The contact owns a 2023 Tesla Model Y. The contact stated that while driving at approximately 60 MPH in light rain, the vehicle suddenly braked and causing the vehicle to lose control. The contact stated that the vehicle was on cruise control and auto drive assist. The contact stated that the vehicle veered to the left, crossed two lanes of traffic, and crashed into the highway divider wall. The contact stated that there were no warning lights. The contact stated that the left front end had crashed first, and then the driver's door and left front fender had crashed into the dividing wall. The contact stated that the driver's front air bag had deployed and burst; there was a large tear starting from the top of the air bag all the way to the steering wheel. The contact stated that he had burns on his hands due to the air bag explosion, bruises on his chest, and right arm. The contact stated that the police and emergency services arrived. A police report was filed. The contact stated that he was transported to the hospital, and the vehicle was towed to a manufacturer's collision center. The contact stated that the manufacturer's collision center downloaded the collision information from the vehicle's main frame and shared the accident report with the contact. The failure mileage was approximately 45,000.

While operating my Tesla Model Y in a commercial parking lot at a T-intersection controlled by a stop sign, the vehicle unexpectedly and automatically accelerated without driver intent. The incident occurred during a low-speed maneuver. Upon the unexpected acceleration, I immediately attempted to regain control by applying the brake pedal repeatedly and firmly. Despite clear manual brake application, the vehicle did not decelerate as expected and continued to move forward. I also attempted to stop the vehicle by pressing the Park button, but this action did not stop the vehicle. The vehicle continued uncontrollably for approximately 150+ yards, traveling over curbs, striking a fence, and ultimately colliding with the garage structure of a residential home. Throughout this sequence, the vehicle remained unresponsive to braking and stop inputs. Vehicle data later confirmed that brake pedal application was detected prior to impact, yet braking response was insufficient to prevent continued motion. At the time of the event, the vehicle recorded system fault states, and collision avoidance systems did not activate. Despite significant structural impact and property damage, airbags did not deploy. The incident raises serious safety concerns regarding: •Brake-by-wire system execution during fault conditions •Pedal input arbitration between accelerator and brake •Failure of collision avoidance systems to intervene •Failure of restraint systems to deploy during a severe impact •Vehicle behavior when control systems are in a fault state This event occurred without warning and despite appropriate driver response. Based on vehicle data and system behavior, this appears to be a system-level failure rather than driver error.

When driving on slippery surfaces, if I release the accelerator pedal—even slightly—the vehicle automatically activates regenerative braking to recapture energy. On ice, this sudden deceleration mimics applying the brakes, which can cause an immediate loss of traction and control. This happened to me recently while driving in snowy and icy conditions. As soon as I eased off the accelerator, the car engaged regenerative braking, the wheels lost grip, and the vehicle began to slide, similar to what would happen if hard braking were applied on ice. I momentarily lost control of the car, creating a serious safety hazard. This behavior is not intuitive and can surprise drivers, particularly in winter climates. Unless regenerative braking is manually adjusted or turned off—something not every driver knows to do—the system can unintentionally put the vehicle into a skid. I am requesting that NHTSA review this issue. A software modification or warning may be needed so drivers are not placed at risk due to unexpected regenerative braking on slippery surfaces. Pulled into hospital parking garage. Stopped to pull parking ticket. When the parking gate was raised, Tesla immediately took off at very high uncontrolled speed. Basically a runaway vehicle. There was no accerlation on my part. Brakes did not work. Hit parked car in the garage about 300 feet from the parking gate. I was taken to hospital with broken wrist. Car has been totaled.

Driving our 2023 Tesla model Y east on [XXX] on [XXX] near Pecos TX. Car suddenly lost power and started hard braking. We were rear ended by a large pick-up truck that was following as we were both traveling approximately 80 miles per hour when this occurred. The car was towed back to Dallas from Big Springs, TX (approximately 280 miles). The car's rear-end damage was such that my Tesla was deemed totaled by the insurance company. This power failure and hard braking had happened to me repeatedly over the last few years. When I spoke to Tesla at the Dallas service center about this issue, they acted as though they had never heard of this happening to anyone. We were both shaken by this accident but other than some soreness the next day we are physically OK. Tesla repair estimate was approximately $22,000. I was forced to buy another car. Breaks unresponsive for 3 seconds, smashed into 4 parked cars, creating a Chain-Reaction crash, my Tesla Model Y Totaled, Airbags deployed. Burlington Police Department arrived, took statements, filed report, no copy yet.

I am writing to formally lodge a complaint against Tesla, Inc. regarding multiple critical safety failures that occurred with my Tesla Model Y (VIN: [XXX] ) during a collision on Saturday, [XXX], while I was traveling eastbound on [XXX] from the [XXX] exit to the [XXX] exit in Pennsylvania. At approximately [XXX], I was driving in the left lane when a BMW SUV abruptly cut into my lane ahead of me, accelerated briefly, and then suddenly braked. I immediately applied the brakes multiple times in an effort to stop my car. Even though there was sufficient space to stop safely, the braking system failed to respond, resulting in a collision with the BMW in front of me. Despite my efforts to avoid the impact, the following dangerous and unacceptable failures occurred at the time of the accident: 1.Failure of Regenerative Braking / Deceleration: When I lifted my foot from the accelerator, the vehicle did not decelerate as expected under Tesla’s regenerative braking system. 2.Failure of Braking System: Despite pressing the brake pedal twice—including a heavy application—the braking system failed to prevent collision, contrary to Tesla’s advertised safety performance. 3.Failure of Collision-Avoidance Features: Critical safety systems—including Automatic Emergency Braking, Forward Collision Warning, and Obstacle-Aware Acceleration—did not engage to mitigate or prevent the impact. 4.Failure of Airbag Deployment: Despite the severity of the crash, none of the airbags deployed, exposing me to serious risk of injury and raising grave concerns about the integrity of the Model Y’s occupant protection systems. 5.Post-Collision Failure of Regenerative Braking: Following the accident, when I attempted to drive the vehicle, the regenerative braking system was completely non-functional, creating a continuing safety hazard and rendering the vehicle unsafe to operate. Driving on I 880 south on the fast lane. The car suddenly braked for no reason and began to shutdown. It warned me to pull over before complete shutdown. Luckily there was space on my left and I was able to rolling stop. Nothing on the screen was working. Turned on hazard light. Got out of car with my phone. Accessed Tesla app and ran battery check. After couple mins the error went away. Tried to slowly move with hazard light on. It appeared to have fixed itself. Slowly merged back into freeway when it was safe. Attached notification when I tried to schedule service request.

The vehicle in front of us came to a sudden stop, but my Tesla failed to issue any alerts or activate automatic braking. When we attempted to brake manually, the car did not allow full braking and continued moving forward, ultimately causing a collision. This strongly suggests a malfunction in the Tesla’s safety or braking system. As a result, I’ve suffered significant financial losses due to the vehicle being totaled and damage caused to other vehicles/other persons injuries involved. This incident is also likely to lead to increased insurance premiums moving forward.

On [XXX] I was driving North on [XXX] at approximately 70 MPH when my 2023 Tesla Model Y Long Range brakes violently applied by them self twice in the space of a few seconds, the sudden unexpected brake application was a shock to me causing me almost lose control of the vehicle and was almost rear ended by a truck that was a few car lengths behind me. My car was in manual mode with the "Emergency Braking" switch enabled (no self drive or cruise control), the weather was sunny and dry and there were no traffic or obstacles in front of my car. The onboard dash cam briefly captured the incident. The problem did not reoccur for the remainder of my drive home. There was no warning prior to the incident, I believe that emergency brakes may have flashed on the screen ( not sure because I was busy reacting to the situation). The car has not been inspected as of yet. I now have to disable the Emergency Braking function each time I drive my car because unfortunately the system defaults to the on position and I don't trust it. This issue appears to be a problem with the system that provides visual information to the Emergency Braking System. I'm making this report because this is an extremely dangerous problem that needs to be addressed by Tesla before it causes injury or worse. This is my 4th safety report about the same issue: unexpected braking while on cruise control. Since my last report, Tesla offered another free month of auto pilot but I did not take it for fear of this braking issue. On Tuesday [XXX] I was returning to my home in Las Cruces NM from Colorado. I was on [XXX] toward Albuquerque, and there was very little traffic. I decided to see if I could get away with the cruise control. As I was pulling into Bernalillo, there were some oncoming cars in a left-turn lane at a signal, and my car braked automatically...enough to lurch me forward. I decided to stop at the "new" Tesla dealer in Bernalillo (I got my car in Santa Fe) and ask about the automatic breaking and whether they'd fixed it. The 2 service employees looked at each other and one said "No". I then asked about whether I needed to re-boot my key card since my screen went blank while I was in CO, during a snow storm, parked. I luckily had my phone and on the Tesla App, under Roadside Service, it gave me the option to re-boot the screen! I did so. The serviceman said he didn't think the blank screen had anything to do with my key card, but I could re-boot it if I want to. (A prior complaint also reported the failed key card in less than a year and Tesla's unwillingness to replace it). I am submitting a complaint regarding a serious safety issue with my 2023 Tesla Model Y. The vehicle experienced a sudden and complete power loss while in operation, creating a hazardous situation. Below are the details of the incident: •Component/System Failure: The vehicle’s electrical system failed, leading to a shutdown. The following alerts were displayed on the screen: •VCFRONT_a191: Electrical system power reduced – Vehicle shutting down •VCFRONT_a192: Electrical system unable to support all features – Switching off features to conserve energy •BMS_a151: Vehicle may not restart – Service is required •DI_a227: Please wait while high voltage system powers up •Safety Risk: The unexpected shutdown posed a serious safety risk. If this had occurred in high-speed traffic, it could have led to a crash, endangering my safety and that of others on the road. •Reproduction of Issue: The issue has not been reproduced yet, but it is an ongoing concern. I have not received a resolution from Tesla at this time. •Inspection by Authorities: The vehicle has not yet been inspected by Tesla, law enforcement, or insurance representatives. However, I am willing to make it available for investigation. •Warning Signs: There were no prior warning signs before the failure. The alerts appeared shortly before the vehicle lost power completely. This issue raises concerns about the reliability and safety of Tesla’s electrical system. I urge the NHTSA to investigate this matter to prevent potential accidents caused by similar failures in other vehicles.

While in an automatic car wash with the vehicle in "car wash mode" the vehicle suddenly stopped free rolling and the vehicle behind the Tesla Model Y crashed into the Tesla. The Brown Bear car wash reported that these types of incidents are common at Brown Bear car washes and that they ocurr when the drivers of Tesla cars remove their seat belts while the car is in the car wash.

After leaving the supercharger (car charged to 92%) I came to a stop light. The car was NOT in autopilot. I took my foot off the accelerator pedal. The car had nearly come to a complete stop when it starting accelerating again. My foot was NOT on the pedal. I tried to brake and the brakes would NOT work. I turned the car to try to avoid hitting the car in front of me while pressing on the brake. Brakes still would not work and I slammed into the car in front of me. I believe that the car was not able to put the energy from the acceleration back into the battery (regenerative braking technology) and therefore pushed it back into acceleration mode. I also believe this caused the brakes to not work. I know there have been many recalls in China and other complaints of the same issue. I've had the car for 2 years and had never had this problem before. Another note when the accident happened the screen was showing a software update that wanted to be installed. This was a very disturbing and rattling happening and I'm not worried to drive the car after a super charge.

Was driving in traffic and when the cars were coming to a stop I was putting my foot on the break to prepare for a stop but it kept creeping forward and I’m pushing down the breaks as hard as I could but nothing. It ended up hitting the big rig in front of me. The mode of my breaks is on hold it’s always been on hold I even tried to push the parking button but it would not budge, continued forward the collision stopped the car

At 10:11 PM driving on 95 south at 78 miles per hour the car stopped on its own violently braking and swerved across the intersection. Then we kept getting alerts about the cameras. I have pictures of the alerts. There was no obstruction on the highway. Car just braked for no reason. Very scary

Date of incident 9/9/24 11:15 AM 2023 Tesla Model Y traveling north bound I-15 using Tesla Autopilot intending soon to exit Pioneer Crossing / American Fork UT in the far right lane — vehicle made sudden move to right glancing off wall causing scraping damage to right front quarter panel, and hub cap. Although I stepped on the brake in attempt to take control of the vehicle I immediately experienced uncontrolled acceleration and difficulting in steering. I chose not to exit and fought to keep the vehicle in varying freeway lanes attempting to brake to which the vehicle continued to attempt to accelerate against the braking, I also simultaneously and frantically continued to push the right stalk upwards in attempt to disengage Autopilot assuming that was causing the unintended acceleration. I was fighting to keep the car in a lane with faulty steering at time awakwardly steering to avoid slower moving vehicles. I eventually ended up coming to a complete stop in the middle lane of I-15 north bound 9/10ths (nine tenths) of mile north of my intended exit (mile post #278 Pioneer Crossing). Because of faulty controls I was afraid to try to move the vehicle to the side — I immediately put on emergency flashing lights and called 911 on my cell phone. While nervously waiting for emergency services approaching vehicles slowed and made their way around me — per instruction I stayed in my vehicle with seatbelt on. One large pickup truck struck my vehicle driverside rear as he passed by and failed to stop. Utah Highway Patrol and Fire Department Ambulance arrived within approximatley ten minutes. Two highway patrol officers stopped traffic and assisted in me slowing driving my vehicle to the center emergency lane. One office stayed with me until tow. He commented the right rear wheel appeared not aligned correctly suggested the rear axle was possibly broken. I was not injured beyond being extremely frightened. No passengers.

2023 Tesla Model Y has frequent phantom braking issues when using cruise control. Let me clarify I am not using any sort of auto-pilot or self driving options. I am simply using cruise control. The incidents that cause many braking issues are unjustified to the point I have no idea what the Tesla may have recognized to make it slam on brakes. Very dangerous

We were about the park in the marked parking lane and the vehicle unintedly accellerated, the brakes did not work and we saw message "Automatic emergency brake not available" and Air bags not available. We tried to control the vehicle to avaoid crashing into the porch of the infront apartment and steared the speeding vehicle and the vehicle hit the pillar of the aparement and finally came to halt few meters away in the parking lot as the front right wheel broke. 1. The ADAS Automatic emergency breaking seems failed 2. Forward collission warning seems not working 3. The safety of the driver and the passenger were at risk while the collission happened 4. I am not sure if the similar incidents happened or can be reproduced by the dealer/manufacturer. 5. Vehicle components inspected by the Tesla collission center but no software logs inspected and they said that they cannot get the device crash/event logs. 6. At the incident/while the incident was underway we saw few message that displayed "Automatic emergency brake is not avaiilable" and "Air bags are not available"

My Tesla car (Y model) had a glitch (malfunction) that caused it to suddenly brake twice when there was no objects in the road. I had my accident on [XXX] on [XXX] when the car enabled emergency brake for no reason when I was driving in my lane with no objects in its way. I was able to gain control of the tesla back and thankfully nobody hit me as a result. The second glitch happened couple minutes later when the phantom braking spinning my car around causing it to hit the concrete barrier and causing a damage to the car. The contact called on behalf of her mother who owned a 2023 Tesla Model Y. The contact stated while the mother was driving at an undisclosed speed, the vehicle independently accelerated. The contact's mother depressed the brake pedal, however the vehicle did not stop. The contact stated that the vehicle crashed into a concrete pillar. Additionally, the contact stated that the vehicle was not equipped with a self-mode drive. The contact's mother sustained injuries and was transported to the local hospital. The contact's mother was treated for a broken right ankle, a broken rib, and a possible torn ACL. A Police report was filed, however, the information was unavailable. The vehicle was condemned as a total loss. The manufacturer was not made aware of the failure. The failure mileage was approximately 8,613.

In May 2023, after I took my car from the Tesla service center in Santa Ana, I realized there was an issue of shaking and shredding from the motor, and it got worse when I pressed the brakes. That happened when I drove the car down a steep road from [XXX] down to [XXX] in the city of Laguna Beach; Then I drove the car to a different road from [XXX] , then down from [XXX] to [XXX] and [XXX] , where it is down steep as well. Then the same issue showed up, and I couldn't keep pressing break because the shaking got worse, so I let the car roll with shaking till it reached the straight road and stopped. Since then, I have started to avoid steeping, but once, it showed up on another downhill street, which was not as steep as the others. Still, it was a longer downhill, coming from [XXX] down to [XXX] till it splits to [XXX] and [XXX] . At that time, I did not have time to take it to the service because I was very disappointed with them after taking it for several issues several times. They never fixed them, and because I had my mother, who was in critical health issues, I needed to be with her all the time and take her to her appointments for her surgeries and wound care for several months until this year in March. Then I realized that the issue keeps changing from existing to disappearing to existing again. Still, instead of starting at a speed of 1 mile, it begins at 9 miles and disappears at a speed of 11 - 12 miles. It does not have a high level of shaking, but it still exists. I am unsure if that changes with the different updates they do through the air for the car. But the issue still exists, and the shaking levels change. Lately, they have taken it seriously after several attempts to report it and bring it to the service center. After nine days, they concluded that they had tried a similar car. This issue does exist, and it is normal when I have tried multiple cars, including their several demo cars at different times. On [XXX] around 9:20 pm we were driving south on [XXX] in Vancouver, WA. The car was on cruise control (not autopilot or autosteer) at 65 mph. The car braked really hard and dropped the speed to nearly 20 mph. We do not know why the car braked, there was no one in front of us. Fortunately there was no one behind us other us we could have been rear-ended. I quickly took control of the car and increased the speed. We have not driven the car on highway since then or taken the car to the dealership yet. There was a message on the screen after the incidence but it disappeared before we could read it. I reported this issue previously (11589255) and am providing an update: I took my car to the nearest dealer with my less-than-1-year-old non-functioning key card that TESLA told me I had to replace at a cost of 2/$45. I mentioned to the dealer, who is kind an helpful, about the automatic braking in cruise control. I had already disabled "Apply Brakes when Regenerative Braking is Limited" under Dynamics on the screen. (see photo) He showed on the screen, under Autopilot, that I could disable Automatic Emergency Braking, which I did. I was concerned that this warning pops up: "The vehicle will not automatically apply the brakes even when a collision is detected" (see photo) but he suggested I try it to see if it would solve the cruise-control braking. I was hopeful, and disabled it, knowing that it might take a while to know if that solved the problem. I drove the 30 miles home on the frontage road instead of the interstate, and when there were no other cars around, I set cruise control at 50 mph, the speed limit. I came to a series of small hills in the road, and was reminded of the first time the automatic braking happened, on a 60 mph road with longer amplitude hills of greater height. Amazingly, it happened again! Pretty significant braking and slowed me to 25. This time I waited it out to see if it would resume the cruise control at 50. It did not. I have since restored the Automatic Emergency Braking as that seems important for a crash situation. I have restored the "Apply Brakes When Regenerative Braking is Limited" since that obviously was not the problem either.

I bought a 2023 Tesla Long-range Y in June 2023 at Tesla Santa Fe, New Mexico. I have had trouble with the brakes applying on their own during cruise control. It was very scary the first time it happened, but luckily there was very little traffic on the 2-lane highway I was traveling on. The Tesla dealer in Santa Fe told me they don't have a software fix yet and he simply avoids cruise control. After a few months I cautiously tried cruise control again and got away with it...but the third time, months later, it happened again. When the free month trial of Auto Pilot was installed, I did not intend to use it. Toward the end of the trial, I used it on [XXX] coming home to Las Cruces and realized it might actually auto-brake on the freeway, which could be a disaster. After that trip a Tesla friend shared his near-accident-causing experience with the cruise control braking. Another issue is that the key card didn't unlock the car, then wouldn't start the car. I called Tesla and was told I could reset the key card using the Tesla App, but it did not work. The next day I called Tesla support; he ran the diagnostics and said "oh yes, your car needs to be re-booted". He did so, and the key card worked to lock the car, but not to unlock or start it. I asked him to send me a new key card. "We sell a set of 2 for $45" (this is what came with the car). I told him I paid $53,000 for the car less than a year ago, it did not come with a charge cable, and now the key card didn't work? He looked to see if the key card was covered under warranty and found it was not. I am reaching out on behalf of my wife who met with accident on 3/24/2024.She was driving Tesla Y model and this accident occurred due to System failure of Vehicle , brakes and Steering wheel get locked and she was not able to do anything and car hit the couple of trees on other side of parking lot . She was not using Autopilot feature and it vehicle didn't stop after applying the brake instead of stopping , its get accelerated and steering was also locked.

Driving on a two lane highway at 70 mph, the vehicle has applied the breaks extremely abruptly without reason, three times in the past week. Slamming on the breaks suddenly reduced our speed from 70 to 45 mph before I could return to the speed limit. I came very close to being rear-ended last time. The drivers behind me were angry as well, assuming that I was “break-checking” them. I am extremely concerned for my family’s safety.

Phantom braking has occurred on 4 different occasions nearly causing a rear end collision during one of them. On all occasions it occurred with no other car in front of us. Tesla Service was contacted through their app for service and we were told this is a known issue and to simply apply pressure to the accelerator. No warning lights or other was given before the incident. The problem has not been reproduced or confirmed by Tesla or independent service center?

The car autopilot system does VERY Hard brakes when coming to stop at red lights ( when there is a car in front of it) very unsafe. Also while driving in the street or freeway, mostly when passing under a bridge, all of a sudden the car pushes brake automatically which is truly dangerous and unsafe. Also when the auto pilot is on it, disengages whenever close to a curb or anything else and you have to take over control very last second to avoid any accidents. This is a safety issues. safety issue. Please fix. I took the car to dealership and both times they say it’s something called phantom braking and that they wouldn’t be able to fix the issue.

On a trip to and from New York to Canada between February [XXX], the car came to a sudden stop on the interstate when there were NO VEHICLES OR OTHER OBSTRUCTIONS IN FRONT OF THE CAR. However there WERE several vehicles behind me on 2 of these occasions and I was extremely concerned that I was going to be hit from behind. It was just like the events described here - [XXX] . Why isn’t NHTSA doing anything about this? There is no way to use cruise control without this technology which is extremely DANGEROUS. We would NEVER have bought the car had we known about this. Who wants a car where they can’t use the cruise control without fearing the safety of themselves and those behind them??!! As I was trying to make a turn on a street which I have used for months, the vehicle started accelerating on its own. So much so that it laterally flew over a median curb and hit another vehicle. I could not steer the vehicle nor stop it despite the fact that I hit the breaks several times. It truly was a very scary experience, and I am not sure what caused it. I have read of similar issues in the past, but I never thought that this truly happens. As a result of the acceleration, I got into an accident that damaged four vehicles. I am a very good and confident driver with clean driving record, and I have no explanation as to what happened, other than that the vehicle took control on its own. There were no warning lamps before, the vehicle has not been inspected yet, but I assume it will be after I have reported this very issue to the insurance company. While I am fine, my safety and the safety of those who were involved in the accident could have been extremely in danger.

After making a right turn, the car accelerated automatically and drove onto the sidewalk. The driver was hitting the brakes and attempting to redirect the vehicle with the steering wheel but both components of the vehicle were locked, causing the vehicle to lose control and crash into several things on the sidewalk (flower pot, parked cars, building pillars). The Tesla is completely dead so we are unable to retrieve any data within the vehicle. The safety of civilians on the street as well has the driver and passenger of the vehicle were at risk due to the lack of control of the vehicle. Two weeks after the accident, Tesla sent out a recall notice in regards to autosteer which can increase risk of collision, but prior to this no notice/warning was in place. The police were involved and at the scene at the time of the event and a police report has been made. No messages were seen on the screen or on the Tesla app in regards to any car mode (autopilot, cruise control, etc.) the vehicle could've been on, nor any warnings about risk of collision due to the recall.

Tesla model Y AWD 2023 Took delivery of a new Tesla about 3 weeks ago. Upon returning to Crofton Maryland from Charlottesville Virginia January 15 on a snowy day, we parked our Tesla model Y AWD in our driveway. The vehicle was subsequently engaged at the parking gear position with the operator and all passengers out, my wife then opened the trunk to unload our groceries to the garage. Within three minutes of parking the vehicle with its trunk still open as we still had more items to unload, we witnessed the Tesla sliding 30 feet from our driveway and coming to a stop due to a dipped in between the driveway and the street as well as the buildup snow. All these happened within moments of my wife moving from behind the vehicle to the garage. Luckily, there was none of my family behind the vehicle or any other pedestrian around. The incident has since been reported to Tesla.

The truck infront of me abruptly stopped as there was another collision in the lane I was on. I applied brakes but the car did not stop on time, and I rear ended the truck. I have received the vehicle data report from Tesla. Attached the report for you reference. I believe the adaptive cruise control did not slow down as it should have and when I manually applied break, the vehicle did not stop.

The vehicle's steering wheel vibrates regardles driving on highways/freeways and/or city streets. It happens since day 1 of ownership and has been owning for 4 months now. The vehicle's screen got blacked out twice when it was moving; thus, i couldn't read speed and perfom other functions such as turn on AC and/or heat. The regenerative braking/one pedal driving sometimes does not work. It sometimes accelerates itself after slowing down after accelarator was not depressed. Autopilot was not used at the times of unintended acceleration. Had submitted questions and service requests to Tesla. Unfortunately, Tesla did not provide any feedback or perform any service related to these issues.

Vehicle reversing malfunctioned , may be a software defect. Vehicle while reversing without pressing the accelerator went berserk and hit another vehicle in the driveway. Vehicle data was provided by Tesla but there is no evidence of collision nor any video recording of the incident. No issues were seen nor any warnings displayed prior to the vehicle reversing itself. However insurance deemed it as driver fault without any confirmation by the manufacturer (Tesla).

On October 24, between 8:45 and 9:05 am, I was traveling on the highway with cruise control on and the car suddenly braked going from 75 to 30 rapidly. It was only me pressing the accelerator that kept it from coming to a complete stop. Fortunately, there was no one close behind. There wasn’t any traffic around me. I was passing an exit. I reported this to Tesla and they remotely investigated but said that the related cruise software and hardware was operating normally. This is extremely dangerous! I’ve checked Tesla forums and this is a known problem that they don’t seem willing to address.

The steering wheel spun and failed. The car sped up and the brakes did not work. The car hit the curb and the left tire burst. It went straight over. Tesla Model Y delivered six weeks ago, I engaged the "Auto Pilot" for the first time on my morning commute and it worked well for a few moments. Suddenly and without warning the vehicle applied full braking force from highway speed, so hard that it glazed the rotors. This happened on EB I-10 JWO I-15, an extremely busy interchange. With possibly divine intervention I was able to retake control of the vehicle, shut down the Auto Pilot and I swiftly returned the vehicle to speed, needing only a restroom on an emergency basis. Relevant factors could include low incident angle of the sun at dawn and being in a construction zone with lane markers not thoroughly scrubbed off.

I bought Tesla and tesla insurance which monitors driving of driver to provide safety score to driver. The safety score enables monthly premiums of tesla insurance. My complaint is related to safety score which provides financial incentives to drivers to not apply break. As part of safety score provided to drivers in tesla app(screen shot attached), one of the factors used is hard breaking to penalize drivers when break is applied, forcing the driver to use regenerative breaking instead of applying break. I have observed, even when I mildly apply break the safety score is impacted. The second factor in safety score that seems unreasonable is driving after 10pm. My daughter worked late shift and would return home after 10pm. Why is this even allowed as most of the emergency workers and many others work late hours. I have also observed safety score change from 99 to 98 when the car is parked overnight, even when I am not driving.

I was driving on the freeway at approximately 67 MPH using the advanced cruise control. There was no car immediately in front of me or directly beside me. All of a sudden, the car decelerated quickly, as if it slammed on the brakes. I was able to react quickly and override the deceleration, but had there been a car directly behind me, I very likely would have been rear-ended. It all happened so fast that I didn't stop to get all of the detail, but I'm guessing I dropped down to about 50 mph before being able to respond and override the breaking. It was terrifying, as I didn't know if I could override it at first. I later read other complaints about "phantom breaking" and realized there is a pervasive problem with the Tesla.

I was driving my 2023 Tesla Model Y northbound on Interstate 69 in Indiana near the 219 mile marker. The time was around 9:30 P.M. My speed was approximately 70 MPH. I was using what Tesla calls "Traffic-Aware Cruise Control". All of a sudden and with warning, my vehicle braked extremely hard, slowing me down very fast. Luckily no one was behind me. I had no idea what was happening and looked all around for danger. After a few seconds, I accelerated and continued on. This phantom braking is very concerning to me. On August 28, 2023, driving back from golf at Tijeras Creek, the vehicle noted several faults: cruise control disabled, autopilot disabled, parking brake disabled, and others. We parked in Newport Beach and the vehicle failed to move—appears the parking brake locked up. The vehicle was taken to Tesla for repair; the vehicle had 4,423 miles on the odometer when it was taken to Tesla for repair. The technician states that they identified the brake fault, removed and replaced the right brake caliper.its disabled the vehicle from safe operation. September 21, 2023, the vehicle was driven locally 23 miles and the condition returned. This time the vehicle was in a shopping mall and the parking brake would not release, disabling the vehicle. Screen shots of the fault codes were taken to preserve the evidence. Eventually, the operator was able to shimmy the vehicle forward and return to the house. On October 4, 2023, only 90 miles after repair by Tesla, the exact same condition—parking brake function degraded. The vehicle, again, refused to disengage the parking brake. The vehicle suffered again the problem that rendered the vehicle inoperable. The vehicle was again towed. On October 13, 2023, the vehicle was picked up from Tesla service for repair of the THIRD DEFECTIVE AND DANGEROUS CONDITION; the vehicle was driven locally and, on the freeway, to determine if Tesla had actually corrected the dangerous and defective condition. After about two hours in varying conditions and 90 miles, the vehicle was parked in the garage overnight. In the morning, the vehicle was loaded for an outing, and again the DANGEROUS AND DEFECTIVE CONDITION occurred. This time, the vehicle could not be backed out of the garage; The tow truck was called

Two times in one week with two different drivers driving the same vehicle the vehicle experienced sudden unintended acceleration while parking. In the first incident this resulted in damage to the bumper into a wired fence which the vehicle did not alert but rather accelerated instead of applied regenerative breaking. Four days later the same incident happened with sudden unintended acceleration occurring while parking the car. When sending the car to Tesla for service due to the safety concern and damage this resulted in they said this was a user error which can’t be the case as this incident happened with two different drivers driving the same vehicle. When reading about this there have been numerous complaints which have shown that there is an electrical “ They show that the sudden unintended acceleration of Tesla cars may have been caused by negative voltage spikes that are logged as a fully depressed pedal, fooling both Tesla and the NHTSA investigators that there has been no hardware or software defect.”

On August 12, 2023, I went to the Plano TX Tesla Delivery Center to pick up my new TeslaY model as scheduled at about 11 am. CST . The vehicle had 3 miles on it. After taking a couple of rounds around the delivery center and becoming comfortable with the default settings, I started driving back home which was 9 miles away. While on the drive, I noticed that the screen was showing “calibration in progress”. When I reached home at around 11:40 am. CST, Tesla showed a total of 12 miles on it, and as the garage door opened, the screen showed STOP as it said the calibration finished. As instructed, I stopped moving the vehicle on seeing HOLD/STOP while entering the garage from the driveway. Suddenly, the car jerked forward forcefully, as if a magnet had attracted it inside with the pedal going down. The car went into the garage and hit the garage wall, damaging the common garage wall and the other side being the master bathroom (by damaging the cabinets, big wall mirror(s), counter top etc), and also damaged the front bumper of the car. I was shocked and terrified for a while. So immediately rushed to the center again and reported the incident at Plano Service Center, who created a ticket on the same day. They said that this case would be sent to the engineering team for review. on Sep 10th, I got a reply from Service saying they have reviewed the Engineering study for the vehicle and the results displayed show that the vehicle operated as intended due to the use of the accelerator pedal at the time of the incident. Also reported to Auto insurance who came and assessed the damage to the bumper which also rattles or squeaks. The service for collision is scheduled for Dec 7th 2023 by Tesla Collision Dallas center. What if the same happened on the main road or in the streets where I live where there are a lot of kids playing on the streets? When I think of it, I get scared and panicked and Thank God that it occurred at my residence without any outside incident

I was driving at a very slow speed of 1 mile per hr as I entered into the Kohls complex in Pleasanton ca. Then as I was pulling the car to a parking spot, all of a sudden I felt the car accelerated on me. My foot was always on the break and I pressed the break but the car still accelerated at a speed I had no control and it went over the parking bump and hit 2 cars that was parked opposite. I felt out of control and scary because the car was not doing as intended, to stop when pressed the break. I then pressed the park button. Then I also reversed because the car was not stopping. There was totally 4 cars damaged including my car. 2 opposite and 1 parked side of me. I could feel the sudden acceleration in the car and out of control feeling. Luckily there were no people in other car so no one was hurt. I was also not hurt. - Not sure what malfunction happened , but the break in that instant of time was not acting as it should for few seconds. Yes, the car is available for inspection - My safety was at risk as the car was no behaving the way it should and I got scared as to what would happened next. Since there were no people in the other car, there was no risk. but if there were they probably would have got hurt. - I reported the issue to Tesla center. They said they will look but never heard back anything from them . - None of them has inspected the vehicle yet. - No warning signs were there. after it hit i could hear the beep

The Tesla Model Y autoaccelerated even with me pressing the brakes. The car was on regenerative breaking and instead of stopping it accelerated lunging into the walkway and almost killing a person. The car hit another car, 2 bike racks and a tree as well as concrete slabs beofr coming to a stop. There have been reports of unintended auto acceleration from Tesla cars. The car components has not been inspected by police, manufacturer or insurance representatives. There were warninngs, especially with the car reporting that sensors were not properly calibrated. I reported this to Tesla agents and they told me not to worry about it. I am injured with a left wrist sprain as well as Neck and back injuries. I need Tesla to take ownership and pay dearly!

The incident occurred because the car in front of me made a sudden stop (there was a 3-car accident ahead of the car in front of me). In response to the sudden stop I hit the brakes hard on my car but my car failed to completely; it kept moving forward until I hit the car in front of me. The car was going at the speed limit or maybe 5-9 miles over speed limit when brakes were pumped to stop car as there was stop and go traffic on a bridge in bay area (CA). The car had quite a distance from the car in front of it. Despite brakes being applied, the car seemed to accelerate and brakes had to be applied hard. However, the car did not stop until it hit the honda in front of it. As the driver of the Telsa at the time of the accident, it seems as though car was accelerating despite brakes being applied. A huge sound was heard by me the driver and then the driver eat belt became malfunctioned and the car would not restart. After a few minutes, the car restarted but now the dash board states service needed and I could no longer fasten the seat belt. The front of the car appeared to have minimal bumper damage but the seat has not worked since the accident.

This is a report of a RECURRING problem commonly known as "phantom braking." When I take long trips in this vehicle at legal freeway speeds and with the Adaptive Cruise Control and Auto Steer (lane keeping) functions engaged, the vehicle will occasionally reduce speed suddenly and violently (emergency braking) even if there are no vehicles ahead and no visible hazards whatsoever. This occurs without warning and for no reason when cruising on open, straight interstate highways. In a typical 5-8 hour driving day on long trips, this will occur 1-2 times daily. If other vehicles were to be following closely behind, this would certainly cause a serious rear end collision or pile up. The vehicle gives no advance warning and no alarms sound. The car just brakes suddenly and violently scaring the bejesus out of my spouse. I have learned to step hard on the accelerator the moment this occurs which will partially offset the severe braking action so that any vehicles behind do not need to brake suddenly. I do wonder what the heck any drivers behind think when they see the Tesla suddenly brake hard then accelerate immediately back to speed. I worries me too that some aggressive driver behind might think I am doing this intentionally and get into a road rage incident with me. People have been shot and killed for less. I don't understand why this has not yet been addressed seriously by Tesla or NHTSA. This is a SERIOUS and dangerous defect! Thank you.

1. Wireless charging extremely slow., connected charging is also slow. 2. Safety Issue (Camera): driving side left camera and blind spot monitoring camera specially sometimes stopped working automatically during driving in the highway or city, it’s pretty dangerous Specially in the highway driving. During my long drive over July 4th weekend between 3-7 July happened several times, it caused auto steer mode forced disengage 3 times. occurrence 8/8 between 12-3 am it happened about 6 times. It happens on and off almost everyday in some part of drive. 3. Safety Issue Phantom Braking, in Auto steer, brakes are suddenly getting applied when even are no objects in the front of car for no reason. Aug 4, it happened about 4 times between 7-9 am.(around 7:30-7:40)Due to this big accident in the highway could have happened with Truck, as Truck was in normal speed 65 mph and due to sudden brake, vehicle speed came down to 35-40 mph, took control and move car out of road avoiding accident. This happened earlier instances as well during driving in the highways. But this time, I avoided death but got injuries. I need this car fixed for good OR a Replacement new car (preferably)OR a refund. Enough, I’m scared of driving this car afterwards, as I’m concerned about security and life risk all the time. Aug 12 braking issues happened on around 6:40 and 7:30 PST. Aug 15 braking issue happened on the highway around 7:45 and 8:15 am pst Hard to get a hold of customer service. they try to avoid Physical Service by Service Center by telling I need to provide exact timestamp and other nonsense remote debug and worst experience with the servicing. I asked for refund, no customer support no way to reach to discuss. 4. Range: Promised range for model Y is 330 m, get only 150-200 max, numbers are bloated and wrong. They tells it’s based on driving pattern and this range is based on 60-65 m/hr drive on a Flat surface, which was not told or advertised in website or during purchas.

While day driving on a 4 lane divided highway during light traffic, we experienced phantom braking several times. We were at the posted 70 mph speed limit while autopilot was engaged. Phantom braking also occurred when cruise control was only engaged. No problem occurred with surrounding traffic. Our car has yet to have a Tesla service center look at our car. No car warnings occurred during these events. These events occurred on July 16th and 24th, 2023 and earlier since we’ve owned this car. Thanks.

On July 17, 2023, while driving on a highway in the state of Texas, my Tesla suddenly and unexpectedly engaged in a hard braking maneuver for several seconds while I was under Full self-Driving activated, despite there being no vehicle directly in front of me. How was your safety or the safety of others put at risk? As a result of this abrupt action, the two cars behind me were forced to swerve abruptly to the right to avoid a collision. I cannot stress enough how dangerous this situation was, and I am immensely thankful that no one was injured. This incident has shaken my confidence in the safety and reliability of the vehicle. As a responsible driver, my utmost priority is the safety of myself, my passengers, and others sharing the road. Has the problem been reproduced or confirmed by a dealer or independent service center? I have raised a formal complaint with tesla by email on July 17th and Also, raised a formal complaint by sending a letter on the initial date of July 18,2023 and by sending the evidence of vedio.. On 8/1/2023, driving between Mojave, California and Inyokern, California on highway 14, a newly paved and striped highway being followed by a Mercedes Van, I was doing 70 in a 65 zone. No traffic in front of us. No road signs. No turn outs. No side roads. No bridges, overpasses or underpasses when suddenly the car braked from 70mph to 48mph before I pressed down on the accellerator to keep the Mercedes from hitting us. the front end of the Mercedes dipped as he hit the brakes hard and he honked his horn which is what I would have done under similar circumstances. In the next half hour we experienced the phantom braking phenomenon five more times before my wife found complaints about "phantom braking", a phenomenon we'd never heard of or had been warned about. We turned off the Automatic Emergency Braking and experienced no more events until after stopping in Lone Pine for charging. When we reentered the highway we experienced one more event before turning off the Automatic Emergency Braking system, once again. Hours before we'd been in Southern California driving on Highways 5, 101 and 405 cruising along at 70 with traffic close in front and behind us. If the phantom braking had occured there, we might not be here today. Calling the Tesla dealership in Reno the salesman was non-plussed about our experience and informed us we should be ready to hit the accellerator at all times. This is not an acceptable response to what could easily have been a major accident.

The contact owns a 2023 Tesla Model Y. The contact stated while pulling into the driveway, the vehicle accelerated at high-speed causing the vehicle to strike a wall at the contact resident. The contact stated the brake assist did not activate. The vehicle had major front-end damages. The contact's resident wall was damaged. Both the contact's wife who was driving and the passenger suffered concussion and whiplash. The contact's wife suffered neck injuries but neither the driver nor the passenger sought medical attention. A police report was not filed. The vehicle was towed to a repair center. The manufacturer was notified of the failure and the vehicle data. The failure mileage was 3,746.

Location: car wash tunnel Symptom: ghost braking/shifting While in car wash mode and neutral, Tesla shifted inexplicably from neutral to park, causing Tesla to be rear-ended by the car behind in the car wash tunnel. My passengers and I could have been injured. Tesla engineers completed a technical review of the safety incident but refused to provide the full report instead directed me to consult lawyers. On the day of the service appointment, they did not even inspect my vehicle but instead told me to request vehicle data online. It turned out to be a waste of time to download vehicle recorded data that only Tesla engineers understand. The service invoice says, “Verified proper operation of the vehicle.” That is false. They did not even admit my car into the service entrance. This crash is preventable, but Tesla is not taking this defect seriously and refuses to share the full report of technical review.

Approximately half a dozen times this year on highway journeys between California and Las Vegas, when the vehicle is on cruise control at approx. 70-75 mph, I have experienced brief instances of unnecessary, automated braking. Each time apparently in response to large black patches in front of the car, either a mismatched repair of the highway or a dark mirage type reflection of the sun on the highway. It appears to me that the vehicle cameras are occasionally mistaking these dark patches as an obstacle. Presumably lidar sensors would not react because the radar would not bounce back off anything, but I believe my car does not have lidar sensors. In each case there was an approximate 10 to 15 mph reduction in speed and then the vehicle quickly recovered. Fortunately, no other vehicle was tailgating me at the time but if one had been, I believe there is a realistic risk of being rear ended. Alternatively, the driver behind would, presumably, apply the brakes, which may cause a chain reaction when the highway is in fact, clear ahead. I have not contacted Tesla. Based on other reports online, it appears to be a waste of time.

Phantom braking occurs in cruise control for no reason and with no warning. The car is available for inspection. Many other Tesla drivers have described the same situation. On a recent cross country trip phantom braking happened frequently. We made sure there was no one behind us when we engaged cruise to avoid being rear ended. We were in NV when it happened again- this time a big rig had safe distance behind us when the brakes engaged ( we were driving 75). The rig driver swerved into another lane to avoid us. We have not taken car to service center yet. Social media is full of these accounts. This is still happening. There is no warning whatsoever. It’s hard to describe how terrifying this is.

The adaptive cruise control malfunctioned while engaged on the highway/interstate. On a recent trip of approx. 1,000 miles, the vehicle experienced "phantom braking" (aggressive emergency braking for no reason) on three occasions while the adaptive cruise control was engaged on the highway/interstate at approx. 65-75 mph, with no obstruction/vehicle in front and the weather was clear sunny skies. No warnings or messages were prompt by the vehicle. This abrupt phantom breaking could have caused serious injury from a rear-end collision or cause vehicles behind to take unnecessary unsafe emergency maneuvers/braking.

I had a very dangerous phantom breaking event while driving on the interstate. The car slammed on the brakes for no reason and I nearly had a rear end collision. There was nobody in front of me, sun was not a factor, road was flat and straight, weather was clear. Car was on cruise control only. I was driving Interstate speed, about 70 mph, and without warning the Tesla Model Y slammed on the brakes and the only reason we didn’t get rear ended was because I quickly hit the accelerator to get out of the braking action. My dogs also went flying forward in the back seat and my passenger was wretched forward and scared b/c it came out of nowhere. There was no indication from the car before, during our after that there was a problem. I have notified Tesla of the incident, but don’t expect they will do anything. The cat keeps applying brakes on its own even when there is no obstacle on road. Car applies brake on highway when there is no car on road in front or on side road. It is a very dangerous fault and is an accident waiting to happen. I have complained to local service center many times. They call it Fantom Braking and say that Tesla is working on it. If so, all Teslas should be off road till such time they fix it. I am concerned that it will lead to fatal accidents.

Multiple moments of phantom braking 15+ times TODAY driving from North Platte, NE to Havelock, IA on cruise or auto pilot. Extremely dangerous, could've caused pile ups/rear end collisions. Screen would 50% of the time show a semi or vehicle in front of me when nothing was there. I tried cleaning the windshield, thought it helped for a bit then it happened again. This happened on both interstate driving and highway driving into the sun and not into the sun. Could be very hazardous - very abrupt emergency braking at interstate speed limits. at the Shelby, IA super charger, another Tesla owner expressed he had also just reported multiple phantom braking incidents today as well and reported it to Tesla.

On a trip to Prescott, AZ and back to Los Angeles, I encountered the following problem about 5 separate times. While driving at freeway speeds (70-80mph), the car suddenly and violently, without warning, applied the brakes vary hard (with no input from me). This threw me forward while the car continued to decelerate. In one instance, there was a class C motorhome traveling behind me fairly closely as if to pass. This was on a 2-lane road in the Arizona “countryside.” It was between 6:20am and 7:15am on Saturday, May 13th. The motorhome following me had to make a split-second decision to either crash into the rear of my car, or swing out left into the oncoming lane. He chose the latter. Fortunately, there was no oncoming traffic or it surely would have been a fatal, head-on accident between the motorhome and a car traveling in the opposite direction in the oncoming lane. The motorhome then continued to pass me and no accident resulted. In the other 4 times that the brakes suddenly applied, there was no one close enough behind me where they could not have slowed down to avoid an accident. Obviously, this was very disturbing to me. I finally realized that the Automatic Emergency Braking (AEB) feature was set to “on” (by default). I then turned it off. No further incidents occurred after this. At this point, I will not travel on a busy freeway with the AEB feature activated……it is simply too dangerous. If this issue happens on a busy freeway, the likelihood of someone crashing into the rear of my car is fairly high.

I reported the issue to the Tesla service center in Oklahoma City, OK and they acknowledged it is an issue, but said there is not a repair for the issue. They said it is a software/mapping issue that will hopefully be resolved in the future.

Phantom braking while using cruise control on interstates. The automatic emergency braking caused severe slowdown reducing speed of vechicle from 75-55mph. On a 900 mi road trip phantom braking occured a minium of 12 times each direction. Sudden and unexcpeted braking nearly caused accidents on numerous occasions. As vehichles following were not expecting a sudden reduction in speed without cause. In no case was there an obvious trigger, so signs, shadows, debris, or any logical explanation for the dangerous sudden reduction in speed. Tesla service center in Madison, WI stated it was most likley 'just' a software issue. Tesla is not responsive to conerns and gravity of situation. Tesla service said they would inspect vehicle cameras but that would take several weeks, offered no loaner car, and dissmissed any cause for concern repeating the issue was simply a software issue that required drivers to better report 'bugs' and that more cars needed to drive these 'routes' (I-80 and I-76 in this case) in order for more accurate navigation. Vehichle repeatidly turned back on automatic emergency braking despite driver having disabled this feature.

My Tesla Model Y is experiencing phantom braking, due presumably to a faulty collission avoidance safety system. With cruise control enabled (not autopilot with lane keeping, only cruise control by depressing the stalk once vs. twice for lane keep) the car will phantom brake, where it slams on the brakes thinking their is a vehicle in its path when there is no vehicle in the front lane. This has happened to us on a clear toll road in Southern California on Hwy 73, both on June 7th, 2023 and in April/May 2023, where the Tesla slams on its brakes, decelerating us from 65mph to 38mph and below (at which point I slammed the accelerator pedal to avoid a rear end collission), with vehicles directly behind us, almost causing a dangerous accident. This happened with my three children in the back seat. We have experiencing this phantom braking twice in the last 2 months. In all cases the vehicle was well centered in the lane and there was no vehicle in the lane ahead of us, nor did we recently change lanes. I created a service ticket with Tesla, including bringing it into the service center at Lake Forest, CA, as scheduled, and they explained in person that there is nothing they can do, that they are unable to retrieve any video footage from the vehicle, and sent us away without even looking at the vehicle, replacing any hardware, hardware testing, or test driving it for themselves. We are at a complete loss of what to do with this vehicle that is only 6 months old. The vehicle has no warning lights or error messages prior to any of these failures. I feel that this vehicle is unsafe for my family to drive and Tesla is not putting forth reasonable effort to fix the vehicle.

I drive in West Texas, no hills but some rises and slopes, mostly flat. Going from Odessa to Lubbock (5AM) maybe 2 phantom braking episodes. Coming back on a clear, sunshiny day I experienced 15-20, out of the blue. Scared me to death. No cars within 100 yards, just BAM and it braked. Another 6-7 when the car thought a car over taking me was a problem and braked or one coming in a two lane (just about 5 miles only) thinking there was a danger. We are talking about excellent 4 lane highways with excellent pavement and markings.

I bought the Model Y in Feb 2023, after 2 months of driving, 3 weeks back on 5/14/2023, all of sudden the car lit up Yellow Brake indicator on dashboard while I was driving downhill. I didn't understand what this indicator meant but when I needed to brake - I could not brake at all, so I let regenerative braking slow the car down which it did. I was horrified as I was on the road at that time, so I tried to find parking to park the car and then read online what this indicator meant, but felt very unsafe to drive the car home. Since then Brakes doesnt work and ABS seems to push back my foot hard and Adaptive cruise controld doesnt work at all. I have service appointment on 6/6 which is the first appointment I could find close to my home.

This caused the car behind me to almost rear end my car. Luckily I was quick enough to press the accelerator to avoid the accident. This same sudden braking happened a total of 4 times within a 200 mile trip, one of which almost caused an accident.

We have had our car for under 3 months and have had repeated episodes (easily 2 to 3 dozen) of extremely rapid braking when the car is in cruise control on the open interstate road when there are no cars ahead of us or any other obstacles that might have triggered the camera sensor. Tried turning off the forward collision avoidance option and the events still happen. Have kept the windshield over the front camera areas clean. It has happened typically during sunny days as we have not usually used the cruise control in the rain. When these events happen there is typical rapid slowing of the vehicle by 20-30 mph within a few seconds. If a car was following close behind us, there would be a high chance of collision. There have been no warning messages or any signs that the vehicle is otherwise malfunctioning. When we contacted Tesla technical assistance, they emailed us a copy of the part of the car manual that says the feature might not function in bad weather, if the camera is not clean, in construction zones, etc. none of which describe the circumstances of when this malfunction happens in the car in broad daylight. They also said whenever it happens to send a bug report which the technicians will look at. They also said it might be fixed with the next update, which of course, it wasn't. Finally, in the same batch of emails, they sent email that said we could report our issue to NHTSA which without any other resolution method to the problem, is why I am reporting it here. It seems as if there are many Tesla owners who are reporting this problem online. It may have been less of a problem before Tesla removed the radar feature in the last year and depended solely on their front cameras to make automatic braking decisions for the car. Whatever is happening, it isn't working and makes driving with cruise control scary and dangerous. Thank you for listening. Please help.

Error message “power braking assist reduced” displayed on the vehicle and the vehicles brake gave out. Meaning I was unable to utilize the brakes for slowing down the vehicle. The message disappeared after parking (and powering down) the vehicle for a bit. Had the same issue in a few days. Tesla is now replacing the entire brake pad assembly due to a brake sensor failure.

Braking. Yes, can be inspected upon request. Phantom braking for no apparent reason occurred at least 12 times in each direction on a long-distance trip on 2- and 4-lane roads on May 15 & 17 and also a trip in April, both in Autopilot and TACC modes, with nothing but the open road ahead, very little traffic, although a couple times when nearing the top of a hill. Driving at 70 mph on 2-lane roads and 80 mph on 4-lane roads. Concern of being rear-ended if passing a vehicle so wait long time to return to driving lane. Fortunately in standard driving no one was following closely to cause concern. One time when I was passing a semi, when I was almost even with the cab. No contact with dealer or service center as they are 240 miles away from my home or inspection by others. Unknown if there were on-screen messages due to watching the road instead.

Incidents of phantom braking: We took our brand new Tesla model Y on a road-trip from Saint Helena CA to Scottsdale AZ and back. We had several incidents of the car slamming on the brakes while in cruise control due to mistakes in recognizing an actual reason to brake. May 11, in Arizona on highway 60, single lane road, we were in the right lane heading east when a large white pickup approached in the left lane. Our car mistook the approaching vehicle as a potential collision and slammed the brakes. This occurred a second time with another approaching vehicle at which point we ceased using cruise control. May 17 return trip on highway 10 in the morning, as I was driving with cruise control on, two lane highway, I approached a swale in the road with a change in pavement color. Gray concrete color contrasted with the blacktop patches in the upslope of the swale and the car slammed on the brakes. Same day, same highway, I was in the left lane having passed a slower vehicle. There were two cars behind me, waiting to pass. I put on the right turn signal and began to move into the right lane. There were a series of white jersey barriers on either side of the road. The car mistook these as a potential collision, slammed on the brakes and pushed me back toward the left lane. I applied the accelerator and forced it back into the right lane, avoiding a collision with the cars behind me. May 18 driving north on highway 5, multi lane highway and as my wife was driving and crested a hill there was a wire hanging across the road. In the center of the wire was a camera or some other device. While the wire was at least 6 feet over us, the car mistook it for an obstruction and slammed on the brakes.

Our car experienced unexpected and sudden braking on 3 occasions. #1: We were in cruise control (cc) and the car was going through a town and the brakes were applied suddenly (5/4/23). It accelerated again on its own. The other two incidents were on 5/8/23) #2: In CC, on a two lane highway. Tesla passed a semi in opposite direction (had plenty of clearance) and car braked suddenly. We were no where close to the semi. #3: Not in CC, on a 4 lane highway, we moved over into another lane to give a bicycle clear berth. Car braked suddenly. In #2 and #3 we had passed other semis and bicycles in tighter situations and braking did not occur. No accident occurred altho we had a car behind us in #2. Will be bringing car into Tesla on 5/20/23 to have them look at the car.

1)2 weeks into owning the car had a powertrain error message which was fixed by replacing the front drive unit oil pump. 2)Static noise behind the display unit was due to the harness of the circuit not being tightened properly. 3)Left hand drive side LatheralLink bolt was loose and dropped off while making a turn. Right hand LatheralLink bolt was not tightened to the recommended torque. Lefthand driver side Latherallink was replaced and bolts have been tightened. I have updated the invoices, videos and pictures of the 3rd issue. Have requested Tesla to replace my car with a new one as I do not feel safe in the car nor do I feel safe having my family in the car. I have raised this issue with Tesla on Saturday (April 30) and have not heard back from them. I could not upload the video of the issue that I have captured on my phone in this ticket. Dear Sir/Madam, I was cruising on autopilot on the highway at close to 80-85MPH and the brakes were suddenly applied bringing down the speed to less than 60MPH. There were cars behind me within the same lane(fortunately at a safe distance) and there were no cars in front of me for at least a mile or either on the side. The hard brake being applied unexpectedly, even without cars or obstacles in the front or side lanes, felt very unusual on autopilot mode. I would like to get this checked as to what is causing this issue as a priority as it can pose to be a real hazard if this happens in a packed highway lane or if any other driver is driving the car unaware of the issue. While driving down the road in traffic, several errors started appearing on the Tesla screen, and the vehicle lost power brakes and steering, rendering it impossible to drive safely. The complete set of car systems that were disabled all at once was: stability control disabled traction control disabled automatic vehicle hold disabled lane departure avoidance unavailable ABS disabled steering assist reduced (no power steering) regenerative braking features temporarily limited automatic emergency braking disabled The car is only weeks old. We had the car towed to a Tesla repair center, and they said due to a wiring issue, the wheel weight on the right rear wheel was rubbing against the wire connecting the wheel speed sensor, which led to this cascading series of errors. I'm concerned this is a systemic issue with newer Model Ys. There's a QA or manufacturing defect, and Tesla, along with the NHTSA, needs to investigate this dangerous safety issue.

At best this is annoying. But there are times when it is extremely dangerous. This happens on highways and interstates at highway speeds. New Tesla Model Y brake failed and accelerated resulted in a serious crash within 2 hrs of taking delivery. I believe it is acceleration functioning and brake due to auto pilot feature. Vehicle will be available for inspection after 4 weeks. After taking delivery of new Tesla Y, we drove to temple. While parking car accelerated and lost control of the vehicle as brakes were not working. It hit the temple wall and when we tried to backup to get the car out of the crashed wall, it got even worse. It reversed in lightning speed and hit another car, run over the bushes and huge palm tree across the street and finally stopped. This terrifying experience caused huge mental trauma, multiple property damage, and financial hardship that we are dealing with right now. Fortunately, there were no injuries. We lost all the trust and confidence in Tesla. We are NOT confident of driving due to huge safety concern. It is a very important safety issue considering the customers at large. Tesla service has not worked on the diagnosis. We tried reaching out Tesla service to investigate the failure. There is no response from them and they keep cancelling the service appointment. On top of it, there is no appointment available in body shop for a month. This incident was reported to police. One observation was made during the incident where Auto pilot beta was displaying on the dash board monitor without invoking any self driving options.

The incident happened on 4/9/2023 at 2:30 pm. I drove in I-10 from Phoenix to California with Cruise Control and Autosteer ON. The speed was set to the highway speed limit. There were three other passengers with me. This seems to be related to the known issue of "Phantom Breaking" in Teslas. I was driving on a 4-lane road with no traffic, then the car suddenly reduced its speed from 65 mph to almost 30 mph, causing a sudden jolt to everyone in the car. There was nothing in front or reason for the car to break. No incoming traffic. It does not seem to have detected a "ghost" obstacle as an emergency. As there was no beeping or emergency message on display. No warning of any type. Luckily, no car was following me; otherwise, it could have caused a rear-end collision.

At highway speed, using the autopilot feature for speed control only, the car applies severe braking when there is nothing obstructing the road or requiring brakes. This has happened dozens of times over the first several thousand miles we have owned the new car. We filed a report with Tesla and they said this is not a problem and is expected behavior. We are concerned this will occur with a vehicle behind us resulting in being rear ended.

Emergency bracking occured on multiple occasions during trip. When there was nothing to brake for, the brakes were automatically applied, sometimes severely. The car is available for inspection if requested. With unexpected hard braking, we could put someone following behind in danger of a rearend collision and possible injury to us and them. The problem has not been confirmed by a dealer or independent service center. The vehicle/components have not been inspected. There was no warning prior to this unexpected braking.

on 30th MAR 2023, when my wife drove on to our drive way with gentle speed, car rammed into closed garage door and broke the door and rammed into living room wall with breaking room entry door . there is neither emergency breaking /stop nor collision warning . it damaged wall , garage door and more living room property damage with addition to damage to the car. i am really worried about the safety of the car..

I have seen this happen on several occassions during my trip from Austin,TX to Lubbock, TX. I out the car on Cruise Control and the car has applied emergency brakes suddenly several times even when there is no cars or other obstacles around. Even after I tried disabling the "Automatic Emergency Braking" feature, I am still facing this issue on several occasions. I have taken 2 trips so far in the past 2 months from Austin to Lubbock (about 360 miles one way) and I ahve decided the car is not at all reliable for me. I am even considering trading-in the car for another brand. This is real safety hazard.

Tesla model y 2023 phantom braked on the intestate. The emergency braking system and adaptive cruise are not correct. I almost got hit because my tesla auto pilot braked on the interstate for no reason.

Car was purchased December 30, 2022. The first time I engaged auto pilot, an oncoming semi was in its lane and my car slammed the brakes on while on auto pilot. It happened the next time I used auto pilot with zero perceived “threat”. I was extremely hesitant to use it, henceforth. I tried auto pilot over and over with my foot, poised and ready to take over the throttle, and my hand poised to disengage auto pilot. 12 times it slammed the brakes on-every time I used the feature it randomly braked, there was absolutely nothing around. These incidences were on two lane highways, no oncoming traffic, nothing in the road. Phantom braking. I do not feel safe using this feature on my brand new car because if someone is traveling behind me, it will cause an accident. I took my car in for minor service (for adjustment and stern issue) shortly after purchasing and mentioned this to the Tesla service Center representative. She said to me, “oh yeah, it does that sometimes. You have to be ready to take over.” Even being poised and ready to take over, I fear my reflexes will not be fast enough if there were someone traveling behind me at 60 mph. and it certainly isn’t how cruise control should work. I have a second service appointment, and actually noted the phantom braking (I subsequently learned this is what it’s called after googling, “random, braking with no threat Tesla model Y 2023”) on my service request to ensure that it has been documented because it’s a problem. A dangerous, unacceptable problem that needs to be fixed. My service appointment is 3/28/2023. Thank you.

The contact owns a 2023 Tesla Model Y. The contact stated that while at a stop with the brake pedal depressed, the vehicle unintendedly accelerated. The contact stated that no warning lights were illuminated. The contact stated that she rear-ended the vehicle in front as she turned the steering wheel to the left and landed in a ditch. No air bags deployed. The contact stated that she and her front-seat passenger sustained no injuries however the other vehicle driver was seen by the EMTs on the scene. A police report was filed. The vehicle was towed to an authorized Tesla Center and was awaiting diagnosis. The manufacturer was notified of the failure and a case was opened. The contact stated that she was informed by the manufacturer that a diagnosis team was working on the diagnosis. The approximate failure mileage was 473.

When using the cruise control function (not hands free) we have experienced multiple disconnects from the cruise control which puts the car into a sudden breaking condition slowing the car until the cruise control reconnects automatically. On a recent road trip we narrowly avoided a serious accident when this happened and we had a big rig truck behind us. We reported this to Tesla and their emailed reply said the cause was "map data, speed limit data, GPS position inaccuracies or predictive position inaccuracies." We drove over 2,500, mostly interstate highways, and in our opinion, none of their reasons for this condition apply to what happened to us. We set up an appointment with Tesla to look at the car but their email response seemed to indicate that as we drive the car more, the system will correct itself. In our opinion, this apparent software glitch is serious and can have fatal consequences. We have spoken with other Tesla owners and they are also aware of this chronic problem. We are reporting this matter in hopes that it will be investigated and promptly correctly before lives are lost.

Driving on highway with cruise control activated. No traffic around. Experienced phantom braking taking me from about 80mph to 65mph before I was able to react. Had this happened in traffic there is a good chance i would have been rear ended. No notifications or emergency lights appeared on my dash.

Car did phantom breaking when I was driving with my wife, 2 kids and Niece from NJ to Pittsburgh, PA. I was driving on autopilot and There was no car in front or on side and yet it suddenly applied break and car went doen to appx 40 mph. Luckily the car behind was at distance and they managed their breaking well. I see it as a big risk to me and my family while being on long rides on autopilot.

When autopilot is engaged the car slows down without warning and there isn't anything in front of the car. We had close calls with cars behind us when the car applies the brakes for no reason. The wipers would come on for no reason and as long as the autopilot was on it couldn't be turn off. So we had to disengage the autopilot and turn off the wipers and drive another 300 miles without autopilot for our safety.

When driving in Traffic aware cruise control in Highway where the speed was around 70 mph, vehicles system auto applied brake and significantly slowed down speed for a few seconds. Had there been a vehicle following us closely, there would have been an accident. There were no close objects or vehicle in front while this incident happened.

While driving on the expressway, I had the cruise control set at about 72 mph. The car to my right went to change lanes but then saw me and rectified; my car automatically applied the brakes hard without me even touching anything. I was lucky that the car behind me was far enough that it didn’t hit me. However, this is a safety hazard for me and others as I couldn’t control the breaks at all. It also caught me by surprise since I did t know it was going to do that.

Drove out of my driveway, drove maybe 50 feet and the car did a shake and stopped suddenly in the middle of of the road. The car wouldn’t let me put the car in park, drive, reverse or neutral. Many alerts appeared on the screen (pics attached). The right side of the car was making a whistling sound that wouldn’t stop even when I got to finally turn off the car. The car was reset, turned off and on but wouldn’t drive in any direction. It’s extremely dangerous that a brand new car, 2 days old with 118 miles on it would suddenly stop while driving with no danger near by, the car was going 7-10 MPH, no other cars near or around me for the car to stop in that manner. I had two elderly (mother and mother-in-law) passengers in the car that could have been hurt because of this sudden unnecessary stop.

I Purchased a new 2023 Telsa Model Y, picked it up today and on the drive home I experienced a phantom braking event which caused the vehicle to immediately stop and become inoperable. Despite my attempts to restart or reset the car it would not move. I sat in the middle lane of a six lane roadway for 90 minutes waiting for a tow truck to take the vehicle to the nearest Tesla Service Center. There was no warning and the vehicle drove flawlessly prior to this event, although I had only driven about 125 miles. I was in heavy traffic and I responded quickly to stop the vehicle and avoid an accident.

When my vehicle is on cruise control it suddenly brakes for no apparent reason. On a road trip, it has happened suddenly and many times during the trip. It’s quite scary and dangerous.
