Center console USB component part#1522264-00-B, available for inspection. USB-C charging port that had been plugged in for more than a year, started smoking and wire melted while vehicle was parked. If this would have happened while my wife was on the highway on the way to work and smoke suddenly appeared, it would have cause her to be startled and degrade her ability on a busy highway. This would have most likely have created a traffic incident in Memphis on highway 240. I asked the dealer if there were any reports of defective part and I was told no. I brought the components in to show but the parts manager or service staff were not interested in the details or my concerns that smoke had been detected and that it melded the metal piece of USB-C charger to that TESLA part. I bought another replacement part from TESLA, but I am not utilizing the charging port for fear of smoke/fire. There were no warning lights or indication prior to the day of event where my wife observed burnt plastics and visible smoke. On November 26th called me in a panic that the car was smoking. She went out to our vehicle during her break and she smelt burnt plastic and observed smoke from the middle console. Shortly after she called me at my work, and after making sense of what she was telling me, I told her to unplug the USB-C but the parts manager melted of and the metal piece was stuck in the charging port. I left my work and after 30 minutes got to the vehicle, the burnt plastic was still evident but smoke no longer visible. I detached the panel with the part unplugged from power source, thank you YouTube for the DIY video. Pictures will be included with this report, but format doesn’t allow for video upload. The contact owns a 2024 Tesla Model Y. The contact stated that while driving at various speeds, the TPMS displayed that the PSI of the tires was low. Additionally, a notification advising that the vehicle was experiencing low voltage and drainage and would shut down was displayed. The contact called towing services. The contact stated that the towing agent who assisted in towing the vehicle informed the contact that the tires were losing air, and the front passenger's side tire or wheel was defective. The vehicle was towed to a Tesla Service Center, where the power converter system(PCS) was corrected. Additionally, the technician informed the contact that the passenger's side tire was leaking air due to a rim defect, but the vehicle was not repaired, and the contact was advised to pick up the vehicle. The contact stated that the vehicle could not be driven at 20 MPH due to the failure. The contact had not retrieved the vehicle from the Service Center. The contact related the failure to NHTSA Campaign Number: 24V935000 (Tires), but the Tesla Service Center informed the contact that the recall repair was completed. The contact stated that NHTSA Campaign Numbers: 24V554000 (Latches/Locks/Linkages, Electrical System) and 24V935000 (Tires) were marked as complete on the Manufacturer's website; however, the NHTSA VIN tool displayed that the recalls were incomplete. Additionally, the contact stated that the driver's seat was not adjustable. The contact stated that while attempting to move the seat higher and forward, the seat turned inward towards the right. I’m reaching out to document and summarize an incident that occurred in Sanford, Florida, and to ask for the appropriate next steps and any documentation you may need from me. Incident summary (Sanford, FL – Appaloosa Court – Dec 2, 2025): In the early morning on Tuesday, December 2, 2025 (around 5:00–5:15 a.m.), a fire occurred at a residence located on [XXX] . A Tesla located in the garage was reported as being involved, and the fire spread to the garage that totaled the house The vehicle was reportedly charging in the garage when smoke/fire was noticed. Emergency responders (Sanford Fire Department and Sanford Police) responded to the scene. The incident resulted in property damage and at least one injury reported as non-life-threatening (per local reporting). Fire forensics stated that the fire started and caused by the TESLA around the charging plug area by the rear left passenger door. On [XXX] at ([XXX]]) my Tesla Model Y (VIN: [XXX] ) was **parked in Park/Hold mode** in the [XXX] The **GPS navigation was set to an address in the center of the park**. the address on the address log of the car navigation system. The **TESLA GPS navigation destination was set to a point marked in the middle of the park** (not a parking spot or road). **Without any driver input** (no accelerator pedal pressed, no hands on wheel), the vehicle **suddenly shifted into Drive, jumped the curb, and accelerated approximately more than 30 feet] into the park grass**. This caused **damage to wheels, and all the passenger side of the vehicle to impact with a large rock placed in the park **Zero pedal input** — confirmed by driver and will be verified in Tesla server logs. No injuries. I am writing to formally report a serious malfunction involving my Tesla vehicle. On my 2024 Tesla model y.I experienced an unexpected accelerator malfunction that caused the vehicle to surge forward uncontrollably. The incident occurred at approximately As I attempted to dive in slowly, the accelerator suddenly malfunctioned and my car creeped in slowly and rapidly increased power without my intention. Despite immediate attempts to brake and regain control, the vehicle continued to accelerate and subsequently collided with a gate, resulting in significant front-end damage. I firmly believe this was due to a mechanical or electronic failure in the vehicle’s accelerator or related control systems. The contact stated that while driving at an undisclosed speed, the front driver’s side seat failed to adjust to the intended position while the digital screen was in use. No warning light was illuminated. In addition, the contact stated that when the manual front driver’s side seat control pad was in use, the manual front seat control pad felt fragile and moved the front driver’s side seat forward into the steering wheel. The contact said that her daughter drove through an intersection and crashed into another vehicle. The contact's vehicle crashed into the other vehicle's driver's left front door. The contact stated that her vehicle's front end was crushed, and the other vehicle's driver's side door, driver's side rear wheel, and tailgate were damaged. The contact stated that both vehicles had to be towed. The contact stated that the air bags had deployed in her vehicle. The contact stated she drove her daughter to the hospital. The daughter only had pain in her neck and shoulders; there were no cuts or abrasions. The police arrived and issued an incident report. The vehicle was towed to an impound lot, and the contact was advised by her insurance provider that the vehicle was a total loss. The manufacturer was not informed of the failure. I am writing to formally raise a complaint regarding a serious malfunction that occurred in my Tesla Model Y (2024) following a recent software update pushed by Tesla. A couple of days ago, my vehicle received a new over-the-air software update. After installation, my car began displaying two error messages, and key safety features, including detection of nearby vehicles and warning systems, ceased functioning. This creates a significant safety risk when driving, as the car no longer alerts me to surrounding traffic or visualizes other vehicles on screen. Upon contacting Tesla customer service, I was instructed to schedule a service appointment, which I did for May 21, 2025, at 2:15 PM. During the appointment, the Tesla service advisor confirmed that the issue began only after the software update. However, I was told I would need to pay $3,000 out-of-pocket to repair the issue because my car is no longer under warranty. I raised the point that the malfunction was caused directly by Tesla’s software update. Despite this, I was informed that Tesla refuses to repair it at their cost, even after acknowledging the connection between the update and the malfunction. Moreover, the technician informed me that this issue could escalate and create safety hazards in 4–6 months, yet no resolution or recall has been offered. Given the acknowledgment that: The problem was introduced by Tesla's software, The malfunction affects critical safety systems, and There is imminent risk to safety, I believe Tesla should bear responsibility for the repair and ensure the vehicle is safe to operate. This situation warrants urgent attention, both from a consumer protection and vehicle safety standpoint. I request that Tesla take full accountability and cover the repair costs immediately, or offer a rollback/fix to the software that caused the issue. AP4 computer failure has led to navigation, cameras, blind spot monitoring, lane departure and other systems being inoperable. Tesla has stated they don’t have the parts to fix and have pushed out my service repair time twice already. Issue reported on December 4th, since then no time table for repair has been given. Tesla keeps stating the car is operable, leaving out the facts it’s dangerous to drive due to small rear windows and blind spots based on the cars design to work with operable cameras that give the drive clearer angles to view.

All safety sensors appear to be out of service. Navigation is stuck at one location and is not working. All cameras, including backup, are out of operation (only shows dark screen). Back up sensor does not work. This issue has been ongoing for 1 month on a brand new vehicle. Tesla service center has no ETA on resolution. Service appointment has already been pushed back to next year (Jan 2025).

I had my computer replaced (AP4) on the Model Y. After a software update around Nov 13, my computer glitched. The car was drivable, but no functionality (cameras, navigation. Etc). After the computer was replaced it seems fine. Just wanted to report after the fact. Thanks

Cameras not working with latest Software update on November 26th. without even inspecting the car in-person, Tesla service agent told need new AP4 computer on 2024 model Y which is 1 month old . I think Tesla knows about this issue and there is lot of wait time for getting the AP4 computer. Tesla service advisor states - car is totally safe to drive with just mirrors till appointment in few weeks. As per NHTSA, if a car is not having a backup camera, that is a safety violation and there are lots of Tesla vehicles driving without backup cameras. No backup camera, navigation, battery life dying faster. It seems like a liability to me to drive this way and very negligent to me.

Bought my Tesla Model Y on [XXX]. Exactly one month later on [XXX] I start my car to find that none of the cameras are working. This means Autopilot, cruise control, rear camera, blind spot monitoring and many more safety features are completely down. In addition the GPS does not function meaning my car is not trackable and Maps don't work, another safety concern and main reason we bought the car. This is a widespread issue that Tesla Service Centers have been informed about from Tesla that it is a bad computer that needs to be replaced - in a 1 MONTH old car. My only option is to wait for a computer to be assigned to me which is at earliest on January 21st of 2025, almost 6 weeks without a fully functioning brand new $46,000 Car with only 1400 miles on it. Tesla are also unwilling to make any kind of restitution such as a loaner car, or a refund for the Full Self Drive and Connectivity packages I just purchased but cannot use as my car is broken. Tesla has told me the AP4 computer has failed in my car which has resulted in them having to replace it. I had this issue start right around Thanksgiving. I scheduled the soonest appointment available for 12/16 however Tesla contacted me and moved my appointment because they stated that the part was unavailable at this time and should be delivered by 01/20/25. Not offering me a rental or anything on my brand new '24 Tesla. It has just over 1,000 miles on it and is ~3 months old now. This issue impacts: emergency braking, blind spot detection, lane keep/centering, assisting with parking, automatic high beams and wipers, cruise control/ adaptive cruise control, Vehicles built in GPS to be stuck at one location every time, has made my touchscreen not automatically brighten or dim, my auto dimming mirrors be to dim in most driving conditions, constantly fail to install system improvements and safety updates, and because my vehicle keeps trying to do this, it has caused increased battery drain as well. This has increased the safety risk of my vehicle tremendously... Tesla has not seemed to publicly acknowledge the issue yet either.

Vehicle cameras stopped working. Navigation stopped working. Screen also stopped working for a time. Was able to get it back on but it went off multiple times since. If I can't see the screen, I can't tell how fast I'm going to do anything else. Cameras control many of not all of the safety systems. So those are all inoperable. Issue is being reported across multiple forums all over the internet as "short in ap4 module."

Since I purchased the car 4 months ago my car has been going into the service center non stop for every issue you can imagine. Tesla spins and blames all issues and ignores me. Corporate says they can’t do anything and then ignore. Service center providing inaccurate or not providing service invoices.Deny my buyback requests. Repairs are failed and new damage occurs at every service appointment. New issues arise with the car every time I drive it. I rarely drive it as it’s not safe. Tesla continually cancels service appointments or does not have parts in stock. issues: -Safety restaurant system fault -electric motor issues: loud high pitched whistle/whine when driving. -car loosing power while driving -seat moving while driving -window clicking -water dripping out of dash vents. -Misalignments: frunk, trunk, trim -paint drips, bubbles, cracks, peeling -battery range issues -battery coolant fan issues -defects interior plastic surfaces -seat defects -hole left in front of car between dash and windshield with damages windshield. All safety features and cameras on this vehicle are currently disabled. Specifically, the following functions are not operational: Forward and parking collision warnings, Blind spot warning, Lane departure and lane-keeping warnings, Rear cross-traffic warning, Self-driving capability, Adaptive cruise control, Navigation system, Location tracking, Sentry mode, Premium connectivity features, Reverse camera, Front and side cameras. Additionally: the headlights remain on continuously during daylight hours. The interior control panel screen and the side and rear mirror displays are stuck in night mode.

With beta self driving engaged, I turned south on [XXX] intending to turn west on [XXX]. At [XXX], there are 2 right turn lanes. I approached the intersection turning from the left lane. Rather than turn right, the car continued straight. The contact owns a 2024 Tesla Model Y. The contact stated that while at an undisclosed speed, the vehicle had accelerated independently. No warning lights were illuminated. On another occasion, while exiting a parking lot the vehicle unintendedly accelerated causing it to crash into a parked vehicle. The vehicle came to a stop partially sideways after it drove over the parked vehicle. The fire department arrived on the scene and safely removed the contact from the vehicle. It was unknown if a police or fire department report was filed. The contact sustained minor bruises on the right arm and sought medical attention a day later. The vehicle was towed to an insurance lot where it remained for further inspection. The vehicle was not repaired. The manufacturer was made aware of the failure. The failure mileage was unknown.

I was on [XXX] heading towards Chicago and following the traffic. I started changing the line to pass the vehicle in front of me and suddenly my tesla completely lost control and started drifting to the sides. I released the accelerator right away but it made it even worse. It seems that acceleration continues to happen that’s what made car to drift. Eventually car slowed down and I was able to make it to my line. I don’t understand why line assistance didn’t work to keep the vehicle stable, why after releasing accelerator the vehicle continued to gain the speed. It was sunny day and dry road. Jun 10, 2024 Crash

The Tesla accelerated by itself while I am driving in a heavy traffic. To avoid hitting the front car, I braked several times, which didn’t work at all. It still accelerated, instinctively I steered wheels to the right front and hit a small hill. Airbags are out. I struggled to get off the car and shouted for help. 911 was called and emergency ambulance took me to ER. Luckily I am alive, it could be much worse that I would tremble when thinking of that crash scene again. This happened on [XXX]. The vehicle was taken to a Tesla Service Center several times; however, the failure could not be replicated. The vehicle was not diagnosed or repaired. The manufacturer was notified of the failure. I purchased my Model Y with the Tow Package. We used the trailer for the first time. There was no 12v auxiliary power to the trailer. The 7 pin trailer connection is incomplete. Post purchase, we found Tesla purposely disconnected the 12v accessory power pin when they switched low voltage systems in the 2022 model year rather than add the necessary power reduction part to send 12v power to the trailer. This is a serious safety issue because the 12v accessory pin on the trailer connection is the used to maintain charge on emergency trailer surge braking systems (engages trailer breaks if the trailer disconnects from vehicle). This known safety hazard was not conspicuously communicated before purchase. A notice does not appear on the Model Y Tow package page (attached). This is very uncommon and unexpected for manufacturer installed tow packages with 7 pin connectors to leave the 12v auxiliary power disconnected.

The battery coolant started leaking when we connected the car to a supercharger. After charging a low coolant warning came up on the screen, but the message read “OK to drive”. Started driving home when more alerts came up “Power reduced” and “Speed limited to 68 mph” but still showed “OK to drive”. About 30 miles later the screen abruptly shut down on the freeway. Was able to pull over to a stop on the shoulder and call a tow truck. Screen never came back up even after attempting to restart, car is now at the service center. There is a pump that has continued running for over 2 days without stopping. The interior lights turn on and off every 30 seconds. There has been no damage from road debris or anything else to this car. The vehicle only had ~350 miles on it, and we only had it for 3 days before it malfunctioned. The screen displays vital information such as speed, turn indicators, and so on. Without the screen being on it is not possible to safely operate the vehicle.

Electrical system had 7 alerts/warning scroll on screen. Said vehicle was going to shut down. Regenerative braking system failed. Pulled car into a parking spot. Vehicle would not go into park so I could not take foot off of brake. Started to shift between park and neutral randomly. Was told by Tesla to reboot car. Did this three times, but nothing changed. Tesla dealership reproduced and confirmed. There were no warnings or messages before the incident.

Unknown. Drove car in front of garage. Car came to a full stop. About to put car into reverse from drive in order to back into the garage, when car suddenly accelerated forward. Turned wheel sharply to the left in order avoid hitting neighbors fences. Vehicle traveled in an arch and collided with a tree next to an fence and stopped.
