The contact owns a 2006 Toyota Sienna equipped Hercules Tires, Tire Line: Roadtour 855 SPE, Tire Size: 215/65/R16, DOT Number: U96VH260H21. The contact stated while driving 35 MPH during a trip, the steering wheel started shuddering. The vehicle was taken to a tire shop, and the contact was informed that the two front tires had sustained tread belt separation and bulges. The two front tires were replaced. Additionally, the contact stated while driving 65-70 MPH, the TPMS warning light illuminated. The vehicle was taken to the tire shop, and the contact was informed that the rear passenger's side tire had sustained tread belt separation, and the metallic belt was poking through the rubber. The contact requested that the rear tires be replaced as a precautionary measure. The rear tires were replaced. All four tires were replacement tires. The dealer and the manufacturer were not notified of the failure. The vehicle failure mileage was 173,414. The tire failure mileage was approximately 16,022. The VIN was not available.

I FILED A COMPLAINT 10/18/2018; HOWEVER, THE COMPLAINT WAS FOR 2 OF THE 4 TIRES. TODAY'S COMPLAINT IS FOR THE REMAINING 2 TIRES WHICH HAD MANUFACTURER DEFECT PER THE SHOP. PLEASE NOTE THESE ARE COOPERTIRE,CS5 NOT CS4.T THERE'S NOT AN OPTION FOR "CS5 "T SO LIKE OTHERS I'M USING THE CLOSET WHICH IS "CS4 T". MY VAN STARTING SHAKING (AT THIS POINT I HAVE TWO NEW TIRES ON 4/18 (COOPER CS5 -T) AND THE OTHER TWO TIRES ARE ALSO COOPER CS5 -T, BUT PURCHASED 1/2016, 80K MILE "WEAR OUT WARRANTY" I'VE PUT 7K ON THE TWO "NEW" TIRES & THE OTHER TWO HAVE 48K (22K LEFT). THE STEERING WHEEL SHAKES, PEOPLE SHAKE, I STOP DRIVING IT ANYWHERE THAT WILL REQUIRE ME GOING OVER 35MPH.SHAKING ISN'T EVEN THE WORD FOR IT WHEN OVER 35 MPH. I CAN'T GET ANYWHERE ON MY 35MPH FOR 1.5 WEEKS.. NOT FUN WITH 4 KIDS TO RUN TO SPORTS SCHOOL & EVENTS. I GET THE VAN IN FINALLY ON THURSDAY, 10/18/2018. I GET A CALL FROM THE SHOP TO TELL ME THERE IS A "MANUFACTURER DEFECT" ON THE TWO OLDER COOPER TIRES. THAT DEFECT WHICH IS TYPED ON MY RCPT IS "FOUND THAT THE TIRE HAD A PLY SEPARATION CAUSING THE VIBRATION." SO I LOOKED IT UP, NOT KNOWING EXACTLY THE JARGON USED "TIRE SEPARATION IS ONE OF THE MOST DANGEROUS THINGS THAT CAN HAPPEN TO A TIRE, AND CAN PUT THE DRIVER OF THE VEHICLE AND EVERYONE ELSE AROUND AT RISK. A SEPARATION CAN LEAD TO A TIRE BLOWOUT, WHICH IN TURN CAN LEAD TO ONE OF THESE EVEN MORE SERIOUS ACCIDENTS. HEAD-ON COLLISIONS. ROLLOVER ACCIDENTS. ROAD RUN-OFFS. SIDE IMPACT INCIDENTS. AND, MORE. OFTEN TIMES, THE INJURIES SUSTAINED FROM THESE ACCIDENTS ARE SEVERE, IF NOT DEADLY." I'M A METICULOUS RECORD KEEPER AND DO REGULAR MAINTENANCE ON MY VEHICLES. THE TIRES WENT ON 1/2016. ROTATED 1/11/2017, 5/17/2017, 3/2018, 4/2018,10/2018. OUT COME SAME AS LAST. 2 NEW TIRES I PAY FOR, NO OPTIONS, SO SAME BRAND ON AGAIN. IN 2016 I SPENT $675 ON 4 NEW TIRES, THEY SHOULD HAVE 22K LEFT OF WEAR. INSTEAD IN TOTAL I'VE PAID $106 TO HAVE THE BRAND TIRE ON IT.TOYOTA SAID ALL 4 TIRES WERE A PROBLEM 2/2018, DENIED

PURCHASED 4 NEW CS5 GRAND TOURING TIRES THAT HAVE 80,000 MILE "WEAR OUT" WARRANTY ON JANUARY 26, 2016. MILES = 174295. IN APRIL 2018, 1 YEAR 4 MONTHS LATER, MILES NOW AT 215089 (40K PUT ON, SO SHOULD HAVE ANOTHER 40K MORE) MY VEHICLE WOULD SHAKE, NOT HOLD AIR, I EVEN HAD PROBLEMS WITH AIR GOING INTO THROUGH THE VALVE, MY VEHICLE FAILED AN ALIGNMENT CHECK BY AN AUTHORIZED TOYOTA DEALERSHIP. TIRE TREAD DEPTH FRONT DRIVER: 4 6 5 5 TIRE TREAD DEPTH FRONT PASS: 5 6 6 5 TIRE TREAD DEPTH REAR DRIVER 6 8 8 7 TIRED TREAD DEPTH READ PASS: 6 7 7 6 OBVIOUSLY NOT GOING TO GET ANOTHER 40K MILES OUT OF THESE TIRES. IN ADDITION THE WEAR ON THEM IS INCONSISTENT ON EACH TIRE. THE TIRE ISN'T WEARING RIGHT. ALIGNMENT RIGHT REAR CAMBER: -1.9 -.9. I HAD AN ALIGNMENT ACTUALLY TWO, BUT ONE CLOSE TO THIS APPOINTMENT. TAKE MY VEHICLE INTO WEAR I PURCHASED THEM, THE FINE PRINT COMES INTO PLAY. SURE THE CRAPPY TIRES, WEARING OUT POURLY, NOT HOLDING AIR, WON'T MAKE IT 80K, BUT I PAY HALF FOR TWO NEW TIRES PLUS LABOR, THE OTHER TWO TIRES DO NOT QUALIFY I'M JUST SOL. REPLACEMENT TIRES TO CHOSE FROM, THE EXACT SAME TIRE. SO I GET TO PAY FOR TWO NEW TIRES OF THE EXACT SAME BRAND AND WARRANTY. I BOUGHT A SET OF 4 DOUGLAS ALL-SEASON 215/65R16 98T SL TIRES AT WALMART. AFTER LESS THAN 4 MONTHS/600 MILES OF USE, 3 OF THE TIRES WERE DIAGNOSED AS SEPARATING INSIDE. THIS IS A VERY DANGEROUS PROBLEM THAT COULD HAVE COST US OUR LIVES. THESE TIRES MUST BE RECALLED. THIS IS UNACCEPTABLE. IF YOUR VEHICLE SHAKES AT A MODERATE SPEED (BETWEEN 25-40MPH) AND YOU HAVE THESE TIRES THEN YOU SHOULD GET THEM REPLACED IMMEDIATELY. IT IS NOT THE BALANCE (I HAD THEM BALANCED TWICE) OR THE ALIGNMENT, IT IS A DEFECT IN THESE PARTICULAR TIRES.

I WANT TO KNOW WHY TOYOTA IS DRAGGING THEIR FEET GETTING THE SPARE TIRE BRACKET AND CABLE CORRECTED. TOYOTA SIENNA RECALL FOR SPARE TIRE CARRIER MOVED THE SPARE TIRE TO THE LUGGAGE COMPARTMENT OF VEHICLE. THE TIRE WAS REMOVED FROM THE UNDERSIDE OF THE VEHICLE, PLACED IN A BAG, AND PLACED IN THE LUGGAGE COMPARTMENT. THE TIRE WAS NOT SECURED BY SERVICE AND IS FREE TO VIBRATE, SHAKE, AND ROLL WHEN TURNING. THIS WAS TO BE A TEMPORARY FIX, WITH THE PERMANENT SOLUTION TO BE DETERMINED BY TOYOTA. IT HAS BEEN OVER 6 MONTHS FROM THE RELEASE DATE OF THE RECALL AND THERE IS STILL NO PERMANENT SOLUTION, AND THERE IS NO ETA FOR A FIX AT THIS TIME, PER THE TOYOTA CUSTOMER SERVICE CENTER. THIS ISSUE NEEDS TO BE RESOLVED AS IT IS LIMITING THE AVAILABLE LUGGAGE COMPARTMENT SPACE, AND THE MOVEMENT OF THE UNSECURED SPARE TIRE IS DISTRACTING. WHILE THE VEHICLE WAS IN MOTION, THE SPARE TIRE DROPPED SUDDENLY. TOYOTA KNEW ABOUT THIS CORROSION PROBLEM BACK IN APRIL 20, 2010 WITH NHTSA CAMPAIGN NUMBER 10V160000. IN MAY 22, 2014, ANOTHER INTERIM CAMPAIGN NUMBER 14V273000 WAS ISSUED. UNTIL TODAY, NOTHING HAVE BEEN FOLLOWED UP TO SOLVE THE ISSUE. TOYOTA'S CUSTOMER SERVICE JUST TOLD ME THAT "UNTIL THEY FOUND A PERMANENT SOLUTION FOR THIS ISSUE, JUST PUT THE SPARE TIRE IN THE TRUNK". THIS TYPE OF SHIRKING RESPONSIBILITIES AND ALOOF ATTITUDE. AT FIRST, I HAVE THE TIRE PRESSURE INDICATOR LIGHT CONTINUING TO LIT UP. THEN ONE MORNING, THE VAN WAS SLOW TO RESPONSE TO SHIFTING GEAR AND ACCELERATION. AFTER STOPPING, IT WON'T START AND MOVE INTO GEAR AT ALL. THERE WAS NO ENGINE LIGHT BEING LIT OR WARNING SIGN. TOWED TO SHORE TOYOTA AT MAYS LANDING, NJ. THEY TOLD ME THAT THE TRANSMISSION OIL HAD ALL LEAKED OUT DUE TO THE BURST OF ABRUPT FAILURE ON THE INLET TO THE RADIATOR. THEY ASKED ME TO REPLACE THE RADIATOR AT A COST OF $1,150.26! AFTER REPAIRED, THEY THEN TOLD ME THAT THE TRANSMISSION IS BAD AND DIFFICULT TO MOVE INTO GEAR. THEY QUOTED ANOTHER $3,780 TO REPLACE THE TRANSMISSION. I WAS FURIOUS. SINCE THAT THE DEALER TOLD ME THAT THE VAN CAN BE DRIVEN. I WAS PREPARED TO TRADE IT IN AND WEIGHING MY OPTIONS. THE VAN HAS 133,279 MILES. I DROVE IT OK INITIALLY, HOWEVER, I EXPERIENCED THAT IT WAS DIFFICULT TO ACCELERATE TWICE. AFTER 15 MILES WHEN I STOPPED AT A TRAFFIC LIGHT, THE VAN JUST STUCK- IT CANNOT MOVE IN ANY OF THE GEARS AT ALL. I TOWED BACK HOME. AFTER COOLING DOWN, IT WAS ABLE TO DRIVE AGAIN. THE NEW TRANSMISSION FLUID SMELLED BURNT. I AM WONDERING WHY: ?WHY THE ENGINE LIGHT WAS NOT LIT UP AT ALL WHEN THE COOLANT TEMPERATURE WAS SO HIGH BEFORE SPOILING THE WHOLE TRANSMISSION? ?WHY WAS THAT NOT EVEN ANY DTC CODES STORED ON THE OBDII SYSTEMS AT ALL AFTER ALL THESE DEFECTS? ?THIS SEEMED THE SAME PROBLEM AS THE STICKY TRANSMISSION THAT OTHER CUSTOMERS HAVE BEEN COMPLAINING ABOUT, YET TOYOTA HAS NOT INFORMING THE CUSTOMER OF ANY ACTIONS NEED TO BE TAKE. THIS TIRE WAS ON THE RIGHT REAR POSITION OF MY 2006 TOYOTA SIENNA THIS DEFECTIVE TIRE CAUSED MY SIENNA TO WOBBLE VIOLENTLY WITH SPEEDS ABOVE 35MPH. THIS VIOLENT WOBBLE ALSO CAUSED ABNORMAL TREAD WEAR ON THE INSIDE SURFACE OF ALL FOUR TIRES ON THE VEHICLE. PROBLEM: RUN ON LOW TIRE PRESSURE (22 PSI INSTEAD OF INDICATED 35 PSI) FOR ONE YEAR FROM THE TIME I PURCHASED THIS VEHICLE FROM THE DEALER TILL THAT I FOUND OUT THE PROBLEM. REASON: THE DEALER (UNIVERSAL TOYOTA SAN ANTONIO) ADMITTED THAT THEY NEEDED TO LOWER THE TIRE PRESSURE OF THE VEHICLE DURING TRANSPORTATION FROM FACTORY TO DEALERSHIP BUT SOMEONE FORGOT TO SET IT BACK BEFORE IT WAS HANDED OUT TO CUSTOMER. THE MAINTENANCE DEALERSHIP (CAVENDER SAN ANTONIO) ALSO DIDN'T DO THEIR JOB TO CHECK THE TIRE PRESSURE UNTIL ONE YEAR LATER I ASKED THEM TO DO SO. TOYOTA SOLUTION FOR THIS PROBLEM: THE MANAGER OF CAVENDER SAN ANTONIO PROMISED TO GIVE ME A FREE TANK OF GAS AND A FREE MAINTENANCE NEXT TIME. I ACCEPTED. HOWEVER, I ASKED THE DEALERSHIP TO REPLACE THE FOUR TIRES BECAUSE THEY WERE DAMAGED, BUT THEY DIDN'T AGREE, ARGUING THAT EVEN UNDER LOW PRESSURE, THESE TIRES SHOULD BE FINE. CONSEQUENCES: THREE OF FOUR TIRES ON THIS VEHICLE WERE DEFLATED ON HIGHWAY IN LESS THAN ONE YEAR (3/22/2008, 9/6/2008, 3/20/2009). RUN FLAT TIRES NEEDING REPLACEMENT ONLY AFTER 2 YEARS OF OWNING THE CAR. 2 BLOWOUTS ON THE FREEWAY DUE TO NO DIRECT TIRE PRESSURE SENSOR TO ALERT US OF THE FLAT TIRE. THE LIGHT ONLY GOES ON AFTER THE TIRE IS BLOWN. THE FIRST BLOW OUT, I DROVE IT DIRECTLY TO THE DEALER. TOTAL COST OF REPLACEMENT $350 FOR TIRE. I THEN HAD PROBLEMS WITH THE "INDIRECT" TIRE SENSOR LIGHT COMING ON. I CALLED THE DEALER BACK, HE TOLD ME TO IGNORE THE LIGHT BECAUSE THE COLD OUTSIDE WAS CAUSING IT TO GO ON. I DROVE IT BACK ANYWAY, THEY CHECKED THE TIRE PRESSURE, PUT SOME MORE AIR IN AND SENT ME HOME WITH MY SENSOR LIGHT STILL LIT. I TOOK IT BACK TO THE DEALER A FEW WEEKS LATER, THEY SAID IT WAS A LEAKY VALVE. THEY REPLACED THE VALVE, FILLED THE TIRE WITH AIR AND SENT ME HOME. LESS THAN 5 DAYS LATER, ANOTHER BLOWOUT ON THE FREEWAY. I DROVE IT TO THE NEAREST TIRE STORE. THEY COULD NOT DO THE REPAIR BECAUSE THE TIRE WOULD RUIN THEIR MACHINE. "IT'S HARDER TO BREAK DOWN" AND TAKE OFF. I WAS SENT TO ANOTHER TIRE STORE. THEY WERE ABLE TO REMOVE THE TIRE AND REPLACE THEM WITH REGULAR TIRES. NOW, I DON'T HAVE RUN FLATS ANYMORE, BUT I DON'T HAVE THE EXCESSIVE COST TO KEEP REPLACING THESE EXPENSIVE TIRES. THE DEALER TOLD ME AT THE TIME OF THE PURCHASE THAT IT WAS A "DREAM" FOR MOM'S TRANSPORTING KIDS BECAUSE I WOULDN'T HAVE TO CHANGE TO A SPARE IF THE TIRE WENT FLAT. THAT THE CARE WAS SAFER THAN SAFE. THE MANAGER OF THE SERVICE DEPARTMENT ALSO TOLD ME I WAS LUCKY THAT I GOT 40,000 MILES OUT OF THEM. BOTH TIRES BLEW OUT ON THE FREEWAY, I ROLLED THE CAR IN AT 25 MILES PER HOUR, ON THE RIM, WITH MY KIDS IN THE CAR. I DID IT SAFELY WITH MY DRIVING, NOT BECAUSE OF THE RUN-FLAT TIRES. I WILL NEVER PURCHASE ANOTHER CAR WITH RUN-FLAT TIRES. THEY DIDN'T TELL ME HOW EXPENSIVE THEY WERE TO REPLACE AND THAT I WOULD NEED TO REPLACE THEM SO FREQUENTLY. EVER SINCE I BOUGHT THIS SIENNA 2006, I HAVE NOTICED BOTH OF MY BACK DUNLOP TIRES ARE LEAKING AIR FREQUENTLY STARTING ON FEB 2008. THE MILEAGE AS OF TODAY IS ONLY 5400. THE RIGHT SIDE OF THE BACK TIRE POPPED AND I HAVE TO REPLACED WITH THE MICHELIN. AS FOR THE LEFT SIDE, EVERY WEEK, IT DROPPED TO 20PSI. I HAVE TO PUMP IT BACK UP TO 35-40PSI EVERY WEEK. TOOK IT TO THE DEALER, NO HOLES OR ANY PROBLEM FOUND ON THE DUNLOP. THEY REFUSED TO REPLACE THE TIRE. I ASK THEM WHY IT DIDN'T HAPPEN TO THE FRONT TIRE THEN, WHY ONLY THE BACK TIRES? THIS REPORT IS FOR MY MOTHER IN LAW'S 2006 TOYOTA SIENNA XE AWD MINIVAN WITH THE DUNLOP RUNFLAT TIRES. THIS MINIVAN HAS HAD 2 "DEFECTIVE" TIRES ON IT SINCE IT'S PURCHASE AND WITH LESS THAN 8000 MILES ON THE TIRES. BOTH TIRES WERE REPLACED BY THE DEALER, BUT WE CONTINUE TO HAVE TIRE/PRESSURE PROBLEMS. I BELIEVE BOTH TIRES WERE RUINED DUE TO THE PRESSURE SENSING SYSTEM NOT DETECTING LOW PRESSURE IN THE TIRES AND CAUSING THE SIDE WALLS TO COLLAPSE AFTER CONTINUED DRIVING. LUCKILY, NOBODY HAS BEEN INJURED, HOWEVER IF DRIVING AT HIGH SPEEDS WITH THIS SYSTEM A POTENTIAL EXISTS FOR A SEVERE ACCIDENT. I HAVE SPOKEN WITH THE NATIONAL TOYOTA AND LOCAL DEALER, AND THEY CONTINUE TO NOT ADDRESS THE PROBLEM. THE DEALER HAS STATED IN WRITING THAT THE PRESSURE SYSTEM IS FUNCTIONING PROPERLY, HOWEVER RUNFLAT TIRES ON THIS SYSTEM DO NOT REGISTER LOW PRESSURE UNTIL MULTIPLE TIRES OR A SINGLE TIRE HAS HARDLY ANY PRESSURE IN IT. THE LOCAL DEALER MANAGER AGREES WITH ME THAT THE RUNFLAT TIRES WITH THE ABS TYPE PRESSURE SENSING SYSTEM IS NOT A SAFE SYSTEM. I HAVE REQUESTED TO HAVE REGULAR TIRES INSTALLED ON THIS VAN SO THE PRESSURE SYSTEM CAN DETECT LOW PRESSURE, HOWEVER TOYOTA WILL NOT CHANGE THEM FOR ME. I FEEL THAT HAVING RUNFLATS AS INSTALLED BY THE MANUFACTURER ON THIS TYPE OF PRESSURE SYSTEM CAN EVENTUALLY LEAD TO AN ACCIDENT WHEN A LOW TIRE BLOWS OFF THE RIM AT A HIGH SPEED, CAUSING THE DRIVER TO LOSE CONTROL. WITH THE RUNFLAT TIRES, A DRIVER CANNOT VISUALLY LOOK AT THE TIRE AND DETECT THAT IT IS LOW, DUE TO THE THICK SIDEWALLS THAT ALLOW THEM TO "RUN FLAT". THIS IS A SAFETY ISSUE THAT I BELIEVE TOYOTA SHOULD ADDRESS AND CORRECT BEFORE SOMEBODY IS KILLED. I LIVE IN ALASKA AND MY MOTHER IN LAW FREQUENTLY MAKES TRIPS TO ANCHORAGE, WHERE FUEL/SERVICE STATIONS ARE HUNDREDS OF MILES APART AND -50F WEATHER IS COMMON. I HAVE A 2006 TOYOTA SIENNA XLE AWD MINIVAN WHICH WAS SEEN 10/17/07 FOR TIRE REPLACEMENT (DUNLOP 4000 DSST RUN FLATS). I HAVE OWNED THIS CARE FOR 1 YEAR AND 2 WEEKS AND HAVE ONLY 14,900 MILES ON IT. RECOMMENDED SERVICE HAS BEEN DONE AT THE DEALERSHIP TWICE SINCE I'VE OWNED THE CAR, INCLUDING TIRE ROTATION...ONCE IN FEBRUARY AND ONCE AT THE END OF JULY. THE CAR WAS DRIVEN ONLY UNDER NORMAL ROAD CONDITIONS AND AIR PRESSURES CHECKED EVERY 2 WEEKS. I AM TWO WEEKS PAST 1 YR. WARRANTY AND EVEN THOUGH PREEXISTING WEAR WAS "MISSED" BY SERVICE DEALER IN LATE JULY, I AM BEING CHARGED FOR REPLACEMENT. TOYOTA AWARE OR PREVIOUS PROBLEMS WITH THESE TIRES, INCLUDING CLASS ACTION LAW SUIT, YET CONTINUED TO PUT THEM ON CARS. MY TREAD WEAR WAS DETERMINED TO BE UNEVEN WITH 0 DEPTH ON THE INSIDE TREAD OF LEFT FRONT, POSING A SAFETY HAZARD AND CREATING AN UNSAFE DRIVING CONDITION. REPORT ALSO INDICATES WEAR ON EDGES AND CUPPING EVIDENT ON ALL OTHER TIRES (ACCORDING TO THE DEALER, MY VIN NUMBER DID NOT MATCH UP WITH THE "SERVICE BULLETIN" AND I WAS CHARGED FOR THE INSTALLATION OF THESE TIRES AND A WHEEL ALIGNMENT) AN ATTEMPT TO CONTACT TOYOTA CUSTOMER CARE WHILE THE CAR WAS IN THE SHOP WAS UNRESOLVED AS CUSTOMER CARE NEVER RETURNED MY CALL. 2006 TOYOTA SIENNA W/RUN FLAT TIRES SHOWED SIGNIFICANT PREMATURE TIRE WEAR. ALL SERVICE MAINTAINED WITH TOYOTA DEALERSHIP AND UP TO DATE. DETERMINED THAT AT 19.400 MILES THE WEAR SHOWED TIRE BELT CREATING A SAFETY HAZARD UNKNOWN TO THE DRIVER/OWNER UNTIL NORMAL SERVICE VISIT. THE VIN WAS OUTSIDE THE EFFECTED VIN LIST ASSOCIATED WITH SUPPLEMENTAL TIRE WARRANTY COVERAGE SPONSORED BY TOYOTA RELATED TO IDENTICAL PROBLEMS WITH PREVIOUS AWD SIENNAS. TOYOTA REFUSED ASSISTANCE IN REPLACING TIRES. DUNLOP PROVIDED PRORATE OF 48% FOR REPLACEMENT. TIRES WERE VIEWED AS UNSAFE BASED ON PREMATURE WEAR THAT WOULD NOT BE NOTICEABLE TO OWNER/DRIVER WITHOUT UNDERSTANDING THE PROBLEMS WITH RUN FLATS. TOYOTA REFUSED ANY SUPPORT SINCE VIN WAS 5000 OUTSIDE THE EFFECTIVITY OF THE SUPPLEMENTAL WARRANTY. FAILURE OF TIRE DUE TO PREMATURE WEAR COULD RESULT IN LOSS OF CONTROL OF THE VEHICLE, AND ULTIMTAELY LOSS OF LIFE.
