I have a 2023 Tesla Y. The Curvature Assist function has nearly caused a half dozen rear end collisions. When in cruise control, it kicks in on (1) highways, near exits when I'm not taking the exit (2) regular roads with no curve or intersection, (3) regular roads with departure lanes that I'm not using. The car behind me only see's a sudden brake light with slow down until I press on the accelerator. Please have Tesla install a disable function for Curvature Assist.

Pulled into hospital parking garage. Stopped to pull parking ticket. When the parking gate was raised, Tesla immediately took off at very high uncontrolled speed. Basically a runaway vehicle. There was no accerlation on my part. Brakes did not work. Hit parked car in the garage about 300 feet from the parking gate. I was taken to hospital with broken wrist. Car has been totaled.

It was raining on I-287 North bound in Bridgewater Township, NJ. I was driving my Tesla Model Y well below the posted speed as it was raining. Suddenly my car took a spin, i lost control of the steering. Car took a 360 degree turn from right most lane on 3-lane highway and ended up on the side of the left most lane, rear ended into the bushes, hit the trees and damaged the back of the car. My car is all wheel drive, still i lost the control. I was so lucky to be alive.

When using the adaptive cruise control or “autopilot” feature on my Model Y, the vehicle will brake extremely hard without warning. This sudden, unnecessary braking is commonly referred to as phantom braking. The failure is available for inspection upon request. This malfunction poses a significant safety risk to myself and others, especially vehicles following behind, as it could lead to rear-end collisions. The issue is widespread, with many other drivers experiencing the same problem. The problem is easily reproducible on highways, particularly at certain intersections where no obstacle is present. The system will sometimes display a “curvature assist” prompt despite the road being straight, causing the vehicle to brake abruptly. I have not taken the vehicle to a dealer because the warranty has expired. The issue has not been confirmed by a dealer or independent service center, nor has it been inspected by the manufacturer, police, or insurance representatives. However, I reported it directly to Tesla and have received no response. No warning lamps or error messages appear before the failure, aside from the inaccurate “curvature assist” prompt. Cause: UNKNOWN.

Driving on I 880 south on the fast lane. The car suddenly braked for no reason and began to shutdown. It warned me to pull over before complete shutdown. Luckily there was space on my left and I was able to rolling stop. Nothing on the screen was working. Turned on hazard light. Got out of car with my phone. Accessed Tesla app and ran battery check. After couple mins the error went away. Tried to slowly move with hazard light on. It appeared to have fixed itself. Slowly merged back into freeway when it was safe. Attached notification when I tried to schedule service request.

I have had Supervised Full Self Driving in the vehicle for well over a year. I have used the Smart Parking feature Monday-Thursday to park the vehicle at work and it has worked flawlessly. On [XXX] around [XXX], I went to [XXX] . After entering the gate to parking on [XXX], I drove slowly. I was able to select a parking spot and tapped on it on the screen for the selection and then started the automatic parking. There were no other cars around as we were all just starting to enter the gate and park. The car began to move in reverse. It then paused turned the wheel a bit, turned it back, tried to decide what it was going to do and started to move forward. The car then drove up the curb on to the walkway and almost drove into the building. I was able to hit the brake quickly to stop the process. There was no damage to the building or the car. I had never had an issue with the Smart Parking until this time. I then backed the car off the sidewalk and parked it myself. UNKNOWN I was driving with no Advanced Driver Assistance Systems (ADAS). My vehicle took control of the steering wheel, swerving uncontrollably, almost hitting approaching cars, and a wooden electricity post. It was diffucult to keep the steering wheel steady. I struggled to pull the vehicle to the side of the road until I made a complete stop. A witness checked on me and was concerned that I had almost crashed.

While using cruise control (TACC) or FSD, the vehicle inadvertently brakes until vehicle stops, due to detecting light glare or other things as a collision threat. This happened many times. Reported to Tesla service, they said it is a bug and I should file a bug report. I have previously filed many bug reports about this but no software update fixed the problem. Incident happened many times, Tesla should have records of all, because when there is a disengagement, driver is asked to tell what happened by voice.

Tesla remotely disabled my vehicle twice (February 28, 2025, and March 6, 2025) without prior notice, legal justification, or a court order. February 28, 2025 – Tesla Disabled My Vehicle for 3 Days My Tesla Model Y was remotely locked without warning, leaving me stranded and unable to use my vehicle. Tesla refused to provide a reason or reinstate access for three days. March 6, 2025 – Tesla Disabled My Vehicle Again & Attempted Repossession Tesla again remotely disabled my vehicle, then sent a tow truck for repossession without legal notice. I called the police, who confirmed Tesla had no legal repossession order. Tesla refused to restore access, forcing me into a coerced “voluntary surrender.” How This Posed a Safety Risk I was stranded in public locations without notice or transportation alternatives. Had the vehicle been in motion, this could have caused a road hazard. Tesla’s lack of due process denied me the chance to dispute the action before losing my vehicle. Confirmed by Law Enforcement & Tesla’s Own Records Police intervened on March 6, 2025, and confirmed Tesla had no valid repossession order. Tesla has refused to provide written documentation or a legal explanation for these actions. Regulatory Concerns Tesla’s unilateral ability to disable vehicles raises serious consumer safety issues: What prevents Tesla from disabling vehicles arbitrarily? What legal safeguards exist to protect owners from unjustified manufacturer lockouts? Should federal law restrict Tesla from disabling vehicles without court approval? Request for Investigation Tesla’s remote disabling practice endangers consumers and should be urgently investigated by NHTSA to determine if federal safeguards are needed.

This is my 4th safety report about the same issue: unexpected braking while on cruise control. Since my last report, Tesla offered another free month of auto pilot but I did not take it for fear of this braking issue. On Tuesday [XXX] I was returning to my home in Las Cruces NM from Colorado. I was on [XXX] toward Albuquerque, and there was very little traffic. I decided to see if I could get away with the cruise control. As I was pulling into Bernalillo, there were some oncoming cars in a left-turn lane at a signal, and my car braked automatically...enough to lurch me forward. I decided to stop at the "new" Tesla dealer in Bernalillo (I got my car in Santa Fe) and ask about the automatic breaking and whether they'd fixed it. The 2 service employees looked at each other and one said "No". I then asked about whether I needed to re-boot my key card since my screen went blank while I was in CO, during a snow storm, parked. I luckily had my phone and on the Tesla App, under Roadside Service, it gave me the option to re-boot the screen! I did so. The serviceman said he didn't think the blank screen had anything to do with my key card, but I could re-boot it if I want to. (A prior complaint also reported the failed key card in less than a year and Tesla's unwillingness to replace it). The contact owns a 2023 Tesla Model Y. The contact stated while the vehicle was parked and attempting to reverse, the vehicle independently accelerated. The contact depressed the brake pedal, and the vehicle stopped. The contact regained control of the vehicle on the curb. The vehicle was towed to an independent mechanic but was not diagnosed or repaired. The manufacturer was not notified of the failure. The approximate failure mileage was 30,000.

I have reported this same issue twice before. When on cruise control, the car brakes unexpectedly. My 2nd and most recent report was on [XXX]: "... I had already disabled "Apply Brakes when Regenerative Braking is Limited" under Dynamics on the screen. (see photo) He showed on the screen, under Autopilot, that I could disable Automatic Emergency Braking, which I did. I was concerned that this warning pops up: "The vehicle will not automatically apply the brakes even when a collision is detected" (see photo) but he suggested I try it to see if it would solve the cruise-control braking. I was hopeful, and disabled it, knowing that it might take a while to know if that solved the problem. I drove the 30 miles home on the frontage road instead of the interstate, and when there were no other cars around, I set cruise control at 50 mph, the speed limit. I came to a series of small hills in the road, and was reminded of the first time the automatic braking happened, on a 60 mph road with longer amplitude hills of greater height. Amazingly, it happened again! Pretty significant braking and slowed me to 25. This time I waited it out to see if it would resume the cruise control at 50. It did not. I have since restored the Automatic Emergency Braking as that seems important for a crash situation. I have restored the "Apply Brakes When Regenerative Braking is Limited" since that obviously was not the problem either." I did not use cruise control again until [XXX], when I happened to be returning home from Silver City to Las Cruces and was on the same highway, [XXX], where the problem first occurred. It is a 2-lane road and often there is no traffic, so I felt safe trying cruise control again. I set it at 55, the speed limit, and after only a few miles the car braked again. I do not use cruise control anymore, and was dismayed to find that Tesla is again offering a free trial of autodrive! This is a huge safety issue! Harsh braking while using cruise control. Known as "PHANTOM BRAKING". This is causing extremely dangerous situations. The vehicle slams on the brakes while at high speeds for no apparent reason. Perfect weather and road identification markers. Happens with oncoming vehicles in normal path of travel as well as when there is a break in painted lines on the roadway. This creates a serious potential for an accident injury or death. Happens consistently.

2023 Tesla Model Y has frequent phantom braking issues when using cruise control. Let me clarify I am not using any sort of auto-pilot or self driving options. I am simply using cruise control. The incidents that cause many braking issues are unjustified to the point I have no idea what the Tesla may have recognized to make it slam on brakes. Very dangerous

Neither vehicle crossed the center line but the crash alarm blared and the car braked almost causing a rear ending accident. We were about the park in the marked parking lane and the vehicle unintedly accellerated, the brakes did not work and we saw message "Automatic emergency brake not available" and Air bags not available. We tried to control the vehicle to avaoid crashing into the porch of the infront apartment and steared the speeding vehicle and the vehicle hit the pillar of the aparement and finally came to halt few meters away in the parking lot as the front right wheel broke. 1. The ADAS Automatic emergency breaking seems failed 2. Forward collission warning seems not working 3. The safety of the driver and the passenger were at risk while the collission happened 4. I am not sure if the similar incidents happened or can be reproduced by the dealer/manufacturer. 5. Vehicle components inspected by the Tesla collission center but no software logs inspected and they said that they cannot get the device crash/event logs. 6. At the incident/while the incident was underway we saw few message that displayed "Automatic emergency brake is not avaiilable" and "Air bags are not available"

While driving straight on a 2 lane divided highway, the Traffic Assisted Cruise Control decided it needed to slow itself down significantly to 15 MPH under the speed limit (posted speed limit was 65 MPH). There were no vehicles in front, behind, or beside my vehicle. The breaking occurred when it came across a line break where cars from the opposite side of track could turn left onto a service highway but no one was in that divide nor in the turn lane. There was no warning or any obvious indication that the vehicle should harshly slow down for any safety reason, where in fact is there was a vehicle behind me, it could cause a crash. I made a request for Tesla to repair the vehicle, but in my attached photos, you can see that they admit to the issue being software related and insisting that it is an unrepairable problem until a software update can be issued.

The contact stated that while driving at 30-40 MPH, the vehicle unintendedly accelerated causing the contact to lose control of the vehicle and crash into the rear of a parked vehicle. The vehicle continued to accelerate and traveled across the street crashing into another parked vehicle. The vehicle continued to accelerate on its own forward and over a sidewalk and crashed into two street signs when it came to a stop. No warning lights were illuminated. No injuries were sustained. No air nags deployed. A police report was filed. The vehicle was towed to an autobody center. The insurance company deemed the vehicle totaled. The manufacturer was made aware of the failure, and a case was opened. The failure mileage was approximately 12,000.

WE WERE TRYING TO PARK THE CAR IN OUR GARAGE WHEN THE CAR SUDDENLY SPED UP. AND HIT OUR THE FENCE . i RELEASED THE PEDAL BUT IT KEPT DRIVING UNTIL I HIT THE BRAKE. AIR BAG DID NOT DEPLOY. THE CAR?S FRONT WAS SMASHED. OUR FENCE IS ALSO BADLY DAMAGED. I HAVE ASKED TESLA TO INVESTIGATE IF IT WAS BECAUSE OF THE AUTOPILOT TAKING OVER BUT HAVE NOT RECEIVED A RESPONSE FROM THEM YET AFTER MORE THAN A WEEK.

Was traveling along the [XXX] about 5 miles north of the [XXX]. Vehicle was in traffic awareness cruise control when it suddenly applied the brakes. Vehicle did not sound any warnings. Vehicle did not display any warnings. Vehicle made no attempt to warn driver of a braking event. Vehicle traffic awareness cruise control was set to 79 or 78 mph in a 70 mile per hour zone. I was able to push the accelerator pedal to regain speed but had already dropped to about 65 mph. Vehicle did not pass under a shaded section ( I. E. no bridge or trees or big rigs) There was change in color of the road top. My foot was resting just in front of the accelerator pedal. Fortunately for us, there was no vehicles directly behind us. This incident is the 3 of this kind in this vehicle. This has me spooked to use any of the convenience software. I already do not like using the autopilot feature because of it erratic behavior. It feels very unsafe and unpredictable. We are unable to predict when this car will act on its own and feel it will cause us harm one day with actions like this. We are glad that no one was following us closely because they might have hit us. We are also upset that the vehicle never stated why it braked suddenly. I am reaching out on behalf of my wife who met with accident on 3/24/2024.She was driving Tesla Y model and this accident occurred due to System failure of Vehicle , brakes and Steering wheel get locked and she was not able to do anything and car hit the couple of trees on other side of parking lot . She was not using Autopilot feature and it vehicle didn't stop after applying the brake instead of stopping , its get accelerated and steering was also locked.

During a 2,300 miles recent trip in Texas, the vast majority of the speed limit signs were not seen by my 2023 Tesla Model Y autosteer/autopilot (not FSD). This created issues with the autopilot/autosteer as it was not usable in several instances and can create safety issues like distractions, etc... I had the latest software revision. The key point is that this is not happening at all in Colorado where I live. Tesla support responded that I needed my cameras recalibrated which is a totally illogical response as it is not happening in CO.

Car warned that Drivability - Acceleration and top speed reduced/Performance may be restored on next drive but gave no opportunity for me to pull over safely. Felt like someone pulled the e-brake during regular driving speeds and prevented me from safely slowing down, steering or maneuvering the vehicle into a safe spot. This occurred during a rainy night with my wife and 3 daughters in the vehicle who were terrified and still afraid to drive in the vehicle. Tesla replaced the front drive inverter and about a month later the same issue happened again on a busy road around 1pm in the middle of traffic and this time the rear drive inverter had to be replaced. Myself nor my family have felt safe in the vehicle but feel stuck with it.

Car was being driven into a car wash. Driver went to put car into neutral and car lurched forward and crashed into car in front of it without gas pedal being engaged. Vehicle has not been inspected by manufacturer or others. No warning lamps or other messages. May have been related to recently installed full self driving software that was automatically installed but was not requested by vehicle owner. Vehicle owner did not intend to engage any sort of self driving or automatic acceleration systems.

Vehicle: Tesla Model Y Blue Purchased in Oct’ 23 Tesla setting: Regenerative braking (single pedal driving) Issue: Brake Pedal/Acceleration Pedal Malfunction I was pulling my car out from the parking lot to pick up my mom, from the hospital entrance. I just turned right and had to take an immediate left to leave out of the parking lot. The Tesla was driving at approximately 5 miles an hour at that point. I lifted my foot from the gas pedal for the car to stop with its regenerative braking. However, instead of coming to a stop, the Tesla started rapidly accelerating, with my foot not even on the pedal. When I slammed the brakes, the car was unresponsive and continued accelerating. I started honking madly to warn people that I had no control over the car I was driving. The vehicle continued accelerating over 100-150 meters in 4-5 seconds and banged a parked car (Lexus SUV) at the end of the parking lot. It continued accelerating while pushing Lexus over the curb, destroying road sign poles and landscaping shrubs; crossing the street, and the middle curb. At the end of the street was a tree and a ditch. Lexus couldn’t move any further and Tesla stopped. All airbags deployed. One of the emergency response detection systems called 911 immediately. The first thing the police asked me was if I pressed the emergency brake. I said the car doesn’t have one. I can’t even turn off the vehicle while it is moving. The parking button takes care of it. I used to love my Tesla but I can’t trust it anymore. What I want from them at this time is: Investigate all the incidents thoroughly instead of blaming these as a user error. a) Think about adding the redundancy system. Something mechanical where the driver reacts within a couple of seconds. Software bugs are hard and sometimes it is impossible to find the root cause. Logs may not contain everything that you need to debug the problem. Consider it as a P0 issue. b) Compensate for the damage and property loss.

You have no doubt heard the news that Tesla has deployed Full Self Driving to all its vehicles as a "free trial." My Tesla received this update in the past few days. Last night I was driving on an expressway and engaged what has up until now been traffic-aware cruise control by pressing down one time on the right stalk. I expected regular cruise control but the car started steering for me. Tesla has changed the default behavior of the cruise control activation (one single downward press on the stalk) to instead activate Autopilot. My attempt to regain control caused me to jerk the wheel side to side in a way that could have led to an accident had I been in heavier traffic. This was unexpected behavior from the vehicle and I find it unacceptable that Tesla has changed a default function in this manner. I had to hunt around on the screen to find the setting to change the behavior back to regular Traffic Aware Cruise Control from one downward press. Furthermore, on the same 20 mile trip I had three phantom braking incidents while the car was on Traffic Aware Cruise Control mode. This is more than typical and these events can be dangerous if they happen in traffic. Tesla should not be allowed to deploy changes, especially without notice, that modify basic functionality of a car in motion from what the driver has become accustomed to.

I want to report what I think is a very dangerous issue with my Tesla Model Y. We have experienced numerous times that the car brakes without warning. I have talked to Tesla about this "phantom braking", and they say it is "normal". The Traffic-Aware Cruise Control cannot be disabled to create a normal manual cruise control. I gave them a hypothetical situation where this could be life-threatening, but they dismissed it. Two weeks later, that very scenario happened, thankfully not ending in tragedy. I was driving at 65 mph on a wide, two lane paved highway (near Dove Creek, CO). A car going in the opposite direction was stopped on the shoulder on the other side of the highway. A large semi truck, also going in the opposite direction, pulled into my lane to pass the parked car on the side of the road. I saw what was happening, let off the accelerator, and steered over onto the paved shoulder (wide shoulder) to avoid the oncoming truck. My car's Traffic-Aware Cruise Control, using the car's cameras, interpreted the situation as an apparent collision danger and automatically braked hard. In this situation there was a large pickup doing what most pickup drivers do.... tailgating close behind me to get me to drive faster. Neither the driver of the pickup, nor me, were expecting my car to brake hard, as we had all assessed the situation correctly as not being dangerous. I responded as quickly as I could by pushing on the accelerator to cancel the cruise control, but the pickup almost rear ended me. At that speed, it could easily have turned into a fatal accident. Tesla says I can't disable the cameras, so I just don't use the cruise control now. I was told you can disable the "traffic-aware" part of cruise control on older model Teslas, but not 2023 and newer. Often the phantom braking occurs with no apparent reason (although the car's cameras and computer would probably disagree). Do many people have to be killed to take a safety hazard seriously?

This car does not have Full Self Driving, and I do not use Auto Steer. I experience multiple phantom braking events every time I use the "Traffic Aware Cruise Control." Sometimes I can see that the cruise control set speed has been reduced from the speed I set, and I have to cancel and reset it to resume the original speed. Sometimes the cruise control set speed is unchanged, and pushing the accelerator will cause the car to resume the set speed. Yesterday I drove 320 miles and had about 6 of these events. I'm concerned about these events causing an accident from the sudden speed reduction, or getting into a road rage situation because the person behind me thinks I am brake-checking them. Internet research indicates that these complaints are very common during the past few years. If Tesla can't - or doesn't want to - solve the problem, they should at least allow us to choose basic cruise control to avoid this problem. Adaptive cruise control would be more reliable with radar, but Tesla is doing everything it can to make the cars cheaper, and radar was eliminated. Personally, I think trying to do everything with cameras is a fatally flawed approach - especially considering cars travel at night and in varied weather and lighting conditions.

Phantom braking has occurred on 4 different occasions nearly causing a rear end collision during one of them. On all occasions it occurred with no other car in front of us. Tesla Service was contacted through their app for service and we were told this is a known issue and to simply apply pressure to the accelerator. No warning lights or other was given before the incident. The problem has not been reproduced or confirmed by Tesla or independent service center?

On almost a daily basis, with the cruise control set to maintain speed (vs the option of advanced speed control that will adjust to signs), the car will inexplicably and quite randomly decelerate aggressively at highway speeds. On four separate occasions just today (although it happens at least once or twice a day), with the cruise set to 70 MPH, the car will randomly decelerate aggressively or even change the max speed of the cruise to 55 MPH. On all occasions, the posted speed limit is 65 or even 75 mph. This is dangerous, particularly in DFW traffic during rush hours, with traffic flow moving at similar and often higher speeds in close proximity. Having my car aggressively decelerate without warning to 55 MPH has caused me to be tailgated and honked at. This has been happening since I've received my car. At first, I tried to use the advanced features of the cruise, but whilst driving on the Dallas Tollways, where the toll road has a speed limit of 75 MPH and the regular highway paralleling at 65, the car is unable to differentiate and would often interpret the wrong speed limit and aggressively brake. Going from the flow of traffic (close to 80 MPH) to 65. This is why I switched the cruise to absolute speed rather than any sort of posted adaptive. It's seems to have gotten progressively worse. So much so that I only use the cruise where traffic is light because of this issue's unexpected and random nature. Besides being dangerous, it's also very scary as it's completely unexpected and aggressive. I did make a service support call, but the Tesla technician said it's a known issue and no data available if/when it may be fixed. Currently, car is running software v11.1 2024.2.3

What component or system failed or malfunctioned, and is it available for inspection upon request? Slowed down entering a driveway when car sped up on its own , Forward Collision Warning/Emergency Automatic Breaking did not warn or do any breaking. NO SOUND or WARNING on the computer screen. I brake but the car still hit the post. I do not know what speed it accelerated to. I am not sure if Autopilot kicked in but the car accelerated when I have already released the accelerator. How was your safety or the safety of others put at risk? YES - injuries sustained. Has the problem been reproduced or confirmed by a dealer or independent service center? When calling Tesla on Dec 23, 2023 - was told by the rep that ticket has to be filed but I was disconnected and I was not able to get through the lines. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? Yes - Car was totalled. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? I never saw any warning lamps come on as I always driven with distance. The camera was worked was worked on due to the wiper going on even when not raining on October 24 but the fix did not work. Once at the Emergency room, trying to call tesla or texting the no that responds when buying the car no longer accepting my text. The promise that when we get car accident, Tesla will call - no phone calls. All I received was text alert car suffered failure and was deactivated. I was in pain and not able to follow up and try to do report and look at pictures.. Today I just received a letter dated 2/8/2024 about Recall no 23V-838 in the mail. BUT the car is gone... The software version it had is 2023.27.7. The tech guy came here to the house on Oct 25, 2023 and checked everything.

After making a right turn, the car accelerated automatically and drove onto the sidewalk. The driver was hitting the brakes and attempting to redirect the vehicle with the steering wheel but both components of the vehicle were locked, causing the vehicle to lose control and crash into several things on the sidewalk (flower pot, parked cars, building pillars). The Tesla is completely dead so we are unable to retrieve any data within the vehicle. The safety of civilians on the street as well has the driver and passenger of the vehicle were at risk due to the lack of control of the vehicle. Two weeks after the accident, Tesla sent out a recall notice in regards to autosteer which can increase risk of collision, but prior to this no notice/warning was in place. The police were involved and at the scene at the time of the event and a police report has been made. No messages were seen on the screen or on the Tesla app in regards to any car mode (autopilot, cruise control, etc.) the vehicle could've been on, nor any warnings about risk of collision due to the recall.

At 9:37 a.m. on January 26, I was driving on Freeway 60. The three cars in front of me braked suddenly. I stepped on the brake quickly, but after I stepped on the brake for 3 seconds, the speed of the car still did not slow down, and it directly hit the rear of the car in front.Afterwards, on the Tesla app on, an alarm was displayed, saying that at 9:37, there was a problem with the steering wheel and brakes(unavailable) And also, there is a Manufacturer recall number:TBD NHTSA Recall Number 24V-035; TC2024-027 (the manufacturer has failed or is unable to remedy this safety recall for my vehicle in a timely manner, I want to file an online complaint with NHTSA)

The his also outs me, my passengers and people behind me at risk. There were no warnings about this behavior. I have the latest update and will also contact Tesla about the issue.

While driving at hwy speeds in the #2 lane the #1 lane to my left was closed with construction cones. I was in the lane with cruise control activated and driving at approx 70 mph with flow of traffic. I had been in this lane for a few miles when all of a sudden the car rapidly decelerated approx 10-15 mph in a very short distance. I took control of the acceleration by quickly taping my brakes. Luckily there was no one behind me, as it could have created an accident risk. My only guess is the car interpreted the line of cones as a hazard even though they were not in my lane.

As I approached an intersection with an oncoming semi-truck, the vehicle slammed on the emergency braking system slowing from 60 mph to 45 mph in about 30 feet. I think it is called phantom braking. I was driving into the sun at 2:45pm.

During highway driving, on two separate occasions that were two days apart, while using cruise control set to ~72MPH the vehicle suddenly tried to stop. It quickly decelerated by ~25MPH before I intervened by pressing the accelerator. There were no obstacles in the road and on the second instance there weren't any other vehicles in site (was in remote northern NH/VT). Had someone been driving behind me we very well could have been rear ended. I was driving with by wife and three kids and did not feel safe.

I am writing to express my dissatisfaction with my new Tesla Model Y RWD, which I received on December 3, 2023. I have encountered two major issues with the vehicle and the customer service. First, I was promised by Tesla representatives that I would receive six months of free Supercharging after delivery, as part of a referral program. However, this offer has not been activated yet. When I contacted Tesla, one of the supervisors told me that I was not eligible for this offer, because I placed my order on September 23, before the program started. This is unacceptable, as I was assured multiple times by Tesla agents that they would honor the offer and match it before delivery. I also offered to take back my self-driving capability which is available to me for free for the first 3 months. Second, I have experienced serious problems with the auto-pilot feature of the vehicle. I have used it four times briefly in the last month, and every time the car slowed down significantly when it was supposed to cruise at 50 mph. Twice, I had to avoid rear collisions by pressing the accelerator or the brakes. I reported these incidents to Tesla through the app, but I have not received any response or explanation. The most alarming event occurred on December 26, when I was with my family on a return trip from IKEA in Woodbridge, VA. The weather was bad and the Tesla suddenly stopped on the road. I suspect that the front camera falsely detected an obstacle and caused the car to brake abruptly. These issues are not only frustrating, but also dangerous and potentially life-threatening. I am very disappointed with the quality and performance of the Tesla Model Y, as well as the lack of customer support and accountability. I request that Tesla either provide me with a satisfactory solution or compensate me for the inconvenience and risk that I have endured. I have a legal background and I am aware of my rights and remedies under the Lemon Law and other consumer protection laws.

UNKWOWN: ASSESSMENT: Date of Event 19 December 2022 Upon taking delivery from Tesla for 2023 Model Y vehicle with the above or contained VIN, driver experiences a series of events that result from a autosteer activation. Approximately seven (7) calendar days after receiving the vehicle a FDU (Front Drive Unit) was replaced. At approximately another twenty-one (21) calendar days after the FDU was replaced while activating active cruise control and or autosteer the vehicle unexpectedly aggressively accelerated (UAA) to unknown speed that was not observed by the driver. It was later found out by the drive through scheduling a service appointment that a series of settings and unknown factors may have caused the incident. This incident was video recorded as a result of a traffic stop conducted by a local sheriff's office. This event contained several other incidents before vehicle was stopped including UAA, collision avoidance (oncoming traffic), vehicle brake lockup, and unresponsive controls.

Vehicle reversing malfunctioned , may be a software defect. Vehicle while reversing without pressing the accelerator went berserk and hit another vehicle in the driveway. Vehicle data was provided by Tesla but there is no evidence of collision nor any video recording of the incident. No issues were seen nor any warnings displayed prior to the vehicle reversing itself. However insurance deemed it as driver fault without any confirmation by the manufacturer (Tesla).

I experienced a sudden unintended acceleration while parking the car in my home garage, something I have done thousands of times with no difficulty. I had this brand new car for a 12 days before it smashed through my garage wall. My garage is a fairly tight fit, so as I always do, I partially entered the garage and stopped to make sure that the side mirrors were going to clear the garage entrance. Then, I lifted my foot off the brake in order to start moving forwards again, at which time the car suddenly accelerated like crazy through the remaining 12 ft of space, smashing the cabinetry through the rear wall of the garage. The car, my garage, and laundry room were severely damaged. I complained to Tesla about this vehicle problem, but they just say there are no problems with the car, and it must be the driver’s fault. They are implying that I floored the accelerator, which is nonsense. I have been driving for more than 40 years, and I do not floor the accelerator while parking in my home garage. This type of sudden unintended acceleration is a danger to public safety, and this matter has to be thoroughly investigated, and a solution has to be found to fix the problem.

Respected Authorities, We bought Tesla Model Y in May of 2023 with our hard earned money, and next day or so we noticed a big paint mark on this brand new car.. Upon following up few times, Tesla agreed to repair this but they damaged further and after repeated visits, they agreed to replace the seat cover.. However, while replacing the cover, they damaged the side door coverings. Again after lot of follow up they agreed to replace that part as well. So, we thought we are happy but later we had to face very scary situations with this faulty car. The steering car completed locked while driving and had to face very dangerous situation. We are fortunate that the traffic was low in the street road, and if it was in highways, then it could have been fatal. Earlier, when we are reversing the vehicle in the garage the vehicle didn't turn well enough and the driver side mirror was hit the garage siding, and earlier to that when we are turning the car to the neighborhood the tire got touched to side walkway and threading was damaged. We strongly believe these could be the result of the steering wheel on and off locking in second which we are not sure at that time. As model y is new to us, we are not sure how that drives but now understand the issues clearly. We humbly reported all the above to Tesla Service, and they wanted to do steering wheel issue only and charge the other two... The important thing here is we were saved earlier due to the steering wheel issue but not sure if I we feel safe driving this car again. On October 24, between 8:45 and 9:05 am, I was traveling on the highway with cruise control on and the car suddenly braked going from 75 to 30 rapidly. It was only me pressing the accelerator that kept it from coming to a complete stop. Fortunately, there was no one close behind. There wasn’t any traffic around me. I was passing an exit. I reported this to Tesla and they remotely investigated but said that the related cruise software and hardware was operating normally. This is extremely dangerous! I’ve checked Tesla forums and this is a known problem that they don’t seem willing to address.

On September 23rd our vehicles cruise control stopped working. We submitted a service request to Tesla and they scheduled a service appt for October 16th. They later cancelled this appt saying that it is a known firmware issue that would be corrected in a future, undetermined, firmware update. On November 16th we took the car to a Tesla service center and they confirmed it is a firmware issue, but could not correct it. We have now been without the adaptive cruise control function on our car for over 2 months with no idea when or if this problem will be fixed.

I bought Tesla and tesla insurance which monitors driving of driver to provide safety score to driver. The safety score enables monthly premiums of tesla insurance. My complaint is related to safety score which provides financial incentives to drivers to not apply break. As part of safety score provided to drivers in tesla app(screen shot attached), one of the factors used is hard breaking to penalize drivers when break is applied, forcing the driver to use regenerative breaking instead of applying break. I have observed, even when I mildly apply break the safety score is impacted. The second factor in safety score that seems unreasonable is driving after 10pm. My daughter worked late shift and would return home after 10pm. Why is this even allowed as most of the emergency workers and many others work late hours. I have also observed safety score change from 99 to 98 when the car is parked overnight, even when I am not driving.

While using cruise control on a recent road trip, the car braked suddenly for no apparent reason. This happened 7 times over a time frame of 11 days of driving. Each time it occured while traveling on a freeway (2 lanes each direction), large median, very light to no traffic, during the day, the car slammed on the brakes, without warning while traveling the speed limit (up to 75 mph). The first time it occured, I almost came to a stop before recovering and accelerating to normal speed. This happened on Oct 15. The second time it happened, was on Oct 16. After the 2nd time, I would not use cruise control unless there was no other traffic. There were four times of phantom braking on Oct 30 alone. I have not yet submitted a service request to the manufacturer.

I was driving on the freeway at approximately 67 MPH using the advanced cruise control. There was no car immediately in front of me or directly beside me. All of a sudden, the car decelerated quickly, as if it slammed on the brakes. I was able to react quickly and override the deceleration, but had there been a car directly behind me, I very likely would have been rear-ended. It all happened so fast that I didn't stop to get all of the detail, but I'm guessing I dropped down to about 50 mph before being able to respond and override the breaking. It was terrifying, as I didn't know if I could override it at first. I later read other complaints about "phantom breaking" and realized there is a pervasive problem with the Tesla.

I was driving November 6, 2023 at about 10:30 in the morning at approximately 25 to 30 mph on a neighborhood road when my car accelerated on me without me applying extra pressure. I was caught off guard and thought it must’ve been an anomaly. We have owned the car since August. But then on my way home, about 1 o’clock, I was again on the neighborhood street doing 25 to 30 mph and the car obviously accelerated on me so that I applied the brakes. My husband was in the car with me and really noticed it surging the second time. At the time, it concerned me so I looked on the Internet and saw that other people have been complaining of a similar issue. I want this on record because I know I did not apply pressure to that pedal. We have scheduled a service with Tesla Friday.

All cargo became airborne and flew forward. Had a car been following, it is highly likely that we would have been rear ended.

My husband and I were driving from Cincinnati Ohio to Harrisburg, PA on Monday, Oct 16, 2023 on the highway (turnpike). At least four times the car tried to kill us by starting to slam to a stop ON THE HIGHWAY when using cruise control. Fortunately each time there were no cars immediately behind us or we would have had a major accident. We stopped and called Tesla in Mechanicsburg who consulted with someone on another line. We had both hands on the steering wheel and were doing everything correctly. They concluded that it was either the car sensing we were driving near mountains, or around curves. it happened once with a mild curve but the other times the road was straight. There was a small drizzle of rain (they said it could be rain) or they also said it could be sun. At this point I said "this is ridiculous as how can we drive this car to Florida (in the sun) if this might happen. Their conclusion: Take it off cruise control. We did and continued home but it is ridiculous that we cannot use the cruise control for fear of it happening again! And they said risks include mountains, rain, sun and curves in road. These conditions are the conditions almost everyday for most people in Pennsylvania! If there had been a truck behind us or a car, we would be dead.

The road was absolutely clear of any real hazard. Almost caused car behind me to collide into the back of me, and then they passed me while honking and very angry. I can't blame them.

Two times in one week with two different drivers driving the same vehicle the vehicle experienced sudden unintended acceleration while parking. In the first incident this resulted in damage to the bumper into a wired fence which the vehicle did not alert but rather accelerated instead of applied regenerative breaking. Four days later the same incident happened with sudden unintended acceleration occurring while parking the car. When sending the car to Tesla for service due to the safety concern and damage this resulted in they said this was a user error which can’t be the case as this incident happened with two different drivers driving the same vehicle. When reading about this there have been numerous complaints which have shown that there is an electrical “ They show that the sudden unintended acceleration of Tesla cars may have been caused by negative voltage spikes that are logged as a fully depressed pedal, fooling both Tesla and the NHTSA investigators that there has been no hardware or software defect.”

I was driving at a very slow speed of 1 mile per hr as I entered into the Kohls complex in Pleasanton ca. Then as I was pulling the car to a parking spot, all of a sudden I felt the car accelerated on me. My foot was always on the break and I pressed the break but the car still accelerated at a speed I had no control and it went over the parking bump and hit 2 cars that was parked opposite. I felt out of control and scary because the car was not doing as intended, to stop when pressed the break. I then pressed the park button. Then I also reversed because the car was not stopping. There was totally 4 cars damaged including my car. 2 opposite and 1 parked side of me. I could feel the sudden acceleration in the car and out of control feeling. Luckily there were no people in other car so no one was hurt. I was also not hurt. - Not sure what malfunction happened , but the break in that instant of time was not acting as it should for few seconds. Yes, the car is available for inspection - My safety was at risk as the car was no behaving the way it should and I got scared as to what would happened next. Since there were no people in the other car, there was no risk. but if there were they probably would have got hurt. - I reported the issue to Tesla center. They said they will look but never heard back anything from them . - None of them has inspected the vehicle yet. - No warning signs were there. after it hit i could hear the beep

I bought a brand new tesla, only to find out that it was not emitting any sound slowly driving forward, but only when in reverse. I read through the cars manual and there it says the car should make a sound moving forward under 19mph, to warn pedestrians of the oncoming tesla (my car). If the car does not do that, then to immediately contact Tesla service. I did that and Actually so was there at my Tesla service station in Burbank CA in person and they said to me, I should not worry what their own manual for the car says. The car was going at the speed limit or maybe 5-9 miles over speed limit when brakes were pumped to stop car as there was stop and go traffic on a bridge in bay area (CA). The car had quite a distance from the car in front of it. Despite brakes being applied, the car seemed to accelerate and brakes had to be applied hard. However, the car did not stop until it hit the honda in front of it. As the driver of the Telsa at the time of the accident, it seems as though car was accelerating despite brakes being applied. A huge sound was heard by me the driver and then the driver eat belt became malfunctioned and the car would not restart. After a few minutes, the car restarted but now the dash board states service needed and I could no longer fasten the seat belt. The front of the car appeared to have minimal bumper damage but the seat has not worked since the accident.

I bought Tesla in May 2023. My Tesla Model Y has suddenly accelerated in my garage on 08/29 @8.25pm EST and damaged my garage front door/drywall and broken my stair case in pieces. It was phantom acceleration. I have video from my ring security system camera video which shows clearly that I was parking in my garage extremely carefully, however the Tesla Model Y suddenly accelerated beyond my control (I was not accelerating) and put me under risk and damaged my property. I have auto insurance to cover my Tesla car damage. However, my biggest fear is how can I use this Tesla Model Y when we see this life threatening behavior. This unintended acceleration can put my life and others life into danger. I would like help from NHTSA to ensure my Tesla Model Y is defect free. I just bought couple of months ago. I would like to request NHTSA to help me get this product replaced from Tesla motors. I am afraid to drive this vehicle now. I am currently going through a big shock, trauma and fear that I may have severely injured people on the road or my family members in garage if this unintended sudden acceleration would have happened around them. The damaged vehicle is outside my garage. It is not drivable. Vehicle front bumper is completely damaged and fluid has been leaking. I have filed insurance claim. I don't know what should I do right now. I would like to seek advice from legal sources on my options. My car is covered under my insurance, however I don't know if my car is fixed - I can drive this vehicle with confidence due to this strange unintended acceleration issues. I was pulling up to a ticket machine in a parking garage. I had my foot on the accelerator and was using regenerative braking and was essentially stopped. I was short a few inches from the button to get the ticket so applied almost no pressure to the accelerator – more like a tap so I could go forward a few inches. The car went into full acceleration mode, broke through the ticket machine barrier and kept accelerating. As soon as I sensed going forward too fast, I took my foot off the accelerator and put it onto the brake. I did not feel any response from the brake. With my foot. I managed to steer away from a wall in front of the car but hit a parked car. The impact pushed that car into a second car and then that car pushed into a third car and the third car pushed into a fourth car. My car was totaled as well as three others. Fortunately, the impacted vehicles were unoccupied and there were no pedestrians. No warning beeps or emergency braking occurred. One of my passengers had minor injuries (3 fractured ribs). A police report was filed. Previously (3 or 4 weeks prior to crash) I had an incident in an above ground parking lot. I had pulled into a parking space, backed up to straighten out then was pulling forward towards the curb. I took my foot off the accelerator, but the car kept going very slowly instead of regeneratively braking as it had done before. I put my foot on the brake. It was unresponsive. I kept pressing and finally it engaged, and I was able to stop the car about five feet over the curb and into the grass.

This is a report of a RECURRING problem commonly known as "phantom braking." When I take long trips in this vehicle at legal freeway speeds and with the Adaptive Cruise Control and Auto Steer (lane keeping) functions engaged, the vehicle will occasionally reduce speed suddenly and violently (emergency braking) even if there are no vehicles ahead and no visible hazards whatsoever. This occurs without warning and for no reason when cruising on open, straight interstate highways. In a typical 5-8 hour driving day on long trips, this will occur 1-2 times daily. If other vehicles were to be following closely behind, this would certainly cause a serious rear end collision or pile up. The vehicle gives no advance warning and no alarms sound. The car just brakes suddenly and violently scaring the bejesus out of my spouse. I have learned to step hard on the accelerator the moment this occurs which will partially offset the severe braking action so that any vehicles behind do not need to brake suddenly. I do wonder what the heck any drivers behind think when they see the Tesla suddenly brake hard then accelerate immediately back to speed. I worries me too that some aggressive driver behind might think I am doing this intentionally and get into a road rage incident with me. People have been shot and killed for less. I don't understand why this has not yet been addressed seriously by Tesla or NHTSA. This is a SERIOUS and dangerous defect! Thank you.

Concern: The "Autopilot" system in my 2023 Tesla Model Y AWD LR has shown diminished performance, particularly in adaptive cruise control, compared to the 2018 Model 3 RWD Long Range. Account: I aim to notify NHTSA about a concerning "Autopilot" behavior in my 2023 Model Y AWD LR. I am comparing this to the performance of my previous 2018 Model 3 RWD Long Range, driven under similar conditions. Incidents: Near Accidents: During commutes, my Model Y AWD LR frequently doesn't slow timely. In some cases, I've had to override Autopilot and brake abruptly to avoid collisions by risking letting the car either fail to brake fully or slam on the brakes so hard. The car's sudden deceleration at the last moment is concerning as it is not only a dangerous scenario for myself and my occupants, but also other drivers behind me, who might also crash. Conditions: These instances took place during daylight with a spotless vehicle, ensuring no camera obstructions. Settings: I set the distance threshold between levels 4 and 5 and ensured the speed was no more than 10 mph above the speed limit. Lane Keeping: The lane-keeping function is efficient. However, adaptive cruise control's reliability is questionable. Sensor Absence: The 2023 Model Y lacks radar and USS (ultrasonic sensors). This could be affecting the performance drop I've noticed. Accidents: Due to these issues, I've narrowly missed three accidents just this week alone, endangering my safety and others. Conclusion & Request: While Autopilot should enhance safety, my experience with the 2023 Model Y AWD LR suggests otherwise. This unexpected performance dip, especially with supposed hardware upgrades, is alarming. I urge NHTSA to investigate this issue swiftly. On 2 occasions, my 2023 Tesla Model Y, accelerated more than expected with the slightest pressure to the accelerator pedal. No one was injured and no damage done, however, damage and injury COULD have resulted if this happened at a different place and time. At this time, I have not contacted the dealer as recent Google searches revealed other such complaints having been reported. The outcome was determined to be driver error each time. This, because the vehicle has no way of recording speed at time of accelerator depression. The acceleration was NOT in proportion to the surprise result, instead a "spurt of speed" rather than the slower anticipated speed required when pulling into the garage or into a parking spot. These 2 episodes occurred after a time of, what could be considered, a normal day of driving, parking, stopping and accelerating at intersections, in parking lots and regular street driving.

The car randomly brakes dangerously hard at overpasses while at highway speed for zero reason while in cruise control mode. This has occurred four times during our brief ownership. I have to imagine these events WILL lead to an accident as the braking is so hard (as if you applied brakes in an emergency) that a driver caught off guard could easily loose control of the vehicle. We have attempted to contact the manufacturer several times, but no one answers calls and their website will not accept inquiries into these highly unsafe incidents to know if there is a problem with our car or software. In fact they hide behind technology so well I do not believe thee is a way to ever talk to a real person or file a written complaint. Please help!

While using adaptive cruise control on 2023 Tesla Model Y we experienced multiple instances of emergency braking (rapid deceleration w/o touching brakes, accelerator steering column, etc.) without any warning during multiple scenarios - dusk, daylight, sunny and cloudy weather. When traveling on the highway at cruising speeds of 65mph or higher while in "cruise control" the car will suddenly slam the brakes on. Luckily there has not been any one close to me to cause a rear end collision. It seems to be random but there are times when it happens more often. On a two lane highway or if a car in front of me is slowing down to make a turn. One time a car entered the highway going in my direction from the shoulder and kicked up dust and debris. That seemed to confuse the cruise control and then it finally decided to slam on the brakes.. The only way to correct it was to press the accelerator. Sometimes the braking is severe and other time mild but always unpredictable.

On 30th July 2023 while driving on interstate 8 freeway while using the FULL self driving our vehicle applied auto brake without anyone around. This air rake was so strong that it slowed the vehicle from 75 mph to less than 50 mph in seconds. It scared everybody in the car. We took off the autopilot and applied the regular autopilot and it happened again two more times. Each time there was no vehicles around us and luckily no one was behind us.

While driving about 75 mph on an expressway using autopilot or just cruise control, the car decelerated to about 40mph within a few seconds. Thankfully there were no other cars behind me. They likely would have crashed into me if so. I believe Tesla calls this problem “phantom braking.” There were no other cars or objects in front of me when this happened. This happened all within 2-3 hours on a road trip in sunny and dry weather on a pristine road. The first time was on “auto pilot.” After that scary experience, I only used cruse control and it happen twice more on the same trip. Again, weather was clear, it was during daylight and there were no other cars or obstacles in front of me when the “phantom breaking” occurred. Tesla said to hit the car icon on the car’s control screen if it happens again. Frankly, I feel it is unsafe to use autopilot or cruise control to do so to try and duplicate the experience to report it for Tesla.

What component or system failed or malfunctioned, and is it available for inspection upon request?: I got delivery of my new Model Y and odometer was reading 3 miles, i took the delivery and drove 10 miles and it stuck the steering wheel , "rear motor got disabled alert" and vehicle went to slow speed on freeway all of sudden, i had truck behind me it would ended up very bad for me and family, i had to immediately put emergency signal and stop on road side, i tried to call tesla road safety helpline and they were totally busy and did not receive call. How was your safety or the safety of others put at risk? ; this car "rear motor disabled " twice in middle of freeway , Has the problem been reproduced or confirmed by a dealer or independent service center?: Yes they confirmed but unable to give me more details . Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? : No they kept it secret with less info. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear?: No warnings, just stopped over in middle of freeway . so scary specially to new car. I would like NHTSA to inspect all new model Y car which have problems reported please check online and see the percentage of people received this kind of risky issues.

I experienced multiple dangerous phantom braking instances on Eastbound I10 between Van Horn and Ozona, Texas when the TACC system was in use. The car is on firmware 2023.20.9. There were no obstructions in the road and no traffic in front of the vehicle when the car suddenly braked going from 80mph to 50mph in a second before I could take control and accelerate. In two instances, cars were behind me creating a dangerous situation both in terms of an unexpected slow down on a barren stretch of interstate and possible road rage if motorists believed I was brake checking. There were no warning messages on the screen and no audible warning sounds. This is a consistent problem with this vehicle and seems to happen regularly on desert interstates or other highways where there are not many trees on bright sunny days. These events seem to happen on stretches of highway in which there may be a loss of cell signal so I wonder if that is a contributing factor. Regardless, Tesla needs to fix this issue as it is unsafe.

Tesla Model Y purchased on 06/23/2023 from Springfield NJ center. I had 2 accidents on the same day. I having been driving in the US from more than 5 years and never had a scratch on my vehicle nor even a parking ticket. i drove approx 600 miles in 2 weeks. So I was very comfortable in driving the tesla model y. Date of incidents - 07/07/2023. At Tinton Falls NJ. I was trying to take a K turn on a flat road at around 9:30 a.m. I put the car in Reverse. When I pressed the accelerator pedal the car went forward and hit an Electric Pole. I then pressed the brake pedal . Made sure the car is on Reverse Gear . On leaving the break pedal the car was automatically moving forward towards the pole. This should never had happen. I put the car in Park mode and waited for 5-10 mins approx. Then after some time when I put the reverse gear and left the brake pedal the car didnt move and on pressing acceleration it reversed. I parked my car at the site where I had my work. Police were called and they gave me a ticket. I had a sprain in my neck. Later by 2 pm I drove with the same car Home at Edison NJ. (1 hr drive) I called Tesla Customer Service and they advised me to take the car to Springfield NJ dealership as it could be a software glitch. I decided to drive the tesla with my family in other car to the dealership incase they decide to keep the car. I was about to enter into Fadem Rd when lifted my feet off the acceleration pedal to slow down. The car all off sudden accelerated with no steering control. I noticed lots of car on the street and a lady exiting out of near by Staples Drive Way. I grabbed the steering and rotated it all the way to the right to avoid any of them, hit a pole on the passenger side , pressing the brakes really hard and hitting a bush. The car stopped, no air bags deployed , no warning signs nothing. I was in a shock and pain . EMTs took me to ER , I had severe muscle sprain and still recovering.

Sudden unexplained Phantom Braking. Travelling on The Chisholm Trail Parkway tool road and while approaching an underpass the car suddenly applied brakes with no cars near me in front or to my sides. Fortunately there were no vehicles behind me. This similar situation also happened to me in my 2022 Model 3, so it is apparent that Tesla has not been able to resolve this long standing potentially serious traffic safety hazard.

I was about to park in car space in a parking lot. Instead of stopping in the car space, it has accelerated suddenly into the lawn in front of the car space. Tried to stop and it accelerated again and then hit the tree. The left front part of the model Y is damaged and is still in the body shop even after more than a month. I thought the only explanation is I have pressed accelerator instead of break. But as I think about it, I don't think I pressed the accelerator. It did sudden intended acceleration twice during that incidence.

While traveling on I70 from Columbia, Missouri to Grand Junction, Colorado and back, we experienced at least 30 incidents of sudden braking while in adaptive cruise control. Some of the incidents were rapid, causing the car to lose 20 to 30 mph within a second. For the most part these occurrences of sudden braking happened on open highway with no other vehicle within 1/2 mile in front of us. We discussed this issue extensively with Telsa and no resolution was provided by Tesla. In fact, one service agent stated that we should just be happy that we were driving a Tesla and that eventually Tesla may solve the problem.

The adaptive cruise control malfunctioned while engaged on the highway/interstate. On a recent trip of approx. 1,000 miles, the vehicle experienced "phantom braking" (aggressive emergency braking for no reason) on three occasions while the adaptive cruise control was engaged on the highway/interstate at approx. 65-75 mph, with no obstruction/vehicle in front and the weather was clear sunny skies. No warnings or messages were prompt by the vehicle. This abrupt phantom breaking could have caused serious injury from a rear-end collision or cause vehicles behind to take unnecessary unsafe emergency maneuvers/braking.

Tesla's Autopilot feature regularly experiences "phantom braking". A typical scenario is while traveling on an interstate highway in Idaho, Oregon or Washington state at the speed limit (70 to 80 MPH), the car will suddenly and violently decelerate. The severity can vary between annoying to so extreme that the passenger is thrown forward hard enough to engage the retention mechanism in the seat belt. This happens under seemingly perfect conditions-- mid-day, clear skies, clear lane markings, smooth pavement in good condition, straight section of road. The deceleration can be overridden by pressing the accelerator or disengaging autopilot but by that time the risk of being rear-ended by anyone traveling close behind is very high. For this reason I've been using the feature less and less which leaves me without any form of basic cruise control. In two different trips of around 500 miles, phantom braking has occurred between 10 and 20 times each. In two different service requests, Tesla has confirmed that my car, including the vision system, is operating within normal specifications. Beyond that they're unwilling to engage in any form of diagnosis or investigation. There are two fundamental problems here: 1) Tesla's Autopilot feature in unreliable and dangerous 2) The car I purchased was advertised to have Cruise Control, implemented by Tesla with their Autopilot feature. Because of 1), it effectively has no Cruise Control.

Multiple moments of phantom braking 15+ times TODAY driving from North Platte, NE to Havelock, IA on cruise or auto pilot. Extremely dangerous, could've caused pile ups/rear end collisions. Screen would 50% of the time show a semi or vehicle in front of me when nothing was there. I tried cleaning the windshield, thought it helped for a bit then it happened again. This happened on both interstate driving and highway driving into the sun and not into the sun. Could be very hazardous - very abrupt emergency braking at interstate speed limits. at the Shelby, IA super charger, another Tesla owner expressed he had also just reported multiple phantom braking incidents today as well and reported it to Tesla.

Just returned from round trip from Kansas City Mo. to Denver Colorado. Used the cruise control quite a bit on the trip. On 10-15 occasions the regenerative braking automatically came on for no reason.In one instance I was driving the 75 mph speed limit and a car was following too close when the regenerative braking activated and almost caused a crash.It seemed a lot of the times it happened on a bridge but not all of the time.Nothing was in front of me that should have caused this to happen.

My Tesla Model Y is experiencing phantom braking, due presumably to a faulty collission avoidance safety system. With cruise control enabled (not autopilot with lane keeping, only cruise control by depressing the stalk once vs. twice for lane keep) the car will phantom brake, where it slams on the brakes thinking their is a vehicle in its path when there is no vehicle in the front lane. This has happened to us on a clear toll road in Southern California on Hwy 73, both on June 7th, 2023 and in April/May 2023, where the Tesla slams on its brakes, decelerating us from 65mph to 38mph and below (at which point I slammed the accelerator pedal to avoid a rear end collission), with vehicles directly behind us, almost causing a dangerous accident. This happened with my three children in the back seat. We have experiencing this phantom braking twice in the last 2 months. In all cases the vehicle was well centered in the lane and there was no vehicle in the lane ahead of us, nor did we recently change lanes. I created a service ticket with Tesla, including bringing it into the service center at Lake Forest, CA, as scheduled, and they explained in person that there is nothing they can do, that they are unable to retrieve any video footage from the vehicle, and sent us away without even looking at the vehicle, replacing any hardware, hardware testing, or test driving it for themselves. We are at a complete loss of what to do with this vehicle that is only 6 months old. The vehicle has no warning lights or error messages prior to any of these failures. I feel that this vehicle is unsafe for my family to drive and Tesla is not putting forth reasonable effort to fix the vehicle.

The Adaptive Speed Control will randomly, abruptly decelerate the vehicle for absolutely NO reason. This certainly is safety item if a car were following me. With the Tesla set in the "Hold" setting for regenerative battery charging purposes, this is the same as slamming on the brakes. Also, this vehicle now only utilizes cameras for sensing everything and it falls short in this area. Other times this happens includes: Orange cones in construction areas, a vehicle crowding the center line in multi-lane highways and also, vehicles traveling the same direction ahead of you in a curve. In these instances, cameras and or computers processing the camera information is getting it wrong...

I experience multiple and frequent phantom braking events when using the Active Cruise Control. This continues to occur every time I use cruise control, and occurs even on straight empty roads on bright sunny or overcast days, and creates a serious safety issue for any vehicles that are following. The deceleration is rapid because of the regenerative braking capabilities of the vehicle. There are no warnings lamps or indications associated with these random braking events. The vehicle has not been inspected by the manufacturer, but my research shows this is a common defect reported by many Tesla owners. Tesla should, at a minimum, provide a software option to deactivate Active Cruise Control and make it a non-traffic aware system until such time as a bug proof system can be produced.

We upgraded our auto pilot to the advanced auto pilot system and when driving from Denver to Santa Fe the Tesla would brake abruptly without notice or reason. The braking reduced speed by at least 30-40 mph in a matter of a second or two. This occurred 6 times during this drive. The concern is if this happens when other cars are driving behind our car, it will cause a collision.

This has been a consistent on-going issue with my brand new 2023 Tesla Model Y. While using Autopilots Traffic-Aware Cruise Control and the Autosteer feature the car will without warning begin braking. Each time is different in regards to severity of the breaking but it is typically like letting off the accelerator pedal which in a Tesla is comparable to significant breaking. Have taken two highway road trips and on both trips, the phantom breaking has occurred multiple times. Some of the incidents the car would go from 75MPH to 45MPH in a matter of seconds. From research done, this "Phantom Breaking" is a serious issue that Tesla says is due to software issues and not hardware issues. Our safety was put at risk because the person behind us might not be paying attention when cruising on open road, during daylight, etc and could rear-end our car. There have been no predictors or weather conditions that cause the braking. Usually, for us, has happened on sunny days on multi-lane highways with not a lot of traffic. Have taken car to dealer and they state it is a software issue. No warning signs that the breaking would happen and no objects or cars show up on the screen when it happens.

Consumer submitted service request. Consumer has listed summary of observations & incidents. Consumer listed several questions: Are all Tesla Y prone to phantom breaking? Did we get a lemon? Can we get it fixed? Can Tesla replace the computer? Can we get a replacement car? Can we make the sales go back and instead buy a safe car? Do you have to be in an accident, having someone injured or having someone killed for Tesla to provide a warning to customers of Tesla Y? As a new Tesla owner what I the next step? Can we trust our Tesla Y not to cause an accident? Should we garage the car until Tesla has made its needed software updates? Can we return the car to Tesla? Get it replaced?

Experienced dangerous situation while using Speed Control, aka: Cruise Control, with NO vehicles ahead of mine, a totally empty road in New Mexico on I-25, the Tesla Model Y with 2,000 miles on it suddenly braked, dropping the speed over 15mph, then suddenly accelerated, I read this is a known problem and is known as Phantom Braking. It happened several times during the trip, once with a vehicle behind me creating a VERY dangerous situation. I have stopped using this feature until Tesla offers a proven fix for the issue.

This caused the car behind me to almost rear end my car. Luckily I was quick enough to press the accelerator to avoid the accident. This same sudden braking happened a total of 4 times within a 200 mile trip, one of which almost caused an accident.

I have owned my new Tesla for about a month and it has had a recent software update. Today, I had the cruise control set to 75mph on the freeway and the car very suddenly reset the speed to 50 mph for no apparent reason, making the car rapidly and suddenly slow down. I figured this was just a fluke and later tried to use the cruise control again. This time the cruise control was set to 75 mph on the freeway and the car very suddenly reset the speed to 30 mph for no apparent reason, the car very suddenly hard braked on the freeway. I had to swerve in the other lane so the car behind me did not rear end me. Luckily I did not get into an accident but it was close. This is very dangerous and unacceptable to car safety. I have never experienced anything like this in my life. I have scheduled an appointment to have my new Tesla looked at but I want to notify the government about a serious safety issue with the Model Y 2023 Tesla.

We have had our car for under 3 months and have had repeated episodes (easily 2 to 3 dozen) of extremely rapid braking when the car is in cruise control on the open interstate road when there are no cars ahead of us or any other obstacles that might have triggered the camera sensor. Tried turning off the forward collision avoidance option and the events still happen. Have kept the windshield over the front camera areas clean. It has happened typically during sunny days as we have not usually used the cruise control in the rain. When these events happen there is typical rapid slowing of the vehicle by 20-30 mph within a few seconds. If a car was following close behind us, there would be a high chance of collision. There have been no warning messages or any signs that the vehicle is otherwise malfunctioning. When we contacted Tesla technical assistance, they emailed us a copy of the part of the car manual that says the feature might not function in bad weather, if the camera is not clean, in construction zones, etc. none of which describe the circumstances of when this malfunction happens in the car in broad daylight. They also said whenever it happens to send a bug report which the technicians will look at. They also said it might be fixed with the next update, which of course, it wasn't. Finally, in the same batch of emails, they sent email that said we could report our issue to NHTSA which without any other resolution method to the problem, is why I am reporting it here. It seems as if there are many Tesla owners who are reporting this problem online. It may have been less of a problem before Tesla removed the radar feature in the last year and depended solely on their front cameras to make automatic braking decisions for the car. A two-month old Tesla model Y encountered phantom braking at least 12 times in 5 days while Autopilot was activated. All incidents happened in a 4000 miles round trip on I-5 in California, I-10 in Arizona, and I-10 in Texas. The vehicle underwent automatic braking without any prior prediction. As a result of these incidents, there was a potential risk of the rear vehicle colliding with my own vehicle, or the potential risk of neck injury. These incidents happened on 05/08/2023, 05/09/2023, 05/19/2023, 05/20/2023, and 05/21/2023.

Driving at 80mph with cruise control on in daylight, car suddenly for no reason braked hard, this occured 2 times hard and several other times enough to alert us while driving and take Cruise Control off by touching brake pedal Car went immidiately from 80mph to 60mph throwing us forward in our seats.

Phantom breaking when adaptive cruise control is on. Manufacturer refuses to help and promises a software fix is on the way. No way to use cruise control without adaptive cruise control, so no way to use cruise control safely. I was pulled over today after the car decided to brake check a sheriff deputy. Lane departure warning activates for no apparent reason. Does not go away when the feature is disabled. Actively corrects for the non existent lane departure; came inches from crashing several times in 8,000mi. Tesla suggests I use an alternate route because this one is "too rough". It's an interstate. Forward collision warning activates for no apparent reason despite being set to off. Does not actively brake but it does make a startling chime.

While driving down the freeway on 4/29/2023 my Tesla Model Y slammed the brakes on for seemingly no reason without warning. This incident occurred with auto pilot and auto steering engaged. The day was clear and road was clear.

Tesla abruptly reduced speed by as much as 30 mph while using cruise control in various scenerios. @ lanes highway with oncoming traffic, divided highways with no traffic or obstacles nearby. It happened at least 6 different times while on a 1400 mile trip. There were no warnings issued in any of the occurances. I discontinued using cruise control because of this issue. I have an appointment to take the Tesla in on May 5, 2023 to correct the problem, but after researching this issue, I now know this is an ongoing problem At 2 of the supercharger stations we stopped at, 2 other Tesla drivers asked me if I had this issue, because they were experiencing it also.

While driving down the road in traffic, several errors started appearing on the Tesla screen, and the vehicle lost power brakes and steering, rendering it impossible to drive safely. The complete set of car systems that were disabled all at once was: stability control disabled traction control disabled automatic vehicle hold disabled lane departure avoidance unavailable ABS disabled steering assist reduced (no power steering) regenerative braking features temporarily limited automatic emergency braking disabled The car is only weeks old. We had the car towed to a Tesla repair center, and they said due to a wiring issue, the wheel weight on the right rear wheel was rubbing against the wire connecting the wheel speed sensor, which led to this cascading series of errors. I'm concerned this is a systemic issue with newer Model Ys. There's a QA or manufacturing defect, and Tesla, along with the NHTSA, needs to investigate this dangerous safety issue.

This is the problem know in Tesla world as "phantom braking." This happens to me frequently. I am driving on a clear road, no obstacles, and the car will brake hard for no clear reason. At best this is annoying. But there are times when it is extremely dangerous. This happens on highways and interstates at highway speeds. Drivers behind me are forced to brake hard to avoid a rear-end collision.

Automatic emergency braking occurs without cause at high speeds on flat divided Hwy. Happened on multiple trips 5 or more times in a 3 hour drive on 3 different highways in NM. Tesla service has addressed the issue twice. said everything is working properly. Happens when Cruse control + auto steering are active.

every time while driving at highway speed with cruise control enabled the car will decelerate or brake hard without anything in our path of travel for as far as the eye can see. this is a safety issue as the cars behind us may not be able to react fast enough to avoid hitting as our car brakes without cause.

New Tesla Model Y brake failed and accelerated resulted in a serious crash within 2 hrs of taking delivery. I believe it is acceleration functioning and brake due to auto pilot feature. Vehicle will be available for inspection after 4 weeks. After taking delivery of new Tesla Y, we drove to temple. While parking car accelerated and lost control of the vehicle as brakes were not working. It hit the temple wall and when we tried to backup to get the car out of the crashed wall, it got even worse. It reversed in lightning speed and hit another car, run over the bushes and huge palm tree across the street and finally stopped. This terrifying experience caused huge mental trauma, multiple property damage, and financial hardship that we are dealing with right now. Fortunately, there were no injuries. We lost all the trust and confidence in Tesla. We are NOT confident of driving due to huge safety concern. It is a very important safety issue considering the customers at large. Tesla service has not worked on the diagnosis. We tried reaching out Tesla service to investigate the failure. There is no response from them and they keep cancelling the service appointment. On top of it, there is no appointment available in body shop for a month. This incident was reported to police. One observation was made during the incident where Auto pilot beta was displaying on the dash board monitor without invoking any self driving options.

Around 3:54 PM EST, I was traveling on US 1 Route 1 North Bound, and I tried to turn right into Target Complex (4196 US-1, Monmouth Junction, NJ 08852) from Route 1. I slowed down to turn into the complex, and my vehicle had Sudden Unintended Acceleration while I am turning into a parking location. It jumped over the curbs and hit a tree. Airbags got deployed, and the car was damaged badly. Insurance deemed it a total loss. I saved videos from the crash from the USB. I can forward or upload them.

On a multi day drive across the country I had multiple episodes of automatic breaking while on autopilot and using Traffic aware cruise control. In all instances there were no vehicles in either lane ahead of my vehicle. Braking episodes did not result in an accident but made the features essentially unusable.

At highway speed, using the autopilot feature for speed control only, the car applies severe braking when there is nothing obstructing the road or requiring brakes. This has happened dozens of times over the first several thousand miles we have owned the new car. We filed a report with Tesla and they said this is not a problem and is expected behavior. We are concerned this will occur with a vehicle behind us resulting in being rear ended.

on 30th MAR 2023, when my wife drove on to our drive way with gentle speed, car rammed into closed garage door and broke the door and rammed into living room wall with breaking room entry door . there is neither emergency breaking /stop nor collision warning . it damaged wall , garage door and more living room property damage with addition to damage to the car. i am really worried about the safety of the car..

While driving South on I75 near Tampa Florida using Autopilot the car suddenly decelrated to 10mph. There were no cars next to me and we were passing through a 6 lane area where the road splits. No distinguishing features, no cars on either side and we were heading slightly up hill. It seems like the cameras could only see the sky. Fortunately there was no car directly behind me. Very scary.

Car was purchased December 30, 2022. The first time I engaged auto pilot, an oncoming semi was in its lane and my car slammed the brakes on while on auto pilot. It happened the next time I used auto pilot with zero perceived “threat”. I was extremely hesitant to use it, henceforth. I tried auto pilot over and over with my foot, poised and ready to take over the throttle, and my hand poised to disengage auto pilot. 12 times it slammed the brakes on-every time I used the feature it randomly braked, there was absolutely nothing around. These incidences were on two lane highways, no oncoming traffic, nothing in the road. Phantom braking. I do not feel safe using this feature on my brand new car because if someone is traveling behind me, it will cause an accident. I took my car in for minor service (for adjustment and stern issue) shortly after purchasing and mentioned this to the Tesla service Center representative. She said to me, “oh yeah, it does that sometimes. You have to be ready to take over.” Even being poised and ready to take over, I fear my reflexes will not be fast enough if there were someone traveling behind me at 60 mph. and it certainly isn’t how cruise control should work. I have a second service appointment, and actually noted the phantom braking (I subsequently learned this is what it’s called after googling, “random, braking with no threat Tesla model Y 2023”) on my service request to ensure that it has been documented because it’s a problem. A dangerous, unacceptable problem that needs to be fixed. My service appointment is 3/28/2023. Thank you.

The contact owns a 2023 Tesla Model Y. The contact stated that while at a stop with the brake pedal depressed, the vehicle unintendedly accelerated. The contact stated that no warning lights were illuminated. The contact stated that she rear-ended the vehicle in front as she turned the steering wheel to the left and landed in a ditch. No air bags deployed. The contact stated that she and her front-seat passenger sustained no injuries however the other vehicle driver was seen by the EMTs on the scene. A police report was filed. The vehicle was towed to an authorized Tesla Center and was awaiting diagnosis. The manufacturer was notified of the failure and a case was opened. The contact stated that she was informed by the manufacturer that a diagnosis team was working on the diagnosis. The approximate failure mileage was 473.

Vehicle brakes suddenly while driving even when there are no cars or obstacles of any kind in front or even in the distance. This has led to several events where I was almost rear-ended because of how strongly and violently the car was breaking on the highway. Last example was on March 5th 2023 at 1:19 p.m. while driving on interstate 90 in Ohio going westbound, there were no cars in front or in the distance in the lane. The car suddenly applied the brakes in a very strong fashion in the middle of the highway. There were at least five examples just like this one which happened prior.

Driving on highway with cruise control activated. No traffic around. Experienced phantom braking taking me from about 80mph to 65mph before I was able to react. Had this happened in traffic there is a good chance i would have been rear ended. No notifications or emergency lights appeared on my dash.

In auto pilot mode , my Tesla suddenly applied emergency brake and brought my model y from 65 to 10 min in 287 Highway. Luckily they was no car or truck behind me. No car in front of me also.

I was driving on the George Bush Turnpike on with cruise control set at 80mph. There were few cars on the road an no cars in front of mine. All of a sudden the car went from 80 down to 60 mph and I had to recover manually by pressing the acceleration pedal. The only interesting point is that there were 2 cars accelerating at a higher speed in the next two right lanes that were passing me and didn't cross into my lane. There were no cars within 10-15 car lengths in front of me. Thankfully there were also no cars behind me so I was not rear ended due to the sudden deceleration. I cannot get in to even discuss the issue as Tesla has failed to service the car after moving 5 scheduled appointments in the last month. They now tell me my next appointment is Feb 24. To be fair they are not aware of this specific issue...the appointment was originally for several other issues unrelated to safety.

We were driving our brand new Tesla Y long range (we picked it up by the end of November 2022, and it was less then 150 miles) on 01/22, there were 3 adults and 2 kids in the car, and the car lost power in the middle of freeway. We had to slide the car to the shoulder and call Tesla service. When we saw the warning message on the screen, we were NOT given any respond time, the car lost power immediately. We were almost killed by it. On 01/27, we were told that the car has been fixed. They said there is a definite failure in the drive motor that was documented. There was an issue with an integrated part of the drive unite called the inverter, which can limit power when it has an issue and was resolved by replacing the drive motor assembly with another complete assembly under warranty.

When returning from the holidays during the last week of December I was using the Tesla AutoPilot for it's cruise control on the open highway. I experienced the "Phantom Braking" twice during the three and a half hour trip. I reached out to Tesla but only received a canned response explaining how AutoPilot is engaged and disengaged.

We were driving on a three lane on the expressway with the autopilot engaged. all of the sudden and without any warning the car engage on the breaks. We didnt have any cars in front or side we did have a car behind us. This was an extreamly scare situation as it took us by surprised. This happened two times while engages on autopilot. We didnt use anymore after. I searched the web and there are thousands of complaint about it without a resolution. Tesla is no help following up. I hope you can do something about it.

When entering the vehicle, the touchscreen does not turn on. The vehicle can be operated but without the touchscreen on, the speedometer is not available. I believe this to be a safety issue.

I was driving in cruise control on the freeway going 70 mph with no other cars in front of me when suddenly the car slammed on the brakes and slowed to less than 40 mph before I could even react. The car behind me came close to crashing into me. The car continued to slam on the brakes every time I put it in cruise control. I am concerned going forward that if I use cruise control and someone is following close behind me, that they may crash into me if it happens again. I reported the incident to Tesla and they stated that they are aware of the issue and are working on a software fix. However, after researching into the issue, it looks like this issue has been occurring for multiple years with still no resolution.

While using “Autopilot” and “Traffic Aware Cruise Control” the vehicle will heavily apply the brakes when there are no forward obstructions or incidents. This causes issues for vehicles behind me and is very unsettling, as there never appears to be a reason for the dramatic reduction in speed.

I was driving at about 40 MPH on local road at rush hour, the car was put in parking mode when I made a lane change, the speed reduced to 0 rapidly and put me in extremely dangerous situation like being rear ended. then I cannot put car in drive mode and called maker's roadside assistance and 911. car was towed to service center. 1a. Q: How was your safety or the safety of others put at risk? A: car might stop quickly unexpectedly when travel at high speed, very likely being rear ended or overturned due to side swipe. 3. Q: Has the problem been reproduced or confirmed by a dealer or independent service center? A: UNKNOWN 4. Q: Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? A: car was towed to service center and was inspected & diagnosed. 5. Q: Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? A: no any warning, it happened unexpectedly.
