The power assist braking system failed while I was driving in the rain. I was approaching a stoplight and I had to push down a lot harder and farther than I typically have to in order to stop. Luckily I was not going that fast and there were no cars ahead of me, but I stopped beyond where I had intended at a busy intersection. No warning lamps or messages had been illuminated prior to this happening. After the failure, the Brake system, ABS, and SLIP indicator lights were on and remained on when the car was restarted. An independent service center used a scan tool and found code C1391 stored in memory, indicating that the brake booster is unable to hold pressure due to an internal leak. The vehicle has not yet been inspected by the manufacturer, police or insurance, but can be available upon request.
I bought this car over a week ago for a good price, performed fine until I’m driving it home on the interstate and some abs/brake lights came on the dash. I enter my city and notice the brakes aren’t performing as tight as they were during the test drive. A few days later I take it to a local dealership to get it looked at. The brake actuator and pump had gone bad, causing this issue. It cost almost $5,000 to fix there, I couldn’t afford it so I had to only pay the $185 for the diagnostic. It’s an on and off thing, sometimes the lights go away and it brakes as it should, but most of the time the lights are on and the brakes don’t work as they should. I fear one day the brakes may eventually give out on me and cause an accident. I am available for inspection.
After 13 years and 112,000 the brake booster and brake booster pump went out on my car. Toyota extended the warranty to 10 years because of this issue. Now just outside of the 10 yr mark my car has this issue and Toyota will not cover it. This is a huge expense and also a major issue with this year and model of car.
I have a Prius v 2012, and the lights for abs turned on and when I checked on internet Toyota was repairing the part for 150k miles and 10 years but this is a safety feature my car is now at 149922, And while I was on road suddenly the signs came up and I the brakes were hard, and this is a problem for my safety and for others too since It doesn’t brake well
While driving on the interstate I went to slow down to merge and my brakes failed. The following warning lights came on: Master Warning Light (yellow), Brake System Warning Light, Brake System warning light (red), ABS Warning Light, and the Slip Indicator light. My vehicle was towed to a Toyota Dealership and after running a diagnostic test, they confirmed that it was the "Break booster with accumulator c1391 stored internal leak of accumulator and need immediate attention" which they quoted at $4,295.68 to fix. After researching the issue saw A Customer Support Program 20TE07 was issued taking responsibility for the faulty Brake Booster Pump Assembly however only covered the repair for a two-year period and only if it was one of the identified diagnostic trouble codes listed in the CSP issued by Toyota but expired in Feb 2022. The code pulled from my vehicle 3 days ago was listed in this document, but the coverage only covers until Feb 2022. This is not a normal wear and tear issue, it is a faulty part that Toyota identified as a life and death safety concern but refuses to do anything about because mine didn't show an issue within their 2-year CSP timeframe. Toyota identified that there are roughly 118,950 2012-2014 model year prius V with a production period of late September 2010 to mid-November 2014 vehicles that this faulty part affects along with everyone else on the road with these vehicles as it causes failure of the brake booster pump assembly, which leads to a loss of braking assist and the deactivation of the Vehicle Stability Control system. For the safety and lives of the owners of these vehicles and all of the other lives of the drivers on the road that this could affect in the case of this faulty part should there be a catastrophic accident, like mine could have been, a nationwide recall on this part MUST be issued.
My wife was driving home and four dashboard error lights illuminated (Brakes, ABS, SLIP and a yellow light_) The brakes suddenly "went soft" (in her words), and it freaked her out. I now learn that I need to have the brake master cylinder and a booster pump assembly replaced at a cost of over $4,000. Ouch!
While driving suddenly the ABS and vehicle traction lights came on and the car became very slippery, was unable to stop without an excessive amount of space between the car in front of me and me. While looking at the car, I realized the actuator brake booster pump. I then found multiple websites and forums dedicated to people with the same problem ( brakes suddenly not functioning correctly while driving) with the same model car as I have, and there was a service bulletin that shows Toyota acknowledged there was a problem ( and a fix) but with the date of the bulletin and the date it was to be fixed within, it was near impossible to actually have it fixed. My car has 87,000 miles and the brake system is dangerous
Brake booster pump assembly. Toyota has sent a notice of extended warranty for this vehicle which has expired. Without repair, the brakes will loose compression and stop working.
Anti lock brake malfunction. Code c1391 leak in brake accumulator. This seems to be a common code for brake issues with Prius. The cost to repair is significant, more than the value of the car. This is a certain safety issue that needs to be addressed. This is clearly a Hazzard and will cost lives to those who cannot afford the cost of repair.
The issue is my brakes failed on me all of a sudden. My abs light, traction control light, and brake light came on out of nowhere. Suddenly didn't have brake pressure. Had to pump the brakes a few times before the car would stop. Luckily I didn't hit anything or anyone. I brought the car to Prius specialist and they diagnosed it saying it was the ABS Actuator Pump Assembly and Master Cylinder. They said it is quite common in the 3rd generation of Prius'. I contacted Toyota North America and they said they had a recall on the parts but it ended Dec 31st, 2021. I bought the car in 2022 unknowingly. Was quoted around $4000 to repair it parts and labor. This is a safety issue that should not be limited to time. Luckily I was not on a Freeway or anywhere where I could have done damage. This issue is a hazard to myself and other people on the road. Toyota needs to resolve this.
I write to add my voice to the litany of complaints in support of NHTSA investigation DP23005. While on an road trip recently the ABS, Traction Control, and Brake warning lights all came on. The brake felt squishy and didn't seem to be functioning as confidently as before. I altered my driving (giving plenty of distance, lowering my speed) so that in the event the brakes quit working I would have an exit strategy. I took it to my Independent Service Center as soon as possible who found the code C1391 and diagnosed the need to replace the master cylinder / booster assembly for several thousand dollars. Of course I must do this in order to remain safe but this is clearly a safety issue that could have been catastrophic. Meanwhile I am out of a car for several weeks until the part arrives and out of at least $2000 as quoted by my mechanic ($1640 for the part). The car only has 146,500 miles on it so otherwise would have plenty of life left in it. I'm filing this complaint to add my voice to the open petition and toward the NHTSA's investigation in strong support of holding Toyota responsible for PREVENTING injury and death through a proper recall, as well as against burdening the car owner with the costs of making up for Toyota's negligence on this issue. At the least they need to cover the cost of these necessary safety repairs when they are warned by the lights and reinstate their warranty extension for this issue

The Toyota Prius of this generation has a known defect in the brake booster pump assembly. About 2 weeks ago the dash lights came on for ABS, Traction control, general warning light. The brakes work but feel soft, and a motor sound pulses behind the steering column every 8-10 seconds. My mechanic pointed out it is a known problem where the ABS booster pump has a faulty valve that doesn’t allow proper pressure to build to engage ABS system. I cannot afford the Repair: $3890 for master cylinder and brake booster. My concern is that most people can’t afford this repair and without a functioning ABS system, people are put at risk from this generation of Prius. Toyota did offer a customer service program acknowledging the issue, but that program only helped if these issues occurred before 2022. With the known issue, why wasn’t a recall required? Toyota should be required to address this safety issue. How many of these Prius are on the road where customers are expected to perform a $4,000. Repair to keep up the safety standards. The car is available for inspection upon request. Thank you.
I was driving and all of a sudden the emergency brake light, the ABS brake light and a couple more turned on and I had no more brakes. I had just driven 25 miles with it and stopped at many red lights along the way with no issue and NO WARNING lights. I could have gotten into a major accident if it had happened anywhere else in my route. Fortunately, I was about a mile from my house and the hybrid motor helped slow the car down. I was unable to stop at any stop sign in my development and here again was very fortunate that there were no cars along the way. Once I got home, I got it towed to Toyota and was told it is an internal failure of the actuator. I contacted Toyota America and was just told that there was a campaign to repair this but that I had passed the date. My car has only been serviced at the same Toyota in my city since I got it. I religiously brought it every 5000 miles. I am shocked that they never mentioned this campaign to me and that they didn't initiate a repair during the campaign. This is a major issue that needs to be addressed and Toyota needs to be held accountable.
About three weeks ago, my daughter told me that several lights were on on her display. I checked and among them were the traction control and 2 other related to the brake system. I looked up possible causes on the Internet and it seemed the brake booster/master cylinder could be a culprit, but then I checked the fluid reservoir and the fluid was low, so I topped it off, drove the car, pumped the brakes a few times and all the lights went off. About a week later the lights were back on. I checked all the lines and saw not visible signs of leaks and decided to take it to a mechanic. I took it to a local mechanic and I was told that the brake master cylinder with brake booster failed and had to be replaced. Afterward the brakes were repaired I went to 2 local Toyota dealerships to check the price of the parts. The first dealership provided the prices without any discussion. At the second dealership, the employee who waited on me, after I provided her the VIN and told her that I need the price of the brake booster/master cylinder,m told me that there was a service notice on the booster/master cylinder because of a high rate of failure and that I would have to check with the service department before she could sell me the part. This is concerning, if Toyota knew/knows that the part has a high rate of failure, why wasn't a recall notice issued?

After starting the car on 7/30/24, the Brake System Warning Light, ABS,Slip Indicator, and Master Warning Light all came on indicating an issue with the brake system. We were able to get it to the local dealership and when they ran diagnostics on it, it returned a code C1391, which confirmed the issue. This was addressed in 2020 by Toyota, but only for cars 10 years or 150,000 miles old. Our car is a 2022 with 105,000 miles on it, but we are outside of the year limit (car was 8 years old when the Customer Service Program was released).

2012 Toyota Prius brake actuator failure . While driving my Toyota Prius the braking system failed, this was on a large road and so posed a major danger to not only me, but the passengers in the vehicle and the vehicles around us. Thankfully nobody was hurt. After having the vehicle inspected by a Toyota dealership the problem was found to be the “Actuator/accumulator” and after speaking with the dealership it has come to be my understanding that this is a common and unavoidable problem with Toyota Prius models. There were no lights or warnings on in the vehicle until after the brakes gave out. I cannot understand why this has not been recalled when it is a manufacturing issue on Toyotas end that poses a significant danger on the roads, and am wondering what the reimbursement policy for this is as it came out to be quite expensive.
The Brake Master Cylinder and Brake Booster pump failed to maintain pressure. I don't have the replaced parts, my mechanic recycled them before I realised I was going to submit this. When the part completely failed, I had the ABS, traction control lights on. I couldn't trust that the car wouldn't have a problem stopped, and the brake pedal also became squishy in responsiveness. My own safety, my kids safety, and the safety of others was at risk and I immediately stopped driving it to get the repair done. The problem was confirmed by a Toyota dealership and this was a recall but since my car is older the recall had expired. I was unaware of this recall until the issue began. It was also confirmed by my personal mechanic to be the failure. The Dashboard lit up the ABS, TCS, Brake, (!) light, VSC, and the sound of the pump changed. Before all the lights came up I had noticed a air leaking sound coming from near the brake pedal at times.

Brake booster has gone out. This is a known, dangerous issue that Toyota knows will occur, It is a dangerous and expensive risk to those driving these ticking time bombs, and those potentially in the path of these vehicles. This is a pure safety risk item. No one asking for these brake actions are gaining anything materially other than the use of their cars safely. To make matters worse, Toyota doesn’t even make these in volumes that don’t generate volume pricing efficiency, meaning replacement runs upward of $4k to $5k at their dealerships to replace. Criminal…. Please help.
I was driving when I felt my brake pedal start to become soft and mushy. I was able to brake, but I had to pump the brakes to get full brakes to stop. I also noticed warnings began to appear on my dashboard. The warnings that were shown are the brake light, traction assistant light, and ABS light. I took it to Gates Toyota in South Bend, IN and paid for a brake inspection. They told me that my brakes themselves were OK (pads, rotors, calipers), but they were getting a C1391 error. I was told that the brake accumulator has an internal leak. I was told that I needed to replace the brake master cylinder and brake booster pump assembly. I bought the car used from the dealership about a year and a half ago. After doing research, I found that this same issue has been a common recall problem on a number of Toyota vehicles starting as far back as 2008. I also found a few class action lawsuits concerning Toyota vehicles with the exact same issue. My vehicle doesn't show any current recalls, but I'm having the same problem as other Prius V vehicles are having. I was quoted $3300 to fix this problem. I shouldn't have to be responsible for paying for a defect that has been recalled on a very large number of vehicles, and also has been the result of class action lawsuits.

8/2/22 My ABS light came on. Took the car to a dealer. They kept it for six weeks and returned it on 10/3/22. No charge and no explanation. 11/7/22 engine light came on. Took in to dealer, they returned it 11/17/22 indicated incorrect air/ fuel mixture. 10/9/23 Traveling 47 mph, came around a mountain and encountered stopped cars. Slammed on the brakes, car did not slow down, steering wheel locked to the right, car yawed to the right, and turned over, hit a curb and the car righted itself. 3 occupants, 2 injuries , 1 death. Researching the 2012 Toyota Prius, found many instances of a faulty ABS system, mimicking exactly my experience. I was never informed by Toyota or the local Toyota dealer.
The hybrid system was shut down while driving. The symptoms are like the description in this recall (NHTSA Campaign Number: 18V684000). However, my VIN is not on the recall list.
Brake actuator making noise every 10 seconds and oil is having to be filled in everyday.
Upon driving out of my school parking lot at approximately 15 to 20 miles per hour I noticed a cushiony filled with a brakes and all of a sudden I heard a shhh all of my lights popped up on my dashboard. When I took my vehicle in to be able to find out what was going on with it I was told that I need a master cylinder and a brake booster. This was after having my vehicle for only 3 years where I purchased it from Rice Toyota Greensboro North Carolina.
Coming to a yellow light, I braked and the vehicle made a strange sound with loss of braking, so I sped up instead, having to go through a red light. Shortly after, that afternoon, when I again started the vehicle, the safety lights lit up in the dashboard and my vehicle kept making a strange cycling noise. As it turns out, it was the whole brake actuator assembly. Under great financial hardship, I had the vehicle repaired ($4300+). I applied to Toyota Corporate for help but they denied it, saying I was outside the extended warranty. I’ve since learned this brake problem was a known issue, and Toyota settled a class action settlement. My vehicle has only 90,000 miles on it, and it is has been used primarily as an in-town vehicle, our only car. I’m asking for a recall and reimbursement on repair for affected vehicles. Toyota has been allowed to get away with responsibility for a known issue by putting it into a extended warranty program, now expired. Meanwhile, this dangerous, known defect continues to happen to unsuspecting drivers. I was lucky with my incident, as no cars came out when the light changed, but it could have been a different outcome.

brake warning lights (ABS, traction, and (!)) went on while driving on the highway with my 8-year old. Needed brake booster replaced. Replaced at dealership for over $4500. Was a year and a half outside of a customer support program that covered cost for our vehicle through Jan 2022. Called Toyota to see if they could do anything and to report safety issue. They did not give me any funds towards repair. I feel that this is a significant and scary safety issue, and an unfair financial burden for Toyota customers to bear.
The contact owns a 2012 Toyota Prius V. The contact stated that while driving at an undisclosed speed, he depressed the brake pedal to stop the vehicle; however, he had to apply extra pressure to the brake pedal and the brake pedal extended to the floorboard to bring the vehicle to a stop. No warning light was illuminated. The vehicle was taken to an independent mechanic where the vehicle was diagnosed, and the mechanic retrieved DTC Code: 1391. The contact was informed that the brake booster assembly needed to be replaced. The vehicle was not repaired. The manufacturer was not notified of the failure. My Prius V experienced brake failure today, with three warning lights appearing at once and the brake pedal not engaging the brakes unless it was pressed almost to the floor. This made it feel extremely scary and unsafe to drive. I was very upset to see that this is a recurring problem with Prius cars in this age range. Although I only have less than 120,000 miles on it, Toyota is not offering an extended warranty for the failing parts (brake master cylinder and actuator and booster) past 10 years or 150,000 miles. This is total BS if they already know that there is a problem with these parts.
Diagnostic code C1391 abs system. Brake accumulator pressure leak. This a known issue and common problem. $3000 dealer only repair. There was a customer service program and extended warranty. This is a major safety issue and Toyota has stopped offering any repair support and expects customers to cover the full repair price themselves.
In July of 2021 the Toyota dealership replaced the inverter on my 2012 Prius V. At the time, it also was exhibiting signs of the brake booster and pump failure as outlined in CSP 20TE07 (see https://www.toyoheadquarters.com/threads/2012-2014-toyota-prius-v-2015-toyota-prius-v-coverage-for-brake-booster-and-brake-booster-pump-assemblies-csp-20te07.2116/). It was inside the extended warranty period, but they would not fix it at the time, because it was not throwing diagnostic codes. Subsequently, the braking system malfunctioned, nearly causing a high-speed accident on the freeway. With diagnostic lights now showing, and code C1391 stored in the OBD system, I took the car back to the dealer, but now they said it was past the 10 year limit and still would do nothing about it. This is not acceptable! They knew the brake system was failing and a safety hazard, but would not address it, just because it hadn't actually failed yet. Toyota needs to further extend the warranty and take care of this brake failure past 10 years and 150,000 miles. Just because I took good care of my car and the brakes didn't fail as early as other owners, I should not have been subjected to the risk of brake failure or the high cost of repairing the brake system when Toyota took care of this problem for so many other hybrid owners. Please help.
As I was driving, all of the brake lights turned on on the dashboard. The moment the lights turned on the brake power felt low on heavier pedal depression. I took the car in to the dealership for assessment and said they found internal failure of both brake actuator and booster and recommended replacement of both actuator pump and booster. I have had both the parts replaced and after now the second replacement of the master cylinder, there is still an error code for the pressure cylinder and all the brake lights are on.

Brake light, Trac light, and ABS light came on after brake pedal became squeshy. Took to nearby repair shop and advised that car was not safe to drive. Scanned for codes. Code C1391 (Abnormal leak of ACC pressure) indicated. Recommended that brake booster assembly with the master cylinder be replaced, and replace the Brake Booster Assembly. Total cost of repair was $4,188.02.
The brake had an issue while driving. It was hard to stop the car. It is probably due to the brake booster. I had this issue and I took my car to the dealership and they could not find the problem. I had the same issue again while driving. Luckily, when both problems happened, I drove slowly on the local roads. Toyota had recalled those specific models but the length of the warranty is not long enough to solve the problem. For instance, they gave a second warranty, either 10 years or 150K miles, which hits first. I got this brake problem 6 months later after the warranty expired. But my mileage is low ~118K. For drivers' safety, they should extend the length of the warranty. The two parts are expensive (~4-5K in total) and apparently, that's the problem with the design of the auto parts. Customers should not pay for them and risk their own life.
In September 2022, my car, a 2012 Toyota Prius V with 79, 056 miles, began making noises in the engine area and the brakes became loose, meaning it took more pressure on the brake pedal to engage the brakes. In November 2022 the following warning lamps on the dashboard came on: Master Warning Light, Brake System Warning Light and Warning Buzzer, Brake System Warning Light, ABS Warning Light and SLIP Indicator. I took it to the Toyota Dealership near me: Lipton Toyota in Ft. Lauderdale, FL, and they diagnosed car with the trouble code of C1391-Abnormal Leak of Accumulator, Internal ABS Actuator Failure. To fix this problem, Lipton Toyota will charge $3124.29 for the repair. Toyota Motor Sales, U.S.A., Inc. issued a customer support program notification that would cover this repair if the malfunctions happened before coverage ended. Primary Coverage was applicable until December 31, 2021 with no year/mileage limitation. Secondary Coverage was applicable for 10 years from the vehicle's date of first use or 150,000 whichever occurs first. Date of first use on my Prius V was 03/01/2012 and my mileage is 79K. So as you can see, I am just out of the coverage window. My private mechanic and myself have appealed our case to Toyota that this problem should be a recall with no limits on coverage. (Toyota Motor Sales, U.S.A., Inc. - case reference # 230206000678) I believe that if I had driven my car more, (which was not done because of the Pandemic 2020-2021) this issue would have come up before the cut off date for repair coverage. I also believe that this problem with the Brake Booster and Brake Booster Pump Assemblies on certain 2012-2014 model year Prius V vehicles should be a recall.
I was driving at night and several brake related dashboard lights suddenly lit up and I wasn’t able to use my brakes. I managed to coast safely to a stop, called a tow truck, and got my car home. Mechanic found a c1391 code and told me to go to Toyota. Toyota says the accumulator has a leak and it causes the brake up issue, but the service plan free program to fix it expired. They want $5000 to fix it. I want Toyota to extend the plan because I didn’t drive as much during the pandemic and I would have had the issue earlier had I been driving at normal amounts,

All the lights on my dash lit up. I took it immediately to Toyota of Dallas . I tried numerous times connecting with Dimatrius service manager. No call back. An inspection was done on the vehicle. He stated it would be a $5000 job and I was out of warranty. Their is too many people having this issue if you search many 2012-2014 people are having this issue in a addition to to a google search search for a letter at Capistrano Toyota.

The brake booster pump has started to run every 30 seconds while the car is operating. When pressing on the brakes, there is an unusual sound and has begun to feel different. There are no warning lights yet, but after consulting with the local Toyota dealership they informed me of the ZJB Customer Assistance Program which addresses this issue. Unfortunately the offer to repair has expired and the dealership has declined coverage. I contacted Corporate Toyota and they are declining to cover the repair. I don't have the $3500 to replace this brake booster pump at this time. This is a safety concern and I don't understand how this is not a RECALL due to the potential safety concern. PLEASE HOLD TOYOTA ACCOUNTABLE TO THIS SAFETY PROBLEM.
2012 Toyota Prius V, 125k. Sudden loss of braking power accompanied with BRAKE, BRAKE SYSTEM, SLIP INDICATOR, and PCS warning lights. Scan shows codes B2060 (ABS malfunction) and C1931 (Abnormal leak of brake ACC pressure). Brake fluid significantly lower. No signs of external leak. Can hear the brake booster pump running almost constantly. All signs indicate this is the issue identified in Customer Support Program 20TE07. This is a significant safety issue that may lead to loss of braking and serious injury / death. It should be a recall.

The ABS system has malfunctioned. It shows "ABS", "Brake" and "Hillside assistance" lights on the dashboard. Have to push the brake pedal till the end for applying the brakes. This is causing a safety issue. Toyota knew about these brake issues as they have a Support program to replace these faulty parts with another part. The Support program is 20TE07 which is expired. Instead of considering this as a support program, they should be considering this as a recall. This is a safety issue as the ABS stops working at anytime.

The brake booster started to make some noise every 5 seconds for about a week and one day when I was driving on the highway, all brake related warnings lights came on my dashboard. I want to brake and the brake did not hold correctly, once I touched it the car actually sped up then, I had to push hard to brake since that time the brake pedal is very hard to push and the car doesn't slow down at it should. I called Toyota many times for the extended warranty they had for this issue, but every time they apologize to me and they say it has expired. Although, I bought the car in March 2022 and the extended warranty expired on February 2022.( as they said). It is not fair that they have an extended warranty for brake booster for 10 years and when I get this same issue outside 10 years by one month, I'm not qualified. Running errands and when I got back to my Toyota prius v many lights were illuminated including my brakes abs tire pressure traction lights were all on. Tool my car to dealer for them to gladly request $5,000 to repair this. Dealership told me "the repairs are going to be a new master cylinder + new brake booster pump + fluids as needed for repairs" . This is a known issue for 2012 to I believe 2014 prius model V. This is a huge safety issue and a manufacture error This should be recalled and covered by Toyota as shouldn't be any fault of my own. On today's world 5k is a crazy amount of money to fix a known issue thay could arise again. Toyota needs to do better to protect their customers. Shame on them.
2012 priusV has intermittent vsc and traction control when cornering left or right at slow speeds, as less than about 20mph, 5-20. This is happening on dry road conditions at unpredictable times. The vehicle may deceleration and have a lungeing behavior with traction and or bracking interrupted momentarily. On accelerating from stop sign turning left or right the vehicle may pause accelerating briefly and pull with the abs brake auto/ intermittently applied.The traction control light may or may not illuminate at the short time interval of these events, but traction control light does not stay illuminated after the event is over. No error code is read from the vehicle with error code reader. These events are very startling and cause sometimes cause the vehicle to speed up or slow down abruptly. These events are happening at low speeds, however with the car surging forward or abruptly slowing on corning and lane changes there is potential issues of keeping the vehicle under proper control at these times. I believe Toyota should correct this problem so that it never occurs. Toyota to Find a solution an implement a recall to update vehicle.
TCS, ABS, Check Engine, and Warning lights all came on at once. Brake pressure immediately decreased. After the following diagnosis, Toyota said the brakes will continue to worsen and fail completely. Error codes C1249 and C1391 were presented. I have a 2012 Prius V with just under 65k miles. This morning I woke up to go to work, drove about 5 minutes down the road when suddenly the ABS, PCS, brake and traction control lights illuminated. My brakes felt much more stiff, the car took longer to brake, and I had to depress the brake pedal down much further than normal to get any braking response at all. I quickly scheduled an appointment to bring my vehicle to my local Toyota dealership for inspection. I was told by the service department that it will cost me $4.5K to replace the ABS actuator pump (47070-12020) and brake booster & master cylinder (47050-47110) because the system malfunctioned/failed. I was in shock! I quickly ask if Toyota had an open recall for the ABS brake malfunction/failure issue and was told no. That answer did not sit well with me because this is a serious problem. Plus, this should not happen to a vehicle with under 65K miles. I've owned several Toyota vehicles in the past and I’ve never had anything like this happen to me. Today, I searched “2012 Prius ABS Brake Issue” online, and found multiple stories similar to mine on blogs, posting, etc... about the Toyota Prius ABS brake problem. I even heard that Toyota had a program in place to address this issue but it expired in 2019. Vehicle owners had to fight to bring the issue to Toyota's attention in order to get their vehicle repaired before the end of the program. Can you tell me why the NHTSA haven't forced Toyota to issue a mandatory recall to address this serious and know safety issue? ABS dashboard light and loss of brake efficiency originally occurred on 2/23/22 and it has stayed lit every time vehicle is driven since then - and brake efficiency is affected since then. The ability to brake on demand is of paramount safety concern to the driver and passengers of our vehicle and also to those around our vehicle. The vehicle is available for inspection upon request. The vehicle was confirmed to have the same issue called out in the Toyota Customer Support Program (CSP) (https://static.nhtsa.gov/odi/tsbs/2020/MC-10181800-9999.pdf): replace Brake Booster and Brake Booster Pump Assemblies. The vehicle had some of the same DTC codes mentioned in the CSP and a few more. I have a photo from 2/24/22 proving ABS lights came on. This falls within the CSP by date (10 year from being put in service for this vehicle would be on 3/2/22) and just 3k over by miles. We are a loyal Toyota household - this is one of 3 Toyota vehicles we have owned (two Prius and Sienna) - and are in the market for an EV, and are excited about Toyota's new entry BZ4X in EV market. Toyota looking other way is an insult to brand loyalty and also violates the "spirit" of such CSPs (as opposed to Recalls, where I would have gotten this repair done timely before this failure happened). Also, equivalent CSP for a different Prius model (Prius C) (announced in May 2021 - 8 months after Prius V program was announced in Sep 2020) allows for the same Brake Booster and Brake Booster Pump Assembly issue to be covered up to Nov 30, 2022 and unlimited miles. How does it make sense that a 2012 Prius C owner experiencing the exact same problem on the exact same components on the exact same date would be covered while a 2012 Prius V owner would be denied the benefit? I would like NHTSA to work with Toyota to extend the CSP program officially to cover more vehicle owners like us and not live them stranded. That is not we expect from a company that prides in quality workmanship, customer-centricity.
Faulty brake and ECU assembly failure. A safety issue when driving. Was told by mechanic NOT to drive car as it could suddenly stop. Took car in 9/11/15 for software update for motor generator ECU and power management ECU as recall letter suggested. This is apparently common and needs to be corrected without cost to driver.
Bought the car from a dealership in january 2022 with 158,000 miles and heard buzzing noise that I was told was normal with hybrid cars.no codes showing on car and passed dealer inspection.Driving down a highway and heading to a stop brakes are hard to mash and caused skidding. No brake lights or abs lights are on. Couple days later brake lights and traction lights come on and then went back off. Take to Tires Plus and I am told I have to schedule appointment with toyota dealer as they do not have the software to diagnose. Take to toyota dealership to see codes 1391c for brake system. After searching issue there was a recall/customer warranty extension for replacement of this part for 150,000 miles /march 19,2022 whichever comes first. This did not show up on a report for dealer to inspect before selling vehicle and causes a huge safety concern to people that did not own this car before 150,000 miles. If a less experienced driver would not have known to reduce brake pressure applied it could have slid and caused collision or if brakes would have been needed during an emergency situation. Car is now sitting at toyota dealership and advised not to drive as brake pressure was lowered while being diagnosed. After talking with toyota they cannot provide assistance due to car being slightly over the 150,000 mile mark. Also if the brake pedal is mashed for an extended period of time the dash lights for brake system and traction control go off and will not come back on until it is driven for awhile

THERE IS A CUSTOMER SERVICE NOTICE THAT COVERS 2012 TOYOTA PRIUS AND PRIUS PLUG IN FOR THE SAME ISSUE REGARDLESS OF MILEAGE UNTIL 2019, AND THEN UP TO 150K MILES AFTER THAT. NEW PUMP ASSEMBLY BRAKE BOOSTER WAS INSTALLED AND CAR WAS PUT BACK IN SERVICE 5/19/2019. TOTAL COST OF THIS SERVICE WAS $2,745.11. WE LATER LEARNED THAT A CUSTOMER SUPPORT PROGRAM CSP ZJB WAS ISSUED BY TOYOTA FOR 'CERTAIN" 2010 PHD PRIUS, "CERTAIN" 2011-2015 PRIUS, AND "CERTAIN" 2012-2015 PHV PRIUS. AS PER THE CSP ZJB MAILING TO OUR RELATIVE WHO ALSO OWNS A PRIUS, THE PROBLEM WAS "WITH THE BRAKE BOOSTER AND BRAKE BOOSTER PUMP ASSEMBLIES". ACCORDING TO DIANNE, CUSTOMER SERVICE REP WITH TOYOTA, THESE VEHICLES INCLUDED IN THE CSP ZJB WERE PRODUCED BETWEEN OCTOBER 2010 TO NOVEMBER 2015 AND INCLUDED SOME 946,100 VEHICLES. MY VEHICLE WAS NOT INCLUDED IN THIS PROGRAM AS THE VIN NUMBER DID NOT MATCH THE AFFECTED VEHICLES. HOWEVER, WE HAD THE SAME PROBLEM WITH OUR VEHICLE THAT THE PROGRAM WAS INITIATED TO INCLUDE. UPON SUBMITTING A REQUEST FOR REIMBURSEMENT FROM TOYOTA FOR THE COST ASSOCIATED WITH REPAIRS, IT WAS REJECTED. WE BELIEVE THE PROBLEM FOR WHICH THE SERVICE PROGRAM WAS INITIATED IS THE SAME PROBLEM WE EXPERIENCED AND OUR VEHICLE VIN SHOULD BE ADDED BY TOYOTA TO THE LIST OF AFFECTED VEHICLES SO THAT IT BECOMES ELIGIBLE FOR EXPENSE REIMBURSEMENT.

IN HARD BRAKING I'VE HAD TO PUMP THE BRAKE PEDAL TO GET STOPPED. IT'S MY WIFE'S CAR BUT SHE WILL NOT DRIVE IT ANYMORE. WHEN BRAKING AND GOING OVER ROUGH SURFACE OR BUMP, THE BRAKES WOULD SEEM TO MOMENTARILY DISENGAGE AND THEN REENGAGE AFTER THE SURFACE SMOOTHED. THE CAR SEEMED TO LUNGE OR SKIP FORWARD AND TAKE LONGER TO STOP. ALERTED THE DEALERSHIP, THEY CHECKED AND SAID EVERYTHING WAS FINE. THIS HAS CONTINUED THROUGHOUT MY OWNERSHIP OF THIS VEHICLE. RECENTLY DURING AN EMERGENCY STOPPING ACTION ON A FREEWAY, MY PRIUS WAS THE 3RD CAR IN A 3 CAR ACCIDENT. THE 1ST CAR FOR SOME REASON, SLOWED DRASTICALLY,AND WAS REAR ENDED BY THE CAR IN FRONT OF ME. I FELT I WAS AT A GOOD DISTANCE BEHIND THE CAR IN FRONT OF ME AND I REACTED AND FULLY DEPRESSED THE BRAKES AND FELT THIS CAR CONTINUE TO ROLL AND CONTACT THE CAR IN FRONT. THE BRAKES DID FINALLY TAKE HOLD, BUT I FELT A HESITATION WHICH LENGTHENED THE STOPPING DISTANCE. I HAVE RESEARCHED AND FOUND THERE ARE DOZENS OF REPORTED ISSUES WITH THE HYBRID'S BRAKE SYSTEMS IN BOTH PRIUS MODELS AS WELL AS SOME REPORTED ISSUES WITH LEXUS HYBRIDS. I WONDER HOW MANY OTHER INCIDENTS HAVE OCCURRED WITH THE MILLIONS OF PRIUS VEHICLES THAT HAVE NOT BEEN REPORTED. THERE SEEMS TO BE ISSUES WITH THE COMPUTERIZED/MECHANICAL BRAKING INTEGRATION AND POSSIBLE ISSUES WITH THE ACTUATOR.

I WAS NEVER NOTIFIED OF A SPECIFIC ISSUE WITH CERTAIN 2011-2015 PRIUS VEHICLES THAT ARE INVOLVED IN A CUSTOMER SUPPORT PROGRAM ZJB. I HAVE NOW PAID $100 TO TOYOTA TO DIAGNOSE THIS PROBLEM, WHICH WAS NOT NECESSARY, AS IT IS OBVIOUSLY THE EXACT ISSUE (5 LIGHTS LUMINATED) FROM THE CUSTOMER SUPPORT PROGRAM, AND THEY HAVE TOLD ME THAT I WAS NOT NOTIFIED OF THE ISSUE BECAUSE MY VIN IS NOT INCLUDED IN THE PROGRAM. IT HAS BEEN DIAGNOSED WITH THE EXACT BREAK BOOSTER ISSUE LISTED IN THE LETTER SENT TO OTHER VIN NUMBERS. I DRIVE CHILDREN AROUND IN THIS CAR, AND THIS ISSUE IS SEVERE AND SHOULD CLEARLY BE FOR A LARGER NUMBER OF CARS THAN THEY ARE ADMITTING. I HAVE NOW BEEN TOLD THAT THEY WILL NOT PAY FOR THE $3,000 IN REPAIRS BECAUSE IT IS NOT INCLUDED. THIS IS UNACCEPTABLE. THIS IS A SAFETY HAZARD ON THE ROAD AND UNBELIEVABLE THAT I WOULD PAY EXTRA MONEY FOR A TOYOTA BECAUSE IT IS SUPPOSED TO BE A SAFE AND RELIABLE CAR, ONLY TO FIND OUT THEY WILL NOT ADMIT WHEN THEY HAVE A WIDESPREAD BREAK MANUFACTURING ISSUE WITH THEIR VEHICLES. SO THEN I GO TO TAKE THE CAR TO THE SERVICE DEPARTMENT TODAY 12/4/19 AND WHILE SITTING THERE THE IDIOT LIGHTS ALL TURN OFF AND THE BRAKE START TO FUNCTION NORMALLY OWN ITS OWN... I ASK THE SERVICE REPRESENTATIVE WHAT HIS THOUGHTS WERE AND HE SAID I SHOULD STILL MAKE THE REPAIRS, BUT HE COULDN'T SAY WHAT THEY WOULD DO IT CAME BACK. I WENT TO THE DEALERSHIP HOWEVER THE DEALERSHIP STATES THAT MY CAR, BASED ON ITS VIN, IS NOT IN ONE OF THE CERTAIN CARS COVERED BY ZJB PHASE 2 WHICH COVERS BRAKE BOOSTER AND BRAKE BOOSTER PUMP ASSEMBLIES FOR CERTAIN 2011-2015 MODEL YEAR PRIUS VEHICLES. THE FAILURE OF THE BRAKE COULD BE A DEADLY DEFICIENCY TO ALL THE PEOPLE IN THE CAR WHEN DRIVING. NOW I HAVE TO THINK TO STOP USING THIS CAR BECAUSE OF THE SEVERE SECURITY RISK. THIS IS THE SECOND TIME THAT THE CAR HAS EXPERIENCED SIMILAR PROBLEMS. IN 2014 WHILE PULLING INTO THE DRIVEWAY THE BRAKE PEDAL WENT TO THE FLOOR AND STRUCK THE HOUSE. SHORTLY AFTER THE BRAKES RETURNED TO NORMAL OPERATIONS. TOYOTA WAS NOTIFIED AND INSPECTED THE CAR'S COMPUTER AND BRAKING SYSTEM PER CASE NUMBER 1410012557. AFTER THE INSPECTION TOYOTA SAID THAT THERE WERE NO BRAKE ISSUES FOUND WITH THE CAR AND STATED THAT IT WAS SAFE TO DRIVE. OBVIOUSLY THERE IS AN ISSUE WITH THE BRAKING SYSTEM IN THESE CARS.

I WAS TOLD THE BRAKE ACTUATOR HAD FAILED AND WAS CHARGED $3288 PLUS TAX FOR THE REPAIR. I RECEIVED THE CAR ON JAN. 9, 2018 FROM THE DEALER. ON JAN. 12, 2018 THE CAR FAILED ONCE AGAIN IN THE EXACT SAME MANNER. THE DEALER KEPT THE CAR UNTIL FEB. 1, 2018 AT WHICH TIME THEY RETURNED THE CAR TO ME AT NO CHARGE, STATING THEY HAD CHANGED THE SAME EXACT PART AS BEFORE. THE CAR HAS 84,000 MILES AND I PURCHASED IT NEW. I UNDERSTAND TOYOTA HAS ISSUED RECALLS FOR THIS ISSUE ON OTHER MODELS BUT NOT ON MY VEHICLE. I WAS SCARED AND SHOCKED AT THE SAME TIME. I TURNED THE CAR OFF AND BACK ON THINKING IT MIGHT FIX THE PROBLEM. IT DID NOT. I WAS ABLE TO GET THE CAR HOME AND THE VERY NEXT MORNING TOOK IT TO SOUTH DADE TOYOTA. I WAS ABLE TO GET IT HOME AND TO THE DEALER BY DRIVING VERY SLOW KEEPING A VERY LARGE SPACE CUSHION AND STANDING ON THE BRAKE PEDAL WHEN I NEEDED TO STOP. AT THE DEALER THEY INFORMED ME THAT THE BRAKE ACTUATOR FAILED AND NEEDED REPLACEMENT COSTING $5734.94. I DID RESEARCH AND FOUND THAT TOYOTA ISSUED A WARRANTY EXTENSION ZG1 FOR THIS EXACT PROBLEM BUT WAS TOLD BY THE DEALER THAT MY VIN WAS NOT PART OF THE PROGRAM EVEN THOUGH MY VEHICLE EXPERIENCED THE EXACT PROBLEM. I REACHED OUT TO TOYOTA COOPERATE AND WHILE THEY AGREED THAT YES I HAD THE SAME PROBLEM MY VIN WAS NOT PART OF THE PROGRAM (THEY GAVE ME A CASE # 1712180647). HOWEVER THEY DID OFFER $2207.57 AS A GOOD WILL GESTURE TOWARDS THE REPAIR. I PAID $3527.37. TOYOTA KNOWS OF A LIFE SAFETY PROBLEM AND HAS CHOSEN TO LIMIT THE PROGRAM. MY MILEAGE WAS 106,451 AND UNDER THE PROGRAM WOULD HAVE BEEN COVERED.

NOVEMBER 4, 2017 - CAR WAS PARKED IN OUR DRIVEWAY. PUT THE CAR IN REVERSE TO BACK OUT OF OUR DRIVEWAY. THE BRAKES FAILED. OUR MAILBOX GOT TOTALED AND DAMAGE WAS DONE TO THE UNDERCARRIAGE OF OUR CAR, AS THERE IS TWISTED METAL HANGING DOWN SCRAPING THE GROUND. THE BRAKES STARTED TO WORK AGAIN A FEW MINUTES LATER, AS I WAS ABLE TO PULL IT BACK INTO THE DRIVEWAY OFF OF THE STREET. ON SEPTEMBER 19, 2017 ' CAR WAS PARKED IN A PARKING LOT. WHEN REVERSING OUT OF THE PARKING SPACE, THE BRAKES DID NOT WORK. AFTER REPEATEDLY PUMPING THE BRAKES, THEY FINALLY ENGAGED JUST BEFORE IT HIT A CAR PARKED BEHIND US. MY 2012 TOYOTA PRIUS V (MILEAGE: 55155) HAS HAD A FAILURE OF THE REAR BRAKE PADS AND ROTORS DUE TO EXCESSIVE RUSTING. PLEASE NOTE THE FRONT BRAKES ARE IN GREAT SHAPE AND SHOW VERY LITTLE WEAR. THE SERVICE MANAGER WAS QUICK TO POINT OUT THAT HE WAS ONLY A FRANCHISE STORE AND WAS NOT RESPONSIBLE FOR THE DESIGN OF THE REAR BRAKES. HE STATED THAT THIS HAPPENS OCCASIONALLY ON THE HYBRID CARS THEY SERVICE AND IF THE MANUFACTURER HAD COATED THE BRAKE (I BELIEVE HE WAS REFERRING TO THE SLIDE AREA OF THE CALIPER WHERE THE BRAKE PADS RIDES) THAT THE BRAKE PAD WOULD NOT HAVE HUNG UP AND WORN OUT ALONG WITH DESTROYING THE ROTORS. I AM A FORMER LICENSED AIRCRAFT MECHANIC AND A LICENSED ENGINEER WITH THE STATE OF ILLINOIS. CLEARLY THERE IS NO REASON FOR THE REAR BRAKES TO FAIL BEFORE THE FRONT BRAKES AND CERTAINLY NOT THE ROTORS. I HAVE CONTACTED THE WARREN OHIO TOYOTA GENERAL MANAGER AND SERVICE MANAGER WITH MY CONCERNS OVER THIS RAPID DECLINE IN THE BRAKING SYSTEM WITH NO WARNING IN LESS THAN 5,000 MILES OF DRIVING. I DESCRIBE BELOW A MORE RECENT INSTANCE OF THE SAME BEHAVIOR, THE TIME WHEN IT FELT THE LEAST SAFE. /// THE RECURRING BEHAVIOR IS THAT WHILE DECELERATING WITH MY FOOT ON THE BRAKE PEDAL, IF THE CAR GOES OVER AN OBSTRUCTION LIKE A ROCK, A ROUGH PATCH OF ROAD, OR A POTHOLE, IT SOMETIMES LURCHES FORWARD BEFORE RESUMING DECELERATION. THE SENSATION IS OF ACCELERATING, BUT EVEN IF IT'S ONLY THAT THE CAR MOMENTARILY STOPS DECELERATING IT'S DISCONCERTING AND IN A FEW INSTANCES HAS GOTTEN ME CLOSE TO AN ACCIDENT. /// THE TIME IT CAME CLOSEST TO CAUSING AN ACCIDENT WAS ON 3/21/13, WHEN I FEARED THE CAR WAS GOING TO ROLL INTO A CROSSWALK AS PEOPLE CROSSED. WITH MY FOOT ON THE BRAKE, COMING TO A STOP AT A STOP SIGN, WE WENT OVER UNEVEN PAVEMENT AND THE CAR LURCHED FORWARD WHILE ONLY A FEW FEET FROM THE CROSSWALK. SEVERAL PEOPLE WAITED CLOSE TO THE EDGE OF THE SIDEWALK AT A BUS STOP. ONE OF THEM SEEMED READY TO STEP OUTTO CROSS THE STREET, AND I BRACED FOR THE WORST. /// ULTIMATELY I STOPPED A FEW FEET BEFORE THE CROSSWALK, BUT THE SITUATION FELT COMPLETELY UNSAFE SINCE IT INVOLVED LOSING STOPPING POWER WHILE COMING TO A STOP SIGN WITH PEDESTRIANS PRESENT. *TR

THE NOISE CONTINUES TO GET LOUDER AS TIME PASSES.-- ITS NOW LOUD ENOUGH TO DROWN OUT THE RADIO.--TOYOTA SERVICE SAYS IT IS JUST RUST ON THE ROTORS.--THIS IS THE 4TH NEW TOYOTA I HAVE OWNED,-- BUT THIS THE ONLY ONE I HAVE HA D THIS PROBLEM WITH.-- WHICH LEADS ME TO BELIEVE THAT THERE IS A FIX FOR THIS PROBLEM.--II WAS TOLD BY THE SERVICE MANAGER THAT TOYOTA KNOWS OF THIS PROBLEM AND ARE WORKING A RECALL TO FIX IT.---SO WHERE IS THE RECALL?????? *TR

PURCHASED PRIUS V IN MARCH 2012, JUNE 2012 TOOK VEHICLE IN FOR SERVICE AND TO HAVE DEALER LOOK INTO NOISE COMING FROM REAR BRAKES. DEALER STATED THEY COULD NOT HEAR THE NOISE AND SAW NOTHING UPON INSPECTION. NOISE BECAME MORE FREQUENT AND LOUDER, TOOK VEHICLE BACK TO DEALER AND THEY COULD HEAR THE NOISE. UPON INSPECTING THE REAR BRAKES THEY FOUND REAR ROTORS HAD ABNORMAL MARKINGS IN THEM FROM IMPROPER MACHINING. BRAKE PAD FRICTION MATERIAL GLAZED. ROTORS AND PADS REPLACED. AFTER A WEEK THE NOISE RETURNED. TOOK VEHICLE BACK TO DEALER AND WAS TOLD THIS WAS A PROBLEM WHICH TOYOTA HAS BEEN EXPERIENCING FOR MONTHS BUT HAS FOUND NO SOLUTION TOO. WAS TOLD I WOULD BE INFORMED WHEN A SOLUTION WAS FOUND AND I COULD BRING THE VEHICLE BACK THEN. I FEEL THIS IS A SAFETY CONCERN WHICH NEEDS TO BE LOOKED INTO. *TR
