrupt driving this Tesla on Uber because of all the 'defects' and false advertising if you can even call it anything other than intentional punishment for whistleblowing at this point. After the car couldn't drive me into oncoming traffic and put me out of my misery; I reset the car and then all the safety features were disabled again, the car couldn't see the solid white freshly painted lanes during the middle of the day with perfect visibility. As I approached the red light I demonstrated how the Tesla has no collision warning or assistance enabled although they are features advertised on the car that I paid for. Once again not a coincidence. Tesla has refused or is incapable of repairing this Frankenstein's monster covid Tesla it needs to be Totaled under lemon law. All the windows are from different countries FFS. Also the doors are sagging and scratching the base of the car but Tesla is also completely blind on that one too, go figure! They've also illegally tracked me home and to the grocery store but that's just what I can prove, I'm sure they've broken more privacy laws than just that. Recall No. 23V-085 weeks ago, my car is currently stuck on an old software version (2022.44.30.10) and is not receiving OTA updates from Tesla. I've contacted Tesla's service center and their customer support line multiple times. They've instructed me to email an address ([email protected]) to be removed from the FSD Beta program which I have done 5+ times. I believe this to be an unreasonable amount of time for them to remedy what should be something simple, and think they should be held accountable for continuing to violate the safety recall violation.

My 2022 Tesla Model Y suddenly stopped on the road with a message on the screen saying “Pullover safely Vehicle Shutting Down”, but the car stopped immediately and would not move. Another error came up saying “Rear motor disabled”. The sudden stopping was very dangerous, and it could have been life threatening had it been on a highway. Luckily for us, I was just pulling the car back from our driveway, but more than half of my car was on the road when the car stopped suddenly with the error message. I can’t imagine what could have happened had I been on a highway with or without Auto Pilot. We are all very disturbed by this experience as the car just suddenly stopped. Tesla is not safe on the road with this problem. Battery was at 235miles left and the car is in great condition otherwise having driven only 8K miles. Car had to be towed to the service center, but next Tesla service appointment shows almost a month later.

I saw [XXX] on Twitter recommending a woman put her children or dogs inside his cars with overheat protection on however I have been recording and reporting this fatal error for several months and they have NOT corrected it. The car has overheat set at 100F I routinely screenshot the car reaching 130F (assuming the cars thermometer is even accurate) this is hot enough to kill basically ANY living thing. My baby, dead. Dog, dead. It was only 82F outside so I'm going to call BS on the 'tempered glass' that 'repels over 90% heat' the car is a fishbowl/greenhouse. So if my car doesn't kill me, my family and those around me 'Full Self Driving' it will kill your kids and pets while you run errands because you trusted a melomaniac. During my driving shift on Lyft my first ride was right next to me at the medical clinic when I accepted the ride but Lyft sent me 10 minutes away to park in the middle of a median facing a memorial for a car crash victim (while playing Stan by Eminem about a fan being killed in a car accident after no one gets his letters(On Tesla Radio which has been proven to be hackable and I have reported to NHTSA before)). She gives me directions; I accept the ride then drive 10 minutes back to where I accepted the ride in the first place. The gps location for Lyft is over a mile off, after a couple minutes of scanning parking lots for people I've never met I find them at the health clinic (Lyft had deleted her directions she sent me so I was going off memory from the phone call). We start talking and it turns out they are currently in a lawsuit against Lyft. On the way to their destination (which was also wrong by several miles and required their directions) the Husband and I start noticing really aggressive drivers around us then all of a sudden the car in front of us hit the brakes short notice and zero of the cars collision warnings and safety precautions worked because there is clearly no audible sound. I have the sounds enabled I also have since day 1 because that's why I bought this car, for safety. Luckily for me I said nothing until the passenger mentioned that they braked too hard, I did a good job reacting and they agree there was no collision warning once I explained to them that the Tesla is supposed to do that but it rarely works when it should and goes off all the time for overly dramatic reasons. It's like Elon himself hacked my car and made it a clown car as described by himself in his own tweets. Upon the riders request I have released the entire recording to them via cloud server for use only with my written consent. They will be forwarding it to their attorney who is currently representing them so I would think twice about using 'kid gloves' on Tesla now on

On Saturday, 3/4/2023, I stopped at a supercharger lot to charge my car during a trip. As I entered the lot, I noticed that the road conditions were poor, almost like a gravel road. To be cautious, I drove slowly towards the three empty parking spaces in the middle of the lot, intending to park in the middle spot. I noticed that the road conditions were even worse in front of the parking spot, but it looked wide and not too deep, so I didn't think it warranted serious concern. I carefully maneuvered the car around to prepare for backing up. I've been using the HOLD braking mode, which allows for one-pedal driving, for more than five months. Just as I lifted my foot off the accelerator, the car jerked forward from a small pothole. I still expected the car to come to a stop, as my foot wasn't touching the pedal. I made a comment to my passenger that I was puzzled why the car was still moving as I looked down at my foot far above the pedals. Suddenly, there was a really loud sound (like revving if I remember correctly) that I had never heard before, which surprised both me and my passenger. We exchanged a puzzled look just as the car suddenly jumped forward at full power and hit the car in front of us before I could do anything to stop it. I believe that this was a software issue related to overcompensation of losing traction control on both sides as the car came to a stop. It's important to note that I did not purchase Tesla's Full Self Driving and was not using any driving assistance features at the time of the accident. In addition, I believe you can request more data from Tesla. I do not have access to the camera recording of interior scene. Luckily, both me and my passenger as well as the two people in the front seats in the other vehicle (also same white Tesla Model Y) were not injured. Hello, me again! So I downloaded the newest software update and in less than 24 hours and less than 1 hour of using 'Full Self Driving' and 'Auto Pilot' before the car tried to kill me and other people as usual. Same road I've been reporting for months 429 Eastbound and Westbound (Sanford FL) the car will confuse 429 with SR46 (1,000ft north) and when it gets to any of the overpasses with round-a-bouts underneath the bridge the car starts to stop on the highway (0mpg in a 70mph zone). This is absolutely terrifying; what's even more terrifying is that Tesla is claiming the cars FSD works on 'Artificial Intelligence' but that is a LIE because the car and Tesla have not learned from my many reports and have made zero changes. The only good change they made was my suggestion to Tesla in one of my many complaints about this lemon car they ripped me off on is allowing me to send them a 10 second voice clip describing what went wrong in the car (which once again was my idea) when actually using it sometimes my heartrate is still so high from cars whipping past me 70mph while I'm going 0mph that its hard to even articulate in words what happened so quickly; assuming Tesla even reviews the report and makes changes which they haven't for about 6 months of me reporting it. Car still tries to drive me off road over a transformer at home that is clearly visible on Tesla gps map which could turn the car into a bomb (so I was told by an Uber passenger who works on transformers and was leaving an OSHA meeting about a transformer blowing up on him and his coworkers). 9/10 times it's right but I know its not truly 'Intelligent' because why would it ever revert to a path I've canceled so many times? This isn't another case of the Auto Pilot being engaged and the driver not paying attention. I was driving, in the rain, on a normal road surface, with both hands on the wheel. The car attempted to steer away and out of my lane when there wasn't an obstacle or any other hazard near me. My hand went from the 10 and 2 o'clock positions to the 11 and 3 o'clock position before it straightened back out. Because of this abrupt movement that the car made, the rear end of the car lost traction and spun around on me before the car came to a stop. Fortunately, no one else was hit during this accident. By looking at the Dashcam video, the car does show brakes were applied before spinning out of control but this wasn't by my input. I feel that the car made a descision for me that I had no control of. The car was not on Auto Pilot but yet still steered for me. Accident report indicates no issues with road surface, no speeding, and no citations.

After leaving the vehicle plugged in and coming back to it after an extended period of time, The vehicle’s camera is stuck on black screen with spinning circles and gps stuck on last location. Only resolution is to walk away and the vehicle goes into a state of deep sleep. This is a major safety issue because the rear windshield is smaller than a typical standard vehicle, so without the cameras, visibility in the rear is almost non existent especially during inclement weather. Vehicle was brought to service several times to address the issue. Today will be the third attempt to remedy the issue. Issue is ongoing and has been existent since vehicle possession.

Dropped off my vehicle for replacement windshield and tesla agent informed me that there was numerous vehicle ahead of me waiting for replacement. Also wipers activates when is not raining or won’t stop after it stops raining while in Auto causing distraction to the road.

There is an outstanding recall 22V702 - automatic window reversal systems and the next course of action is that Tesla will deploy an over-the-air (“OTA”) firmware update to affected vehicles. However, my tesla account doesn't have that software available yet. Registrar of Imported Vehicles has searched the records for the above referenced vehicle and has either detected an outstanding recall, or has been unable to verify the current recall status of this vehicle. So unfortunately, they are unable to release an inspection form until such time as they need confirmation that the recall, if any, has been corrected, or that there are no outstanding recalls for this vehicle. Steps taken by me to eradicate this: 1) I tried contacting the tesla dealer at various locations in Canada and USA. 2) I did reach out to the Tesla manufacturer and they told me that they will try to deploy firmware updated for my vehicle and they told me to wait for 1 business day( this conversation happened like 5 days ago) and some associate from this location will reach out to me and i can visit that service center for the resolution of recall. Tesla dealers along with their service centres are totally unaware of what’s going on and don’t have visibility of when this software will be available. This is very big blocker for me as my car will not be registered in Canada. Can we please resolve this recall so that my import process gets completed. This is surely emergency case where I am unable to drive my vehicle and I will be charged too. I have already exhausted those days. Tesla is unable to solve this. Can we please escalate this issue and solve this recall on my car and also once done Tesla should issue recall clearance certificate that I need to submit to RIV for my inspection process.

Interior Cabin Cameras prior to a certain build date around late May, for all models and trims, do not have the necessary Infrared (IR) Leds to see driver at night/and or dark environments, leading to system to disengage due to “being blinded or covered” if a user has their car in the FSD beta testing software. This issue was reproduced by the Mobile Service Technician and then replaced accordingly, with the new Interior Cabin Camera Fixture with the IR Leds fully solving and correcting the error from before. Their part was not inspected by any agencies and error is displayed before ADAS system disengagement. This issue is also known and reproduced across many different people, with this replacement correcting the issue. Necessary action would involve ordering a retrofit for all cars with the old interior cabin camera to the new interior cabin camera.

Hi, My main concerned about having a Tesla is not having warning beeping sound when reversing. I almost ran over someone because Tesla rear-view camera doesn't provide virtual alert/audio when getting too close to an object or if a pedestrian decides to walk behind your vehicle while you are about to reverse or pull in reverse into a parking space. I feel this should be a mandatory monitoring system for all Tesla when reversing, just like BMW or ACURA, you do get a beeping warning sound when there's something there or when vehicle is getting too close to any objects. Narrative November 18, 2022 At approximately 8:30 in the morning I was driving my new Tesla Model Y through the rolling hills of South Central Pennsylvania on I-70 heading to Breezewood in order to catch the Pennsylvania Turnpike West. Suddenly and without warning, the car began to break as hard as a car can brake without skidding. Almost like a full panic stop. The car must have stopped in four or five seconds from 60 miles an hour. I was in the left lane at the time and I tried to get over onto the shoulder but I was unable. The car came to a complete stop in the middle of the right lane. The car was in the center of the right lane and was completely locked up. I was unable to get it into neutral. I was unable to get it into a gear so I turned on the emergency flashers. Right behind me were three semis. The first one had to swerve to avoid me. The trailer almost flipped over into me. The semi behind him had to swerve to avoid me and thank God there were no cars in the left lane. The third semi had to take evasive action to get around me. I am grateful those three truck drivers were on the ball at that moment. After they went by, I got out of the car and ran up the hill to flag other trucks and cars to move into the left lane. The car had come to a stop on the other side of the top of a crest so cars and trucks didn't have much notice coming up on my car. Meanwhile, I called 911. I stood at the top of the crest for the better part of an hour and a half, maybe two hours waiting for the police to show, but they never did. I did receive some help from passersby who stopped to help and called 911 as well. After almost two hours, a road crew showed up to lay down flares. And finally a tow truck came to tow me to Crystal Lake PA. Almost 8 hours later I finally got my car into a Tesla dealer in Mechanicsburg PA. Intermittently the ability to interact with my touch screen has stopped. This means I cannot access crucial vehicle controls while moving. After trying to get a service appointment to address, Tesla claims that this should be resolved in a future software update but refuses to give a timeline for the release while I continue to have to drive my vehicle in an unsafe manner.

The turn signals do not work correctly with Teslas. You have to hit the switch multiple times in order to register your intent. For example, when I plan to turn left, I hit the turn signal switch, nothing happens. Then I try it again and it works. There are times it takes three switches in order to get the signal to register correctly. This is a known issue and complaints in forums that Tesla turn signal don’t register the first time. This is dangerous if I have to quickly switch lanes or exit and having to hit the turn signal switch multiple times in order for it to work. Very dangerous. This should work every single time, the first time.

Was stopped at a light and the car rapidly accelerate on its own hitting the car in front of me. The air bags didn't deploy. I stepped on the break pedal but the car continued to accelerate, I have informed Tesla and opened a service ticket. A police report was filed for the resulting accident involving 2 other cars. I have also had instances in the recent past where while driving on the highway the car has rapidly slowed down / applied the breaks for now obvious reason. The phantom breaking issue reported by many.

On August 21, 2022, the front hood ("frunk'") of this car inexplicably flew open, swinging around and severely damaging the windshield (and the hood, naturally). The car was not being driven, it was being transported from California to Florida atop a large vehicle transportation truck. On the third day of driving, after nearly 2000 miles of the trip on a highway in Louisiana, the hood inexplicably popped open while the car was riding on the top of the transport vehicle. The truck driver was made aware of the issue by a passing motorist, pulled over, and was able to pull the hood down and latch it. Tesla customer support has told me via a chat app that their systems indicate that the hood remained latched during the entire journey, and at no point was unlatched during the transportation period. Tesla also cancelled a scheduled inspection at its own service center, indicating the company has no interest in understanding why a presumably latched hood would suddenly fly open. I have had the vehicle evaluated by two repair shops, and the windshield damage is very obviously and perfectly consistent with a hood swinging around on its hinges and smashing the windshield. The hood, at the near corners which smashed into the windshield, also show scratches and damage/bent metal. I feel very fortunate to not have been driving this vehicle at the time it happened, as I would have completely lost visibility with the hood raised. The transport driver was in possession of a fob, which when pushed very deliberately in a specific manner, could unlatch the front hood. However, the driver did not do so. I believe the safety of Tesla's Model Y hood latch should be investigated, from a mechanical and software/electronic/radio systems activation perspective.

There needs to be some type of proactive repair/advisory on this issue so owners can see if they have a problem and get it fixed before there are more accidents.

My husband took ownership of a brand new Tesla Y on Saturday 18 June from the Tesla Loveland dealership in Colorado. In less than 2 months of receiving the car, the electrical system ‘totally failed and ‘malfunctioned’ 3 times, while driving on busy roads/highways which placed my family’s life and safety at high risk of injury and death from a potential head on collision and crash from vehicles driving behind. The 3rd and most recent ‘malfunction’ occurred on Saturday 6 August at 12.46pm when (with no warning from the car’s warning or emergency system) vibrated aggressively before completely stopping in the middle lane of a busy road. This potentially fatal ‘malfunction of the cars electrical system’ was on one of Boulder’s busiest roads. I had turned onto a busy road (so was traveling at a very low speed) when the car started vibrating very loudly and stopped. There was no safety warning by the car nor any time to drive the car to the side of the road. This ‘malfunction’ and total outage put my life and my children’s lives at risk as we were trapped in a 4-lane carriage way and risked our safety to avoid oncoming traffic to get to safety. My husband called the police who created a safety barrier around the car while a tow truck arrived to tow the car to Tesla in Loveland Colorado. The incident put my family’s life at risk including my two young children (aged 8 and 6) who were passengers in the back seat. The first ‘electrical malfunction’ occurred on Thursday July 14 at 9.30pm when the car’s electrical system totally malfunctioned and stopped on a busy road. The car was towed to Tesla in Loveland. Several days later when Tesla technicians advised the problems were fixed and arranged for a Tesla technician to drive to us, the car malfunctioned and stopped on a busy road while the Tesla driver was in the car. It’s was towed again back to the dealership. This car is a lemon, unroadworthy and poses a death risk.

Phantom braking occurs very frequently and the dealerships tell me there’s nothing that they can do. We almost got rear ended on a 65 mph highway because of one instance but it happens on a daily basis if using cruise control.

While driving the frunk/hood opened while vehicle was in motion. Everything says your not able to drive or opening while in drive. My wife had just used it shopping. Drove 300 yards with No warnings. Then stopped at a light then when she was accelerating it opened and broke windshield, hood and hinges, scratched left fender inside. She was able to pull over without cause anymore damage to her or anyone else. I then meet her and I was able to drive the car with the hood open and damaged. I then removed the hood completely in order to drive it home all while no safety warnings going off. Tesla has had it since 6/3 and no determination of why or fault. $4600 estimate for repairs Still not fixed

Several times, after over the air software updates, several components of the electrical systems would stop working, including GPS & cameras, resulting in no cruise control, no back up camera, no navigation, no blind spot warning/heads up collision warning/lane departure warning. Essentially, all safety features, cruise control and navigation fail when GPS and cameras fail. In all cases, the reset options provided such as powering off the carf, powering off the screen etc did not help and instead the 12 V battery had to be disconnected, sometimes more than once, before car would reset to a safe state. this requires removal of a panel, removal of cabin filter, removal of cable to battery. This is a time consuming process and could theoretically void warranties. We are waiting for the next software update to see if replacement of a control board by Tesla this week fixed the issue.

Dear Sir/Madam, We bought the car from Tesla two weeks ago. We got a flat tire and have been stranded for a while. We been given very conflicting responses. First, Tesla told us that within 24 hours they will send us a tire tech. Second, Tire tech was scheduled, but in the last minute they cancelled. Then, they said Saturday. It was cancelled again. They said Monday. Then, they said we do not have your wheel so need to tow. It has been very frustating.

Child opened door to exit vehicle. Finger was resting near window. Window retracted as door opened and took child's finger with it. Child's finger was trapped and smashed between glass and door. Finger was able to be removed with injury to finger. Window did not pop back up when door was closed in an effort go release the finger.

Tesla Y has a special setting for carwashes, that driver has to enable, which puts the car in neutral, closses all the windows and plus some more. Unfortunately system failed; car accelerated on its own and I hit a car in front of me, so it failed the second time and car didn't stop on its own to avoid collision. I called Tesla and this incident is under their investigation.

After taking the vehicle on a road trip through Canada, the vehicle’s heating and cooling system failed. We had been driving for ~15 hours. The error message was VCFRONT_a447 Cabin Climate control system requires service. This is a safety issue for traveling in cold climates as well as eliminating fog on the windshield that would cause a driver to lose visibility. A two button reset did not resolve the issue. A hard reset seemed to, but 15 minutes back onto the road, we saw the same error message again, and the heating no longer was warm. Please take action to ensure the safety of drivers and passengers.

The side pillar cameras on the Tesla are not sealed and contain moisture. I live in Minnesota and this moisture will frost over in cold weather, thereby blocking these cameras and disengage autopilot while driving or make it unusable.
