I was involved in an accident on the interstate traveling at about 80 mph and I hit a guard rail. I swerved, and then the lane assistance kicked in and made me lose control of the car completely, which is how I slammed into the guard rail. In that process, none of the airbags deployed and I had a passenger in the car.

1. The passenger side airbag has had a recall notification since 12/3/23. It is now 11/8/25. 2. The life and safety of my passengers are at risk. 3. The problem has been confirmed by NHTSA recall notification on 12/3/23, and personal experience. 4. The passenger side airbag warning came on the day I bought the vehicle on [XXX] at CarMax in Oxnard, CA, and was about to drive my Toyota Rav4 XLE off the lot. I was told by the CarMax salesperson that it was because my personal bag on the passenger seat was not heavy enough to indicate an actual person and, therefore, not to worry about this warning signal, this was normal. This obviously was not normal as there has now been a recall of the passenger side air bag system by Toyota/NHTSA as of December 2023. It is now November 2025 and there is no remedy available yet by Toyota per the NHTSA website. Your website stated if no timely remedy is completed, to file a complaint. On May 1, 2025, my 2021 Toyota RAV4 Hybrid displayed an SRS Airbag malfunction warning (Recall 23TA15). The passenger airbag was disabled, making the vehicle unsafe to drive. Toyota had announced this recall in December 2023, but I never received any notification. When I contacted Toyota on May 9, 2025, I was told there were no parts available and was even offered a $200 paid inspection instead of a free recall repair. My vehicle remained at the dealership from May until September 29, 2025, without any available remedy. During this period, I continued paying my auto loan and insurance for a car I could not use. Toyota also failed to disclose my right to a free rental vehicle; I only received temporary rentals (Camry, later Tacoma) after repeated demands, and they did not meet my transportation needs. While the airbag recall was still unresolved, a second safety recall (25TD01 – Rear Traction Motor Cable Corrosion) was announced in mid-September 2025. This meant my vehicle had two active safety recalls at the same time while in Toyota’s custody. Both defects were only repaired at the end of September and early October 2025. Toyota failed to notify me in time, delayed the remedy for nearly two years after the recall announcement, concealed rental rights, and allowed my vehicle to remain unsafe under multiple recalls for months. An independent arbitrator has already issued a repurchase decision, confirming Toyota’s responsibility. This demonstrates a systemic failure by Toyota to comply with recall and safety obligations.”

My 2021 Toyota RAV4 has been under airbag recall for over a year (starting around August 2023), and I have been unable to safely use my vehicle since then. Toyota has no estimated timeline for parts availability or repair, and the recall remains unresolved, they originally estimated October 2024, but that was nearly 1 year ago. Their provided timelines are now unreliable. I was provided a rental car, but it is not comparable to my RAV4 (smaller, less safe, lacks features like AWD and collision protection). Toyota has told me that if I accept the inferior rental, I will not be allowed to drive my own vehicle until the airbag is replaced — an indefinite period. I have contacted the dealer and Toyota Corporate repeatedly, but have been told there is no fix in the foreseeable future. Toyota has effectively deprived me of my own vehicle for more than 12 months, with no compensation, no buyback, and no acceptable substitute. This indefinite delay in addressing a critical safety defect (airbags) puts me and other owners at risk. I am submitting this complaint in hopes that NHTSA can hold Toyota accountable and accelerate a fix or require them to offer alternatives such as a buyback, permanent replacement, or compensation. I received notice of the recall for front passenger seat airbag sensors in Dec 2023, with assurances that it would be fixed by a certain quarter, which kept getting pushed out for almost 2 years now. I haven't received any further updates in the mail or email and had to call the dealer and Toyota directly to get any updates or info. They finally had an update that the remedy fix would happen in phases. I'm in CA and the last phase 8 which is estimated for Jan 2026. I do not have confidence that the fix will happen by then as these dates have continuously been delayed for 2 years. We've been driving our cars with a potentially unsafe situation for a long time. How will we as consumers really know when our car can be fixed?

I've spoken with Toyota and confirmed that there is a chance my passenger airbag will not deploy. They have no remedy, and the expected date of resolution has been expired for 10 months. Their only recommended action is to not have a passenger ride in my vehicle!

Driving straight on side road. Vehicle entered road by attempting a left turn onto road without yielding to me. I swerved to the left and slammed on the breaks but was t boned by their vehicle. I'm quite concerned at the length of time its taking to correct a simple OCS sensor. should be as easy as just inspect and/or replace the sensor. My pregnant wife now has to sit in a seat where the airbag may or may not deploy in an accident for the unknown future since no remedy has been made.

In early March 2024 we received the recall notice for our Toyota RAV4 LE because the OCS (air bag sensor) on passenger seat were found to possibly be faulty. I have called the dealer and Toyota's customer service hotline about once a month with no response given that a remedy is available as yet. What is the problem with Toyota taking so long to provide a remedy for this faulty sensor. We are two senior citizens who own a very nice basic RAV4 which we really enjoy especially the 30-32 gasoline mpg even in town. We have absolutely no complaints about the vehicle other than we cannot go anywhere together because no one can sit in the front seat without being a safety hazard. Yes, we could use the back seat but would you like to have to do that? This is very frustrating as a consumer to have purchased a vehicle and we cannot use it or choose not to because of the Toyota recall. Is there any way the government can put some pressure on Toyota to hurry up and get a remedy/replacement parts for the problem. Or have the dealer voluntarily check to see if the current OCS might happened to not be faulty. We would greatly appreciate any intervention or assistance you could provide. Toyota issued a preliminary recall in December 2023. I visited my dealership in January 2024, where I purchased my car and learned that there was no fix. Given I use my car to transport my twin babies, I asked for a loaner since they confirmed (at that time) that my car would have a fix later in 2024. I repeatedly checked in on the vehicle and to date, there is no fix. I am paying my car note and do not have possession of my car. I have been told my NY state that since this is a recall, it is not subject to the lemon law program. I've tried working with Toyota and they refuse to rebuy the car or fix the car. They also refuse to release the car to me. I am being forced to drive a loaner vehicle and pay a car note with NO fixes or repairs being done to the vehicle. I need assistance getting my car fixed or refunded for the purchase. It's been over a year that they've held the car and they have no plans to fix it. I'm including a letter that details the incident. I'm happy to follow up with other materials (copy of repair slip when the car was taken by Toyota, proof of ownership, etc.) This should not be allowed to continue. Toyota has had over a year to fix this and we've passed their scheduled remedy month by over 4 months. Thank you for your consideration.

I was promised by Toyota that a Remedy would be made available in February 2024, June 2024, and Sept, 2024. The Website even indicates that the remedy would be available in the 3rd Quarter of 2024. Toyota has the parts to manufacture and sell new cars, but my recall apparently is Not a priority on anyone's profit list since I had paid my money to purchase the car. I have a need to drive my car at home to drop-off my son at school and drive to doctor's appointments, Yet, I was told I should not drive the car nor let anyone ride in my car. This is not a realistic solution and if this is all Toyota has to offer when they should have fixed the major recall by now, I am demanding a full refund of the cost of purchase and they can have the car back. Again, my car is subject to OCS sensors in the front passenger seat, that was improperly manufactured and installed. The front passenger seat is unusable as the OCS sensor has turned off the airbags for that seat. Every time the car is started, or begins driving after sitting idle for a couple of minutes, it will beep and alarm constantly for several minutes, warning me of the passenger airbag. It is a serous distraction, and now prevents me from transporting a passenger. This has been going on since June 2024, and dealer refuses to look at car due to recall. I was even willing to pay myself to make the repair. No one will touch it. The remedy was originally set for the summer time, then end of 3rd quarter 2024. There is a recall on our passenger side air bag - NHTSARecall #23V-865. Toyota recall#23TA15 Bellingham Toyota verified this - and we have an indicator/problem light that is on. I am very upset becuase we were told a fix would be available in 3rd qtr 2024 and now it is 4th qtr 2024. When I called Toyota headquarters, they have no answer as to when there will be a fix. They offered a loaner - however they prohibit the use of that loaner in Canada. Additionally, we would have to pay the cost of a rental up front and then submit receipts later for reimb but they CAN't guarantee reimb. We are very low income and so we cannot front that money, NOR take a chance that it would not be reimbursed. We live right on the US/Canadian border and we go to Canada a lot to visit family - two of which are currently in the hospital/his mom and my sister. My husband is a quadriplegic and we only have this one vehicle to go back and forth with our 2 daughters. There is nowhere near enough room with his wheelchair/walker for when we all want to travel. Additionally, when it's just him and me, he can't hear me speaking to him when he has to sit in the back seat all the time. At this point, we would like Toyota to take our current vehicle back in a trade and provide us with a new car that has none of these problems.

On Saturday [XXX], I was driving my car when the seatbelt light started beeping as well as a warning on the dash saying there was an airbag malfunction. I immediately took my car into the Toyota dealership in Roseville, California and Monday I was notified that this was part of the recall for defective airbags, I was also told that there was no fix currently and there wouldn’t be one until March 2026. they said the delay was due to the fact that the problem with high humidity so they were taking care of cars in high humidity areas first and California would be later. Until then they said they would keep my car and give me a loaner which turned out to be a regular RAV rather than the hybrid which I own. I told them that was unacceptable that I bought the car for the hybrid feature and they said they didn’t have any hybrids available. I certainly hope I get a hybrid to drive for a year and 7 months! I was at a complete stop and was rear ended. The person who hit me was going over 45mph, and the impact caused me to hit the car in front of me, which crushed my car at both ends. My (driver) airbag did not deploy, and my head struck the steering wheel. It is available for inspection, and has not been confirmed by a dealer - it is at the insurance company’s tow lot. The police officer at the scene was surprised that my airbag did not deploy. There were no warning lights, messages, or any other indication that anything was malfunctioning/in need of service.

There was a stalled vehicle in a traffic lane. The vehicle ahead of me was a large that hit the stalled vehicle. I couldn’t see what he hit but he came to a dead stop and I couldn’t avoid him. My problem is that the airbag under the dashboard on the driver side hit my shin with such force that it caused an abrasion that didn’t seem bad at first but developed into a wound requiring a large hematoma and surgery to remove it and to have skin graft.

I found out my RAV4 had a recall on passenger side airbag that could not be fixed, and it would be in the third quarter of this year before it could be fixed if then. I asked if I could get a good deal on a new RAV4, for a discount since I had just quit my job and sold my house and planned to leave this area. My grandson lives with me and travels with me so now I don’t trust my car. I only found out about the recall when I took my car in for service. I called Toyota and spoke to Tina Stubbs, who said she would see what she could do on getting me a discount on a new RAV4 like mine. My car is in perfect condition except for this, so I never considered trading it in until this happened. Tina was supposed to contact me by this evening, which has not happened. I am staying in a motel until I can get something settled. I have owned Toyotas since 1996, and never had a complaint until now. I don’t trust driving my car with a passenger now. Jan 3, 2024 Crash

The contact's wife owns a 2021 Toyota Rav4. The contact stated that while driving at 30 MPH in an intersection, a Ford F-150 was crashed into head-on. The vehicle's front end collided with the Ford vehicle but the air bags did not deploy. The Ford vehicle did a 180 during the impact and its air bags deployed. The contact sustained 2 herniated discs in the 212 and L1 that required medical attention. A police report was filed. The vehicle was towed to a collision center where it was diagnosed that the front air bag sensor needed to be replaced. The vehicle was repaired. The manufacturer was notified of the failure but a callback was not completed The failure mileage was approximately 12,000.

Nov 29, 2023 Crash

On Nov 05, at approximately 11:10pm, I was driving my RAV4 on Alafaya Trail, Orange County, Orlando. While approaching the traffic signal which had turned red, despite multiple attempts to engage the brakes, the vehicle continued moving, ultimately leading to the collision with the traffic turning left at the signal (coming from her right). The impact of the collision resulted in significant damage to the RAV4, and unfortunately, the airbags did not deploy as they should have. As a result, I hit my face on the steering wheel, and then hit the head on the left window when a second vehicle hit my RAV4 on the passenger side. The complete failure of airbag deployment has raised serious safety concerns for me. As of 28th November, I have been informed by our insurance company that the vehicle cannot be repaired and has been totaled. Our insurance company GEICO has been requested to hold on to the vehicle for necessary investigations.

I was traveling on a nearby road at approximately 40mph when another vehicle ran a stop sign causing me to t-bone his vehicle and spin around. There was no time for evasive measures so the vehicles collided at full speed. I hit my head on the left side as a result of the collision. The point of impact on my car was the entire front end. Neither my front nor side airbags deployed during the collision. They should have.

I was in a bad car accident last Wednesday July 26, 2023 It was very scary and Bizarre; My car accelerated (started racing)and when I stepped on brake, it accelerated more. I saw a message flash up on my GPS screen but obviously couldn't read it as I was trying to stop my car. The Auto brakes didn’t work, I crashed into a tree; air bags didn’t deploy and I got this message ("CHECK ENGINE! A MALFUNCTION IN THE ELECTRONIC CONTROL OF THE ENGINE,THROTTLE, OR AUTOMATIC TRANSMISSION HAS BEEN DETECTED.CONTACT YOUR TOYOTA DEALER IN ORDER TO HAVE YOUR VEHICLE INSPECTED." (See attached). Luckily I was in an almost empty parking lot when this happened, if not i could have been killed or killed someone else. All the safety back ups failed. I took a picture of the message-will attach it to this report. I was taken to Huntington Hospital ER and had a cat scan to make Sure my sternum and ribs aren’t broken-showed Soft tissue abdominal swelling and bruised sternum and ribs from my seat belt. Pain to chest, ribs an right side. I believe my car is totaled Just had it inspected and serviced a few weeks ago at the Toyota dealership where I purchased it and it only has 10,000 miles on it! I am Fighting with insurance company to make sure the car's computer is interrogated before they do anything else.-The insurance company was pressuring me to release the car to Copart without this being done and I refused. My car was towed to and remains at Combined Auto Collision 4035 Jericho Tpk, East Northport, NY. This incident could have resulted in a catastrophic outcome and loss of life. Aa investigation needs to be taken to insure this does not happen to anyone else. Thank you for your kind assistance The consumer stated that the dealer identified the problem being that the water drainage pillars short out the electrical system.

The contact owned a 2021 Toyota Rav 4. The contact stated that while sitting in traffic, the contact's vehicle was rear-ended at 45 MPH, and due to the impact, the contact rear-ended the vehicle in front. The "malfunction" warning message was displayed after the crash. The driver's side air bag did not deploy. The vehicle was declared a total loss by the insurance company. The contact stated that she suffered a concussion and memory loss due to the accident that required medical attention. The manufacturer was made aware of the failure but provided no assistance and did not confirm the status of the black box inspection. The failure mileage was approximately 10,100.

May 23, 2023 Crash

On May 20th, 2023 @ approximately 9:50 P.M. we struck a deer head on. Impact was severe enough that the passenger airbag should have deployed, but failed to deploy. I looked at the wiring guide for this vehicle after the incident, and I believe that in photo #4, it shows the path of the airbag wire and the airbag wire was cut at the time of impact.

Apr 5, 2023 Crash Fire

While accelerating ~6ft from a complete stop, a minor collision occurred affecting the front right headlight. The air-bags did not activate and the engine compartment was immediately on fire. Within ~1minute, the fire had spread to and completely engulfed the passenger compartment. The contents were immediately destroyed. The speed with which the fire spread through the vehicle was a significant risk to my life and the motorists around me. There were no warning lights on the dashboard prior to the incident, and the car had never before experienced an accident. Witnesses and firefighters estimated that any fire barrier between the engine and passenger compartments had failed, and that a fire of that severity from such a minor accident indicated some fault with the vehicle. The vehicle was an immediate total loss as inspected by our insurer.

No safety devices worked during accident that cause by another vehicle driver who ran red light. Neither air bags deployed, nor safety belt worked properly during an accident. Due to these mal functions of safety devices, my wife on the passenger seat hit the dashboard badly. She was wearing the seat belt properly which was released later by the ER personal at the accident location to transfer to an ambulance.
