"I am reporting a fatal crash involving a 2024 Toyota Corolla (VIN [XXX] I do not know the exact mechanical cause, but the following symptoms occurred: 1) The driver reported dashboard lights (red and blue) flickering on and off, windows and locks going on or off. 2) There was hesitation in acceleration after taking off, 3) The vehicle failed to stop and went airborne causing a FATAL accident. Insurance Co. failed to investigate porperly and sold the car To Co-part without waiting for Probate forms to be complete for this vehicle and the Estate. We have discovered this car was an Avis fleet vehicle sold as 'new' at Beaver Toyota. I am requesting a federal investigation to determine if a mechanical or electronic defect caused this loss of control." Subject: Vehicle safety complaint — dashboard/windshield reflection severely impairing visibility on 2014 Toyota Corolla (VIN: [XXX] ) To Whom It May Concern, I am writing to report a serious safety issue with my 2014 Toyota Corolla, VIN [XXX] . Under certain daylight conditions, the dashboard (or a component of it) reflects sharply on the windshield, producing a glare so intense that it significantly impairs my ability to see the road ahead. Vehicle details: Make: Toyota Model: Corolla Year: 2014 VIN: [XXX] Approx. current mileage: [insert mileage] Problem description: - At specific times of day — when sun angle, interior lighting, and windshield geometry combine — the dashboard reflection becomes very pronounced. - The reflected dashboard elements (vents, panels, interior contours) appear on the windshield glass and overlap the driver’s forward view. - This glare regularly blocks or obscures pedestrians, other vehicles, road signs, or hazards ahead. - The issue is not cosmetic; it directly reduces visibility and increases risk while driving. - The vehicle remains stock — no aftermarket modifications have been made to dashboard or windshield that could cause this reflection. Impact on safety: - I frequently struggle to see clearly under these lighting conditions, compromising my ability to react properly to traffic, obstacles, or sudden changes — especially when approaching intersections or dealing with glare. - This defect endangers not only me and my passengers, but also other road users: drivers, cyclists, and pedestrians. Request: I request that National Highway Traffic Safety Administration (NHTSA) review this issue for the 2014 Toyota Corolla (and possibly other model years) to assess whether dashboard materials, interior design, or windshield geometry create a widespread visibility defect. I am willing to provide additional documentation — such as photographs or videos showing the reflection INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C.

I am reporting an ongoing safety issue involving the Skid Control ECU and loss of power break recall on my 2024 Toyota corolla hybrid. The first time, my vehicle experienced loss of power brake assist and displayed a “Hybrid System Malfunction” message on screen. I towed my car to Toyota, confirmed it was related to the Skid Control ECU recall and repaired it under the recall at no cost, they did a software update. The second time, the exact same problem reoccurred. I had the vehicle towed again to a Toyota dealership, but this time, they refused to perform diagnostics unless I paid out of pocket for a 12-volt battery replacement. The dealership stated they cannot confirm if the issue is recall-related until diagnostics are complete, but they also refuse to complete diagnostics without me paying first. My vehicle is currently undrivable and unsafe, and I rely on it for daily transportation and work. So they made my pay for this 12v battery to do the diagnostics and now they tell me it is not recall related but one of the relays not working, the repair will be cover under warrant but i had to pay for battery and rental cars . I believe Toyota is in violation of its obligations under 49 U.S.C. §30120, which requires manufacturers to correct recall-related safety defects free of charge. This issue leaves me without a vehicle and unable to verify that the recall has been properly resolved. I have opened a case with toyota brand engagement. I am requesting that NHTSA review this case and ensure that Toyota complies with recall repair requirements so my vehicle can be safely diagnosed and repaired without charge.

My car was parked in my driveway on am 80 degree afternoon. I went into the car to get something. When I closed the driver door, the entire back window shattered. I am the only driver of the vehicle and there were no issues with the window prior to this- no cracks, no incidents, no rocks on the road.. The only indication there was any issue was condensation on the inside of the back window two days before this happened. I thought this was unusual, but it did not seem like something to call the dealer about. I took the car to IRA Toyota in Saco, Maine. They informed me there was no way to 'prove' a defect, as there was no longer a window to inspect. They suggested I call my insurance and lie about the incident, stating that 'nobody would believe a window just blew out' and they were just trying to help me get the repair covered by my insurance. I know there is currently a class-action suit against Honda for this same issue, and found over 20 shared instances in Toyotas (of various models) from 2021,2022, 2023, and 2024. I understand glass is not included in warranties, and I think this only makes sense for manufacturers. The fact that a window could spontaneously shatter at any moment is certainly a safety issue and should be covered under warranties or some other level of accountability. When my window combusted, all of the glass fell into the back window and back seat. If any kids or animals had been in the back seat (as they almost always are), they would have been covered in glass. Thank you for your attention to this matter.

When the sun is overhead (this happens daily in South Florida) the reflection of the ambient light on the instrument panel cover makes it impossible to look at the information on the instrument panel. This essentially makes one drive while being blind. Not a safe concept. It is pathetic that no action has been taken against the company.

I purchased a used 2024 Toyota Corolla from Chapman Chrysler Jeep on 8/9/25 at 31,890 miles. When exiting the freeway 3 miles later, the vehicle suddenly swerved uncontrollably without any steering input from me. This was an extremely dangerous incident that could have caused a serious accident injuring myself and others.

Dear Better Business Bureau, I am filing a formal complaint against Toyota Motor North America regarding a serious and unresolved safety defect in my 2024 Toyota Corolla LE (VIN [XXX]). The vehicle experienced two incidents of sudden unintended acceleration—the second, on [XXX], resulted in a collision. Despite my repeated efforts to resolve the issue, Toyota has failed to respond adequately or provide critical diagnostic data. In both incidents ([XXX] and [XXX]), while stopped at a red light, the vehicle suddenly surged forward despite my foot being on the brake. On [XXX] this caused a collision. My vehicle has remained undrivable since. Toyota conducted a diagnostic inspection on May 29 under Case #[XXX]. I’ve repeatedly requested access to the raw data from this inspection to understand the defect, but Toyota has not responded. Emails to the assigned case manager, Ms. Cynthia Ware, have gone unanswered. When I was directed to reply to a support email, it bounced back as a “do-not-reply” address, redirecting me to a login page which falsely claimed I had no active cases. This created an endless communication loop. Toyota’s lack of transparency and support is unacceptable—especially given the severity of the issue and the fact that the vehicle is still under warranty. I am requesting: Immediate release of the raw diagnostic data from the May 29 inspection A prompt resolution, including full repair, replacement, or buyback Clear, responsive communication from an authorized representative This is not only a financial burden, but a serious safety risk. I respectfully request BBB’s assistance in securing a resolution and holding Toyota accountable. Thank you for your time.

I was able to distinguish today the car noises my vehicle is making happens when I break. Like a “eeeeek” sound. My mileage is under 8,000, had the car almost a year. Also I notice my wheel stiffens. I’m able to verify it happens when I merge - specifically when the lane dashes end. The wheel somehow gets tight even when driving with both hands. As if the vehicle gets confused. This has happened about 5 times this year but every time it happens- I brace myself for the vehicle to crash since I can’t control the wheel, no matter how strong I try to gain control. The wheel gets too tight to steer. These episodes lasts about 3-4 seconds. I am going to take it to the dealership on April 1, 2025. Also there were no messages on my dashboard when these episodes occurred.

[XXX] In reading the above article, Toyota recalled Corolla Cross Hybrid vehicles with many complaints and situations mirroring our accident. Before the accident, the vehicle displayed no warning signs. Despite the driver pressing the brakes and the assisted braking feature seemingly turning on (e.g., signal lights on the computer system), neither seemed to work in this situation. The damage was far too severe for the assisted braking feature to have activated, along with the driver pressing the brakes. After viewing the matter with those involved in the accident, many of us concurred that we believe either the braking system or the computer system failed. In the aftermath of the accident, the police, manufacturer, or third party hadn't reviewed the vehicle's status. On top of that, our insurance refused to investigate, urging us to discuss the matter with Toyota. Thus, we'd propose that the Corolla SE may experience an issue similar to the vehicles in the article. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

My wife and I have been having great difficulty reading the center portion of the instrument panel because the illumination is too dim. We have visited the dealership and were instructed how to maximize the brightness, but it is still not sufficient. The small icons that appear at the top of the center section of the instrument panel cannot be viewed while we are driving, unless we place our head close to the screen. To do this our head must be at the level of the bottom of the front window. In that position we cannot see the highway sufficiently to be “safe.” We both have good vision. In the past, we have owned over thirty automobiles and have never had a problem seeing the instrument panel.I have discovered that occassionally the illumination of the center screen of my instrument panel is fine. The car’s computer apparently dim’s it without my consent. I drove 60 miles (about an hour) on a California freeway, and for the first 20 minutes I had good illumination. Then it suddenly got to its usual dim state. I have asked several people at my Toyora dealership, and they cannot help me. I, therefore, am requesting someone at your offices to look into this problem, and call me by phone to help me solve this important problem. The best way to viualize this problem would be to inspect my auto at my home.

Morning of 08/11/2024, started car and put in Sport mode. Drove under 20 mph for 3 blocks to leave neighborhood. Got to main street intersection and waited for an opening, then pressed the gas and nothing happened except the check engine light came on and dash message of Ignition On, but car was not operating. Pushed ignition button and it came on and dash cleared. Has not happened again and dealership could not replicate and no codes saved.

I was accelerating on an on-ramp to merge with traffic on a freeway, and my corolla’s engine shut itself off. All the electronics stayed on and flashed the messages ‘Parking brake malfunction’ and something along the lines of ‘Secondary collision brake system malfunction’. I was stuck coasting in traffic and could not start the engine in neutral. I had to make the car come to a complete stop on the freeway in a lane with no shoulder, put it in park, turn it off, and then back on again. All messages and lights disappeared. I took it to a toyota dealership the next day and they couldn’t find anything. They told me they don’t know what caused it and sent me on my way.

Steering—car seems to slip when driving at 30mph and taking a curve. Very brief. Traveling at 70mph car shakes.
