Date & Time: December 25, approximately 6:00 PM Location: Northbound I-880 between the Mowry Avenue and Thornton Avenue exits, Fremont, CA Weather & Road Conditions: Heavy rain at the time of the incident. Road surface was wet with reduced visibility. Vehicle & System State: Tesla Model Y. Driver assistance / self-driving functionality was active at the time of the incident (traffic-aware cruise control / Autopilot functionality). Description of the Incident: While driving northbound on I-880 in heavy rain, the vehicle had been operating normally. Without warning, the vehicle appeared to incorrectly detect that it was drifting out of its lane and attempted to correct this perceived drift. Instead of a minor lane correction, the vehicle abruptly applied heavy braking and initiated a sudden and aggressive left steering maneuver. The car crossed approximately four lanes of traffic in the same direction of travel and continued rotating, ultimately performing an unintended U-turn while remaining on the northbound side of the highway. As a result, the vehicle came to a stop oriented against the direction of travel, facing oncoming northbound traffic. Immediately afterward, the vehicle unexpectedly began reversing at high speed without driver input, creating an additional imminent risk of collision with the outer highway wall (noise barrier separating the highway from adjacent residential areas). During this entire sequence of events, I was unable to regain control of the vehicle despite attempting to intervene. There were no vehicles immediately surrounding us at that moment. Had there been nearby traffic, this incident would likely have resulted in a severe or fatal multi-vehicle collision. Occupants: My wife and two young children were in the vehicle. The children were asleep in the back seat, secured in car seats. Outcome: No physical collision occurred. However, the incident caused extreme fear and distress, and we remain shaken by the event.
My 2024 Tesla Model Y hit a parked vehicle on the right side of the street during FSD self driving. The car suddently slanted to the right and hit the parked vehicle. During FSD drive, it did not give any warning before the accident. My dauther was injured on her face. Clearly the new version FSD v14.1 software is very defective. I am located in San Jose, CA.
I would like to report a significant incident involving the auto-corrective steering in my vehicle. While driving on the highway, the steering wheel experienced a strong jolt, which I believe was caused by the auto-corrective steering system. This resulted in the vehicle spinning three times and ultimately coming to rest on the grassy center divider. Fortunately, no damage occurred at that time, and I initially attributed the incident to road conditions. I continued driving for approximately 30 minutes in rainy weather without further incident. However, a few hours later, during my return journey, the same forceful jolt to the steering wheel recurred about 20 minutes into the drive. On this occasion, the car spun 1.5 times and subsequently collided backward with the concrete center divider. It is fortunate that no other vehicles were involved, given the heavy traffic and rainy conditions at the time. During both of the incidents, I had both hands on the steering wheel and was not able to correct steering due to the strong force from the steering wheel jolt. I did have a Police Report filed, but have to get number when I return home.
On [XXX] at ([XXX]]) my Tesla Model Y (VIN: [XXX] ) was **parked in Park/Hold mode** in the [XXX] The **GPS navigation was set to an address in the center of the park**. the address on the address log of the car navigation system. The **TESLA GPS navigation destination was set to a point marked in the middle of the park** (not a parking spot or road). **Without any driver input** (no accelerator pedal pressed, no hands on wheel), the vehicle **suddenly shifted into Drive, jumped the curb, and accelerated approximately more than 30 feet] into the park grass**. This caused **damage to wheels, and all the passenger side of the vehicle to impact with a large rock placed in the park **Zero pedal input** — confirmed by driver and will be verified in Tesla server logs. No injuries. No other vehicles involved. **Root cause**: **Full Self-Driving (FSD) software / navigation system** incorrectly engaged **Lane Keeping Assistance** while parked, triggering **unintended acceleration** via **Speed Control** logic. The car was on software update and suddenly disappeared. The car **autonomously drove itself** toward the GPS point **from a complete stop**. Police report filed to LAPD (Case #[XXX]). I would like to report an incident where Tesla FSD got confused by tire marks on a bridge deck and, soon after, by shadows on pavement. On both occasions, FSD steered the car across the double yellow before returning to its lane. There was no traffic and the car slowed down, so I let it play to see what would happen. I am writing to formally raise a complaint regarding a serious malfunction that occurred in my Tesla Model Y (2024) following a recent software update pushed by Tesla. A couple of days ago, my vehicle received a new over-the-air software update. After installation, my car began displaying two error messages, and key safety features, including detection of nearby vehicles and warning systems, ceased functioning. This creates a significant safety risk when driving, as the car no longer alerts me to surrounding traffic or visualizes other vehicles on screen. Upon contacting Tesla customer service, I was instructed to schedule a service appointment, which I did for May 21, 2025, at 2:15 PM. During the appointment, the Tesla service advisor confirmed that the issue began only after the software update. However, I was told I would need to pay $3,000 out-of-pocket to repair the issue because my car is no longer under warranty. I raised the point that the malfunction was caused directly by Tesla’s software update. Despite this, I was informed that Tesla refuses to repair it at their cost, even after acknowledging the connection between the update and the malfunction. Moreover, the technician informed me that this issue could escalate and create safety hazards in 4–6 months, yet no resolution or recall has been offered. Given the acknowledgment that: The problem was introduced by Tesla's software, The malfunction affects critical safety systems, and There is imminent risk to safety, I believe Tesla should bear responsibility for the repair and ensure the vehicle is safe to operate. This situation warrants urgent attention, both from a consumer protection and vehicle safety standpoint. Back in late November 2024, Tesla pushed an Over-The-Air (OTA) update to my 2024 Tesla Model Y. Immediately after pushing this OTA, all of my car's cameras stopped working (INCLUDING my rear or "backup" camera). This is in violation of federal law as all new vehicles are required to have a backup camera. Additional safety-critical items were also impacted: the rear-view and both of the side mirrors remain tinted to the darkest level, reducing my visibility. The side mirrors are also tilted in the "downward" position, making lane changes incredibly difficult. Since the front-facing and side-facing cameras do not work the vehicle's forward and side collision detection systems are inoperable. The vehicle suffers from additional failures (GPS navigation, Maps, Autopilot, among many others). I have presented these issues to Tesla and they have continously pushed out my repair date. I have also asked for a loaner vehicle due to the failure of the car's safety systems (some of which are federally mandated!!). Tesla has ignored my pleas for a loaner vehicle and has also remained silent when I have repeatedly asked to return the vehicle under Washington State lemon laws. My safety and the safety of others have been put at grave risk due to the backup camera and collision detection cameras not working. Tesla has NOT inspected the vehicle as they continuously push my service date further out. No obvious warning lamps, messages, or other symptoms appear. The user doesn't know these federally-mandated safety features are inoperable until they put the car in "reverse" or attempt a lane change.
Around the end of November, my 2024 Tesla Model Y's cameras stopped functioning. The sensors for blind spots and possible collision prevention stopped working. The GPS was also not working, the car believes it's been in the same spot for over a month. The lights of the car also do not work as promised, as now I have to manually turn them on on my dashboard and it goes on and off. I have been trying to get into Tesla to have this fixed but the appointment keeps getting pushed out due to the part that needs to be fixed not being in stock. I brought my car into Tesla for them to confirm that it is the part that is missing and that is the case but they couldn't fix it because they don't have the part. It's been over a month and my date was pushed out again, probably the sixth or seventh time. Tesla claims that there will be a software update that will fix this issue, however, my car is not updating and won't even accept a software update.

Software update applied OTA. Complete failure of ECU on reboot, including cameras, rear backup camera, all proximity warnings, cruise control, FSD, GPS, navigation, battery preheating, etc. Car is unsafe to drive in this state.
Hello , I just bought the new Tesla Model Y 2024 in the October and all of a sudden all the driver assistance programs are not working, including the Rear Camera(Reverse camera) I took it for the service on December 4th and they fixed issue on December 5th. After few days on December 13th the same issue happened again and Tesla service center is saying I need to wait till February 20th 2025 for the repair. All cameras are not working in the car, it does not show any backup camera, No forward collision warnings or no lane change assist Tesla is not considering this as a safety issue I am including a chat summary with the service center via PDF file. The car lost all ADAS functions, and GPS on the onboard navigation system. In addition. it can't use regular cruise control and it does not have rear view mirror blind spot warning capability any more. This all happened when the car attempted to install the "holiday" updated from Tesla automatically around 11/29/24 .
They advertise it as a computer failure. The cruise control doesn't work, none of the cameras displays, causing me to hit the lawnmower in my garage. Other functions as navigation, range prediction, and automatic light/dark display switching don't work either.

Starting on 12/4/2024, my car's camera stopped working, no blind spot warning, no navigation, no rear view camera and no cruise control. Reported to Tesla on 12/8 and they said it is a known problem nationwide and some Autopilot computers are getting shut or something. They have given me an appointment for 1/4/2025. The vehicle is really unsafe to drive without these components working and the vehicle draws on heavy amount of battery usage without these controls and for my commute on highway, I am always nervous about if there is going to be enough for me to reach the next charging station. I would expect this falls into safety recall and Tesla should at least send out a service bulletin as well as provide alternate vehicles for the owners to use until this can be fixed.

All cameras are not working anymore including back up camera. Tesla acknowledged the issue and stated that the Computer needs to be replaced and a lot of cars have similar issues.
The display went black. I reset it. It came back but none of the cameras work.

All of the cameras on the vehicle are no longer working after the latest update. This means all sensors (even as simple as windshield wipers) do not work. All driver safety assistance does not work.

Tesla has confirmed that the car needs a new computer for all cameras and software navigation. The replacement computers are on back order and have scheduled a service replacement in six weeks time. In the meantime, they refuse to provide a loaner car. This car is brand new with only two months worth of usage. They say the car is drive able, but without the main safety cameras I don’t feel safe, the aesthetic of the car was designed to have working cameras. It is very hard to use the tiny side view mirrors and look out the tilted rear window. This is a commuter car, driving over 90 miles on highway per day. I am concerned for safety issues.

Not offering me a rental or anything on my brand new '24 Tesla. It has just over 2500 miles on it and is ~2 months old now. This issue impacts: emergency braking, blind spot detection, lane keep/centering, assisting with parking, automatic high beams and wipers, cruise control/ adaptive cruise control, Vehicles built in GPS to be stuck at one location every time, has made my touchscreen not automatically brighten or dim, my auto dimming mirrors be too dim in most driving conditions, constantly fail to install system improvements and safety updates, and because my vehicle keeps trying to do this, it has caused increased battery drain as well. Additionally, the vehicle came to a stop on the highway when my wife and I were in the car on the night of [XXX], forcing us to perform an emergency pullover and abandon the car 22 miles north of Rochester, MN overnight. This has increased the safety risk of my vehicle tremendously... Tesla has not seemed to publicly acknowledge the issue yet either. All cameras stopped working. With this, all safety features are not working. I can’t see to back up at night. No collision avoidance, no lane departure warning, high beams don’t stay on, turn signal doesn’t stay on. It has been two weeks and Tesla has no fix. There was no warning, just noticed it one day as I was driving it and was very dangerous in dark snowy conditions. I called Tesla service immediately and they gave me an appointment for following week. Then next day I got message from them that there are a large number of vehicles having the issue and they said they don’t have a fix for it and moved my appt out 7 weeks. I have gotten no help from them and I told them I don’t feel safe driving the car. They have not provided another car.
All Cameras not working with latest Software update - told need new AP4 computer on 2024 model Y which is 2 months old. Tesla service advisor states - car is totally safe to drive with just mirrors till appointment in few weeks. No backup camera, navigation, battery life dying faster. It seems like a liability to me to drive this way and very negligent to me.
All Cameras not working with latest Software update - told need new AP4 computer on 2024 model Y which is 2 months old. Tesla service advisor states - car is totally safe to drive with just mirrors till appointment in few weeks. No backup camera, navigation, battery life dying faster. It seems like a liability to me to drive this way and very negligent to me.

At the same time the Lane Departure Warning and Lane Keeping Assistance stop functioning without warning. On the image below I let the car drift over the line. As I crossed the line, the display should have highlighted the line in blue and there should have been a flashing warning at the top of the screen and an audible warning. None of those occurred. Since lane keeping assistance was enabled. It should have also steered the car back into the lane. I can regularly reproduce the issue. We don't understand why they are using the pillar cameras for this and not the forward cameras that can take advantage of the headlights. It is unsafe, especially since I am not warned of the system is not available.

In a 90 mile drive I experience phantom breaking 4 different times. No car was in front of me, in fact nothing at all was in front of me when my car slammed on the brakes 4 separate times. One time there was a car somewhat close behind me. I also twice experienced audible warnings and flashing alarms to take control immediately of the car when my hands were on the wheel and I was already in control of the car, it was alarming and shocking to get these alarms while already in control and driving. I believe this car, as currently configured, can pose a danger to other drivers on the road.
This is a complaint regarding a malfunction of the Tesla Full Self-Driving (FSD) feature, which resulted in damage to my vehicle and may potentially pose a significant safety risk to public. On 07/02/2024, my newly purchased Tesla [Model-Y 2024] was parked next to a pillar in a parking garage. After ensuring that the Full Self-Driving sign was on, I activated the FSD feature. However, as soon as the function was engaged, the vehicle instantly took off and made a right turn. This sudden and unexpected action caused the right side of the back passenger door to scrape against the iron guardrail on the pillar and caused damages to the right back passenger door and a portion of right back fender. This action happened so sudden that did not have the chance to react and take manual control. I contacted Tesla to report the issue and seek assistance, but after +4 weeks of contacting and restless following up with Tesla service teams, Tesla stated that the software was functioning properly and denied any responsibility for the malfunction, and they closed my case without offering any resolution or further investigation. Luckily, this incident happened in a confined parking garage. It could have resulted in more severe damage or injury had the circumstances been different. Additionally, Tesla's dismissal of the issue and refusal to accept responsibility is deeply concerning, especially about the reliability and safety of their Full Self-Driving technology. I sincerely request your office to investigate into this incident, and I hope you could advise any appropriate action to examine and address this issue. I attached a few photographs of where my vehicle was parked before the FSD was turned on, and the damages on both my vehicle and the guardrail at the time of the incident. The contact owns a 2024 Tesla Model Y. The contact stated that while driving at an undisclosed speed with auto-polite activated, the vehicle suddenly pulled in the left direction across three lanes of traffic. As a result, the vehicle collided with the passenger-side front headlight and came to a complete stop in the roadway. The air bags did not deploy. No warning lights were illuminated. The vehicle was towed to an independent mechanic, who was unable to diagnose the failure. The vehicle was not repaired. A police report was filed, and injuries were reported. The contact transported himself to his primary care doctor, who treated him for nondescript injuries to his neck, shoulders, right hand, and hips. The manufacturer was notified of the failure, and a case was opened. The failure mileage was 9,000.

I was on [XXX] heading towards Chicago and following the traffic. I started changing the line to pass the vehicle in front of me and suddenly my tesla completely lost control and started drifting to the sides. I released the accelerator right away but it made it even worse. It seems that acceleration continues to happen that’s what made car to drift. Eventually car slowed down and I was able to make it to my line. I don’t understand why line assistance didn’t work to keep the vehicle stable, why after releasing accelerator the vehicle continued to gain the speed. It was sunny day and dry road.
