The contact owned a 2023 Tesla Model Y. The contact stated that a software update (2025.44.25.5) was performed on the vehicle, which was for a temporary free self-driving mode. The contact stated that while driving at approximately 75 MPH, the vehicle independently accelerated sideways and crashed into the highway's concrete retaining wall. The air bags deployed upon impact. The contact was taken to the hospital by ambulance, where he was treated bruised left knee, cracked ribs, and several other bruises to various parts of his body. No property damage was reported. A police report was filed. The vehicle was towed from the scene to an independent mechanic, where it was deemed a total loss. Neither the manufacturer nor the service center had been notified of the failure. The failure mileage was approximately 22,500.

On November 16, 2025, my Tesla Model Y experienced a complete and dangerous failure while driving. The touchscreen went totally blank and stopped responding. All essential driving information and safety functions were lost, including speedometer, gear selection, turn signals, HVAC/defrost, and vehicle controls. At the same time, the car made a continuous loud humming/buzzing sound and displayed a low coolant warning. This made the vehicle unsafe to operate because I could not see my speed, cannot shift properly, and could not access any safety controls. This is the second time my vehicle has experienced a critical malfunction. The first major incident occurred on July 28, 2025. A complaint has been filed with the NHTSA under tracking number 11676662. Both failures involved sudden loss of essential functions, raising serious concerns about electrical, cooling system, or display system defects. I am reporting this because it is a serious safety risk and could lead to vehicle loss of control, inability to signal, or inability to see speed or gear position. The problem has been reported to Tesla and is pending evaluation at a Tesla service center. Warning messages and symptoms appeared at the moment of failure with no prior signs.

First cold day of the year and the heater stopped working. With the heater not available the windshield couldn’t be cleared it would always fog and cover the windshield blocking the view infront of the vehicle. Next couple days it was even cooler and the windshield was frozen with no way of heating and clearing the view. Yes the issue was reproduced by the service center upon taking the vehicle there once the vehicle was a little warm. Tesla service center mentioned there was an internal fault in the compressor which they can’t be clear so have to replace a compressor, exv valve, pressure temperature sensors and coolant. Vehicle was parked when I tried to turn the heat on in the morning it would just blow cool air. No warning before the failure. Once the compressor faulted it would give a warning “vcfront_a447” cabin climate control system requires service. And just blow cold air everytime the vehicle is started. If turning on heat from the phone app or inside the cabin it would blows cold air for about 10 mins then it will show the warning lamp.

Driving our 2023 Tesla model Y east on [XXX] on [XXX] near Pecos TX. Car suddenly lost power and started hard braking. We were rear ended by a large pick-up truck that was following as we were both traveling approximately 80 miles per hour when this occurred. The car was towed back to Dallas from Big Springs, TX (approximately 280 miles). The car's rear-end damage was such that my Tesla was deemed totaled by the insurance company. This power failure and hard braking had happened to me repeatedly over the last few years. When I spoke to Tesla at the Dallas service center about this issue, they acted as though they had never heard of this happening to anyone. We were both shaken by this accident but other than some soreness the next day we are physically OK. Tesla repair estimate was approximately $22,000. I was forced to buy another car. Both B-pillar camera housings on my 2023 Tesla Model Y Performance become extremely hot even when the vehicle is parked in a closed garage and not in use. On the morning of the incident, the garage temperature was approximately 78°F. The car had been parked overnight for over 12 hours, plugged in, and charging to 80%. While the vehicle was preconditioning, no defrost features were active. Despite this, both pillar surfaces measured 156°F with an infrared thermometer—hot enough to cause skin burns within seconds according to standard burn-threshold data for human skin (~155°F). Both sides heating to this temperature simultaneously suggests an electrical or control malfunction keeping the pillar camera assemblies energized. These components sit near the seatbelt anchor and side-curtain airbag area, raising concern of fire, electrical, or occupant-safety risk if the condition persists. Continuous heating while parked could also degrade wiring insulation or camera electronics used by the vehicle’s driver-assistance systems. The vehicle and affected components are available for inspection upon request. The issue has not yet been inspected or confirmed by Tesla, but a service appointment request was submitted immediately after the event. Tesla repeatedly rescheduled the appointment, delaying it by more than six weeks despite the described safety concern. No diagnostic alerts, warning messages, or notifications appeared in the vehicle or Tesla app. This abnormal heating has occurred more than once under similar conditions and does not appear to be caused by ambient temperature or sunlight exposure.

RE: VIN [XXX] Battery problem Car is less than 3 years old. Suddenly stopped in the middle of the road. Only had about 5 seconds or so to react and the car stuttered and shut down. Could not move it or drive it all. Dashboard was still working but it would not move and flashing red in the console. Very scary and frustrating experience. Fortunately, I was on a city and not on a highway. The car had an error: "Pull to the side" which was very short of an alert response. I tow the car to Tesla center and was told that more than likely main battery issue with the inverter or similar. The Service center does confirm the inverter has been reset but they cannot find anything wrong with it. I do not think it is safe to drive as incident could happen again with the battery/inverter. The vehicle went into Turtle mode and there was a warning sign that power was reduced. The car eventually stopped moving on the highway seconds later. The warning time was minimal and I was on a two lane highway when the car stopped in one of the lanes during rush hour. This is the third time I have had this warning since February 2025 and I started leasing the vehicle December 2024. The first time, the service replaced the accelerator pedal. The second time, the rear motor. This third time, they are saying it's the accelerator pedal again. Each time there is no warning or signs that there is an issue and it is unacceptable for a vehicle that is only one year old.

Driving on I 880 south on the fast lane. The car suddenly braked for no reason and began to shutdown. It warned me to pull over before complete shutdown. Luckily there was space on my left and I was able to rolling stop. Nothing on the screen was working. Turned on hazard light. Got out of car with my phone. Accessed Tesla app and ran battery check. After couple mins the error went away. Tried to slowly move with hazard light on. It appeared to have fixed itself. Slowly merged back into freeway when it was safe. Attached notification when I tried to schedule service request.

The tesla computer freezes. Sudden there is no information from the car, no miles per hour, no navigation, no side view cameras. I can drive the car but that's is all. Since the navigation is entirely dependent on the computer I can't tell where I am going. It has happened more than 30+ times in the car.

On Memorial Day weekend, my car was parked outside, and apparently back passenger window was left open, which caused the backseat area of the car to be soaked in water. We noticed that in the morning, and parked the car inside the garage, and kept the door and windows open so it could dry quickly. However, a few hours later, around 4:30 pm, the car caught fire while parked in the garage. We called 911 right away and quickly using two fire extinguishers, quickly put out the fire. The Frisco Fire Department opened up the investigation, and I believe they concluded the cause of the fire was an electrical short circuit due to water or battery-related issues. The fire incident occurred inside the garage, which could have potentially burned the entire house. However, due to the brave actions of my [XXX] , we put the fire out, but he ended up in the hospital due to severe smoke inhalation. The car computer (HW3) in my 2023 Tesla Model Y failed without warning while I was traveling from Maryland to New Jersey. When I attempted to drive the car, the main screen was completely black and unresponsive, meaning I could not see or control any essential functions—no speed display, gear indicator, or system feedback. The vehicle was not drivable. Acceleration was extremely delayed (took more than 10 seconds to start moving from stop) when attempting to move, and the car felt like it could stop at any moment without warning. I had no way to safely monitor or control the vehicle’s basic operation. This created a serious safety risk—not only was I unable to access the gear or speed, but the vehicle was in a condition where it could shut down suddenly at any time due to the computer failure. This left me stranded while away from home and could have resulted in dangerous situations if I had been on the road when it failed. Tesla Service inspected the vehicle and confirmed the issue was an internal fault in the car computer (HW3), diagnosed by their senior technician. They provided a repair estimate of approximately $3,000. No police, insurance, or other third parties have inspected the vehicle. There were no warning messages, alerts, or symptoms before the failure occurred. It happened suddenly and completely at around 59,000 miles—just 9,000 miles past the basic warranty limit. The failed computer is being fixed by Tesla service center in Paramus, NJ location.

Response: All 3 times has been with a faulty inverter 2)How was your safety or the safety of others put at risk? Response: Safety was a HUGE concern. The Tesla model Y has stalled and completely shut down while driving through traffic and at speed. The complete failure did not allow any coasting to try and get out of harms way. 3) Has the problem been reproduced or confirmed by a dealer or independent service center? Response: All 3 times this issue has occurred were all serviced by Tesla directly and has always been a faulty inverter. 4) Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? Response: Yes, all 3 times the car failed the car was assessed by Tesla's service station Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? 5) Yes, the first 2 times the warning appeared there was only a matter of seconds before the car completely stopped in the middle of traffic. This last time there was a warning stating the car will not restart if stopped.

I am writing to raise a safety concern regarding the use of Tesla’s Full Self-Driving (FSD) system on my 2023 Tesla Model Y when a rear hitch-mounted bike rack is installed. Numerous users, including myself, have experienced dangerous or erratic behavior from FSD when carrying bicycles on a rear-mounted rack. Specific issues include: False detection of a trailing vehicle directly behind the car. Phantom braking or swerving, including abrupt lane changes or acceleration. Unreliable navigation at intersections (e.g., nearly running stop signs) due to apparent misinterpretation of sensor data. In some cases, tailgating or aggressive following behavior by the vehicle. These behaviors appear to stem from the FSD system misinterpreting the presence or visual signature of the rack and bikes as another vehicle. In several online reports, this has led to unsafe maneuvers. To temporarily mitigate the issue, some drivers have resorted to taping over the rear camera, which is neither safe nor user-friendly. While I recognize Tesla has made improvements such as camera obstruction warnings (e.g., in FSD v12.5.6+), these do not address the core misclassification problem. My requests: That Tesla officially acknowledge and investigate FSD misbehavior in the presence of rear-mounted bike racks. That Tesla provide clear driver guidance on whether FSD should be used when such a rack is installed. That the FSD system include an option for a “Bike Rack Mode” or alternative logic to prevent rear-camera misinterpretation. That this concern be escalated within your safety and AI development teams to ensure user safety is prioritized.

We experienced an extremely dangerous and life threatening situation with Tesla Y model on [XXX] around [XXX] and would like to report to NHTSA. We were driving on [XXX] going towards Nashua and my TESLA-Y car when the steering wheel froze and I got message that front camera was not working. When I tried to steer the car in the lane, the car suddenly braked and took two 360 degree spins (turns) on the highway with cars streaming at high speed around us. We went from the left lane to the right most lane after two 360 degree spins and ended in the right lane. Luckily, no other autos hit us and I was able to take control of the car and pull over in the right lane. The incident is recorded int the Tesla's harddrive which we can make this available to you. the car started working normally so we drove back home. We have had minor control incidents with the lane assist and auto pilot systems of this car in 2023 and 2024. We have complained and showed the problem to Tesla dealer in Watertown, MA. However they said there is nothing wrong with the car. We request you to investigate this incident with Tesla and find out the cause for the car to take this dangerous breaking and spinning incident on a highway. Vehicle had a open window during rain storm. Interior got wet. Next day the vehicle was left to dry out. Vehicle caught on fire and engulfed in flames in the backseat. Nobody was harmed fire was contained by the Frisco Fire Department. Vehicle was then towed away.

On [XXX], the vehicle displayed multiple alerts: “Electrical power reduced,” “Climate cabin controller requires service,” “Electrical system may not support all features,” and “Vehicle may not restart.” I scheduled service for May 21, 2024, and dropped the vehicle off at the Tesla Service Center in Fresno, CA. Tesla failed to provide a written invoice for that visit. A service advisor told me “It was a glitch” and that I should check the app, but no documentation appeared. Despite multiple in-person and written requests, I never received a record of the visit. On [XXX], the vehicle failed to start and was towed back to Tesla. A May 24 invoice confirmed a major safety issue—a high-voltage current fault that triggered the Pyrotechnic Battery Disconnect. Tesla later marked the May 21 appointment as “canceled” in their system. I photographed this cancellation screen in July, confirming the visit was erased despite a clear safety-related defect. This raises concerns about Tesla’s compliance with documentation and reporting obligations. I also submitted two reimbursement requests for $470.12 in transportation costs, which Tesla has not honored. These incidents show a pattern of concealment and unfair warranty practices. This is my 5th complaint and involves, again, the key card. This is the used key card the El Paso Tesla Dealer was kind enough to give me, and help me program, when one of the original key cards, less than a year old, stopped working. (Tesla wanted me to pay $45 for 2 new key cards) The used key card stopped functioning so I planned to call Tesla and have them "reprogram it"...something that must be done while I am in the car. I went on a last-minute trip and took my spare, barely-used key card. I also had my phone but didn't realize I'd not re-downloaded the Tesla app when I recently troubleshot my phone. Later in the day I parked at CIty of Rocks State Park in NM, still without connectivity, and locked the car with the "good" key card. Then I misplaced the key card! I called one of my 2 backup drivers and asked her to unlock and start my car for me. Her App did not allow her to do so, even though it showed the location of my car at City of Rocks...it said "move closer to the car". (She lives in CA). I had to get a ride to Wifi, download the Tesla App, and when I tried using it to unlock the car, it said "you must be closer to the car". When I got to the car, I was able to unlock and start it using the phone key. After arriving home, I used my App to get Roadside assistance and asked for a phone call when they texted to confirm my location. I explained that I didn't need service, just needed help re-programing my defective card. (I had since found the good card!) The woman on the other end was helpful but it was difficult for her also. We ended up removing the defective key from my profile, though it required a lot of work and also required the "good" key card. I have to call Tesla on Monday (it was Sunday) to get the card re-programmed. Had my difficulty been in a high-traffic or unsafe area I would have been in trouble. The "backup" drivers on my App are there for emergency access to my car, supposedly from any location.

Tesla remotely disabled my vehicle twice (February 28, 2025, and March 6, 2025) without prior notice, legal justification, or a court order. February 28, 2025 – Tesla Disabled My Vehicle for 3 Days My Tesla Model Y was remotely locked without warning, leaving me stranded and unable to use my vehicle. Tesla refused to provide a reason or reinstate access for three days. March 6, 2025 – Tesla Disabled My Vehicle Again & Attempted Repossession Tesla again remotely disabled my vehicle, then sent a tow truck for repossession without legal notice. I called the police, who confirmed Tesla had no legal repossession order. Tesla refused to restore access, forcing me into a coerced “voluntary surrender.” How This Posed a Safety Risk I was stranded in public locations without notice or transportation alternatives. Had the vehicle been in motion, this could have caused a road hazard. Tesla’s lack of due process denied me the chance to dispute the action before losing my vehicle. Confirmed by Law Enforcement & Tesla’s Own Records Police intervened on March 6, 2025, and confirmed Tesla had no valid repossession order. Tesla has refused to provide written documentation or a legal explanation for these actions. Regulatory Concerns Tesla’s unilateral ability to disable vehicles raises serious consumer safety issues: What prevents Tesla from disabling vehicles arbitrarily? What legal safeguards exist to protect owners from unjustified manufacturer lockouts? Should federal law restrict Tesla from disabling vehicles without court approval? Request for Investigation Tesla’s remote disabling practice endangers consumers and should be urgently investigated by NHTSA to determine if federal safeguards are needed.

I am submitting a complaint regarding a serious safety issue with my 2023 Tesla Model Y. The vehicle experienced a sudden and complete power loss while in operation, creating a hazardous situation. If this had occurred in high-speed traffic, it could have led to a crash, endangering my safety and that of others on the road. •Reproduction of Issue: The issue has not been reproduced yet, but it is an ongoing concern. I have not received a resolution from Tesla at this time. •Inspection by Authorities: The vehicle has not yet been inspected by Tesla, law enforcement, or insurance representatives. How did Tesla pass the Safety Standards for the position of rear door handles to manually open the vehicle? The vehicle gets into an accident OR the car plunges into water - and this almost always results in a non-responsive/deal battery. *** The rear passengers are hurt and you want them to find that little piece of rubber mat hidden in a cranny, lift it, and then use their nail to open the plastic cover to reach that piece of junk to manually open the door? ** NO WAY !! Manual door handle in the rear doors should be placed where it's easily accessible, just like you have on the front doors. This is truly a life threatening safety issue with how Tesla has hide the rear passenger manual door handles. Please force Tesla to issue a recall and fix the issue by providing an easy access to manually open the rear door in emergencies. The car gets on fire and rear passengers may burn alive because they won't be able to open the door due to the current system of a super hidden rear door handle. While Driving, the system prompt electrical system failure. pull to the road side. within 20 or so seconds, the car lost power. About 10 minutes or so later, the whole car (emergency light, door, trunk etc lost power too. Consider the car lost driving power in seconds, then lost emergency light can be highly dangerous to all parties. Before the failure, the car shows "front motor is disabled, power is reduced, OK to drive" This OK to drive turns out not OK to drive, misleading. Now the car is fixed (yet to pickup), reason Front drive unit and pyro fuse were replaced (new car, but with refurnished part).

Driving Incident with Auto High Beam: I was driving on the freeway early in the morning with the auto high beam feature activated. However, due to heavy fog, I wanted to turn it off. I asked my passenger to help me find the option on the screen. Unfortunately, she clicked the wrong button, which completely turned off all the lights while we were driving. This caused a moment of panic, and I had to slam the brakes, realizing I had veered off the road. Thankfully, there was no one behind us. While it was an oversight on my passenger’s part, I can’t help but wonder: shouldn’t there be a warning or a disabled button while driving, especially in the dark? I’m not sure if this qualifies as a safety issue, but it certainly felt life-threatening at the moment. Had there been another vehicle behind us or if we had hit a divider, the situation could have been much worse. Suggestion for Improvement: To enhance safety, I suggest implementing a feature that disables the off button in light controls while the vehicle is in motion, especially in low-visibility conditions. Additionally, a clear warning message could pop up if a driver attempts to adjust the lighting settings while driving, ensuring that safety remains the top priority.

After the crash, the vehicle battery caught on fire and there was an explosion. The contact sustained bruises on the left hand that required medical attention. A police report was filed. The vehicle was towed but no further information was available. The vehicle was not diagnosed or repaired. The manufacturer was not notified of the failure. The VIN was included in NHTSA Campaign Number: 23V651000 (SERVICE BRAKES, HYDRAULIC). The failure mileage was unknown.

The battery lasted me up to August 2024 before I received an error saying that my car won’t charge any more and it’s at maximum charge. The contact received notifications of NHTSA Campaign Numbers: 24V051000 (Electrical System) and 24V554000 (Latches/Locks/Linkages, Electrical System); however, the contact was not able to perform the over the air software updates. The contact called and informed the manufacturer that the current operating system software was out of date. The contact stated that the operating system software version was 2023.44.30.8 and the newest version was 2024.20.3. The manufacturer informed the contact that the operating system was adequate. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The contact had not experienced a failure. Parts distribution disconnect.

The hood latch failed and caused the fronk to open and smash the windshield. This happened while the car was on a trailer being transported. The front hood had opened multiple times while in garage over the last few months and unfortunately it decided to open while on the trailer. The hood flew open and smashed the windshield and caused $6k worth of damages and tesla will not pay for the fix even though there is an active recall for hood latch on my tesla Extremely scary and dangerous that this can happen on a brand new 2023 car

We went to Apple pick farm, Homestead Farm, Maryland on August 25th. While we are returning back around 6pm (Fully sunny, no rain and vision clear). After 1 mile from Hoestead Farm, Tesla autopilot hit the side road block and it dragged with scratch and dent on right side of the back door. (That driver can not even control.) Short curve make it scratch. I tried to reach tesla customer care but its did not work. No one pick to answer.

I was driving down I90 with no one in front of me and a clear road. Unexpectedly, the right side passenger airbags exploded and struck me in the face and shoulder. The car immediately braked and I saw a semi truck pass me on the right with pieces of what appeared to be tire debris flying in the air. I pulled over and my car become inoperable. I inspected the outside of the vehicle and there was no damage or collision at all yet the passenger side airbags went off.

In May 2023, after I took my car from the Tesla service center in Santa Ana, I realized there was an issue of shaking and shredding from the motor, and it got worse when I pressed the brakes. That happened when I drove the car down a steep road from [XXX] down to [XXX] in the city of Laguna Beach; Then I drove the car to a different road from [XXX] , then down from [XXX] to [XXX] and [XXX] , where it is down steep as well. Then the same issue showed up, and I couldn't keep pressing break because the shaking got worse, so I let the car roll with shaking till it reached the straight road and stopped. Since then, I have started to avoid steeping, but once, it showed up on another downhill street, which was not as steep as the others. Still, it was a longer downhill, coming from [XXX] down to [XXX] till it splits to [XXX] and [XXX] . At that time, I did not have time to take it to the service because I was very disappointed with them after taking it for several issues several times. They never fixed them, and because I had my mother, who was in critical health issues, I needed to be with her all the time and take her to her appointments for her surgeries and wound care for several months until this year in March. Then I realized that the issue keeps changing from existing to disappearing to existing again. Still, instead of starting at a speed of 1 mile, it begins at 9 miles and disappears at a speed of 11 - 12 miles. It does not have a high level of shaking, but it still exists. I am unsure if that changes with the different updates they do through the air for the car. But the issue still exists, and the shaking levels change. Lately, they have taken it seriously after several attempts to report it and bring it to the service center. After nine days, they concluded that they had tried a similar car. This issue does exist, and it is normal when I have tried multiple cars, including their several demo cars at different times. I bought a 2023 Tesla Long-range Y in June 2023 at Tesla Santa Fe, New Mexico. I have had trouble with the brakes applying on their own during cruise control. It was very scary the first time it happened, but luckily there was very little traffic on the 2-lane highway I was traveling on. The Tesla dealer in Santa Fe told me they don't have a software fix yet and he simply avoids cruise control. After a few months I cautiously tried cruise control again and got away with it...but the third time, months later, it happened again. When the free month trial of Auto Pilot was installed, I did not intend to use it. Toward the end of the trial, I used it on [XXX] coming home to Las Cruces and realized it might actually auto-brake on the freeway, which could be a disaster. After that trip a Tesla friend shared his near-accident-causing experience with the cruise control braking. Another issue is that the key card didn't unlock the car, then wouldn't start the car. I called Tesla and was told I could reset the key card using the Tesla App, but it did not work. The next day I called Tesla support; he ran the diagnostics and said "oh yes, your car needs to be re-booted". He did so, and the key card worked to lock the car, but not to unlock or start it. I asked him to send me a new key card. "We sell a set of 2 for $45" (this is what came with the car). I told him I paid $53,000 for the car less than a year ago, it did not come with a charge cable, and now the key card didn't work? He looked to see if the key card was covered under warranty and found it was not. Car warned that Drivability - Acceleration and top speed reduced/Performance may be restored on next drive but gave no opportunity for me to pull over safely. Felt like someone pulled the e-brake during regular driving speeds and prevented me from safely slowing down, steering or maneuvering the vehicle into a safe spot. This occurred during a rainy night with my wife and 3 daughters in the vehicle who were terrified and still afraid to drive in the vehicle. Tesla replaced the front drive inverter and about a month later the same issue happened again on a busy road around 1pm in the middle of traffic and this time the rear drive inverter had to be replaced. Myself nor my family have felt safe in the vehicle but feel stuck with it.

(1). Electrical wire related to passenger airbag function is allowed for rodent chewing. I was told by technician that if this wire chewed by rodent or broken, passenger air bag will not function at all when there is an impact or an accident. I'm not really sure whether the driver will be even alerted whenever this wire is broken. This is simply a design fault because such an important wire should not be placed in a vulnerable location where rodent can chew it. I have never come across this in any car in my life so far. (2). If an airbag does not function, a passenger could get hurt or die (3). I asked the service team about the design fault and filled their service visit survey as well, but no one answered me so far. (4). Service technicians inspected the broken electrical wire, repaired it, but there is no guarantee that rodent can not chew it again because repaired wire is at the same place as before. (5). There is no prior warning for me. Service technicians came over to replace the passenger seat. Once they replaced it, the senior technician tried to check all the system using his laptop and it appeared that he noticed a fault on his laptop screen. So, he opened the frunk and found the broken wire. (6). Instead of replacing this part with OEM part he repaired it and even charged me for the cost of repair though my car is under warranty. This is the design fault and this wire can be chewed again and manufacture can keep charging the customers till the life of the car.

I then stepped on the brake pedal to bring the car to a complete stop and turned on the hazard light. I bought Model Y on December 1st and after two weeks the car suddenly stopped working with a message that electrical system failure unable to support all function and vehicle will stop, it only gave a minute or two before car getting stopped and not responding. The vehicle got towed and they diagnosed the front inverter getting too much current causing the fuse to blow. I got the car fixed and after 2 months same thing happened [XXX] with same message and car stopped working and only gave a minute to park. The car is in repair shop with Tesla. This raises a serious safety issue where the driver only has a minute or two to take the car off of the road and park at safe location. Imagine if you are on a busy highway and you only have a minute to pull it on the side. You might be stuck in the middle of the road and waiting for someone to hit your car. NHTSA needs to closely look at it and take up the issue with Tesla and also raise the question on the quality of car produced. I brought the car home after purchasing and came back out to the car in the garage, a notice appeared on the screen and the car shut down locking me in. I had to call Tesla who instructed me to use the manual release on the door to get out. This occurred on the first day of ownership and had to be towed to Tesla for repair the next day. From my understanding looking online one of the parts that was replaced (pyrotechnic battery disconnect - please refer to attached repair invoice) was involved in a Tesla recall earlier in 2023. I view this as a safety concern and am fortunate such an event occurred while parked in my garage and not while driving.

Bought this Model Y lease and had it delivered from Jacksonville to miami after sales guy convinced us from getting the 2024 model since this one was cheaper and was a long range (I wonder why). On that same night, I took the car to the gym and coming back, it shut down on me on the middle of the road. (Screenshots of the error attached). On [XXX], Model Y shut down on my wife entering her aunts complex picking up the kids. Same day wife was driving on a 3 lane main road and the car shut down on her in the middle of the road with cars passing by. Model Y made us get out the vehicle and re-enter the vehicle multiple times putting us in danger, not to mention the abrupt stop could’ve been worse if this was on the highway and/or God forbid an accident. Took car to service and in 1-day Tesla claims the inspected and replaced the inverter and everything should be up to code. I told them that my wife and I don’t feel comfortable driving the same vehicle. They say “they completely understand where I’m coming from and that the vehicle is now performing as designed. I hit a rock at approximately 35 mph that got projected under my car and caused a bad leak in the battery coolant system. Received a low coolant warning about one minute after the incident. The alert indicated that the car was safe to drive but to schedule service asap. Tesla instructed me to drive to the local SC that is 15 min away when though enough coolant had leaked to render the user display black. I had no turn signals, no speedometer, nothing except the ability to move the car. Tesla alerted me the next day that the damaged occurred at a poorly protected point where the coolant lines connect to the HV battery and that the battery with less than 20K miles needs to be replaced. As a result, they discharged the energy from the battery to avoid a “thermal event”. In doing some research, I discovered a number of documented cases of similar failure due to the fact that a plastic part is used to protect this critical area and is known to break and not offer ample protection to the coolant lines and front drive system. I’m very concerned that had this occurred on the highway and I continued to drive that a “thermal event” would occur and put me in grave danger of a vehicle fire. This area is too critical for proper vehicle operation and safety around high voltage components to use plastic parts to protect them. I also discovered that earlier model Model 3s used a heavy cloth for this role before changing to the plastic version.

We bought a 2023 Model Y from the Mesa Tesla on 12/17/23. The staff rushed orientation (we are first-time EV owners) saying they were so busy that we could only get 15-30 minutes max for a complex vehicle. They told us calibration is normal and to reboot the Y if anything happens and showed us how. Once we drove off the lot the A/C went out and we got calibration prompts saying Pull Over. We pulled over less than 0.5 miles out and the A/C came on. Because of the bullying at the dealer we we afraid to go back and decided to go home. Again the calibration prompts came on and Pull Over. My wife was driving with my daughter and my in-law's and I was behind her in another car with our son. She suddenly started slowing down on the road because the vehicle would not let her accelerate. I pulled over to the side but she could not. I went to her and rebooted the Y, per Tesla's directions, and the car went dark and locked me and my in-law's inside in moving traffic (my wife had exited with my daughter). We had to call Gilbert police to direct traffic around us (had many angry drivers until Gilbert police came not understanding why a Y with a paper license tag was stopped in the road) and a tow truck for help. Two other drivers did stop to help too. I found the manual release but my in-law's could not. The tow truck driver had to climb (we had previouly rolled the windows down with the A/C issue) from the front seats to the back to get my in-law's out and paid $140.00 to have our new Y towed back to the dealer. For one week the story keeps changing - there is a coolant leak, no coolant leak but bad sensor, now the PCS is bad. I do not understand how the government can sanction such vehicles. We saved for a long time to purchase what was a dream vehicle for my wife and our famil'ys life was endangered within minutes. We are terrified of this vehicle and being around others like it on the road.

I purchased the car and took delivery on 12/29/2022 where immediately I noticed while driving the car home that there was a high pitch creaking sound from what I think is the front drive unit of the car. I have brought back the car to the service dealer over 8 times to rectify the concern of the noise. There were many service technicians that heard the sound while I test drove before the car went into service and they all acknowledged the sound and said it was not normal. However, the service techs had picked and choose which item they thought on the car that created this sound and this is documented on the service receipts/invoices. They kept taking my car apart and replacing parts on the car and when I would pick up my car the sound was still present int he car. Now I have had the car for 11 months and now the sound is STILL present and Tesla has no fix for this. The last service I brought the car in for for this high pitch sound and they replaced the front drive unit and still the noise is present.

I purchased a Model Y Performance on October 31, 2023. I was driving the vehicle on 11/1 and I received an error that my rear motor was disabled and my top speed was reduced to 68 mph. This occurred on a high traffic highway. Took the car to the dealership and they replaced the rear motor inverter. Vehicle only had 130 miles on it. This should not occur in a brand new vehicle. I felt unsafe while driving it in this condition.

On October 25, 2023, between 10:40 pm and 10:50 pm, while driving the car on the 105 (Century Freeway), the computer system suddenly went completely black. The car did continue to travel and never did stop. I did not make any lane changes. I am unable to say if the the head lights continued to function. The computer did reboot after about a minute or so. The car is brand new. I've only had it since September 16, 2023. I am reporting it here in case it is a concern. I've read that the computer could only have been rebooting from an update that I had installed earlier. Don't know for sure, but it was quite a hair raising one and a half minute. Additionally, there are times when the car's adaptive cruise control does not recognize red light intersections. It would accelerate instead of slowing down to stop. During those times, I would need to slam on the brake to make it stop. I have not relayed any of this information to Tesla but I intend to very shortly.

On August 28, 2023, driving back from golf at Tijeras Creek, the vehicle noted several faults: cruise control disabled, autopilot disabled, parking brake disabled, and others. We parked in Newport Beach and the vehicle failed to move—appears the parking brake locked up. The vehicle was taken to Tesla for repair; the vehicle had 4,423 miles on the odometer when it was taken to Tesla for repair. The technician states that they identified the brake fault, removed and replaced the right brake caliper.its disabled the vehicle from safe operation. September 21, 2023, the vehicle was driven locally 23 miles and the condition returned. This time the vehicle was in a shopping mall and the parking brake would not release, disabling the vehicle. Screen shots of the fault codes were taken to preserve the evidence. Eventually, the operator was able to shimmy the vehicle forward and return to the house. On October 4, 2023, only 90 miles after repair by Tesla, the exact same condition—parking brake function degraded. The vehicle, again, refused to disengage the parking brake. The vehicle suffered again the problem that rendered the vehicle inoperable. The vehicle was again towed. On October 13, 2023, the vehicle was picked up from Tesla service for repair of the THIRD DEFECTIVE AND DANGEROUS CONDITION; the vehicle was driven locally and, on the freeway, to determine if Tesla had actually corrected the dangerous and defective condition. After about two hours in varying conditions and 90 miles, the vehicle was parked in the garage overnight. In the morning, the vehicle was loaded for an outing, and again the DANGEROUS AND DEFECTIVE CONDITION occurred. This time, the vehicle could not be backed out of the garage; The tow truck was called

The contact owns a 2023 Tesla Model Y. The contact stated while dropping her son off at work, she placed the vehicle in drive(D) and the “Car Unable to Charge” message was displayed. The contact stated that the driver’s side of the vehicle had no electric power. The contact stated that the passenger’s side of the vehicle still had power. The contact stated that she could not open the driver's side door and used an emergency override switch to exit the vehicle. The contact stated that she was unable to close and secure the door afterwards. The contact had the vehicle towed to a dealer. The contact stated that the tow truck driver had to use a bungee cord to keep the driver's side door closed. However, during transporting the vehicle to the dealer, the driver's side window cracked and fell inside the door frame. The contact had taken the vehicle to a local dealer, where it was diagnosed and determined that the driver’s side body control module needed to be replaced. The vehicle was repaired. On August 12, 2023, I went to the Plano TX Tesla Delivery Center to pick up my new TeslaY model as scheduled at about 11 am. CST . The vehicle had 3 miles on it. After taking a couple of rounds around the delivery center and becoming comfortable with the default settings, I started driving back home which was 9 miles away. While on the drive, I noticed that the screen was showing “calibration in progress”. When I reached home at around 11:40 am. CST, Tesla showed a total of 12 miles on it, and as the garage door opened, the screen showed STOP as it said the calibration finished. As instructed, I stopped moving the vehicle on seeing HOLD/STOP while entering the garage from the driveway. Suddenly, the car jerked forward forcefully, as if a magnet had attracted it inside with the pedal going down. The car went into the garage and hit the garage wall, damaging the common garage wall and the other side being the master bathroom (by damaging the cabinets, big wall mirror(s), counter top etc), and also damaged the front bumper of the car. I was shocked and terrified for a while. So immediately rushed to the center again and reported the incident at Plano Service Center, who created a ticket on the same day. They said that this case would be sent to the engineering team for review. on Sep 10th, I got a reply from Service saying they have reviewed the Engineering study for the vehicle and the results displayed show that the vehicle operated as intended due to the use of the accelerator pedal at the time of the incident. Also reported to Auto insurance who came and assessed the damage to the bumper which also rattles or squeaks. The service for collision is scheduled for Dec 7th 2023 by Tesla Collision Dallas center. I was driving at a very slow speed of 1 mile per hr as I entered into the Kohls complex in Pleasanton ca. Then as I was pulling the car to a parking spot, all of a sudden I felt the car accelerated on me. My foot was always on the break and I pressed the break but the car still accelerated at a speed I had no control and it went over the parking bump and hit 2 cars that was parked opposite. I felt out of control and scary because the car was not doing as intended, to stop when pressed the break. I then pressed the park button. Then I also reversed because the car was not stopping. There was totally 4 cars damaged including my car. 2 opposite and 1 parked side of me. I could feel the sudden acceleration in the car and out of control feeling. Luckily there were no people in other car so no one was hurt. I was also not hurt. - Not sure what malfunction happened , but the break in that instant of time was not acting as it should for few seconds. Yes, the car is available for inspection - My safety was at risk as the car was no behaving the way it should and I got scared as to what would happened next. Since there were no people in the other car, there was no risk. but if there were they probably would have got hurt. - I reported the issue to Tesla center. They said they will look but never heard back anything from them . - None of them has inspected the vehicle yet. - No warning signs were there. after it hit i could hear the beep

The Tesla Model Y autoaccelerated even with me pressing the brakes. The car was on regenerative breaking and instead of stopping it accelerated lunging into the walkway and almost killing a person. The car hit another car, 2 bike racks and a tree as well as concrete slabs beofr coming to a stop. There have been reports of unintended auto acceleration from Tesla cars. The car components has not been inspected by police, manufacturer or insurance representatives. There were warninngs, especially with the car reporting that sensors were not properly calibrated. I reported this to Tesla agents and they told me not to worry about it. I am injured with a left wrist sprain as well as Neck and back injuries. I need Tesla to take ownership and pay dearly!

I was turning right from a residential street onto a busy street during rush hour. I heard a sound, which made me think I might have run over something. Immediately following the sound, an alert showed up on the car’s touch screen with the message: “Vehicle May not restart: Service required”. The car was immediately inoperable - the car could not move and it could not shift into neutral to be pushed off the road. The 12V battery of the car continued to function for roughly 10 minutes after the car became stuck, which I knew because I was able to exit my vehicle normally, the touch screen remained illuminated and I was able to turn my hazards on. It did eventually die and I became locked out of the car at that point. I was in the middle of a busy intersection with no hazard lighting at that point and the car could not be moved, so I called emergency services and requested a police officer for traffic redirection while I waited around an hour and a half for a tow. Regarding the initial sound that I heard, I checked under the car as well as in the street around it, and there were no signs of debris or any visible evidence of damage to the undercarriage of the vehicle. I can only assume that the sound that I heard was whatever failure that had occurred internally in the car. At the time of this incident, the car was charged to around 85% of battery, was only a few weeks old, had never been in any type of collision, or required any service. Tesla has inspected the vehicle and identified the malfunction as a failure of the front and rear drive inverter. The situation that I was in was dangerous to myself and to the other drivers on the road with me. A sudden and complete stop on a busy road or in a location without cell coverage could be life threatening. The fact that there were no warning lamps, messages or any warning symptoms of the failure is extremely concerning and puts drivers and others on the road with them at a high risk.

The vehicle completely locked me out. It was unresponsive to key card access as well as unresponsive to mobile app access. I was unable to unlock or open any door/latch/trunk or frunk. I’m no one was inside because they would’ve been stuck with no way out. This is a major safety concern. I happened to be in a safe area but if this were on a really hot day or if people/minors/children were in the car, it could’ve been fatal. This is a design flaw in the vehicle. If the 12V battery dies, the car locks and you cannot get in or out.

I bought Tesla in May 2023. My Tesla Model Y has suddenly accelerated in my garage on 08/29 @8.25pm EST and damaged my garage front door/drywall and broken my stair case in pieces. It was phantom acceleration. I have video from my ring security system camera video which shows clearly that I was parking in my garage extremely carefully, however the Tesla Model Y suddenly accelerated beyond my control (I was not accelerating) and put me under risk and damaged my property. I have auto insurance to cover my Tesla car damage. However, my biggest fear is how can I use this Tesla Model Y when we see this life threatening behavior. This unintended acceleration can put my life and others life into danger. I would like help from NHTSA to ensure my Tesla Model Y is defect free. I just bought couple of months ago. I would like to request NHTSA to help me get this product replaced from Tesla motors. I am afraid to drive this vehicle now. I am currently going through a big shock, trauma and fear that I may have severely injured people on the road or my family members in garage if this unintended sudden acceleration would have happened around them. The damaged vehicle is outside my garage. It is not drivable. Vehicle front bumper is completely damaged and fluid has been leaking. I have filed insurance claim. Today at 8:30am while Driving, my brand new Tesla: the touch screen went completely BLANK. I did not know how fast I was going. I was beyond scare. The safety FAILURE of this car is at its limit. What's next? The screen was blank the entire time while driving to a safe destination in Pasadena, CA. While I pulled over, I still had no way of know whether the car was in park, I could not control any devices as the screen was blank. (BLACK SCREEN). I called my husband who is a techy and he told me to hold down the two buttons on the steering wheel for 15 secs. which did reset the touchscreen. This car has too many BUGS and I do not want to die in this car. I can't pull over safety on the freeway all the time nor do I want to wait and see what happens down the road. I do have an appointment TOMORROW for something completely different (the dash cam ) but have been in contact with TESLA through the text portal and they have been made aware of my issueS. THIS IS A SERIOUS SAFETY CONCERN AS THE CAR IS ONLY 8 MONTHS OLD. MILES ON THE CAR 7,656 THERE IS A LEAK IN THE TRUNK CURRENTLY. THE DASHCAM DOESN'T WORK. The dash cam will turn on if the glovebox is open but it doesn't always work, however when the glovebox is closed it does not work at all. 1. Wireless charging extremely slow., connected charging is also slow. 2. Safety Issue (Camera): driving side left camera and blind spot monitoring camera specially sometimes stopped working automatically during driving in the highway or city, it’s pretty dangerous Specially in the highway driving. During my long drive over July 4th weekend between 3-7 July happened several times, it caused auto steer mode forced disengage 3 times. occurrence 8/8 between 12-3 am it happened about 6 times. It happens on and off almost everyday in some part of drive. 3. Safety Issue Phantom Braking, in Auto steer, brakes are suddenly getting applied when even are no objects in the front of car for no reason. Aug 4, it happened about 4 times between 7-9 am.(around 7:30-7:40)Due to this big accident in the highway could have happened with Truck, as Truck was in normal speed 65 mph and due to sudden brake, vehicle speed came down to 35-40 mph, took control and move car out of road avoiding accident. This happened earlier instances as well during driving in the highways. But this time, I avoided death but got injuries. But my car after updating the new Sft on July/2023 a battery range max charge of only 285 miles for 100%. Please have a solution to fine Tesla company by fraud and lying to consumers badly. I had to call 911 and tow truck. I towed it to tesla and they said the rear unit failed… car was just purchased new a few months before this.

Leased a brand new Tesla Model Y @ 01 Apr 2023. I put @ 2100 miles on the car. On @ 01 May 2023 I get a fault stating vehicle could not be charged. Took the vehicle to Tesla for repair on 04 May 2023, and was told that it needed a new HV battery. Original completion date was to be 20 May 2023. This was pushed back to 26 May 2023 and has now been pushed back to 31 May 2023. This is a vehicle that was a month old. Asked about a loaner, and was told that there were none available. I will have been without my car that I am paying a lease payment, insurance payment and self drive subscription payment for almost a months worth of time. Was told by a Tesla rep that no compensation is available.

Then stated it two or three more times, stating the vehicle is safe to drive under 50 miles, and no steel grades. I do not feel safe driving this vehicle at all at this time, and feel that Tesla needs to replace my vehicle as the vehicle is a week old, and the motor isn’t working. This is a huge safety concern as it could’ve caused a major injury accident, especially if I was on the freeway.

This is the second instance of this issue happening, first at 1800 miles ODO, and second at 4690. Both instances, when driving, the vehicle suddenly and violently shuddered to a stop. The first instance happened as I pulled out from a stop sign to take an unprotected left onto a street. In the middle of the intersection, the car shuddered to a stop violently and a lot of warnings and alerts came up on screen. Cars moving both directions had to suddenly stop to not hit my car dead in the intersection. The gear stalk inputs to put the car in drive or reverse and allow function of the vehicle did not work. I pushed the park button on the right stalk and then moved it into drive, which it allowed. I was then able to limp the car out of the intersection (with power vastly reduced and speed reduced) and await a tow. The second instance I had pulled into a parking lot at around the same speed (~15 mph) in which the car again shuddered violently to a stop with no ability to override. Traffic behind me had to brake as to not hit my vehicle which stopped in the middle of the lot. Again the car was inoperable until this time locking and unlocking the car from outside of it and reentering to limp it to a parking spot for tow. Safety of myself and others were put severely at risk suddenly and without warning from the car stopping itself. If this were to happen in other, less fortunate scenarios, risk to human lives is great. Problem has been confirmed by Tesla service center, but not reproduced while in their possession. The vehicle was inspected by Tesla service after the first incident and was explained to have found no faults with any hardware, but found a corrupt line of code in the firmware (no update or firmware change was noted to be available). Warning messages and alerts populated at time of incident and no sooner (no warning is given prior to issue). Alerts include power reduced, rear motor disabled, vehicle shutting down, automatic emergency braking disabled and more.

I got the delivery of the car VIN [XXX] on March 10 7 PM and got the car out and drive to home from Kennesaw tesla delivery center. On my way home the car stopped right after crossing the light signal at the intersection of Medlock bridge rd. and parson rd. And it does not move, I had to turn on the emergency lights to save myself from accident and the cop had come after some time. We were lucky to save ourselves without an accident as the car stopped all of the sudden and there was no car behind. I think this is a problem with the car and needs to be inspected and nobody should be in this situation in the future. I want to raise this as a safety concern please for Model Y in the very first hour after the delivery of the car. After just 250 miles on the car and just after one month of the purchase, my wife along with my 4 years old daughter was driving to get my 7 years old son from the school when it suddenly stopped on the road with displaying errors on the screen as follows, the car battery was sufficiently charged, later from the inspecting what it appeared that the "rear drive unit" of the car got screwed up. Warning and error messages were: "Acceleration and top speed reduced" "Power Reduced" There was a huge school traffic jammed behind the car, luckily other car owners offered help but the car was not moving, it stranded in the middle of the road. We contacted Tesla they towed the car after one hour of the call, they kept the car for a week and told us the whole new "rear drive unit" need to be replaced. After a frustrating experience they offered no replacement but to fix the car. Driving 70mph on the highway, car abruptly clunked and three warning messages appeared. APP_W009 Automatic Emergency Braking is disabled; DIF_A062 Power reduced; and BMS_a170 Acceleration and top speed recuded. Car immediately dropped to 40mph and gave me enough time to exit the freeway to the shoulder. After putting in park, the car would no longer shift into any gear. Soft reset and full reset, exiting and re-entering the car didn't fix the issue. Tow required. Failure of rear motor (inverter) was cited in the service center. Seems to be a common issue across Model 3 (as far back as 2017), Model Y (deliveries in Nov/Dec 2022) and Model S (the last 3-4 years). Very dangerous if someone had hit us when it decelerated hard on the highway. Traveling with a 6yr old in a car seat. Only 500 miles on the car.

Replaced rear drive unit inverter and verified vehicle now operates as expected. Safety: car speed reduced to 68mph max and the regenerative braking barely worked. All functionality like cruise control/adaptive were restricted Yes - reported to Tesla dealer/service center and had verified and fixed under warranty No warnings prior. Driving down the road I heard a loud clunk and then a warning appeared saying power reduced, rear motor disabled. Ok to drive.
